5. Asthma inflammatory phenotypes on four continents: most asthma is non-eosinophilic

- Abstract
BACKGROUND: Most studies assessing pathophysiological heterogeneity in asthma have been conducted in high-income countries (HICs), with little known about the prevalence and characteristics of different asthma inflammatory phenotypes in low-and middle-income countries (LMICs). This study assessed sputum inflammatory phenotypes in five centres, in Brazil, Ecuador, Uganda, New Zealand (NZ) and the United Kingdom (UK). METHODS: We conducted a cross-sectional study of 998 asthmatics and 356 non-asthmatics in 2016-20. All centres studied children and adolescents (age range 8-20 years), except the UK centre which involved 26-27 year-olds. Information was collected using questionnaires, clinical characterization, blood and induced sputum. RESULTS: Of 623 asthmatics with sputum results, 39% (243) were classified as eosinophilic or mixed granulocytic, i.e. eosinophilic asthma (EA). Adjusted for age and sex, with NZ as baseline, the UK showed similar odds of EA (odds ratio 1.04, 95% confidence interval 0.37-2.94) with lower odds in the LMICs: Brazil (0.73, 0.42-1.27), Ecuador (0.40, 0.24-0.66) and Uganda (0.62, 0.37-1.04). Despite the low prevalence of neutrophilic asthma in most centres, sputum neutrophilia was increased in asthmatics and non-asthmatics in Uganda. CONCLUSIONS: This is the first time that sputum induction has been used to compare asthma inflammatory phenotypes in HICs and LMICs. Most cases were non-eosinophilic, including in settings where corticosteroid use was low. A lower prevalence of EA was observed in the LMICs than in the HICs. This has major implications for asthma prevention and management, and suggests that novel prevention strategies and therapies specifically targeting non-eosinophilic asthma are required globally.

8. Food frequency questionnaire for children in rural coastal zones of Ecuador: Development, relative validity and calibration

- Abstract
Food frequency questionnaires are research tools widely used at the population level to assess food intake, especially in childhood. The objectives of this study were to develop and determine the relative validity of the Food Frequency Questionnaire (FFQ) for the evaluation of food intake among Ecuadorian schoolchildren. One hundred children from rural coastal areas of Ecuador, aged 6 to 8 years, were included. Mothers completed 1 FFQ frequency and two 24-hour recalls (R24h gold standard). Pearson correlation, weighted Kappa and the Bland Altman method were used to assess validity. Linear regression models were used including R24h as a dependent variable and FFQ as an independent variable to derive calibration factors. The Pearson coefficients ranged between -0.03 and 0.24. The Kappa values varied between -0.05 and 0.24, the Bland-Altman plots were between the limits near the midline and the calibration factors ranged from -0.01 to 0.16. The FFQ was considered appropriate to evaluate energy and nutrient consumption, however, due to its weak to moderate agreement this must be corrected using calibration values. This FFQ is a useful tool for evaluating diet and the relationship between diet and disease in this population group.

Asthma prevalence in children varies substantially around the world, but the contribution of known risk factors to this international variation is uncertain. The International Study of Asthma and Allergies in Childhood (ISAAC) Phase Two studied 8-12 year old children in 30 centres worldwide with parent-completed symptom and risk factor questionnaires and aeroallergen skin prick testing. We used multilevel logistic regression modelling to investigate the effect of adjustment for individual and ecological risk factors on the between-centre variation in prevalence of recent wheeze. Adjustment for single individual-level risk factors changed the centre-level variation from a reduction of up to 8.4% (and 8.5% for atopy) to an increase of up to 6.8%. Modelling the 11 most influential environmental factors among all children simultaneously, the centre-level variation changed little overall (2.4% increase). Modelling only factors that decreased the variance, the 6 most influential factors (synthetic and feather quilt, mother's smoking, heating stoves, dampness and foam pillows) in combination resulted in a 21% reduction in variance. Ecological (centre-level) risk factors generally explained higher proportions of the variation than did individual risk factors. Single environmental factors and aeroallergen sensitisation measured at the individual (child) level did not explain much of the between-centre variation in wheeze prevalence.

