In biomedical image analysis, chosen performance metrics often do not reflect the domain interest, and thus fail to adequately measure scientific progress and hinder translation of ML techniques into practice. To overcome this, we created Metrics Reloaded, a comprehensive framework guiding researchers in the problem-aware selection of metrics. Developed by a large international consortium in a multistage Delphi process, it is based on the novel concept of a problem fingerprint-a structured representation of the given problem that captures all aspects that are relevant for metric selection, from the domain interest to the properties of the target structure(s), dataset and algorithm output. On the basis of the problem fingerprint, users are guided through the process of choosing and applying appropriate validation metrics while being made aware of potential pitfalls. Metrics Reloaded targets image analysis problems that can be interpreted as classification tasks at image, object or pixel level, namely image-level classification, object detection, semantic segmentation and instance segmentation tasks. To improve the user experience, we implemented the framework in the Metrics Reloaded online tool. Following the convergence of ML methodology across application domains, Metrics Reloaded fosters the convergence of validation methodology. Its applicability is demonstrated for various biomedical use cases.

However, increasing evidence shows that, particularly in image analysis, metrics are often chosen inadequately. Although taking into account the individual strengths, weaknesses and limitations of validation metrics is a critical prerequisite to making educated choices, the relevant knowledge is currently scattered and poorly accessible to individual researchers. Based on a multistage Delphi process conducted by a multidisciplinary expert consortium as well as extensive community feedback, the present work provides a reliable and comprehensive common point of access to information on pitfalls related to validation metrics in image analysis. Although focused on biomedical image analysis, the addressed pitfalls generalize across application domains and are categorized according to a newly created, domain-agnostic taxonomy. The work serves to enhance global comprehension of a key topic in image analysis validation.

International benchmarking competitions have become fundamental for the comparative performance assessment of image analysis methods. However, little attention has been given to investigating what can be learnt from these competitions. Do they really generate scientific progress? What are common and successful participation strategies? What makes a solution superior to a competing method? To address this gap in the literature, we performed a multicenter study with all 80 competitions that were conducted in the scope of IEEE ISBI 2021 and MICCAI 2021. Statistical analyses performed based on comprehensive descriptions of the submitted algorithms linked to their rank as well as the underlying participation strategies revealed common characteristics of winning solutions. These typically include the use of multi-task learning (63%) and/or multi-stage pipelines (61%), and a focus on augmentation (100%), image preprocessing (97%), data curation (79%), and post-processing (66%). The 'typical' lead of a winning team is a computer scientist with a doctoral degree, five years of experience in biomedical image analysis, and four years of experience in deep learning. Two core general development strategies stood out for highly-ranked teams: the reflection of the metrics in the method design and the focus on analyzing and handling failure cases. According to the organizers, 43% of the winning algorithms exceeded the state of the art but only 11% completely solved the respective domain problem. The insights of our study could help researchers (1) improve algorithm development strategies when approaching new problems, and (2) focus on open research questions revealed by this work.

Failure is an integral part of life and by extension academia. At the same time, failure is often ignored, with potentially negative consequences both for the science and the scientists involved. This article provides several strategies for learning from and dealing with failure instead of ignoring it. Hopefully, our recommendations are widely applicable, while still taking into account individual differences between academics. These simple rules allow academics to further develop their own strategies for failing successfully in academia.

