PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks.

Stress, anxiety and fatigue have impact both on the physical and mental health. Stress and anxiety for example are implicated in the onset or progression of immunological, cardiovascular, circulatory or neurodegenerative diseases. Therefore, they have been included ab initio into the list of factors that need to be assessed for a successful health monitoring in the context of SEMEOTICONS. This deliverable presents the outcome of the work of Tasks 5.3 and 5.4, which includes the research, development and implementation of non-intrusive measurement methods of features describing stress, anxiety and fatigue. These methods take advantage of the information of colour and motion of the human face as captured by a video camera. The extracted features describe eye lid activity and opening, eye gaze and eye bags, dark circles, head motion, mouth activity and yawning, the validity of which has been studied thoroughly.

The accurate definition of the user's profile is necessary to set, for each subject, a reliable starting point for the wellness status assessment. Many medical and behavioural data such as: age, height, weight, cholesterol level, phenotype, habits, sports, others may influence the user's profile. Also the psychological information, such as anxiety, stress, and attitude towards health issues should be taken into account because of their strong relevance in the choice of the support to be provided by the system.

This report describes the results of the first periodic verification of the methods and devices developed by WP3, WP4, and WP5. The assessment of the different methods in real settings is described. Methods and devices are tested on a non-aggregated basis. At the current stage of the project, the validation mainly regards the different pieces (method and devices) separately and is mainly focused to attest that they work as expected. Data used for testing are mainly from publicly available databases, and widely accepted by the scientific community for algorithm benchmarking. The reference data acquired in acquisition campaign held in Pisa in May 2014 were also used whenever needed.

HEARTFAID is a research and development project aimed at devising, developing and validating an innovative knowledge based platform of services, able to improve early diagnosis and to make more effective the medical-clinical management of heart diseases within elderly population. Chronic Heart Failure is one of the most remarkable health problems for prevalence and morbidity, especially in the developed western countries, with a strong impact in terms of social and economic effects. All these aspects are typically emphasized within the elderly population, with very frequent hospital admissions and a significant increase of medical costs. Recent studies and experiences have demonstrated that accurate heart failure management programs, based on a suitable integration of inpatient and outpatient clinical procedures, might prevent and reduce hospital admissions, improving clinical status and reducing costs. HEARTFAID aims at defining efficient and effective health care delivery organization and management models for the "optimal" management of the care in the field of cardiovascular diseases. The HEARTFAID innovative computerized system will improve the processes of diagnosis, prognosis and therapy provision, providing the following services: * electronic health record for easy and ubiquitous access to heterogeneous patients data;* integrated services for healthcare professionals, including patient telemonitoring, signal and image processing, alert and alarm system;* clinical decision support in the heart failure domain, based on pattern recognition in historical data, knowledge discovery analysis and inferences on patients' clinical data.The formalization of the pre-existing clinical knowledge and the discovery of new elicited knowledge represent the core of the HEARTFAID platform.

What if you could discover your health status by looking at yourself in the mirror? Since November 2013, the EU FP7 Project SEMEOTICONS is working to make this possible. The Project is building a multi-sensory device, having the form of a conventional mirror, able to read the semeiotic code of the face and detect possible evidence of the onset of cardio-metabolic diseases. The device, called Wize Mirror, integrates unobtrusive imaging sensors used to capture videos, images and 3D scans of the face. These are processed to assess the risk of a cardio-metabolic disease and thereby suggest possible strategies to prevent its onset.

The platform is based on a multi-functional middleware that adopts the most common standards for data encoding and communication, as well as the standard solutions actually available to support the interoperability among heterogeneous systems, such as IHE profiles, HL7 messaging, Service Oriented Architectures and Enterprise Service Bus. The main focus of this paper is on the description of the challenges and the solutions adopted for the implementation of a suitable Middleware; however, for a better comprehension the implemented solutions are selected and shown in the context of the overall platform of services for the management of heart failure.

