Purpose Roentgen stereophotogrammetric analysis is a technique to make accurate assessments of the relative position and orientation of bone structures and implants in vivo. While the precision and accuracy of stereophotogrammetry for hip and knee arthroplasty is well documented, there is insufficient knowledge of the technique’s precision and, especially accuracy when applied to rotational movements in the spinal region. Methods The motion of one cadaver lumbar spine segment (L3/L4) was analyzed in flexion–extension, lateral bending and internal rotation. The specific aim of this study was to examine the precision and accuracy of stereophotogrammetry in a controlled in vitro setting, taking the surrounding soft tissue into account. The second objective of this study was to investigate the effect of different focal spot values of X-ray tubes. Results Overall, the precision of flexion–extension measurements was found to be better when using a 0.6 mm focal spot value rather than 1.2 mm (± 0.056° and ± 0.153°; respectively), and accuracy was also slightly better for the 0.6 mm focal spot value compared to 1.2 mm (− 0.137° and − 0.170°; respectively). The best values for precision and accuracy were obtained in lateral bending for both 0.6 mm and 1.2 mm focal spot values (precision: ± 0.019° and ± 0.015°, respectively; accuracy: − 0.041° and − 0.035°). Conclusion In summary, the results suggest stereophotogrammetry to be a highly precise method to analyze motion of the lumbar spine. Since precision and accuracy are better than 0.2° for both focal spot values, the choice between these is of minor clinical relevance.

Older children with online schooling requirements, unsurprisingly, were reported to have increased screen time during the first COVID-19 lockdown in many countries. Here, we ask whether younger children with no similar online schooling requirements also had increased screen time during lockdown. We examined children’s screen time during the first COVID-19 lockdown in a large cohort (n = 2209) of 8-to-36-month-olds sampled from 15 labs across 12 countries. Caregivers reported that toddlers with no online schooling requirements were exposed to more screen time during lockdown than before lockdown. While this was exacerbated for countries with longer lockdowns, there was no evidence that the increase in screen time during lockdown was associated with socio-demographic variables, such as child age and socio-economic status (SES). However, screen time during lockdown was negatively associated with SES and positively associated with child age, caregiver screen time, and attitudes towards children’s screen time. The results highlight the impact of the COVID-19 lockdown on young children’s screen time.

The dry lowlands of Ethiopia are seasonally affected by long periods of low rainfall and, coinciding with rainfall in the Amhara highlands, flood waters which flow onto the lowlands resulting in damage to landscapes and settlements. In an attempt to convert water from storm generated floods into productive use, this study proposes a methodology using remote sensing data and geographical information system tools to identify potential sites where flood spreading weirs may be installed and farming systems developed which produce food and fodder for poor rural communities. First, land use land cover maps for the study area were developed using Landsat-8 and MODIS temporal data. Sentinel-1 data at 10 and 20 m resolution on a 12-day basis were then used to determine flood prone areas. Slope and drainage maps were derived from Shuttle RADAR Topography Mission Digital Elevation Model at 90 m spatial resolution. Accuracy assessment using ground survey data showed that overall accuracies (correctness) of the land use/land cover classes were 86% with kappa 0.82. Coinciding with rainfall in the uplands, March and April are the months with flood events in the short growing season (belg) and June, July and August have flood events during the major (meher) season. In the Afar region, there is potentially >0.55 m ha land available for development using seasonal flood waters from belg or meher seasons. During the 4 years of monitoring (2015–2018), a minimum of 142,000 and 172,000 ha of land were flooded in the belg and meher seasons, respectively. The pastoral systems of Eastern Africa have been affected by the alternated incidence of recurrent drought and flood for the last decades, aggravating poverty and local conflicts. We have introduced an innovation to convert floods to productive use using water spreading weirs (WSW) as an entry point to capture and spread the torrential flood emerging in the neighboring highlands into rangelands and crop fields of low-lying pastoral systems in Afar, Ethiopia. The productivity and landscape feature have changed from an abandoned field to a productive landscape within 3 years of intervention. The flood patterns and sediment loads created at least four different crop management zones and productivity levels. Based on moisture and nutrient regimes, we developed land suitability maps for integrating crops and forages fitting to specific niches. The outcome was a fast recovery of landscapes, with 150% biomass yield increment, increased access to dry season feed and food. These positive outcomes could be attributed to the proper design of weirs, joint planning and execution between pastoralists, researchers and development agents, identification and availing best-fitting varieties for each management zone and developing simple GIS-based parcel level maps to guide development agents and pastoralists. The major ‘agents’ were community leaders (‘Kedoh Abbobati’) who keenly debated potential benefits and drawbacks of innovations, enforced customary rules and byelaw and suggested changes in approaches and choices of interventions. In general, an innovation system approach helped to create local confidence, attract attention of government institutions and helped local actors to identify investment areas, develop implementation strategies to increase productivity, define changes as it occurs and minimize conflicts between competing communities. However, the risk of de facto use of a plot of communal land translating into long-term occupation and ownership may be impacting a communal territory and social cohesion that was subject to other collective choice customary rules.

