Abstract

Schizotypy, characterised as a cluster of personality traits, provides a unifying framework for understanding risk, aetiology, developmental trajectory, and prognosis for psychopathology. Besides reflecting a latent liability for psychosis, schizotypy also allows investigation of individual differences beyond clinical settings. Impaired social functioning has been well-documented as a feature of psychiatric disorders and is posited to present before illness onset, remain stable over illness course, and persist long after symptomatic remission. Despite the transdiagnostic nature of loneliness and its likely frequent co-occurrence with schizotypal traits, the role of loneliness and poor social functioning in schizotypy research has been scarce. The broad aim of this thesis is to elucidate the underlying mechanisms of social functioning in individuals with schizotypy, social anhedonia and psychotic-like experiences (PLEs). One of the more specific aims was the examination of the role of facial expressions of emotion, loneliness, and social support in their relationships with schizotypy.

Abstract

NBEAL2 encodes a multidomain scaffolding protein with a putative role in granule ontogeny in human platelets. Mutations in NBEAL2 underlie gray platelet syndrome (GPS), a rare inherited bleeding disorder characterized by a lack of α-granules within blood platelets and progressive bone marrow fibrosis. We present here a novel Nbeal2(-/-) murine model of GPS and demonstrate that the lack of α-granules is due to their loss from platelets/mature megakaryocytes (MKs), and not by initial impaired formation. We show that the lack of Nbeal2 confers a proinflammatory phenotype to the bone marrow MKs, which in combination with the loss of proteins from α-granules drives the development of bone marrow fibrosis. In addition, we demonstrate that α-granule deficiency impairs platelet function beyond their purely hemostatic role and that Nbeal2 deficiency has a protective effect against cancer metastasis.

Abstract

Background.The overall improvement in treatment outcomes and survival of multiple myeloma (MM) patients has highlighted the persistently poor outlook for patients with del(17p). Around a third of MM patients with del(17p) are also reported to have a TP53mutation, while this mutation is rare in the absence of del(17p). The prognostic impact of TP53mutations in the context of del(17p) is unresolved. We investigated the presence of TP53mutations in a cohort of del(17p) patients to correlate mutation status with clinical outcome. We assessed the types of TP53mutations, and the timing of mutations in relation to the acquisition of del(17p).

Abstract

Purpose: Immune dysregulation is described in multiple myeloma. While preclinical models suggest a role for altered T-cell immunity in disease progression, the contribution of immune dysfunction to clinical outcomes remains unclear. We aimed to characterize marrow-infiltrating T cells in newly diagnosed patients and explore associations with outcomes of first-line therapy., Experimental Design: We undertook detailed characterization of T cells from bone marrow (BM) samples, focusing on immune checkpoints and features of immune dysfunction, correlating with clinical features and progression-free survival., Results: We found that patients with multiple myeloma had greater abundance of BM regulatory T cells (Tregs) which, in turn, expressed higher levels of the activation marker CD25 compared with healthy donors. Patients with higher frequencies of Tregs had shorter PFS and a distinct Treg immune checkpoint profile (increased PD-1, LAG-3) compared with patients with lower frequencies of Tregs. Analysis of CD4 and CD8 effectors revealed that low CD4effector (CD4 eff ):Treg ratio and increased frequency of PD-1-expressing CD4 eff cells were independent predictors of early relapse over and above conventional risk factors, such as genetic risk and depth of response. Ex vivo functional analysis and RNA sequencing revealed that CD4 and CD8 cells from patients with greater abundance of CD4 eff PD-1 + cells displayed transcriptional and secretory features of dysfunction., Conclusions: BM-infiltrating T-cell subsets, specifically Tregs and PD-1-expressing CD4 effectors, negatively influence clinical outcomes in newly diagnosed patients. Pending confirmation in larger cohorts and further mechanistic work, these immune parameters may inform new risk models, and present potential targets for immunotherapeutic strategies., (©2020 American Association for Cancer Research.)
