2. Impact of early postnatal support by lactation consultant on breastfeeding outcomes in urban population: A prospective observational study

Background: Early breastfeeding support helps in achieving exclusive breastfeeding by reducing the incidence of painful breast problems. A comparison of the impact of lactation counselling by lactation consultants and nursing staff would help to optimise care. Methods: In this prospective study conducted in the maternity ward of a private sector tertiary care hospital in an urban area, breastfeeding outcomes such as achievement of exclusive breastfeeding and incidence of painful breast problems such as sore nipples and breast engorgement as well as breastfeeding technique were compared between two groups of mother–newborn dyads receiving support from lactation consultants (study group) and maternity ward nursing staff (control group), respectively. Results: A total of 477 mother–newborn pairs were enrolled. Sixty-three out of 225 mothers (28%) in the study group complained of sore nipples as against 122 out of 252 mothers (48.4%) in the control group (odd ratio [OR] 0.414, 95% confidence intervals [CI] 0.282–0.607, P < 0.001). Breast engorgement was reported in 45 mothers (20%) in the study group and 117 mothers (46.4%) in the control group (OR 0.288, 95% CI 0.191–0.434, P < 0.001). Two hundred and three newborns (90.2%) from the study group were discharged on exclusive breastfeeding as compared to 149 newborns (59.1%) in the control group (OR 0.156, 95% CI 0.094–0.260, P < 0.001). Conclusions: Lactation consultants are significantly better than nursing staff at improving breastfeeding outcomes by providing trained support to optimise breastfeeding position and attachment, thereby preventing painful breast problems.

6. Statistical Study of Plasmoids associated with post-CME Current Sheet

We investigate the properties of plasmoids observed in the current sheet formed after an X-8.3 flare followed by a fast CME eruption on September 10, 2017 using Extreme Ultraviolet (EUV) and white-light coronagraph images. The main aim is to understand the evolution of plasmoids at different spatio-temporal scales using existing ground- and space-based instruments. We identified the plasmoids in current sheet observed in the successive images of {\it Atmospheric Imaging Assembly} (AIA) and white-light coronagraphs, K-Cor and LASCO/C2. We found that the current sheet is accompanied by several plasmoids moving upwards and downwards. Our analysis showed that the downward and upward moving plasmoids have average width of 5.92 Mm and 5.65 Mm, respectively in the AIA field of view (FOV). However, upward moving plasmoids have average width of 64 Mm in the K-Cor which evolves to a mean width of 510 Mm in the LASCO/C2 FOV. Upon tracking the plasmoids in successive images, we observe that downward and upward moving plasmoids have average speeds of $\sim$272 km s$^{-1}$ and $\sim$191 km s$^{-1}$ respectively in the EUV passbands. We note that the plasmoids become super-Alfv\'enic when they reach at LASCO FOV. Furthermore, we estimate that the null-point of the current sheet at $\approx$ 1.15 R$_\odot$ where bidirectional plasmoid motion is observed. We study the width distribution of plasmoids formed and notice that it is governed by a power law with a power index of -1.12. Unlike previous studies there is no difference in trend for small and large scale plasmoids. A quiescent prominence was observed at north-west limb of the Sun using different channels of Atmospheric Imaging Assembly (AIA) onboard Solar Dynamics Observatory (SDO). We report and analyse twisting/swirling motions during and after the prominence eruption. We segregate the observed rotational motions into small and large scale. Small scale rotational motions manifest in the barbs of the prominence while the large scale rotation manifests as the roll motion during the prominence eruption. We noticed that both footpoints of the prominence rotate in the counter-clockwise direction. We propose that similar sense of rotation in both footpoints leads to prominence eruption. The prominence erupted asymmetrically near the southern footpoint which may be due to uneven mass distribution and location of the cavity near southern footpoint. Furthermore, we study the swirling motion of the plasma along different circular paths in the cavity of the prominence after the prominence eruption. The rotational velocities of the plasma moving along different circular paths are estimated to be $\sim$ 9-40 km s$^{-1}$. These swirling motions can be explained in terms of twisted magnetic field lines in the prominence cavity. Finally, we observe the twist built up in the prominence, being carried away by the coronal mass ejection (CME) as seen in the Large Angle Spectrometric Coronagraph (LASCO) onboard Solar and Heliospheric Observatory (SOHO)., Comment: 26 pages and 12 figures. Transesophageal echocardiography (TEE) has unique advantages over conventional transthoracic echocardiography (TTE). Compared with TTE, TEE generally provides excellent quality images, especially of the posteriorly situated cardiac structures, such as atria, pulmonary veins, mitral valve, and left atrial appendage. TEE also offers a useful alternative to TTE in patients in whom transthoracic acoustic windows are suboptimal. Furthermore, TEE is the most suited imaging modality for use in the operating rooms and cardiac catheterization labs for guiding cardiac surgical or interventional procedures. However, specific training and competence are required for performing TEE successfully, smoothly, safely and with minimum patient discomfort. This document describes the basic principles of TEE examination, including patient selection and preparation, periprocedural monitoring, and probe handling and maneuvers. Commonly recommended views and the techniques to obtain these views are described in detail, followed by evaluation of specific cardiac structures. Finally, the role of TEE in certain specific clinical settings, such as during advanced circulatory support, is also discussed.