The purpose of this paper is to describe how Chinese national adolescent mental health policy is translated into local plans and practice by means of the theoretical perspective of the policy triangle. Document analysis of national, provincial and local policy documents and semi-structured interviews with local policy actors were performed. National policies were implemented by a top-down process. The ministries were strong policy-makers on the national level, while the policy actors at the local level were mainly found in the educational sector. The content of the policy actions was moral education, knowledge dissemination and learning, rather than counselling and treatment.

In this chapter, the authors highlight that the challenges related to managing modern projects are that the management problems have changed since traditional project management methods were developed. This calls for new project management methods, adapted to situations of rapid technological change and flexibility. The authors identify and highlight the limitations of both traditional and agile management methods. They pay special attention to such project characteristics as obstacles to agility, skills, leadership, stakeholders and customers. They discuss hybrid project management and identify which method is the best fit in a given situation.

Technological advances and the evolution of mobile technologies enable patients to meet their doctors through their smartphones. While offering the opportunity of digital visits to patients, there are also challenges for this development. The purpose of this study is to enhance the understanding of digital visits, as perceived by experts working in telemedicine companies. To serve this purpose, the authors conducted semi-structured interviews with managers and employees in eight telemedicine companies. The analysis of the empirical data confirmed the importance of digital visits and their efficiency. The potential of digital visits is expected to increase significantly, should they make use of remote devices to transfer real-time data from patients to physicians. In such a case, digital visits are expected to cover approximately 70–75% of medical cases. However, the use of remote devices must be taken with caution and specific conditions need to be taken into account. We encourage researchers to perform research on promising technologies such as artificial intelligence and remote diagnostic devices, which could make more diagnoses and conditions possible to be treated by digital visits. This is even more important in light of the ongoing Covid-19 pandemic.

The bend and free recovery (BFR) test according to ASTM F2082 is a standard method to determine the transition temperatures of Nitinol shape memory alloys (SMAs). Unfortunately, this standard method is limited to SMA wires which are straight in its trained shape. Thus, the standard BFR test is not suitable for thermomechanical characterization of curved Nitinol SMA wires which should serve as actuators in cochlear implants in future. We developed a modified BFR measurement setup to determine the active austenite finish (AF) temperature of these very thin wires (Ø100 μm). The active AF temperature specifies the completion of the shape recovery upon heating. A parametric study of the measurement setup was carried out to investigate the influence of the heating rate on the observed active AF temperature and to verify the repeatability of the measurement setup. First, the curved wire was straightened in a cold water bath before inserting it into a water bath that is gradually heated from 5 °C to 45 °C. The shape change of the previously straightened wire was then recorded throughout the experiment using a digital microscope. Five different heating rates were employed: 0.25 K/min, 0.33 K/min, 0.5 K/min, 1 K/min as well as an unregulated maximum heating rate achievable of approximately 1.5 K/min. Furthermore, an investigation on the test-retest reliability was performed with three wires by repeating the experiment ten times with each wire. The results of this study revealed no influence of the heating rate on the thermomechanical response of the wires. Based on data from this study, a regulated heating rate of 1 K/min is suggested for future investigations, as this reduces the duration of the measurement from four hours to less than an hour. The values obtained from each wire through the test-retest reliability investigation showed a standard deviation of 1.9 K, 1.1 K and 2.1 K respectively. Our developed measurement setup demonstrates appropriate repeatability of the measurements.

