A survey in Dhaka reported that 48% of respondents with symptoms of acute respiratory illness (ARI) identified local pharmacies as their first point of care. This study explores the factors driving urban customers to seek health care from pharmacies for ARI, their treatment adherence, and outcome.Methods: A cross-sectional study was conducted among 100 selected pharmacies within Dhaka from June to December 2012. Study participants were patients or patients’ relatives aged >18 years seeking care for ARI from pharmacies without prescription. Structured interviews were conducted with customers after they sought health service from drug sellers and again over phone 5 days postinterview to discuss treatment adherence and outcome.Results: We interviewed 302 customers patronizing 76 pharmacies; 186 (62%) sought care for themselves and 116 (38%) sought care for a sick relative. Most customers (215; 71%) were males. The majority (90%) of customers sought care from the study pharmacy as their first point of care, while 18 (6%) had previously sought care from another pharmacy and 11 (4%) from a physician for their illness episodes. The most frequently reported reasons for seeking care from pharmacies were ease of access to pharmacies (86%), lower cost (46%), availability of medicine (33%), knowing the drug seller (20%), and convenient hours of operation (19%). The most commonly recommended drugs were acetaminophen dispensed in 76% (228) of visits, antihistamine in 69% (208), and antibiotics in 42% (126). On follow-up, most (86%) of the customers had recovered and 12% had sought further treatment.Conclusion: People with ARI preferred to seek care at pharmacies rather than clinics because these pharmacies were more accessible and provided prompt treatment and medicine with no service charge. We recommend raising awareness among drug sellers on proper dispensing practices and enforcement of laws and regulations for drug sales. Abstract

In this study, a complete study of the effect of hydrothermal reaction temperatures on the synthesis and physical properties of V2O5 using the green facile mild hydrothermal method has been performed with six different temperatures 100 {\deg}C to 200 {\deg}C, in the step of 20 {\deg}C. . The XRD pattern confirm the stable orthorhombic crystal structure of the synthesized samples at all reaction temperatures. The SEM and TEM images demonstrate the particle-like morphology, and these characterizations affirmed that the particles size became larger with the increase of reaction temperatures. The FTIR analysis is employed to study the functional groups, and the obtained results are consistent with the XRD analysis. The bandgap has been estimated at various reaction temperatures using UV-vis diffuse reflectance spectra (UV-DRS) and was found to be varied 2.09 eV to 2.15 eV that are suitable range to absorb a significant amount of visible light. The photocatalysis of methylene blue (MB) with synthesized samples has been accomplished to investigate photocatalytic efficiency. Abstract

Yb-substituted Ni-Zn ferrites have been synthesized using sol-gel auto combustion method. The structural characterization of the compositions has been performed by X-ray diffraction analysis, field emission scanning electron microscopy (FESEM), quantum design physical properties measurement system (PPMS). That ensured the formation of single phase cubic spinel structure. Crystallite and average grain size are calculated and found to decrease with increasing Yb3+ contents. Saturation magnetization and Bohr magnetic moment decrease while the coercivity increases with the increase in Yb3+ contents successfully explained by the Neels collinear two sub-lattice model and critical size effect, respectively. Critical particle size has been estimated at 6.4 nm, the transition point between single domain regime (below the critical size) and multi-domain regime (beyond the critical size). Curie temperature reduces due to the weakening of A-O-B super exchange interaction and redistribution of cations, confirmed by the M-T graph. The compositions retain ferromagnetic ordered structured below Curie temperature and above Curie temperature, it becomes paramagnetic, making them plausible candidates for high temperature magnetic device applications. The relative quality factor peak is obtained at a very high frequency, indicating the compositions could also be applicable for high frequency magnetic device applications.

