2. Evolution of the Research Literature and the Scientific Community of Alzheimer’s Disease from 1983-2017: A 35-Year Survey

This study surveys the development of Alzheimer's disease (AD) in the research literature, the scientific community, and the journals containing AD papers over a 35-year period. Research papers on AD published from 1983 to 2017 in journals indexed in the Web of Science were analyzed in seven five-year periods. The number of AD papers increased from 1,095 in 1983-1987 to 50,532 by 2013-2017 and in the same time period, the number of participating countries went from 27 to 152. The US was the most prolific country throughout, followed by several European countries, Canada, Australia, and Japan. Asian countries have emerged and by 2013-2017, China surpassed all but the US in productivity. Countries in Latin America and Africa have also contributed to AD research. Additionally, several new non-governmental institutions (e.g., ADNI, ADI) have emerged and now play a key role in the fight against AD. Likewise the AD scientific publishing universe evolved in various aspects: an increase in number of journals containing AD papers (227 journals in 1983-1987 to 3,257 in 2013-2017); appearance of several AD-focused journals, e.g., Alzheimer's & Dementia, Journal of Alzheimer's Disease; and the development of special issues dedicated to AD. Our paper complements the numerous extant papers on theoretical and clinical aspects of AD and provides a description of the research landscape of the countries and journals contributing papers related to AD.

6. Evolution of the scientific literature on drug delivery: A 1974–2015 bibliometric study

This study charts the growth of the drug delivery literature published during 1974-2015 from journals indexed in the Science Citation Index Expanded database. The growth of publications on drug delivery paralleled the total scientific publications for three decades (1974-2003); however, from 2004 to 2015 it exploded fourfold, while the total increased only 1.75 fold. Industrialized countries (USA, UK, Germany, Japan, Italy, France and Canada) were the most prolific during the first decades, but in 2014-2015 China, India and South Korea ranked 1st, 3rd and 4th respectively among the productive countries. The number of participating countries increased fivefold (from 19 to 96). During the last 15years, the journals targeted by drug delivery research increased nearly 2.4 fold (416 to 1001) and three journals (Journal of Controlled Release, Advanced Drug Delivery Reviews, and International Journal of Pharmaceutics) published nearly one-fifth of the drug delivery research in 2014-2015.

This study charts the evolution of the scientific literature on Parkinson's disease (PD) from 1983 to 2017 to inform communities of scientists, physicians, patients, caregivers and politicians concerned with PD. Articles published in journals indexed in the Science Citation Index-Expanded database of the Web of Science were retrieved and analyzed in seven five-year periods: 1983–1987, 1988–1992, 1993–1997, 1998–2002, 2003–2007, 2008–2012 and 2013–2017. Over 35 years the number of research papers on PD increased 33-fold: 885 papers in 1983–1987 to 29,972 in 2013–2017. At the same time the number of countries contributing to PD research increased from 37 to 131. The USA was the most prolific country throughout, followed by several European (UK, Germany, Italy and France) and English-speaking (Canada and Australia) countries. By 2003, several Asian countries (China, South Korea, India and Turkey) emerged with rapid increases in publications related to PD. By 2013–2017, China surpassed all but the USA to rank 2nd globally in productivity. Despite an increase from 4 to 22 African countries publishing PD research from 1983 to 2017, most were either unproductive or contributed ≤5 papers in each five-year period. There has also been a 12-fold increase in the number of journals (232–2824) containing papers on PD. In 2013–2017 three PD-focused journals (Parkinsonism & Related Disorders, Movement Disorders and Journal of Parkinson's Disease) contained 6.8% of all PD papers while a large majority (82.5%) of journals published ≤ 10 papers. This quantitative study complements the numerous extant qualitative reviews to provide a global perspective on PD research.