Background Short stem hip prostheses have become a viable alternative for the treatment of hip osteoarthritis. In combination with minimally invasive surgery, short stem hip prostheses offer potential advantages of reduced surrounding tissue damage and faster rehabilitation. However, the limited perioperative visibility of these minimally invasive techniques may lead to reduced primary stability compared to conventional techniques. We hypothesized: a) increased migration of the minimally invasive group due to reduced primary stability and b) increased clinical scores of the minimally invasive group. Methods Sixty total hip arthroplasty patients were randomly assigned to undergo minimally invasive anterolateral modified Watson-Jones or conventional Hardinge surgery. All patients were treated with a Metha short stem prosthesis. Roentgen stereophotogrammetry and clinical scores were calculated after surgery and at 3, 6, 12, and 24 months postoperatively. Findings No statistically significant differences in implant migration and clinical scores were observed between the minimally invasive and conventional groups at any follow-up point. The largest average resultant migrations were 1.22 (SD 1.31) mm (conventional) and 1.18 (SD 1.18) mm (minimally invasive) after 24 months. The mean Harris Hip Score score improved from 54 (SD 10, conventional) and 52 (SD 13, minimally invasive) preoperatively to 97 (SD 5, both groups) after 24 months. Interpretation Overall, the differences in implant migration and clinical scores between the minimally invasive and conventional surgery groups were marginal and not clinically relevant. The data from this study suggest that good results can be expected from both the investigated approaches.

The G-protein coupled cannabinoid receptor 2 (CB2) has been implicated in the regulation of adult neurogenesis in the hippocampus. The contribution of CB2 towards basal levels of proliferation and the number of neural progenitors in the subgranular zone (SGZ) of the dentate gyrus, however, remain unclear. We stained hippocampal brain sections of 16- to 17-week-old wildtype and CB2-deficient mice, for neural progenitor and immature neuron markers doublecortin (DCX) and calretinin (CR) and for the proliferation marker Ki67 and quantified the number of positive cells in the SGZ. The quantification revealed that CB2 deficiency neither altered overall cell proliferation nor the size of the DCX+ or DCX and CR double-positive populations in the SGZ compared to control animals. The results indicate that CB2 might not contribute to basal levels of adult neurogenesis in four-month-old healthy mice. CB2 signaling might be more relevant in conditions where adult neurogenesis is dynamically regulated, such as neuroinflammation.

