Purpose To define the burden of morbidity and mortality arising from COVID-19 in individuals with primary (PID) and secondary immunodeficiency (SID) in the United Kingdom. Methods In March 2020, the United Kingdom Primary Immunodeficiency Network (UKPIN) established a registry of cases to collate the outcomes of individuals with PID and SID following SARS-CoV-2 infection and treatment. Anonymised demographic data, pre-SARS-CoV-2 infection lymphocyte counts, co-morbidities, targeted treatments and outcomes were collected. Three groups were analysed in further detail: individuals with common variable immunodeficiency (CVID), individuals with any PID, including CVID, receiving immunoglobulin replacement therapy (IgRT) and individuals with secondary immunodeficiency. Results A total of 310 cases of SARS-CoV-2 infection in individuals with PID or SID have now been reported in the UK. The overall mortality within the cohort was 17.7% (n = 55/310). Individuals with CVID demonstrated an infection fatality rate (IFR) of 18.3% (n = 17/93), individuals with PID receiving IgRT had an IFR of 16.3% (n = 26/159) and individuals with SID, an IFR of 27.2% (n = 25/92). Individuals with PID and SID, had higher inpatient mortality and died at a younger age than the general population. Increasing age, low pre-SARS-CoV-2 infection lymphocyte count and the presence of common co-morbidities increased the risk of mortality in PID. Access to specific COVID-19 treatments in this cohort was limited: only 22.9% (n = 33/144) of patients admitted to hospital received dexamethasone, remdesivir, an anti-SARS-CoV-2 antibody-based therapeutic (e.g. REGN-COV2 or convalescent plasma) or tocilizumab as a monotherapy or in combination. Dexamethasone, remdesivir and anti-SARS-CoV-2 antibody-based therapeutics appeared efficacious in PID and SID. Conclusion Compared to the general population, individuals with PID or SID are at high risk of mortality following SARS-CoV-2 infection. Increasing age, low baseline lymphocyte count and the presence of co-morbidities are additional risk factors for poor outcome in this cohort.

Background: The HIV epidemic poses significant challenges to the healthcare providers including Physiotherapists. The study aimed at investigating the extent to which Physiotherapy College students know about human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS) transmission, infection and prevention, and the transfer of such knowledge to sexual behavior change & and to know whether knowledge has any influence on the attitude and willingness to treat HIV/AIDS patients.Methods: A cross-sectional survey was carried out on 217 physiotherapy students studying at the Banarsidas Chandiwala Institute of Physiotherapy, Kalkaji, New Delhi, India. The students completed a predesigned self-administered questionnaire assessing the knowledge, attitude and willingness to treat HIV/AIDS patients. The data were analyzed using ANOVA test (all the results are calculated at 1% level of significance) and Pearson correlation test.Results: The total mean knowledge score was 73.23% (good knowledge). There was a significant difference in knowledge among the Fourth-year & first-year students, which was found to be statistically significant (P0.001%). Karl Pearson correlation test showed no significant correlation between the knowledge of HIV/AIDS and the attitude of students towards HIV/AIDS patients (P>0.01%).Conclusions: The findings suggest that although the students had adequate knowledge about HIV/AIDS, their attitude towards this group of people was negative. From the study, fear of HIV contagion was observed as a major reason for the negative attitude of students towards HIV/AIDS patients.

UNLABELLED CHARGE syndrome comprises coloboma of the eye, heart defects, choanal atresia, growth and developmental retardation, genitourinary anomalies and ear and hearing defects. The association between CHARGE syndrome and T-cell immunodeficiency is recognized, but has not been reported widely in the literature. We report four patients meeting the diagnostic criteria for CHARGE syndrome, who had moderate or severe T-cell lymphopenia complicated by infections. The patients presented in Leicester, UK, between 2000 and 2007. All patients were negative for 22q11.2 deletions by FISH analysis, but mutations in the CHD7 gene were identified in three patients in whom the analysis was performed. Our cases indicate that patients with CHARGE syndrome may have a spectrum of T-cell immune deficiency, and that this association may be more common than has previously been appreciated. We recommend that all patients diagnosed with CHARGE syndrome should have lymphocyte subsets evaluated as part of their initial investigation. CONCLUSION Thymic hypoplasia should be included in the clinical features associated with CHARGE syndrome. All patients with CHARGE syndrome should have lymphocyte subset analysis performed, to exclude T-cell immunodeficiency.

Objective: Constraint Induced Movement Therapy has been found to be a promising treatment for substantially improving the use of extremities affected by neurologic injuries such as stroke and traumatic brain injury in adults. The purpose of this study was to determine the applicability of a child friendly form of CI Therapy on young children with cerebral palsy. Method: Thirty Children with hemiplegic Cerebral Palsy age 4-8 years received CI Therapy and completed evaluations. They were randomized to either Group A or Group B. In both the groups children wore mitt as restraint on their non-involved upper extremity and the involved upper extremity was engaged into play & functional activities for six hours per day for ten week days. In Group A all the activities done by children were under the supervision of therapist where as Group B was more of home based program although activities were therapist guided. Changes in upper extremity function were evaluated with Quality of Upper Extremity Skills Test. The evaluations took place at the onset of intervention and after completing two weeks of intervention. Results: Children in both the groups demonstrated significant improved functional efficiency of their involved upper extremity (p Conclusion: Results suggest that the CI Therapy can improve motoric function among children with hemiparesis and that this efficacy is more in a therapist supervised CIMT program.
