Background: Acrylamide is classified as a probable human carcinogen by the IARC but epidemiological evidence on the carcinogenicity of acrylamide from dietary sources is limited. This study aimed to investigate the associations between dietary acrylamide and breast cancer risk in the NutriNet-Santé cohort. Methods: This prospective cohort study included 80,597 French women (mean [SD] age at baseline: 40.8 [14] years) during a mean (SD) follow-up of 8.8 (2.3) years. Acrylamide intake was evaluated using repeated 24h dietary records (n= 5.5 [SD 3.0]), linked to a comprehensive food composition database. Associations between acrylamide intake and breast cancer risk (overall, premenopausal and post-menopausal) were assessed by Cox hazard models adjusted for known risk factors. Results: The mean (SD) dietary acrylamide intake was 30.1 (21.9) µg/d (main contributors: coffee, potato fries and chips, pastries and cakes, and bread). During follow-up, 1016 first incident breast cancer cases were diagnosed (431 premenopausal, 585 postmenopausal). A borderline significant positive association was observed between acrylamide intake and breast cancer risk overall (HRQ4 vs Q1= 1.21 [95% CI: 1.00-1.47]) and a positive association was observed with premenopausal cancer (HRQ4 vs Q1= 1.40 [95% CI: 1.04-1.88]). Restricted cubic spline analyses suggested evidence for non-linearity of these associations, with higher HR for intermediate (Q2) and high (Q4) exposures. Receptor-specific analyses revealed a positive association with estrogen receptor-positive breast cancer, which represented 86% of total cancer cases. Acrylamide intake was not associated with post-menopausal breast cancer. Conclusions: Results from this large prospective cohort study suggest the potential deleterious role of dietary acrylamide in breast cancer etiology, especially in premenopausal women, and provide new insights that should encourage further mitigation strategies to reduce the content of acrylamide in food. OBJECTIVE The deleterious effects of trans fatty acids (TFAs) on cardiovascular health are well established; however, their impact on type 2 diabetes remains poorly understood. In particular, little is known about the impact of specific TFA types on type 2 diabetes etiology. We aimed to explore the associations between different types of TFAs (total, ruminant, industry produced [iTFAs], and corresponding specific isomers) and risk of type 2 diabetes. RESEARCH DESIGN AND METHODS A total of 105,551 participants age >18 years from the French NutriNet-Santé cohort (2009–2021) were included (mean baseline age 42.7 years; SD 14.6 years); 79.2% were women. Dietary intake data, including usual TFA intake, were collected using repeated 24-h dietary records (n = 5.7; SD 3.1). Associations between sex-specific quartile of dietary TFAs and diabetes risk were assessed using multivariable Cox models. RESULTS Total TFA intake was associated with higher type 2 diabetes risk (hazard ratio [HR]quartile 4 vs. 1 1.38; 95% CI 1.11–1.73; Ptrend < 0.001; n = 969 incident cases). This association, specifically observed for iTFAs (HR 1.45; 95% CI 1.15–1.83; Ptrend < 0.001), was mainly driven by elaidic acid (HR 1.37; 95% CI 1.09–1.72; Ptrend < 0.001) and linolelaidic acid (HR 1.29; 95% CI 1.04–1.58; Ptrend = 0.07). In contrast, ruminant TFAs were not significantly associated with risk of type 2 diabetes. CONCLUSIONS In this large prospective cohort, higher intakes of total and iTFAs were associated with increased type 2 diabetes risk. These findings support the World Health Organization’s recommendation to eliminate iTFAs from the food supply worldwide. Consumers should be advised to limit the consumption of food products containing partially hydrogenated oils (main vector of iTFAs). This may contribute to lowering the substantial global burden of type 2 diabetes.

Resume Au cours des dernieres decennies, l’offre agro-alimentaire s’est considerablement etoffee en matiere d’aliments dits « ultra-transformes » (AUT). Il s’agit de produits ayant subi d’importants procedes de transformation impactant fortement la matrice alimentaire et/ou contenant des additifs alimentaires ou autres substances d’origine industrielle (huiles hydrogenees, maltodextrine, sirop de glucose, etc.). Dans la cohorte NutriNet-Sante (2009-en cours), sept etudes ont ete realisees, sur des sous-echantillons allant de 26 000 a plus de 100 000 participants. Ces travaux ont mis en evidence des associations entre consommation d’AUT et incidences de cancers, maladies cardiovasculaires, mortalite, diabete de type 2, symptomes depressifs, surpoids et obesite, et presence de troubles fonctionnels digestifs. Les analyses tenaient compte de nombreux facteurs de confusion potentiels, y compris nutritionnels. Au-dela des aspects de qualite nutritionnelle, d’autres facteurs pourraient donc etre impliques, tels que certains additifs alimentaires, des composes neoformes lors des procedes de transformation, ou des contaminants issus des emballages. Ces resultats sont en coherence avec un nombre croissant d’etudes epidemiologiques et experimentales recentes a travers le monde qui suggerent un role etiologique des AUT dans la survenue de differentes maladies. Sur la base de ces travaux, les autorites de sante publique en France et dans divers pays recommandent aujourd’hui d’en limiter la consommation.

BACKGROUND Fermentable Oligosaccharides, Disaccharides and Monosaccharides And Polyols (FODMAPs) have been shown to be involved in gastrointestinal disorders. In view of their pro-inflammatory potential and their interactions with the gut microbiota, their contribution to the etiology of other chronic diseases such as cancer has been postulated. However, no epidemiological study has investigated this hypothesis so far. OBJECTIVE Our objective was to investigate the associations between FODMAP intake (total and by type) and cancer risk (overall, breast, prostate and colorectal) in a large prospective cohort. DESIGN The study was based on the NutriNet-Sante cohort (2009-2020); 104,909 adult participants without cancer at baseline were included in our analyses (median follow-up time = 7.7y, 78.7% women, mean age at baseline 42.1y (SD = 14.5)). Baseline dietary intakes were obtained from repeated 24h-dietary records linked to a detailed food composition table. Associations between FODMAP intake (expressed in quintiles, Q) and cancer risks were assessed by Cox proportional hazard models adjusted for a large range of lifestyle, sociodemographic and anthropometric variables. RESULTS Total FODMAP intake was associated with increased overall cancer risk (n = 3374 incident cases, HR for sex-specific Quintile 5 versus Quintile 1: 1.21; 95%CI: 1.02, 1.44; P-trend = 0.04). In particular, oligosaccharides were associated with cancer risk: a trend was observed for overall cancer (HR Q5 vs. Q1: 1.10; 95%CI: 0.97, 1.25; P-trend = 0.04) and colorectal cancer (n = 272, HR Q5 vs. Q1: 1.78; 95%CI: 1.13-2.79; P-trend = 0.02). CONCLUSION Results from this large population-based study on French adults from the NutriNet-Sante cohort show a significant association between FODMAP intake and the risk of cancer development. Further epidemiological and experimental studies are needed to confirm these results and provide data on the potential underlying mechanisms.