Purpose Obstructive sleep apnea (OSA) has been described as a common sleep respiratory disorder. Its prevalence in the adult population has been reported to be high, varying from 3% to 28%. Dental practitioners play a key role in the treatment of this disease, using tailor-made mandibular advancement devices (MADs). This pilot study assessed the efficacy and compliance of a custom-fitted thermoplastic MAD for the treatment of moderate to severe OSA syndrome. Materials and Methods In this open study without a control group, OSA syndrome sufferers were enrolled by four centers. One specific MAD was custom fitted to the patients. Polysomnography, Epworth, and snoring scales were administered from inclusion to 45 days postinclusion. Results The study population consisted of 33 men and 8 women; 35 patients completed the study. Patient response was high with 69% of them considered as responders, and 60% showing a complete response. Also, 77.3% of patients with moderate OSA syndrome presented a complete response. An improvement was observed in the apnea hypopnea index, which decreased from 34.1 ± 18.9 to 12.8 ± 14.1. The Epworth Sleepiness Scale score, snoring, and quality of sleep scores decreased with the device (p < 0.0001). Compliance rates were high, with patients wearing the device 6.5 nights a week. Side effects and patient complaints were minor and transient. Conclusion This custom-fitted MAD improved respiratory and somnolence parameters, with response rates similar to those published in the literature with other devices.

In this chapter, we will describe the main endogenous, central modulation and related maladaptive mechanisms involved in processing oral and craniofacial pain. In particular, we will explain how the functional anatomy and pathophysiology of brainstem, hypothalamic, and corticofugal networks may alter the excitability of the trigeminal system. We will describe our recent findings showing a direct anatomo-functional relationship between cortical, hypothalamic excitability disturbances and dysfunctions of medullary trigeminovascular regions. We will analyze the impact of such dysfunctions as putative biomarkers of central sensitization phenomena at the origin of sustained trigeminal pain.

While functional imaging and deep brain stimulation studies point to a pivotal role of the hypothalamus in the pathophysiology of migraine and trigeminal autonomic cephalalgias, the circuitry and the mechanisms underlying the modulation of medullary trigeminovascular (Sp5C) neurons have not been fully identified. We investigated the existence of a direct anatomo-functional relationship between hypothalamic excitability disturbances and modifications of the activities of Sp5C neurons in the rat. Anterograde and retrograde neuronal anatomical tracing, intrahypothalamic microinjections, extracellular single-unit recordings of Sp5C neurons, and behavioral trials were used in this study. We found that neurons of the paraventricular nucleus of the hypothalamus (PVN) send descending projections to the superior salivatory nucleus, a region that gives rise to parasympathetic outflow to cephalic and ocular/nasal structures. PVN cells project also to laminae I and outer II of the Sp5C. Microinjections of the GABAAagonist muscimol into PVN inhibit both basal and meningeal-evoked activities of Sp5C neurons. Such inhibitions were reduced in acutely restrained stressed rats. GABAAantagonist gabazine infusions into the PVN facilitate meningeal-evoked responses of Sp5C neurons. PVN injections of the neuropeptide pituitary adenylate cyclase activating peptide (PACAP38) enhance Sp5C basal activities, whereas the antagonist PACAP6-38 depresses all types of Sp5C activities. 5-HT1B/Dreceptor agonist naratriptan infusion confined to the PVN depresses both basal and meningeal-evoked Sp5C activities. Our findings suggest that paraventricular hypothalamic neurons directly control both spontaneous and evoked activities of Sp5C neurons and could act either as modulators or triggers of migraine and/or trigeminal autonomic cephalalgias by integrating nociceptive, autonomic, and stress processing mechanisms.

To summarize the diversity of assessment and management for chronic orofacial pain associated with a disease (COFPAD) in children. We performed a review of the literature up to May 2012. Hetero-evaluation and self-assessment are used according to age of children. Strict management of the cause is not sufficient for children with COFPAD without a multidisciplinary approach combining pharmacotherapy, psychology and physiotherapy. Conclusion The multidisciplinary approach is the key of management for children with COFPAD.

