Apart from diet and exercise, medication and surgery are other options. After disappointing side effects of various obesity drugs, new treatments showed promising results. This review discusses the following anti-obesity drugs: liraglutide, semaglutide, tirzepatide, orlistat, as well as the phentermine/topiramate and bupropion/naltrexone combinations. These drugs have been approved by the Food and Drug Administration (FDA) for weight reduction except for tirzepatide which is still under evaluation. Abstract

Background: Obesity is the main obstacle for metabolic control in patients with type 2 diabetes. Turkey has the highest prevalence of obesity and type 2 diabetes in Europe. The effect of obesity on the metabolic control, and the macro-and microvascular complications of patients are not apparent. Objectives: This nationwide survey aimed to investigate the prevalence of overweight and obesity among patients with type 2 diabetes and to search for the impact of obesity on the metabolic control of these patients. We also investigated the independent associates of obesity in patients with type 2 diabetes. Methods: We consecutively enrolled patients who were under follow-up for at least 1 year in 69 tertiary healthcare units in 37 cities. The demographic, anthropometric, and clinical data including medications were recorded. Patients were excluded if they were pregnant, younger than 18 years, had decompensated liver disease, psychiatric disorders interfering with cognition or compliance, had bariatric surgery, or were undergoing renal replacement therapy. Results: Only 10% of patients with type 2 diabetes (n = 4,648) had normal body mass indexes (BMI), while the others were affected by overweight (31%) or obesity (59%). Women had a significantly higher prevalence of obesity (53.4 vs. 40%) and severe obesity (16.6 vs. 3.3%). Significant associations were present between high BMI levels and lower education levels, intake of insulin, antihypertensives and statins, poor metabolic control, or the presence of microvascular complications. Age, gender, level of education, smoking, and physical inactivity were the independent associates of obesity in patients with type 2 diabetes. Conclusion: The TEMD Obesity Study shows that obesity is a major determinant of the poor metabolic control in patients with type 2 diabetes. These results underline the importance of prevention and management of obesity to improve health care in patients with type 2 diabetes. Also, the results point out the independent sociodemographic and clinical associates of obesity, which should be the prior targets to overcome, in the national fight with obesity. Abstract

Aims: Turkey has the highest prevalence of diabetes in Europe. It is therefore essential to know the overall cardiovascular risk and reveal the predictors of metabolic control in Turkish adults with diabetes mellitus. Methods: A nationwide, multicenter survey consecutively enrolled patients who were under follow up for at least a year. Optimal control was defined as HbA1c < 7%, home arterial blood pressure (ABP) < 135/85 mmHg, or LDL-C < 100 mg/dL. Achieving all parameters indicated triple metabolic control. Results: HbA1c levels of patients (n = 5211) were 8.6 ± 1.9% (71 ± 22 mmol/mol) and 7.7 ± 1.7% (61 ± 19 mmol/mol), in Type 1 and Type 2 diabetes, respectively. Glycemic control was achieved in 15.3% and 40.2%, and triple metabolic control was achieved in 5.5% and 10.1%, respectively. Only 1.5% of patients met all the criteria of being non-obese, non-smoker, exercising, and under triple metabolic control. Low education level was a significant predictor of poor glycemic control in both groups. Conclusions: Few patients with Type 2, and even fewer with Type 1 diabetes have optimal metabolic control in Turkey. TEMD study will provide evidence-based information to policy makers to focus more on the quality and sustainability of diabetes care in order to reduce the national burden of the disease. © 2018 Elsevier B.V.

Abstract

Under ongoing climate change and increasing anthropogenic activity, which continuously challenge ecosystem resilience, an in-depth understanding of ecological processes is urgently needed. Lakes, as providers of numerous ecosystem services, face multiple stressors that threaten their functioning. Harmful cyanobacterial blooms are a persistent problem resulting from nutrient pollution and climate-change induced stressors, like poor transparency, increased water temperature and enhanced stratification. Consistency in data collection and analysis methods is necessary to achieve fully comparable datasets and for statistical validity, avoiding issues linked to disparate data sources. The European Multi Lake Survey (EMLS) in summer 2015 was an initiative among scientists from 27 countries to collect and analyse lake physical, chemical and biological variables in a fully standardized manner. This database includes in-situ lake variables along with nutrient, pigment and cyanotoxin data of 369 lakes in Europe, which were centrally analysed in dedicated laboratories. Publishing the EMLS methods and dataset might inspire similar initiatives to study across large geographic areas that will contribute to better understanding lake responses in a changing environment.

