1. A novel control strategy to neutralize heat source within solid oxide electrolysis cell (SOEC) under variable solar power conditions

Abstract

The integration of a solid oxide electrolysis cell (SOEC) with a photovoltaic (PV) system presents a viable method for storing variable solar energy through the production of green hydrogen. To ensure the SOEC's safety and longevity amidst dramatic fluctuations in solar power, control strategies are needed to limit the temperature gradients and rates of temperature change within the SOEC. Recognizing that the reactant supply influences the current, a novel control strategy is developed to modulate heat generation in the SOEC by adjusting the fuel flow rate. The effectiveness of this strategy is assessed through numerical simulations conducted on a coupled PV-SOEC system using actual solar irradiance data, recorded at two-second intervals, to account for rapid changes in solar exposure. The results indicate that conventional control strategies, which increase airflow rates, are inadequate in effectively suppressing the rate of temperature variation in scenarios of drastic solar power changes. In contrast, our proposed strategy demonstrates successful management of the SOEC's heat generation, thereby reducing the temperature gradient and rate of variation within the SOEC to below 5 K/cm and 1 K/min, respectively.

13. Applications of metal nanoclusters supported on the two-dimensional material graphene in electrocatalytic carbon dioxide reduction.

Abstract

Metal nanoclusters (MNCs) have been demonstrated to exhibit superior catalytic performance compared to single nanoparticles. This is attributed to their quantized electronic structure, unique geometrical stacking and abundant active sites. While the exposed metal atoms can markedly enhance the efficiency of catalysis, unfortunately, MNCs are susceptible to agglomeration, which impairs their catalytic activity and stability. Graphene is a two-dimensional material consisting of a single atomic layer formed by the hybridization of the s and p orbitals of carbon atoms. It exhibits stable physical and chemical properties and has an easily controllable structure, making it an ideal carrier for MNCs. When metal nanoclusters (MNCs) are loaded on a graphene substrate, the MNCs can form a stable binding site on the graphene substrate. Furthermore, the construction of a defective structure on the graphene substrate enables the formation of robust interactions between the metal atoms of the MNCs and the substrate, facilitating the rapid establishment of electron conduction pathways and markedly enhancing the electrocatalytic performance. This paper presents a review of the applications of metal nanoclusters supported on graphene skeletons in the field of the electrocatalytic CO2 reduction reaction (CO2RR). Firstly, we briefly introduce the reaction mechanism of the CO2RR, then we systematically discuss the synthesis strategies, properties and applications of metal nanoclusters in electrocatalytic carbon dioxide reduction from both experimental and theoretical perspectives, and lastly, we discuss the opportunities and challenges of metal nanocluster catalysts supported on carbon materials. 21. Arthroscopic Bone Grafting and Robot‐assisted Fixation for Scaphoid Nonunion.

Abstract

Objectives: Scaphoid nonunion remains a challenging problem to manage with no general consensus on its treatment recommendations. We propose a novel minimally invasive (MIS) technique of arthroscopic bone grafting (ABG) with robot‐assisted fixation for the treatment of scaphoid nonunions. Methods: Patients with radiographically proven scaphoid nonunion treated by this novel surgical technique were included. Following arthroscopic debridement and iliac crest bone grafting, the scaphoid was fixed percutaneously using either multiple Kirschner (K)‐wires or a headless compression screw using a robotic navigation system. Results: Six male patients with an average age of 29.2 years were enrolled. Four patients had scaphoid waist fractures, and the other two were proximal pole fractures. During wrist arthroscopy, punctate bleeding of the proximal scaphoid fragment was observed in four out of the six patients. Half of the patients were fixed using a headless compression screw and the other half using multiple K‐wires. All the guidewires were placed with a single‐attempt using the robotic navigation system. Postoperatively, all the scaphoid fractures had complete radiographic union by 16 weeks. At a mean follow‐up of 18.3 months, there were significant improvements in wrist range of motion, grip strength, and patient‐rated outcomes. No intraoperative or early postoperative complications were encountered in any of our patients. Conclusion: Arthroscopic bone grafting with robot‐assisted fixation is a feasible and promising therapeutic option for scaphoid nonunions, regardless of the vascularity of the proximal pole fragment. This novel technique allows for anatomic restoration of the scaphoid alignment and accurate, targeted placement of implants into the scaphoid nonunion site within a single‐attempt using a robotic navigation system. Abstract

This study aimed to develop and evaluate a convolutional neural network for identifying scaphoid fractures on radiographs. A dataset of 1918 wrist radiographs (600 patients) was taken from an orthopaedic referral centre between 2010 to 2020. A YOLOv3 and a MobileNetV3 convolutional neural network were trained for scaphoid detection and fracture classification, respectively. The diagnostic performance of the convolutional neural network was compared with the majority decision of four hand surgeons. The convolutional neural network achieved a sensitivity of 82% and specificity of 94%, with an area under the receiver operating characteristic of 92%, whereas the surgeons achieved a sensitivity of 76% and specificity of 96%. The comparison indicated that the convolutional neural network's performance was similar to the majority vote of surgeons. It further revealed that convolutional neural network could be used in identifying scaphoid fractures on radiographs reliably, and has potential to achieve the expert-level performance. Abstract