Background Dietary trans fatty acids (TFAs) are either natural (from ruminant sources) or generated through industrial processes by partial hydrogenation of vegetable oils. Extended evidence demonstrated their role as cardiovascular risk factors. In contrast, their involvement in cancer etiology is suspected, but epidemiological evidence remains limited. Objectives Our objective was to investigate associations between TFA intake of different types (total, rumninant [rTFAs], industrial [iTFAs], and corresponding specific isomers) and the risk of cancer (overall and main cancer sites) in the NutriNet-Santé prospective cohort (2009-2020). Methods Overall, 104,909 participants were included. Usual TFA intake was estimated from validated repeated 24-h dietary records matched with detailed composition table. Associations between sex-specific quartiles of TFA intake and cancer risk were assessed using multi-adjusted Cox proportional hazard models. Results A total of 3,374 incident cancer cases occured during follow-up (among which 982 breast and 405 prostate cancers). Dietary intake of total TFAs was associated with higher prostate cancer risk (HRfor quartile 4 versus 1: 1.27, 1.11-1.77 Ptrend=0.005). rTFAs were associated with increased overall cancer risk (1.16, 1.02-1.32 Ptrend=0.07), in particular the conjugated linoleic acid isomers (CLA) (1.19, 1.04-1.36 Ptrend=0.04). These associations were specifically observed for breast cancer (rTFAs: 1.35, 1.06-1.72 Ptrend=0.01; CLA: 1.29, 1.00-1.66 Ptrend=0.048), in particular before menopause (rTFAs: 1.68, 1.06-2.67 Ptrend=0.02; CLA: 2.013, 1.25-3.23 Ptrend=0.003). Several iTFAs were associated with overall (1.18, 1.06-1.31 Ptrend=0.02 for transdocosenoic acid), breast (isomer 18:2t: 1.30, 1.06-1.58 Ptrend=0.01; hexadecenoic acid: 1.28, 1.05-1.56 Ptrend=0.02) and prostate (transdocosenoic acid: 1.52, 1.09-2.12 Ptrend=0.07) cancer risks. Conclusion In this large prospective study, several types of TFAs were associated with increased overall, breast and prostate cancer risks. Although further studies are needed to better understand underlying mechanisms, these results support the WHO's goal of achieving industrially produced TFAs elimination from food supplies. Meanwhile, the consumption of food products containing partially-hydrogenated oils should be avoided. Background: Added sugars’ deleterious effects have been established for several chronic diseases, leading food industries to use artificial sweeteners as alternatives in a wide range of foods and beverages. Their safety is debated and findings remain contrasted regarding their role in the etiology of various diseases. In particular, their carcinogenicity has been suggested by several experimental studies but robust epidemiological evidence is lacking. Objective: The objective was to investigate the associations between sweetener intakes (total from all dietary sources, and most frequently consumed ones: aspartame e951, acesulfame-K e950 and sucralose e955) and cancer risk (overall and by sites: breast, prostate and obesity-related cancers). Design: Population based prospective cohort study. Setting and Participants: Overall, 102,046 adults from the French NutriNet-Santé prospective cohort (2009-2021) were included. Consumption of sweeteners was obtained by repeated 24h-dietary records including brands and commercial names of industrial products. Associations between sweeteners and cancer incidence were assessed by multi-adjusted Cox hazard models. Main outcome measures: Association between sweetener intakes and cancer risk were assessed by Cox proportional hazard models adjusted for known risk factors (sociodemographic, anthropometric, lifestyle, medical history, and nutritional factors). Results: Compared to non-consumers, higher consumers of total sweeteners (i.e. above the median exposure in consumers) had higher risk of overall cancer (n=2,527 cases, hazard ratio=1.12, 95% confidence interval=1.00-1.25, P-trend=0.005). In particular, aspartame (HR=1.20 [1.05-1.38] P=0.001) and acesulfame-K (HR=1.18 [1.04-1.34] P=0.003) were associated with increased cancer risk. Similarly, higher risks were observed for breast (n=723 cases, HR=1.25 [1.02-1.53] P=0.01 for total sweeteners, HR=1.33 [1.05-1.69] P=0.007 for aspartame and HR=1.39 [1.11-1.74] P=0.003, for acesulfame-K) and obesity-related cancers. (n=1,509 cases, HR=1.16 [1.00-1.33] P=0.02 for total sweeteners, HR=1.22 [1.02-1.45] P=0.01 for aspartame and HR=1.23 [1.04-1.45] P=0.01, for acesulfame-K). Conclusion: In this large prospective cohort, artificial sweeteners (especially aspartame and acesulfame-K), which are found in >10,000 foods and beverage references worldwide, were associated with increased cancer risk. These findings provide important and novel insights for the ongoing re-evaluation of food additive sweeteners by the European Food Safety Authority and other health agencies globally. Background Nitrates and nitrites occur naturally in water and soil and are commonly ingested from drinking water and dietary sources. They are also used as food additives in processed meats to increase shelf life and to avoid bacterial growth. These compounds could have a role in the carcinogenicity of processed meat. Objective To investigate the relationship between nitrate and nitrite intakes (distinguishing between natural food, water and food additive sources) and the risk of cancer in a large prospective cohort with detailed and up to date dietary assessment. Design Population based prospective cohort study. Setting Overall, 101,056 adults from the French NutriNet-Santé cohort study (2009-ongoing) were included. Consumption of nitrites and nitrates was evaluated using repeated 24h dietary records, linked to a comprehensive food composition database and accounting for details of commercial names/brands of industrial products. Main outcome measures Associations between nitrite and nitrate exposures and the risk of cancer (overall and by main cancer sites) were assessed by Cox hazard models adjusted for known risk factors. Results During follow-up, 3311 first incident cancer cases were diagnosed (among which 966 breast and 400 prostate cancers). Compared with non-consumers, higher consumers of nitrates from food additives had higher risk of breast cancer (HR=1.24 (1.03-1.48), P=0.02); this was more specifically observed for potassium nitrate (e252, HR=1.25 (1.04-1.50), P=0.01). Higher consumers of nitrites from food additives had higher risk of prostate cancer (HR=1.58 (1.14-2.18), P=0.008), specifically for sodium nitrite (e250, HR=1.62 (1.17-2.25), P=0.004). No significant association was observed for nitrates and nitrites from natural sources. Conclusions In this large prospective cohort, food additive nitrates were positively associated with breast cancer risk and food additive nitrites were positively associated with prostate cancer risk. While these results need confirmation in other large-scale prospective studies, they provide new insights in a context of lively debate around the ban of nitrite additives in the food industry. International audience; The impact of dairy product consumption for long-term health remains unclear, in particular regarding their involvement in cancer etiology for frequent locations like breast or prostate. Besides, little is known about potentially different effects of dairy product subtypes. Our objective was therefore to evaluate the associations between dairy product consumption (total and subtypes) and cancer risk. A total of 101 279 participants from the French NutriNet-Santé cohort study were included (78.7% women; mean [SD] age = 42.2 [14.5] years). Dairy product consumption was assessed using validated web-based 24-hour dietary records. Multiadjusted Cox models were computed. After a median [interquartile range] follow-up time of 5.9 [2.7-8.3] years, we documented 2503 incident cancer cases (783 breast, 323 prostate and 182 colorectal cancers). Total dairy product consumption was not significantly associated with cancer. However, the consumption of “fromage blanc” (a French type of quark/cottage cheese) was associated with an increased risk of cancer overall (HR for 1 serving increment [95% CI] = 1.11 [1.01-1.21]; P-trend = .03) and of colorectal cancer (HR = 1.39 [1.09-1.77]; P-trend < .01). Besides, sugary dairy dessert consumption was directly associated with colorectal cancer risk (HR for 1 serving increment = 1.58 [1.01-2.46]; P-trend = .046]. No association was observed between the consumption of dairy products or sugary dairy desserts and the risk of prostate and breast cancers. In our study, the consumption of dairy products was not associated with the risk of overall, colorectal, breast or prostate cancers. The consumption of “fromage blanc” and sugary dairy desserts were associated to an increased risk of colorectal cancer, but this warrants further investigations.

Fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAPs) are increasingly studied because they are suspected unfavorably to impact health (irritable bowel syndrome in particular). However, little is known about FODMAP intake in the general population, or which groups are more likely to consume them, because their intakes are usually assessed in inpatient settings.This study aimed to describe FODMAP consumption in a large French cohort and its association with sociodemographic and lifestyle characteristics.This cross-sectional study described FODMAP intakes in 109,362 volunteers (78.0% female, mean age 43.8 ± 14.7 y) from the French NutriNet-Santé cohort, using an ad hoc FODMAP composition table. Associations between FODMAP intakes and sociodemographic characteristics were investigated using χ2 tests or Kruskal-Wallis tests according to the qualitative or quantitative status of the variable, and multinomial logistic regressions were performed after adjusting for energy intake in sensitivity analyses. Eligible participants had completed ≥3 detailed 24-h food records.We observed a mean intake of 18.9 ± 9.5 g/d FODMAPs in this French cohort, and 11.7% of participants had intakes9 g/d (i.e., low-FODMAP diets). Participants with FODMAP intakes9 g/d were more likely to have lower caloric intakes (Δ = 383 kcal/d compared with participants with FODMAP intakes ≥16 g/d), to be smokers, to have lower incomes, and to have lower levels of physical activity. Total FODMAPs accounted for a mean intake of 18.9 ± 9.5 g/d, which was 3.7 ± 2.0% of total energy intake. The highest intake of FODMAPs was represented by lactose followed by excess fructose, fructans, polyols, and galacto-oligo-saccharides.FODMAP consumption by a large sample of adults from the general population is ∼19 g/d, with half of the population having a FODMAP intake16 g/d.This trial was registered at www.clinicaltrials.gov as NCT03335644.

Background Evidence is accumulating that high dietary glycaemic index (GI) and glycaemic load (GL) are potential risk factors for several metabolic disorders (e.g. type-2 diabetes, cardiovascular diseases), but remains limited concerning cancer risk. Although, mechanistic data suggest that consuming high-GI foods may contribute to carcinogenesis through elevated blood glucose levels, insulin resistance or obesity-related mechanisms. Our objective was to study the associations between dietary GI/GL and cancer. Methods In total, 103 020 French adults (median age = 40.2 years) from the NutriNet-Santé cohort (2009–2020) with no cancer or diabetes at baseline were included (705 137 person-years, median follow-up time = 7.7 years). Repeated 24-h dietary records linked with a detailed food-composition table (>3500 food/beverage items). We computed the average dietary GI and GL at the individual level. Associations between GI, GL, contribution of low- and medium/high-GI foods to energy and carbohydrate intake and cancer risk (overall, breast, prostate and colorectal) were assessed using multivariable Cox proportional-hazard models. Results Higher dietary GL was associated with higher overall cancer risk [n = 3131 cases, hazard ratios (HRs) for sex-specific quintile 5 vs 1 = 1.25, 95% confidence interval (CI) = 1.03–1.52; Ptrend = 0.008] and specifically postmenopausal breast cancer (n = 924, HRQ5vs.Q1 = 1.64, 95% CI = 1.06–2.55; Ptrend = 0.03). A higher contribution of low-GI food/beverages to energy intake was associated with lower cancer risk whereas a higher contribution of medium/high-GI items to energy intake was positively associated with higher risk of overall, breast and postmenopausal breast cancers (Ptrend ≤ 0.02). Conclusions These results support a possible impact of GI/GL on cancer risk. If confirmed in other populations and settings, dietary GI/GL could be considered as modifiable risk factors for primary cancer prevention. Objective:To assess in 2021 the acceptance and perception of the French tax on sweetened beverages, following its revision in 2018, and factors associated with a higher level of acceptance.Design:A cross-sectional survey within the NutriNet-Santé cohort study. Participants were invited to complete a self-reported questionnaire in March 2021. Weighting was applied to the sample to allow inferences on the French population. Individual characteristics associated with support for the tax were investigated using logistic regression modelling.Settings:NutriNet-Santé prospective cohort study.Participants:Adults engaged in the NutriNet-Santé cohort, aged 18 years or older (n 28 344), living in mainland France.Results:Almost two-thirds (63·4 %) of the participants were aware of the existence of a tax on sweetened beverages, although less than a quarter had specific knowledge regarding its design and the 2018 revision. In turn, 64·7 % of participants expressed a favourable opinion towards the taxation scheme. This proportion was higher if tax revenues were used to finance health-related measures (respectively 68·8 % of favourable opinion if used to finance a reduction in prices of healthy products and 76·4 % if used to finance the healthcare system). Multivariable analyses showed that support towards the tax varied among subgroups of the population. Groups who tended to be less financially affected by the measure and those who perceived sugar-sweetened beverages as having detrimental effects were more likely to support the tax.Conclusion:The revised French sugar-sweetened beverage tax appeared to be favourably received and perceived by the public.