Objectives: Incomplete or ambiguous evidence for identifying high-risk patients with acute respiratory distress syndrome for enrollment into randomized controlled trials has come at the cost of an unreasonable number of negative trials. We examined a set of selected variables early in acute respiratory distress syndrome to determine accurate prognostic predictors for selecting high-risk patients for randomized controlled trials. Design: A training and testing study using a secondary analysis of data from four prospective, multicenter, observational studies. Setting: A network of multidisciplinary ICUs. Patients: We studied 1,200 patients with moderate-to-severe acute respiratory distress syndrome managed with lung-protective ventilation. Interventions: None. Measurements and Main Results: We evaluated different thresholds for patient's age, Pao 2 /Fio 2, plateau pressure, and number of extrapulmonary organ failures to predict ICU outcome at 24 hours of acute respiratory distress syndrome diagnosis. We generated 1,000 random scenarios as training (n = 900, 75% of population) and testing (n = 300, 25% of population) datasets and averaged the logistic coefficients for each scenario. Thresholds for age (< 50, 50-70, > 70 yr), Pao 2 /Fio 2 (= 100, 101-150, > 150 mm Hg), plateau pressure (< 29, 29-30, > 30 cm H2O), and number of extrapulmonary organ failure (< 2, 2, > 2) stratified accurately acute respiratory distress syndrome patients into categories of risk. The model that included all four variables proved best to identify patients with the highest or lowest risk of death (area under the receiver operating characteristic curve, 0.86; 95% CI, 0.84-0.88). Decision tree analyses confirmed the accuracy and robustness of this enrichment model. Conclusions: Combined thresholds for patient's age, Pao 2 / Fio 2, plateau pressure, and extrapulmonary organ failure provides prognostic enrichment accuracy for stratifying and selecting acute respiratory distress syndrome patients for randomized controlled trials.

- Abstract
Asthma prevalence in children varies substantially around the world, but the contribution of known risk factors to this international variation is uncertain. The International Study of Asthma and Allergies in Childhood (ISAAC) Phase Two studied 8–12 year old children in 30 centres worldwide with parent-completed symptom and risk factor questionnaires and aeroallergen skin prick testing. We used multilevel logistic regression modelling to investigate the effect of adjustment for individual and ecological risk factors on the between-centre variation in prevalence of recent wheeze. Adjustment for single individual-level risk factors changed the centre-level variation from a reduction of up to 8.4% (and 8.5% for atopy) to an increase of up to 6.8%. Modelling the 11 most influential environmental factors among all children simultaneously, the centre-level variation changed little overall (2.4% increase). Modelling only factors that decreased the variance, the 6 most influential factors (synthetic and feather quilt, mother’s smoking, heating stoves, dampness and foam pillows) in combination resulted in a 21% reduction in variance. Ecological (centre-level) risk factors generally explained higher proportions of the variation than did individual risk factors. Single environmental factors and aeroallergen sensitisation measured at the individual (child) level did not explain much of the between-centre variation in wheeze prevalence.

Study Protocol - Abstract
The World Asthma Phenotypes (WASP) study started in 2016 and has been conducted in five centres, in the UK, New Zealand, Brazil, Ecuador and Uganda. The objectives of this study are to combine detailed biomarker and clinical information in order to 1) better understand and characterise asthma phenotypes in high-income countries (HICs) and low and middle-income countries (LMICs), and in high and low prevalence centres; 2) compare phenotype characteristics, including clinical severity; 3) assess the risk factors for each phenotype; and 4) assess how the distribution of phenotypes differs between high prevalence and low prevalence centres. Here we present the rationale and protocol for the WASP study to enable other centres around the world to carry out similar analyses using a standardised protocol. Large collaborative and integrative studies like this are essential to further our understanding of asthma phenotypes. - Abstract
Background: Childhood obesity and asthma are increasing worldwide. A possible link between the two conditions has been postulated. Methods: Cross-sectional studies of stratified random samples of 8-12-year-old children (n=10 652) (16 centres in affluent and 8 centres in non-affluent countries) used the standardized methodology of ISAAC Phase Two. Respiratory and allergic symptoms were ascertained by parental questionnaires. Tests for allergic disease were performed. Height and weight were measured, and overweight and obesity were defined according to international definitions. Prevalence rates and prevalence odds ratios were calculated. Results: Overweight (odds ratio=1.14, 95%-confidence interval: 0.98; 1.33) and obesity (odds ratio=1.67, 95%-confidence interval: 1.25; 2.21) were related to wheeze. The relationship was stronger in affluent than in non-affluent centres. Similar results were found for cough and phlegm, rhinitis and eczema but the associations were mostly driven by children with wheeze. There was a clear association of overweight and obesity with airways obstruction (change in FEV1/FVC, 20.90, 95%-confidence interval: 21.33%; 20.47%, for overweight and 22.46%, 95%-confidence interval: 23.84%; 21.07%, for obesity) whereas the results for the other objective markers, including atopy, were null. Conclusions: Our data from a large international child population confirm that there is a strong relation of body mass index with wheeze especially in affluent countries. Moreover, body mass index is associated with an objective marker of airways obstruction (FEV1/FVC) but no other objective markers of respiratory and allergic disorders. © 2014 Weinmayr et al.

