BACKGROUND & AIMS: Home parenteral nutrition (HPN) is a necessary treatment for patients with chronic, type 3, intestinal failure (IF). HPN often requires lifestyle adaptations, which are likely to affect quality of life (QoL) in both patients and family members. The aim of this study was to identify the level of burden on family members who are involved with HPN care and to understand specific factors that contribute to any burden.METHODS: Patients over the age of 18 and receiving HPN were identified in IF clinics from multiple centres across the U.K. Eligible patients were asked to complete the parenteral nutrition impact questionnaire (PNIQ) to assess their QoL, while family members were asked to complete the burden scale for family caregivers (BSFC). Logistical regression was undertaken giving adjusted odds ratios (aOR).RESULTS: 678 participants completed the survey representing 339 patients with their appointed family member. Mean PNIQ score was 11.53 (S.D. 5.5), representing a moderate impact of HPN on patients' QoL. On the BSFC scale, 23% of family members reported a moderate to very severe subjective burden indicating an increased risk of psychosomatic symptoms. After adjusting for age and gender, predictors of BSFC included: family members self-reported health status using the EuroQol visual analogue scale (aOR 19.91, 95% CI 1.69, 233.99, p = 0.017) and support received by health services (aOR = 5.83, 95% CI = 1.93, 17.56, p = 0.002). Employment status, disease type, number of nights on HPN and length of time on HPN were not associated with BSFC.CONCLUSIONS: Family members with a poor health status or lack of support by health service were more likely to have a moderate to very severe subjective burden. Tailored support from the multi-professional IF team may reduce the burden experienced by family members of people dependent on HPN.

Many work on flushing out what our consciousness means in cognitive and phenomenological terms, but no one has yet connected the dots on how consciousness and truth intersect, much less how our phenomenal consciousness can form the ground for most of our models of truth. Here, I connect those dots and argue that the basic structure of our phenomenal consciousness grounds the nature of truth as concordance, to harmonize in agreement, and that most extant theories on truth are well explained in that grounding. Said another way, the unifying and bifurcating intentional structure of phenomenal consciousness is the non-epistemic ground of truth, such that most theories of truth can be explained as particular expressions of concordance based upon the differing aspects of that ground.

The goal of this research was to use a computational model of human metabolism to predict energy metabolism for lean and obese men. The model is composed of 6 state variables representing amino acids, muscle protein, visceral protein, glucose, triglycerides, and fatty acids (FAs). Differential equations represent carbohydrate, amino acid, and FA uptake and output by tissues based on ATP creation and use for both lean and obese men. Model parameterization is based on data from previous studies. Results from sensitivity analyses indicate that model predictions of resting energy expenditure (REE) and respiratory quotient (RQ) are dependent on FA and glucose oxidation rates with the highest sensitivity coefficients (0.6, 0.8 and 0.43, 0.15, respectively, for lean and obese models). Metabolizable energy (ME) is influenced by ingested energy intake with a sensitivity coefficient of 0.98, and a phosphate-to-oxygen ratio by FA oxidation rate and amino acid oxidation rate (0.32, 0.24 and 0.55, 0.65 for lean and obese models, respectively). Simulations of previously published studies showed that the model is able to predict ME ranging from 6.6 to 9.3 with 0% differences between published and model values, and RQ ranging from 0.79 to 0.86 with 1% differences between published and model values. REEs >7 MJ/d are predicted with 6% differences between published and model values. Glucose oxidation increases by ∼0.59 mol/d, RQ increases by 0.03, REE increases by 2 MJ/d, and heat production increases by 1.8 MJ/d in the obese model compared with lean model simulations. Increased FA oxidation results in higher changes in RQ and lower relative changes in REE. These results suggest that because fat mass is directly related to REE and rate of FA oxidation, body fat content could be used as a predictor of RQ.

