Abstract

Background: Currently, there is no clearly established therapy to treat mild cognitive impairment (MCI); consequently, alternative therapies, such as acupuncture, have been attempted. In many clinical studies, the potential benefits of acupuncture for cognitive improvement have been identified in clinical outcomes; however, the mechanism remains unclear. Accordingly, this study aims to investigate the therapeutic mechanism of acupuncture therapy using functional near-infrared spectroscopy and its feasibility in treating individuals with impaired cognitive function. Methods: This study is designed to be a prospective, two-arm, parallel clinical trial involving 24 participants. The patient group will be treated with acupuncture twice per week for 12 weeks; meanwhile, the healthy control group will not undergo acupuncture treatment. Functional near-infrared spectroscopy assessment and a working memory test will be performed at baseline and every 6 weeks to investigate the therapeutic mechanism of acupuncture. The primary outcome will be measured using the Korean version of the Montreal Cognitive Assessment. The secondary outcomes will be the Alzheimer's Disease Assessment Scale-cognitive subscale score, working memory task accuracy, response rate, response time, and hemodynamic response of the prefrontal lobe. The outcomes will be evaluated at baseline, and at 6 and 12 weeks after subject allocation. Discussion: This clinical pilot trial is designed to determine the feasibility of acupuncture as an effective and safe treatment for improving cognitive function in patients with MCI. 13. Direct Plan Comparison of RapidArc and CyberKnife for Spine Stereotactic Body Radiation Therapy

Abstract

We compared the treatment planning performance of RapidArc (RA) vs. CyberKnife (CK) for spinal stereotactic body radiation therapy (SBRT). Ten patients with spinal lesions who had been treated with CK were re-planned with RA, which consisted of two complete arcs. Computed tomography (CT) and volumetric dose data of CK, generated using the Multiplan (Accuray) treatment planning system (TPS) and the Ray-Trace algorithm, were imported to Varian Eclipse TPS in Dicom format, and the data were compared with the RA plan using analytical anisotropic algorithm (AAA) dose calculation. The optimized dose priorities for both CK and RA plans were similar for all patients. The highest priority was to provide enough dose coverage to the planned target volume (PTV) while limiting the maximum dose to the spinal cord. Plan quality was evaluated with respect to PTV coverage, conformity index (CI), high-dose spillage, intermediate-dose spillage (R50% and D2cm), and maximum dose to the spinal cord, which are criteria recommended by RTOG 0631 spine and 0915 lung SBRT protocols. The mean CI +/- SD values of PTV were 1.11 +/- 0.03 and 1.17 +/- 0.10 for RA and CK (p = 0.02), respectively. On average, the maximum dose delivered to the spinal cord in CK plans was approximately 11.6% higher than in RA plans, and this difference was statistically significant (p < 0.001). High-dose spillages were 0.86% and 2.26% for RA and CK (p = 0.203), respectively. Intermediate-dose spillage characterized by D2cm was lower for RA than for CK; however, R50% was not statistically different. Even though both systems can create highly conformal volumetric dose distributions, the current study shows that RA demonstrates lower high- and intermediate-dose spillage than CK. Therefore, RA plans for spinal SBRT may be superior to CK.

Abstract

Cyclin-dependent kinase 4 and 6 (CDK4/6) inhibitors, including abemaciclib, have been approved for the treatment of hormone receptor-positive, human epidermal growth factor receptor 2 (HER2)-negative advanced, and metastatic breast cancer. Despite the high therapeutic efficacy of CDK4/6 inhibitors, they are associated with various adverse effects, including potentially fatal interstitial lung disease. Therefore, a combination of CDK4/6 inhibitors with letrozole or fulvestrant has been attempted but has demonstrated limitations in reducing adverse effects, highlighting the need to develop new combination therapies. This study proposes a combination strategy using CDK4/6 inhibitors and tricyclic antidepressants to enhance the therapeutic outcomes of these inhibitors while reducing their side effects. The therapeutic efficacies of abemaciclib and desipramine were tested in different cancer cell lines (H460, MCF7, and HCT-116). The antitumor effects of the combined abemaciclib and desipramine treatment were evaluated in a xenograft colon tumor model. In vitro cell studies have shown the synergistic anticancer effects of combination therapy in the HCT-116 cell line. The combination treatment significantly reduced tumor size compared with control or single treatment without causing apparent toxicity to normal tissues. Although additional in vivo studies are necessary, this study suggests that the combination therapy of abemaciclib and desipramine may represent a novel therapeutic approach for treating solid tumors. This pilot study aimed to assess the feasibility of pharmacopuncture therapy adjuvant to acupuncture treatment for rotator cuff disease. Patients and Methods: This was a parallel-grouped, pragmatic randomized controlled, pilot study. Forty patients were randomly allocated to either the experimental or the control group. All patients received acupuncture treatment for four weeks, and pharmacopuncture was additionally administered to the experimental group. Results: Both groups showed that each treatment effectively improved rotator cuff disease in most assessments. Particularly, the group that received acupuncture plus pharmacopuncture required fewer rescue medications than the group that received acupuncture alone. However, there was little statistically significant difference between the two groups. There were no serious adverse events experienced by patients in this study. Conclusion: Although there was little statistical difference between the two groups, the combination of acupuncture and pharmacopuncture for rotator cuff disease was associated with a reduction in the rescue medicine dosage compared with acupuncture alone. Also, it confirmed the safety of pharmacopuncture therapy. This pilot study would help design future research on the effectiveness of pharmacopuncture in rotator cuff disease. [ABSTRACT FROM AUTHOR]

Abstract

Given a measured geodesic lamination on a hyperbolic surface, grafting the surface along multiples of the lamination defines a path in Teichmuller space, called the grafting ray. We show that every grafting ray, after reparametrization, is a Teichmuller quasi-geodesic and stays in a bounded neighborhood of a Teichmuller geodesic. As part of our approach, we show that grafting rays have controlled dependence on the starting point. That is, for any measured geodesic lamination Lambda, the map of Teichmuller space which is defined by grafting along Lambda is L-Lipschitz with respect to the Teichmuller metric, where L is a universal constant. Abstract

For two measured laminations $\nu^+$ and $\nu^-$ that fill up a hyperbolizable surface $S$ and for $t \in (-\infty, \infty)$, let $L_t$ be the unique hyperbolic surface that minimizes the length function $e^t l(\nu^+) + e^{-t} l(\nu^-)$ on Teichmuller space. We characterize the curves that are short in $L_t$ and estimate their lengths. We find that the short curves coincide with the curves that are short in the surface $G_t$ on the Teichmuller geodesic whose horizontal and vertical foliations are respectively, $e^t \nu^+$ and $e^{-t} \nu^-$. By deriving additional information about the twists of $\nu^+$ and $\nu^-$ around the short curves, we estimate the Teichmuller distance between $L_t$ and $G_t$. We study the Lipschitz metric on Teichmuller space (defined by Thurston) and compare it with the Teichmuller metric. We show that in the thin part of Teichmuller space the Lipschitz metric is approximated up to bounded additive distortion by the sup metric on a product of lower-dimensional spaces (similar to the Teichmuller metric as shown by Minsky), and in the thick part, the two metrics are comparable within additive error. Pharmacopuncture has been widely used to treat LBP in Korea; however, randomized clinical trials (RCT) or active control have not been conducted to evaluate its effectiveness.