Aim This study charts the growth of the scientific journal literature on headache for 30+ years (1983–2014). Methods Using the Web of Science, articles published in four two-year periods (1983–1984, 1993–1994, 2003–2004, 2013–2014) from journals indexed in the Science Citation Index Expanded database were retrieved and analyzed. Results From 1983–1984 to 2013–2014, the scientific literature on headache increased nearly fourfold (3.8) from 468 to 1776 articles; the number of participating countries more than doubled (26 to 67); and the two most prolific countries in each period were the USA and Italy. While several European countries (Italy, Germany, UK and Denmark) were among the top 10 in each period, the notable appearance of Turkey, China and Brazil among the top 10 in 2013–2014 indicates the growing geographic spread of publications on headache research. Meanwhile, the comet-like distribution of journals has not changed: two journals, Headache and Cephalalgia, persist throughout as the nucleus, with the tail increasing more than threefold from 141 journals in 1983–1984 to 462 in 2013–2014. Conclusion Our study follows the recent growth and spread of the scientific literature on headache research and should stimulate further bibliometric investigation in this field.

Objective. This study traces the evolution of the scientific literature on pain published during the last 30+ years (1976–2007). Methods. Using the Web of Science®, pain-focused journal articles from the Science Citation Index Expanded™ published in 1977, 1987, 1997, and 2007 were retrieved and analyzed. Results. The number of pain-related publications rose from 1,562 articles for 1976–77 to 9,159 PubMed for 2006–2007, with slow growth for the period 1976–1995, and rapid increases from 1995–2007. The analysis of contributing countries showed two major players, the United States and the UK; the doubling of the number of countries involved in pain research from 40 in 1977 to 82 in 2007; and the appearance in 2007 of The Netherlands, Turkey, China, and Brazil among the top-15 most prolific contributors. During the 30-year period, the number of journals publishing pain-related research increased nearly 2.5-fold (363 journals in 1977 vs 972 in 2007), including 14 new, international pain-focused journals since 2000. Additionally, while there were only two pain journals (Pain and Headache) in 1977, 15 pain-focused journals were indexed in 2007 with the result that 17 of the top-20 pain-focused journals in 2007 did not exist in 1977. Conclusion. The rapid evolution and explosion of pain research in the last 30+ years was reflected in substantial changes in the landscape of the contributing countries and in the scientific journals targeted by pain researchers.

This study traces the evolution of the scientific literature on cytochrome P450 (P450) published during the last 30+ years (1977-2008). Using the Web of Science®, P450 articles from the Science Citation Index Expanded™ published from 1977 to 2008 were retrieved and analyzed. The number of P450 papers has increased from 342 articles in 1977-1978 to 2,357 in 2007-2008, and the number of contributing countries has grown from 23 countries for 1977-1978 to 76 for 2007-2008. While the USA and Japan were the most productive countries, along with several industrialized countries (e.g. UK, Germany and Canada), two Asian countries have recently joined the group of leading countries (in 2007-2008 China ranked 4th and South Korea, 7th). During 1977-2008, the number of journals publishing papers in P450 research increased more than seven-fold (7.7): 94 journals in 1977-1978 and 724 in 2007-2008; however, citation by readers (as measured by the journal impact factor) of the top-ten leading journals increased only slightly from 3.25 for 1977-1978 to 3.81 for 2007-2008. While Biochemistry & Molecular Biology and Pharmacology and Pharmacy are the two main targeted subject areas for P450 research during the period considered, there has been a gradual shift from the biophysical and biochemical fields of interest to aspects of genomics and clinical approaches. The rapid evolution of P450 research in the last 30+ years was accompanied by important changes in the landscape of the contributing countries, in the subject domains, and consequently in the scientific journals targeted by researchers.

This study quantifies the utilization of acetaminophen in life sciences and clinical medicine using bibliometric indicators. A total of 1626 documents involving acetaminophen published by 74 countries during 2003-2005 in the Thompson-Scientific Life sciences and Clinical Medicine collections were identified and analyzed. The USA leads in the number of publications followed by the UK, and industrialized countries, including France, Japan and Germany; the presence of countries such as China, India and Turkey among the top 15 countries deserves to be noticed. The European Union stands as a comparable contributor to the USA, both in terms of number of publications and in terms of profile of papers distributed among subcategories of Life Sciences and Clinical Medicine disciplines. All documents were published in 539 different journals. The most prolific journals were related to pharmacology and/or pharmaceutics. All aspects of acetaminophen (chemistry, pharmacokinetics, metabolism, etc.) were studied with primary interest for therapeutic use (42%) and adverse effects (28%) comprising a large part of publications focusing on acetaminophen hepatotoxicity. This quantitative overview provides as to the interest of the scientific community in this analgesic and completes the various review documents that regularly appear in the scientific literature.