Rapid advances in image processing capabilities have been seen across many domains, fostered by the application of machine learning algorithms to "big-data". However, within the realm of medical image analysis, advances have been curtailed, in part, due to the limited availability of large-scale, well-annotated datasets. One of the main reasons for this is the high cost often associated with producing large amounts of high-quality meta-data. Recently, there has been growing interest in the application of crowdsourcing for this purpose; a technique that has proven effective for creating large-scale datasets across a range of disciplines, from computer vision to astrophysics. Despite the growing popularity of this approach, there has not yet been a comprehensive literature review to provide guidance to researchers considering using crowdsourcing methodologies in their own medical imaging analysis. In this survey, we review studies applying crowdsourcing to the analysis of medical images, published prior to July 2018. We identify common approaches, challenges and considerations, providing guidance of utility to researchers adopting this approach. PURPOSE: A method for automatically quantifying emphysema regions using High-Resolution Computed Tomography (HRCT) scans of patients with chronic obstructive pulmonary disease (COPD) that does not require manually annotated scans for training is presented.METHODS: HRCT scans of controls and of COPD patients with diverse disease severity are acquired at two different centers. Textural features from co-occurrence matrices and Gaussian filter banks are used to characterize the lung parenchyma in the scans. Two robust versions of multiple instance learning (MIL) classifiers that can handle weakly labeled data, miSVM and MILES, are investigated. Weak labels give information relative to the emphysema without indicating the location of the lesions. The classifiers are trained with the weak labels extracted from the forced expiratory volume in one minute (FEV1) and diffusing capacity of the lungs for carbon monoxide (DLCO). At test time, the classifiers output a patient label indicating overall COPD diagnosis and local labels indicating the presence of emphysema. The classifier performance is compared with manual annotations made by two radiologists, a classical density based method, and pulmonary function tests (PFTs).RESULTS: The miSVM classifier performed better than MILES on both patient and emphysema classification. The classifier has a stronger correlation with PFT than the density based method, the percentage of emphysema in the intersection of annotations from both radiologists, and the percentage of emphysema annotated by one of the radiologists. The correlation between the classifier and the PFT is only outperformed by the second radiologist.CONCLUSIONS: The presented method uses MIL classifiers to automatically identify emphysema regions in HRCT scans. Furthermore, this approach has been demonstrated to correlate better with DLCO than a classical density based method or a radiologist, which is known to be affected in emphysema. Multiple instance learning (MIL) is a form of weakly supervised learning where training instances are arranged in sets, called bags, and a label is provided for the entire bag. This formulation is gaining interest because it naturally fits various problems and allows to leverage weakly labeled data. Consequently, it has been used in diverse application fields such as computer vision and document classification. However, learning from bags raises important challenges that are unique to MIL. This paper provides a comprehensive survey of the characteristics which define and differentiate the types of MIL problems. Until now, these problem characteristics have not been formally identified and described. As a result, the variations in performance of MIL algorithms from one data set to another are difficult to explain. In this paper, MIL problem characteristics are grouped into four broad categories: the composition of the bags, the types of data distribution, the ambiguity of instance labels, and the task to be performed. Methods specialized to address each category are reviewed. Then, the extent to which these characteristics manifest themselves in key MIL application areas are described. Finally, experiments are conducted to compare the performance of 16 state-of-the-art MIL methods on selected problem characteristics. This paper provides insight on how the problem characteristics affect MIL algorithms, recommendations for future benchmarking and promising avenues for research. Supervised learning is ubiquitous in medical image analysis. In this paper we consider the problem of meta-learning – predicting which methods will perform well in an unseen classification problem, given previous experience with other classification problems. We investigate the first step of such an approach: how to quantify the similarity of different classification problems. We characterize datasets sampled from six classification problems by performance ranks of simple classifiers, and define the similarity by the inverse of Euclidean distance in this meta-feature space. We visualize the similarities in a 2D space, where meaningful clusters start to emerge, and show that the proposed representation can be used to classify datasets according to their origin with 89.3% accuracy. These findings, together with the observations of recent trends in machine learning, suggest that meta-learning could be a valuable tool for the medical imaging community.

Classification of emphysema patterns is believed to be useful for improved diagnosis and prognosis of chronic obstructive pulmonary disease. Emphysema patterns can be assessed visually on lung CT scans. Visual assessment is a complex and time-consuming task performed by experts, making it unsuitable for obtaining large amounts of labeled data. We investigate if visual assessment of emphysema can be framed as an image similarity task that does not require expert. Substituting untrained annotators for experts makes it possible to label data sets much faster and at a lower cost. We use crowd annotators to gather similarity triplets and use t-distributed stochastic triplet embedding to learn an embedding. The quality of the embedding is evaluated by predicting expert assessed emphysema patterns. We find that although performance varies due to low quality triplets and randomness in the embedding, we still achieve a median F 1 score of 0.58 for prediction of four patterns.

