1. Mining Breast Cancer Classification Rules from Mammograms

Abstract

Breast cancer is a leading cause of cancer death in women. Early diagnosis and treatment are crucial to reduce the mortality rate and increase patients’ lifespan. Mammography is effective in early detection. This study proposes a computer-aided diagnosis system based on the mini-Mammographic Image Analysis Society database for analyzing mammograms. After selecting the regions of interest, we computed three typical features: the shape, spatial, and spectral domain features. We then applied the structural equation model to obtain relations between the features and the breast tissue type, lesion class, and tumor severity after feature extraction by information gain. Finally, we used the decision tree and classification and regression tree to construct computer-aided diagnosis rules; we generated 10 rules for predicting the classification of abnormal lesions and 11 rules for classifying the tumor severity. These rules can help clinicians detect and identify breast cancer efficiency from mammograms and improve medical care quality.

2. Superconductivity with Wannier-Stark Flat Bands

Abstract

We investigate superconducting transport in the DC field induced Wannier-Stark flat bands in the presence of interactions. Flat bands offer the possibility of unconventional high temperature superconductivity, where the superfluid weight, $D_s$, is enhanced by the density overlap of the localized states. However, construction of flat bands typically requires very precise tuning of Hamiltonian parameters. To overcome this difficulty, we propose a feasible alternative to realize flat bands by applying a DC field in a commensurate lattice direction. We systematically characterize the superconducting behavior on these flat bands by studying the effect of the DC field and attractive Hubbard interaction strengths on the wavefunction, correlation length, pairing order parameter and the superfluid weight $D_s$. Our main result is that the superfluid weight is dramatically enhanced at an optimal value of the interaction strength and weak DC fields.

3. Designer Flat Bands: Topology and Enhancement of Superconductivity

Abstract

We construct quasi one-dimensional topological and non-topological three-band lattices with tunable band gap and winding number of the flat band. Using mean field (MF) and exact density matrix renormalization group (DMRG) calculations, we show explicitly how the band gap affects pairing and superconductivity (SC) in a Hubbard model with attractive interactions. We show excellent agreement between MF and DMRG. When a phase twist is applied on the system, a phase difference appears between pairing order parameters on different sublattices, and this plays a very important role in the SC density. The SC weight, $D_s$, on the gapped topological, $W\neq0$, flat band increases linearly with interaction strength, $U$, for low values, and with a slope that depends on the details of the compact localized state at $U=0$. As $U\to 0$ for the gapped non-topological flat band ($W=0$), $D_s$ decays with a power law faster than quadratic but slower than exponential. This indicates that isolated non-topological flat bands are less beneficial to SC. In the gapless case (flat band touching the band above it), we find at low $U$ (both for $W=0$ and $W\neq 0$) that $D_s\propto U^\varphi$ with $\varphi<1$ contrary to the $U{\rm ln}\, ({\rm const.}/U)$ behavior reported in the literature. In other words, $D_s$ increases faster than linearly for low $U$ thus favoring SC at weak interaction more than the gapped case. For systems with touching bands, we observe that the one-body correlation length, $\xi$, diverges as a power law as $U\rightarrow0$, while for the isolated flat band $\xi(U\to 0)$ is a constant smaller than one lattice spacing. Both behaviors are distinct from the exponential divergence of $\xi$ in the dispersive case. Our results re-establish that the BCS mean field and quantum metric alone are insufficient to characterize SC at weak coupling.

4. A Split-face Study of Novel Robotic Prototype vs Human Operator in Skin Rejuvenation Using Q-switched Nd:Yag Laser: Accuracy, Efficacy and Safety