Karen Barad develops a view she calls ‘posthumanism,’ or ‘agential realism,’ where the human is reconfigured away from the central place of explanation, interpretation, intelligibility, and objectivity to make room for the epistemic importance of other material agents. Barad is not alone in this kind of endeavor, but her posthumanism offers a unique epistemological position. Her aim is to take a performative rather than a representationalist approach to analyzing ‘socialnatural’ practices and challenge methodological assumptions that may go unnoticed in some disciplinary fields. Yet for all the good of the challenge, Barad must support it with sound epistemological theorizing, theorizing that would apply to any methodology, whether that be sociological, historical, anthropological, or philosophical. Thus, where one might critique Barad on her assessments of sociological, historical, or anthropological incorporations of humans and the nonhuman, I critique Barad’s epistemology on its sense of objectivity and dismissal of the centrality of the human. I argue that Barad’s epistemology must retain a particular form of humanism, a humanism that stakes human subjectivity as the locus of rationality and objectivity, without which it creates intractable problems. To recuperate Barad’s challenge to contest assumptive distinctions while avoiding her epistemological problems, I offer some parting reflections.

KGaA, Weinheim Scope: This work aims to evaluate changes in the fecal metabolomic profile due to grape seed extract (GSE) intake by untargeted and targeted analysis using high resolution mass spectrometry in conjunction with multivariate statistics. Methods and results: An intervention study with six crossbred female pigs was performed. The pigs followed a standard diet for 3 days, then they were fed with a supplemented diet containing 1% (w/w) of MegaNatural® Gold grape seed extract for 6 days. Fresh pig fecal samples were collected daily. A combination of untargeted high resolution mass spectrometry, multivariate analysis (PLS-DA), data-dependent MS/MS scan, and accurate mass database matching was used to measure the effect of the treatment on fecal composition. The resultant PLS-DA models showed a good discrimination among classes with great robustness and predictability. A total of 14 metabolites related to the GSE consumption were identified including biliary acid, dicarboxylic fatty acid, cholesterol metabolites, purine metabolites, and eicosanoid metabolites among others. Moreover, targeted metabolomics using GC-MS showed that cholesterol and its metabolites fecal excretion was increased due to the proanthocyanidins from grape seed extract. Conclusion: The results show that oligomeric procyanidins from GSE modifies bile acid and steroid excretion, which could exert a hypocholesterolemic effect.