This deliverable summaries the activities carried out, within Task 5.4 "Implementation of the Decision Support System", for developing the HEARTFAID CDSS Prototype, following the design that was performed in the previous years of the Project, within Task 5.3, and summarized in Deliverable D15 "Functional Specifications of Data Processing and Decision Support Services" (DEL D15, 2007). Such a design was based on a deep investigation of CHF stakeholders' needs and expectations which resulted in a detailed list of CDSS functional requirements; while an accurate analysis of the methodological and technological foundations resulted in the detailed definition of the CDSS functional specifications. The CDSS has been then devised by integrating, in functionally advanced settings: (i) deductive knowledge, elicited from guidelines and medical experts; (ii) inductive knowledge, extracted by data mining techniques applied to significant sets of data; (iii) computational methods for the analysis and interpretation of diagnostic data (i.e., ECG signals and Echocardiographic images). This has required the collection of some results of the activities carried out in other Tasks or Work Packages (WP) of the project. In particular, the deductive knowledge base has been formalized in WP4 and a precise description of its content can be found in Deliverable D22 "Ontologies and Knowledge Representation" (DEL D22, 2007). Inductive models have resulted from the activity of knowledge discovery and described more precisely in Deliverable D29. Methods and algorithms for processing and analysing diagnostic ECG and Echo images have been developed in Task 5.2 and detailed in Deliverable D30 "Models and Methods for Signals and Images Processing" (DEL D30, 2008). This document mainly focuses on the implementation of each component of the CDSS architecture and their integration into the system, and on the integration of the CDSS itself within the platform (integration that is realized mainly by means of the HEARTFAID Web Portal). For the implementation activity, Semantic Web Technologies (SWT) have been used as the most advanced tools for formalizing, re-using and sharing medical knowledge, and reasoning on it; while a service oriented approach has been adopted for the integration and easy access to a number of functionalities. Moreover, realistic clinical scenarios have been carefully defined in cooperation with clinical partners for assuring the realness and effectiveness of the developed functionalities of the system. More in details, the document is organized as follows. In the first introductory chapter, a recall of the significance of decision support in the management of CHF is reported along with a brief description of the main issues to be faced when developing a decision support system. Afterwards, in the second chapter, HEARTFAID CDSS architecture, as was defined in the previous Tasks 5.3 of the Work-Package 5, is described and motivated. Also, the integration of the system within HEARTFAID platform is summarized for assuring a global view of the system functioning. In Chapter 3, attention is focused on the implementation details, hence each component of the CDSS architecture is considered and its implementation described in details along with the problems faced and the solutions adopted. Particular attention is devoted to the ontological Knowledge Base, the computational reasoning methods included into the Model Base, as well as the algorithms defined and developed for processing ECG signals and Echocardiographic images. Finally, the implementation of the Meta Level of the system is described and discussed. In the fourth chapter, the integration within the Platform through the Middleware is discussed along with the issues faced and the technological choices made. The implementation of the showcase related to the clinical management of a HF patient is reported in Chapter 5. Two appendices close the document.

Signal and imaging investigations are currently a basic step of the diagnostic, prognostic and follow-up processes of heart diseases. Besides, the need of a more efficient, cost-effective and personalized care has lead nowadays to a renaissance of clinical decision support systems (CDSS). The purpose of this paper is to present an effective way to achieve a high-level integration of signal and image processing methods in the general process of care, by means of a clinical decision support system, and to discuss the advantages of such an approach. Among several heart diseases, we treat heart failure, that for its complexity highlights best the benefits of this integration. Architectural details of the related components of the CDSS are provided with special attention to their seamless integration in the general IT infrastructure. In particular, significant and suitably designed image and signal processing algorithms are introduced to objectively and reliably evaluate important features that, in collaboration with the CDSS, can facilitate decisional problems in the heart failure domain. Furthermore, additional signal and image processing tools enrich the model base of the CDSS.