Abstract

Background: Robotic technologies involved in skin laser are emerging. Objective: To compare the accuracy, efficacy and safety of novel robotic prototype with human operator in laser operation performance for skin photo-rejuvenation. Methods: Seventeen subjects were enrolled in a prospective, comparative split-face trial. Q-switch 1064nm laser conducted by the robotic prototype was provided on the right side of the face and that by the professional practitioner on the left. Each subject underwent a single time, one-pass, non-overlapped treatment on an equal size area of the forehead and cheek. Objective assessments included: treatment duration, laser irradiation shots, laser coverage percentage, VISIA parameters, skin temperature and the VAS pain scale. Results: Average time taken by robotic manipulator was longer than human operator; the average number of irradiation shots of both sides had no significant differences. Laser coverage rate of robotic manipulator (60.2 +-15.1%) was greater than that of human operator (43.6 +-12.9%). The VISIA parameters showed no significant differences between robotic manipulator and human operator. No short or long-term side effects were observed with maximum VAS score of 1 point. Limitations: Only one section of laser treatment was performed. Conclusion: Laser operation by novel robotic prototype is more reliable, stable and accurate than human operation.

5. Pairing and superconductivity in quasi one-dimensional flat band systems: Creutz and sawtooth lattices

Abstract

We study the pairing and superconducting properties of the attractive Hubbard model in two quasi one-dimensional topological lattices: the Creutz and sawtooth lattices. They share two peculiar properties: each of their band structures exhibits a flat band with a non-trivial winding number. The difference, however, is that only the Creutz lattice is genuinely topological, owing to a chiral (sub-lattice) symmetry, resulting in a quantized winding number and zero energy edge modes for open boundary conditions. We use mean field and exact density matrix renormalization group in our work. Our three main results are: (a) For both lattice systems, the superconducting weight, $D_s$, is linear in the coupling strength, $U$, for low values of $U$; (b) for small $U$, $D_s$ is proportional to the quantum metric for the Creutz system but not for the sawtooth system because its sublattices are not equivalent; (c) conventional BCS mean field is not appropriate for such systems with inequivalent sublattices. We show that, for a wide range of densities and coupling strengths, these systems are very well described by a full multi-band mean field method where the pairing parameters and the local particle densities on the inequivalent sublattices are variational mean field parameters.

16. Can LLMs Have a Fever? Investigating the Effects of Temperature on LLM Security.

Abstract

Large Language Models (LLMs) play a crucial role in increasing productivity across various applications, yet concerns about their security persist. We defined a novel research area for LLM security -- grey box testing, which involved adjustments of partially known information about LLMs, i.e. parameters such as temperature. This is the first investigation on the impact of temperature on LLM security, more specifically, jailbreaking. Jailbreak prompts, concatenated with inappropriate questions, were sent into top-performing open-source LLMs -- Vicuna, Llama 2, and Mistral, with benchmarking against ChatGPT. Experiments were repeated over 5 temperature points: 0.0, 0.25, 0.5, 0.75 and 1.0, revealing a strong correlation between temperature and jailbreaking success. An increase in temperature generally results in higher jailbreaking success for Llama 2 and ChatGPT, and the converse is true for Vicuna and Mistral. We revised our hypothesis such that LLMs with low jailbreak success rate at 0 temperature are more vulnerable to jailbreaking as temperature increases, while LLMs with high jailbreak success rate at 0 temperature are less susceptible to jailbreaking as temperature rises. This research emphasises the importance of cautious usage of open-source LLMs and highlights the need for further research under grey box settings in other areas including data extraction and hallucinations. Abstract