For many epistemologies, theory and practice remain bifurcated concepts that facilitate the application of theoretical, epistemic structures upon groups of people and their practices without consideration for the actuality of these people and practices. In contrast, an appropriate epistemology should recognize existing practices that produce what we take to be knowledge, and develop theories of knowledge and rationality based upon them, not in spite of them. In short, an appropriate epistemology should be a "practicist" epistemology. Endorsing an epistemology that gives fundamental attention to the structure and purpose of our actual practices is not new, but oftentimes, convincing arguments for a practicist epistemology are not forthright. In this paper, I argue that when we consider the nature of epistemological theorizing, and the nature of theories themselves, whether they are meant to organize or fit reality and our experiences, we are unable to separate out the role and importance of practices. Therefore, we should likewise give due respect to the primacy of epistemic practices which comprise the impetus for our epistemological theorizing. he rift between theory and practice persists in areas of epistemology where abstract preoccupations relegate concrete human practices and their consequences to the fringes of concern. It is commonplace to measure the fruitfulness of epistemologies based on how well they reconcile dilemmas such as global skepticism, the lottery paradox, and Gettier's instances of justified true belief that do not warrant the designation of 'knowledge'. Other means of evaluation include how well epistemologies sustain their theories of justification and avoid the regress problem, as is frequently done with foundationalist, coherentist, and contextualist programs. But while many of these theoretical concerns are appropriate at times - I do not discount them - they cannot be esteemed at the expense of not taking seriously our existing epistemic practices. There is a tendency, a bad tendency, to make epistemic agency the central focus of epistemology. In brief, epistemologists have traditionally elevated epistemic agency as the crucial issue to be addressed, and ask all other epistemological questions in light of that issue. This is not surprising given the Cartesian influence on epistemology, but I argue that epistemic agency should not always be the central focus of epistemology. There are times when giving central place to epistemic agency gets in the way of good epistemology. In this paper, I show how one approach to a communitarian epistemology falters precisely because of the primacy given to epistemic agency. Specifically, I take a close look at Lynn Hankinson Nelson’s notion of epistemological communities in terms of epistemic agency and analyze both its merits and its shortcomings. In Heidi Grasswick’s response to “‘Epistemological communities’ and the problem of epistemic agency,” she criticizes my move to reconceptualize epistemology as an affair primarily centered on epistemic practises instead of epistemic agency. In this paper, I address some of Grasswick’s counterpoints, and I restate my argument for why epistemology should be centered on practises instead of epistemic agency. However, to advance the discussion, I urge that a more fruitful dialogue would engage looking at what consequences and advantages might follow from reframing epistemology around practises. Therefore, I entertain an additional advantage this turn to practises makes regarding the danger of apriorism, the prior construction and application of theoretical structures upon groups of people and their practises without consideration for the actuality of these people and their practises. Realism about the external world enjoys little philosophical support these days. I rectify this predicament by taking a relatively pragmatist line of thought to defend commonsense realism; I support commonsense realism through an interpretation and application of Donald Davidson’s notion of triangulation, the triangle composed of two communicators coordinating and correcting their responses with a shared causal stimulus. This argument is important because it has a crucial advantage over the often used abductive argument for realism. My argument avoids unwarranted conclusions, whereas the abductive argument is “inflationary” because it reaches beyond the limits of evidence for its realist conclusion. To illustrate the problems of the abductive argument and motivate my Davidsonian approach, I take a brief look at the abductive argument for realism in Frederick Will’s work.

Barry Barnes and David Bloor of the Strong Programme of the sociology of knowledge advance a naturalized epistemology that reduces all accounts of normativity to social causes. I endorse their program of naturalizing one kind of normativity, but I argue that there is another kind they cannot naturalize. Within the context of sociological explanations of rationality, there are (1) norms of rationality instantiated by scientists that Barnes and Bloor study, and (2) Barnes and Bloor's own normative ascriptions of scientists as rational beings. I argue that Barnes and Bloor successfully justify their naturalization of the norms of rationality of the scientists they describe by a methodological caution and arguments from under-determination and finitism. But they inevitably fail to naturalize their own normative ascriptions of those scientists as rational.

Background The newborn mammal is rapidly colonized by a complex microbial community, whose importance for host health is becoming increasingly clear. Understanding the forces that shape the early community, especially during the nursing period, is critical to gain insight into how this consortium of microbes is assembled. Pigs present an attractive model for nursing humans, given physiological and compositional similarity of pig and human milk and the utility of pigs in experimental studies. However, there is a paucity of data examining the gut microbiome in nursing pigs from birth through weaning using modern molecular methods and fewer experimental studies that examine the impact of diet on these microbial communities. Results We characterized the fecal microbiome of pigs from birth through 7 weeks of age, during which the animals were transitioned from an exclusive diet of sow milk to a starter diet composed of plant and animal-based components. Microbial communities were clearly distinguishable based on diet, being relatively stable absent dietary changes. Metagenomic sequencing was used to characterize a subset of animals before and after weaning, which identified glycan degradation pathways differing significantly between diets. Predicted enzymes active on milk-derived glycans that are otherwise indigestible to the host animal were enriched in the microbial metagenome of milk-fed animals. In contrast, the bacterial metagenome of weaned animals was enriched in functional pathways involved in plant glycan deconstruction and consumption. Conclusions The gut microbiome in young pigs is dramatically shaped by the composition of dietary glycans, reflected by the different functional capacities of the microbiome before and after weaning. Proanthocyanidin (PAC) consumption has been linked to better colonic health, but PACs are poorly absorbed, making them a target for colonic metabolism. The resulting metabolites are low molecular weight and could potentially be absorbed. To understand the effects of dietary PACs it would be important to resolve the metabolic issue and link these changes to microbial population changes in a suitable model for human digestion. Here, six crossbred female pigs were fed a diet containing 1% (w/w) of MegaNatural® Gold grape seed extract (GSE) daily for 6 days. Fecal samples were analyzed by normal phase LC coupled to fluorescence detection and LC-MS/ToF. DNA was extracted from pig fecal samples and the V3/V4 region of the 16S rRNA gene was sequenced using an Illumina MiSeq. Intact parent PACs (dimer-pentamer) were observed in the feces on days 3 and 6 at similar high levels (∼400 mg kg(-1) total) during ingestion of GSE but were absent 48 h post-feeding. The major phenolic metabolites were 4-hydroxyphenylvaleric acid and 3-hydroxybenzoic acid which increased by ∼30 and 3 mg kg(-1) respectively. The GSE diet also caused an ecological shift in the microbiome, dramatically increasing Lachnospiraceae, Clostridales, Lactobacillus and Ruminococcacceae. The relationship between dietary PACs and colon health may be attributable to the altered bacterial populations or phenolic compounds in the colon.

