2. Implementation of investigator-led drug clinical trial recruitment

Abstract

Objective Taking the implementation of recruitment for a drug clinical trial as an example, to discuss the implementation method of investigator-led recruitment for drug clinical trials from the perspective of the investigator’s organization, with a view to providing a reference for optimizing the recruitment for clinical trials as well as strengthening the construction of the research team. Methods From April 14, 2020 to July 4, 2022, General Hospital of Eastern Theater Command conducted an international multicenter phase Ⅲ clinical trial for the EMPA-KIDNEY study, with no third-party involvement in the recruitment process, which was led by the investigator. The study recruited subjects from the Chronic Kidney Disease Patient Registry Study patient pool, on-site recruitment, physician referral, and database screening, and took measures such as developing a recruitment system, clarifying personnel responsibilities to improve team efficiency, and strengthening quality control to manage recruitment. Results A total of 356 subjects were screened and 305 subjects were enrolled in the study, successfully completing the recruitment plan. Among them, 196 subjects were recruited through the “registration study”, accounting for two-thirds of the EMPA enrolment, while the rest of the patients were recruited on-site, referred by the clinic and screened by the database. “The enrolment study” achieved good recruitment results, but the number of outpatient referrals and the success rate of enrolment were lower than expected. Conclusion Compared to the third-party recruitment team, the investigators are more professional in their understanding of the protocol and in their control of the enrolment criteria, and are also more likely to gain the trust of the patients and be more familiar with the hospital environment, and therefore, have an advantage in the implementation of recruitment in hospitals. Researchers should not transfer their functions to recruiting companies or clinical research assistants, but should be more involved in the recruitment process. Recruitment for clinical trials is affected by a variety of factors such as protocol design, team organization and recruitment strategy, so it is important to establish an efficient research team, choose an appropriate recruitment strategy, and strengthen recruitment management for the implementation of recruitment.

8. Robust proteome profiling of cysteine-reactive fragments using label-free chemoproteomics.

Abstract

Identifying pharmacological probes for human proteins represents a key opportunity to accelerate the discovery of new therapeutics. High-content screening approaches to expand the ligandable proteome offer the potential to expedite the discovery of novel chemical probes to study protein function. Screening libraries of reactive fragments by chemoproteomics offers a compelling approach to ligand discovery, however, optimising sample throughput, proteomic depth, and data reproducibility remains a key challenge. We report a versatile, label-free quantification proteomics platform for competitive profiling of cysteine-reactive fragments against the native proteome. This high-throughput platform combines SP4 plate-based sample preparation with rapid chromatographic gradients. Data-independent acquisition performed on a Bruker timsTOF Pro 2 consistently identified ~23,000 cysteine sites per run, with a total of ~32,000 cysteine sites profiled in HEK293T and Jurkat lysate. Crucially, this depth in cysteinome coverage is met with high data completeness, enabling robust identification of liganded proteins. In this study, 80 reactive fragments were screened in two cell lines identifying >400 ligand-protein interactions. Hits were validated through concentration-response experiments and the platform was utilised for hit expansion and live cell experiments. This label-free platform represents a significant step forward in high-throughput proteomics to evaluate ligandability of cysteines across the human proteome.This paper presents a label-free chemoproteomics platform using data-independent acquisition to profile covalent fragment binding across the human proteome. 10. Diameter thresholds for pure ground-glass pulmonary nodules at low-dose CT screening: Chinese experience

Abstract

BackgroundLimited research exists on screening thresholds for low-dose CT in detecting malignant pure ground-glass lung nodules (pGGNs) in the Chinese population.Materials and methodsA retrospective analysis of the Guangzhou Lung-Care programme was conducted, retrieving average transverse diameter, location, histopathology, frequency and follow-up intervals. Diagnostic performances for ‘lung cancers’ were evaluated using areas under the curve (AUCs), decision curve analysis (DCA), sensitivities and specificities, with thresholds ranging from 5 mm to 10 mm. We divide malignant pGGNs into three groups: (1) minimally invasive adenocarcinoma (MIA) and invasive adenocarcinoma (IA), (2) atypical adenomatous hyperplasia (AAH) and adenocarcinoma in situ (AIS) and MIA and IA and (3) IA-only.ResultsIn ‘MIA+IA’, increasing the threshold from 5 mm to 8 mm improved specificity (60.97% to 88.85%, p<0.001) and positive predictive values (PPVs; 5.87% to 14.88%, p<0.001), but decreased sensitivity (94.44% to 75.56%, p<0.001). Further raising threshold from 8 mm reduced sensitivity (75.56% to 60.00%, p<0.001), while slightly increasing specificity (88.85% to 93.47%, p<0.001) and PPVs (14.88% to 19.15%, p<0.001). Increasing threshold from 5 mm to 7 mm enhanced the AUC for ‘MIA+IA’ (from 0.711 to 0.829), ‘AAH+AIS+MIA+IA’ (from 0.748 to 0.804) and ‘IA-only’ (from 0.783 to 0.833). At 8 mm, the AUCs for these categories were similar. However, increasing the threshold from 7 mm to 10 mm resulted in reduced AUCs for ‘MIA+IA’ (0.829 to 0.767), ‘AAH+AIS+MIA+IA’ (0.804 to 0.744) and ‘IA-only’ (0.833 to 0.800). DCA reveals that the 8 mm predictive model demonstrates greater clinical utility compared with models with other thresholds.ConclusionsIncreasing the diameter threshold for positive results for pGGNs, up to 8 mm could enhance diagnostic performance.Trial registration numberNCT04938804.

