La gastronomía tradicional es importante en la identidad de un pueblo o nación, es ahí donde el marketing es un aliado que permite la difusión de la cultura y las tradiciones alimentarias. El objetivo de este estudio se centra en la necesidad de conocer la influencia directa del marketing relacional con la gastronomía. Se realizó una revisión bibliográfica, aplicando el método deductivo y descriptivo que permitió la identificación de los constructos a investigar, también se reunió la información necesaria que apoyará este estudio. La investigación de campo determina que el marketing relacional y el boca a boca tienen influencia en la gastronomía tradicional. Los resultados evidencian el beneficio que trae consigo la implementación del marketing relacional en los establecimientos de comidas típicas. Con esto, se pudo corroborar las hipótesis planteadas que dictan que existen determinadas especialidades de marketing, así como también estrategias que ayudan a impulsar el reconocimiento de este tipo de gastronomía. Por lo tanto, es muy importante reconocer que dentro de toda la variedad de especialidades de marketing hay, en específico, dos especialidades que se emplean a menudo para que los establecimientos de comida tradicional tengan una clientela que le sea fiel y, sobre todo, que ayude a propagar las percepciones satisfactorias o positivas sobre los servicios mediante la recomendación de persona a persona.

Isolated insertional ruptures of the pectoralis minor tendon at the coracoid process are a rare condition. Hitherto, very few cases have been reported in the literature. A precise diagnosis is often difficult to obtain and commonly requires advanced imaging to confirm the suspicion and rule out concomitant injuries. All cases reported in the literature to date have been treated conservatively, with excellent results and no further complications. Here, however, we present the case of a patient who had developed a subclavian vein thrombosis. Furthermore, we provide an overview over and draw comparisons to the cases described in the literature. Despite the effectiveness of the conservative treatment, physicians should be aware that adverse events may occur.

Abstract Cardiac papillary fibroelastoma (CPF) is a primary cardiac neoplasm usually detected by echocardiography. Left ventricular fibroelastomas are extremely rare. The incidence of CPF is between 0.0017 and 0.33% during autopsy studies. We report a 70-year-old man who had papillary fibroelastoma discovered and resected in 2005 that recurred in 2013. The tumor grew rapidly from 2013 to 2014. A bioprosthetic mitral valve was placed in 2014. Due to the location and nature of the recurrent tumor, mitral valve replacement was the treatment of choice to prevent a third recurrence of the fibroelastoma. The patient was discharged from the hospital on postoperative day 9.

Objetivo: Analizar retrospectivamente los factores pronósticos y los resultados de la cirugía exclusiva o con radioterapia adyuvante de leiomiosarcomas uterinos. Material y Métodos: Desde 1992 a 2002, 18 pacientes con leiomiosarcomas uterinos fueron tratados en el servicio de oncología del Hospital Van Buren. Según FIGO: 61,1% estaba en etapa I, 5,5% en etapa II, 11,1% en etapa III y 22,2% en etapa IV. 17 % presentaban metástasis al diagnóstico. La edad promedio fue de 54,5 años. 33,3% de las pacientes fueron operadas con el diagnóstico de leiomiosarcoma y 66,7% con otro diagnóstico, principalmente leiomioma. 61,1% de las pacientes recibieron radioterapia adyuvante. Resultados: La mediana de sobrevida global fue 13 meses. Las sobrevida global, libre de enfermedad y libre de recurrencia loco-regional fueron a 5 años de 27,8%, 22,2% y 81,4%, respectivamente. Solamente 5,5% de las pacientes recidivaron exclusivamente en pelvis y 38,8% desarrolló metástasis a distancia exclusivas. Los factores pronósticos de sobrevida global fueron: la etapa FIGO, la cito reducción quirúrgica y la edad de las pacientes. La radioterapia adyuvante no aumentó la sobrevida global, la sobrevida libre de enfermedad, ni la sobrevida libre de recurrencia loco-regional comparado con el grupo de observación. Conclusiones: Concluimos que la histerectomía abdominal total con o sin salpingo-ooforectomía bilateral permanece como tratamiento de elección en leiomiosarcomas uterinos. La radioterapia adyuvante no tuvo impacto en resultados de tratamientoPurpose: With a retrospective study we propose to analyze the prognostic factors and results of uterine leiomyosarcomas treated with surgery alone or with adjuvant radiotherapy. Material and methods: From 1992 to 2002, 18 uterine leiomyosarcomas were treated at the department of oncology from Carlos Van Buren Hospital, corresponding to 61.1% stage I, 5.5% stage II, 11.1% stage III, and 22.2% stage IV. 17% of patients had metastatic disease at diagnosis. The mean age was 54.5 years. Preoperative diagnosis was 33.3% leiomyosarcomas and 66.7% with other diagnosis, principally leiomyoma. 61.1% of patients received adjuvant radiotherapy. Results: The median overall survival was 13 months with 5 year overall survival, disease-free survival and loco-regional disease free survival of 27.8%, 22,2% and 81,4%, respectively. Only 5.5% had recurrence disease in pelvis alone and 38.8% developed exclusively metastatic disease. Prognostic factors for overall survival were FIGO stage, surgical cytoreduction and age. Adjuvant radiotherapy does not appear to significantly affect overall survival, disease free survival and loco-regional disease free survival. Conclusion: We conclude that total abdominal hysterectomy with or without bilateral salpingo-oophorectomy remain the treatment of choice for uterine leiomyosarcoma. The application of a cervical collar is a standard procedure in trauma patients in emergency medicine. It is often observed that cervical collars are applied incorrectly, resulting in reduced immobilization of the cervical spine. The objective of this study was to analyze the practical skills of trained professional rescue personnel concerning the application of cervical collars.Within emergency medical conferences, n = 104 voluntary test subjects were asked to apply a cervical collar to a training doll, wherein each step that was performed received an evaluation. Furthermore, personal and occupational data of all study participants were collected using a questionnaire.The test subjects included professional rescue personnel (80.8%) and emergency physicians (12.5%). The average occupational experience of all study participants in pre-clinical emergency care was 11.1±8.9 years. Most study participants had already attended a certified training on trauma care (61%) and felt "very confident" in handling a cervical collar (84%). 11% applied the cervical collar to the training doll without errors. The most common error consisted of incorrect adjustment of the size of the cervical collar (66%). No association was found between the correct application of the cervical collar and the occupational group of the test subjects (trained rescue personnel vs. emergency physicians) or the participation in certified trauma courses.Despite pronounced subjective confidence regarding the application of cervical collars, this study allows the conclusion that there are general deficits in practical skills when cervical collars are applied. A critical assessment of the current training contents on the subject of trauma care must, therefore, be demanded.

