Abstract

Aims: Patients with left ventricular assist device (LVAD) suffer from persistent exercise limitation despite improvement of their heart failure syndrome. Exercise training (ET) programmes to improve aerobic capacity have shown modest efficacy. High‐intensity interval training (HIIT), as an alternative to moderate continuous training, has not been systematically tested in this population. We examine the feasibility of a short, personalized HIIT programme in patients with LVAD and describe its effects on aerobic capacity and left ventricular remodelling. Methods and results: Patients on durable LVAD support were prospectively enrolled in a 15‐session, 5 week HIIT programme. Turndown echocardiogram, Kansas City Cardiomyopathy Questionnaire, and cardiopulmonary exercise test were performed before and after HIIT. Training workloads for each subject were based on pretraining peak cardiopulmonary exercise test work rate (W). Percentage of prescribed training workload completed and adverse events were recorded for each subject. Fifteen subjects were enrolled [10 men, age = 51 (29–71) years, HeartMate II = 12, HeartMate 3 = 3, and time on LVAD = 18 (3–64) months]. Twelve completed post‐training testing. HIIT was well tolerated, and 90% (inter‐quartile range: 78, 99%) of the prescribed workload (W) was completed with no major adverse events. Improvements were seen in aV̇O2 at ventilatory threshold [7.1 (6.5, 9.1) to 8.5 (7.7, 9.3) mL/kg/min, P = 0.04], work rate at ventilatory threshold [44 (14, 54) to 55 (21, 66) W, P = 0.05], and left ventricular end‐diastolic volume [168 (144, 216) to 159 (124, 212) mL, n = 7, P = 0.02]. HIIT had no effect on maximal oxygen consumption (V̇O2peak) or Kansas City Cardiomyopathy Questionnaire score. Conclusions: Cardiopulmonary exercise test‐guided HIIT is feasible and can improve submaximal aerobic capacity in stable patients with chronic LVAD support. Further studies are needed on its effects on the myocardium and its potential role in cardiac rehabilitation programmes. [ABSTRACT FROM AUTHOR]

Abstract

Introduction:Gastrointestinal bleeding (GIB) in LVAD patients is most commonly caused by gastrointestinal angiodysplasia (GIAD). Non-pulsatile blood flow in LVAD patients can cause splanchnic mucosal hypoperfusion and hypoxia leading to activation of a Hypoxia Inducible Factor (HIF)1-? /Vascular Endothelial Growth factor (VEGF)/Angiopoietins (Ang) signaling cascade, that can trigger pathological angiogenesis. We have recently shown that the use of digoxin, an inhibitor of HIF1-?, is associated with a significant reduction in GIAD related GIB in LVAD patients.Hypothesis:We hypothesize that GIAD development driven by dysregulation of VEGF, Ang-1, and Ang-2 expression in patients with LVADs can be corrected by digoxin treatment.Methods:Blood samples were collected from 13 patients 1 week before and 6 weeks after LVAD implant (3 HeartMate (HM)2, 9 HM3), and from 12 patients supported with LVAD (5 HM2 and 7 HM3 devices) before and after 2 weeks of digoxin therapy (0.125 ?g daily). VEGF, Ang-1, and Ang-2 levels were measured in patients? serum by ELISA. Human microvascular endothelial cells (HMVEC) were incubated under hypoxic (1% O2) or normoxic conditions (21% O2), with or without pretreatment with digoxin (1nM, 10nM, and 100nM). Change in HIF1-? levels was assessed by Western blot and secreted VEGF, and Ang-2 levels were measured in cell culture media by ELISA.Results:In vivo studies: VEGF (114.8 to 389.1 ng/ml, p=0.044) and Ang-2 levels increased (6.8 to 10.4 ng/ml, p=0.016), and Ang-1 levels decreased (16.2 to 10.4 ng/ml, p=0.047) after pump implantation, indicating the development of angiogenic dysregulation. Digoxin treatment in LVAD patients decreased VEGF (405.0 to 272.8 ng/ml, p=0.036) and Ang-2 (14.2 to 8.5 ng/ml, p=0.007), and increased Ang-1 levels (10.6 to 26.4 ng/ml, p=0.003), reversing angiogenic switch. In vitrostudies: Digoxin prevented, in a dose-dependent fashion, the hypoxia-induced increase in HIF-1? levels, VEGF (169.7 vs 96 pg/ml, p=0.032) and Ang-2 (63.5 vs 29 ng/ml p=0.021) secretion in HMVECs.Conclusions:Digoxin by inhibiting hypoxia triggered HIF-1? signaling cascade may reverse angiogenic switch in patients supported by LVADs. The relevance of this finding for the prevention of GIB needs to be prospectively studied.