20. ÖĞRENME STİLLERİ ANKETİNİN GEÇERLİK ve GÜVENİRLİK ÇALIŞMASI NEDİR?

Abstract

Bu çalışmanın amacı, Cohen, Oxford ve Chi (2001) tarafından geliştirilen ‘Öğrenme Stilleri Anketi'(ÖSA)'nin Türkçe formunun dil eşdeğerliğinin, geçerliğinin ve güvenirliğinin incelenmesidir. Anket,Yıldız Teknik Üniversitesi, İTÜ, BÜ, Maltepe Üniversitesi, Bahçeşehir Üniversitesi, İ. BilgiÜniversitesi ve Sabancı Üniversitesinin aralarında bulunduğu toplam yedi üniversitenin hazırlık sınıfıprogramına devam eden 768 öğrenciden oluşan çalışma grubuna uygulanmıştır. ÖSA'nın İngilizce veTürkçe formu arasındaki tutarlığını incelemek için yapılan Pearson korelasyon katsayıları sonucunda48., 86. ve 87. dışındaki tüm maddelerde 0.36 ile 0.83 arasında değişen ve 0.01 düzeyinde pozitif veanlamlı değerlere ulaşılmıştır. Anketin yapı geçerliğini saptamak için yapılan faktör analizi altı temelboyut altında, 12 alt ölçekli, 52 maddeli yapı ortaya koymuştur. Anketin bütününün iç tutarlıkgüvenirliği 0.88 alpha katsayısıdır. Bulgular, alt ölçek maddelerinin iç tutarlık güvenirliğinin 0.20-0.45 arasında değişen madde-toplam korelasyonu olduğunu göstermiştir. Alt ölçeklerin dış tutarlıkgüvenirliği için yapılan test-tekrar test tekniği bulguları ise 0.51- 0.79 arasında korelasyon değerialmıştır. Sonuçlar, anketin Türkçe formunun geçerliği ve güvenirliği bakımından tartışılmıştır.Anahtar Sözcükler: Öğrenme stilleri, ölçek geliştirme, geçerlik ve güvenirlik.

Eğitim Programlan ve Öğretim Yüksek Lisans Tezi Bu araştırmada İngilizce Hava Trafik Terminolojisi ve Frezyolojisi kursunda bellek stratejilerinin kullanılmasının, öğrencilerin terim hatırlama düzeylerine ve basanlarına etkisi incelenmiştir. Bu bağlamda, araştırmada bellek stratejilerinin kelime öğrenme ve hatırlamaya etkisi ve öğrencilerin hangi bellek stratejilerini sıklıkla kullandıkları araştırılmıştır. Bu amaçla İngilizce Hava Trafik Terminolojisi ve Frezyolojisi kursuna katılan 60 öğrenci üzerinde yürütülen araştırmada kontrol gruplu ön test son test desen kullanılmıştır. Araştırmanın verileri, seviye tespit - sınavı. ve kelime bilgisi sınavı ile toplanmıştır. Araştırmada öğrencilere bellek stratejileri eğitimi verilmiştir. Araştırmadan elde edilen verilerin bir bölümü, ön test ve son test ortalama, standart sapmaları ve kovaryans analizi hesaplanarak bulunmuştur. Elde edilen bulgular şu şekilde özetlenebilir: Yabancı dil öğreniminde, bellek stratejileri öğrencilerin terim hatırlama düzeyini etkilemekte fakat bu anlamlı düzeyde olmamaktadır. Öğrencilerin terim hatırlama ve öğreniminde bellek stratejilerini farklı oranlarda kullandıkları tespit edilmiştir. Öğrenciler kelime öğrenimi ve hatırlamada bazı bellek stratejilerini daha çok tercih etmekte ve kullanmaktadırlar. Öğrencilerin özellikle kelimeyi metin içerisinde ve yaşantılarla ilişkilendirerek öğrendiği ve hatırladığı ortaya çıkmıştır. Bununla beraber, öğrencilerin anahtar kelime yöntemini tercih etmediği saptanmıştır. Buna ek olarak, öğrencilerin kelime hatırlamada anlam haritalan stratejisini çok az kullandıkları sonucuna varılmıştır. In this study, the effects of memory strategies on immediate retention of English as a foreign language (EFL) vocabulary and on the students' success in natural classroom settings were examined. In addition, the author attempted to reveal which memory strategies and how often they are used by the students. 2 classes with 60 students in air traffic terminology and phraseology course were assigned 1 of 2 learning conditions: rote rehearsal and memory strategy condition. A placement test and a pretest in air traffic terminology were given to both groups. The memory strategy group received a brief training in memory strategies whereas rote rehearsal group studied the words by employing the methods they wanted. Both groups were assessed immediately after the course. Average, standard deviation and co variance were computed according to the data gathered from, the pretest and post test. Results demonstrated that, there was not â significant difference between memory 'strategy group and rote rehearsal group both in recall test and success. It was also found that students use context method and attachment with experience method more than the others. 25. Tropisetron plus subhypnotic propofol infusion is more effective than tropisetron alone for the prevention of vomiting in children after tonsillectomy.

