1. Contribution of vitamin D insufficiency to the pathogenesis of multiple sclerosis

Abstract: The contribution of vitamin D insufficiency to the pathogenesis of multiple sclerosis (MS) is reviewed. Among the multiple recently discovered actions of vitamin D, an immunomodulatory role has been documented in experimental autoimmune encephalomyelitis and in humans. This action in the peripheral immune system is currently the main known mechanism through which vitamin D might influence MS, but other types of actions could be involved within the central nervous system. Furthermore, vitamin D insufficiency is widespread in temperate countries and in patients with MS at the earliest stages of the disease, suggesting that the deleterious effects related to vitamin D insufficiency may be exerted in these patients. In fact, many genetic and environmental risk factors appear to interact and contribute to MS. In genetics, several human leukocyte antigen (HLA) alleles (more particularly HLA-DRB1*1501) could favour the disease whereas some others could be protective. Some of the genes involved in vitamin D metabolism (e.g. CYP27B1) also play a significant role. Furthermore, three environmental risk factors have been identified: past Epstein–Barr virus infection, vitamin D insufficiency and cigarette smoking. Interactions between genetic and environmental risk or protective factors may occur during the mother’s pregnancy and could continue during childhood and adolescence and until the disease is triggered in adulthood, therefore possibly modulating the MS risk throughout the first decades of life. Furthermore, some clinical findings already strongly suggest that vitamin D status influences the relapse rate and radiological lesions in patients with MS, although the results of adequately powered randomized clinical trials using vitamin D supplementation have not yet been reported. While awaiting these incontrovertible results, which might be long in coming, patients with MS who are currently in vitamin D insufficiency should be supplemented, at least for their general health status, using moderate doses of the vitamin.

2. Relationship between 25-OH-D serum level and relapse rate in multiple sclerosis patients before and after vitamin D supplementation

Abstract: Background: Vitamin D could play a protective role in multiple sclerosis. Methods: In an observational, uncontrolled study, vitamin D3 supplementation (3010 IU/day on average) was given to 156 consecutive patients with relapsing–remitting multiple sclerosis, under first-line immunomodulatory therapy and with initial 25-OH-D serum level lower than 100 nmol/l (40 ng/ml). Relapses were determined for 29.1 ± 8.4 months during vitamin D and 29.8 ± 10.1 months before supplementation. The 25-OH-D level was measured before supplementation and several times during supplementation. The incidence rate of relapses before and during supplementation was estimated using negative binomial regression models with follow-up durations as offset terms. The incidence rate and incidence rate ratio of relapses at various 25-OH-D levels were also calculated using negative binomial regression models. Results: In 76 patients, immunomodulatory therapy preceded vitamin D supplementation (by 4.2 ± 2.7 years) and in 80 patients both treatments were started simultaneously. Under supplementation, the 25-OH-D level increased from 49 ± 22 nmol/l to 110 ± 26 nmol/l on average. Pooling data collected before and during supplementation, we found a significant strong inverse relationship between the relapse incidence rate and the 25-OH-D level ( p < 0.0001), suggesting that vitamin D did indeed influence the relapse rate. Results of univariate, bivariate and multivariate analyses were analogous: in the multivariate model adjusted for age, disease duration and previous use of immunomodulatory therapy, every 10 nmol increase in 25-OH-D level was associated with a reduction in the relapse incidence rate of 13.7%. Dividing iteratively the population made up of pooled periods into two subgroups according to the 25-OH-D levels, the relapse incidence rate ratio decreased as the 25-OH-D level increased up to 110 nmol/l, but a plateau effect was observed beyond this limit. Conclusion: Further studies are warranted for accurate quantification of the vitamin D effect.

Abstract: Identification of drug-induced liver disease (DILI) is difficult, even among hospitalized patients. The aim of this pilot study was to assess the impact of a specific strategy for DILI screening.We prospectively compared the number of acute DILI cases identified in one week of a proactive strategy based on centralized elevated ALT values to those identified with a standard of care strategy for 24-week period based on referral cases to the hepatology unit. In the centralized strategy, a designated study biochemist identified patients with ALT greater than 3 times the upper limit of normal values (ULN) and notified the designated hepatologists, who then went to the patients' wards, analyzed the charts, and if necessary, interviewed the identified patients. During these two periods, patients with possible DILI were included after signing an informed consent in an ongoing European diagnostic study (SAFE-T consortium).During the 24-week period of the standard strategy, 12 (0.04%) patients out of a total of 28,145 were identified as having possible DILI, and 11 of these accepted to be included in the protocol. During the one-week proactive period, 7 patients out of a total of 1407 inpatients (0.498%) [odds ratio vs. standard = 12.1 (95% CI, 3.9-32.3); P3 ULN by designated biochemists and hepatologists identified 12 times more acute cases of drug-induced liver disease than the standard strategy. This pilot cohort is registered on the number AP-HP P110201/1/08-03-2011 and AFSSAPS B110346-70.

