This study provides an overview of the multi-sectoral coastal aquaculture development in Zanzibar (Tanzania) over the last thirty years based on empirical evidence from interviews, field observations, policy reports and literature reviews. Despite the immense potential of aquaculture for food and livelihoods, only seaweed farming has so far established into commercial-scale production. This activity is dominated by women and became widespread in the early 1990s as a small but regular source of income. However, seaweed farming constraints such as frequent seaweed die-offs, as well as economic and institutional constraints inhibit its development. Other types of aquaculture activities such as fish farming, mud crab fattening, half-pearl farming, sea cucumber farming and sponge and coral cultures are under development with limited production or in experimental stages. Common constraints among these activities are economic limitations, lack of technical infrastructure and skills, small and irregular production, and limited trade and market availabilities. At the same time, there is a lack of sufficient management and monitoring systems, while there are no formal regulations or clear strategies to boost aquaculture at the national level. In addition, new aquaculture initiatives are often dominated by donor-driven projects instead of local entrepreneurships. This situation does not encourage engagement in aquaculture and thus such activities are outcompeted by other already established sectors (e.g. agriculture and fisheries). We conclude that aquaculture has great potential to evolve due to high environmental capacity. Nevertheless, achieving profitable production and a stronger commitment within local communities, as well as developing effective mariculture governance through support mechanisms and clear strategies to boost the sector at the national level, are essential for sustainable mariculture development in Zanzibar.

The Workshop on the production of annual estimates of abundance of sensitive species (WKABSENS) met to define sensitive species, collate ICES assessments of abundance where these are available, and estimate indices of their abundance per swept-area where not, for the OSPAR area. The analyses identified 140 potentially sensitive species or species complexes, among which 10 are diadromous and three are coastal, 20 have uncertain species ID and nine were identified as sensitive in only one of the sources examined. Among the sensitive species and species complexes, there was sufficient data to provide abundance indices for 50 species, of which 16 had existing stock assessments whereas the workshop derived abundance estimates for the remaining 34 species from survey data. Three statistical modelling approaches (binomial, General Additive Models (GAMs) and VAST) and were explored and the final abundance indices were calculated using GAMs. The species were divided into stocks before estimating abundance indices where these could be identified from the spatial distribution of the species in the survey. The group considered that a similar analysis using data from additional surveys, commercial indices or data from bycatch observers can potentially provide improved abundance estimates for species with variable or low catchability, such as deep-water and pelagic species.

Staphylococcus aureus is an infrequent cause of community-associated (CA-SA) pneumonia in children. The aim of this study was to evaluate the clinical, epidemiological, microbiological, and molecular characteristics of CA-SA pneumonia among children hospitalized in two large tertiary care referral centers during an 8-year period. Cases of CA-SA pneumonia admitted between 2007 and 2014 were retrospectively examined through medical record review. Molecular investigation was performed for available strains; mecA, Panton-Valentine leukocidin (PVL) (lukS-lukF-PV), and fibronectin binding protein A (fnbA) genes were detected by polymerase chain reaction (PCR). Clones were assigned by agr groups, pulsed-field gel electrophoresis (PFGE), SCCmec, and multilocus sequencing typing (MLST). In total, 41 cases were recorded (boys, 61 %), with a median age of 4.3 months (range, 1-175). Methicillin-resistant S. aureus (MRSA) accounted for 31 cases (75.6 %). Complications included empyema (25/41, 61 %), pneumatoceles (7/41, 17 %), and lung abscess (1/41, 2.5 %). Intensive care unit (ICU) admission was required in 58.5 %. Two deaths occurred (4.9 %). Definitive therapy was based on vancomycin with or without other antibiotics (55.9 %), followed by clindamycin and linezolid (26.5 % each). All isolates were susceptible to vancomycin (MIC90 2 mg/L, range 1-2), teicoplanin, and linezolid, whereas 26.8 % were resistant to clindamycin. Among the 25 studied strains, 20 were mecA-positive (MRSA), carrying also the fnbA gene. Of these, 90 % belonged to the ST80-IV/agr3/PVL-positive clone. Methicillin-susceptible S. aureus (MSSA) strains showed polyclonality, 3/5 were PVL-positive, and 3/5 were fnbA-positive. MRSA and particularly the ST80-IV clone predominated among staphylococcal pneumonia cases in children. Treatment provided was effective in all but two patients, despite the relatively high minimum inhibitory concentration (MIC) of vancomycin and a high resistance to clindamycin.