Abstract

Polycrystalline Yb substituted NiZn nanoferrites with the compositions of Ni0.5Zn0.5YbxFe2-xO4 (x= 0.00, 0.04, 0.08, 0.12, 0.16 and 0.20) have been synthesized using sol gel auto combustion technique. Single phase cubic spinel structure has been confirmed by the X ray diffraction (XRD) patterns. Larger lattice constants of the compositions are found with increasing Yb3+ concentration while the average grain size (52 to 18 nm) has noticeable decrease as Yb3+ content is increased. The presence of all existing elements as well as the purity of the samples has also been confirmed from energy dispersive X ray spectroscopic (EDS) analysis. Frequency dependent dielectric constant, dielectric loss, dielectric relaxation time, AC and DC resistivity of the compositions have also been examined at room temperature. The DC resistivity value is found in the order of 10 to power 10 (omega-cm) which is at least four orders greater than the ferrites prepared by conventional method. This larger value of resistivity attributes due to very small grain size and successfully explained using the Verwey and deBoer hopping conduction model. The contribution of grain and grain boundary resistance has been elucidated using Cole Cole plot. The study of temperature dependent DC resistivity confirms the semiconducting nature of all titled compositions wherein bandgap (optical) increases from 2.73 eV to 3.25 eV with the increase of Yb content. The high value of resistivity is of notable achievement for the compositions that make them a potential candidate for implication in the high frequency applications where reduction of eddy current loss is highly required., Comment: 27 pages

Abstract

We report the synthesis of Y-substituted Mg-Zn ferrites using conventional standard ceramic technique. XRD patterns confirm the single phase cubic spinel structure up to x = 0.03 and appearance of a secondary phase of YFeO3for higher Y contents. FESEM images depict the distribution of grains and EDS spectra confirmed the absence of any unwanted element. Completion of solid state reaction and formation of spinel structure has been revealed from FTIR spectra. The FTIR data along with lattice constant, bulk density and porosity were further used to calculate the stiffness constant (Cij), elastic constant and Debye temperatures. Mechanical stability of all studied compositions is confirmed from Cij using Born stability conditions. Brittleness and isotropic nature are also confirmed using Poisson ratio and anisotropy constants, respectively. The enhancement of dc electrical resistivity with Y content is observed. The energy band gap (increased with Y contents) is found in good agreement with dc electrical resistivity. Ferrimagnetic to paramagnetic phase change has been observed from the field dependent high temperature magnetization curves. The magnetic moments and saturation magnetization were found to be decreased with increasing temperature. The Curie temperature (Tc) has been measured from temperature dependent magnetic moment (M-T) and initial permeability and found to be in good agreement with each other. Decrease in Tc with Y content is due to redistribution of cations and weakening of the exchange coupling constant. The magnetic phase transition has been analyzed by Arrott plot and found to have second order phase transition. Abstract

Querying graph structured data is a fundamental operation that enables important applications including knowledge graph search, social network analysis, and cyber-network security. However, the growing size of real-world data graphs poses severe challenges for graph databases to meet the response-time requirements of the applications. Planning the computational steps of query processing - Query Planning - is central to address these challenges. In this paper, we study the problem of learning to speedup query planning in graph databases towards the goal of improving the computational-efficiency of query processing via training queries.We present a Learning to Plan (L2P) framework that is applicable to a large class of query reasoners that follow the Threshold Algorithm (TA) approach. First, we define a generic search space over candidate query plans, and identify target search trajectories (query plans) corresponding to the training queries by performing an expensive search. Subsequently, we learn greedy search control knowledge to imitate the search behavior of the target query plans. We provide a concrete instantiation of our L2P framework for STAR, a state-of-the-art graph query reasoner. Our experiments on benchmark knowledge graphs including DBpedia, YAGO, and Freebase show that using the query plans generated by the learned search control knowledge, we can significantly improve the speed of STAR with negligible loss in accuracy., Comment: Published in the Proceedings of the 27th International Conference on Automated Planning and Scheduling (ICAPS), 2017

Abstract

Attention-based models have recently shown great performance on a range of tasks, such as speech recognition, machine translation, and image captioning due to their ability to summarize relevant information that expands through the entire length of an input sequence. In this paper, we analyze the usage of attention mechanisms to the problem of sequence summarization in our end-to-end text-dependent speaker recognition system. We explore different topologies and their variants of the attention layer, and compare different pooling methods on the attention weights. Ultimately, we show that attention-based models can improves the Equal Error Rate (EER) of our speaker verification system by relatively 14% compared to our non-attention LSTM baseline model., Comment: Submitted to ICASSP 2018