Abstract: The benefits provided by disease-modifying treatments in multiple sclerosis have been demonstrated in clinical trials, but the extent to which they can be extrapolated to everyday care is less clear, as are the long-term benefits of treatment. The objective of this prospective observational cohort study performed in France was to evaluate the effectiveness and safety of glatiramer acetate in patients with relapsing–remitting multiple sclerosis over a 5-year period. All neurologists in France were invited to participate and enroll adult patients starting a first treatment with brand glatiramer acetate 20 mg. Given the observational nature of the study, no fixed study visits were imposed; consultations took place according to the investigator’s normal practice. Occurrence of disease exacerbations and adverse events was documented and neurological disability evaluated with the EDSS at each consultation. Overall, 852 patients were analysable and 269 took glatiramer acetate continuously for 5 years. Median treatment duration was 3.4 years. Principal reasons for discontinuation were inadequate efficacy (38.9%), local tolerability (22.6%) and personal convenience (21.3%). Age, employment status, baseline EDSS score and number of previous exacerbations were variables associated with treatment persistence. The annualised exacerbation rate (5 years) was 0.41 [95% CI 0.39–0.44]; 316 patients (37.2%) remained exacerbation-free throughout. The risk of confirmed disability worsening (5 years) was 43.8% [95% CI 39.9–47.9%]. The most frequent adverse drug reactions were local injection site reactions (584 patients; 68.5%) and systemic immediate post-injection reactions (168 patients; 19.7%). Overall, these findings are consistent with those of previous clinical trials. Abstract: Resume Pathologies cerebrovasculaires Le benefice de la thrombectomie a la phase aigue de l’infarctus cerebral est maintenant etabli lorsqu’il existe une occlusion proximale d’une artere intracrânienne, independamment de son mecanisme etiologique. L’indication des anticoagulants dans les dissections des troncs supra-aortiques demeure incertaine alors que le controle de la tension arterielle reste determinant dans la prevention des recidives d’hemorragies intracerebrales. Epilepsie Le developpement d’un parcours de soins specialise apres la survenue d’une premiere crise et l’affectation d’un psychiatre au sein des centres tertiaires ameliorent de maniere significative la prise en charge de l’epilepsie. Les taux de concordance diagnostiques entre neurologues analysant les memes observations sont faibles, montrant que le diagnostic de l’epilepsie reste difficile. Une atrophie temporale est frequemment observee dans l’epilepsie debutante du sujet âge. Mouvements anormaux D’importants progres ont ete realises ces dernieres annees dans la connaissance de la physiopathologie de la maladie de Parkinson, ce qui a ete confirme a ce congres par la decouverte de nouveaux biomarqueurs, en particulier genetiques, qui peuvent aboutir a de futurs traitements. Des progres marques concernent aussi la determination de l’histoire naturelle des maladies neurodegeneratives depuis leur phase prodromale jusqu’a leur classification neuropathologique sur la base des depots intracerebraux de proteines anormales. Demences Les communications sur la maladie d’Alzheimer ont precise la valeur respective des biomarqueurs dans le diagnostic in vivo, les espoirs therapeutiques qui reposent principalement sur des essais d’immunisation anti-Aβ des les stades precliniques (et non plus prodromaux) de la maladie, afin d’inflechir l’evolution de la perte neuronale. Au-dela des diverses et nombreuses presentations sur la genetique et la physiopathogenie des demences, des resultats prometteurs de la stimulation cerebrale dans la depression resistante au traitement medical ont ete presentes. Neuropathies peripheriques On retiendra les bons resultats enregistres avec l’ibudilast dans la sclerose laterale amyotrophique en association avec le riluzole, et ceux, prometteurs avec le patisiran dans la neuropathie amyloide familiale a transthyretine. La piste hereditaire semble se confirmer pour les neuropathies a petites fibres douloureuses. Le diagnostic des lymphomes du systeme nerveux peripherique reste difficile et leur traitement decevant. La caracterisation de nouveaux auto-anticorps dans les polyneuropathies inflammatoires demyelinisantes chroniques est en progres avec de possibles implications therapeutiques. Sclerose en plaques (SEP) La biotine dans les formes progressives de SEP et le daclizumab dans les formes a poussees paraissent etre des traitements prometteurs. L’alemtuzumab pourrait devenir le recours en cas d’echec d’un ou plusieurs autres traitements. Le teriflunomide, le dimethyle fumarate et le fingolimod ont confirme leur efficacite et gardent une tolerance acceptable. Outre l’insuffisance en vitamine D et le tabac, qui sont des facteurs de risque confirmes, l’insuffisance en testosterone (chez les hommes) et l’obesite sont des facteurs de risque emergents, sur lesquels on pourrait agir aussi.

Abstract: International audience; The most recent findings linking exposure to sun and vitamin D insufficiency to multiple sclerosis (MS) are reviewed. Due to insufficient sunshine and changing lifestyles, hypovitaminosis D is widespread in temperate countries. Numerous epidemiological studies have strongly suggested that sunshine and vitamin D insufficiency contributes to MS risk in these countries. Moreover, several large genetic studies in MS patients have recently stated unequivocally that diverse abnormalities involving vitamin D metabolism are related to the risk of the disease. The important implications of such results are discussed here. Then, the interactions of hypovitaminosis D with the other genetic and environmental protective and risk factors, such as the allele HLA DRB1*1501, Epstein-Barr virus infection, obesity, smoking and sexual hormones, are summarized. Vitamin D insufficiency and sufficiency could be a risk and a protective factor, respectively, among many other factors possibly continuously modulating the global MS risk from the mother's pregnancy to the triggering of MS in adulthood. However, many interactions between these different factors occur more particularly between conception and the end of adolescence, which corresponds to the period of maturation of the immune system and thymus and may be related to the dysimmune nature of the disease. The main mechanisms of action of vitamin D in MS appear to be immunomodulatory, involving the various categories of T and B lymphocytes in the general immune system, but neuroprotector and neurotrophic mechanisms could also be exerted at the central nervous system level. Furthermore, several controlled immunological studies performed in MS patients have recently confirmed that vitamin D supplementation has multiple beneficial immunomodulatory effects. However, there is still an enduring absence of major conclusive randomized clinical trials testing vitamin D supplementation in MS patients because of the quasi-insurmountable practical difficulties that exist nowadays in conducting and completing over several years such studies involving the use of a vitamin. Nevertheless, it should be noted that similar robust statistical models used in five different association studies have already predicted a favorable vitamin D effect reducing relapses by 50–70%. If there is now little doubt that vitamin D exerts a beneficial action on the inflammatory component of MS, the results are as yet much less clear for the progressive degenerative component. Lastly, until more information becomes available, vitamin D supplementation of MS patients, using a moderate physiological dose essentially correcting their vitamin insufficiency, is recommended.

