Aim: Chronic Subdural hematomas (CDSH) first reported in 1656 by JJ Wepfer. In 1857 CSDH was defined as Pachimenengitis hemoragyca interna. Frequency of CSDH changes with age progression. İt’s reported in literature; CDSH coexists %60-80 with minor head trauma. We aimed to present our experience in follow-up and treatment of patients with CSDH which are frequently encountered in routine neurosurgical practice. Materials and Methods: Cases of CSDH which were operated between January 2016- December 2017 evaluated retrospectively from Clinical Files. The patients' clinical files were evaluated retrospectively according to age, gender, personal background, complaint, use of anticoagulant, presence of head trauma, radiologic images, hematologic parameters, type of anesthesia, surgical method and recurrence rates.Results: The study consists of total 67 cases which %20,9 (n=14) female and %79,1 (n=53) male. The ages of the cases ranged from 16 to 91 years with an average of 68,60 ± 16,45 years. The duration of hospitalization according to gender and age, the duration of hospitalization according to the presence of additional disease and the types of additional diseases seen, the duration of hospitalization according to drug use and anesthesia type (p> 0.05), between N / L and residence time (r: 0,222; p > 0.05) were not statistically significant.Conclusions: However, it may seem so basic; Choices about Surgical techniques and type of anesthesia are effective about mortality and morbidity. Because of duration of surgery is shorter; it is shown that Surgeries performed with Sedoanalgesia and local anesthesia can be used safely especially for patients with comorbidity., Amaç: Kronik subdural hematom (KSDH) ilk defa 1656 yılında JJ Wepfer tarafından bildirilmiş, 1857 yılında Virchow tarafından Pakimenenjitis hemorajika interna olarak tanımlanmıştır. KSDH sıklığı yaşla değişmektedir. Hafif kafa travması ile birlikteliği literatürde %60-80 oranında bildirilmektedir. Bu çalışma ile rutin nöroşirurji pratiğinde sık karşılaşılan KSDH hastalarının takip ve tedavilerinde klinik deneyimimizi sunmayı amaçladık.Gereç ve Yöntem: 2016 Ocak - 2017 Aralık tarihleri arasında KSDH tanısı ile opere edilen hastaların klinik dosyalarından kayıtlar incelendi. Hastaların klinik dosyalarından yaş, cinsiyet, özgeçmiş, geliş şikâyeti, antikoagülan kullanımı, kafa travması varlığı, radyolojik görüntüleri, hematolojik parametreleri cerrahide uygulanan anestezi şekli, cerrahi method, rekürrens oranları retrospektif olarak değerlendirildi.Bulgular: Çalışma, hastaların %20,9’u (n=14) kadın, %79,1’i (n=53) erkek olmak üzere toplam 67 olgu ile gerçekleştirilmiştir. Olguların yaşları 16 ile 91 arasında değişmekte olup, ortalama 68,60±16,45 yıldır. Cinsiyete ve yaşa göre yatış süreleri, ek hastalık varlığı ve görülen ek hastalık türlerine göre yatış süreleri arasında, ilaç kullanımı ve anestezi şekline göre yatış süreleri arasında(p>0,05), N/L ile yatış süresi arasında (r:0,222; p>0,05) istatistiksel olarak anlamlı farklılık saptanmamıştır.Sonuç: Cerrahi seçimler, anestezi tercihi ne kadar basit olarak görülse de mortalite ve morbiditede oldukça etkilidir. Sedoaneljezi ve lokal anestezi altında yapılan ameliyatların, cerrahi sürenin kısa olması nedeniyle özellikle komorbiditesi olan hasta grubunda güvenle tercih edilebileceğini göstermektedir.

İnsan bu yolla özündeki yalnızlığını – kesinçözüm mümkün olmamakla beraber - kısmen hafifletmiş olur. Said Nursi de insanadair yaptığı tanımlamalarda Varoluşçu bir yaklaşım sergileyerek onu üstündonanımı yüksek düşünme kapasitesiyle dünyaya yalnız ve tek başına olarakgelmiş bir varlık olarak ifade etmektedir. Aynı zamanda bulunduğu zaman ve mekândüzleminde her birey duyguları aracılığıyla nesnelere yönelik ilişkilerkurmakla beraber içinde bulunduğu zaman akışı bu bağları kopararak insanaayrılık acısını yaşatmakta ve onu tek başına bırakmaktadır. Bu noktada Nursîinsanın Allah ile kurduğu dinamik bir ilişki olan ‘iman deneyimi’ üzerindedurmakta ve iman eden insan (mümin) için -Allah’ın devamlılığı bağlamında- ayrılıkve ölümlerin zahirî olup hakikatte ölen ya da ayrılan her varlık daha üst birhayat formuna geçmektedir. Bu nedenle ölüm ve ayrılık olgusuna dinî başa çıkmabağlamında yok oluş değil, daha üst bir varoluş olarak bakılmalıdır.

