Abstract

Declines in activities of daily living (ADL) and instrumental activities of daily living (IADL) performances due to cognitive impairments hinder mild cognitive impairment (MCI) and Alzheimer’s disease (AD) patients’ independent and safe daily lives. In order to prevent and treat this, several cognitive interventions have been implemented, but their ecological validity was not ensured due to that their contents are far from real life. Virtual reality (VR) can resemble real life with immersive stimuli, but there have been few studies confirming its ecological effects on ADL and IADL. Therefore, this study conducted a meta-analysis of VR-based cognitive training to investigate its ecological effects on ADL and IADL in MCI and AD patients. From February 2012 to February 2022, a search was conducted for articles published in PubMed, Cochrane, Science Direct, and Web of Science. Quality assessment was assessed by the PEDro scale, and the Cochrane Collaboration tool was used to assess risk of bias. Publication bias was assessed by Egger’s regression. Five studies that met inclusion criteria were included in this study. The VR-based cognitive training showed significant effects on ADL and IADL in both MCI and AD patients. When comparing effects in each group, both MCI and AD patients showed significant effects on ADL and IADL, but MCI patients showed lower effects on ADL and IADL than AD patients. The results indicated that VR-based cognitive training would be beneficial to improve ADL and IADL in MCI and AD patients, suggesting that VR-based cognitive training is ecologically valid.

Abstract

Background: Recently, numerous studies on non-invasive neuro-feedback training for improving cognitive function have been explored to identify its feasibility. However, little is known about the clinical efficacy of functional near-infrared spectroscopy-based neuro-feedback (fNIRS-based NF) training in individuals with or without cognitive impairment. Method: Studies on fNIRS-based NF training were searched through Embase, Medline, PubMed, Web of Science, PsychINFO, and Google scholar and then four studies were finally selected. The overall cognitive domains, working memory, and executive function were separately pooled to investigate fNIRSbased NF training's effect size. Results: The overall effect on cognitive outcomes across four studies was large (Hedges'g = 0.682, confidence interval (CI) = 0.079-1.285) without publication bias. Moderate to large effects were found for working memory (Hedges'g = 1.143) and executive function (Hedges'g = 0.406) without publication bias. fNIRS-based NF training was beneficial in improving working memory and executive function. Conclusion: Therefore, this finding shed new light on fNIRS-based NF training as a promising treatment for improving cognitive function. [ABSTRACT FROM AUTHOR]

Abstract

Background: The Montreal Cognitive Assessment (MoCA) has been widely used to discriminate individuals with mild cognitive impairment (MCI). As a tele-neuropsychological service has become important due to Coronavirus Disease-19 (COVID-19), computerized neuropsychological assessments delivered by telehealth have recently been developed. However, to date, their reliability is unclear yet. Thus, this study was to investigate the reliability of the tele-neuropsychological assessment using the computerized MoCA (c-MoCA). Method: Thirty-four community-dwelling older adults (age: 65–86 years, 17 females) participated in this study. While the primary assessor scored the participant’s performance by telehealth, the secondary assessor independently scored the performance by directly observing face-to-face. To establish the inter-rater reliability of the c-MoCA by telehealth, the absolute agreement and the intra-class correlation coefficient (ICC) were analyzed. Results: The inter-rater reliability of the c-MoCA by telehealth was high (ICC = 0.923; absolute agreement = 0.924, p’s < 0.001). In addition, there was no significant difference in the c-MoCA score between primary and secondary assessors. Conclusion: This study demonstrated that the c-MoCA delivered as a tele-neuropsychological assessment was reliable. This finding suggests that the c-MoCA could be used as a tele-neuropsychological service in older adults who have a low accessibility to medical settings. [ABSTRACT FROM AUTHOR]