The incidence, type and mortality of bacteremias were evaluated in a pediatric patient cohort, during the entire course of treatment for acute lymphoblastic leukemia (ALL). Eighty-six patients with newly diagnosed ALL were studied. A bacteremic episode was defined as blood isolation of a pathogen in the presence of clinical symptomatology of septicaemia. Bacteremias were analyzed according to the treatment element being delivered and the degree of neutropenia. A central venous catheter (CVC) was inserted at diagnosis in all patients. Fifty-two episodes of bacteremias were encountered in 38/86 (44%) patients, while 48/86 patients had no positive blood culture. Three out of the 38 patients had bacteremia and CVC area infection, simultaneously. Most blood stream infections (29/52, 56%) were documented during the induction phase. Isolated Gram-positive organisms were 48%, Gram-negative 50% and 2% of the positive blood cultures represented fungaemias. The most common Gram-positive isolates were Staphylococcus species (N=22) and the commonest Gram-negative isolated pathogens were Escherichia coli and Pseudomonas aeruginosa. The majority of bacteremias (75%) occurred during neutropenia. The initial antibiotic treatment was ceftazidime or piperacillin/tazobactam and amikacin or tobramycin. CVC was not removed in the majority of bacteremias (94%). No infection related fatality was recorded. Bacteremias constituted a severe and common complication in our patient cohort. However, infection-related fatality rate was negligible, most probably due to the prompt initiation of broad coverage antimicrobial therapy.

Serotype 19F pneumococci were a leading cause of infections among children in Athens, Greece during 2001–2006. In total, 143 19F isolates were typed by pulsed-field gel electrophoresis (PFGE), and 38 isolates representing the main PFGE types were also characterised by multilocus sequence typing. A diversity of distinct strains belonging to sequence types 236, 1035, 274, 172 and 319 were identified, but multidrug-resistant isolates related to the Taiwan19F-14 clone (ST236) constituted 76.9% of the isolates. Spread of the Taiwan19F-14 clone explains, in part, the high incidence of antibiotic resistance observed among pneumococci reported recently from Athens.

Background. suPAR, the soluble form of the urokinase-type plasminogen activator receptor, has been identified as a biomarker of infection in adults but its properties in neonatal infection are not known. Methods. Plasma suPAR levels were determined by ELISA in 47 term neonates with infection (19 bacterial and 28 viral) and in 18 healthy neonates as controls. Thirteen out of 47 infected neonates were septic. In all infected neonates, suPAR levels were repeated at 24 hours, 48 hours, 3-5 days, and 7-10 days following admission. Results. Plasma suPAR levels were significantly increased in infected neonates upon admission, whereas they were highest in septic neonates, in comparison with controls (P < 0.001) and correlated positively with serumCRP levels (P = 0.001). At infection subsidence, suPAR concentrations decreased significantly in comparison with baseline (P < 0.001) but remained higher than in controls (P = 0.01). Receiver operating characteristic analysis resulted in significant areas under the curve for detecting either infected or septic neonates, but not for discriminating between bacterial and viral cause of infection. Conclusions. suPAR is a diagnostic biomarker of infection or sepsis in term neonates; however, it cannot discriminate bacterial from viral infections and also its utility for monitoring the response to treatment is questioned.

Pitted keratolysis is a skin disorder characterized by crateriform pitting, usually affecting the pressure-bearing aspects of the plantar surface of the feet [1]. Studies based on conventional microbiology mainly indicate Gram-positive bacteria as causative agents. Nevertheless, comparable data from studies using molecular methods are lacking [2, 3].