Conclusion: Silicone injection of the cadaveric cow brain, based on the anatomical and physiological assessment of the vasculature of the specimen for microanatomical studies, is suggested as an alternative to using human brain specimens.

WOS: 000351690100018, Objective: Percutaneous lumbar transforaminal epidural steroid injections are frequently used to treat low back pain. However, many reports have indicated that this procedure has a high risk of morbidity. The aim of this study was to determine the difference in the incidence of vascular penetration during lumbosacral transforaminal epidural injections between patients with previous lumbar spinal surgery and patients who did not undergo surgery. Design: This is a prospective clinical study. Methods: In this study, patients who received lumbar transforaminal epidural injections at an outpatient spine center were included and followed. An interventional spine physician observed fluoroscopically guided lumbosacral transforaminal epidural steroid injections under live fluoroscopy and recorded contrast patterns. A total of 296 fluoroscopically guided lumbosacral transforaminal epidural steroid injections under live fluoroscopy in 108 patients were evaluated. Results: The incidence of vascular contrast patterns was observed under live fluoroscopy. Vascular injections were observed in 28 of the 296 injections, for an overall incidence of 9.5%. The incidence of intravascular injections among patients was 40.0% (20/50) in the previously operated group and 13.8% (8/58) in the non-operated group. Conclusion: The aim of this study was to develop an innovative model to create a life-like microneurosurgical training system. This model simulates bifrontal and frontolateral approaches performed on the human brain.

Sacrum fractures are rare pathologies seen after spinal traumas. The incidence of a sacral fracture after trauma is 0.6% in childhood. A Malgaigne fracture is composed of fractures and dislocations of the anterior and posterior regions of the pelvis. This is the first reported case of Malgaigne fracture during childhood. A 12-year-old girl was admitted to our emergency room after having suffered a fall. Radiological tests revealed a zone 3 sacral fracture according to the Denis scoring, a subtype 2 sacral fracture according to the Roy-Camille classification, and a detachment in the symphysis pubis. Appropriate load distribution through a bilateral L5–S1–S2 transpedicular screw and a bilateral iliac wing screw, as well as neural decompression were performed together with an S1–S2 total laminectomy. It is very difficult to make a generalization for treatment of sacral fractures and Malgaigne fractures in childhood due to the small number of patients. Each patient should be individualized and lumbosacroiliac instability should be treated.

Intracranial lipomas are seen less than %1 in of all intracranial tumours. Mostly placed in midbrain. They occur often in pericallosal area, but can be seen any part of the brain. They are diagnosed incidentally and mostly are asympthomatic. They become sympthomatic rarely because of mass effect. Most sympthom is headache. Nevertheless epilepsy, hemiplegia are also seen. This benign masses can be diagnosed on MRI and CT scans. Differential diagnosis should be done radiologically. As benign, occuring in deep regions and being adherent to periferic tissues; radical surgeries are not adviced for treatment.We concede to report this intracranial lipoma with a review of litreture though being a giant lipoma and been diagnosing incidentally., İntrakranial lipomlar(İKL) tüm intrakraniyal tümörlerin %1'inden azını oluştururlar. Genellikle orta hatta yerleşim gösterirler. En sık perikalozal bölgede görülmelerine rağmen kraniyal bölgenin her yerinde görülebilirler. Çoğunlukla rastlantısal olarak yakalanırlar ve genellikle asemptomatiktirler. Çok nadiren kitle etkisi nedeni ile semptomatik hale gelirler. En sık semptom baş ağrısıdır. Bununla birlikte epilepsi, hemipleji gözlenebilir. Bu benign karekterdeki kitlelere BBT ve kraniyal magnetik rezonans(MRG) görüntüleme ile tanı konulabilir. Radyolojik olarak ayrıcı tanı yapılmalıdır. Benign karakterde olması, lokalizasyon olarak derin yerlesimli ve cevre dokular ile yapışıklık oluşturmaları nedeniyle radikal cerrahi önerilmemektedir. Biz rastlantısal olarak rastlanan bir olguyu boyutlarının büyüklüğü nedeniyle literatür eşliğinde sunmayı uygun bulduk.