Abstract: International audience; The number of serum 25-hydroxyvitamin D (25OHD) assays has increased tenfold in France in less than 10 years, sometimes for invalidated reasons. In 2013, the French National Authority for Health (Haute autorité de santé, or HAS) limited the indications for serum 25OHD measurements to rickets/osteomalacia, older adults with recurrent falls, monitoring of kidney transplant in adults, and surgical treatment of obesity in adults. Our aim here was to note that other indications for serum 25OHD measurements are supported by previous literature and by a number of national and international recommendations, in particular the following: any situation of bone fragility, any chronic renal failure

Abstract: ObjectiveTo evaluate the safety and efficacy of cholecalciferol in patients with relapsing-remitting MS (RRMS).MethodsIn this double-blind, placebo-controlled parallel-group, 2-year study, 181 patients with RRMS were randomized 1:1. Key inclusion criteria were a low serum 25-hydroxy vitamin D (25OHD) concentration (ResultsThe primary end point was not met. In patients who completed the 2-year follow-up (45 with cholecalciferol and 45 with placebo), all efficacy parameters favored cholecalciferol with an ARR reduction (p = 0.012), less new hypointense T1-weighted lesions (p = 0.025), a lower volume of hypointense T1-weighted lesions (p = 0.031), and a lower progression of EDSS (p = 0.026). The overall rate of adverse events was well balanced between groups.ConclusionsAlthough the primary end point was not met, these data suggest a potential treatment effect of cholecalciferol in patients with RRMS already treated with interferon beta-1a and low serum 25OHD concentration. Abstract: La maladie de Parkinson est, après la maladie d'Alzheimer, l'affection neurologique la plus répandue au sein de la population après 50 ans. Elle touche près de 2% des personnes de plus de 65 ans. Ce pourcentage augmentant avec l'âge, on peut prévoir qu'avec le vieillissement de la population, le nombre de patients ne cessera d'augmenter dans les années à venir. La troisième édition de cet ouvrage, entièrement en couleurs, consacrée à la maladie de Parkinson rend compte de façon didactique des avancées de ces cinq dernières années, en abordant de façon exhaustive tous les aspects de cette pathologie : l'épidémiologie, l'étiopathogénie, les aspects génétiques, la physiopathologie, les signes moteurs et non moteurs, notamment les troubles cognitifs, psychiques et comportementaux. 18. Relationship between 25-OH-D serum level and relapse rate in multiple sclerosis patients before and after vitamin D supplementation

Abstract: Background: Vitamin D could play a protective role in multiple sclerosis. Methods: In an observational, uncontrolled study, vitamin D3 supplementation (3010 IU/day on average) was given to 156 consecutive patients with relapsing–remitting multiple sclerosis, under first-line immunomodulatory therapy and with initial 25-OH-D serum level lower than 100 nmol/l (40 ng/ml). Relapses were determined for 29.1 ± 8.4 months during vitamin D and 29.8 ± 10.1 months before supplementation. The 25-OH-D level was measured before supplementation and several times during supplementation. The incidence rate of relapses before and during supplementation was estimated using negative binomial regression models with follow-up durations as offset terms. The incidence rate and incidence rate ratio of relapses at various 25-OH-D levels were also calculated using negative binomial regression models. Results: In 76 patients, immunomodulatory therapy preceded vitamin D supplementation (by 4.2 ± 2.7 years) and in 80 patients both treatments were started simultaneously. Under supplementation, the 25-OH-D level increased from 49 ± 22 nmol/l to 110 ± 26 nmol/l on average. Pooling data collected before and during supplementation, we found a significant strong inverse relationship between the relapse incidence rate and the 25-OH-D level ( p < 0.0001), suggesting that vitamin D did indeed influence the relapse rate. Results of univariate, bivariate and multivariate analyses were analogous: in the multivariate model adjusted for age, disease duration and previous use of immunomodulatory therapy, every 10 nmol increase in 25-OH-D level was associated with a reduction in the relapse incidence rate of 13.7%. Dividing iteratively the population made up of pooled periods into two subgroups according to the 25-OH-D levels, the relapse incidence rate ratio decreased as the 25-OH-D level increased up to 110 nmol/l, but a plateau effect was observed beyond this limit. Conclusion: Further studies are warranted for accurate quantification of the vitamin D effect.

Abstract: RESUME La deficience en vitamine D en France dans tous les groupes d'âge est importante car elle synthetisee a partir des UVB au niveau du derme (d'ou ensoleillement obligatoire) et que l'alimentation contient peu de vitamine D (uniquement les poissons gras). La vitamine D joue un role preponderant dans le metabolisme phosphocalcique : absorption intestinale du calcium et fixation du calcium dans l'os. La carence en vitamine D entraine un rachitisme chez l'enfant et l'adolescent et une osteomalacie ou une osteopenie chez l'adulte. La vitamine D aurait egalement d'autres effets : prevention dans le developpement de certaines maladies infectieuses, de certaines maladies auto-immunes ou a composante auto-immnune (Sclerose en plaques, diabete de type 1, polyarthrite rhumatoide et lupus) syndrome metabolique chez l'enfant et chez l'adulte, diabete de type 2 ; eventuel role dans la differentiation cellulaire en relation avec l apparition de certains cancers. Le statut vitaminique D d'un individu est defini par la mesure de la 25(OH)D serique, en accord avec l'ensemble de la communaute scientifique internationale. Le taux de 25OHD serique doit etre superieur a 75 nmol/L (ou 30 ng/ml) pour admettre un statut vitaminique normal selon la majorite des auteurs. La mesure de la 25 OHD serique necessite un immuno-essai valable ou la spectrometrie de masse pour le diagnostic d'une deficience en vitamine D chez l'individu. Les apports quotidiens en vitamine D doivent etre reevalues en France compte tenu de l'âge, de la saison et le deficit en vitamine D doit etre corrige uniquement par une supplementation par voie orale.. Ces recommandations ne tiennent pas compte de la pathologie de l'individu c est-a-dire des maladies hepatiques, renales, intestinales et osseuses.

