Abstract

Prenatal micronutrient deficiencies are associated with negative maternal and birth outcomes. Multiple micronutrient supplementation (MMS) during pregnancy is a cost-effective intervention to reduce these adverse outcomes. However, important knowledge gaps remain in the implementation of MMS interventions. The Child Health and Nutrition Research Initiative (CHNRI) methodology was applied to inform the direction of research and investments needed to support the implementation of MMS interventions for pregnant women in low- and middle-income countries (LMIC). Following CHNRI methodology guidelines, a group of international experts in nutrition and maternal health provided and ranked the research questions that most urgently need to be resolved for prenatal MMS interventions to be successfully implemented. Seventy-three research questions were received, analyzed, and reorganized, resulting in 35 consolidated research questions. These were scored against four criteria, yielding a priority ranking where the top 10 research options focused on strategies to increase antenatal care attendance and MMS adherence, methods needed to identify populations more likely to benefit from MMS interventions and some discovery issues (e.g., potential benefit of extending MMS through lactation). This exercise prioritized 35 discrete research questions that merit serious consideration for the potential of MMS during pregnancy to be optimized in LMIC.

8. Intersectional analysis of the experiences of women who fail to conceive in low and middle income neighbourhoods of Delhi, India: Findings from a qualitative study.

Abstract

Background: Experiences of delayed conception and infertility have been reported among women. However, the concept of intersectionality is rarely utilised in studies of infertility, and it is particularly uncommon in research from low- and middle- income countries. Research question: What are the lived experiences of women with delayed conception in low to -middle income neighbourhoods of Delhi, India? Methods: This was a qualitative study (n = 35) that recruited women who had failed to conceive after 18 months of regular unprotected sexual intercourse. Data were collected between February and July 2021. Data were collected through focus group discussions in low income to middle income neighbourhoods of Delhi, India. Analysis identified themes related to intersecting axes of inequality. Results: The results showed that gender intersected with economics, masculinity, patriarchal norms and class to influence the experiences of women. The intersection of gender, economics and patriarchal norms compromised women's agency to be active generators of family income, and this dynamic was exacerbated by patrilocal residence. In addition, masculinity contributed to stigmatisation and blaming of women, due to the inaccurate perception that men did not contribute to a couple's infertility. The intersection of gender and social class in medical settings created barriers to women's access to medical information. Conclusion: Findings from this study provide representative examples of the variety of axes of inequality that shape women's experiences in the study setting. Although these findings may not be generalisable to all women who are experiencing delayed conception, they highlight a need for improved awareness and education on infertility, as well as a need to ensure the availability and accessibility of fertility care for couples in need. Abstract