HEARTFAID aims at defining efficient and effective health care delivery organization and management models for an "optimal" patients' management in the field of cardiovascular diseases. An informative and decision support platform of services will be devised for improving all the processes related to diagnosis, prognosis, treatment and personalization of health care of the Heart Failure (HF) in elderly population. To this end, innovative results on computational modelling, knowledge discovery methodologies, visualization and imaging techniques, and the medical knowledge of the relevant domain will be opportunely integrated to design and develop an effective and reliable Clinical Decision Support System (CDSS): the HEARTFAID CDSS, corresponding to the core of HEARTFAID intelligence. This system will be able to process clinical knowledge and patient-related information, intelligently filtered, processed and presented at appropriate times, to enhance patient care. It will be devised as a service of the HEARTFAID platform for providing an effective support to the daily practice of the clinicians, by implementing adequate data processing algorithms, by providing guidelines to medical protocols as well as access to the knowledge base, by sending alarms in case of critical situations, and by supplying diagnostic suggestions. Two peculiar issues are involved in the development of the HEARTFAID CDSS for supporting medical decision making, i.e. innovative approaches for biomedical signal and image processing; robust and reliable reasoning approaches, based on Machine Learning and inference methodologies on declarative and procedural domain knowledge. The first, fundamental step towards the design and development of the HEARTFAID CDSS consists in analysing the requirements and specifying the functional details of both data processing and decision support services. These topics are the focus of this deliverable, which collects the results of Task T5.1, "Identification of representation features for signals and images processing", and Task T5.3, "Requirements and functional specification of the Decision Support System", of Work Package WP 5. A deep investigation into issues inherent in (i) data typologies and their characterization, (ii) reasoning methodologies, (iii) HEARTFAID CDSS logical- functional vision, and (iv) current available technologies are herein provided, with the final aim of supplying the guidelines for the development of the HEARTFAID data processing and decision support services. The document layout has two main "souls", addressing, from one side, all the methodological and technological aspects of signal and images processing, and from the other, all the topics related to decision support. Moreover, it is ideally divided into two parts: in the first part (Chapters 2, 3, and 4) the discussion consists of a revision of the State of the Art; whereas in the second part (Chapters 5, 6, 7, and 8) the attention is focused on the HEARTFAID specificity, regarding the final goal of the deliverable. More in detail, the document is organized as follows. At first, an introduction supplies an overview of the problems to face for reaching the HEARTFAID Grand Vision about the intelligent support to the medical decision making. In Chapter 2, the available diagnostic resources for signals and images acquisition are introduced, analysing their peculiarities, relevance to the HEARTFAID clinical domain, and open-problems. An overview of the principles of signal and image processing and of the mathematical models for features extraction is also provided. Afterwards, in the third chapter, a careful survey of the methodological foundations of decision support systems (DSS) is reported, discussing the main aspects of decision theory and typical DSS models and structures. According to HEARTFAID definition as a knowledge-based platform of services, medical domain knowledge plays a fundamental role in HEARTFAID decision support development. Thus, particular attention is reserved to the category of Expert Systems or Knowledge-based DSS, and related issues, such as knowledge representation and inference engine modelling. Clinical applications and topics inherent to CDSS, e.g. guidelines modelling, are widely addressed as well, for better understanding the design implications involved. The chapter ends with a discussion about the important issues regarding DSS design and success factors, which should be taken into account when devising the HEARTFAID CDSS. Chapter 4 reports a critical description and analysis of the available up-to-date technologies for both data processing and decision support services. In Chapter 5, the biomedical parameters, relevant to HF and selected by the medical partners, are critically reviewed in the perspective of data processing. From this analysis, the most relevant problem in data processing w.r.t. HEARTFAID platform is selected and the functional specifications of the planned signals and image processing are described. The HEARTFAID clinical decision making problems are deeply investigated in Chapter 6, in order to identify the overall objectives of the HEARTFAID CDSS, i.e. what it should accomplish and why, specifying the element of the knowledge to be formalized. The system requirements are defined responding to HF clinicians' needs, as also stated in the deliverable D5, in order to devise an efficient and usable system for supporting the medical personnel in their daily activity.

