El peñasco (porción petrosa del hueso temporal) es una estructura anatómica compleja, que contiene muchas estructuras nobles. Las fracturas del peñasco son frecuentes y suelen deberse a accidentes de tráfico. Pocas veces son fracturas aisladas y una buena colaboración entre especialistas permite un tratamiento adecuado. El tratamiento inicial se basa en el manejo de los trastornos de las funciones vitales, pero no debe omitir la evaluación de la función facial. Las lesiones vasculares se deben sospechar y buscar desde el período inicial. Si existe una parálisis facial inmediata y completa, se debe plantear la indicación de un tratamiento quirúrgico de urgencia. Las afectaciones cocleovestibulares pueden requerir la administración de una corticoterapia sistémica en la fase inicial. Aparte de la sordera progresiva por fístula perilinfática, cuya evolución puede interrumpirse mediante una obliteración quirúrgica semiurgente, el tratamiento de las secuelas auditivas suele ser secundario, tras la reabsorción del hemotímpano. Las rupturas meníngeas con fístula de líquido cefalorraquídeo se resuelven espontáneamente en la gran mayoría de los casos. Está indicado el cierre quirúrgico de la ruptura si persiste la fístula más de 7-10 días para limitar el riesgo de meningitis. Se recomienda la vacunación en todos los casos. Pueden aparecer complicaciones tardías, como una meningitis o un colesteatoma, de forma diferida y justifican un seguimiento a largo plazo.

Le complicanze craniche ed endocraniche delle sinusiti acute sono gravate da un tasso di mortalità e di sequele non trascurabile. Devono essere ricercate davanti alla comparsa di qualsiasi segno neurologico in un paziente con sinusite acuta o di una cefalea che evolve in due fasi (cefalea localizzata che migliora inizialmente prima di peggiorare e diventare più diffusa). I seni frontali e sfenoidali sono quelli il più delle volte coinvolti. La TC dei seni e dell’encefalo con iniezione di mezzo di contrasto è l’esame di prima intenzione; può essere poi completata dalla risonanza magnetica (RM) cerebrale. Il trattamento, aggressivo, si basa solitamente sul drenaggio del seno colpito e su una terapia antibiotica ad ampio spettro, adattata in base ai prelievi batteriologici. A volte viene proposto un gesto endonasale, ma questo deve, se possibile, rimanere limitato, a causa del rischio di incidente iatrogeno in un contesto emorragico e infiammatorio. In alcune situazioni, deve essere eseguito anche il drenaggio neurochirurgico delle raccolte profonde. In tutti i casi, il miglioramento clinico deve essere raggiunto entro le 48 ore: in caso contrario, gli esami devono essere ripetuti e deve essere discusso un gesto complementare. Le sinusiti fungine invasive, in primis la mucormicosi, costituiscono un’entità separata. Devono essere prese in considerazione in presenza di qualsiasi sintomo di rinosinusite in un paziente immunocompromesso. Il loro trattamento prevede la correzione massima dell’immunodepressione, lo sbrigliamento ripetuto dei tessuti infetti e la somministrazione di antimicotici sistemici.

Las complicaciones craneales y endocraneales de las sinusitis agudas presentan una tasa de mortalidad y de secuelas no desdeñable. Se deben buscar ante la aparición de cualquier signo neurológico en un paciente con sinusitis aguda o cefalea que evoluciona en dos tiempos (cefalea localizada que mejora inicialmente antes de empeorar y hacerse más difusa). Los senos frontales y esfenoidales son los más frecuentemente implicados. La tomografía computarizada de los senos paranasales y del encéfalo con inyección de contraste es la prueba complementaria de elección; se puede completar con una resonancia magnética (RM) cerebral. El tratamiento, agresivo, se basa habitualmente en el drenaje del seno afectado y en una antibioticoterapia de amplio espectro, adaptada posteriormente en función del análisis de las muestras bacteriológicas. En ocasiones se recomienda una intervención endonasal, pero debe estar limitada, siempre que sea posible, debido al riesgo de accidente iatrogénico en un contexto hemorrágico e inflamatorio. En algunos casos, se debe realizar también un drenaje neuroquirúrgico de las colecciones purulentas profundas. En cualquier caso, se debe observar mejoría clínica en 48 horas: en caso contrario, se deben repetir las pruebas de imagen y considerar la realización de una intervención complementaria. Las sinusitis fúngicas invasivas, entre las cuales la mucormicosis se encuentra en primer lugar, constituyen una entidad aparte. Deben sospecharse ante cualquier síntoma rinosinusal en un paciente inmunodeprimido. Su tratamiento incluye la máxima corrección de la inmunodepresión, el desbridamiento repetido de los tejidos infectados y la administración de antifúngicos por vía sistémica.

Actuellement, le cancer localisé de la corde vocale est traité par radiothérapie ou par chirurgie endoscopique. Ces deux modalités ont prouvé leur intérêt chez le patient âgé, la place exacte de chaque traitement est peu documentée. Cette étude évalue notre prise en charge des patients très âgés, atteints d’un cancer des cordes vocales.

Il peut être utile de rechercher un effet traitement différent dans des sous-groupes, lorsque l'on utilise des données observationnelles avec un critère de jugement censuré à droite, et un score de propension (SP). Cependant, des questions subsistent quant à la mise en œuvre du SP, notamment lorsque les sous-ensembles sont très déséquilibrés en termes de caractéristiques pronostiques et de prévalence du traitement.