The diet of the feral domestic cat consists of primarily birds and small mammals, but the nutritional composition is relatively unknown. Because of the increasing popularity of natural diets for cats and other wild captive carnivores, the purpose of this study was to describe the body composition and AA concentrations of select birds and small mammals in northern and central California: wild-caught mice (n = 7), Norway rats (n = 2), roof rats (n = 2), voles (n = 4), moles (n = 2), gophers (n = 3), and birds (n = 4). Body water, crude fat (CFa), CP, ash, and AA composition for each specimen were determined. Results are reported as mean ± SD. All results are reported on a DM basis except body water (as-is basis) and AA (g/16 g N). Combined, carcasses had this mean composition: 67.35 ± 3.19% water, 11.72 ± 6.17% CFa, 62.19 ± 7.28% CP, and 14.83 ± 2.66% ash. Concentrations of Arg, Tau, Cys, and Met were 5.63 ± 0.46, 0.92 ± 0.33, 1.91 ± 0.89, and 1.82 ± 0.19 g/16 g N, respectively. Using NRC physiologic fuel values for CP, CFa, and carbohydrate by difference, the combined average energy content of the carcasses was 3,929 kcal/kg DM, but the fiber content was not determined. With the exception of mice and rats, little historical data exist regarding the body and AA composition of many of the species analyzed in this study. Wild-caught mice and rats were composed of less fat but more ash compared with previously reported data in their purpose-bred counterparts. The CP content of mice in this study was similar to previous reports in purpose-bred mice. The CP content of rats was similar or slightly greater compared with historical findings in purpose-bred rats. The N content of rats and AA concentrations on a per-N basis for both rats and mice were similar to previously published data on purpose-bred rodents. The discrepancies in nutrient composition, especially fat concentration, indicate that using purpose-bred animals to represent the diet of the feral domestic cat may not be valid in many instances. When consumed to meet energy needs, the nutrient content of the species reported in the present study exceed the NRC (2006) recommended allowances (RA) for total fat, CP, and essential AA for felines at all life stages.

SummarySperm plasma membrane (PM) proteins that demonstrate affinity for egg PM preparations have the potential to be biologically important during sperm-egg binding and/or fusion. In this study four such proteins have been identified. To provide quantitative evidence for possible biological function, the large natural variation among different porcine sperm populations with regard to their ability to interact with the egg was compared with the relative binding of egg PM material to individual proteins. An aliquot from each of 24 porcine ejaculates was evaluated by the zona-free hamster ova bioassay and the remainder processed to yield sperm PM vesicles. Aliquots of sperm PM were solubilised, separated by SDS-PAGE, western blotted and probed with partially purified, biotinylated egg PM protein. Bound egg PM proteins were visualised on western blots by an avidin/biotin/horseradish peroxidase system and analysed by scanning laser densitometry. Four sperm PM proteins (62, 39, 27 and 7 kDa estimated molecular mass) were the predominant binders of egg PM. The amount of egg PM bound to the 62 kDa protein was significantly correlated with the ability of sperm from the 24 ejaculates to penetrate zona-free hamster ova (percentage of ova penetrated, p = 0.01, R = 0.65; number of penetrated sperm per ovum, p = 0.02, R = 0.63)

