Abstract Objective To provide an overview of the various treatment options available in the clinic to achieve periorbital rejuvenation. Data Sources Literature review and expert opinion. Conclusions Periorbital rejuvenation in the clinic can be accomplished through a variety of treatment modalities, including topical therapies, skin resurfacing, and fillers and injectables. Furthermore, some surgical approaches, such as upper blepharoplasties, ptosis, and brow lifts, can be performed in the clinic under local anesthesia with only mild oral sedation. However, the successful execution of such procedures depends on proper patient selection and maximizing patient comfort.

IntroductionTo investigate the therapeutic effect of clitoris exposure + episioplasty + dermabrasion + platelet-rich plasma (PRP) injection + chemexfoliation on vulvar lichen sclerosus (VLS).MethodsTwenty children with VLS (under 14 years old) at our hospital from July 2020 to November 2022 were enrolled and treated with clitoris exposure + episioplasty + dermabrasion + PRP injection + chemexfoliation. Additionally, symptomatic changes and improvements in signs were recorded.ResultsSignificant therapeutic effects were achieved in all children enrolled in this study. The Cattanco score was 8.02 ± 1.22 points before surgery, 2.21 ± 0.70 points 3 months after surgery, and 2.61 ± 0.59 points 6 months after surgery, demonstrating that the score after surgery was significantly lower than that before surgery (p < 0.05). Mild complications (one case of mild vulvar swelling, one case of minor bleeding, and one case of superficial ulcer) were observed in three children after surgery, with an overall complication incidence of 15%; all complications were improved after the intervention, and no severe adverse reactions were observed. Since antiquity, chemical peelings are still widely used to treat skin aging. There are a variety of substances, techniques, and considerations for each type of peeling. However, this case series aims to present a non-described form of application of trichloroacetic acid (TCA) and retinoic acid to treat fine static wrinkles on the face. The technique consists of applying the TCA peeling directly to the facial rhytids and, subsequently, retinoic acid peeling all over the face for 6 hours.

Background: Chemical peeling is the controlled wounding of the epidermis and dermis for skin rejuvenation, involving the application of ablative agents to induce keratolysis and regeneration of damaged cell layers. Prolonged erythema is one complication of this procedure. We report the prevalence and probable etiology of prolonged facial erythema in a cohort of patients treated with medium‐depth chemical peels. Materials and Methods: A retrospective audit was conducted of all medium‐depth facial chemical peels performed at two major teaching hospitals. All patients had severe facial photodamage affecting at least 75% surface area of the face. The occurrence of prolonged erythema following this peel was then identified and analyzed. Results: Of our treatment cohort (n = 82, 51 women, 31 men) with 60 years mean (61.3 years for women, 56.7 years for men), 10 patients (12%; eight women, two men) experienced prolonged erythema beyond a month of treatment. Facial psoriasis was not apparent at the time of chemical peel but manifested as prolonged erythema beyond the expected timeframe following the procedure. Conclusion: When patients experience prolonged erythema beyond a month of treatment and fail to respond to standard treatments, clinicians should examine carefully for extra‐facial psoriasis prior to this procedure, and also consider facial psoriasis a possible cause of prolonged post‐peel erythema. BackgroundFew studies illustrate the contributions made by various specialties in pioneering commonly performed noninvasive and minimally invasive cosmetic and surgical procedures.ObjectiveTo evaluate the contributions made by various specialties in pioneering noninvasive and minimally invasive cosmetic and surgical procedures.Materials and methodsKey words using the Medical Search Headings Database were used to query in the Thomson Reuters Web of Science Database. The 25 most highly cited studies were sorted according to the citation number. The author specialty was identified and assigned scores based on the number of citations.ResultsDermatology was identified as the specialty with the greatest contribution to botulinum toxin treatments for rhytides, injectable fillers, laser treatments, chemical peels, and noninvasive body contouring. Neurology was identified as the leading specialty in botulinum toxin treatments for hyperhidrosis, and vascular surgery was identified as leading specialty in sclerotherapy. Plastic surgery was identified as having made the greatest contribution to hair transplantation, and liposuction.ConclusionDermatology was identified as the leading specialty for the majority of noninvasive and minimally invasive cosmetic procedures. Other specialties have also contributed important literature to this area.