17. Targeted Delivery of Quinoxaline-Based Semiconducting Polymers for Tumor Photothermal Therapy

Abstract

Photothermal therapy (PTT) holds great potential in the field of cancer treatment due to its high specificity and low invasiveness. However, the low conversion efficiency, inadequate tumor accumulation, and limited cellular uptake continue to impede PTT effectiveness in treating tumors. The present study focuses on the utilization of quinoxaline and its nanoparticles to develop an organic semiconducting photothermal agent (PAQI-BDTT) for tumor photothermal therapy. To achieve this, PAQI-BDTT was encapsulated within liposomes modified with cyclic Arg-Gly-Asp (cRGD) peptide targeting tumors (named T-BDTT-Lipo). Notably, T-BDTT-Lipo demonstrated a positive photothermal conversion efficiency of 74% when exposed to an 808 nm laser, along with NIR-II fluorescence imaging capabilities. The efficacy of T-BDTT-Lipo in tumor tissue accumulation and precise targeting of malignant cells has been confirmed through both in vitroand in vivoexperiments guided by fluorescence imaging. Under single dose and 808 nm light irradiation, T-BDTT-Lipo generated local intracellular hyperthermia at the tumor site. The elevated temperature additionally exerted a significant inhibitory effect on tumor growth and recurrence, thereby extending the survival duration of mice harboring tumors. The therapeutic nanosystem (T-BDTT-Lipo) proposed in this work demonstrates the enormous potential of semiconducting photothermal agents in photothermal therapy, laying the foundation for the next clinical application.

Abstract

Aims The activation of nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3) inflammasome in endothelial cells (ECs) contributes to vascular inflammation in atherosclerosis. Considering the high glycolytic rate of ECs, we delineated whether and how glycolysis determines endothelial NLRP3 inflammasome activation in atherosclerosis. Methods and results Our results demonstrated a significant up-regulation of 6-phosphofructo-2-kinase/fructose-2,6-biphosphatase 3 (PFKFB3), a key regulator of glycolysis, in human and mouse atherosclerotic endothelium, which positively correlated with NLRP3 levels. Atherosclerotic stimuli up-regulated endothelial PFKFB3 expression via sterol regulatory element-binding protein 2 (SREBP2) transactivation. EC-selective haplodeficiency of Pfkfb3 in Apoe−/− mice resulted in reduced endothelial NLRP3 inflammasome activation and attenuation of atherogenesis. Mechanistic investigations revealed that PFKFB3-driven glycolysis increased the NADH content and induced oligomerization of C-terminal binding protein 1 (CtBP1), an NADH-sensitive transcriptional co-repressor. The monomer form, but not the oligomer form, of CtBP1 was found to associate with the transcriptional repressor Forkhead box P1 (FOXP1) and acted as a transrepressor of inflammasome components, including NLRP3, caspase-1, and interleukin-1β (IL-1β). Interfering with NADH-induced CtBP1 oligomerization restored its binding to FOXP1 and inhibited the glycolysis-dependent up-regulation of NLRP3, Caspase-1, and IL-1β. Additionally, EC-specific overexpression of NADH-insensitive CtBP1 alleviates atherosclerosis. Conclusion Our findings highlight the existence of a glycolysis-dependent NADH/CtBP/FOXP1-transrepression pathway that regulates endothelial NLRP3 inflammasome activation in atherogenesis. This pathway represents a potential target for selective PFKFB3 inhibitors or strategies aimed at disrupting CtBP1 oligomerization to modulate atherosclerosis. [ABSTRACT FROM AUTHOR]

Abstract

The aim of our study was to assess the associations of HSP90AB1 copy number variations (CNVs) with systemic lupus erythematosus (SLE) risk and glucocorticoids (GCs) efficacy, as well as the relationship between HSP90AB1 single-nucleotide polymorphisms (SNPs) and GCs efficacy. HSP90AB1 CNVs and SLE risk were analysed in 519 patients and 538 controls. Patients treated with GCs were followed up for 12 weeks and were divided into sensitive and insensitive groups to investigate the effects of CNVs (419 patients) and SNPs (457 patients) on the efficacy of GCs. Health-related quality of life (HRQoL) was also measured by SF-36 at baseline and week 12 to explore the relationship between CNVs/SNPs and HRQoL improvements in Chinese SLE patients. Our results indicated a statistically significant association between HSP90AB1 CNVs and SLE (PBH = 0.039), and this association was more pronounced in the female subgroup (PBH = 0.039). However, we did not detect association of HSP90AB1 CNVs/SNPs with efficacy of GCs. But we found a marginal association between SNP rs13296 and improvement in Role-emotional, while this association was not strong enough to survive in the multiple testing corrections. Collectively, our findings suggest that the copy number of HSP90AB1 is associated with SLE susceptibility. But copy number and polymorphisms of HSP90AB1 may not be associated with efficacy of GCs.