Abstract: Key points In the past 10 years, our knowledge of vitamin D has been revolutionized on two main points. Firstly , this vitamin is not only crucial for bone and calcium metabolism but also exerts major hormonal actions via its active metabolite (calcitriol) and specific receptors in almost all organs. The diverse non-classical actions of vitamin D–i.e. anti-inflammatory, immunomodulatory, antiproliferative and as a neurotransmitter–could have protective and preventive effects for a wide variety of pathologies, such as autoimmune diseases, cancer, infections and cardiovascular affections. Secondly, daily vitamin D requirements have been redefined thanks to many recent metabolic and pathological studies and are about 10 times higher than the amount considered sufficient until a few years ago. The fact that sunshine is the essential natural source of vitamin D and is limited in temperate and Nordic countries, coupled with the fact that modern lifestyle increasingly removes people from exposure to the sun, could explain why a great majority of the general population in these countries are in a state of vitamin D insufficiency . A lack of vitamin D can therefore also be observed in all pathologies but it may play a pathogenic role only in some of them . The incrimination of hypovitaminosis D as a risk factor is a reasonable assumption when several different research approaches used in a given pathology have consistently concluded that vitamin D is likely involved in that pathology. In multiple sclerosis , taken here as a prime example, there is a substantial rationale for vitamin D involvement, based on the findings of different experimental, epidemiological, genetic and immunological studies. Possible curative effects of vitamin D , in addition to a preventive action, are currently being tested but have not yet been demonstrated in most pathologies. However, these two questions appear to be clearly distinct and may involve notably different mechanisms. Lastly , since vitamin D insufficiency exists in most people living in mid- or high-latitude countries, vitamin D could exert multiple major preventive actions, simple supplementation is both safe and inexpensive and, for a vitamin-hormone, supplementation seems obligatory from a general preventive medical point of view alone, it follows that vitamin D supplementation should be organized in these countries to treat all those currently in a state of insufficiency, patients and ‘normal’ subjects alike, without further delay .

Abstract: Resume Contexte Le coma est une cause frequente d’hospitalisation en reanimation. Parmi les echelles mesurant sa profondeur, le score de Glasgow (GCS) est le plus utilise. Ce score presente cependant certaines limitations. Recemment, un nouveau score tenant compte de ces limitations, le full outline of unresponsivenesss score (FOUR) a ete valide. Ce score, disponible en langue anglaise possede une reproductibilite interobservateur comparable a celle du GCS. Methodes Nous avons realise la traduction en langue francaise du FOUR score et avons, dans une etude preliminaire, evaluer sa reproductibilite interobservateur. L’etude a ete realisee sur huit patients de reanimation et huit patients de soins continus dits de « post-reanimation ». Les patients ont ete cotes successivement par deux medecins neurologues, quatre infirmiers experimentes et cinq infirmiers debutants, soit 176 cotations au total. La reproductibilite entre les differentes evaluations, pour le FOUR score et le GCS, a ete calculee au moyen du kappa pondere (κW). Resultats L’âge moyen des patients etait de 62 ans et variait entre 23 ans pour le patient le plus jeune et 85 ans pour le patient le plus âge. La version francaise du FOUR score possede une reproductibilite interobservateur excellente (κW = 0,86 ; IC 95 % : 0,83–0,89) comparable a la version originale decrite en anglais et comparable a celle du GCS (κW = 0,85 ; IC 95 % : 0,82–0,88). Conclusion Le FOUR score en francais possede une reproductibilite interobservateur excellente. Ce score est facilement realisable et accepte, meme avec une formation simple et courte.

Abstract: Hypovitaminosis D is currently one of the most studied environmental risk factors for multiple sclerosis (MS) and is potentially the most promising in terms of new clinical implications. These practical consequences, which could be applied to MS patients without further delay, constitute the main purpose of this review. Vitamin D is involved in a number of important general actions, which were not even suspected until quite recently. In particular, this vitamin could play an immunomodulatory role in the central nervous system. Many and varied arguments support a significant role for vitamin D in MS. In animal studies, vitamin D prevents and improves experimental autoimmune encephalomyelitis. Epidemiologically, latitude, past exposure to sun and the serum level of vitamin D influence the risk of MS, with, furthermore, significant links existing between these different factors. Clinically, most MS patients have low serum levels of vitamin D and are in a state of insufficiency or even deficiency compared to the international norm, which has been established on a metabolic basis. Large therapeutic trials using vitamin D are still lacking but the first results of phase I/II studies are promising. In the meantime, while awaiting the results of future therapeutic trials, it can no longer be ignored that many MS patients have a lack of vitamin D, which could be detected by a serum titration and corrected using an appropriate vitamin D supplementation in order to restore their serum level to within the normal range. From a purely medical point of view, vitamin D supplementation appears in this light to be unavoidable in order to improve the general state of these patients. Furthermore, it cannot currently be ruled out that this supplementation could also be neurologically beneficial.