Abstract

A series of Ni0.6-x/2Zn0.4-x/2SnxFe2O4 (x = 0.0, 0.05, 0.1, 0.15, 0.2 and 0.3) (NZSFO) ferrite composites have been synthesized from nano powders using standard solid state reaction technique. The spinel cubic structure of the investigated samples has been observed by the X-ray diffraction (XRD). The magnetic properties such as saturation magnetization (Ms), remanent magnetization (Mr), coercive field (Hc) and Bohr magneton (B) are calculated from the hysteresis loops. The value of Ms is found to decrease with increasing Sn content in the samples. This change has been successfully explained by the variation of A-B interaction strength due to Sn substitution in different sites. The compositional stability and quality of the prepared ferrite composites have also been endorsed by the fairly constant initial permeability (/) over a wide range of frequency region. The decreasing trend of / with increasing Sn content has been observed. Curie temperature (TC) has found to increase with the increase in Sn content. Wide spread frequency utility zone indicates that the NZSFO can be considered as a good candidate for use in broadband pulse transformer and wide band read-write heads for video recording. Abstract

The Sn substituted Ni-Zn ferrites were synthesized by the standard double sintering technique using nano powders of nickel oxide (NiO), zinc oxide (ZnO), iron oxide (Fe2O3) and tin oxide (SnO2). The structural and electrical properties have been investigated by the X-ray diffraction, scanning electron microscopy, DC resistivity and dielectric measurements. Extra intermediate phase has been detected along with the inverse cubic spinel phase of Ni-Zn ferrite. Enhancement of grain size is observed in Sn substituted Ni-Zn ferrites. DC resistivity as a function of temperature has been investigated by two probe method. The DC resistivity was found to decrease whereas the dielectric constants increase with increasing Sn content in Ni-Zn ferrites. Abstract

Background: The COVID-19 pandemic heightened risks for individuals with hematological malignancies due to compromised immune systems, leading to more severe outcomes and increased mortality. While interventions like vaccines, targeted antivirals, and monoclonal antibodies have been effective for the general population, their benefits for these patients may not be as pronounced. Methods: The EPICOVIDEHA registry (National Clinical Trials Identifier, NCT04733729) gathers COVID-19 data from hematological malignancy patients since the pandemic's start worldwide. It spans various global locations, allowing comprehensive analysis over the first three years (2020–2022). Findings: The EPICOVIDEHA registry collected data from January 2020 to December 2022, involving 8767 COVID-19 cases in hematological malignancy patients from 152 centers across 41 countries, with 42% being female. Over this period, there was a significant reduction in critical infections and an overall decrease in mortality from 29% to 4%. However, hospitalization, particularly in the ICU, remained associated with higher mortality rates. Factors contributing to increased mortality included age, multiple comorbidities, active malignancy at COVID-19 onset, pulmonary symptoms, and hospitalization. On the positive side, vaccination with one to two doses or three or more doses, as well as encountering COVID-19 in 2022, were associated with improved survival. Interpretation: Patients with hematological malignancies still face elevated risks, despite reductions in critical infections and overall mortality rates over time. Hospitalization, especially in ICUs, remains a significant concern. The study underscores the importance of vaccination and the timing of COVID-19 exposure in 2022 for enhanced survival in this patient group. Abstract

The effect of sintering temperatures (Ts) on the structural and magnetic properties of Ni0.6Zn0.4Fe2O4 (NZFO) ferrites synthesized by conventional double sintering method has been reported. The samples are sintered at 1200, 1250 and 1300 {\deg}C. The X-ray diffraction (XRD) analysis reveals the formation of a single phase cubic spinel structure of the sample. The magnetic parameters such as saturation magnetization, Ms; coercive field, Hc; remanent magnetization, Mr and Bohr magneton, {\mu}B are determined and well compared with reported values. The obtained values are found to be 71.94 emu/gm and 1.2 Oe for Ms and Hc, respectively at Ts=1300 oC. Curie temperature (Tc) at various Ts has also been calculated. Abstract