Frequenti, le fratture della rocca sono il più delle volte dovute a incidenti stradali. Si tratta raramente di fratture isolate e una buona cooperazione tra le specialità consente una gestione adeguata. L’assistenza iniziale si basa sulla gestione dei deficit vitali, ma non deve tralasciare la valutazione della funzione facciale. Il danno vascolare deve essere sospettato e ricercato fin dal periodo iniziale. In caso di paralisi facciale immediata e completa, deve essere discussa l’indicazione al trattamento chirurgico d’urgenza. Il danno cocleovestibolare può richiedere una terapia corticosteroidea sistemica nella fase iniziale. A parte la sordità progressiva dovuta a una fistola perilinfatica, la cui progressione può essere arrestata mediante sigillatura chirurgica in semiurgenza, la gestione delle sequele uditive è il più delle volte secondaria, dopo il riassorbimento dell’emotimpano. Le lesioni meningee con perdite di liquido cerebrospinale si seccano spontaneamente nella stragrande maggioranza dei casi. La chiusura chirurgica della breccia se il flusso persiste oltre i 7-10 giorni resta indicata per limitare il rischio di meningite. La vaccinazione è consigliata in tutti i casi. Complicanze tardive come il meningocele o il colesteatoma possono apparire in maniera ritardata e richiedere un follow-up a lungo termine.

Spatial gene expression in the intestine is mediated by specific regulatory sequences. The three genes of the apoA-I/C-III/A-IV cluster are expressed in the intestine following cephalocaudal and crypt-to-villus axes. Previous studies have shown that the -780/-520 enhancer region of the apoC-III gene directs the expression of the apoA-I gene in both small intestinal villi and crypts, implying that other unidentified elements are necessary for a normal intestinal pattern of apoA-I gene expression. In this study, we have characterized transgenic mice expressing the chloramphenicol acetyltransferase gene under the control of different regions of the apoC-III and apoA-IV promoters. We found that the -890/+24 apoC-III promoter directed the expression of the reporter gene in crypts and villi and did not follow a cephalocaudal gradient of expression. In contrast, the -700/+10 apoA-IV promoter linked to the -500/-890 apoC-III enhancer directed the expression of the reporter gene in enterocytes with a pattern of expression similar to that of the endogenous apoA-IV gene. Furthermore, linkage of the -700/-310 apoA-IV distal promoter region to the -890/+24 apoC-III promoter was sufficient to restore the appropriate pattern of intestinal expression of the reporter gene. These findings demonstrate that the -700/-310 distal region of the apoA-IV promoter contains regulatory elements that, in combination with proximal promoter elements and the -500/-890 enhancer, are necessary and sufficient to restrict apoC-III and apoA-IV gene expression to villus enterocytes of the small intestine along the cephalocaudal axis.

By use of immunodepletion studies, we characterized four monoclonal antibodies reactive with rabbit brush-border (BB) as specific for aminopeptidase N (AP), dipeptidylpeptidase IV (DPPIV), neutral endopeptidase (EP), and angiotensin-converting enzyme (ACE), and we used these antibodies for immunohistochemical detection of these four hydrolases. Expression within the kidney was studied by light and electron microscopy. All four hydrolases are expressed on the various segments of the proximal tubule. In addition, EP and DPPIV are detectable on visceral epithelial cells of the glomerulus and AP on the cells of Bowman's capsule. Outside the kidney, the four hydrolases are expressed within the digestive and genital tracts, where AP, EP, and DPPIV predominate on epithelial structures, whereas ACE is essentially located in vascular structures. The latter localization is also characteristic of ACE in the other organs studied, where clear-cut systematic distribution of the other hydrolases was often difficult to demonstrate. In addition, AP, DPPIV, and EP were detected on lymphoid cells. As compared to reports of data obtained essentially by enzymatic or immunoradiometric assays, these observations suggest considerable interspecies variations of extrarenal expression of the major BB hydrolases. This should be taken into account in attempting to define a general physiological role for a given enzyme.

Intermicrovillar areas and apical vesicles characterized by an extensive clathrin coat can be identified in some epithelial cell types. We describe a 280-kD protein, characteristic of these areas in the proximal tubule brush border and epithelial cells of the visceral yolk sac. When injected to 9-d pregnant rats, mAbs to the 280-kD protein regularly induced fetal resorption and/or malformations. Antibodies to a 330-kD protein that is also coated-pit-restricted had no effect. Our observations point to a key function for p280 and suggest that immunity to specific constituents of the receptor-mediated endocytotic system may be involved in the induction of fetal abnormalities.

We used a dual immunomorphological and physiological approach to demonstrate that the RC.SVtsA58 rabbit cortical cell line exhibits features of highly differentiated cortical collecting tubule (CCT) principal cells (PC). First, we raised monoclonal antibodies against RC.SVtsA58 cells and screened their reactivity with the rabbit kidney: three were specific for the basolateral domain of CCT PC and bound to 100% of RC.SVtsA58 cells. Second, we showed that bradykinin, atrial natriuretic peptide, and prostaglandin E2 increased intracellular Ca2+, guanosine 3',5'-cyclic monophosphate, and adenosine 3',5'-cyclic monophosphate (cAMP), respectively. In addition, 10 nM bradykinin inhibited desmopressin-elicited cAMP production by > or = 40%; this effect was suppressed by 10 microM of indomethacin and was reproduced with 1 nM of prostaglandin E2, indicating the conservation of arginine vasopressin-related regulatory loops described in microdissected CCT and freshly isolated cells. However, RC.SVtsA58 cells also express intercalated cell markers even after repeated cloning, which suggests that tsA58, a temperature-sensitive strain of simian virus-40, has transformed a multipotent type of PC in keeping with the cell interconversion hypothesis.
