Differences in patient characteristics between the PTVT and TVT Studies, including the level of preoperative intraocular pressure, contributed to the differences in surgical failure rates in each trial.

Objectives: Allergic fungal rhinosinusitis (AFRS) often results in expansion of disease beyond the paranasal sinuses, which may put important structures, such as the anterior ethmoid artery (AEA) or lateral lamella of the cribiform, at risk of injury during endoscopic sinus surgery (ESS). This study aims to compare the AEA to skull base (AEA-SB) length in patients with AFRS versus chronic rhinosinusitis with nasal polyps (CRSwNP), as well as additional anatomic variants. Methods: A single institutional retrospective chart review of patients undergoing ESS for AFRS and CRSwNP was performed. AEA-SB length were compared between the 2 groups. Other anatomic variants, including Keros measurement and presence of supraorbital ethmoid air cells (SOEC), concha bullosa (CB), sphenoethmoidal, and infraorbital ethmoid cells were measured and compared between the 2 groups. Results: Twenty-one patients were included in each cohort. The AFRS group was younger in age (P =.015) and had a significantly longer AEA-SB length (P =.014) compared to the CRSwNP group. No significant differences were observed between the 2 groups regarding Keros measurement, presence of concha bullosa, infraorbital ethmoid, sphenoethmoidal, or SOEC. No association was seen between AEA-SB length and Keros class in either group. Conclusions: AFRS harbors anatomical differences when compared to CRSwNP, with the former associated with a longer AEA-SB length. To investigate macula and optic nerve head (ONH) mitochondrial metabolic activity using flavoprotein fluorescence (FPF) in normal, glaucoma suspect (GS), and open-angle glaucoma (OAG) eyes we performed a cross-sectional, observational study of FPF in normal, GS, and OAG eyes. The macula and ONH of each eye was scanned and analyzed with a commercially available FPF measuring device (OcuMet Beacon, OcuSciences Inc., Ann Arbor, MI). One-way analysis of variance was used to compare macula and ONH FPF scores between groups. Linear regression models investigated the correlation between FPF scores and structural and functional parameters. We included 25 normal, 16 GS, and 54 OAG eyes. The average age in years ± SD for normal, GS, and OAG groups was 60.6 ±17.4, 67.8 ± 10.3, and 67.9 ± 11.6, respectively (P = 0.064). There was no significant difference in gender, race/ethnicity, visual acuity, and intraocular pressure between groups. OAG eyes had larger cup-to-disc ratio, thinner retinal nerve fiber and macula thicknesses, and worse visual field indices compared to normal and GS eyes (P ≤ 0.018). There was no significant difference in any FPF metric between the study groups in either the macula or the ONH, despite normalizing FPF data for structural differences between groups (e.g. retinal nerve fiber layer and ganglion cell-inner plexiform layer thickness). In conclusion, no significant differences in metabolic activity as measured by FPF were found in macula and ONH FPF scores using the integrated clinician report generator between normal, GS, and OAG eyes. Agentic women are frequently stereotyped negatively in politics. While public opinion suggests higher levels of egalitarian gender attitudes toward women in politics, these analyses miss important variations in explicit sexism. Using a survey experiment in the Canadian multi-party context, we show that for those reporting high levels of explicit sexism, agentic women are, as potential candidates, disadvantaged compared to similarly agentic men. Given the uneven distribution of sexist attitudes among partisans, these results suggest that gender representation differences between parties can be traced both to party recruitment strategies and the political behavior of party members. Public trust and perceptions of institutional legitimacy are vital to the functioning of the federal court system, and recent work challenges the long-standing belief that these attitudes are relatively stable in the populace. We posit that one threat to perceptions of trust and legitimacy is the lack of representation for women in the federal judiciary. Using a series of experiments, we show that, while women desire descriptive representation, this is an insufficient condition for preserving support for the institution. Substantive representation on issues critical to women leads to significantly increased trust and legitimacy and lowered perceptions of institutional bias among women. While female judges may bring their own social identities to bear in their decision-making, our work suggests that efforts to diversify the federal judiciary, while important, are unlikely to bolster public support for the courts if the new judges and justices fail to substantively represent the communities of interest. Objective: Critical components of the nasal endoscopic examination have not been definitively established for either the normal examination or for clinical disorders. This study aimed to identify concordance among rhinologists regarding the importance of examination findings for various nasal pathologies. Study Design: A consortium of 19 expert rhinologists across the United States was asked to rank the importance of findings on nasal endoscopy for 5 different sinonasal symptom presentations. Setting: An online questionnaire was distributed in July 2023. Methods: The questionnaire utilized JotForm® software and featured 5 cases with a set of 4 identical questions per case, each covering a common indication for nasal endoscopy. Rankings were synthesized into Normalized Attention Scores (NASs) and Weighted Normalized Attention Scores (W‐NASs) to represent the perceived importance of each feature, scaled from 0 to 1. Results: General concordance was found for examination findings on nasal endoscopy within each case. The perceived features of importance differed between cases based on clinical presentation. For instance, in evaluating postnasal drip, the middle meatus was selected as the most important structure to examine (NAS, 0.73), with mucus selected as the most important abnormal finding (W‐NAS, 0.66). The primary feature of interest for mucus was whether it was purulent or not (W‐NAS, 0.67). Similar analyses were performed for features in each case. Conclusion: The implicit framework existing among rhinologists may help standardize examinations and improve diagnostic accuracy, augment the instruction of trainees, and inform the development of artificially intelligent algorithms to enhance clinical decision‐making during nasal endoscopy. PurposeTo compare the late complications in the Ahmed Baerveldt Comparison Study during 5 years of follow-up.DesignMulticenter, prospective randomized clinical trial.Methodssetting: Sixteen international clinical centers.Study populationTwo hundred seventy-six subjects aged 18-85 years with previous intraocular surgery or refractory glaucoma with intraocular pressure of >18 mm Hg.InterventionsAhmed Glaucoma Valve FP7 or Baerveldt Glaucoma Implant BG 101-350.Main outcome measuresLate postoperative complications (beyond 3 months), reoperations for complications, and decreased vision from complications.ResultsLate complications developed in 56 subjects (46.8 ± 4.8 5-year cumulative % ± SE) in the Ahmed Glaucoma Valve group and 67 (56.3 ± 4.7 5-year cumulative % ± SE) in the Baerveldt Glaucoma Implant group (P = .082). The cumulative rates of serious complications were 15.9% and 24.7% in the Ahmed Glaucoma Valve and Baerveldt Glaucoma Implant groups, respectively (P = .034), although this was largely driven by subjects who had tube occlusions in the 2 groups (0.8% in the Ahmed Glaucoma Valve group and 5.7% in the Baerveldt Glaucoma Implant group, P = .037). Both groups had a relatively high incidence of persistent diplopia (12%) and corneal edema (20%), although half of the corneal edema cases were likely due to pre-existing causes other than the aqueous shunt. The incidence of tube erosion was 1% and 3% in the Ahmed Glaucoma Valve and Baerveldt Glaucoma Implant groups, respectively (P = .04).ConclusionsLong-term rates of vision-threatening complications and complications resulting in reoperation were higher in the Baerveldt Glaucoma Implant than in the Ahmed Glaucoma Valve group over 5 years of follow-up.