Background: Evidence is accumulating that high dietary glycemic index (GI) and glycemic load (GL) are risk factors for several metabolic disorders, e.g. diabetes or cardiovascular diseases, but evidence on GI/GL and cancer is less consistent. However, mechanistic data suggest that food with high-GI may contribute to carcinogenesis through the insulin resistance pathway.Objective: The objective was to study the associations between dietary GI/GL and breast cancer risk.Design: Population based prospective cohort study.Setting and Participants: 81,526 women aged ≥18y (mean age: 40.8±14.0y), free of cancer at baseline and with no prevalence or incidence of diabetes, from the French NutriNet-Santé cohort (2009-2019) were included in the analyses. 24-hour dietary records linked with a nutritional composition table for >3,500 food/beverage items containing GI values and carbohydrate content enabled us to compute average dietary GI and GL.Main outcome measures: Associations between GI; GL; proportion of energy and proportion of carbohydrates from low-GI (GI≤55) and from medium/high-GI (GI>55) foods and risk of breast cancer were assessed by Cox proportional hazard models adjusted for known risk factors (sociodemographic, anthropometric, lifestyle, medical history, and nutritional factors).Results: A higher dietary GL was associated with higer postmenopausal breast cancer risk (n=572, hazard ratio for quintile 5 versus quintile 1: 1.64 confidence interval 1.06-2.55 Ptrend=0.03). Percentage of energy intake from medium/high-GI foods was associated with higher breast cancer risk (HRQ5vs.Q1=1.34 (1.05-1.71), Ptrend=0.04), more specifically postmenopausal breast cancer (HRQ5vs.Q1=1.36 (1.00-1.84), Ptrend=0.02). Carbohydrates intake from medium/high-GI foods was also associated with higher breast cancer risk (HRQ5vs.Q1=1.34 (1.05-1.71), Ptrend=0.04) in both premenopausal (HRQ5vs.Q1=1.49 (1.03-2.17), Ptrend=0.04) and postmenopausal (HRQ5vs.Q1=1.44 (1.08-1.93), Ptrend=0.03) women, whereas carbohydrates from low-GI foods was associated with lower breast cancer risk (HRQ5vs.Q1=0.74 (0.59-0.92), Ptrend=0.04), only among postmenopausal women (HRQ5vs.Q1=0.69 (0.52-0.92), Ptrend=0.03). Conclusion: The consumption of foods with high-GI was associated increased breast cancer risk. If these results are confirmed dietary GI and GL should be considered as modifiable risk factor for primary breast cancer prevention. Background Excessive sugar intake is now recognized as a key risk factor for obesity, type 2 diabetes, and cardiovascular diseases. In contrast, evidence on the sugar-cancer link is less consistent. Experimental data suggest that sugars could play a role in cancer etiology through obesity but also through inflammatory and oxidative mechanisms and insulin resistance, even in the absence of weight gain. Objective The objective was to study the associations between total and added sugar intake and cancer risk (overall, breast, and prostate), taking into account sugar types and sources. Methods In total, 101,279 participants aged >18 y (median age, 40.8 y) from the French NutriNet-Sante prospective cohort study (2009-2019) were included (median follow-up time, 5.9 y). Sugar intake was assessed using repeated and validated 24-h dietary records, designed to register participants' usual consumption for >3500 food and beverage items. Associations between sugar intake and cancer risk were assessed by Cox proportional hazard models adjusted for known risk factors (sociodemographic, anthropometric, lifestyle, medical history, and nutritional factors). Results Total sugar intake was associated with higher overall cancer risk (n = 2503 cases; HR for quartile 4 compared with quartile 1: 1.17; 95% CI: 1.00, 1.37; Ptrend = 0.02). Breast cancer risks were increased (n = 783 cases; HRQ4vs.Q1 = 1.51; 95% CI: 1.14, 2.00; Ptrend = 0.0007). Results remained significant when weight gain during follow-up was adjusted for. In addition, significant associations with cancer risk were also observed for added sugars, free sugars, sucrose, sugars from milk-based desserts, dairy products, and sugary drinks (Ptrend ≤ 0.01). Conclusions These results suggest that sugars may represent a modifiable risk factor for cancer prevention (breast in particular), contributing to the current debate on the implementation of sugar taxation, marketing regulation, and other sugar-related policies. Background: The impact of sugar consumption on diabetes and cardiovascular disease risks is well established. In contrast, the level of evidence regarding the role of sugar in cancer onset is still very limited. However, simple sugars could have a deleterious effect on cancer risk via different mechanisms, such as weight gain, but also through inflammation or insulin resistance. We recently showed that an increased consumption of sugary drinks was associated with cancer incidence, in particular, with breast cancer in the NutriNet-Santé cohort (Chazelas BMJ 2019). Objective: Our objective was to expand this research to all sugary products and to prospectively investigate the association between the consumption of sugar (simple sugar, added sugar, sugar from different food sources) and risk of breast cancer (overall, premenopausal and postmenopausal) in a large cohort of French adults. Design: Population based prospective cohort study. Setting and participants: 79,742 female participants aged ≥ 18y (mean age: 41.0±14.0y) from the French NutriNet-Santé cohort (2009- 2019) were included. Consumption of sugar were assessed using repeated 24h-dietary records, designed to register participants’ usual consumption for more than 3,500 different food and beverages items. Main outcome measures: Prospective associations between sugar consumption and risk of breast (premenopausal, postmenopausal and both) cancer were assessed by multivariable Cox proportional hazard models adjusted for known risk factors. Results: The consumption of sugar was significantly associated with higher breast cancer risk (n=783 cases, HRfor a 10g/d increase=1.05, 95% confidence interval 1.01 to 1.09, P for trend=0.02), so was the consumption of added sugar (n=783 cases, HRfor a 10g/d increase=1.07, 95% confidence interval 1.02 to 1.12, P for trend=0.01) and more specifically with postmenopausal breast cancer (n=459 cases, HRfor a 10g/d increase=1.08, 95% confidence interval 1.01 to 1.16, P for trend=0.02). The consumption of sugar from sugary drinks was significantly associated with breast cancer (n=783 cases, HRfor a 10g/d increase=1.09, 95% confidence interval 1.01 to 1.17, P for trend=0.03). The overall consumption of sugar from sugary food and beverages (excluding fruits) was significantly associated with breast cancer risk (n=783 cases, HRfor a 10g/d increase=1.07, 95% confidence interval 1.02 to 1.11, P for trend=0.003). Conclusion In this large prospective study, a greater consumption of sugar, added sugars, sugary food and beverages was associated with an increased risk of breast cancer, especially after menopause. These results need replication in other large-scale prospective studies. They suggest that sugar and sugary foods, whose consumption is increasing in Western countries, may potentially represent a modifiable risk factor for breast cancer prevention. ObjectivesTo study the associations between artificial sweeteners from all dietary sources (beverages, but also table top sweeteners, dairy products, etc), overall and by molecule (aspartame, acesulfame potassium, and sucralose), and risk of cardiovascular diseases (overall, coronary heart disease, and cerebrovascular disease).DesignPopulation based prospective cohort study (2009-21).SettingFrance, primary prevention research.Participants103 388 participants of the web based NutriNet-Santé cohort (mean age 42.2±14.4, 79.8% female, 904 206 person years). Dietary intakes and consumption of artificial sweeteners were assessed by repeated 24 h dietary records, including brand names of industrial products.Main outcomes measuresAssociations between sweeteners (coded as a continuous variable, log10 transformed) and cardiovascular disease risk, assessed by multivariable adjusted Cox hazard models.ResultsTotal artificial sweetener intake was associated with increased risk of cardiovascular diseases (1502 events, hazard ratio 1.09, 95% confidence interval 1.01 to 1.18, P=0.03); absolute incidence rate in higher consumers (above the sex specific median) and non-consumers was 346 and 314 per 100 000 person years, respectively. Artificial sweeteners were more particularly associated with cerebrovascular disease risk (777 events, 1.18, 1.06 to 1.31, P=0.002; incidence rates 195 and 150 per 100 000 person years in higher and non-consumers, respectively). Aspartame intake was associated with increased risk of cerebrovascular events (1.17, 1.03 to 1.33, P=0.02; incidence rates 186 and 151 per 100 000 person years in higher and non-consumers, respectively), and acesulfame potassium and sucralose were associated with increased coronary heart disease risk (730 events; acesulfame potassium: 1.40, 1.06 to 1.84, P=0.02; incidence rates 167 and 164; sucralose: 1.31, 1.00 to 1.71, P=0.05; incidence rates 271 and 161).ConclusionsThe findings from this large scale prospective cohort study suggest a potential direct association between higher artificial sweetener consumption (especially aspartame, acesulfame potassium, and sucralose) and increased cardiovascular disease risk. Background Nitrates and nitrites occur naturally in water and soil. They are also used as food additives (preservatives) in processed meats. They could play a role in the carcinogenicity of processed meat. The objective was to investigate the relationship between nitrate and nitrite intakes (natural food, water and food additive sources) and cancer risk in a large prospective cohort with detailed dietary assessment. Methods Overall, 101 056 adults from the French NutriNet-Santé cohort (2009–ongoing, median follow-up 6.7 years) were included. Nitrites/nitrates exposure was evaluated using repeated 24-h dietary records, linked to a comprehensive composition database and accounting for commercial names/brands of industrial products. Associations with cancer risk were assessed using multi-adjusted Cox hazard models. Results In total, 3311 incident cancer cases were diagnosed. Compared with non-consumers, high consumers of food additive nitrates had higher breast cancer risk [hazard ratio (HR) = 1.24 (95% CI 1.03–1.48), P = 0.02], more specifically for potassium nitrate. High consumers of food additive nitrites had higher prostate cancer risk [HR = 1.58 (1.14–2.18), P = 0.008], specifically for sodium nitrite. Although similar HRs were observed for colorectal cancer for additive nitrites [HR = 1.22 (0.85–1.75)] and nitrates [HR = 1.26 (0.90–1.76)], no association was detected, maybe due to limited statistical power for this cancer location. No association was observed for natural sources. Conclusion Food additive nitrates and nitrites were positively associated with breast and prostate cancer risks, respectively. Although these results need confirmation in other large-scale prospective studies, they provide new insights in a context of lively debate around the ban of these additives from the food industry.