Key Points: Question: How does a package of multidomain interventions addressing health, nutrition, psychosocial care and support, and environmental hygiene delivered during preconception, pregnancy, and early childhood affect child neurodevelopment at 24 months? Findings: In this second report of a randomized trial in India assessing interventions during preconception, pregnancy, and early childhood on childhood preterm births and childhood growth as primary outcomes, the secondary outcome was neurodevelopment at 24 months. Among 1712 children assessed, preconception, pregnancy, and early childhood interventions were associated with modest improvements in scores and lower incidence of moderate to severe neurodevelopmental delay in the cognitive, language, and socioemotional domains. Meaning: Multidomain interventions in the preconception period, along with those in pregnancy and early childhood, may be beneficial for child neurodevelopment. Importance: Multidomain interventions in pregnancy and early childhood have improved child neurodevelopment, but little is known about the effects of additional preconception interventions. Objective: To evaluate the effect of a multifaceted approach including health; nutrition; water, sanitation, and hygiene (WASH); and psychosocial support interventions delivered during the preconception period and/or during pregnancy and early childhood on child neurodevelopment. Design, Setting, and Participants: In this randomized trial involving low- and middle-income neighborhoods in Delhi, India, 13 500 participants were assigned to preconception interventions or routine care for the primary outcome of preterm births and childhood growth. Participants who became pregnant were randomized to pregnancy and early childhood interventions or routine care. Neurodevelopmental assessments, the trial's secondary outcome reported herein, were conducted in a subsample of children at age 24 months, including 509 with preconception, pregnancy, and early childhood interventions; 473 with preconception interventions alone; 380 with pregnancy and early childhood interventions alone; and 350 with routine care. This study was conducted from November 1, 2000, through February 25, 2022. Interventions: Health, nutrition, psychosocial care and support, and WASH interventions delivered during preconception, pregnancy, and early childhood periods. Main Outcomes and Measures: Cognitive, motor, language, and socioemotional performance at age 24 months, assessed using the Bayley Scales of Infant and Toddler Development 3 tool. Results: The mean age of participants at enrollment was 23.8 years (SD, 3.0 years). Compared with the controls at age 24 months, children in the preconception intervention groups had higher cognitive scores (mean difference [MD], 1.16; 98.3% CI, 0.18-2.13) but had similar language, motor, and socioemotional scores as controls. Those receiving pregnancy and early childhood interventions had higher cognitive (MD, 1.48; 98.3% CI, 0.49-2.46), language (MD, 2.29; 98.3% CI, 1.07-3.50), motor (MD, 1.53; 98.3% CI, 0.65-2.42), and socioemotional scores (MD, 4.15; 98.3% CI, 2.18-6.13) than did controls. The pregnancy and early childhood group also had lower incidence rate ratios (RRs) of moderate to severe delay in cognitive (incidence RR, 0.62; 98.3% CI, 0.40-0.96), language (incidence RR, 0.73; 98.3% CI, 0.57-0.93), and socioemotional (incidence RR, 0.49; 98.3% CI, 0.24-0.97) development than did those in the control group. Children in the preconception, pregnancy, and early childhood intervention group had higher cognitive (MD, 2.60; 98.3% CI, 1.08-4.12), language (MD, 3.46; 98.3% CI, 1.65-5.27), motor (MD, 2.31; 98.3% CI, 0.93-3.69), and socioemotional (MD, 5.55; 98.3% CI, 2.66-8.43) scores than did those in the control group. Conclusions and Relevance: Multidomain interventions during preconception, pregnancy and early childhood led to modest improvements in child neurodevelopment at 24 months. Such interventions for enhancing children's development warrant further evaluation. Abstract