Recent genetic studies, using maternally inherited mitochondrial DNA, indicate a complex evolutionary history for baboons Papio spp. in general, and for eastern African baboons in particular. To further address this topic and to improve our understanding of phylogeographic patterns of baboons in eastern Africa, mitochondrial cytochrome b sequence data were analysed from 148 baboon samples from 103 locations in eastern Africa. The resultant phylogenetic reconstructions suggest an initial split of baboons into four main clades: southern chacma baboons, baboons from Mahale Mountains in Tanzania, main southern, and main northern. We confirm that the boundary between southern and northern clades lies along the Ugalla-Malagarasi River and Ruaha-Rufiji River of central Tanzania. We detected new mitochondrial haplogroups, most notably the Mahale Mountains clade, and refined haplogroup distributions. As part of a geoarchaeological project exploring the Early Bronze Age settlement of Fidvar, this geomorphologic study explores the fluvial dynamics of the Žitava River in southwestern Slovakia, casting light for the first time on the regional river's Holocene and late Pleistocene development. In order to reconstruct the Žitava's fluvial evolution, floodplain deposits were investigated by Electrical Resistivity Tomography, selected percussion drillings, and AMS radiocarbon dating. In combination, these methods provided a detailed view into the river's history. As a result of the climatic variations engendered by the last glacial cycle, the fluvial style repeatedly changed between a braided and anastomosing system. However, during long time periods of the Middle and Upper Plenivistulian, the river retained an anastomosing or even meandering style, in contrast to Central European rivers. Therefore, in agreement with other investigations, our results suggest different climatic conditions in the northern Pannonian Basin from 40 to 20 ka BP. During the Early and Middle Holocene, a dense vegetation cover (dry-mesic oak forests) led to stable geomorphological conditions and a meandering channel resulted in lateral erosion of the floodplain fringes. Accumulation of alluvial loamy sediments did not start until the Neolithic and has been related to anthropogenic landscape changes, similar to other naturally favoured areas (Central Europe). Finally, two reconstructed river positions indicate a considerable lateral fluvial erosion of the Early Bronze Age settlement at Fidvar. Consequently, the original settlement area – already thought to be the largest in the region – was once significantly larger.

BACKGROUND: In recent years, the interest of performing research about and providing e-services for individuals with disabilities has grown significantly. However, there is a knowledge gap in the existing literature on how to stimulate the pre-requisites needed for a sustainable implementation of e-services. AIM AND METHOD: The study aims to contribute to this knowledge gap by performing semi-structured email interviews with twelve project coordinators from the EU programmes of FP6-IST and FP7-ICT. RESULTS: The findings showed that knowledge-based and financially based resources, the complexity of the project context, ethical issues and the structural responsibility of the implementation process were key issues in creating a sustainable implementation. Furthermore, users often were not able to perceive added value from the e-services as they seldom were developed beyond prototypes within the research project. CONCLUSION: The implementation of e-services and/or ICT based applications for individuals with disabilities face similar challenges to the challenges that have been identified when implementing IT-based innovations. Further studies should focus on how users can be involved in the choice and development of e-services they understand as effective, and how to ensure that the implementation strategy will contribute to long-term social inclusion, that is, sustainability.

Background Getting research into policy and practice in healthcare is a recognised, world-wide concern. As an attempt to bridge the gap between research and practice, research funders are requesting more interdisciplinary and collaborative research, while actual experiences of such processes have been less studied. Accordingly, the purpose of this study was to gain more knowledge on the interdisciplinary, collaborative and partnership research process by investigating researchers’ experiences of and approaches to the process, based on their participation in an inventive national research programme. The programme aimed to boost collaborative and partnership research and build learning structures, while improving ways to lead, manage and develop practices in Swedish health and social services. Methods Interviews conducted with project leaders and/or lead researchers and documentation from 20 projects were analysed using directed and conventional content analysis. Results Collaborative approaches were achieved by design, e.g. action research, or by involving practitioners from several levels of the healthcare system in various parts of the research process. The use of dual roles as researcher/clinician or practitioner/PhD student or the use of education designed especially for practitioners or ‘student researchers’ were other approaches. The collaborative process constituted the area for the main lessons learned as well as the main problems. Difficulties concerned handling complexity and conflicts between different expectations and demands in the practitioner’s and researcher’s contexts, and dealing with human resource issues and group interactions when forming collaborative and interdisciplinary research teams. The handling of such challenges required time, resources, knowledge, interactive learning and skilled project management. Conclusions Collaborative approaches are important in the study of complex phenomena. Results from this study show that allocated time, arenas for interactions and skills in project management and communication are needed during research collaboration to ensure support and build trust and understanding with involved practitioners at several levels in the healthcare system. For researchers, dealing with this complexity takes time and energy from the scientific process. For practitioners, this puts demands on understanding a research process and how it fits with on-going organisational agendas and activities and allocating time. Some of the identified factors may be overlooked by funders and involved stakeholders when designing, performing and evaluating interdisciplinary, collaborative and partnership research. Objective To explore healthcare staffs’ and managers’ perceptions of how and when discrete event simulation modelling can be used as a decision support in improvement efforts. Design Two focus group discussions were performed. Setting Two settings were included: a rheumatology department and an orthopaedic section both situated in Sweden. Participants Healthcare staff and managers (n=13) from the two settings. Interventions Two workshops were performed, one at each setting. Workshops were initiated by a short introduction to simulation modelling. Results from the respective simulation model were then presented and discussed in the following focus group discussion. Results Categories from the content analysis are presented according to the following research questions: how and when simulation modelling can assist healthcare improvement? Regarding how, the participants mentioned that simulation modelling could act as a tool for support and a way to visualise problems, potential solutions and their effects. Regarding when, simulation modelling could be used both locally and by management, as well as a pedagogical tool to develop and test innovative ideas and to involve everyone in the improvement work. Conclusions Its potential as an information and communication tool and as an instrument for pedagogic work within healthcare improvement render a broader application and value of simulation modelling than previously reported.

