Abstract

The postprandial glucose response is an independent risk factor for type 2 diabetes. Observationally, early glucose response after an oral glucose challenge has been linked to intestinal glucose absorption, largely influenced by the expression of sodium–glucose cotransporter 1 (SGLT1). This study uses Mendelian randomization (MR) to estimate the causal effect of intestinal SGLT1 expression on early glucose response. Involving 1,547 subjects with class II/III obesity from the Atlas Biologique de l'Obésité Sévère cohort, the study uses SGLT1 genotyping, oral glucose tolerance tests, and jejunal biopsies to measure SGLT1 expression. A loss-of-function SGLT1 haplotype serves as the instrumental variable, with intestinal SGLT1 expression as the exposure and the change in 30-min postload glycemia from fasting glycemia (Δ30 glucose) as the outcome. Results show that 12.8% of the 1,342 genotyped patients carried the SGLT1 loss-of-function haplotype, associated with a mean Δ30 glucose reduction of −0.41 mmol/L and a significant decrease in intestinal SGLT1 expression. The observational study links a 1-SD decrease in SGLT1 expression to a Δ30 glucose reduction of −0.097 mmol/L. MR analysis parallels these findings, associating a statistically significant reduction in genetically instrumented intestinal SGLT1 expression with a Δ30 glucose decrease of −0.353. In conclusion, the MR analysis provides genetic evidence that reducing intestinal SGLT1 expression causally lowers early postload glucose response. International audience; The T.A.T. to the test by structural analysis 2/2: diegesis, tri-partition of narratives and operators of analysis Abstract: The T.A.T. (Thematic Apperception Test) can be considered not in its mimetic narrative dimension, but rather as a diegetic universe. The structural analysis of the narratives articulated in psychoanalysis makes it possible to think of the narrative as overdetermined by the unconscious issue of the subject. We propose an alternative to the analyses of the School of Paris by excluding the primacy of diagnostic. The recourse to structuralism allows us to formalize six operators of analysis: the canonical scheme of the narrative, with its tripartition in IS-C-R (Initial Situation, Complication, Resolution), plate place, actants, narrator/recipient, temporality, narrative effects. The form and recipients of a story are thus more important than its theme. Since the narrative is no longer read as a mimesis of the patient's universe, it becomes the witness of the diegetic universe of the subject. (Thematic Apperception Test) est envisagé comme un univers diégétique, non pas comme un récit mimétique. L'analyse structurale des récits articulée à la psychanalyse permet de penser le récit comme surdéterminé par la problématique inconsciente du sujet. Nous proposons ainsi une alternative aux analyses de l'École de Paris en écartant le primat des préoccupations diagnostiques. Le recours au structuralisme nous permet de formaliser six opérateurs d'analyse le schéma canoni ue du récit avec sa tripartition en SI-C-R (Situation Initiale, Complication, Résolution), la place de la planche, les actants, le narrateur/destinataire, la temporalité, les effets narratifs. La forme et les destinataires d'un récit sont ainsi plus importants que son thème. Le récit n'étant plus lu comme mimèse de l'univers du patient, il devient le témoin de l'univers diégétique du sujet. Notre méthode promeut donc la logique d'analyse et de formalisation du fantasme, du transfert et de la pro lémati ue inconsciente du sujet.

International audience; Le T.A.T. (Thematic Apperception Test) est une méthode projective au succès remarquable. Cependant, épistémologiquement et historiquement, les méthodes d’analyse de protocole laissent peu de place au concept de structure narrative. En France, au moment des scissions dans le mouvement psychanalytique, l’École de Paris a produit une méthode psychanalytique avec les P.E.D. (procédés d’élaboration du discours) synthétisée dans une feuille de dépouillement. Nous proposons une option à cette méthode en recourant à l’analyse structurale des récits. Ceci permet, d’une part, de se dégager de préoccupations nosographiques et de réduire nettement l’intérêt pour les mécanismes de défense du moi et, d’autre part, de promouvoir l’analyse des processus inconscients organisant tout récit. Notre méthode structurale demeure ainsi psychanalytique, mais s’écarte de la logique diagnostique au profit d’une logique d’analyse et de formalisation de la problématique inconsciente du sujet.