We investigated abnormal MRI findings of the triangular fibrocartilage complex in 154 asymptomatic volunteers (21–79 years). Except prevalence, we focused on the morphological features of abnormal signals in relation to age. The majority of full-thickness tears were located in the articular disc (63 participants). The incidence of disc perforation with characteristics of ulnar impaction syndrome increased significantly with age. Asymptomatic full-thickness tears of the ulnar attachment were found in ten participants (seven over 60 years old). The proximal and distal laminae of the ulnar attachment could not be differentiated in 36 participants. In conclusion, MRI is of limited value for the elderly in diagnosing triangular fibrocartilage disorders. For young subjects, MRI is still valuable, especially in diagnosing ulnar detachment, although the ability to distinguish between proximal and distal laminae remains questionable. Disc perforations in volunteers mimicked ulnar impaction syndrome, therefore age, clinical signs and other factors should also be considered in clinical diagnosis. Abstract

Introduction Congenital proximal radioulnar synostosis (CPRUS) is a relatively rare forearm deformity that is characterized by a fixed pronated forearm of varying severity. The osseous synostosis between the proximal part of the ulna and the radius can be seen on the X-ray images in most cases. Many researchers have attempted to identify methods to measure the disease severity to guide in the treatment of CPRUS. However, to describe the overall deformity, the use of multiple indicators is essential, and some of these measurements require special software or need to be conducted on cadavers. Objective The aim of the current study was to introduce the radius pronation angle (RPA), a novel radiological evaluation index of CPRUS, and analyze the relationship between the RPA and the severity of the deformity. Methods Three-dimensional models of 43 CPRUS forearms (19 left forearms and 24 right forearms) of 32 patients (23 males and 9 females; average age was 6 years 8 months; range, from 1.5 to 27 years) treated at Beijing Ji Shui Tan Hospital during 2016 to 2019 were reconstructed using a computer-assisted technique. The special flexed posterior-anterior views of the X-ray image (the f-PA view) of the forearms were obtained, and the forearm rotation angle and the ulnar inner rotation angle were measured on each forearm. The RPA was measured on the f-PA view, and the lengths of the osseous synostosis, ulna, and the radial head were measured on the computed tomography scan images using the multiplanar reconstruction function. The Pearson index was analyzed between the RPA and the other measurements. Results The RPAs were correlated with the forearm rotation angle, ulnar inner rotation angle, relative length of the osseous synostosis, and the relative length of the radial head (P Conclusions The RPA can be measured quickly and easily on the f-PA view of the X-ray image and can be used as a reliable indicator of the severity of CPRUS.

40. Donor Site Evaluation after Full-Width Metatarsal Transfer for Metacarpal Reconstruction in Severe Thumb Hypoplasia: A Clinical Retrospective Study

Abstract

Background: Pollicization remains the default treatment for severe thumb hypoplasia, while the metatarsal transfer is an effective choice for metacarpal reconstruction, with competitive outcome of reconstructed thumb. However, rare researches focused on the outcome of donor foot of the metatarsal transfer. The purpose of this study was to evaluate the short-term outcome of donor foot after full-width metatarsal transfer for Manske Type IIIB and IV thumb hypoplasia.Methods: Fourteen patients with Manske Type IIIB and IV thumb hypoplasia were enrolled in the study. A full-width metatarsal graft (vascularised, from the second metatarsal or non-vascularised, from the fourth metatarsal) was harvested; an adjacent half-width graft from the third metatarsal was transposed horizontally to replace the donor defect. Objective and subjective evaluations were performed at least 12 months postoperatively. The appearances of the toes and the radiographic findings of the metatarsals were evaluated by two authors, the length and width of the metatarsals were measured on digital anteroposterior radiographs using a paired sample t test. The parental satisfaction was evaluated using a self-administered questionnaire. Results: Among these 14 patients (eight type ⅢB and six type Ⅳ thumbs), there were ten male and four female patients. The mean age at operation was 34.2 months (range: 14 to 127 months), and the mean follow-up was 16.1 months (range: 12 to 30 months). Toe lengthening, toe overriding, and synostosis between the metatarsals were observed in two, two and two patients; toe shortening, toe deviation, metatarsal lengthening and metatarsal angulation were observed in one, one, one and one patient. The lengths of third and fourth metatarsals in donor site were significantly larger than those in contralateral side, while the widths of third and fourth metatarsals were not significantly different between donor and contralateral sides. The mean score in the parental satisfaction questionnaire (full score, 30) was 27.2, (range: 17 to 30), which rated as “good”. Conclusions: Following full-width metatarsal transfer for Type IIIB and IV thumb hypoplasia, transposition of the adjacent half-width metatarsal for donor reconstruction results in minor donor deficit and overall parenteral satisfaction. 46. Consecutive flap transfer for repairing massive soft tissue defects in the opisthenar with improved donor site closure

Abstract

【Abstract】Objective: To explore a surgical model of utilizing consecutive free scapular flap and adjacent pedicled fl ap transfer for repairing massive soft tissue defects on the dorsum of the hand while minimizing the donor site morbidity. Methods: Six patients with massive soft tissue injuries on the opisthenar and forearm were treated with free scapular flaps. Afterwards, a pedicled flap adjacent to the donor site was transferred to cover the donor site defect by direct closure. Results: All six free scapular flaps survived without signs of infection. Three adjacent pedicled fl aps presented minor signs of insufficient blood flow on the distal apex, which resolved after six weeks with only conservative therapy. All the incisions healed without other complications. At six-month follow-up, the patients regained full shoulder function. Conclusion: With the assistance of an adjacent pedicled flap, the scapular flap is a highly applicable approach in repairing massive soft tissue defects in theopisthenar.