Objective To explore the effects of geohelminth infections and age on atopy, wheeze, and the association between atopy and wheeze. Methods A case-control study was done in 376 subjects (149 cases and 227 controls) aged 7–19 years living in rural communities in Ecuador. Wheeze cases, identified from a large cross-sectional survey, had recent wheeze and controls were a random sample of those without wheeze. Atopy was measured by the presence of allergen-specific IgE (asIgE) and skin prick test (SPT) responses to house dust mite and cockroach. Geohelminth infections were measured in stools and anti-Ascaris IgE in plasma. Results The fraction of recent wheeze attributable to anti-Ascaris IgE was 45.9%, while those for SPT and asIgE were 10.0% and 10.5% respectively. The association between atopy and wheeze was greater in adolescents than children. Although Anti-Ascaris IgE was strongly associated with wheeze (adj. OR 2.24 (95% CI 1.33–3.78, P = 0.003) and with asIgE (adj. OR 5.34, 95% CI 2.49–11.45, P < 0.001), the association with wheeze was independent of asIgE. There was some evidence that the association between atopy and wheeze was greater in uninfected subjects compared with those with active geohelminth infections. Conclusions and clinical relevance Atopy to house dust mite and cockroach explained few wheeze cases in our study population, while the presence of anti-Ascaris IgE was an important risk factor. Our data provided only limited evidence that active geohelminth infections attenuated the association between atopy and wheeze in endemic areas or that age modified this association. The role of allergic sensitization to Ascaris in the development of wheeze, independent of atopy, requires further investigation. Salvador