Abstract

This study evaluated the efficacy of tropisetron compared with tropisetron and a subhypnotic propofol infusion in preventing postoperative vomiting following tonsillectomy. One hundred and forty healthy children, aged four to 12 years, undergoing tonsillectomy were recruited in a randomised, double-blind study. After induction with sevoflurane, anaesthesia was maintained with sevoflurane and nitrous oxide. All patients received tropisetron (0.2 mg/kg up to 5 mg; intravenously) and fentanyl (2 microg/kg; intravenously) and were intubated after atracurium which was reversed with neostigmine (and atropine). The tropisetron-plus-propofol group received a single dose of propofol (1 mg/kg) before intubation and a continuous infusion of propofol throughout surgery at 15 microg/kg/min. Data for postoperative vomiting were grouped into zero to four and four to 24 hour time intervals. A P value of < 0.05 was considered statistically significant. The percentage of patients exhibiting a complete response (no retching or vomiting for 24 hours) was 47.1% (33/70) in the tropisetron-alone group and 72.8% (51/70) in the tropisetron-plus-propofol group (P = 0.002). The 0.257 absolute risk reduction of vomiting with the addition of propofol represents a number needed to treat of 3.87, and a risk ratio of 0.51 (95% CI 0.32 to 0.79). Significantly fewer patients vomited in the tropisetron-plus-propofol group than in the tropisetron-alone group during the zero to four post-surgery interval (P = 0.016), but the difference was not statistically significant for the four to 24 hour postoperative period (P = 0.116). Intraoperative subhypnotic propofol infusion combined with tropisetron is more effective than tropisetron alone in reducing postoperative vomiting after tonsillectomy in children. 36. Genesis of Late Miocene-Pliocene lacustrine palygorskite and calcretes from Kirsehir, central Anatolia, Turkey.

Abstract

The palygorskite and associated calcretes within the lacustrine mudstones of the Kizilirmak Formation are authigenic and were formed from soil-derived percolating water, as evidenced by a near-surface setting (vadose zone). The calcite C-13 isotope values, however, reflect a high input of soil-derived CO2, indicating a pedogenic origin. The calcite O-18 values suggest that the calcretes formed under the influence of meteoric water. Dessication cracks and vadose pisoids in the calcretes and bridge-like formation of the palygorskite indicate the formation of both minerals in a vadose environment. The calcretes formed through the percolation of soil water and precipitation under alternating wet and dry, near-surface climatic conditions. The development of palygorskite fibres in association with resorbed calcite and at the edges of disordered smectite flakes suggests that the palygorskite formed diagenetically during or post-dating calcification. The formation of palygorskite may have occurred via the conversion of smectite or by direct precipitation due to the increased Al+Fe and decreased Ca/Mg ratios in meteoric-water under alkaline conditions at an advanced stage of evaporation during calcification. Abstract