Obesity and metabolic syndrome reflect the dysregulation of molecular pathways that control energy homeostasis. Here, we show that the p75 neurotrophin receptor (p75(NTR)) controls energy expenditure in obese mice on a high-fat diet (HFD). Despite no changes in food intake, p75(NTR)-null mice were protected from HFD-induced obesity and remained lean as a result of increased energy expenditure without developing insulin resistance or liver steatosis. p75(NTR) directly interacts with the catalytic subunit of protein kinase A (PKA) and regulates cAMP signaling in adipocytes, leading to decreased lipolysis and thermogenesis. Adipocyte-specific depletion of p75(NTR) or transplantation of p75(NTR)-null white adipose tissue (WAT) into wild-type mice fed a HFD protected against weight gain and insulin resistance. Our results reveal that signaling from p75(NTR) to cAMP/PKA regulates energy balance and suggest that non-CNS neurotrophin receptor signaling could be a target for treating obesity and the metabolic syndrome.

Astrocytes modulate neuronal activity and inhibit regeneration. We show that cleaved p75 neurotrophin receptor (p75(NTR)) is a component of the nuclear pore complex (NPC) required for glial scar formation and reduced gamma oscillations in mice via regulation of transforming growth factor (TGF)-β signaling. Cleaved p75(NTR) interacts with nucleoporins to promote Smad2 nucleocytoplasmic shuttling. Thus, NPC remodeling by regulated intramembrane cleavage of p75(NTR) controls astrocyte-neuronal communication in response to profibrotic factors.

AbstractPolitical science research usually locates limited statehood outside the territory of the European Union (EU), although forms of limited statehood are hardly foreign phenomena in some parts of the EU, for example, Southern Italy. To understand these configurations of statehood, we introduce the concept of hybrid statehood as a specific form of societal order, characterized by a complex mixture of elements of European statehood and other forms of social order. It challenges the dichotomous understanding of statehood as either consolidated or limited, putting more emphasis on the interdependent coexistence of societal order, the practices of doing statehood and negotiating societal order, and the impact of historical constellations of societal order in the longue durée. We argue that bridging the gap between political science, history and social anthropology allows for a more in-depth understanding of statehood there. We need to take the challenge of understanding such societies very seriously, considering that Italy and the EU have failed to implement their visions of statehood and societal order for decades. This is even more the case if we consider that such configurations of hybridity will increasingly travel to other parts of the EU due to migration from the southern Mediterranean.

Abstract Tactile sensation and vision are often both utilized for the exploration of objects that are within reach though it is not known whether or how these two distinct sensory systems combine such information. Here in mice, we used a combination of stereo photogrammetry for 3D reconstruction of the whisker array, brain-wide anatomical tracing and functional connectivity analysis to explore the possibility of tacto-visual convergence in sensory space and within the circuitry of the primary visual cortex (VISp). Strikingly, we find that stimulation of the contralateral whisker array suppresses visually evoked activity in a tacto-visual sub-region of VISp whose visual space representation closely overlaps with the whisker search space. This suppression is mediated by local fast-spiking interneurons that receive a direct cortico-cortical input predominantly from layer 6 neurons located in the posterior primary somatosensory barrel cortex (SSp-bfd). These data demonstrate functional convergence within and between two primary sensory cortical areas for multisensory object detection and recognition.