Corona structures and processes during the pre-impulsive stage of solar eruption are crucial to understanding the physics leading to the subsequent explosive energy release. Here we present the first microwave imaging study of a hot flux rope structure during the pre-impulsive stage of an eruptive M7.7 solar flare, with the Nobeyama Radioheliograph (NoRH) at 17 GHz. The flux rope is also observed by the SDO/AIA in its hot passbands of 94 and 131 \AA\. In the microwave data, it is revealed as an overall arcade-like structure consisting of several intensity enhancements bridged by generally weak emissions, with brightness temperatures ($T_B$) varying from ~10,000~K to ~20,000 K. Locations of microwave intensity enhancements along the structure remain relatively fixed at certain specific parts of the flux rope, indicating that the distribution of emitting electrons is affected by the large scale magnetic configuration of the twisted flux rope. Wavelet analysis shows a pronounced 2-min period of the microwave $T_B$ variation during the pre-impulsive stage of interest. The period agrees well with that reported for AIA sunward-contracting loops and upward ejective plasmoids (suggested to be reconnection outflows). This suggests that both periodicities are controlled by the same reconnection process that takes place intermittently at a 2-min time scale. We infer that at least a part of the emission is excited by non-thermal energetic electrons via the gyro-synchrotron mechanism. 20. Modeling solar coronal bright point oscillations with multiple nanoflare heated loops

Intensity oscillations of coronal bright points (BPs) have been studied for past several years. It has been known for a while that these BPs are closed magnetic loop like structures. However, initiation of such intensity oscillations is still an enigma. There have been many suggestions to explain these oscillations, but modeling of such BPs have not been explored so far. Using a multithreaded nanoflare heated loop model we study the behavior of such BPs in this work. We compute typical loop lengths of BPs using potential field line extrapolation of available data (Chandrashekhar et al. 2013), and set this as the length of our simulated loops. We produce intensity like observables through forward modeling and analyze the intensity time series using wavelet analysis, as was done by previous observers. The result reveals similar intensity oscillation periods reported in past observations. It is suggested these oscillations are actually shock wave propagations along the loop. We also show that if one considers different background subtractions, one can extract adiabatic standing modes from the intensity time series data as well, both from the observed and simulated data., Comment: 7 figures, Accepted for publication in APJ

Transesophageal echocardiography (TEE) has unique advantages over conventional transthoracic echocardiography (TTE). Compared with TTE, TEE generally provides excellent quality images, especially of the posteriorly situated cardiac structures, such as atria, pulmonary veins, mitral valve, and left atrial appendage. TEE also offers a useful alternative to TTE in patients in whom transthoracic acoustic windows are suboptimal. Furthermore, TEE is the most suited imaging modality for use in the operating rooms and cardiac catheterization labs for guiding cardiac surgical or interventional procedures. However, specific training and competence are required for performing TEE successfully, smoothly, safely and with minimum patient discomfort. This document describes the basic principles of TEE examination, including patient selection and preparation, periprocedural monitoring, and probe handling and maneuvers. Commonly recommended views and the techniques to obtain these views are described in detail, followed by evaluation of specific cardiac structures. Finally, the role of TEE in certain specific clinical settings, such as during advanced circulatory support, is also discussed.