The presence of residual cardiovascular disease (CVD) risk is a current dilemma in clinical practice; indeed, despite optimal management and treatment, a considerable proportion of patients still undergo major CV events. Novel lipoprotein biomarkers are suggested as possible targets for improving the outcomes of patients at higher risk for CVD, and their impact on major CV events and mortality have previously been investigated. Innovative antidiabetic therapies have recently shown a significant reduction in atherogenic lipoproteins, beyond their effects on glucose parameters; it has also been suggested that such anti-atherogenic effect may represent a valuable mechanistic explanation for the cardiovascular benefit of, at least, some of the novel antidiabetic agents, such as glucagon-like peptide-1 receptor agonists. This emphasizes the need for further research in the field in order to clearly assess the effects of innovative treatments on different novel biomarkers, including atherogenic lipoproteins, such as small dense low-density lipoprotein (LDL), lipoprotein(a) (Lp(a)) and dysfunctional high-density lipoprotein (HDL). The current article discusses the clinical importance of novel lipid biomarkers for better management of patients in order to overcome residual cardiovascular risk.

Abstract

Insight into how environmental change determines the production and distribution of cyanobacterial toxins is necessary for risk assessment. Management guidelines currently focus on hepatotoxins (microcystins). Increasing attention is given to other classes, such as neurotoxins (e.g., anatoxin-a) and cytotoxins (e.g., cylindrospermopsin) due to their potency. Most studies examine the relationship between individual toxin variants and environmental factors, such as nutrients, temperature and light. In summer 2015, we collected samples across Europe to investigate the effect of nutrient and temperature gradients on the variability of toxin production at a continental scale. Direct and indirect effects of temperature were the main drivers of the spatial distribution in the toxins produced by the cyanobacterial community, the toxin concentrations and toxin quota. Generalized linear models showed that a Toxin Diversity Index (TDI) increased with latitude, while it decreased with water stability. Increases in TDI were explained through a significant increase in toxin variants such as MC-YR, anatoxin and cylindrospermopsin, accompanied by a decreasing presence of MC-LR. While global warming continues, the direct and indirect effects of increased lake temperatures will drive changes in the distribution of cyanobacterial toxins in Europe, potentially promoting selection of a few highly toxic species or strains. © 2018 by the authors. Abstract

Under ongoing climate change and increasing anthropogenic activity, which continuously challenge ecosystem resilience, an in-depth understanding of ecological processes is urgently needed. Lakes, as providers of numerous ecosystem services, face multiple stressors that threaten their functioning. Harmful cyanobacterial blooms are a persistent problem resulting from nutrient pollution and climate-change induced stressors, like poor transparency, increased water temperature and enhanced stratification. Consistency in data collection and analysis methods is necessary to achieve fully comparable datasets and for statistical validity, avoiding issues linked to disparate data sources. The European Multi Lake Survey (EMLS) in summer 2015 was an initiative among scientists from 27 countries to collect and analyse lake physical, chemical and biological variables in a fully standardized manner. This database includes in-situ lake variables along with nutrient, pigment and cyanotoxin data of 369 lakes in Europe, which were centrally analysed in dedicated laboratories. Publishing the EMLS methods and dataset might inspire similar initiatives to study across large geographic areas that will contribute to better understanding lake responses in a changing environment.

Abstract

Natural dyes extracted from fruit (FBV) and root (RBV) of barberry (Berberis vulgaris) and fruit (FPA) and stalk (SPA) of pokeweed (Phytolacca americana) were used as sensitizers for dye-sensitized solar cells (DSSCs). All the extracts were employed without any modifications, such as purifications or addition of stabilizers. Anthocyanin and isoquinoline alkaloid were identified as the main components for FBV and RBV extracts, respectively, whereas betalain for FPA and SPA extracts. SPA and FPA displayed much broader absorption spectra than the others, which is beneficial to light harvesting capability. When tested in DSSCs, the highest power conversion efficiency (PCE) of 3.04% is obtained by SPA, and this followed by PCEs of 2.97, 2.35 and 2.01% for FPA, RBV and FBV, respectively. The lowest PCE for FBV can be attributed to poor electron transfer ability, which may cause reduced photocurrent and photovoltage. On the other hand, after 120 h of sunlight irradiation, FBV, RBV, FPA and SPA retained the PCE values as 91, 54, 63 and 69% of the initial efficiencies. The moderate photovoltaic performance with good photostability for the anthocyanin extract may favorable for low-cost and environmental-friendly DSSC applications.