Sugary drinks consumption has increased worldwide in recent years, while evidence demonstrating their detrimental impact on cardio-metabolic health is accumulating ([1][1],[2][2]). Artificially sweetened beverages (ASB) are marketed as a healthier alternative, but their cardiometabolic impact is

(1) Background: Dietary carbohydrates are likely correlated with mental health in general, and with anxiety in particular. Our aim was to investigate the cross-sectional relationship between trait anxiety and carbohydrate (especially sugar) intake in a large sample derived from the general French population. (2) Methods: The analyses included 20231 non-diabetic adults enrolled in the NutriNet-Santé e-cohort, who had completed the trait anxiety subscale of the Spielberger State-Trait Anxiety Inventory (T-STAI, 2013–2016) and who were subsequently divided into high and low trait anxiety groups (T-STAI cut-off of 40 points). Sugar-rich food and macronutrient intake was calculated from ≥3 self-administered 24-h dietary records. The association between trait anxiety and carbohydrate intake was evaluated by ANCOVA according to age category (&lt, 45 and ≥45 years). (3) Results: In the full sample, 7942 (39,3%) individuals fell into the high trait anxiety category. They were more likely to be women (82,2% versus 69,2%, p &lt, 0,0001) and younger (mean age 51,6 versus 55,1 years, 0,0001) compared to the low trait anxiety group. In fully-adjusted models, high-anxiety individuals aged under 45 years had significantly higher mean consumption of added simple sugars (43,9 versus 42,3 g/d, 0,0007), whereas those aged over 45 years with high trait anxiety had significantly lower mean consumption of fruit (214,0 versus 219,5 g/d, 0,02) compared to their low-anxiety counterparts. (4) Conclusion: This cross-sectional study revealed modest age-specific associations between anxiety status and sugar intake among adults. Prospective studies with representative samples are needed to explore potential bi-directionality of the observed associations.

International audience; Purpose Red and processed meats are recognized by the International Agency for Research on Cancer as probably carcinogenic and carcinogenic to humans, respectively. Heme iron has been proposed as a central factor responsible for this effect. Furthermore, anxiety affects the intestinal barrier function by increasing intestinal permeability. The objective of this work was to assess how anxiety modifies the association between red and processed meat consumption and cancer risk in the NutriNet-Santé prospective cohort (2009–2019).Methods Using multi-adjusted Cox models in a sample of 101,269 subjects, we studied the associations between the consumption of red and processed meat, the amount of heme iron coming from these meats and overall, colorectal, prostate, and breast cancer risks, overall and separately among participants with and without anxiety.Results An increase in red and processed meat consumption was associated with an increased risk of developing colorectal cancer in the total population (HR for an increase of 50 g/day = 1.18 (1.01–1.37), p = 0.03). After stratification on anxiety, the HR 50 g/day was 1.42 (1.03–1.94, p = 0.03) in anxious participants and 1.12 (0.94–1.33, p = 0.20) in other participants. Similar trends were observed for overall cancer risk. Analyses conducted with heme iron also provided similar results. Conclusions Our results strengthen the existing body of evidence supporting that red and processed meat consumption and heme iron intake are associated with an increased risk of overall and more specifically colorectal cancer, and suggest that anxiety modifies these associations, with an increased risk in anxious participants.

Introduction Sugary drinks consumption has increased worldwide in recent years and evidence demonstrating their detrimental impact on cardio-metabolic health is accumulating. Artificially sweetened beverages (ASB) are marketed as a healthier alternative, but their cardio-metabolic impact is being debated in the scientific community. This study aimed to investigate the relationships between the consumption of ASB, sugary drinks and the risk of first incident cardiovascular disease (CVD) in a large prospective cohort. Methods The French NutriNet-Santé cohort was launched in 2009. Every 6 months participants are asked to fill 3 validated web-based 24-hour dietary records. All major health events reported by participants were validated based on their medical records by a committee of physicians. Data were also linked to national health insurance system and to the French national cause of specific mortality registry. For each type of beverage, 3 categories were defined as follows: non-consumers, low consumers and high consumers (separated by sex-specific median among consumers). Multi-adjusted Cox proportional hazard models with age as the primary time scale were performed. Results A total of 104,761 participants were included. During follow-up (2009-2019), 1,379 first incident cases of CVD occurred. Compared to non-consumers, higher consumers of sugary drinks had a higher risk of overall CVD (HR = 1.21, 95% CI 1.04 to 1.40, Ptrend=0.009). Higher consumers of ASB had also a significantly higher risk of CVD (HR = 1.30, 95% CI 1.00 to 1.71, Ptrend =0.04). Conclusions Both sugary drinks and ASB were similarly associated with CVD risk. The health effects of non-nutritive sweeteners is currently being debated based on contrasted epidemiological results. Mechanistic data suggests metabolic effects through gut microbiota perturbation and body weight gain. To imply a causal link, they need replication in other large-scale prospective cohort as well as further mechanistic investigations. Key messages Higher consumption of sugary drinks and ASB was associated with higher risk of CVD, suggesting that ASB might not be a healthy substitute for sugary drinks when considering cardiovascular health. These data provide additional arguments to feed the current debate on taxes, labeling and regulation of sugary and artificially sweetened beverages.

