1 januari 2020, Contains fulltext : 219893.pdf (Publisher’s version ) (Closed access), BACKGROUND: Treatment of venous thromboembolism in children is based on data obtained in adults with little direct documentation of its efficacy and safety in children. The aim of our study was to compare the efficacy and safety of rivaroxaban versus standard anticoagulants in children with venous thromboembolism. METHODS: In a multicentre, parallel-group, open-label, randomised study, children (aged 0-17 years) attending 107 paediatric hospitals in 28 countries with documented acute venous thromboembolism who had started heparinisation were assigned (2:1) to bodyweight-adjusted rivaroxaban (tablets or suspension) in a 20-mg equivalent dose or standard anticoagulants (heparin or switched to vitamin K antagonist). Randomisation was stratified by age and venous thromboembolism site. The main treatment period was 3 months (1 month in children <2 years of age with catheter-related venous thromboembolism). The primary efficacy outcome, symptomatic recurrent venous thromboembolism (assessed by intention-to-treat), and the principal safety outcome, major or clinically relevant non-major bleeding (assessed in participants who received >/=1 dose), were centrally assessed by investigators who were unaware of treatment assignment. Repeat imaging was obtained at the end of the main treatment period and compared with baseline imaging tests. This trial is registered with ClinicalTrials.gov, number NCT02234843 and has been completed. FINDINGS: From Nov 14, 2014, to Sept 28, 2018, 500 (96%) of the 520 children screened for eligibility were enrolled. After a median follow-up of 91 days (IQR 87-95) in children who had a study treatment period of 3 months (n=463) and 31 days (IQR 29-35) in children who had a study treatment period of 1 month (n=37), symptomatic recurrent venous thromboembolism occurred in four (1%) of 335 children receiving rivaroxaban and five (3%) of 165 receiving standard anticoagulants (hazard ratio [HR] 0.40, 95% CI 0.11-1.41). Abstract

System instability and chaotic response are the failure modes that could significantly impact the reliability and operating safety of high-speed rotor-dynamical machines. Initiation and propagation of surface cracks in rotary shafts are common causes for such failure modes. To be able to detect the onset and progression of these faults will considerably extend the lifetime and improve the reliability of the mechanical system. A waveletbased algorithm effective in identifying mechanical chaotic response has been applied to determine the nonlinear dynamical characteristics of a model-based, cracked rotor. This investigation confirms reported correlation of surface crack breathing with rotor chaotic motions. The effectiveness of the algorithm in detecting rotor-dynamic instability induced by mechanical faults as contrast to algorithms that are based on nonlinear dynamics is discussed. The results show not just the feasibility of the algorithm in mechanical fault diagnosis but also suggest its applicability to in-line, real-time condition monitoring at both the system and component levels. Abstract

A coupled-mode model, which describes the threshold gain and the effective reflectivity of a surface emitting DBR laser, reveals that the removal of even modes occurs when the cavity is developed to operate on the odd modes. The addition of minimal disturbance to the dielectric constant inhibits these odd, symmetrical lateral modes. The narrow far-field pattern characterizes the single mode through which lasing takes place.

Abstract

Life-long ART for pregnant and breastfeeding women (Option B+) was rolled-out in Zimbabwe from 2014 with simultaneous raising of the CD4 treatment-threshold in non-pregnant/breastfeeding adults and children >5 years to 500 cells/mm. \ud \ud Lablite is an implementation project in Zimbabwe, Malawi and Uganda evaluating ART rollout. Routine patient-level data were collected for 6 months prior to and 12 months after Option B+ rollout at a district hospital and three primary care facilities in Zimbabwe (two with outreach ART; one with no ART provision prior to Option B+). \ud \ud Between September 2013-February 2015 there were 1,686 ART initiations in the four facilities; 91% adults and 9% children aged 350 after the CD4-threshold increase. Abstract

Cryptococcal meningitis associated with human immunodeficiency virus (HIV) infection causes more than 600,000 deaths each year worldwide. Treatment has changed little in 20 years, and there are no imminent new anticryptococcal agents. The use of adjuvant glucocorticoids reduces mortality among patients with other forms of meningitis in some populations, but their use is untested in patients with cryptococcal meningitis.In this double-blind, randomized, placebo-controlled trial, we recruited adult patients with HIV-associated cryptococcal meningitis in Vietnam, Thailand, Indonesia, Laos, Uganda, and Malawi. All the patients received either dexamethasone or placebo for 6 weeks, along with combination antifungal therapy with amphotericin B and fluconazole.The trial was stopped for safety reasons after the enrollment of 451 patients. Mortality was 47% in the dexamethasone group and 41% in the placebo group by 10 weeks (hazard ratio in the dexamethasone group, 1.11; 95% confidence interval [CI], 0.84 to 1.47; P=0.45) and 57% and 49%, respectively, by 6 months (hazard ratio, 1.18; 95% CI, 0.91 to 1.53; P=0.20).