Supervised classification is widely used for image segmentation. To work effectively, these techniques need large amounts of labeled training data, that is representative of the test data. Different patient groups, different scanners or different scanning protocols can lead to differences between the images, thus representative data might not be available. Transfer learning techniques can be used to account for these differences, thus taking advantage of all the available data acquired with different protocols. We investigate the use of classifier ensembles, where each classifier is weighted according to the similarity between the data it is trained on, and the data it needs to segment. We examine 3 asymmetric similarity measures that can be used in scenarios where no labeled data from a newly introduced scanner or scanning protocol is available. We show that the asymmetry is informative and the direction of measurement needs to be chosen carefully. We also show that a point set similarity measure is robust across different studies, and outperforms state-of-the-art results on a multi-center brain tissue segmentation task.

Multiple instance learning (MIL) is an extension of supervised learning where the objects are represented by sets (bags) of feature vectors (instances) rather than individual feature vectors. For example, an image can be represented by a bag of instances, where each instance is a patch in that image. Only bag labels are given, however, the standard assumption is that that a bag is positive if and only if it contains a positive, or concept instance. In other words, only concept instances are informative for the bag label. The goal is to learn a bag classifier, although an instance classifier may also be desired. This scenario is suitable for applications where objects are heterogeneous and representing them as a single feature vector may lose important information, and/or in cases where only weakly labeled data is available. Several approaches to MIL exist. Instance-based approaches rely on stronger assumptions about the relationship of the instance labels and the bag labels, and define a bag classifier through an instance classifier. Bag-based approaches learn a bag classifier directly, often by converting the problem into a supervised problem. These methods often disregard the standard assumption, and instead use the collective assumption, where all instances are informative. One way to convert the problem into a supervised one, is to describe each bag by a vector of its distances to a set of reference prototypes. In this so-called dissimilarity representation, supervised classifiers can be used. The goal of this thesis is to study the dissimilarity representation as a method for dealing with multiple instance learning problems. Chronic obstructive pulmonary disease (COPD) is a lung disease where early detection benefits the survival rate. COPD can be quantified by classifying patches of computed tomography images, and combining patch labels into an overall diagnosis for the image. As labeled patches are often not available, image labels are propagated to the patches, incorrectly labeling healthy patches in COPD patients as being affected by the disease. We approach quantification of COPD from lung images as a multiple instance learning (MIL) problem, which is more suitable for such weakly labeled data. We investigate various MIL assumptions in the context of COPD and show that although a concept region with COPD-related disease patterns is present, considering the whole distribution of lung tissue patches improves the performance. The best method is based on averaging instances and obtains an AUC of 0.742, which is higher than the previously reported best of 0.713 on the same dataset. Using the full training set further increases performance to 0.776, which is significantly higher (DeLong test) than previous results.

Nearest-neighbor (NN) classification has been widely used in many research areas, as it is a very intuitive technique. As long as we can defined a similarity or distance between two objects, we can apply NN, therefore making it suitable even for non-vectorial data such as graphs. An alternative to NN is the dissimilarity space [2], where distances are used as features, i.e. an object is represented as a vector of its distances to prototypes or landmarks. This representation can be used with any classifier, and has been shown to be potentially more effective than NN classification on the same dissimilarities. Defining distance measures on complex objects is not a trivial task. Due to human judgments, suboptimal matching procedures or simply by construction, distance measures on non vectorial data may often be asymmetric. A common solution for NN approaches is to symmetrize the measure by averaging the two distances [2]. However, in the dissimilarity space, symmetric measures are not required. We explore whether asymmetry is an artifact that needs to be removed, or an important source of information. This abstract highlights one example of informative asymmetric measures, covered in [1].

One of the key factors in customer satisfaction is application performance. To be able to guarantee good performance, it is necessary to take appropriate measures before a server overload occurs. While in small systems it is usually possible to predict server overload using a subjective human expert, an automated overload prediction mechanism is important for ultra-large scale systems, such as multi-tenant Software-as-a-Service (SaaS) systems. An automated prediction mechanism would be an initial step towards self-adaptiveness of such systems, a property which leads to less human intervention during maintenance, resulting in less errors and better quality of service. In order to provide such a prediction mechanism, it is important to have a solid overload detection approach, which is (1) a first step towards automated prediction and (2) necessary for automated testing of a prediction mechanism. In this paper we propose a number of steps which help with the design and optimization of a statistical pattern classifier for server overload detection.