Introduction:The consumption of ultra-processed foods has been increasing during the last decades, and has been previously associated with increased risks of mortality and several chronic diseases such as cancer, cardiovascular diseases, hypertension, and dyslipidemia. The objective of this study is to assess the prospective associations between consumption of ultra-processed foods and the risk of type 2 diabetes (T2D).Methods:Overall, 104707 participants aged at least 18 years (median age 41.5 years) from the French NutriNet-Santé cohort (2009–2019) were included. Dietary intakes were collected using repeated 24 hour dietary records (5.7/subject in average), designed to register participants' usual consumption for 3300 different food items. These were categorized according to their degree of processing by the NOVA classification. Associations between ultra-processed food intake and risk of type 2 diabetes were assessed using multivariable Cox proportional hazard models adjusted for known risk factors (sociodemographic, anthropometric, lifestyle, medical history and nutritional factors).Results:Ultra-processed sugary products, fruits and vegetables, and beverages were the highest contributors of the ultra-processed category (respectively 27.9, 18.5 and 15.6%). Ultra-processed food intake was associated with a higher risk of T2D (n = 821 incident cases; hazard ratio for an absolute increment of 10 in the percentage of ultra-processed foods in the diet = 1.15 (95% confidence interval 1.06 to 1.25); P = 0.0009, median follow-up: 6.0y, 582252 person-years). These results remained statistically significant after adjustment for other metabolic comorbidities, for several markers of the nutritional quality of the diet (red meat and sugary drinks consumption, intakes of saturated fatty acids, sodium, sugar, dietary fiber or Healthy/Western patterns derived by principal component analysis) and after a large range of sensitivity analyses.Conclusions:In this large observational prospective study, higher consumption of ultra-processed foods in the diet was associated with a higher risk of type 2 diabetes. These results need to be confirmed in other populations and settings, and causality remains to be established. Various dimensions of processing such as nutritional composition of the final product, food additives, contact materials, and neoformed contaminants might play a role in these associations and further studies are needed to better understand their relative contribution. Importance Ultraprocessed foods (UPF) are widespread in Western diets. Their consumption has been associated in recent prospective studies with increased risks of all-cause mortality and chronic diseases such as cancer, cardiovascular diseases, hypertension, and dyslipidemia; however, data regarding diabetes is lacking. Objective To assess the associations between consumption of UPF and risk of type 2 diabetes (T2D). Design, Setting, and Participants In this population-based prospective cohort study, 104 707 participants aged 18 years or older from the French NutriNet-Sante cohort (2009-2019) were included. Dietary intake data were collected using repeated 24-hour dietary records (5.7 per participant on average), designed to register participants' usual consumption for more than 3500 different food items. These were categorized according to their degree of processing by the NOVA classification system. Main Outcomes and Measures Associations between UPF consumption and risk of T2D were assessed using cause-specific multivariable Cox proportional hazard models adjusted for known risk factors (sociodemographic, anthropometric, lifestyle, medical history, and nutritional factors). Results A total of 104 707 participants (21 800 [20.8%] men and 82 907 [79.2%] women) were included. Mean (SD) baseline age of participants was 42.7 (14.5) years. Absolute T2D rates in the lowest and highest UPF consumers were 113 and 166 per 100 000 person-years, respectively. Consumption of UPF was associated with a higher risk of T2D (multi-adjusted hazard ratio [HR] for an absolute increment of 10 in the percentage of UPF in the diet, 1.15; 95% CI, 1.06-1.25; median follow-up, 6.0 years; 582 252 person-years; 821 incident cases). These results remained statistically significant after adjustment for several markers of the nutritional quality of the diet, for other metabolic comorbidities (HR, 1.13; 95% CI, 1.03-1.23), and for weight change (HR, 1.13; 95% CI, 1.01-1.27). The absolute amount of UPF consumption (grams per day) was consistently associated with T2D risk, even when adjusting for unprocessed or minimally processed food intake (HR for a 100 g/d increase, 1.05; 95% CI, 1.02-1.08). Conclusions and Relevance In this large observational prospective study, a higher proportion of UPF in the diet was associated with a higher risk of T2D. Even though these results need to be confirmed in other populations and settings, they provide evidence to support efforts by public health authorities to recommend limiting UPF consumption. IntroductionPrevious epidemiological studies have found associations between the consumption of ultra-processed foods and the risk of obesity-related outcomes, such as post-menopausal breast cancer, cardiovascular diseases, hypertension and mortality. However, only one Spanish prospective study has explored the associations between the consumption of ultra-processed foods and the risk of overweight and obesity. The aim of this study is to investigate the associations between ultra-processed food consumption and the risk of overweight and obesity, as well as the associations between ultra-processed food consumption and weight trajectories, in middle-aged adults included in the French large scale NutriNet-Santé cohort.MethodsOverall, 110260 participants aged at least 18 years from the French NutriNet-Santé cohort (2009–2019) were included. Dietary intakes were collected using repeated 24 hour dietary records, merged with a food composition database of 3300 different products. These were categorized according to their degree of processing by the NOVA classification. Associations between ultra-processed food intake and risks of overweight and obesity were assessed using multivariable Cox proportional hazard models. Associations between ultra-processed food intake and weight trajectories were assessed using multivariable linear mixed models for repeated measures with random slope and intercept. Models were adjusted for known risk factors (sociodemographic, lifestyle, and nutritional factors).ResultsUltra-processed food intake was associated with a higher risk of overweight (n = 7063 incident cases; hazard ratio for an absolute increment of 10 in the percentage of ultra-processed foods in the diet = 1.11 (95% confidence interval 1.08 to 1.14); P < 0.0001, median follow-up: 4.1y, 260304 person-years) and obesity (n = 3066 incident cases; HR = 1.09 (95% confidence interval 1.05 to 1.13); P < 0.0001, median follow-up: 8.0y 365344 person-years). Higher consumers of ultra-processed foods (4th quartile) were more likely to present an increase in body mass index over time (change of BMI/time-unit in Q4 vs Q1 = 0.04, P < 0.0001). These results remained statistically significant after adjustment for several markers of the nutritional quality of the diet (fruits and vegetables and sugary drinks consumption, intakes of saturated fatty acids, sodium, sugar, dietary fiber or Healthy/Western patterns derived by principal component analysis) and after a large range of sensitivity analyses.ConclusionIn this large observational prospective study, higher consumption of ultra-processed foods in the diet was associated with a higher risk of overweight and obesity. Introduction et but de l’etude L’impact de la consommation de sucre sur le risque de diabete et de maladies cardiovasculaires est aujourd’hui bien etabli. En revanche, le niveau de preuve concernant le role joue par les sucres simples sur l’apparition de cancers est tres limite, or ils pourraient avoir un effet deletere sur le risque de cancer via differents mecanismes, tels que la prise de poids, mais egalement l’inflammation, le stress oxydant ou la resistance a l’insuline. Nous avons recemment montre que la consommation de boissons sucrees etait positivement associee a l’incidence de cancer, en particulier de cancer du sein, dans la cohorte NutriNet-Sante (Chazelas BMJ 2019). L’objectif etait ici d’etendre cette recherche a tous les produits sucres et d’etudier prospectivement l’association entre la consommation de sucres (sucres simples totaux, sucres ajoutes, et sucres provenant de differentes sources) et le risque de cancer du sein (au global et selon le statut menopausique) au sein d’une large cohorte d’adultes francais. Materiel et methodes Cette etude est basee sur une cohorte prospective. Au total 79 742 femmes de plus de 18 ans (âge moyen : 41,0 ± 14,1) de la cohorte NutriNet-Sante (2009–019) ont ete incluses. La consommation de sucre ete evaluee a l’aide d’enregistrements alimentaires de 24 heures repetes, concus pour evaluer les consommations habituelles des participants pour plus de 3500 items alimentaires. Les associations entre la consommation de sucre et le risque de cancers du sein (premenopausique, postmenopausique et au global) ont ete evaluees a l’aide de modeles de Cox multi-ajustes sur les principaux facteurs de risque connus. Resultats et analyse statistique La consommation de sucre etait significativement associee avec un risque plus eleve de cancer du sein (n = 783, HR Q4 vs. Q1 = 1,51 ; intervalle de confiance a 95 % : 1,14–2,00 ; P de tendance = 0,0007), de meme pour la consommation de sucres ajoutes (n = 783, HR Q4 vs. Q1 = 1,47 ; IC 95 % : 1,12–1,91 ; p = 0,02). Ces associations etaient plus specifiquement observees chez les femmes pre-menopausees (p = 0,003 pour les sucres simples et p = 0,03 pour les sucres ajoutes, 324 cas), celles-ci consommant plus de produits sucrees que les femmes post-menopausees, chez qui les tendances correspondantes n’etaient pas significatives (p = 0,06 pour les sucres simples et 0,3 pour les sucres ajoutes, 459 cas). Concernant les sources alimentaires, les sucres simples des boissons (HRQ4 vs. Q1 = 1,38 ; IC 95 % : 1,09–1,74 ; p = 0,002), des produits laitiers (HRQ4 vs. Q1 = 1,32 ; IC 95 % : 1,05–1,65 ; p = 0,01), et des desserts lactes sucres (HRQ4 vs. Q1 = 1,29 ; IC 95 % : 1,05–1,59 ; p = 0,02) etaient plus particulierement associes au risque de cancer du sein. Conclusion Cette large etude prospective suggere une association entre consommation accrue de sucre et risque de cancer du sein. Ces resultats doivent etre reproduits dans d’autres etudes prospectives a grande echelle. Ils suggerent que le sucre et les aliments sucres, dont la consommation augmente dans les pays occidentaux, pourraient representer un facteur de risque modifiable pour la prevention primaire du cancer du sein.