Male reproductive capacity was examined in 5 lines of mice which differed markedly in mature body weight (30-73 g) due to selection for growth and/or substitution of the high growth gene (hg). Testes weight ranged from 0.75-0.77% of body weight in control lines to 0.33% in the largest mice which had the hg gene in a growth selected background. Both selection for growth and the hg gene in a control background reduced absolute testes size (0.200 and 0.207 g vs 0.236 and 0.228 g in control lines) as well as relative testes size although body weight was increased by at least 50%. Although the combination of growth background and hg gene reduced sperm production per g testis compared to the outbred control, the primary cause of reduced sperm production per mouse in lines containing either the growth background or the hg gene alone was reduced absolute testes size. At 24, but not at 11, weeks of age, the hg gene reduced sperm motility. In these lines, high genetic potential for post-weaning growth was associated with decreased male reproductive capacity.

Catch-up growth during recovery from undernutrition is characterized by rapid body weight gain often marked by disproportionately high fat gain. In this study dietary zinc intake of mouse pups during recovery from undernutrition affected composition of gain as well as growth. Mouse pups were undernourished during the suckling period and then fed 25% casein recovery diets containing 5, 10, 40 or 110 micrograms Zn/g diet. Pups given dietary zinc levels of 40 micrograms Zn/g ad libitum achieved recovery body weights and had normal body composition. However, previously undernourished pups given marginal levels of dietary zinc (10 micrograms Zn/g diet) had similar food intakes, similar final body weights, lower lean body mass gains and greater fat gains (males only) than controls. Pups fed low zinc diets (5 micrograms Zn/g diet) were unable to attain recovery body weight and had less protein gain than previously undernourished animals fed higher zinc-containing diets or healthy pups fed the same zinc levels. The results were consistently more pronounced in males. Thus dietary zinc deficiency during catch-up growth after undernutrition limited both recovery and protein gain, whereas marginal dietary zinc intake limited protein gain in mice.

Previous research in this laboratory demonstrated the existence of a major gene (hg), expressed as a homozygous recessive, which increases postweaning growth by 60% in C57Bl/6 mice (line Ch) compared to the same genetic stock without the major gene (line CH). The effect of the hg gene on the maintenance energy requirement and net energetic efficiency of mice between 21 and 42 d of age was examined in a comparative slaughter experiment. Regressions of changes in body energy per kilogram0.75 on metabolizable energy intake per kilogram0.75 indicated that the maintenance energy requirements averaged 164 kcal/(kg0.75 X d) and 155 kcal/(kg0.75 X d) for lines CH and Ch, respectively. Net efficiency of energy utilization was 38.5 and 49.5% for lines CH and Ch, respectively. Ad libitum-fed mice of line Ch gained 46% more total body energy per unit of metabolic body weight (kilogram0.75) than ad libitum-fed mice from line CH while their energy intake per kilogram0.75 was equal. It was estimated that the decreased maintenance energy requirement and increased net energetic efficiency accounted for approximately 25 and 75%, respectively, of the increased energy gain of ad libitum-fed mice of line Ch as compared to CH. The increased energy gain of line Ch was largely (78%) due to an increase fat energy deposition. Regressions of fat energy gain per kilogram0.75 on metabolizable energy intake per kilogram0.75 indicated that the increased fat energy gain was due to an increased efficiency of fat deposition and/or an increased proportion of metabolizable energy available for gain used for fat gain.