Abstract: A 51-year-old man developed positional vertigo, ataxia, dysgeusia, diplopia, and oscillopsia. Eye movement examination and video-oculographic recording disclosed primary position upbeat nystagmus (PPUN) and a right internuclear ophthalmoplegia. Brain MRI showed a small focal lesion in the right dorsal tegmentum of the caudal pons with signal characteristics consistent with a primary demyelinating central nervous system disease. PPUN has not been described previously with a lesion in such a location. Clinicoanatomic correlation in this patient suggests that a lesion of the superior vestibular nucleus and its efferent crossing ventral tegmental tract could be responsible for the PPUN. This case report contributes to a better understanding of the role of this pathway in humans.

Abstract: The posterior parietal cortex is involved in numerous visuospatial tasks, but little is known about the lateralization of these functions. We used functional magnetic resonance imaging to map the posterior parietal areas involved in saccades. Cerebral activation was studied during three different steps of saccadic elaboration: internal Decision of where to direct a horizontal saccade, motor Preparation and saccade Execution. These steps activated distinct areas: Decision and Preparation selectively activated the left posterior parietal cortex (left deep posterior intraparietal sulcus and left medial posterior intraparietal sulcus), whereas Execution activated only the right posterior parietal cortex (right medial posterior intraparietal sulcus). In humans, left but not right posterior parietal cortex might be specifically related to decision making and preparation of forthcoming ocular saccades.

Abstract: In the anti-saccade paradigm, subjects are instructed not to make a reflexive saccade to an appearing lateral target but to make an intentional saccade to the opposite side instead. The inhibition of reflexive saccade triggering is under the control of the dorsolateral prefrontal cortex (DLPFC). The critical time interval at which this inhibition takes place during the paradigm, however, is not exactly known. In the present study, we used single-pulse transcranial magnetic stimulation (TMS) to interfere with DLPFC function in 15 healthy subjects. TMS was applied over the right DLPFC either 100 ms before the onset of the visual target (i.e. -100 ms), at target onset (i.e. 0 ms) or 100 ms after target onset (i.e. +100 ms). Stimulation 100 ms before target onset significantly increased the percentage of anti-saccade errors to both sides, while stimulation at, or after, target onset had no significant effect. All three stimulation conditions had no significant influence on saccade latency of correct or erroneous anti-saccades. These findings show that the critical time interval at which the DLPFC controls the suppression of a reflexive saccade in the anti-saccade paradigm is before target onset. In addition, the results suggest the view that the triggering of correct anti-saccades is not under direct control of the DLPFC.

Abstract: Deciding where to look is mandatory to explore the visual world. To study the neural correlates subserving the cognitive phase of self-initiated eye movements in humans, we tested 12 healthy participants, using event-related functional MRI. Changes in the frontal-cortical activity preceding voluntary saccades were studied when the participants freely decided the direction of a forthcoming saccade, compared with a condition in which they had only to prepare an externally cued saccade. Self-initiation of saccades, before their execution, was specifically associated with frontal-lobe activation in the dorsolateral prefrontal cortex, and in the right presupplementary eye field and frontal eye fields, suggesting the roles of these areas in the decision process of where to look when facing two possible visual targets.

Abstract: RESUME Les mouvements oculaires sont au service de la vision. Ils se sont particulierement developpes chez les especes predatrices, a vision frontale binoculaire. Les noyaux abducens assurent la synergie oculomotrice laterale tandis que les noyaux des nerfs moteurs oculaires communs commandent la verticalite oculomotrice et la convergence. Les deux grands types de mouvements oculaires conjugues — les saccades, changeant les images, et les mouvements lents, les stabilisant — sont controles par des structures premotrices specifiques du tronc cerebral, reticulaires et vestibulaires, respectivement. En amont du tronc cerebral, les principaux circuits oculomoteurs corticaux et sous-corticaux commencent a etre bien connus. Les saccades servent meme desormais de modele moteur pour etudier les differents processus cognitifs preparant les mouvements en general, tels que la memoire spatiale, la prediction et la decision. A la classique methode d’etude des effets des lesions, il s’est ajoute recemment la stimulation magnetique transcrânienne et l’imagerie cerebrale fonctionnelle qui ont apporte des resultats complementaires permettant de mieux comprendre le fonctionnement cerebral. Les mouvements oculaires peuvent donc etre testes tres simplement au lit du patient pour la semiologie clinique elementaire, mais ils peuvent aussi maintenant etre utilises de facon beaucoup plus sophistiquee en laboratoire pour explorer l’univers des neurosciences cognitives.

Abstract: The pathophysiology of spontaneous upbeat (UBN) and downbeat (DBN) nystagmus is reviewed in the light of several instructive clinical findings and experimental data. UBN due to pontine lesions could result from damage to the ventral tegmental tract (VTT), originating in the superior vestibular nucleus (SVN), coursing through the ventral pons and transmitting excitatory upward vestibular signals to the third nerve nucleus. A VTT lesion probably leads to relative hypoactivity of the drive to the motoneurons of the elevator muscles with, consequently, an imbalance between the downward and upward systems, resulting in a downward slow phase. The results observed in internuclear ophthalmoplegia suggest that the medial longitudinal fasciculus (MLF) is involved in the transmission of both upward and downward vestibular signals. Since no clinical cases of DBN due to focal brainstem damage have been reported, it may be assumed that the transmission of downward vestibular signals depends only upon the MLF, whereas that of upward vestibular signals involves both the MLF and the VTT. The main focal lesions resulting in DBN affect the cerebellar flocculus and/or paraflocculus. Apparently, this structure tonically inhibits the SVN and its excitatory efferent tract (i.e. the VTT) but not the downward vestibular system. Therefore, a floccular lesion could result in a disinhibition of the SVN-VTT pathway with, consequently, relative hyperactivity of the drive to the motoneurons of the elevator muscles, resulting in an upward slow phase. UBN also results from lesions affecting the caudal medulla. An area in this region could form part of a feedback loop involved in upward gaze-holding, originating in a collateral branch of the VTT and comprising the caudal medulla, the flocculus and the SVN, successively. Therefore, it is suggested that the main types of spontaneous vertical nystagmus due to focal central lesions result from a primary dysfunction of the SVN-VTT pathway, which becomes hypoactive after pontine or caudal medullary lesions, thereby eliciting UBN, and hyperactive after floccular lesions, thereby eliciting DBN. Lastly, since gravity influences UBN and DBN and may facilitate the downward vestibular system and restrain the upward vestibular system, it is hypothesized that the excitatory SVN-VTT pathway, along with its specific floccular inhibition, has developed to counteract the gravity pull. This anatomical hyperdevelopment is apparently associated with a physiological upward velocity bias, since the gain of all upward slow eye movements is greater than that of downward slow eye movements in normal human subjects and in monkeys.