Structural properties, impedance, dielectric and electric modulus spectra have been used to investigate the sintering temperature (Ts) effect on the single phase cubic spinel Ni0.6Zn0.4Fe2O4 (NZFO) ceramics synthesized by standard ceramic technique. Enhancement of dielectric constants is observed with increasing Ts. The collective contribution of n-type and p-type carriers yields a clear peak in notable unusual dielectric behavior is successfully explained by the Rezlescu model. The non-Debye type long range dielectric relaxation phenomena is explained by electric modulus formalism. Fast response of the grain boundaries of the sample sintered at lower Ts sample leading to small dielectric spin relaxation time, t (several nanoseconds) have been determined using electric modulus spectra for the samples sintered at different Ts. Two clear semicircles in impedance Cole-Cole plot have also been successfully explained by employing two parallel RC equivalent circuits in series configuration taking into account no electrode contribution. Abstract

A true monolithic infrared photonics platform is within reach if strain and bandgap energy can be independently engineered in SiGeSn semiconductors. Herein, we investigate the structural and optoelectronic properties of a 1.5 μm-thick Si0.06Ge0.90Sn0.04 layer that is nearly lattice-matched to a Ge on Si substrate. Atomic-level studies demonstrate high crystalline quality and uniform composition and show no sign of short-range ordering and clusters. Room-temperature spectroscopic ellipsometry and transmission measurements show direct bandgap absorption at 0.83 eV and a reduced indirect bandgap absorption at lower energies. Si0.06Ge0.90Sn0.04 photoconductive devices operating at room temperature exhibit dark current and spectral responsivity (1 A/W below 1.5 μm wavelengths) similar to Ge on Si devices, with the advantage of a near-infrared bandgap tunable by alloy composition. These results underline the relevance of SiGeSn semiconductors in implementing a group IV material platform for silicon-integrated infrared optoelectronics. [ABSTRACT FROM AUTHOR]

Abstract

COVID-19 is a multisystem disease and sometimes patient can present with life threatening extra-pulmonary manifestations. Amongst GI symptoms nausea, vomiting, abdominal pain and diarrhoea are common. Severe hepatobiliary manifestations like acute pancreatitis, acute hepatitis and cholecystitis are rare in SARS-CoV-2 infection. In this case series we described three cases presented with hepatobiliary manifestations. All of them had acute abdominal pain and finally proven to be the case of acute pancreatitis and acalculous cholecystitis in addition to classical COVID-19 symptoms. Two of them had severe symptoms and required high volume of oxygen, while one case (acalculous cholecystitis) had mild symptom only. All of them recovered successfully. High degree of clinical suspicion is required while managing atypical presentation of COVID-19 cases. This is also vital to refer the cases to advance care hospitals in time to ensure multidisciplinary care. [ABSTRACT FROM AUTHOR]

Abstract

Background: Combating viral hepatitis is part of the UN Sustainable Development Goals (SDGs), and WHO has put forth hepatitis B elimination targets in its Global Health Sector Strategy on Viral Hepatitis (WHO-GHSS) and Interim Guidance for Country Validation of Viral Hepatitis Elimination (WHO Interim Guidance). We estimated the global, regional, and national prevalence of hepatitis B virus (HBV), as well as mortality and disability-adjusted life-years (DALYs) due to HBV, as part of the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019. This included estimates for 194 WHO member states, for which we compared our estimates to WHO elimination targets. Methods: The primary data sources were population-based serosurveys, claims and hospital discharges, cancer registries, vital registration systems, and published case series. We estimated chronic HBV infection and the burden of HBV-related diseases, defined as an aggregate of cirrhosis due to hepatitis B, liver cancer due to hepatitis B, and acute hepatitis B. We used DisMod-MR 2.1, a Bayesian mixed-effects meta-regression tool, to estimate the prevalence of chronic HBV infection, cirrhosis, and aetiological proportions of cirrhosis. We used mortality-to-incidence ratios modelled with spatiotemporal Gaussian process regression to estimate the incidence of liver cancer. We used the Cause of Death Ensemble modelling (CODEm) model, a tool that selects models and covariates on the basis of out-of-sample performance, to estimate mortality due to cirrhosis, liver cancer, and acute hepatitis B. Findings: In 2019, the estimated global, all-age prevalence of chronic HBV infection was 4·1% (95% uncertainty interval [UI] 3·7 to 4·5), corresponding to 316 million (284 to 351) infected people. There was a 31·3% (29·0 to 33·9) decline in all-age prevalence between 1990 and 2019, with a more marked decline of 76·8% (76·2 to 77·5) in prevalence in children younger than 5 years. Abstract