Introduction: Tuberculosis (TB) remains endemic in Singapore. Singapore's clinical practice guidelines for the management of tuberculosis were first published in 2016. Since then, there have been major new advances in the clinical management of TB, ranging from diagnostics to new drugs and treatment regimens. The National TB Programme convened a multidisciplinary panel to update guidelines for the clinical management of drug-susceptible TB infection and disease in Singapore, contextualising current evidence for local practice. Method: Following the ADAPTE framework, the panel systematically reviewed, scored and synthesised English-language national and international TB clinical guidelines published from 2016, adapting recommendations for a prioritised list of clinical decisions. For questions related to more recent advances, an additional primary literature review was conducted via a targeted search approach. A 2-round modified Delphi process was implemented to achieve consensus for each recommendation, with a final round of edits after consultation with external stakeholders. Results: Recommendations for 25 clinical questions spanning screening, diagnosis, selection of drug regimen, monitoring and follow-up of TB infection and disease were formulated. The availability of results from recent clinical trials led to the inclusion of shorter treatment regimens for TB infection and disease, as well as consensus positions on the role of newer technologies, such as computer-aided detectionartificial intelligence products for radiological screening of TB disease, next-generation sequencing for drugsusceptibility testing, and video observation of treatment. It focused on how social reproduction was possible in school education, how school education was worked in general, and how school education was developed. Moreover, it employed a three-scale analysis of situational analysis and a historical analysis to analyze the above three research foci. In situational analysis, at classroom scale, a symbolic reproduction of the lifeworld of students was depicted, whereas at school scale, an uncoupling of the system from the lifeworld was seen, and finally, at regional scale, a colonization of the lifeworld by the system was concluded. Moreover, its historical development depicted a phenomenon of a colonization of the lifeworld by the system. In sum, the above, other than at classroom scale, showed mainly an undesired and distorted way of development of school education. However, educators and the public sphere strove for the desired or logic development, and resisted the undesired or dynamic one conducted by the government administration. Based on the summary and conclusions drawn from the analysis of school education in Hong Kong, the study elucidated a new and critical approach in sociology of education. This approach was set out in two dimensions. The first dimension was a three-level analysis of education, with meta-theoretical level concerned learning and teaching action as communicative action, with methodological level concerned interpretive understanding, and with empirical level concerned development of education as a process of rationalization. The second dimension was three concerns in the study of school education, with communicative rationality as the rationality problematic in inquiry, with the integration of the perspectives of action and the system, and with the logic development of education. This approach, based on the theory of communicative action, did provide a conceptual framework for a fuller picture of education, and for a pathway to further development in education.

Abstract

Insufficient sleep contributes negatively to child developmental processes and neurocognitive abilities, which argues the need for implementing interventions to promote sleep health in children. In this study, we evaluated the effectiveness of a multimodal and multilevel school-based sleep education program in primary school children using a cluster randomized controlled design. Twelve schools were randomly assigned to either the sleep education or nonactive control groups. The sleep education group included a town hall seminar, small class teaching, leaflets, brochures, and a painting competition for children. Parents and teachers were invited to participate in a one-off sleep health workshop. Parental/caregiver-reported questionnaires were collected at baseline and 1-month follow-up. A total of 3769 children were included in the final analysis. There were no significant improvements observed in the sleep-wake patterns, daytime functioning, and insomnia symptoms between the two groups at follow-up, whereas the intervention group had significantly improved parental sleep knowledge than the controls (paternal: adjusted mean difference: 0.95 [95% confidence interval (CI): 0.18 to 1.71]; maternal: adjusted mean difference: 0.87 [95% CI: 0.17 to 1.57]). In addition, children receiving the intervention had a lower persistence rate of excessive beverage intake (adjusted odds ratio: 0.49 [95% CI: 0.33 to 0.73]), and experienced greater reductions in conduct problems (adjusted mean difference: 0.12 [95% CI: 0.01 to 0.24]) compared with the controls at 1-month of follow-up. Moreover, a marginally significant reduction for emotional problems in the intervention group was also observed (adjusted mean difference: 0.16 [95% CI: −0.00 to 0.32]). These findings demonstrated that school-based sleep education was effective in enhancing parental sleep knowledge and improving behavioral outcomes in children, but not sufficient in altering the children’s sleep-wake patterns and sleep problems.