Abstract

Yirminci yüzyılın ikinci yarısından itibaren hızlı bir değişimin içinde olan dünyada liderlere ve geleceği etkili bir şekilde tasarlayacak yönetim anlayışlarına duyulan ihtiyaç ortaya çıkmıştır. Stratejik yönetim, bu yönetim anlayışlarının başında gelmektedir. Resmi ve özel anaokullarında liderlik ve stratejik yönetime ilişkin öğretmen görüşlerinin araştırılması bu çalışmanın temel amacını oluşturmaktadır. Bu amacın gerçekleştirilmesi için literatür taraması ve anket uygulaması yöntemlerinden bir arada faydalanılmıştır. Elde edilen sonuçlara göre özel ve resmi anaokullarında stratejik yönetim anlayışının yeterli düzeyde hakim olmadığını gösteren sonuçlara ulaşılmıştır. Liderlerin stratejik yönetime geçiş konusunda yeterli yetkinlikte olmaması ve öğretmenler dıĢ şındaki eğitim personelinin stratejik yönetime iliĢkin bilgisinin yetersiz kalması stratejik yönetime geçilmemesi ve geleneksel yönetim anlayışının devam etmesinin temel nedeni olarak belirlenmiştir. Araştırmanın İstanbul ili Anadolu yakasındaki anaokullarında görev yapan öğretmenlerin görüşlerini içermesi, ulaşılan sonuçların sınırlılıkları olarak görünmektedir., Since the second half of the 20th century, the request for the leadership and new management styles and its planners have been emerged and developed. The strategic management is one of these new generation management styles. The main goal of this study, is to determine the teacher‟s opinions about leadership and strategic management. To reach this goal literature review and survey application methodologies have been together. The results showed that leadership and strategic management intellections are not enough in formal and private pre-schools. The traditional learning methodologies are still being used. Because of limited information regarding strategic leadership and most of the time out of the teachers, no one knows about the leadership principles. This study only contains from the opinions of Ġstanbul Province‟s Anatolia preschool teachers and this can be accepted as the limitedness of this research.

Although dyslipidemia represents a key factor in this disease, its impact on serum levels of distinct lipoprotein subfractions is largely unknown.OBJECTIVE: To assess the full low-density lipoprotein (LDL) and high-density lipoprotein (I-EDL) profiles in patients with NAFLD.METHODS: Seven LDL and 10 HDL subfractions were assessed by gel electrophoresis (Lipoprint, Quantimetrix Corporation, USA) in men with biopsy proven NAFLD (simple steatosis [n = 17, age, 34 7 years] and nonalcoholic steatohepatitis [NASH; n = 24, age, 32 +/- 6 years]). Exclusion criteria included robust alcohol consumption, infection with hepatitis B or C virus, body mass index >= 40 kg/m(2), diabetes mellitus, and hypertension. RESULTS: Compared with simple steatosis, NASH patients had similar body mass index, homeostasis model assessment of insulin resistance index and plasma lipids, with increased levels of.both aspartate aminotransferase and alanine transaminase. NASH subjects had lower levels of larger LDL1 (10 4 vs 13 4%, P =.010) and increased smaller LDL3 and LDL4 particles (9 5 vs 5 5%, P =.017 and 3 3 vs 1 2%, P =.012, respectively). No changes were found in the HDL subclass profile. By multiple regression analysis, we found that NASH was associated only with increased levels of LDL3 (P =.0470).CONCLUSIONS: The increased levels of small, dense LDL3 and LDL4 in NASH may help to at least partly explain the increased risk for atherosclerosis and cardiovascular diseases in these patients. Abstract