16. The evolution of the sleep science literature over 30 years: A bibliometric analysis

During the 1974–2004 period, the sleep literature had quadrupled (2384 publications in 1974, and 9721 in 2004) while overall scientific productivity had only doubled. The set of the seven most productive countries (USA, Japan, United Kingdom, Germany, France, Canada and Italy) in sleep research, and the geographical region distribution remained stable over the three decades. On the other hand several indicators appeared in the sleep research literature during the 1990s: the increasing productivity of sleep researchers; the growing number of countries publishing on sleep; the continuous creation of sleep-focused journals; the scattering of sleep publication among increasingly more scientific journals; the turnover among the leading journals; and the emergence of new entities such as China, Turkey, and the European Union.

Patient response was high with 69% of them considered as responders, and 60% showing a complete response. Also, 77.3% of patients with moderate OSA syndrome presented a complete response. An improvement was observed in the apnea hypopnea index, which decreased from 34.1 ± 18.9 to 12.8 ± 14.1. The Epworth Sleepiness Scale score, snoring, and quality of sleep scores decreased with the device (p0.0001). Compliance rates were high, with patients wearing the device 6.5 nights a week. Side effects and patient complaints were minor and transient.This custom-fitted MAD improved respiratory and somnolence parameters, with response rates similar to those published in the literature with other devices.

These articles were authored by EU researchers; Germany, the United Kingdom, France, and Italy rank at the top of the EU countries. The output distribution of the most productive EU countries are also presented and discussed. Despite the limitations of the methods used, the present results give an interesting snapshot of the EU publishing behavior in sleep research.

This study provides an overview of the sleep research literature in medicine and biology through bibliometric analyses of the distribution of journal articles per country, the average journal impact factor of each country, the journal subject category of each article, and the population and gross domestic product of the top-producing countries. The authors of the 2325 articles were from 66 countries — the most prolific being the USA, Germany, Japan, The United Kingdom, France and Canada. This study complements continuing subject reviews on sleep research and offers readers a better understanding of the literature size and research richness in this field.

The aim of this study was to provide a quantitative view of the utilisation of the c-fos immunohistochemical method. Articles including the term "c-fos" in their title, abstract or keywords and published in 2004 were retrieved from the Current Content/Life Sciences or Current Content/Clinical Medicine collection of the SCI database. The 933 article-type documents retained were distributed in almost all the sub-disciplines of the Life Sciences and Clinical Medicine, but were principally published in the field of neuroscience. They were authored by researchers from 44 countries - the most prolific were the USA (435 articles), Japan (135) and the UK (55). The 933 articles were published in 283 different journals; all but one of the top-20 most prolific journals are in the Life Sciences discipline, and their Impact Factors ranged from 2.0 to 7.9. A comparison of the USA and the European Union scientific profiles is also made.

La musicothérapie est une discipline très ancienne qui fut pratiquée par de nombreuses cultures antiques (grecque, hébraïque, arabe et chinoise). Elle connait aujourd’hui différentes applications dans le domaine de la santé et des soins, tels que la néonatologie, l’autisme, la maladie d’Alzheimer, et bien d’autres. L’objectif de cette thèse d’exercice est de proposer d’introduire la musicothérapie au sein du cabinet dentaire afin de promouvoir ses bienfaits déjà prouvés, permettre une communication plus douce et établir une relation de confiance entre le praticien et son patient.