The HEARTFAID (HF) project aims to devise, design, develop and deploy advanced and innovative computerized systems and services that, by collecting, integrating and processing all relevant biomedical data and information, are able to improve medical knowledge and make more effective and efficient all the processes related to diagnosis, prognosis, treatment and personalization of the Heart Failure care in elderly patients. This general goal will be achieved by: o developing an innovative technological platform for informative and decision support, which can make the procedures of diagnosis, prognosis and therapy more effective and reliable for the patient and optimal in the use of medical and clinical resources. This platform, by exploiting innovative results on computational modelling, knowledge discovery methodologies, visualization and imaging techniques, and using the medical knowledge of the relevant domain, is able to effectively integrate and process biomedical data and information at different levels of structure; o defining new health care delivery organization and management models for the relevant domain, which may result in more effective and efficient use of the needed total resources (health care operators, health care equipments, financial resources). A key work-package to achieve the first point is WP3 - MIDDLEWARE, INTEROPERABILITY AND INTEGRATION, which has the main goal to analyse, design and implement the core software architecture of the HEARTFAID platform, i.e. the Middleware. The Middleware is responsible to guarantee the integration and the interoperability among all the modules of the platform, as well as of the services provided to the end-users. As described in the Description of Work (DoW) of the project concerning the architectural design of the platform, the Middleware has been decomposed into two different, but integrated, levels: the Interoperability Middleware and the Integration Middleware. The first level has the main objective to guarantee the interoperability of the HEARTFAID system components and, using the approach of service-oriented integration, to integrate the functionalities the platform will provide to the end users, while the second level has the goal to design and develop a Data Management System that is responsible to guarantee the following features: all the data flowing within the entire platform are compliant with the standards identified in work-package WP3; management of the heterogeneous repository in order to allow the treatment of raw data, laboratory data, structured information (Electronic Patient Record (EPR), data entry services, and so on), multimedia and other data (reports, images, ultrasound signals, and so on). The first fundamental step for the attainment of these objectives, is to analyse the requirements of the final users, and hence of the platform, and to define the functional specifications of the platform itself. This deliverable has the main goal to analyse the problems that should be faced to achieve the Grand Vision of the project, and to define the functional specifications of the different components of the Interoperability and Integration Middleware. In more details, this document has been organised as follows. At first, an introduction will provide a general overview about the Health Information Systems. The main international initiatives to standardise protocols, roles, messaging procedures and data encoding procedures, will be described. Moreover, the vision that will drive the design and development of the HEARTFAID Electronic Health Records will be presented. Afterwards, we will analyse the state of the art and the methodological foundations that will support the "application integration". The issue of interoperability, including data access, exchange, and integration becomes more and more sophisticate especially for medical companies and healthcare structures, due to the heterogeneity of the informative systems available. Therefore, at this level it is nowadays very important to adopt adequate standards for data encoding and communication. These standards and the most common approaches, when they are considered suitable for the purposes of the HF project, will be adopted when defining the main infrastructure of the HF platform of services, i.e. the middleware. After a description of the state of the art on the most advanced solution able to support interoperability, we will analyse the requirements that the middleware should satisfy. These requirements are mainly concerned with interoperability issues of the healthcare structures. In fact, although health organizations are important users of the Information Technology, each of them and the system implemented typically use their own protocols and adopts different standards, principally because there is a lack of divulgation of information concerning standards. Furthermore, concerning data retention and exchange, information models in use are not always based on standards, and even when they are, such standards are not interoperable and even not enough spread within the stakeholders. Typically those solutions address the needs for local administration and management of services, that when available are not supported by its own ontology. Thus, health information is sparse, and with severe interoperability problems as people move across borders or even across health systems independently of the place. In the future, interoperability will enable the seamless integration of any Health heterogeneous systems. Thus, health data will be made available independently of the standards adopted for retrieving and exchanging information. A visionary future seeks a methodology to enhance organization's interoperability, keeping he same organization's technical and operational environment, improving its methods of work and the usability of the installed technology through ontological harmonization of the organization's models in use. Procedures will take complete care regarding ethics and privacy. This will allow secure and fast access to comparable public health data and to patient information located in different places over a wide variety of repositories and medical devices. After the requirements have been identified, we will analyse and define the functional specifications of the Interoperability and the Integration Middleware, as well as the issues related with the integration of the HF Decision Support System. In particular, the different components of the middleware layers will be described. The challenge of determining the functional specifications of each module lies in identifying the user needs and designing and implementing comprehensive and convenient interfaces, both with the final users and between the modules themselves. Since the Dow of the HEARTFAID project does not foresee a specific document on the design of the Middleware, we made a step forward with respect to the identification of the functional specifications and in Section 6 we will introduce some models that can be adopted in the subsequent implementation of the platform. Preliminary hardware specifications will also be identifies. Finally, due to the particular context in which the platform of services will operate and the sensibility of the data that will be handled, the security aspects related to safety, secrecy and privacy of data will examined.

- Abstract
HEARTFAID is a research and development project aimed at devising, developing and validating an innovative knowledge based platform of services, able to improve early diagnosis and to make more effective the medical-clinical management of heart diseases within elderly population. Chronic Heart Failure is one of the most remarkable health problems for prevalence and morbidity, especially in the developed western countries, with a strong impact in terms of social and economic effects. All these aspects are typically emphasized within the elderly population, with very frequent hospital admissions and a significant increase of medical costs. Recent studies and experiences have demonstrated that accurate heart failure management programs, based on a suitable integration of inpatient and outpatient clinical procedures, might prevent and reduce hospital admissions, improving clinical status and reducing costs. HEARTFAID aims at defining efficient and effective health care delivery organization and management models for the "optimal" management of the care in the field of cardiovascular diseases. The HEARTFAID innovative computerized system will improve the processes of diagnosis, prognosis and therapy provision, providing the following services: * electronic health record for easy and ubiquitous access to heterogeneous patients data;* integrated services for healthcare professionals, including patient telemonitoring, signal and image processing, alert and alarm system;* clinical decision support in the heart failure domain, based on pattern recognition in historical data, knowledge discovery analysis and inferences on patients' clinical data.The formalization of the pre-existing clinical knowledge and the discovery of new elicited knowledge represent the core of the HEARTFAID platform
