23. Association Between Sexual Autonomy and Reproductive Tract Infections Among Married Women in India: A Cross-Sectional Study of a Nationally Representative Sample.

Abstract

This study explores the links between sexual autonomy and selfreported reproductive tract infections (RTIs) among married Indian women using data from the National Family Health Survey-5 (2019–2021). It also investigates heterogeneous associations of sexual autonomy, intimate partner violence, husband's controlling attitudes, women's participation in decision-making, and physical mobility with RTI. Binary logistic regression models were employed to unravel the association and interaction terms introduced in the model to assess heterogeneous associations. Results suggest that sexual autonomy was significantly associated with a lower likelihood of RTIs. Women's sexual autonomy may enable their agency to practice safer marital sexual relations, which may further translate into reducing the risk of RTIs. [ABSTRACT FROM AUTHOR]

Abstract

Objective: Critical components of the nasal endoscopic examination have not been definitively established for either the normal examination or for clinical disorders. This study aimed to identify concordance among rhinologists regarding the importance of examination findings for various nasal pathologies. Study Design: A consortium of 19 expert rhinologists across the United States was asked to rank the importance of findings on nasal endoscopy for 5 different sinonasal symptom presentations. Setting: An online questionnaire was distributed in July 2023. Methods: The questionnaire utilized JotForm® software and featured 5 cases with a set of 4 identical questions per case, each covering a common indication for nasal endoscopy. Rankings were synthesized into Normalized Attention Scores (NASs) and Weighted Normalized Attention Scores (W‐NASs) to represent the perceived importance of each feature, scaled from 0 to 1. Results: General concordance was found for examination findings on nasal endoscopy within each case. The perceived features of importance differed between cases based on clinical presentation. For instance, in evaluating postnasal drip, the middle meatus was selected as the most important structure to examine (NAS, 0.73), with mucus selected as the most important abnormal finding (W‐NAS, 0.66). The primary feature of interest for mucus was whether it was purulent or not (W‐NAS, 0.67). Similar analyses were performed for features in each case. Conclusion: The implicit framework existing among rhinologists may help standardize examinations and improve diagnostic accuracy, augment the instruction of trainees, and inform the development of artificially intelligent algorithms to enhance clinical decision‐making during nasal endoscopy. 27. Ectopic Pituitary Adenomas Presenting as Sphenoid or Clival Lesions: Case Series and Management Recommendations

Abstract

Background An ectopic pituitary adenoma presenting as a clival or sphenoid mass is a rare clinical occurrence that may mislead the clinician and result in unnecessary interventions or potential medicolegal consequences. Here, we present one of the largest multi-institutional case series and review the literature with an emphasis on radiological findings and critical preoperative workup. Methods Retrospective chart review. Results Nine patients were identified with ectopic pituitary adenomas of the sphenoid or clivus. There were four females and five males. Median age was 60 years old (range, 36-73 years). The most common presenting symptom was headache (56%). Five (56%) patients presented with a mass arising from the clivus while four (44%) presented with a mass in the sphenoid. Six (67%) patients demonstrated biochemical evidence of hypersecretion on full endocrinology panel. All masses showed evidence of enhancement with gadolinium with a propensity for adjacent bone involvement. Lesions also had a predilection for growth toward the cavernous sinus, carotid artery, or sellar floor. Surgical intervention was performed in eight patients (89%). In eight patients (89%), tumors demonstrated immunoreactivity to prolactin. Conclusions Pituitary adenomas can rarely present as an isolated sphenoid or clival mass. Lesions displayed similar magnetic resonance imaging findings with an erosive growth pattern toward the sellar floor, cavernous sinus, or adjacent carotid artery. Patients with clival or parasellar lesions with comparable features should have a preoperative workup which includes prolactin level and alert the physician to consider an ectopic pituitary adenoma in the differential to prevent unnecessary surgery and potential complications.

Abstract

Men's childhood exposure to inter-parental violence is linked with their indulgence, as partners and husbands, in Intimate Partner Violence (IPV). However, there is evidence that this linkage is variably moderated by different factors, including men's attitude toward IPV. This study is based on Domestic Violence Module (DVM) data from National Family and Health Survey (NFHS-5) conducted during 2019–2021. NFHS-5 used a uniform sample design representative at country and state levels, wherein one woman was interviewed face-to-face for DVM from each sampled household across India (N = 47,918). We aim to test the conditional effects of men's justifying IPV attitudes on the association of their childhood exposure to parental violence and indulgence in IPV. This was achieved by including an interaction term in the regression model. Findings suggest that men's "justifying attitude toward wife beating" independently associates with their indulgence in IPV and not in combination with "childhood exposure to inter-parental violence". It suggests that the linkage between men's childhood exposure to parental violence and indulgence in IPV is modifiable by positive changes in the men's attitude. We suggest a heightened policy emphasis on transforming men's attitudes through education, awareness-raising initiatives, and sensitization programs to address the issue of IPV in India. Abstract

The propagation of spherical shock waves through metallic mediums have been considered in this paper. The effect of overtaking disturbances in the propagation of spherical shock waves through solids, especially metals such as Aluminium and Copper, has been analysed by employing Chisnell-Chester-Whitham method. Under the influence of spherical shock waves flow variables have been obtained analytically as well as estimated numerically. Expressions for the pressure and the particle velocity behind the shock wave have also been obtained. It is found that shock strength in both the metals decreases as shock propagates through them. Shock velocity and modified shock velocity depict a decreasing pattern with increasing propagation distance. These observations suggest many industrial and medical applications of shock waves.