La réalisation des soins en Médecine Bucco-Dentaire chez les personnes âgées, oscille entre complications et complexité. Le temps a un effet incontestable sur l’organisme et sur la prise en charge du patient âgé en Odontologie. Cependant chaque personne âgée est unique et les modes de vieillissements sont divers et variés. Pour certains tous les soins semblent être envisageables tandis que pour d’autres, l’abstention est la meilleure thérapeutique. Dans ce travail, nous étudions les possibilités et les limites à la réalisation des soins chez le sujet âgé selon les caractéristiques qui lui sont propre. Pour cela, nous considérons d’une part les impacts de la sénescence et des pathologies sur la réalisation des soins. D’autre part, nous nous interrogeons sur la pertinence des soins chez la personne âgée en considérant son environnement, son autonomie et la structure de soins dans laquelle elle est prise en charge. Ensuite, nous évaluons l’aspect éthique de la réalisation ou non des soins. Plusieurs concepts de gériatrie sont présentés. Ils constituent des outils qui aident le chirurgien-dentiste à établir et adapter le plan de traitement dans sa pratique quotidienne. Les étapes du raisonnement dans la prise en charge du sujet âgé sont présentées. Dans un dernier temps, nous proposons un arbre décisionnel de prise en charge en Odontologie de la personne âgée.

La population française ne cesse de vieillir du fait de l’allongement de l’espérance de vie et de la diminution du nombre des naissances. Les personnes âgées sont plus susceptibles aux maladies chroniques par conséquent le nombre de patients âgés polypathologiques augmente. Le traitement de ces maladies concomitantes mène inévitablement à la polymédication. La multiplicité des prescripteurs peut conduire à des prescriptions à cascade potentiellement inappropriées. Le processus de vieillissement provoque des modifications physiologiques impactant la pharmacodynamie et la pharmacocinétique des médicaments. Les prescriptions doivent donc être adaptées à ces modifications mais également contrôlées à chaque nouvelle spécialité prescrite. En effet les personnes âgées sont plus sujettes à l’iatrogénie médicamenteuse et au défaut d’observance. Le gouvernement a mis en place des mesures afin de contrer cet enjeu majeur de santé publique dont la plus récente est le dispositif PAERPA. Les chirurgiens-dentistes ont également un rôle dans la prise en charge des patients âgés polymédiqués par leur qualité de prescripteur ainsi que pour le diagnostic et le traitement de la xérostomie médicamenteuse qui est le premier effet indésirable de la polymédication au niveau de la sphère orale. Enfin en tant que professionnels de santé, les chirurgiens-dentistes participent à la pharmacovigilance et doivent signaler tout effet indésirable susceptible d’être dû à un médicament.

Parmi les actes de chirurgie orale, l’intervention sur les dents de sagesse reste celle qui peut être la plus invasive. Cette intervention peut systématiquement conduire en post-opératoire à un processus inflammatoire aigu caractérisé par l’apparition d’un œdème, d’un trismus et d’une douleur. En s’appuyant sur le principe de la médecine fondée sur la preuve, une synthèse narrative de revues systématiques a été effectuée sur l’intérêt des GCS dans l’extraction des dents de sagesse. Quatre revues systématiques ont été sélectionnées, soit un total de 63 articles. La qualité des revues a été appréciée sur la base de l’évaluation de la qualité méthodologique des revues à l’aide d’une grille de qualité (outil AMSTAR) et sur l’évaluation du niveau de preuve des résultats (basée sur le type d’études). L’ensemble des 63 articles conclut à l’efficacité des GCS dans la réduction des complications post-opératoires des dents de sagesse. Selon la revue la plus fiable, cette amélioration reste pourtant faible à modérée. Devant la qualité faible à modérée des revues et devant l’hétérogénéité des études, la réalisation d’une étude randomisée contrôlée à grande échelle, fondée sur les recommandations de la Société Francophone de Chirurgie orale serait pertinente.