Insulin-producing β cells can partially regenerate in adult pancreatic tissues, both in human and animal models of type 1 diabetes (T1D). Previous studies have shown that treatment with mycobacterial adjuvants such as CFA and bacillus Calmette-Guérin prevents induction and recurrence of T1D in NOD mice with partial recovery of β cell mass. In this study, we investigated factors involved in the regeneration of β cells in the pancreas of NOD mice during diabetes development and after treatment with adjuvants. The Regeneration (Reg) gene family is known to be involved in regeneration of various tissues including β cells. Reg2 expression was found to be upregulated in pancreatic islets both during diabetes development and as a result of adjuvant treatment in diabetic NOD mice and in C57BL/6 mice made diabetic by streptozotocin treatment. The upregulation of Reg2 by adjuvant treatment was independent of signaling through MyD88 and IL-6 because it was not altered in MyD88 or IL-6 knockout mice. We also observed upregulation of Reg2 in the pancreas of diabetic mice undergoing β cell regenerative therapy with exendin-4 or with islet neogenesis-associated protein. Reg2 expression following adjuvant treatment correlated with a reduction in insulitis, an increase in insulin secretion, and an increase in the number of small islets in the pancreas of diabetic NOD mice and with improved glucose tolerance tests in streptozotocin-treated diabetic C57BL/6 mice. In conclusion, adjuvant immunotherapy regulates T1D in diabetic mice and induces Reg2-mediated regeneration of β cells. Copyright © 2010 by The American Association of Immunologists, Inc.

In diesem Beitrag wird uber Biegeschubversuche mit Leichtbetonbalken ohne Querkraftbewehrung berichtet, die ein ausgepragtes Nachbruchverhalten aufwiesen. Die Versuchsergebnisse der Leichtbetonbalken werfen die Frage auf, ob die Querkraftbemessung in der neuen DIN 1045-1 in der Lage ist, hoch sprode Betone wie ALWAC (All Lightweight Aggregate Concrete) hinreichend sicher zu erfassen. In diesem Zusammenhang wird naher auf den Masstabseffekt eingegangen. Es wurde ein Modell fur das Tragverhalten von Biegeschubbalken nach Auftreten des Biegeschubrisses entwickelt, das auf der Kinematik von miteinander verbundenen Festkorpern anstelle von Fachwerkstaben beruht. Ein Ansatz der Tragkapazitat im Nachbruchbereich fur die Praxis wird nicht empfohlen. Shear Strength of Lightweight Concrete without Shear Reinforcement This paper presents results of shear beam tests with lightweight concrete and without shear reinforcement exhibiting a distinct post peak behavior. The shear test results raise the question whether the shear design in DIN 1045-1 can also cover very brittle concrete types as ALWAC (All Lightweight Aggregate Concrete) with a sufficient safety level. The size effect in shear beams is discussed. A model for the load bearing behavior after formation of the inclined crack based on the kinematics of a set of rigid bodies is developed. A consideration of the additional load capacity in the post peak range in design formulas is not recommended.