24. Characteristics of polar coronal hole jets

High spatial- and temporal-resolution images of coronal hole regions show a dynamical environment where mass flows and jets are frequently observed. These jets are believed to be important for the coronal heating and the acceleration of the fast solar wind. We studied the dynamics of two jets seen in a polar coronal hole with a combination of imaging from EIS and XRT onboard Hinode. We observed drift motions related to the evolution and formation of these small-scale jets, which we tried to model as well. We found observational evidence that supports the idea that polar jets are very likely produced by multiple small-scale reconnections occurring at different times in different locations. These eject plasma blobs that flow up and down with a motion very similar to a simple ballistic motion. The associated drift speed of the first jet is estimated to be $\approx$ 27 km s$^{-1}$. The average outward speed of the first jet is $\approx 171$ km s$^{-1}$, well below the escape speed, hence if simple ballistic motion is considered, the plasma will not escape the Sun. The second jet was observed in the south polar coronal hole with three XRT filters, namely, C$_{-}$poly, Al$_{-}$poly, and Al$_{-}$mesh filters. We observed that the second jet drifted at all altitudes along the jet with the same drift speed of $\simeq$ 7 km s$^{-1}$. The enhancement in the light curves of low-temperature EIS lines in the later phase of the jet lifetime and the shape of the jet's stack plots suggests that the jet material is falls back, and most likely cools down. To support this conclusion, the observed drifts were interpreted within a scenario where reconnection progressively shifts along a magnetic structure, leading to the sequential appearance of jets of about the same size and physical characteristics. EUV jets situated in coronal holes are thought to play an important role in supplying heated material to the corona and solar wind. The multi-wavelength capabilities and high signal-to-noise of detectors on-board SDO allows for detailed study of these jet's evolution. We aim to exploit SDO's capabilities to reveal information on the jet dynamics and obtain estimates for plasma properties associated with the jet. We study the dynamics an EUV jet with AIA/SDO at a coronal hole boundary. The details of the jet evolution are discussed and measurements of the jet's parameters, e.g. length, width, life time, outward speed, are obtained. Further, automated emission measure analysis is exploited to determine estimates for the temperature and density of the jet. A propagating transverse wave supported by the jet spire is also observed. Measurement of the wave properties are exploited for magneto-seismology and are used in conjunction with the emission measure results to estimate the magnetic field strength of the jet. We present a detailed description of the jet's evolution, with new evidence for plasma flows, prior to the jet's initiation, along the loops at the base of the jet and also find further evidence that flows along the jet spire consist of multiple, quasi-periodic small-scale plasma ejection events. In addition, DEM analysis suggests that the jet has temperatures of $\log{5.89\pm0.08}$ K and electron densities of $\log{8.75\pm0.05}$ cm$^{-3}$. Measured properties of the transverse wave suggest the wave is heavily damped as it propagates along the jet spire with speeds of $\sim110$ km/s. The magneto-seismological inversion of the wave parameters provides values of $B=1.21\pm0.2$ G along the jet spire, which is in line with previous estimates for open fields in coronal holes.}, Comment: Accepted Astronomy and Astrophysics

The \textit{Sun Watcher using Active Pixel system detector and Image Processing}(SWAP) on board the \textit{PRoject for OnBoard Autonomy\todash 2} (PROBA\todash 2) spacecraft provides images of the solar corona in EUV channel centered at 174 \AA. These data, together with \textit{Atmospheric Imaging Assembly} (AIA) and the \textit{Helioseismic and Magnetic Imager} (HMI) on board \textit{Solar Dynamics Observatory} (SDO), are used to study the dynamics of coronal bright points. The evolution of the magnetic polarities and associated changes in morphology are studied using magnetograms and multi-wavelength imaging. The morphology of the bright points seen in low-resolution SWAP images and high-resolution AIA images show different structures, whereas the intensity variations with time show similar trends in both SWAP 174 and AIA 171 channels. We observe that bright points are seen in EUV channels corresponding to a magnetic-flux of the order of $10^{18}$ Mx. We find that there exists a good correlation between total emission from the bright point in several UV\todash EUV channels and total unsigned photospheric magnetic flux above certain thresholds. The bright points also show periodic brightenings and we have attempted to find the oscillation periods in bright points and their connection to magnetic flux changes. The observed periods are generally long (10\todash 25 minutes) and there is an indication that the intensity oscillations may be generated by repeated magnetic reconnection.