Acne vulgaris is a common condition. Alpha hydroxy acid (AHA) peels have been previously recommended as an option for patients with intolerance to first‐line treatments especially when long‐term use. The safety and efficacy of AHA peels for acne have been established, but the factors influencing the ultimate effects are unknown. We recruited patients diagnosed with acne who were intolerant to or refused the first‐line treatments from July 2017 to December 2019 at our hospital and retrospectively collected the medical and demographic information of patients treated with a full course of AHA peels; data collected included age, sex, treatment history, compliance status, and efficacy after treatment. The efficacy score was defined by revised scales: 2 points indicated significant improvement, 1 point indicated mild improvement, and 0 point indicated no improvement. Additionally, only efficacy scores of 2 points were classified in the satisfactory group; the others were classified in the unmet effect group. Analyses were used to evaluate the potential influencing factor(s). A total of 141 patients (120 females, 21 male) were included in the final analysis. The patients in the satisfactory group were significantly older (higher proportion aged >28 years) (47.4% vs 29.5%, P =.046) and showed better compliance (88.7% vs 54.5%, P =.001) than the patients in the unmet effect group. Multiple analyses confirmed the effects of younger age (OR 2.70, 95% CI 1.15‐6.34, P = 022) and poor compliance (OR 2.74, 95% CI 1.74‐4.32, P =.001) on the unmet effect. The aim of the study is to evaluate the effectiveness of water as a substitute for sodium bicarbonate plus solution in the neutralisation process of chemical peeling using a 35% glycolic acid solution. This randomised, double‐blind, split‐face, controlled trial was conducted in an outpatient setting. A total of 126 healthy patients with skin phototypes IV–V aged between 18 and 60 years old were recruited. Chemical peeling was performed with 35% glycolic acid solution. One side of the face was neutralised with water, and the other side of the face was neutralised with a sodium bicarbonate plus solution. The main outcome of this study was measured by the degree of erythema, pruritus and pain scores. Significant difference in the pain score immediately after the neutralisation process of the chemical peeling with glycolic acid was seen. However, there were no significant differences in the degree of erythema or the pruritus score. These results indicate that water can be used as a substitute for sodium bicarbonate plus solution in the neutralisation process of chemical peeling with 35% glycolic acid in patients with skin phototypes IV–V. Background: This study was conducted to improve standards of care in the cosmetic treatment of sun damage, fine lines, and wrinkles. Chemical Peels and Neurotoxins have been traditionally used cosmetically as monotherapies. This study aimed to confirm that the same-day combination created no additional side effects while also improving outcomes., Methods: The multi-generational study enrolled 30 patients with Fitzpatrick I-VI representation. The Roberts Skin Type Classification System was used to establish baseline patient information. Patients were treated with a VI Peel&reg;, followed by Botox&reg;. Objectively, photographic matching, Wrinkle Severity Scale, Uniformity of Pigment Scale, and Skin Tone Scales were used to evaluate skin improvement. Patient questionnaires were issued to assess satisfaction., Results: Safety of the same-day combination was established with no adverse events reported. Improvements on the Wrinkle Severity Scale showed an average rating dropping from 1.46 to 0.59 representing a 60% improvement. Improvements on the Uniformity of Pigment Scale showed an average rating dropping from 2.27 to 0.92 representing a 59% improvement. Improvements on the Skin Tone Scale showed an average rating dropping from 2.35 to 0.71 representing a 70% improvement. Questionnaires correlated with objective findings with high satisfaction.