L’anxiété dentaire est un phénomène courant, qui représente le principal obstacle à la réalisation des traitements dentaires, et qui peut conduire à une détérioration importante de la santé bucco-dentaire. La gestion de l’anxiété dentaire est une tâche indispensable pour le chirurgien-dentiste, mais aussi une des plus difficiles, et les possibilités thérapeutiques existantes requièrent généralement un équipement et une formation spécifiques. Bien qu’il existe également des thérapeutiques médicamenteuses, elles ne correspondent pas à la demande croissante du grand public pour des pratiques de soins plus naturelles. La lavande est une plante réputée pour le soulagement de l’anxiété, mais ce n’est que récemment que ses propriétés anxiolytiques ont été étayées par des études scientifiques. Le but de cette thèse est de présenter les données actuelles concernant l’efficacité de l’huile essentielle de Lavandula angustifolia dans la réduction de l’anxiété et d’étudier son intérêt pour la prise en charge des patients anxieux en Médecine Bucco-Dentaire. Deux méthodes d’administration sont analysées : l’administration par inhalation et l’administration par voie orale d’un extrait standardisé d’huile essentielle de Lavandula angustifolia, le Silexan.

La gestion de la douleur dans le cadre d’un traitement orthodontique est trop souvent négligée et de nombreux patients peuvent alors pratiquer l’automédication pour lutter contre cette dernière. Cela est d’autant plus important car nous sommes souvent face à de jeunes patients, qui peuvent alors assimiler leurs rendez-vous avec un moment déplaisant et douloureux. L’inflammation provoquée par le mouvement orthodontique est à la fois motrice du traitement, mais aussi génératrice d’un inconfort pour le patient. L’arsenal thérapeutique du praticien doit intégrer cette composante dans son traitement, afin de gérer au mieux le bien-être du patient, la coopération de ce dernier mais aussi l’efficacité de sa thérapeutique, à l’aide de moyens pharmacologiques ou non pharmacologiques. Pas assez efficace, perturbant le mouvement orthodontique, ou trop puissant, le choix de l’antalgique doit se faire de manière raisonnée. Par ailleurs, la prescription devra se faire après une évaluation systématique de la douleur afin de juger au mieux son efficacité. L’intérêt de ce sujet est d’observer la place de la médication antalgique dans la gestion de la douleur. Après avoir posé les bases de la douleur, nous nous pencherons plus spécifiquement sur la douleur en orthodontie avec ses causes et ses facteurs. Puis, nous évaluerons l’efficacité des antalgiques dans la gestion de la douleur et, enfin, nous décrirons les alternatives à la prescription antalgique.

L’OMS (Organisation Mondiale de la Santé) définit la pharmacovigilance comme “la surveillance du risque d’effets indésirables résultant de l’utilisation de médicaments”. La pharmacovigilance se base principalement sur le recueil des effets indésirables liés aux médicaments via les notifications spontanées des professionnels de santé, des patients, des industriels. Chaque année, environ 50 000 effets indésirables survenant en France sont collectés par les centres régionaux de pharmacovigilance (CRPV) ; En France, on estime à plus de 100 000, les hospitalisations causées annuellement par les effets indésirables graves (EIG), dont 50 % peuvent être considérés comme évitables. Ces effets indésirables constituent donc un véritable problème de santé publique eu égard aux séjours hospitaliers, à la charge financière et souvent à la mortalité associée. L’objectif de ce travail est de connaître le rôle du chirurgien-dentiste par rapport à la Pharmacovigilance et donc son implication dans ce système.

Une origine dentaire doit être suspectée lors de sinusites unilatérales récidivantes ou de sinusites prolongées unilatérales. Face à ce type de pathologies, un scanner doit être effectué afin d’identifier la cause de l’infection. Une lésion unilatérale, sans destruction osseuse d’origine tumorale ou de problème anatomique entravant le drainage du sinus, doit renforcer la suspicion d’une origine dentaire. Une anamnèse de douleurs dentaires ou de geste dentaire récent doit être recherchée et augmente la probabilité du diagnostic. En phase aiguë de surinfection bactérienne, une antibiothérapie est indiquée. Le traitement définitif repose sur l’élimination de la source infectieuse par un traitement dentaire, ou une extraction combinée à une méatotomie moyenne permettant de rétablir la fonction du sinus.