The majority of the spinal cord involvement in the pediatric age group are sarcomas. Granulocytic sarcoma is a rare tumor composed of immature granulocytes. The most commonly occur during the course of acute myeloid leukemia. Granulocytic sarcoma most often occurs during the course of acute myeloid leukemia. Sometimes the first sign of leukemia may granulocytic sarcoma. Admitted to our emergency department because of acute onset paraparesis 16-year-old male patient was diagnosed as a result of spinal extradural mass was detected between 3-7 thoracic vertebrae. Patients were operated as an emergency. Pathology results came as granulocytic sarcoma. Bone marrow examination and flow cytometry results of acute myeloid leukemia was diagnosed., Pediatrik çağda spinal tutulum gösteren kitlelerin çoğunluğunu sarkomlar oluşturmaktadır. Granülositik sarkom, immatür granülositlerin oluşturduğu nadir rastlanan bir tümördür. En sık akut myelositer löseminin seyri sırasında ortaya çıkmakta olup, bazen de ilk bulgusu granülositik sarkom olabilmektedir. Akut gelişen paraparezi nedeniyle acil kliniğimize başvuran 16 yaşındaki erkek olgunun yapılan tetkikleri sonucunda torakal 3-7 vertebraları arasında ekstradural spinal kitle saptanması üzerine olgu, acil olarak ameliyat edildi. Patoloji sonucu granülositik sarkom olarak geldi. Yapılan kemik iliği muayenesi ve flow sitometri sonucu, akut myelositer lösemi tanısı kondu.

Introduction: Benign tumors of the skin are one of the most common pathologies that can be seen in clinical practice. Scalp is one of the most common localization for the subcutanous skin lesions. Many benign tumors can be occured from different cells that forms the histological structure of the skin. The trichilemmal cyst, epidermoid cyst and lipoma are tumoral lesions that causes subcutaneous nodules in the scalp. This retrospective study examines,among 52 patients, 62 subcutaneous nodular lesions that excised totally from scalp.Material-method: In this retrospective study,62 subcutanous nodular lesions that excised totally from scalp, among 52 patients who were admitted to Dermatology and/or Neurosurgery Clinics of Sakarya University Training and Research Hospital between 2009 and 2011,are examined.Findings: 27 (51,9 %) of the patients were female, and 25 (48,1 %) were male. The mean age was found as 47,5 (ages 21-82). 34 (54,8 %) lesions were revealed as trichilemmal cysts, 16 (25,8 %) of them were revealed as lipomas and 8 (12,4 %) of them were found as epidermoid cysts, the rest of the pathologies were 2 (3,2 %) pilomatrixomas, and 2 (3,2 %) fibrolipomas. Result: Our study signified that trichilemmal cysts are the most common subcutaneous nodules that occures on scalp. Additionally, lipomas and epidermoid cysts were the other common subcutanous scalp lesions that can be seen in clinical practice., Giriş: Derinin selim tümörleri klinik pratikte sık gözlenen patolojilerdendir. Saçlı deri, derinin selim tümörlerinin yerleşimi açısından sık görülen bölgelerden birisidir. Derinin histolojik yapısını oluşturan farklı hücrelerden çok sayıda selim tümör oluşabilmektedir. Trikilemmal kist, epidermal kist ve lipom, saçlı deride subkutan nodüle neden olan tümoral lezyonlardandır. Bu çalışmada, 52 hastada, saçlı deri yerleşimli ve total eksize edilen 62 subkutan nodül retrospektif olarak incelenmiştir.Materyal-metod: Bu retrospektif çalışmada, 2009-2011 yılları arasında, Sağlık Bakanlığı Sakarya Üniversitesi Eğitim ve Araştırma Hastanesi Dermatoloji ve/veya Beyin Cerrahisi polikliniğine saçlı deride kitle şikayetiyle başvuran 52 hastada, beyin cerrahisi tarafından total olarak eksize edilen toplam 62 lezyon incelenmiştir.Bulgular: Hastaların 27'si kadın (% 51,9), 25'i erkek (% 48,1) idi. Yaş ortalaması 47,5 (21-82 yaş) olarak belirlendi. Lezyonların 34'ü (% 54,8) trikilemmal kist, 16'sı (% 25,8) lipom, 8'i (% 12,4) epidermal kist, 2'si (% 3,2) pilomatriksoma ve 2'si (% 3,2) fibrolipom olarak saptandı. Sonuç: Çalışmamız sonucunda trikilemmal kistlerin saçlı deride yerleşen subkutan nodüller arasında en sık gözlemlenen patoloji olduğu saptandı. Lipom ve epidermal kistler de diğer sık görülen subkutan nödüller olarak tespit edildi.