&nbsp; Conclusion: This study confirmed the safety of the same-day combination. The efficacy of VI Peel & Botox same-day treatment was clinically proven by the improvements to Wrinkle Severity, Uniformity of Pigment, and Skin Tone via photographic matching. Cosmetic procedures involve the maintenance, restoration, or enhancement of one's physical appearance through surgical or medical techniques. Restorative or reconstructive procedures, on the other hand, are essential procedures that restore normal function or appearance to abnormal structures caused by trauma or infection. Cosmetic procedures are generally elective and may either be surgical or nonsurgical. Nonsurgical or minimally invasive cosmetic procedures include laser hair or tattoo removal, chemical peeling, micro-dermabrasion, and soft tissue augmentation with injectables. Introduction. Acne vulgaris is one of the most common skin diseases affecting more than 85% of individuals worldwide. The study aimed to compare the therapeutic efficacy of Salicylic acid peel and Salicylic acid - mandelic acid peel for treatment of acne scars. Material and Methods. A comparative study was conducted on 41 patients, divided into two groups based on treatment choice by the subjects. Group A (23) were treated with 30% salycilic acid peels, and group B (18) were treated with 20% salycilic acid and 10% mandelic acid peels at an interval of 2 weeks for three sessions. The physician's global assessment scale and clinical photographs were used at the end of three sessions. All the data were analysed using SPSS considering P<0.05 as significant. Results. Of the total 41 patients analyzed, the mean age in both the groups were comparable. (Group A: 20.4 years; Group B: 20.5 years). Duration of acne was >12 months in both the groups 13 (56.52%) & 10 (55.56%)). Thirteen patients (56.52%) in group A and 1 (5.56%) in group B had no side effects which was statistically significant (p=0.01). Using the physician's global assessment scale 51-75% (Good) an improvement was seen in grade I 4 (17%) and 18 (78%) in grade II in group A and 2 (11%) in grade I and 12 (67%) in grade II in group B, respectively which was statistically significant (p=0.001). Conclusion. Both agents showed almost equal efficacy in improving mild-to-moderate acne scars. Adverse effects were less with Salicylic acid peel compared with mandelic acid peel.. Melasma is an acquired, chronic, variable incidence hypermelanosis. It is characterized by macules located in areas exposed to the sun. It has been a pathology difficult to manage due to the low response to treatments, although chemical peels remain a key tool. Peelings based on azelaic acid, in concentrations greater than 20%, produce an induced chemical peel, with progressive precipitation of epidermal proteins with minimal melanocytic reaction, antiproliferative and cytotoxic action in melanocytes, with inhibition of mitochondrial oxidoreductase activity and synthesis of DNA reduces hyperpigmentation by inducing desquamation and an inhibitory effect on tyrosinase. Dermatoscopy and trichromatic analysis facilitate the characterization and monitoring of dermatosis. The development of a protocol that allows the quantification, location, and distribution of the pigment, thanks to the calculation of pixels in each RGB channel, would be very useful to establish an appropriate treatment. Abstract The tretinoin peel, also known as retinoic acid peel, is a superficial peeling often performed in dermatological clinics in Brazil. The first study on this was published in 2001, by Cuce et al., as a treatment option for melasma. Since then, other studies have reported its applicability with reasonable methodology, although without a consistent scientific background and consensus. Topical tretinoin is used for the treatment of various dermatoses such as acne, melasma, scars, skin aging and non-melanoma skin cancer. The identification of retinoids cellular receptors was reported in 1987, but a direct cause-effect relation has not been established. This article reviews studies evaluating the use of topical tretinoin as agent for superficial chemical peel. Most of them have shown benefits in the treatment of melasma and skin aging. A better quality methodology in the study design, considering indication and intervention is indispensable regarding concentration, vehicle and treatment regimen (interval and number of applications). Additionally, more controlled and randomized studies comparing the treatment with tretinoin cream versus its use as a peeling agent, mainly for melasma and photoaging, are necessary.

