Background Under the background of new medical science, the deep integration of information technology and medical education is encouraged to train first-class medical talents to serve the construction of healthy China.Currently, empathy training in doctor-patient communication mainly consists of simulated communication and group discussion, with less reliance on artificial intelligence technology for learning. Objective To develop a system for teaching and evaluating doctor-patient communication empathy language. This system will be used in course teaching to pave the way for future doctor-patient communication empathy teaching methods. Carry out teaching applications to enhance the communication and empathy language expression skills of medical students and doctors, and gather feedback to optimize and improve the system. Methods Between September 2021 and February 2022, the research group focus on utilizing iFlytek speech recognition technology and the empathy semantic recognition algorithm. A system called the "Doctor-patient Communication Virtual Simulation Teaching and Evaluation System of empathic language" was developed using 10 typical cases of doctor-patient communication, demonstrations of empathic language, a semantic database of empathic language, empathic language skills, and an overall scoring standard.A total of 950 students from Nanjing Medical University, including 515 undergraduates, 102 medical doctoral students, and 333 clinicians participating in doctor-patient communication courses or training, were selected as the research subjects from March to May 2022. Based on this system, the Doctor-patient Communication Skills Course (2 class hours) teaching experiment was conducted at Nanjing Medical University. A self-designed questionnaire was used to gather information on the subjects' understanding of empathetic language connotations, their improved empathetic language skills, their perception of system ease of use, and their perception of how the system integrates into the rationality of teaching. NVivo software was used to analyze the subjects' feedback, comments, and suggestions. Long time series of spatiotemporally continuous phytoplankton functional type (PFT) products are essential for understanding marine ecosystems, global biogeochemical cycles, and effective marine management. In this study, by integrating artificial intelligence (AI) technology with multi-source marine big data, we have developed a Spatial–Temporal–Ecological Ensemble model based on Deep Learning (STEE-DL), and then generated the first AI-driven Global Daily gap-free 4 km PFTs product from 1998 to 2023 (AIGD-PFT), significantly enhancing the accuracy and spatiotemporal coverage of quantifying eight major PFTs (i.e., Diatoms, Dinoflagellates, Haptophytes, Pelagophytes, Cryptophytes, Green Algae, Prokaryotes, and Prochlorococcus). The input data encompass physical oceanographic, biogeochemical, spatiotemporal information, and ocean color data (OC-CCI v6.0) that have been gap-filled using a Discrete Cosine Transform with a Penalized Least Square (DCT-PLS) approach. The STEE-DL model utilizes an ensemble strategy with 100 ResNet models, applying Monte Carlo and bootstrapping methods to estimate optimal PFT values and assess model uncertainty through ensemble means and standard deviations. The model's performance was validated using multiple cross-validation strategies—random, spatial-block, and temporal-block—combined with in-situ data, demonstrating STEE-DL's robustness and generalization capability. The daily updates and seamless nature of the AIGD-PFT product capture the complex dynamics of coastal regions effectively. Finally, through a comparative analysis using a triple-collocation (TC) approach, the competitive advantages of the AIGD-PFT product over existing products were validated. The AIGD-PFT product not only provides the foundation for detailed analyses of PFT trends, interannual variability, and the impacts of climate change on phytoplankton composition across various temporal and spatial scales, but also has the potential to facilitate precise quantification of marine carbon flux and enhances the accuracy of biogeochemical models. Background: Paraneoplastic neurological syndromes (PNS) are rare disorders associated with malignant tumours, which are triggered by autoimmune reactions. Paraneoplastic cerebellar degeneration (PCD) is the PNS type most commonly associated with ovarian and breast cancer. Two bladder cancers manifesting in PCD were previously reported. However, the cancers in these cases had poor outcomes.Case Presentation: Here, we present a 68-year old man with history of high-grade papillary urothelial carcinoma of the bladder. The patient suffered from persistent cerebellar ataxia accompanied by bladder cancer recurrence five months after transurethral resection of the bladder tumour (TURBt). Laboratory screening for the specific antibodies of paraneoplastic neurological syndromes revealed no positive results. Symptoms were not remitted after a 7-day-course of high-dose glucocorticoid therapy. To our surprise, the patient recovered fully after laparoscopic radical cystectomy. Postoperative pathology revealed that surgical specimens were urothelial carcinoma in situ (CIS) and squamous cell carcinoma of the bladder. The patient remained asymptomatic and there was no evidence of recurrence after the followup period of 11 months.Conclusion: To our knowledge, this is the third report of PCD in a patient with bladder cancer. This case showed that tumour resection cured the PCD. To assist clinical evaluation and management, literature regarding basic PNS characteristics and bladder cancers was reviewed. Cardiac arrest (CA) is a fatal disease with high rates of neurological impairment. At present, targeted temperature management (TTM) is the only strategy with firm clinical evidence to prove its effectiveness. However, there is still controversy on the implementation of TTM, particularly on its depth, with a lack of elucidated underlying therapeutic mechanisms. Six Wistar rats were subjected to 8 min asphyxia-CA and randomly divided into TTM at 33 o C(n=3) or 35 ° C groups (n=3). The spatiotemporal characteristics of cerebral glucose metabolism after CA were investigated by 18F-FDG microPET/CT. Myelin Basic Protein (MBP) immunofluorescence staining was used to assess acute injury and recovery of oligodendrocytes. Functional recovery was evaluated using the neurological deficit score (NDS). There was a significant improvement in functional recovery by NDS (p < 0.05) in the 33 o C group compared with the 35 ° C group. Glucose metabolism of the 33 ° C group was higher than that of the 35 o C group early after resuscitation (within 10 minutes). Immunofluorescence analysis showed that positive MBP signals in the cortex and hippocampus in the 33 o C group were greater than in the 35 o C group. In conclusion, compared to 35 o C TTM, 33 ° C TTM changed the spatiotemporal characteristics of brain glucose metabolisms with improved neurological function, which may be through oligodendrocyte participation.

Malignant glioma is notorious for its aggressiveness and poor prognosis, and the invasiveness of glioma cells is the major obstacle. Accumulating evidence indicates that kinesin superfamily proteins (KIFs) may play key roles in tumor invasiveness, but the mechanisms remained unresolved. Our previous study demonstrated that membrane type 1-matrix metalloproteinase (MT1-MMP) was involved in Kinesin family member 1B (KIF1B)-modulated invasion of gastric cancer cells. Therefore, the role of KIF1B in glioma cell invasion and its relationship with MT1-MMP were explored in the present study. We found that aberrantly increased expression of KIF1B was associated with worse WHO pathological classification and Karnofsky performance status (KPS), which also showed a trend towards worse prognosis. In the transwell assay, knockdown of KIF1B using siRNA repressed U87MG and A172 glioma cell migration and invasion. Silencing KIF1B inhibited expression of membranal MT1-MMP; however, the amount of MT1-MMP in the whole cell lysate was not affected. In conclusion, targeting KIF1B may be an option for anti-invasive therapies targeting glioma.