WOS: 000342659400009, This paper addresses the mineralogy, geochemistry, and genesis of palygorskite and associated calcretes in the Kirsehir region. In this area, the Quaternary calcretes are present within and/or on lacustrine mudstones of the Kizilirmak Formation (Late Miocene-Pliocene). The calcretes occur in the form of nodules, tubes, fracture-infills, and hard laminated crusts (hardpans). Calcrete samples are predominantly composed of calcite associated with smectite, quartz, feldspar, and minor palygorskite. The lacustrine mudstone samples consist mainly of smectite and palygorskite with minor calcite, quartz, feldspar, illite, kaolinite and amorphous material. Petrographic studies revealed that the calcrete samples are predominantly micrites that were partially converted to microsparites by recrystallization. Scanning and transmission electron microscopy images indicate that palygorskite fibres formed authigenically on and between calcite rhombs in the calcrete samples and at the edges of smectite flakes of host rocks adjacent to the calcretes. The oxygen and hydrogen stable isotope values of the calcite suggest that it precipitated from meteoric water under arid or seasonally arid climatic conditions. Calcite precipitation occurred in the vadose zone, as evidenced by desiccation cracks and vadose pisolites. The enrichment of Rb+Ba +/- Sr and Light Rare Earth Elements (LREE) relative to Heavy Rare Earth Elements (HREE) with a distinct negative Eu anomaly are responses to the fractionation of volcanogenic feldspar and hornblende under the influence of meteoric water. The palygorskite associated with calcretes was precipitated from alkaline water rich in Si, Al and Fe and low in Mg. The palygorskite precipitation occurred at increased temperatures, characterising the advanced stage of calcrete formation which lowers the Ca content in the soil water. Abstract

Background and Objectives Dyslipidemia is a modifiable major risk factor for coronary heart disease. The objective of this study was to determine the prevalence of dyslipidemia among Turkish adults and its associations with other cardiovascular risk factors. Methods This study included 4309 people ages 20 to 83 years old from 7 provinces of Turkey. People from the city centers, districts, and villages were selected by a stratified sampling method. Weight, height, and waist and hip circumferences were measured. Blood samples were obtained to determine glucose, total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), and triglycerides (TG); these parameters were measured with an autoanalyzer. Dyslipidemia was defined according to National Cholesterol Education Program Adult Treatment Panel III diagnostic criteria. Results Of 4309 subjects, 43% had high TC, 41.5% had low HDL-C, 36.2% had high LDL-C, and 35.7% had high TG. Of these measures, at least 1 lipid abnormality was diagnosed in 78.7% of men and 80.4% of women. The prevalence of high TC, LDL-C, and TG increased with age, with the highest prevalence in the 46-to-65-year-old age group. The mean values (mg/dL) of TC, LDL-C, HDL-C, and TG were 194.2 ± 47.7, 117.7 ± 41.1, 50.3 ± 16.3, and 145.4 ± 96.3, respectively. Dyslipidemia was positively associated with age, body mass index, waist circumference, fasting blood glucose, and blood pressure, and negatively associated with altitude. Conclusions The high prevalence of dyslipidemia in Turkey is an important public health problem. Enhanced public health preventive measures should be implemented to better diagnose and comprehensively treat dyslipidemia in Turkey.

Abstract

Rheumatoid arthritis is a chronic inflammatory disease. Cardiovascular events are the most important cause of mortality and morbidity in patients with rheumatoid arthritis. Beyond the traditional cardiovascular risk factors, chronic systemic inflammation has been shown to be a crucial factor in atherosclerosis development and progression from endothelial dysfunction to plaque rupture and thrombosis. Many studies have shown that atherosclerosis is not a passive event like accumulation of lipids in the vessel walls; by contrast, it represents an active inflammation of the vessels. Inflammatory cells such as macrophages, monocytes and T cells play important roles in the development of both rheumatoid arthritis and atherosclerosis. Abstract

The present study investigates the effects of intrathecal ketamine on the concentration and composition of fatty acids in blood-brain barrier (BBB) tissues of New Zealand male rabbits. Ketamine is a drug that produces analgesia following intrathecal injection. It is very well known that fatty acids (FAs) play an important role in membrane fluidity of BBB tissues, which control the transportation of substances into brain. The total cellular fatty acids in brain and three spinal cord sections (cervical, thoracic, lumbar) of rabbits with or without drug administration were determined by gas chromatography using the Sherlock software (Microbial ID, Newark, DE) with the eukary database of fatty acid methyl ester (FAME) profiles for eukaryotic cells (Sherlock version 4.0). The fatty acid profiles of three different spinal cord sections were quite similar, but different than those of brain tissues. There were 23-25 fatty acids in spinal cord sections and 11 in the brain tissues of control animals compared to 23-27 fatty acids in spinal cord sections and 11 in the brain tissues of treated animals. The concentrations and compositions of the fatty acid methyl esters (FAMEs) in spinal cord and brain tissues were significantly changed by ketamine treatments. The overall changes in the concentrations and numbers of FAs indicate that the spinal drug tested in this study has neurotoxic effects on spinal cord and brain.