The main purpose of this study is to develop a practical microscopic training model of a fresh cadaveric sheep cervical spine that simulates the human anterior cervical discectomy under operating microscope. According the study results, it has been claim that this model simulates well standard anterior cervical discectomy of the human spine and is a useful method for trainees.

Arka plan: İnsan beyin damar sisteminin anatomik ve fizyolojik farklılıkları, arteriyel ve venöz sistemlerin silikon enjeksiyonu ile görülebilir. Yeni bir yöntem olarak, taze inek kranium kadavrasının silikon injeksiyonu, mikroanatomik çalışmalar için insan beyninin silikon enjeksiyonuna bir alternatiftir. Amaç: Amacımız, silikon enjeksiyon tekniği kullanarak taze inek kranium kadavrasının renkli silikon enjeksiyonu için geliştirilmiş bir metot bildirmektir. Yöntem: İnsan beynine alternatif olarak mikroanatomik diseksiyonlarda kullanmak üzere beş adet taze inek kraniumu, injekte edildi. Hazırlık, majör damarların yıkanması ve renkli silikon injeksiyonunu içerir. Sırasıyla internal karotis arter ve internal juguler venlerin kateterle kanülasyonu, ana arter ve venlerin su ile yıkanması, kraniumun alkol veya formaldehit ile fikse edilmesi, kırmızı ve mavi silikon ile arter ve venlerin renkli injeksiyonu uygulandı. Bulgular: Silikon injeksiyonları, taze inek kranium kadavrasında, küçük beyin damarlarının ve mezenkimal yapıların içine renkli solüsyonların derin penetrasyonunu sağladı. İnjeksiyon yapılmış beş kraniumun dördünde başarılı olundu. Birinde ise suboptimal sonuçlara varıldı. Sonuç: Mikroanatomik çalışmalar için kraniumun damarlarının anatomik ve fizyolojik değerlendirilmesi temelinde, silikon injekte edilmiş inek kranium kadavrası, insan beyin kadavrasına bir alternatif olarak önerilmiştir., Background: Anatomical and physiological variations of the human brain's vascular system can be observed via silicone injection of the arterial and venous systems. As a novel method, the injection of a fresh cadaveric cow cranium with silicone is an alternative to using the human brain for microanatomical studies. Objective: To report on an improved method for the colored silicone injection of a fresh cadaveric cow cranium using a silicone injection technique. Methods: Five fresh cow crania were injected as an alternative to human brains for microanatomical dissection, in which the preparation consisted of the irrigation of the major vessels and the injection of colored silicone. Cannulation of the internal carotid arteries and the internal jugular veins with catheters was performed, and the vasculature was irrigated with water (major arteries and veins). The fixation of the specimen with either formaldehyde or alcohol, and the colored injection of the arteries and veins with red and blue silicone, respectively, was then performed. Results: The silicone injections resulted in the deeper penetration of the colored solutions into the small cerebral vessels and mesenchymal structures of the fresh cadaveric cow crania. Of the five injected specimens, four exhibited successful injections, while one had suboptimal results. Conclusion: Silicone injection of the cadaveric cow brain, based on the anatomical and physiological assessment of the vasculature of the specimen for microanatomical studies, is suggested as an alternative to using human brain specimens.

We present a case of delayed intracerebral hemorrhage that developed seven years after initial ventriculoperitoneal shunt surgery. A seven-year-old boy was admitted to emergency when he experienced sudden headache and vomiting. Computed tomography (CT) scanning showed an intracerebral hemorrhage around the ventricular catheter that cannot be explained by known predisposing factors such as head trauma, coexisting bleeding disorder, occult vascular malformation, and intratumoral hemorrhage. The presumed mechanism in this case is that the ventricular catheter caused contusion of cerebral tissue because the shunt tube at the neck had stretched during the growing up of the child.

Bu olgu sunumunda ventriküloperitoneal şant cerrahisinden 7 yıl sonra geç gelişen bir intraserebral hemoraji olgusunu sunuyoruz. 7 yaşında erkek hasta acil servise ani başlayan baş ağrısı ve kusma şikayetleri ile başvurdu. Bilgisayarlı beyin tomografi incelemesi,ventriküler katater etrafında intraserebral hemoraji varlığını gösteriyordu. Hastanın öyküsü,labaratuvar bulguları ve radyolojik incelemelerinde intraserebral hemorajiye neden olabilecek kafa travması, kanama diyatezi, vasküler malformasyon ve intratümoral hemoraji saptanmadı.
