4. Acute lower motor neuron syndrome and spinal cord gray matter hyperintensities in HIV infection.

Abstract

ObjectiveTo describe a novel manifestation of lower motor neuron disease in patients with well-controlled HIV infection.MethodsA retrospective study was performed to identify HIV-positive individuals with acute, painful lower motor neuron diseases.ResultsSix patients were identified with HIV and lower motor neuron disease. Two patients met the inclusion criteria of well-controlled, chronic HIV infection and an acute, painful, unilateral lower motor neuron paralytic syndrome affecting the distal portion of the upper limb. These patients had segmental T2-hyperintense lesions in the central gray matter of the cervical spinal cord on MRI. One patient stabilized and the second patient improved with immunomodulatory therapy.ConclusionsThis newly described syndrome expands the clinical spectrum of lower motor neuron diseases in HIV.

A 49-year-old woman originally from India who was seen in a neurology clinic because of painful peripheral nerve disease and skin lesions was diagnosed with leprosy. Although leprosy was feared in ancient times, it is not particularly infectious. It is caused by a bacteria related to the bacteria that causes tuberculosis. The patient was treated with dapsone and rifampin.

Abstract

The European Union (EU) is a prominent player in the politics of climate change, operating as an authoritative regional actor that influences policy-making at the national and international levels. The EU’s climate change policies are thus subjected to multiple pressures that arise from the domestic politics of its twenty-seven individual member states and the international politics of non-EU states with which it negotiates. Facing these multiple pressures, how and why could such a non-traditional actor develop into a prominent player at different levels of climate change policy-making? This article argues that the EU’s rise to prominence can be understood by tracking a number of historical-legal institutional developments at the domestic and international levels. The article also provides a preliminary investigation of the EU emissions trading scheme, a new institutional mechanism that illustrates the policy pressures arising from different levels.

Abstract

Motor neuron diseases are usually chronic and often progressive. They include ailments such as polio and amyotrophic lateral sclerosis (ALS), which attack the nerve cells in the brain and spinal cord that directly control the movement of muscles. In the case of ALS, sometimes called Lou Gehrig's disease, the disease generally progresses inexorably to death. However, among thousands of cases of ALS, there are scattered a few patients who were diagnosed with ALS, but recovered completely. The authors present the case histories of four patients who developed symptoms of motor neuron disease and then recovered. The patients ranged in age from 39 to 74. The initial symptoms were generally muscle weakness, twitching, and fasciculations, in which bundles of muscle cells quiver involuntarily. Many disorders other than motor neuron disease can produce such symptoms, such as systemic illness, poisoning with lead or other heavy metals, and cancer. However, these diseases and other disorders that might produce similar symptoms were all ruled out in the present cases. Electrophysiological recordings showed changes consistent with motor neuron disease. Using current diagnostic standards, two of the patients were diagnosed with ALS; the other two patients were suspected of having ALS. However, all four patients recovered without treatment over periods ranging from 5 to 12 months. There is no diagnostic procedure that can prove that these patients had a motor neuron disorder resembling ALS. The symptoms were strikingly similar, however, and no other cause could be identified. Abstract

Blockchain technologies and business models offer a multitude of decentralized and distributed services, but our understanding of the conditions necessary to reach the emerging technology’s full potential is incomplete. The benefits are perhaps most pronounced in developing economies where high institutional misalignment, or even dysfunction, leaves much room for improvement, but we know even less about these contexts due to a dearth of primary research. In this article, we survey thirteen key entrepreneurs and managers in Kenya to identify and observe the current challenges associated with unlocking the potential of blockchain-enabled services. We identify three conditions that need to be met: sufficient technical capacity, appropriate regulative interventions, and most importantly, the adoption of decentralization logics. To truly leverage blockchain technology, market and institutional actors need to collaborate to overcome the paradox of centrally legitimate, decentralized solutions.