Facial resurfacing is a fundamental part of rejuvenation but it is often ignored because of the perceived difficulty. Lasers are an option, but they have proved inadequate for difficult rhytids (ie, perioral) both in quality and longevity. Croton oil peels can give excellent results with remarkable permanence. Once considered the standard for deep facial resurfacing, the classical Baker-Gordon phenol-croton oil peel has largely been replaced by formulas with lower concentrations of phenol and croton oil. The improved safety profile of deep peels has ushered in a new era in chemical peeling. Wrinkles can be improved and skin can be tightened with more subtle and natural results. No longer does a deep peel denote "alabaster white" facial depigmentation with complete effacement of wrinkles. Gregory Hetter's research showed that the strength and corresponding depth of penetration of the phenol-croton oil peel can be modified by varying the concentration of croton oil. Chemical peeling, or chemexfoliation, has been used for centuries to improve signs of ultraviolet light-induced sun damage. Over the last 30 years, the science behind chemical peeling has evolved, increasing our understanding of the role of peeling ingredients and treatment indications. The depth of peels is directly related to improved results and to the number of complications that can occur. Solar Lentigo, also known as senile spots, are pigmented lesions on the surface of the epithelial tissue. They are light brown, dark or black in color and appear in regions of the body most exposed to the sun, such as a face, back of hands and arms, neck and shoulders. In addition, they increase in number and size with aging, being more frequent in light skin. The aim of this study was to describe the mechanisms of action and the efficacy of the main technologies and procedures for treatment of senile spots. The present study is a bibliographical review that presents an overview of the alternatives available in aesthetic market for the treatment of solar lentigo. Intense Pulsed Light, Lasers, Chemical Peels, Ultrasonic Depigmentation and Microdermabrasion are some of the treatment available worldwide to treat senile solar lentigo. These resources associated or not, act superficially, as depigmenting therapy, bleaching and cell renewal, providing efficient results in the short and medium term. Studies suggest that these therapies combined with preventive maintenance with photoprotection and depigmenting may provide satisfactory results. The treatment of Solar Lentigo requires specific knowledge and professional qualification for clinical evaluation and elaboration of new treatment protocols. [ABSTRACT FROM AUTHOR]

Chemical peels are among the oldest and most used procedures in aesthetic medicine worldwide. Chemical peeling—or chemexfoliation—is a procedure wherein a chemical agent of a defined strength is applied to the skin, which causes a controlled destruction of the layers of the skin, and is followed by regeneration and remodelling, with improvement of texture and surface abnormality. The mechanical action of peeling, even when limited to the epidermis, is able to stimulate regeneration via pathways in the dermis. The depth of destruction depends on the substance used, its concentration and the number of layers applied. Actinic keratoses (AKs) are common precancerous skin lesions that primarily affect older adults due to cumulative sun exposure. Given the increased vulnerability of older adults to developing AKs, appropriate therapeutic strategies are crucial to prevent their progression to invasive squamous cell carcinoma. This comprehensive review aims to explore the various treatment modalities available for AKs in the elderly population, focusing on their efficacy, safety, and suitability for this specific age group. The article discusses topical treatments, cryotherapy, photodynamic therapy, chemical peels, and surgical interventions, providing a detailed analysis of their mechanisms of action, benefits, limitations, and considerations in geriatric patients. Periorbital hyperpigmentation (POH) is a common aesthetic concern that impacts patients' emotional well-being and quality of life. POH can be difficult to manage as the etiology is often multifactorial or difficult to elucidate. An understanding of different contributing factors and ability to classify hyperpigmentation can aid in the management of POH. Classification of POH is divided into pigmented, vascular, structural, and mixed subtypes. A wide array of treatment options has been proposed belying the challenges inherent to improving POH. Modalities vary from topical therapies, chemical peels, dermal fillers, and lasers, to surgical intervention. Postinflammatory hyperpigmentation (PIH) is an acquired, reactive hypermelanosis. It commonly occurs after various endogenous and exogenous factors. The treatment of PIH should be started treating the underlying condition. The therapeutic modalities are depigmenting topical medications, oral agents, peeling procedures and lasers. It is important to note that these treatments may also play a role in causing PIH and to use caution with all depigmenting modalities to prevent irritation and worsening. [ABSTRACT FROM AUTHOR]

Objective: To study the post-peeling analgesia with a deionized water-based gel and tea tree oil mask, the Water Jel® (WJ), a material that is already used in the initial approach of burn injuries since there are pain reduction reports due to the injury cooling and act as an inhibitor of local inflammatory response. Methods: The study was performed in 17 patients. Pain was evaluated from a comparison between right and left hemiface, one covered with the WJ mask and the other one with a saline covered mask without the active ingredients of the studied product, right after the trichloroacetic acid peeling. Results: The average scores of the two hemifaces on a numerical scale were similar before applying the WJ mask and the saline mask. This result remained on the evaluation according to faces pain scale. These variables were evaluated by the Wilcoxon test, which identified no significant difference between their right side and left side. Discussion: There are few studies discussing analgesic measures for this type of procedure. Studies demonstrated that heparin is quite effective in slowing down or disappearing this painful complain. Also, Burnshield® showed comparable efficacy to running water in the initial burn treatment, with better results after 24 hours of evolution. Conclusion: It was concluded that both hemifaces masks showed the same effect, neither in comparison provided greater analgesia than the other one. [ABSTRACT FROM AUTHOR]