Objective To provide an estimate of the burden of postpartum depression in Indian mothers and investigate some riskfactorsfor the condition. Methods We searched PubMed[R], Google Scholar and Embase[R] databases for articles published from year 2000 up to 31 March 2016 on the prevalence of postpartum depression in Indian mothers. The search used subject headings and keywords with no language restrictions. Quality was assessed via the Newcastle-Ottawa quality assessment scale. We performed the meta-analysis using a random effects model. Subgroup analysis and meta-regression was done for heterogeneity and the Egger test was used to assess publication bias. Findings Thirty-eight studies involving 20043 women were analysed. Studies had a high degree of heterogeneity ([I.sup.2]=96.8%) and there was evidence of publication bias (Egger bias = 2.58; 95% confidence interval, CI: 0.83-4.33). The overall pooled estimate of the prevalence of postpartum depression was 22% (95% CI: 19-25). The pooled prevalence was 19% (95% CI: 17-22) when excluding 8 studies reporting postpartum depression within 2 weeks of delivery. Small, but non-significant differences in pooled prevalence were found by mother's age, geographical location and study setting. Reported risk factors for postpartum depression included financial difficulties, presence of domestic violence, past history of psychiatric illness in mother, marital conflict, lack of support from husband and birth of a female baby. Conclusion The review shows a high prevalence of postpartum depression in Indian mothers. More resources need to be allocated for capacity-building in maternal mental health care in India. Methodes Nous avons recherche dans les bases de donnees PubMed[R], Google Scholar et Embase[R] des articles, publies entre l'annee 2000 et le 31 mars 2016, sur la prevalence de la depression post-partum chez les meres indiennes. Nous avons articule nos recherches autour de vedettes-matiere et de mots-cles, sans restrictions de langues. La qualite a ete evaluee au moyen de l'echelle d'evaluation de la qualite Newcastle-Ottawa. Nous avons realise la meta-analyse a l'aide d'un modele a effets aleatoires. Une analyse par sous-groupes et une meta-regression ont ete effectuees a l'egard de l'heterogeneite et le test Egger a ete utilise pour evaluer le biais de publication. Resultats Trente-huit etudes portant sur 20 043 femmes ont ete analysees. Les etudes presentaient un degre eleve d'heterogeneite (I2=96,8%) et l'existence de biais de publication a ete demontree (bais Egger = 2,58; intervalle de confiance, IC, a 95%: 0,83-4,33). L'estimation combinee globale de la prevalence de la depression post-partum etait de 22% (IC a 95%: 19-25). Apres exclusion de 8 etudes rendant compte de depressions post-partum dans les 2 semaines suivant l'accouchement, la prevalence combinee a ete estimee a 19% (IC a 95%: 17-22). Quelques petites differences negligeables au niveau de la prevalence combinee ont ete constatees selon l'age de la mere, la situation geographique et le cadre de l'etude. Les facteurs de risques associes a la depression post-partum qui ont ete Identifies incluaient des difficultes financieres, la presence de violence domestique, des antecedents de maladie psychiatrique chez la mere, des conflits conjugaux, une absence de soutien de la part du mari et la naissance d'une fille. Conclusion La revue a revele une prevalence elevee de la depression post-partum chez les meres indiennes. Il est necessaire d'allouer davantage de ressources au renforcement des capacites en ce qui concerne les soins de sante mentale destines aux meres indiennes. Objetivo Ofrecer una estimacion de la carga de la depresion posparto en madres indias e investigar algunos factores de riesgo de la enfermedad. Metodos Se realizaron busquedas en las bases de datos de PubMed[R], Google Scholar y Embase[R] para encontrar articulos publicados desde el ano 2000 hasta el 31 de marzo de 2016 sobre la prevalencia de la depresion postparto en madres indias. En la busqueda se utilizaron epigrafes tematicos y palabras clave sin restricciones de lenguaje. La calidad se evaluo con la escala de evaluacion de calidad de NewcastleOttawa. Se realizo un metaanalisis utilizando un modelo de efectos aleatorios. El analisis y la metarregresion de los subgrupos se realizaron con fines de heterogeneidad y se utilizo la prueba de Egger para evaluar las tendencias de las publicaciones. Resultados Se analizaron treinta y ocho estudios que incluian 20043 mujeres. Los estudios tuvieron un alto grado de heterogeneidad (I2=96,8%) y se encontraron pruebas de tendencias de publicaciones (tendencia de Egger =2,58; intervalo de confianza, CI, del 95%: 0,83-4,33). La estimacion general calculada sobre la prevalencia de la depresion posparto fue del 22% (IC del 95%: 19-25). La prevalencia obtenida fue del 19% (IC del 95%: 17-22), salvo en 8 estudios que informaron de depresion posparto dentro de las 2 primeras semanas despues del parto. Se descubrieron pequenas diferencias con poca importancia en la prevalencia obtenida segun la edad de la madre, la ubicacion geografica y el marco del estudio. Los factores de riesgo descubiertos sobre la depresion posparto incluian dificultades financieras, violencia domestica, historial pasado de enfermedad psiquiatrica, conflicto marital, ausencia de apoyo por parte del marido y nacimiento de una nina. Conclusion El analisis muestra una alta prevalencia de depresion posparto en madres indias. Abstract