Introduction et but de l’etude En 2018, suite a la parution de son rapport de reference, le World Cancer Research Fund (WCRF) actualisait ses recommandations en matiere de prevention des cancers basees sur les facteurs nutritionnels pour lesquels un niveau de preuve suffisant a ete etabli selon leur systeme de gradation du poids des preuves. Notre objectif etait d’etudier si le respect de ces nouvelles recommandations etait effectivement associe a un moindre risque de cancer dans une large cohorte prospective. Materiel et methodes Cette etude incluait 80 385 participants de la cohorte NutriNet-Sante (2009–2019). Le respect des recommandations 2018 du WCRF a ete evalue a l’aide d’un score propose par Shams-White et al. (Nutrients 2019) base sur 7 points correspondant chacun a une des composantes suivantes : statut ponderal, activite physique, consommation de fruits et legumes, de fibres d’aliments ultra-transformes, de viandes rouges et charcuteries, de boissons sucrees et d’alcool. Les apports alimentaires usuels ont ete estimes a partir d’enregistrements de 24 h repetes, et l’activite physique a partir d’une version courte du questionnaire IPAQ. Des modeles de Cox multi-ajustes sur des facteurs de confusion potentiels ont ete utilises pour etudier les associations entre respect des recommandations et risque de cancer (total et principales localisations). Resultats et analyse statistique Au total, 2428 cas de cancers incidents ont ete valides au cours d’un suivi median de 7,4 ans. Le score median de respect des recommandations 2018 du WCRF etait de 3,75 (IQR : 3,25–4,50). L’augmentation d’un point de ce score (un score plus eleve indiquant un meilleur respect des recommandations) etait associee a un moindre de risque de cancer au global (HR = 0,90 [IC95 % : 0,86–0,94] ; p Conclusion Nos resultats suggerent que le respect des recommandations 2018 du WCRF est bien associe a une diminution du risque de cancers. Nos resultats montrent donc l’interet des recommandations 2018 du WCRF pour construire et justifier les recommandations nutritionnelles de sante publique nationales et internationales, dans une optique de prevention des cancers mais aussi plus generalement des maladies chroniques.

Background: Breast cancer remains the leading cancer in women. In 2018, following its summary report, the World Cancer Research Fund (WCRF) issued its recommendations for cancer prevention based on the nutritional risk factors with a sufficient level of evidence, several of which being related to breast cancer. Our objective was to study whether adherence to these new recommendations was actually associated with reduced risk of breast cancer. Methods: A total of 62,546 women from the NutriNet-Santé population-based cohort study (2009-2019) were included in our analyses. Adherence to the 2018 WCRF recommendations was assessed using the operationalized score on a 7-point scale, proposed by Shams-White et al (Nutrients 2019), including: weight, physical activity, fruit and vegetables, dietary fibers, ultra-processed foods, red and processed meat, sugary drinks and alcohol. Usual dietary intakes were assessed using repeated 24h-dietary records and physical activity level using the IPAQ questionnaire. Cox proportional hazard models were computed with adjustment for the following potential confounders: educational level, height, smoking status, number of 24h dietary records, family history of cancer, number of biological children, age at menarche, age at parity, menopausal status, use of oral contraception or hormonal treatment for menopause. Results: During a median follow-up of 7.3 years, 745 women were diagnosed with a first incident breast cancer. The median WCRF 2018 adherence score was 3.75 (IQR: 3.25-4.50). An increase of 1-point increment in the score was associated with a decreased risk of breast cancer (HR=0.92; 95%CI 0.85-1.00; P=0.05), and especially of breast cancer post-menopause (n=456 cases, HR=0.88; 0.79-0.98; P=0.02). No association was observed for breast cancer pre-menopause (n=289, HR=0.99; 0.87-1.12; P=0.8). Sugary drink, alcohol and weight components of the score particularly contributed to the observed association. Conclusions: Our results suggest that a higher adherence to the WCRF 2018 recommendations for cancer prevention is associated to a decreased risk of breast cancer. Considering that the WCRF cancer recommendations are consistent with overall food-based dietary guidelines, such recommendations should be promoted to the general public and transposed as public health actions to contribute to decrease the burden of cancer. Background and aims Data regarding the association between ultra-processed food (UPF) consumption and eating disorders (ED) are scarce. Our aim was to investigate whether UPF intake was associated with different ED types in a large population-based study. Methods 43,993 participants (mean age = 51.0 years; 76.1% women) of the French NutriNet-Santé web-cohort who were screened for ED in 2014 via the Sick-Control-One stone-Fat-Food (SCOFF) questionnaire, were included in the analysis. The clinical algorithm Expali TM tool was used to identify four ED types: restrictive, bulimic, binge eating, and other (not otherwise specified). Mean dietary intake was evaluated from at least 2 self-administered 24-h dietary records (2013–2015); categorization of food as ultra-processed or not relied on the NOVA classification. The associations between UPF intake (as percent and reflecting mean daily UPF quantity (g/d) within the dietary intake, %UPF) and ED types were evaluated using polytomous logistic regression models. Results 5,967 participants (13.6%) were categorized as likely ED (restrictive n = 444; bulimic n = 1,575; binge eating n = 3,124; other ED n = 824). The fully-adjusted analyses revealed a positive association between UPF intake and bulimic, binge eating, and other ED: ED risk (odds ratio, OR) for an absolute 10-percentage point incremental increase in %UPF intake were 1.08 (1.01–1.14; P = 0.02), 1.21 (1.16–1.26; P < 0.0001), and 1.11 (1.02–1.20; P = 0.02), respectively. No significant association was detected for restrictive ED. Discussion and Conclusion This study revealed an association of UPF intake with different ED types among French adults. Future research is needed to elucidate the direction of the observed associations.

