Abstract

We sought (i) to validate a new prediction rule of mortality (Progetto Nazionale Emorragia Digestiva (PNED) score) on an independent population with non-variceal upper gastrointestinal bleeding (UGIB) and (ii) to compare the accuracy of the Italian PNED score vs. the Rockall score in predicting the risk of death.We conducted prospective validation of analysis of consecutive patients with UGIB at 21 hospitals from 2007 to 2008. Outcome measure was 30-day mortality. All the variables used to calculate the Rockall score as well as those identified in the Italian predictive model were considered. Calibration of the model was tested using the chi2 goodness-of-fit and performance characteristics with receiver operating characteristic (ROC) analysis. The area under the ROC curve (AUC) was used to quantify the diagnostic accuracy of the two predictive models.Over a 16-month period, data on 1,360 patients were entered in a national database and analyzed. Peptic ulcer bleeding was recorded in 60.7% of cases. One or more comorbidities were present in 66% of patients. Endoscopic treatment was delivered in all high-risk patients followed by high-dose intravenous proton pump inhibitor in 95% of them. Sixty-six patients died (mortality 4.85%; 3.54-5.75). The PNED score showed a high discriminant capability and was significantly superior to the Rockall score in predicting the risk of death (AUC 0.81 (0.72-0.90) vs. 0.66 (0.60-0.72), P0.000). Positive likelihood ratio for mortality in patients with a PNED risk score8 was 16.05.The Italian 10-point score for the prediction of death was successfully validated in this independent population of patients with non-variceal gastrointestinal bleeding. The PNED score is accurate and superior to the Rockall score. Further external validation at the international level is needed.

15. Lead anchors and metal ingots from the Arburese coast of Sardinia (Italy)

Abstract

During a series of nautical surveys, lead stocks of wooden-lead Roman anchors and metal ingots have been found along the Arburese coast (south-western Sardinia, Italy) very close to Montevecchio mine basin. because the Montevecchio basin is characterised by the presence of a large metal ore deposits with lead, tin, copper, silver, zinc, cadmium, nickel and iron ores and classical authors indicate that the metal ore outcrops of these area have been surely exploited since the Phoenician-Punic period, the lead anchors and metal ingots have been analysed for determining their chemical composition, microchemical structure and provenance via lead isotope analysis. On the base of the results it is possible to identify the chemical nature and some microchemical features of the ingots and of lead anchors. Concerning the provenance of these materials, on the base of lead isotope analysis results, it is possible to propose only for some lead ingots and anchors two production centres that are the lead mines of the Cartagena (Spain) and the Montevecchio mine basin, respectively.

Abstract

Differential thermal analysis (DTA) and thermogravimetry (TG), X-ray photoelectron spectroscopy (XPS), and scanning electron microscopy (SEM) and energy dispersive spectrometry (EDS) were used to study the thermal decomposition of calcium sulphate in the CaSO4 (25 wt%)-bonded silica investment which is the commonly used material for casting jewellery gold-based alloys. The thermal decomposition of CaSO4 generates sulphur dioxide, leads to gas porosity in the molten gold-based alloys and therefore, defective jewellery products. This latter reaction was studied as a function of the temperature and atmosphere (air, argon and argon-5% hydrogen). in order to simulate, as for as possible, different casting conditions used by the jewellery industry. Furthermore, special attention was given to the effect of the presence of Zn, Cu2O. CuO and Ag2O on the thermal decomposition of CaSO4. DTG-TG results confirmed that the temperature of the thermal decomposition of CaSO4 bonded with silica was lower with respect to the nearly pure CaSO4. Unfortunately, it was very close to the casting temperature of some typical gold alloys. in addition, the temperature of the decomposition was further lowered when inert and reducing atmospheres are used, as well as in the presence of ZnO, Cu2O. Abstract

Objectives: From an Italian Registry of patients with upper gastrointestinal hemorrhage (UGIH), we assessed the clinical outcomes and explored the roles of clinical, endoscopic, and therapeutic factors on 30-day mortality in a real life setting., Methods: Prospective analysis of consecutive patients endoscoped for UGIH at 23 community and tertiary care institutions from 2003 to 2004. Covariates and outcomes were defined a priori and 30-day follow-up obtained. Logistic regression analysis identified predictors of mortality., Results: One thousand and twenty patients were included. A total of 46 patients died for an overall 4.5% mortality rate. In all, 85% of deaths were associated with one or more major comorbidity. Sixteen of 46 patients (35%) died within the first 24 h of the onset of bleeding. Of these, eight had been categorized as ASA class 1 or 2 and none of them was operated upon, despite a failure of endoscopic intention to treatment in four. Regression analysis showed advanced age, presence of severe comorbidity, low hemoglobin levels at presentation, and worsening health status as the only independent predictors of 30-day mortality (P < 0.001). The acute use of a PPI exerted a protective effect (OR 0.23, 95% CI 0.09-0.73). Recurrent bleeding was low (3.2%). Rebleeders accounted for only 11% of the total patients deceased (OR 3.27, 95% CI 1.5-11.2)., Conclusions: These results indicate that 30-day mortality for nonvariceal bleeding is low. Deaths occurred predominantly in elderly patients with severe comorbidities or those with failure of endoscopic intention to treatment.

Abstract

The cause of gastric ulcer is still poorly understood, even though a certain number of pathophysiologic abnormalities have been described in subgroups of gastric ulcer patients. It is not known whether these abnormalities lead to ulceration or occur as a result of ulcers. Environmental factors, such as aspirin intake or stressful life events, may be related to gastric ulcer in some patients. The main pathophysiologic events (dismotility, prostaglandin formation, alteration in gastric mucosal barrier and blood flow, impaired gastric secretions), and the environmental factors involved in the development of gastric ulcer (cigarette smoking, alcohol consumption, drug ingestion, psychologic stress, infectious agents) are reviewed in this paper. Knowledge about the natural history of gastric ulcer has been increased by the availability of fiberoptic endoscopy and by controlled clinical trials, but ulcer pathogenesis still remains obscure, and perhaps multifactorial.

Abstract

Prevention of ulcer relapse and of its complications is a problem which remains to be solved. Our study involved 250 patients, with healed duodenal ulcer. We evaluated efficacy and costs of three different maintenance therapies: ranitidine 150 mg/day, omeprazole 20 mg/day every other day and omeprazole 20 mg/day. Six months later, we found the incidence of relapse to be 24.4% (32/131) in the once-a-day ranitidine group, 19.7% (13/66) in the day every-other-day omeprazole group, and 3.8% (2/53) in the once-a-day omeprazole group. Further, we evaluated costs relative to relapsing patients, and total costs for each treatment group. From these data, we conclude that personalized maintenance therapy with omeprazole is the most cost-effective: a dosage of 20 mg/day is extremely effective in maintaining remission, and is therefore most indicated in patients at risk; omeprazole 20 mg/day every-other-day affords better compliance, lower costs and fewer relapses with respect to standard H2-antagonist dosages.

Abstract

Seventeen cases of Infective Endocarditis diagnosed over a seven-year period by clinical-echocardiographic criteria in a Division of General Medicine are reviewed. More significant aspects regard the observation of the disease in some subjects at risk (elderly persons, patients with normal native valves affected by cirrhosis, by intravenous drugs abuse, by haematological neoplasms), the diagnostic difficulties in cases with oligosymptomatic or atypical beginning, and the frequent negative blood cultures. The value and the limitations of echocardiography and some aspects of therapy are discussed.