Abstract: Background Prefrontal dysfunction in neuropsychiatric disorders such as schizophrenia has been shown to impair inhibition of reflexive saccadic eye movements; however, it is unclear whether reflexive saccade inhibition can be attributed to a distinct subregion of the human prefrontal cortex. Methods We tested 15 patients with acute unilateral ischemic lesions of the prefrontal cortex and 20 control subjects with an antisaccade task. Lesions were reconstructed using Talairach coordinates, and possible candidate regions for reflexive saccade inhibition were identified. Results Significantly increased antisaccade error rates were observed in patients with lesions affecting a region in mid-dorsolateral prefrontal cortex or the white matter between this region and the anterior portions of the internal capsule. Antisaccade error rates of patients with lesions outside this region were normal. These findings were largely independent of lesion volume, postlesion delay, and subject age. Conclusions Our findings suggest that inhibition of reflexive saccades depends on a circumscribed subregion of the human dorsolateral prefrontal cortex. This region closely corresponds to Brodmann area 46 as defined by recent cytoarchitectonic studies. Increased antisaccade error rates in patients with prefrontal pathology may be explained by dysfunction of this region.

Abstract: Resume L’evolution des connaissances sur la motricite oculaire est revue au travers de l’experience de 25 ans de l’auteur dans ce domaine. Les avantages de la vision frontale binoculaire, qui est un caractere commun a tous les predateurs (comprenant l’espece humaine), sont d’abord compares a ceux de la vision laterale, caracterisant les proies. La vision frontale binoculaire impose un parallelisme parfait des yeux, qui est assure grâce au noyau abducens (VI) dans le pont, controlant aussi bien l’abduction que l’adduction. L’exemple pathologique du syndrome « un et demi », dans lequel le noyau du VI et le faisceau longitudinal median voisin sont simultanement atteints, est rappele. Les principaux syndromes de la verticalite oculaire sont ensuite revus : en particulier le syndrome nucleaire du nerf moteur oculaire commun (III), dans lequel il existe une paralysie ipsilaterale du III et une hypotropie contralaterale marquee, par atteinte isolee de ce cote des motoneurones du muscle droit superieur. Puis, un cas de nystagmus battant vers le haut (en position primaire) du a une petite lesion pontique est decrit. Il est remarque a cette occasion que, s’il a ete rapporte un certain nombre de cas de nystagmus battant vers le haut qui sont dus a des lesions focales du tronc cerebral touchant le circuit de la verticalite vestibulaire vers le haut (VVH), aucun cas de nystagmus battant vers le bas (en position primaire), pourtant plus frequent en clinique que le premier, ne parait pouvoir etre impute a une lesion focale du tronc cerebral. Ce dernier nystagmus resulterait d’une hyperactivite du meme circuit de la VVH secondaire a une lesion flocculaire et l’hypothese que ce circuit serait physiologiquement predominant par rapport au circuit vers le bas, peut-etre du fait de la gravite, est developpee. Puis, le controle cortical des saccades est revu, en rappelant d’abord que les saccades reflexes sont declenchees par l’aire oculomotrice parietale et les saccades volontaires par l’aire oculomotrice frontale. L’inhibition des saccades reflexes depend essentiellement du controle exerce par le cortex prefrontal dorsolateral (CPFDL). Enfin, quelques exemples de l’utilisation des saccades comme outil de recherche dans les neurosciences cognitives sont donnes. Les saccades dites memorisees ont permis d’etudier la memoire spatiale et d’arriver a une conception originale de son controle cortical, dans lequel le CPFDL, le cortex parahippocampique et la formation hippocampique interviendraient successivement dans des tranches de temps specifiques. Enfin, une experience d’imagerie par resonance magnetique fonctionnelle est decrite pour montrer l’implication forte aussi du CPFDL dans les processus decisionnels preparant le mouvement. L’interet de la motricite oculaire, aussi bien en clinique neuro-ophtalmologique, au lit du patient, qu’en neurophysiologie ou meme neuropsychologie, c’est-a-dire en recherche clinique, est souligne.