Food additives (e.g. artificial sweeteners, emulsifiers, dyes, etc.) are ingested by billions of individuals daily. Some concerning results, mainly derived from animal and/or cell-based experimental studies, have recently emerged suggesting potential detrimental effects of several widely consumed additives. Profiles of additive exposure as well as the potential long-term impact of multiple exposure on human health are poorly documented. This work aimed to estimate the usual intake of food additives among participants of the French NutriNet-Santé cohort and to identify and describe profiles of exposure (single substances and mixtures). Overall, 106,489 adults from the French NutriNet-Santé cohort study (2009-ongoing) were included. Consumption of 90 main food additives was evaluated using repeated 24 h dietary records including information on brands of commercial products. Qualitative information (as presence/absence) of each additive in food products was determined using 3 large-scale composition databases (OQALI, Open Food Facts, GNPD), accounting for the date of consumption of the product. Quantitative ingested doses were estimated using a combination of laboratory assays on food matrixes (n = 2677) and data from EFSA and JECFA. Exposure was estimated in mg per kg of body weight per day. Profiles of exposure to food additive mixtures were extracted using Non-negative Matrix Factorization (NMF) followed by k-means clustering as well as Graphical Lasso. Sociodemographic and dietary comparison of clusters of participants was performed by Chi-square tests or linear regressions. Data were weighted according to the national census. Forty-eight additives were consumed by more than 10% of the participants, with modified starches and citric acid consumed by more than 90%. The top 50 also included several food additives for which potential adverse health effects have been suggested by recent experimental studies: lecithins (86.6% consumers), mono- and diglycerides of fatty acids (78.1%), carrageenan (77.5%), sodium nitrite (73.9%), di-, tri- and polyphosphates (70.1%), potassium sorbate (65.8%), potassium metabisulphite (44.8%), acesulfame K (34.0%), cochineal (33.9%), potassium nitrate (31.6%), sulfite ammonia caramel (28.8%), bixin (19.5%), monosodium glutamate (15.1%) and sucralose (13.5%). We identified and described five clusters of participants more specifically exposed to five distinct additive mixtures and one additional cluster gathering participants with overall low additive exposure. Food additives, including several for which health concerns are currently debated, were widely consumed in this population-based study. Furthermore, main mixtures of additives were identified. Their health impact and potential cocktail effects should be explored in future epidemiological and experimental studies.

Introduction et but de l’etude La consommation de boissons sucrees a augmente dans le monde ces dernieres annees, tandis que les preuves de leur effet nefaste sur la sante cardiometabolique s’accumulent. Les boissons edulcorees, commercialisees comme une alternative plus saine, voient aussi leur impact sur la sante cardiometabolique debattu. L’objectif etait d’etudier les relations entre la consommation de boissons sucrees, edulcorees et le risque de maladies cardiovasculaires (MCV) dans une large cohorte prospective. Materiel et methodes La cohorte francaise NutriNet-Sante a ete lancee en 2009. Tous les six mois, les participants sont invites a remplir trois enregistrements alimentaires valides en ligne sur 24 heures. Les boissons sucrees etaient composees de toutes les boissons contenant ≥ 5 % de sucre. Les boissons edulcorees ont ete definis comme les boissons contenant des edulcorants intenses. Tous les evenements de sante majeurs signales par les participants ont ete valides sur la base de leur dossier medical. Les donnees ont egalement ete reliees au systeme national d’assurance maladie (SNIIRAM) et au registre national de mortalite francais (CepiDC). Cette etude s’est concentree sur les premiers cas incidents d’accident vasculaire cerebral, d’accident ischemique transitoire, d’infarctus du myocarde, de syndrome coronarien aigu et d’angioplastie. Pour chaque type de boisson, trois categories de consommation ont ete definies : non-consommateurs, consommateurs faibles et eleves (separes par une mediane sexe-specifique parmi les consommateurs). Des modeles de Cox avec multiples ajustements (âge en echelle de temps) ont ete realises. Les participants ont contribue jusqu’a la date du diagnostic de MCV, du dernier questionnaire rempli, du deces ou du 23 octobre 2019, selon la premiere eventualite. R® (3.5.2) a ete utilise pour les analyses. Resultats et analyse statistique Au total, 104 760 participants ont ete inclus [âge moyen de reference = 42,9 ans (ET = 14,6)]. Le nombre moyen de recueils alimentaires etait de 5,7 (ET = 3,1). L’IMC moyen etait de 23,7 kg/m2 (ET = 4,5), 17,2 % etaient des fumeurs actuels, 31,7 % avaient des antecedents familiaux de MCV. La prevalence de base du diabete de type 2 etait de 1,4 %, l’hypercholesterolemie de 8,0 %, l’hypertension de 8,2 % et l’hypertriglyceridemie de 1,4 %. Au cours du suivi (2009–2019), 1379 premiers cas incidents de MCV ont ete identifies. Compare aux non-consommateurs, les plus forts consommateurs de boissons sucrees et edulcorees etaient plus a risque de MCV [HR = 1,20 (1,04–1,40), ptendance = 0,009] et [HR = 1,32 (1,00–1,73), ptendance = 0,03], respectivement. Conclusion Dans cette cohorte, des consommations plus importantes de boissons sucrees et edulcorees etaient associees a un risque plus eleve de MCV, ce qui suggere que les boissons edulcorees pourraient ne pas etre un substitut sain aux boissons sucrees. Ces donnees fournissent des arguments supplementaires pour alimenter le debat actuel sur les taxes, l’etiquetage et la reglementation des boissons sucrees et edulcorees. Pour etablir un lien de causalite, il est necessaire de reproduire ces donnees dans d’autres cohortes prospectives a grande echelle et de proceder a des etudes mecanistiques.