Background: Early breastfeeding support helps in achieving exclusive breastfeeding by reducing the incidence of painful breast problems. A comparison of the impact of lactation counselling by lactation consultants and nursing staff would help to optimise care. Methods: In this prospective study conducted in the maternity ward of a private sector tertiary care hospital in an urban area, breastfeeding outcomes such as achievement of exclusive breastfeeding and incidence of painful breast problems such as sore nipples and breast engorgement as well as breastfeeding technique were compared between two groups of mother-newborn dyads receiving support from lactation consultants (study group) and maternity ward nursing staff (control group), respectively. Results: A total of 477 mother-newborn pairs were enrolled. Sixty-three out of 225 mothers (28%) in the study group complained of sore nipples as against 122 out of 252 mothers (48.4%) in the control group (odd ratio [OR] 0.414, 95% confidence intervals [CI] 0.282-0.607, P < 0.001). Breast engorgement was reported in 45 mothers (20%) in the study group and 117 mothers (46.4%) in the control group (OR 0.288, 95% CI 0.191-0.434, P < 0.001). Two hundred and three newborns (90.2%) from the study group were discharged on exclusive breastfeeding as compared to 149 newborns (59.1%) in the control group (OR 0.156, 95% CI 0.094-0.260, P < 0.001). Conclusions: Lactation consultants are significantly better than nursing staff at improving breastfeeding outcomes by providing trained support to optimise breastfeeding position and attachment, thereby preventing painful breast problems. Wolbachia is a group of bacteria that is known to infect many arthropods and nematodes. It is one of the most common parasitic microbes and is possibly the most common reproductive parasite in the biosphere. The bacterium is best known for its ability to manipulate host reproductive biology where it can induce cytoplasmic incompatibility, parthenogenesis, feminization and male-killing. In the present study, onion thrips (Thrips tabaci Lindeman) were collected from 9 locations in India along with melon thrips (Thrips palmi Karny) and chilli thrips (Scirtothrips dorsalis). From the molecular level detection by using 16s rDNA, the Wolbachia infection has been detected in the onion thrips collected from 6 locations out of 9. Melon thrips and chilli thrips samples were also found to be infected by Wolbachia. The phylogenetic analysis revealed that all detected Wolbachia samples showed that all were distantly related to the previously known Wolbachia samples. [ABSTRACT FROM AUTHOR]

Objectives: To assess the maternal and fetal outcomes using new screening criteria with upper serum thyroid stimulating hormone (TSH) cut off as >3mIU/L, for diagnosing hypothyroidism in pregnancy. Materials and Methods: This study was a cross sectional study, carried out in the department of Obstetrics and Gynaecology. During one year of study period from February 2016 to January 2017, pregnant women with ≤ 20 weeks gestation, attending antenatal OPD were included in the study and they were followed till delivery. On the basis of serum TSH level, women were divided into study group with serum TSH level between 3.1 to 6 mIU/L, (new range to be studied) and an equal number of ages and parity matched control group with serum TSH levels between 0.4 to 3 mIU/L. The maternal and fetal outcomes were compared between study and control groups. Results: During the study period, in study group 96 women had serum TSH between 3.1-6 mIU/L. Maternal and fetal outcomes in both the groups were comparable. Study did not find any difference in the rate of spontaneous abortion between women in study and control group [p > 0.99]. There was no significant difference in the maternal complications like preeclampsia, gestational diabetes and placental abruption in the study and control group. Vaginal delivery and caesarean section were similar in both the groups. Study shows no significant difference of foetal outcome in study and control group [p > 0.05]. This review describes the preparation of nonedible vegetable oil (NEVO)-based polyols and their application in anticorrosive and antimicrobial polyurethane (PU) coatings. PUs are a class of versatile polymers made up of polyols and isocyanates. Renewable vegetable oils are promising resources for the development of ecofriendly polyols and the corresponding PUs. Researchers are interested in NEVOs because they provide an alternative to critical global food issues. The cultivation of plant resources for NEVOs can also be popularized globally by utilizing marginal land or wastelands. Polyols can be prepared from NEVOs following different conversion routes, including esterification, etherification, amidation, ozonolysis, hydrogenation, hydroformylation, thio-ene, acrylation, and epoxidation. These polyols can be incorporated into the PU network for coating applications. Metal surface corrosion and microbial growth are severe problems that cause enormous economic losses annually. These problems can be overcome by NEVO-based PU coatings, incorporating functional ingredients such as corrosion inhibitors and antimicrobial agents. The preferred coatings have great potential in high performance, smart, and functional applications, including in biomedical fields, to cope with emerging threats such as COVID-19.