Abstract Background Most malignant peripheral pulmonary lesions (PPLs) are situated in the peripheral region of the lung. Although the ultrathin bronchoscope (UTB) can access these areas, a robust navigation system is essential for precise localisation of these small peripheral PPLs. Since many UTB procedures rely on automated virtual bronchoscopic navigation (VBN), this study aims to determine the accuracy and diagnostic yield of the manual bronchial branch tracing (BBT) navigation in UTB-guided radial endobronchial ultrasound (rEBUS) procedures. Methods Single-centre retrospective study of UTB-rEBUS patients with PPLs smaller than 3 cm over a two year period. Results Our cohort consisted of 47 patients with a mean age of 61.6 (SD 9.53) years and a mean target size of 1.91 (SD 0.53) cm. Among these lesions, 46.8% were located in the 6th airway generation, and 78.7% exhibited a direct bronchus sign. Navigation success using BBT was 91.5% based on positive rEBUS identification. The index diagnostic yield was 82.9%, increasing to 91.5% at 12 months of follow-up. Malignant lesions accounted for 65.1% of cases, while 34.9% were non-malignant. The presence of a direct bronchus sign was the sole factor associated with higher navigation success and diagnostic yield. Abstract

Background Lymphadenitis is the most common manifestation of non-tuberculous mycobacteria (NTM) infection in children. We describe the epidemiology and clinical characteristics of NTM lymphadenitis, determine diagnostic yield from tissue sampling, and review management and outcomes. Methods This was a 10-year retrospective review of children aged 0–16 years diagnosed with NTM cervicofacial lymphadenitis who were seen in a pediatric infectious disease clinic in a tertiary public hospital. Data relating to patient demographics, clinical features, surgical and antimicrobial treatment, complications, and outcomes were retrieved from patients' electronic medical records and analyzed. Results There were 48 episodes of NTM cervicofacial lymphadenitis in 45 children (17 males and 28 females). Of these episodes, 43.7% manifested as a unilateral single node, mostly parotid (39.6%) and submandibular (29.2%). All patients underwent diagnostic fine-needle aspiration or surgery. Surgical excision more frequently yielded positive histological findings (P =.016). NTM was identified in 22/48 episodes (45.8%) via culture or molecular sequencing. Mycobacterium abscessus was most commonly found (47.8%). Thirty-eight children (79.2%) received antibiotics. Outcomes in 43 episodes revealed full resolution in 69.8%, while 25.6% had de novo disease and 4.6% experienced recurrence at the same site. Overlying skin changes and multiple or bilateral nodal diseases were significantly associated with de novo disease or recurrence (P =.034 and.084, respectively). Complications occurred in 11/70 (15.7%) procedures. Antibiotic-associated adverse effects occurred in 14/38 (36.8%) episodes. Conclusions NTM lymphadenitis remains a challenging condition. 42. Bolstering functionality in multilayer and bilayer WS2 via focused laser micro-engraving.

Abstract

Laser thinning of multilayer transition metal dichalcogenide (TMD) crystals has long been explored for its potential to produce localized domains of optically functional monolayers. Despite the high spatial control and resolution of the laser, the range of appropriate applications for these laser-thinned domains is still limited by the peripheral regions of unaltered thick multilayers. Herein, we report a newfound property of the laser-thinned regions to adhere strongly to the underlying substrate when lasered down to the monolayer limit. Upon a brief sonication in various solvents, the surrounding pristine multilayers can be lifted off, solely leaving behind the post-lasered monolayer patterns. Coupled with the flexible maneuver of the laser, various monolayer designs can be achieved, including micro-antenna, waveguide, and electrode geometries with lateral line widths at 1.5 µm resolution. Furthermore, we also detail the optical properties of the monolayers obtained from laser thinning, with those acquired from multilayers containing more defect-bound excitons, whereas those lasered from bilayers presenting a trion-dominated profile similar to as-grown monolayers. Together, these findings allow for faster and more efficient multilayer removal and areal shaping of optically active monolayer domains, in comparison to multistep and complex standard lithographic etching (lithography–etch–develop) procedures. Abstract

Background: Pediatric pheochromocytoma is very rare, accounting for only about 1% of pediatric hypertension. Case Presentation: We presented a case of pediatric pheochromocytoma in a 12-year-old girl undergoing laparoscopic adrenalectomy, with the aim of highlighting an anesthetic challenge. Conclusion: Preoperative screening of hereditary conditions, investigations for end-organ dysfunction, preoperative blood pressure optimization, intensive intraoperative hemodynamic management and good communication with surgeons were the cornerstones for this successful surgery.