Abstract: Resume Introduction La sclerose concentrique de Balo est une forme neuropathologique de sclerose en plaques, caracterisee par une alternance d’anneaux sains et d’anneaux demyelinises. Le diagnostic repose actuellement sur un aspect evocateur en imagerie par resonance magnetique (IRM), mais il n’existe que peu de donnees sur les caracteristiques de ces lesions en spectroscopie protonique par resonance magnetique (SRM). Methodes Nous rapportons un cas de sclerose concentrique de Balo avec la description en SRM des lesions et de leur evolution. Resultats L’etude en SRM de deux lesions au stade aigu (prenant le contraste en bulbe d’oignon sur l’IRM apres injection de gadolinium) a montre une diminution du pic de N-acetylaspartate (NAA), une augmentation du pic de la choline, la presence d’un doublet de lactates et d’un pic de lipides centre a 0,9 ppm. Six mois plus tard, le pic de NAA restait bas, tandis que le pic de choline avait diminue et le doublet de lactates avait disparu. Conclusion Ces donnees spectrometriques sont conformes a l’evolution habituelle des lesions demyelinisantes, comme cela peut etre observe dans la sclerose en plaques. Dans le cadre d’une sclerose concentrique de Balo, la SRM peut donc apporter des arguments neurochimiques en faveur de la nature inflammatoire et demyelinisante des lesions, et permet en outre de preciser le degre d’atteinte et de recuperation axonale.

Abstract: We report a case of cerebral fat embolism which occurred in a 33-year-old man after a diaphyseal femoral fracture without cranial traumatism. The initial examination showed an incomplete picture of coma with tetrapyramidal syndrome and cutaneomucous purpura. There was no respiratory damage. We present a magnetic resonance spectroscopy analysis of the cerebral lesions observed in the initial phase of the embolism, as well as follow-up, which has strengthened the clinical and imaging features for the diagnosis.

Abstract: We report a patient with an isolated large upbeat nystagmus (UBN) in the primary position of gaze. Eye movements were filmed and recorded using electro-oculography. The upward vestibulo-ocular reflex gain, evaluated by pitching the head forward, was markedly reduced compared to when pitching the head back. The lesion was a probable lacunar infarction located in the paramedian and posterior part of the basis pontis, at the upper pons level. This UBN case, with one of the smallest brainstem lesions reported so far, supports the existence in humans of the crossing ventral tegmental tract, described in the cat and transmitting excitatory upward vestibular signals to the third nerve nucleus. It is also suggested that the decussation of this tract lies at the same upper pons level as in the cat but in a slightly more ventral location, i.e. in the posterior basis pontis.

Abstract: Eye movements serve vision, which has two different aims: changing images using saccades, i.e. rapid eye movements, and stabilizing new images on the retina using slow eye movements. Eye movements are performed by ocular motor nuclei in the brainstem, on which supranuclear pathways--originating in the cerebral cortex, cerebellum and vestibular structures--converge. It is useful for the neurologist to know the clinical abnormalities of eye movements visible at the bedside since such signs are helpful for localization. Eye movement paralysis may be nuclear or infranuclear (nerves), involving all types of eye movements, i.e. saccades as well as the vestibulo-ocular reflex (VOR), or supranuclear, in which case the VOR is usually preserved. Lateral eye movements are organized in the pons, with paralysis of adduction (and preservation of convergence) when the lesion affects the medial longitudinal fasciculus (internuclear ophthalmoplegia), paralysis of conjugate lateral eye movements when the lesion affects the abducens nucleus (VI) and the "one-and-a-half" syndrome when both these structures are involved. Vertical eye movements are organized in the midbrain, with ipsilateral oculomotor (III) paralysis and contralateral paralysis of the superior rectus muscle when the third nerve nucleus is unilaterally damaged, supranuclear upward gaze paralysis when the posterior commissure is unilaterally damaged and supranuclear downward gaze paralysis (often coupled with upward gaze paralysis) when the mesencephalic reticular formations are bilaterally damaged. Numerous types of abnormal eye movements exist, of which nystagmus is the most frequent and usually due to damage to peripheral or central vestibular pathways. Cerebral hemispheric or cerebellar damage results in subtle eye movement abnormalities at the bedside, in general only detected using eye movement recordings, because of the multiplicity of eye movement pathways at these levels and their reciprocal compensation in the case of a lesion. Lastly, eye movements can also help the neuroscientist to understand the organization of the brain. They are a good model of motricity allowing us, using eye movement recordings, to study the afferent pathways of the cortical areas that trigger them, and thus to analyze relatively complex neuropsychological processes such as visuo-spatial integration, spatial memory, motivation and the preparation of motor programs.

Abstract: Purpose of review This review focuses on eye movement control by the cerebral cortex, mainly in humans. Data have emerged based on the important contribution of recent techniques such as transcranial magnetic stimulation and functional magnetic resonance imaging, which provide complementary results to those of the classical lesion and electrical stimulation studies. Recent findings The location of the human frontal eye field and its role in pursuit eye movement control were recently detailed. Cumulative evidence for the role of the dorsolateral prefrontal cortex in unwanted reflexive saccade inhibition, short-term spatial memory and prediction suggests that this area controls decisional processes governing ocular motor behaviour. The organization of spatial memory in the dorsolateral prefrontal cortex (short-term), the parahippocampal cortex (medium-term) and the hippocampal formation (long-term) is also reviewed with the results of recent transcranial magnetic stimulation studies. The relatively complicated anatomy of the posterior parietal cortex in humans is briefly described followed by some additional results concerning the location of the parietal eye field - within the posterior half of the intraparietal sulcus - and its role in visuo-spatial integration and attention. The other areas involved in spatial attention are also examined in the light of several recent contributing reports. Lastly, there are also new functional magnetic resonance imaging findings concerning the posterior cingulate cortex, which appears to be mainly involved in the control of externally guided eye movements and attentional mechanisms. Summary Many new findings on the organization of saccades and pursuit eye movements at the cortical level have recently been reported. Furthermore, eye movements are increasingly used as a tool to elucidate relatively complex neuropsychological processes such as attention, spatial memory, motivation and decisional processes, and a considerable number of reports dealing with these questions have been observed.