Abstract

Objectives: Human recombinant enzyme replacement therapy, given to compensate for genetic enzyme deficiency in lysosomal storage diseases, delays the progression of the disease and improves the quality of life. However, enzyme replacement therapy may cause hypersensitivity reactions. Within the scope of this research, we aimed to elucidate the frequency and clinical features of hypersensitivity reactions against enzyme replacement therapy in children with lysosomal storage diseases and clarify the management of these reactions., Methods: Medical records of pediatric patients with lysosomal storage disease and receiving enzyme replacement therapy were retrospectively reviewed, and patients who experienced allergic reactions were included in the study. The demographic characteristics of the patients, their diagnosis, the responsible enzyme, the time at which the reaction started and at what dose, the signs and symptoms associated with the reaction, diagnostic tests, the management of the reaction, and the protocol applied for the maintenance of enzyme replacement therapy after the reaction were recorded., Results: Hypersensitivity reactions developed in 18 of 71 patients (25.3 %) who received enzyme replacement therapy. The most common cutaneous findings were observed. Anaphylaxis developed in 6 of 18 patients. Patients who experienced recurrent hypersensitivity reactions with premedication or a slower infusion rate, those with positive skin test results, and patients who developed anaphylaxis were given enzyme replacement therapy with desensitization., Conclusions: HSR may develop during enzyme replacement therapy, which are vital in lysosomal storage diseases, and discontinuation of enzyme replacement therapy is a significant loss for patients with metabolic disorders. Abstract

Molecular diagnosis of inborn errors of immunity (IEI) plays a critical role in determining patients' long-term prognosis, treatment options, and genetic counseling. Over the past decade, the broader utilization of next-generation sequencing (NGS) techniques in both research and clinical settings has facilitated the evaluation of a significant proportion of patients for gene variants associated with IEI. In addition to its role in diagnosing known gene defects, the application of high-throughput techniques such as targeted, exome, and genome sequencing has led to the identification of novel disease-causing genes. However, the results obtained from these different methods can vary depending on disease phenotypes or patient characteristics. In this study, we conducted whole-exome sequencing (WES) in a sizable cohort of IEI patients, consisting of 303 individuals from 21 different clinical immunology centers in Türkiye. Our analysis resulted in likely genetic diagnoses for 41.1% of the patients (122 out of 297), revealing 52 novel variants and uncovering potential new IEI genes in six patients. The significance of understanding outcomes across various IEI cohorts cannot be overstated, and we believe that our findings will make a valuable contribution to the existing literature and foster collaborative research between clinicians and basic science researchers., (© 2024. Abstract

Context: Awareness of typical and atypical ultrasonographic (US) features of parathyroid adenomas (PAs) is crucial since US is the most widely used first-line imaging modality., Objective: The purpose of this study was to describe the atypical features of PAs on US and other possible factors leading to a false negative examination in a large single-center cohort., Materials and Methods: The US records of 457 PAs in 445 patients with biochemically proven primary hyperparathyroidism (PHPT) were evaluated in a prospectively maintained database. Atypical size, composition, shape, echogenicity, location, and vascular pattern on US were noted. For patients who previously had at least one negative US examination in referring centers, the main possible reason was defined accordingly., Results: The study group included 359 female and 86 male patients with PHPT. Typical sonographic features were observed in 231 PAs (51%), whereas 226 (49%) had at least one atypical US feature. The most common atypical features were atypical size (29%), followed by atypical echogenicity (19%), shape (8%), location (7%), and composition (7%), respectively. There were 122 initially missed PAs in all groups. The most frequent main atypical US features leading to false negative examinations were atypical size (22.1%) and atypical location (18.8%). Inexperience was third most common reason (16.3%) for false negative US examinations., Conclusions: Almost half of PAs have at least one atypical feature on US. Awareness of the high prevalence of atypical US features could increase the accuracy of US examination and potentially decrease demand for more expensive second-line imaging modalities., (© The Author(s) 2023.