We describe immune phenotypes (innate and adaptive cytokines) according to environmental exposure using latent class analysis. A total of 310 schoolchildren living in Ecuador were assayed for spontaneous cytokine production as well as mitogen (SEB)-stimulated cytokines in whole blood cultures. We collected data on environmental exposures by questionnaire and on intestinal parasites by examination of stool samples. Latent class analysis (LCA) was used to group children according to their innate (IL-6, IL-8, IL-10 and TNF-α) and adaptive (IL-5, IL-13, IL-17, IFN-γ and IL-10) cytokine profile. We also conducted multiple-group LCA and LCA with covariates to evaluate the effect of predictors on profile membership. We identified both hyporesponsive and Th2-modified immune phenotypes produced by peripheral blood leukocytes (PBLs) that were associated with intestinal worms and birth order, providing insights into how poor hygiene mediates immunologic effects on immune-mediated diseases. Introduction: Malaria and curable sexually transmitted and reproductive tract infections (STIs/RTIs) are important causes of adverse birth outcomes (ABO) and are both prevalent in most parts of sub-Saharan Africa. From a public health perspective, control of these infections requires interventions that are part of an integrated antenatal care package. The extent to which there may be coinfection increases the importance of such an integrated approach to reduce ABO. A systematic review and meta-analysis published in 2012 showed that the prevalence of malaria and curable STIs/RTIs among antenatal attendees in sub-Saharan Africa is considerable. However, the prevalence of malaria and curable STI/RTI coinfection has not been reported in any epidemiological setting. The primary objective of this thesis is to address this knowledge gap by estimating the prevalence of malaria, curable STIs/RTIs and their coinfection and to highlight the importance of an integrated approach to control malaria and STIs/RTIs in pregnancy. Secondary objectives of the study were to: (1) determine risk factors for malaria, curable STIs/RTIs and their coinfection; (2) estimate the prevalence of ABO and identify risk factors for ABO; (3) measure the in vivo efficacy and the prophylactic effect of sulphadoxine-pyrimethamine (SP) in pregnant women, and (4) characterise the molecular markers associated with parasite resistance to SP among pregnant women. Methods: A prospective cohort study of 1,086 antenatal attendees was conducted in Nchelenge District, Zambia. Consenting women visiting two health centres for their first antenatal care (ANC) visit were screened for malaria and curable STIs/RTIs (Chlamydia, gonorrhoea, trichomoniasis, bacterial vaginosis [BV] and syphilis). Socio-demographic data and maternal characteristics were also collected at enrolment. Sulphadoxine-pyrimethamine was administrated as intermittent preventive treatment to eligible women and they were followed up at day 28 for a second 13 malaria screening to determine the therapeutic and prophylactic failure of SP. At delivery participants were screened for placental malaria and data on birth outcomes were recorded. Univariate and multivariate analyses were conducted to determine the association between the potential risk factors for infection and ABO. Results: Of the 1086 women recruited 729 were successfully followed to delivery. The prevalence of malaria infection measured by microscopy was 31.8% (95% CI, 29.1-34.6) and by PCR was 57.8% (95% CI, 54.9-60.8). The risk of malaria infection was higher among pregnant women recruited from Nchelenge health centre compared to those attending the Kashikishi health centre (adjusted odds ratio [aOR] = 1.81; 95% CI, 1.38-2.37, P < 0.001), and HIV-infected women across health centres had a greater risk of malaria infection compared to HIV-uninfected women (aOR = 1.46; 95%, 1.00-2.13, P = 0.045). Infection with at least one STI/RTI was observed in 64.8% (95% CI, 61-67.4) of the participating women. With the exclusion of BV the prevalence of infection with at least one curable STI was 34.5% (95% CI, 31.7-37.4). Infection with at least one STI was associated with BV. In comparison to uninfected women, women infected with BV were at a higher risk of being infected with at least one STI (aOR 1.44; 95% CI, 1.08-1.92, P = 0.012). HIV-infected women had a higher risk of infection with BV than HIV-uninfected women (aOR 1.87; 95% CI, 1.24-2.83, P = 0.003) and women infected with at least one STI had a higher risk of BV (aOR 1.40; 95% CI (1.07 -1.84, P = 0.01). Among participants with complete results (n=1071), 38.7% (95% CI,35.7-41.6) were coinfected with malaria parasites and at least one STI/RTI; 18.9% (95% CI, 16.5-21.2) were infected with malaria parasites only; 26.0% (95% CI, 23.5-28.8) were infected with at least one STI/RTI but no malaria parasites, and 16.4% (95% CI, 14.1-18.6) had no infection. The risk of malaria and curable STI/RTI coinfection was higher among HIV infected women than HIV-uninfected women (OR; 3.59 [95% CI, 1.73-7.48], P < 0.001). The prevalence of composite ABO was 35.1%. Women shorter than 1.5m were at a higher risk of experiencing at least one ABO (aOR 1.55; 95% CI, 1.10-2.18, P = 0.02). The risk of having ABO among para II was less than half of the risk observed in 14 primiparous women (aOR 0.41; 95% CI, 0.27-0.61, P < 0.001) and much lower among multiparous women (aOR 0.32; 95% CI, 0.22-0.48, P < 0.001). Having taken two or more doses of SP during pregnancy was protective against ABO (aOR 0.47; 95% CI, 0.31-0.72, P = 0.001). None of the infections (malaria, curable STIs/RTIs and their coinfection) diagnosed at first ANC were associated with ABO. The prevalence of highly resistant quintuple mutant was 68.8% among first ANC attendees. Despite the moderate prevalence of the quintuple mutant among pregnant women, SP cleared parasitaemia in 86% of the asymptomatic malaria cases among HIV-negative women Conclusion: The prevalence of malaria, STI/RTI and their coinfection at first ANC in this study population was considerable. However, no association was found between ABO and infection with malaria or STI/RTI or their coinfection. This lack of association is partially a result of interventions within the ANC package including treatment of some STI/RTI, intermittent preventive treatment in pregnancy with SP and iron and folic acid supplementation. Sulphadoxine-pyrimethamine retains partial efficacy against P. falciparum malaria in this area with moderate prevalence of the quintuple mutant. While continuing the policy of offering intermittent preventive treatment with SP during pregnancy, an alternative preventive therapy that is effective against both malaria and STIs/RTIs needs to be considered.