Undernourished pregnant and lactating women from food insecure settings, most commonly found in low- and middle-income countries, are at risk of adverse maternal and infant health outcomes. Often, undernourishment is characterized by low body mass index (BMI), low mid upper arm circumference (MUAC), short stature, and/or micronutrient deficiencies, which put pregnant women at risk for complications such intrauterine growth restriction and preterm birth (Kozuki et al. 2015). The World Health Organization recommends balanced energy-protein (BEP) supplementation in populations at risk of undernourishment (defined as more than 20% of women with a BMI less than 18.5 kg/m2) to reduce the risk of stillbirth and small for gestational age neonates (World Health Organization 2016). The consequences of undernourishment among lactating mothers have commonly been overlooked. Lactating mothers require additional calories to produce quality and quantity milk, which in turn confers health and development benefits to the infant (Nommsen et al. 1991; Dewey et al. 1991; American College of Obstetricians and Gynecologists 2016). There is a high prevalence of malnutrition in mothers and children from low- and middle-income countries, and research from randomized controlled trials is needed to clarify if BEP could be a valuable intervention during lactation. BEP supplementation products are ready-to-eat or prepared foods that provide protein (accounts for less than 25% of the total energy content) and energy. When given during pregnancy, BEP products are fortified with micronutrients or an additional micronutrient supplement is given (typically multiple micronutrients or iron-folic acid). The current evidence coming from systematic reviews and meta-analyses has focused on studies where BEP is given during pregnancy (Imdad and Bhutta 2012; Ota et al. 2015; Stevens et al. 2015; Lassi et al. 2020; Perumal et al. 2021) but not lactation. This evidence is difficult to disentangle because there is significant heterogeneity in the populations, comparison/control groups, study design, and BEP product formulations (i.e., energy and protein content). Given these limitations, in 2017, an expert panel organized by the Bill and Melinda Gates Foundation (BMGF) created guidelines for the macro- and micronutrient content of BEP supplements (Report of an Expert Consultation held at the Bill & Melinda Gates Foundation 2017). This group also suggested that these BEP products should be developed and tested on pregnant and lactating women in low-resource settings. A number of studies (all randomized interventions) were subsequently funded by BMGF. As these studies’ investigators were developing protocols independently, in 2020 BMGF convened the Maternal Nutrition Harmonization Workshop to harmonize key variables across these trials. This effort was expanded, and the Maternal BEP Studies Harmonization Initiative was formed to examine the pooled effect of BEP across pregnancy and lactation. The current protocol describes our prospective individual participant data (IPD) meta-analysis that focuses on BEP given during lactation. Subsequent work will examine BEP given during pregnancy. Given the high prevalence of infant stunting in low- and middle-income countries (World Health Organization 2018b; Local Burden of Disease Child Growth Failure Collaborators 2020), and the importance of supporting the nutrient intake of mothers providing breastmilk to infants, this IPD meta-analysis will focus on assessing the effect of BEP given to mothers during lactation on infant length-for-age z scores (LAZ) at 6 months of age. We will also examine additional infant and maternal anthropometric and health outcomes.

Abstract

Folate and vitamin B12 are essential for growth. Our objective was to estimate their long-term effects on linear growth in North Indian children. This is a follow-up study of a factorial designed, double-blind, randomised, placebo-controlled trial in 1000 young children. Starting at 6–30 months of age, we gave folic acid (approximately 2 RDA), vitamin B12 (approximately 2 RDA), both vitamins or a placebo daily for 6 months. Six years after the end of supplementation, we measured height in 791 children. We used the plasma concentrations of cobalamin, folate and total homocysteine to estimate vitamin status. The effect of the interventions, the association between height-for-age z-scores (HAZ) and baseline vitamin status, and the interactions between supplementation and baseline status were estimated in multiple regression models. Mean (sd) age at follow-up was 7·4 (0·7) years (range 6 to 9 years). There was a small, non-significant effect of vitamin B12 on linear growth and no effect of folic acid. We observed a subgroup effect of vitamin B12 supplementation in those with plasma cobalamin concentration < 200 pmol/l (P for interaction = 0·01). The effect of vitamin B12 supplementation in this group was 0·34 HAZ (95 % CI 0·11, 0·58). We found an association between cobalamin status and HAZ in children not given vitamin B12 (P for interaction = 0·001). In this group, each doubling of the cobalamin concentration was associated with 0·26 (95 % CI 0·15, 0·38) higher HAZ.