The current global initiative to end Cholera by 2030 emphasizes the use of oral cholera vaccine (OCV) combined with feasible household Water-Sanitation-Hygiene (WASH) interventions. However, little is known about how improved WASH practices and behaviors and OCV interact to reduce the risk of cholera. We reanalyzed two arms of a cluster-randomized trial in urban Bangladesh, to evaluate the effectiveness of OCV given as a 2-dose regimen. One arm (30 clusters, n = 94,675) was randomized to vaccination of persons aged one year and older with OCV, and the other arm (30 clusters, n = 80,056) to no intervention. We evaluated the prevention of cholera by household WASH, classified at baseline using a previously validated rule, and OCV over 2 years of follow-up. When analyzed by assignment to OCV clusters rather than receipt of OCV, in comparison to persons living in "Not Better WASH" households in the control clusters, reduction of severe cholera (the primary outcome) was similar for persons in "Not Better WASH" households in vaccine clusters (46%, 95% CI:24,62), for persons in "Better WASH" households in the control clusters (48%, 95% CI:25,64), and for persons in "Better WASH" households in the vaccine clusters (48%, 95% CI:16,67). In contrast, when analyzed by actual receipt of a complete OCV regimen, , in comparison to persons in "Not Better WASH" households in the control clusters, protection against severe cholera increased steadily from 39% (95% CI:13,58) in residents of "Better WASH" households in the control clusters to 57% (95% CI:35,72) in vaccinated persons in "Not Better WASH" households to 63% (95% CI:21,83) in vaccinated persons in "Better WASH" households. This analysis suggests that improved household WASH and OCV received may interact to provide greater protection against cholera. However, the divergence between findings related to intent to vaccinate versus those pertaining to actual receipt of OCV underscores the need for further research on this topic.

Abstract

This paper represents an approach to speech-to-text conversion in the Bengali language. In this area, we have found most of the methodologies were focused on other languages rather than Bengali. We started with a novel dataset of 56 unique words from 160 individual subjects was prepared. Then in this paper, we illustrate the approach to increasing accuracy in a speech-to-text over the Bengali language where initially we started with Gated Recurrent Unit(GRU) and Long short-term memory (LSTM) algorithms. During further observation, we found that the output of the GRU failed to give any stable output. So, we moved completely to the LSTM algorithm where we achieved 90% accuracy on an unexplored dataset. Voices of several demographic populations and noises were used to validate the model. In the testing phase, we tried a variety of classes based on their length, complexity, noise, and gender variant. Moreover, we expect that this research will help to develop a real-time Bengali speak-to-text recognition model.

Abstract

Objectives: Elderly patients with hematologic malignancies face the highest risk of severe COVID-19 outcomes. The infection's impact on different age groups remains unstudied in detail. Methods: We analyzed elderly patients (age groups: 65-70, 71-75, 76-80, and >80 years old) with hematologic malignancies included in the EPICOVIDEHA registry between January 2020 and July 2022. Univariable and multivariable Cox regression models were conducted to identify factors influencing death in COVID-19 patients with hematological malignancy. Results: The study included data from 3,603 elderly patients (aged 65 or older) with hematological malignancy, with a majority being male (58.1%) and a significant proportion having comorbidities. The patients were divided into four age groups, and the analysis assessed COVID-19 outcomes, vaccination status, and other variables in relation to age and pandemic waves. The 90-day survival rate for patients with COVID-19 was 71.2%, with significant differences between groups. The pandemic waves had varying impacts, with the first wave affecting patients over 80 years old, the second being more severe in 65-70, and the third being the least severe in all age groups. Factors contributing to 90-day mortality included age, comorbidities, lymphopenia, active malignancy, acute leukemia, less than three vaccine doses, severe COVID-19, and using only corticosteroids as treatment. Conclusion: These data underscore the heterogeneity of elderly hematological patients, highlight the different impacts of COVID-19 waves and the pivotal importance of vaccination, and may help in planning future healthcare efforts.