Abstract: Suppression of unwanted reflexive saccades is a crucial process allowing to sustain voluntary fixation, when required. This inhibition process, which is mainly controlled by the dorsolateral prefrontal cortex, may also involve other cortical and subcortical structures. We prospectively studied the effect of frontal cortical resections involving adjacent regions to the anterior cingulate cortex on the ability to inhibit reflexive saccades. This lesion study included six patients undergoing resection of frontal low grade gliomas, studied prior and after surgery with electro-oculography, using the antisaccade paradigm. Lesions were normalized and mapped in Talairach space allowing to detail the structures whose lesions were associated with antisaccade deficits. In three of the six patients significant early post-operative antisaccade errors were associated with resection of a common critical region, mainly involving the posterior part of the anterior cingulate cortex. This same region was spared in the three remaining patients with no antisaccade deficit, suggesting that the anterior cingulate cortex, known as the cingulate eye field, could play a role in suppression of unwanted saccades.

Abstract: The performance of memory-guided saccades with two different delays (3 and 30 s of memorization) was studied in seven healthy subjects. Double-pulse transcranial magnetic stimulation (dTMS) with an interstimulus interval of 100 ms was applied over the right dorsolateral prefrontal cortex (DLPFC) early (1 s after target presentation) and late (28 s after target presentation). Early stimulation significantly increased in both delays the percentage of error in amplitude (PEA) of contralateral memory-guided saccades compared to the control experiment without stimulation. dTMS applied late in the delay had no significant effect on PEA. Furthermore, we found a significantly smaller effect of early stimulation in the long-delay paradigm. These results suggest a time-dependent hierarchical organization of the spatial working memory with a functional dominance of DLPFC during the early memorization, independent from the memorization delay. For a long memorization delay, however, working memory seems to have an additional, DLPFC-independent component.

Abstract: Introduction Alors qu’une hypovitaminose D est un facteur de risque connu de developper une, l’utilisation de la Vitamine D (VitD) dans le traitement de la SEP n’est supportee par aucune evidence clinique. Objectifs Evaluer le benefice d’une supplementation en Vitamine D3 100 kUI chez des patients atteints d’une SEP-R avec un taux serique de VitD ≤ 75 nmol/L traites par Interferon beta-1a 44 μg 3 fois par semaine. Patients et methodes Etude multicentrique, randomisee, double aveugle, controlee. Une supplementation en VitD3 100kUI 2x/mois a ete administree pendant 2 ans. Le critere principal etait la reduction du taux annualise de poussee (TAP) teste par une regression de Poisson via ajustement des conditions de baseline. Les parametres IRM etaient parmi les criteres secondaires. La selection en intention de traiter a ete utilisee dans l’analyse principale. Vingt sept centres ont inclus un total de 129 patients (63 VitD et 66 placebo) et 90 ont termine l’etude (45/45). Resultats Des TAPs moyens de 0,34 (bras placebo) et 0,45 (bras VitD) ont ete observes avec un rapport RR = TAPvitD/TAPplb de 0,80 ([0,48, 1,32], p = 0,379) en intention de traitement et 0,40 ([0,20, 0,81], p = 0,011) pour les patients ayant complete l’essai. 48. Behavioural relevance modulates access to spatial working memory in humans

Abstract: Neurophysiological studies in monkeys suggest selective representation of behaviourally relevant information in working memory. So far, no behavioural evidence for this has been reported for humans. Here, we investigated the role of behavioural relevance for access to human visuospatial working memory by using delayed oculomotor response tasks. Subjects were presented two successive visual cues in different and unpredictable locations while fixating on a central fixation point. After a delay, an unpredictable auditory signal (one beep or two beeps) sounded and the central fixation point was turned off, initiating the oculomotor response (i.e. memory-guided saccade) phase. Two groups of 10 subjects each were studied in two conditions: in the ‘relevant’ condition, subjects were instructed to memorize both visual cues and to move the eyes to the remembered position of the first cue (one beep) or the second cue (two beeps). The same stimuli were used in the ‘irrelevant’ condition, but subjects were instructed to memorize and move the eyes to the position of the first cue only, regardless of the second cue and the auditory signal. In the ‘relevant’ condition, we found a significant increase in errors of memory-guided saccades to the first cue, when the second cue was located between central fixation point and first cue. This spatially selective interference effect disappeared in the ‘irrelevant’ condition, despite identical stimuli. On a behavioural level, these results show for the first time the significance of behavioural relevance for access to human spatial working memory. These findings complement recent single-neuron studies in monkeys, showing that the neuronal substrates of working memory selectively represent behaviourally relevant perceptual information.

Abstract: Anatomical studies in monkeys, and functional imaging and lesion studies in humans, suggest that, within the primate medial temporal neocortex, the parahippocampal cortex (PHC) is particularly involved in spatial tasks. However, evidence for a functional specialization of the PHC regarding its spatial memory functions has so far been lacking. Here, we investigated spatial memory functions of the human perirhinal cortex (PRC) and PHC. Patients with lesions affecting the PRC but sparing the PHC, and patients with lesions affecting both PRC and PHC, performed an oculomotor delayed response task with unpredictably varied memory delays of up to 30 s. Compared to controls, patients with PRC+PHC lesions showed a significant delay-dependent inaccuracy of memory-guided eye movements contralateral to the lesion side, whereas patients with PRC lesions showed no significant inaccuracy. Our results show that the PHC is a critical component for spatial memory in humans and suggest that (i) extrahippocampal spatial memory functions of the medial temporal lobe may not be equally distributed in the medial temporal neocortex, but may be largely confined to the PHC, and (ii) damage to connections between cortices involved in spatial cognition and rostral regions of the temporal lobe is unlikely to account for the observed spatial memory deficits with PHC lesions.