The durability and reliability of battery management systems in electric vehicles to forecast the state of charge (SoC) is a tedious task. As the process of battery degradation is usually non-linear, it is extremely cumbersome work to predict SoC estimation with substantially less degradation. This paper presents the SoC estimation of lithium-ion battery systems using six machine learning algorithms for electric vehicles application. The employed algorithms are artificial neural network (ANN), support vector machine (SVM), linear regression (LR), Gaussian process regression (GPR), ensemble bagging (EBa), and ensemble boosting (EBo). Error analysis of the model is carried out to optimize the battery’s performance parameter. Finally, all six algorithms are compared using performance indices. ANN and GPR are found to be the best methods based on MSE and RMSE of (0.0004, 0.00170) and (0.023, 0.04118), respectively.

- Abstract
This paper aims to understand the different injury mechanisms involved with traumatic Bilateral Facet Dislocation (BFD) and fracture of the cervical spine. The intent is to demonstrate and elucidate tensile and compression induced injury mechanisms producing BFD by employing real-world crash investigations in association with all the past laboratory testing and studies done by numerous researchers. The study indicates that in a frontal crash scenario, maintaining the position of the shoulder belt is paramount, and any migration towards the base of the neck allows the fulcrum formation that amplifies distractive moments on the neck producing BFD. Similarly, in a rollover crash scenario, roof intrusion magnitude, and its rate along with roof deformation pattern can impose a rotational constraint on the head and plays a vital role in producing BFD. Roof design must address the formation of pocketing in the roof due to deformations imposing rotational head constraint exposing the neck to buckling and subsequent BFD as the roof intrusion continues.

The common γ chain (γc; IL-2RG) is a subunit of the interleukin (IL) receptors for the γc cytokines IL-2, IL-4, IL-7, IL-9, IL-15, and IL-21. The lack of appropriate neutralizing antibodies recognizing IL-2RG has made it difficult to thoroughly interrogate the role of γc cytokines in inflammatory and autoimmune disease settings. Here, we generated a γc cytokine receptor antibody, REGN7257, to determine whether γc cytokines might be targeted for T cell–mediated disease prevention and treatment. Biochemical, structural, and in vitro analysis showed that REGN7257 binds with high affinity to IL-2RG and potently blocks signaling of all γc cytokines. In nonhuman primates, REGN7257 efficiently suppressed T cells without affecting granulocytes, platelets, or red blood cells. Using REGN7257, we showed that γc cytokines drive T cell–mediated disease in mouse models of graft-versus-host disease (GVHD) and multiple sclerosis by affecting multiple aspects of the pathogenic response. We found that our xenogeneic GVHD mouse model recapitulates hallmarks of acute and chronic GVHD, with T cell expansion/infiltration into tissues and liver fibrosis, as well as hallmarks of immune aplastic anemia, with bone marrow aplasia and peripheral cytopenia. Our findings indicate that γc cytokines contribute to GVHD and aplastic anemia pathology by promoting these characteristic features. By demonstrating that broad inhibition of γc cytokine signaling with REGN7257 protects from immune-mediated disorders, our data provide evidence of γc cytokines as key drivers of pathogenic T cell responses, offering a potential strategy for the management of T cell–mediated diseases.
