Abstract

Surgical patients who develop sepsis (contamination or poisoning of the system by bacterial infection) have a high mortality. Death usually is the result of progressive multiple organ failure or septic shock (inadequate peripheral blood flow to the heart caused by bacterial infection). Changes in the immune system have been shown to occur in severely septic patients. The blood levels of the antibody immunoglobulin G (IgG) tend to be lower in septic patients who do not survive. A study was undertaken to determine if treatment with high-dose IgG can reduce mortality among high-risk surgical patients in the intensive care unit. A standardized scoring system (developed by Elebute and Stoner) was used to evaluate the severity of the sepsis. The study included 62 consecutive patients with scores of 20 or higher; 29 patients were given IgG and 33 patients received albumin (the control group). The two patient groups were of similar age and initial sepsis scores. The mortality of the IgG-treated group was significantly lower than that of the controls: 38 versus 67 percent. The causes of death in all except one patient who had a heart attack, were septic shock and multiple organ failure. Seven percent of the IgG treated patients died from septic shock, compared with 33 percent of the control patients. These results indicate that treatment with IgG for surgical patients with high (20 or higher) sepsis scores decreases mortality due to septic shock.
