Se realiza el estudio de la dinámica del proceso de altura máxima de inundación por maremoto. Se considera el enfoque lagrangiano, en dos dimensiones, el cual permite identificar partículas de fluido y seguir su movimiento considerando efectos no lineales. Se elige el escenario del maremoto de Callao de 1746. El área de estudio es la región Costa Verde, donde se presentan las mayores alturas de inundación. Para las partículas en la superficie del agua se obtienen las soluciones analíticas del nivel de la superficie, las velocidades en la vertical y en la horizontal, en función de la posición horizontal y el tiempo. Con los resultados analíticos se realizan las gráficas para algunos instantes de tiempo determinados. La dinámica de los resultados es consistente con la dinámica de la inundación y de la velocidad en maremotos, mostrada en la literatura, mediante la simulación numérica y la medición a partir de datos de cámaras de video.

El 28 de octubre de 1746 a las 22:30 hora iocal ia ciudad de Lima y ei Callao fueron remecidas por un fuerte terremoto de magnutud 8.6 Mw e intensidad X en la escala de Mercalli, el epicentro se ubicó en el mar frente al Callao. El maremoto generado destruyó la ciudad del Callao y produjo la muerte de más de 5000 personas, quedando sólo 200 sobrevivientes. A partir de catálogos históricos se obtienen los parámetros focales del terremoto, luego se calcula los parámetros del área de ruptura. Con la ayuda del modelo WinITDB se calcula la máxima altura de la ola del maremoto. De la ecuación de onda para aguas someras y con los datos de batimetría se calculan los tiempos de arribo de la primera ola. Para el cálculo del rup-up o máxima altura de inundación, se utiliza un modelo dinámico y energético de una partícula. Este dato servirá para la elaboración del mapa de inundación, el cual será de utilidad para efectos de mitigación del desastre por parte de las autoridades correspondientes.

Introduction and Objectives: Alcohol-associated hepatitis (AH) is a severe entity associated with high mortality. Corticosteroids might be used in cases with severe disease and several dynamic models can predict mortality and response to corticosteroids in AH patients. However, there is no consensus on the best of them. This study aimed to evaluate dynamic models to predict response to corticosteroid treatment based on short-term mortality in patients with severe AH based on a worldwide cohort. Materials and Methods: A retrospective cohort study of patients with severe AH (between 2009 – 2019). We included patients who received corticosteroid treatment and calculated the Lille model of day 4 (Lille-4), day 7 (Lille-7) (cut-off value ≥0.45), and the Trajectory of Serum Bilirubin (TSB)(cut-off value ≥0.8 of the ratio between bilirubin at admission and day 7) to predict mortality. We estimated up to 30-day survival using Kaplan-Meier curves, and we performed multivariable analyzes using Cox regression. Specifically, we constructed two models to compare Lille-4 vs. TSB and Lille-7 vs. TSB, adjusting by well-known clinical variables associated with higher mortality in AH (age, sex, and creatinine at admission). Results: 1,066 patients were included (30 centers, 10 countries), age 47.7 ± 10.9 years, 30% women. The MELD score on admission was 25 [21-30]. The last decade has witnessed the development of pernicious polarization in Brazil, partly due to the emergence of a conservative, populist-nationalist, and neoliberal agenda. This article addressees the viewpoints of individuals who identify themselves as part of the right-wing, drawing on 21 semi-structured interviews with members of the Movimento Brasil Livre. By analyzing the interviews following the philosophy of Paulo Freire, we show how their narrative of oppression echoes Freire’s ideas of conscientization and liberation in form but not in substance. We also suggest that a Freirean approach opens new ways to discuss and potentially break the pernicious polarization apart, incorporating a significant distinction between sectarians and radicals.

Liver injury has been widely described in patients with Coronavirus disease 2019 (COVID-19). We aimed to study the effect of liver biochemistry alterations, previous liver disease, and the value of liver elastography on hard clinical outcomes in COVID-19 patients. We conducted a single-center prospective observational study in 370 consecutive patients admitted for polymerase chain reaction (PCR)-confirmed COVID-19 pneumonia. Clinical and laboratory data were collected at baseline and liver parameters and clinical events recorded during follow-up. Transient elastography [with Controlled Attenuation Parameter (CAP) measurements] was performed at admission in 98 patients. All patients were followed up until day 28 or death. The two main outcomes of the study were 28-day mortality and the occurrence of the composite endpoint intensive care unit (ICU) admission and/or death. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels were elevated at admission in 130 patients (35%) and 167 (45%) patients, respectively. Overall, 14.6% of patients presented the composite endpoint ICU and/or death. Neither ALT elevations, prior liver disease, liver stiffness nor liver steatosis (assessed with CAP) had any effect on outcomes. However, patients with abnormal baseline AST had a higher occurrence of the composite ICU/death (21% versus 9.5%, p = 0.002). En este articulo se estudia la dinámica y simulación numérica del maremoto de Lambayeque, Perú ocurrido el 20 de noviembre de 1960 (Mw = 7.6) en sus tres procesos: generación, propagación e inundación. El modelo numérico TUNAMI fue utilizado para simular los procesos de propagación e inundación. Se requiere como datos de entrada la información de batimetría y topografía obtenidas del modelo GEBCO con resolución de 30 segundos de arco (aproximadamente 927 m). Los resultados del modelo son los parámetros como: tiempos de arribo de la primera ola del maremoto, máximas alturas de olas y las áreas de inundación para cada zona a modelar, estos resultados han sido corroborados con información proporcionada por la población que soportó los efectos del sismo y posterior maremoto y que se detalla en el presente artículo. Los resultados de los tiempos de arribo para las zonas de Pimentel, Santa Rosa y Puerto Eten fueron de 30, 35 y 40 minutos respectivamente. La máxima inudación en Pimentel fue de 1.86 km de distancia y la altura de la ola llegó aproximadamente a los 6 m.

En este trabajo se evaluó el realismo de las simulaciones de vientos superfciales realizados con el modelo numérico Weather Research and Forecasting (WRF) con datos obtenidos del satélite QuikSCAT (Quick Scatterometer). Se utilizan como parámetros de comparación las velocidades del viento, así como los esfuerzos producidos sobre la superficie del agua. El conocimiento de la configuración del viento sobre la superficie oceánica es importante ya que genera procesos como el afloramiento, la espiral y el bombeo de Ekman (Ekman pumping). La intensidad con las que se llevan a cabo estos procesos, determina la diversidad y riqueza biológica y pesquera de una región. Se analiza el comportamiento del viento en cuanto a sus velocidades horizontales, el esfuerzo producido por estas velocidades, que determinan el arrastre sobre una superficie libre y el rotacional del viento. Se obtienen correlaciones superiores a 0.6 en las áreas oceánicas, alejadas de la costa, mientras que para regiones costeras las correlaciones son menores a 0.4.

A 70-year-old woman consulted us for dyspnea. Echocardiography revealed moderate aortic regurgitation secondary to ascending aorta dilatation. Study was completed with aortic angiography computed tomography, showing stenosis and dissection of several visceral arteries. There were no abnormal inflammatory or autoimmune markers, nor fluorine-18-fluorodeoxyglucose positron emission tomography uptake. Segmental arterial mediolysis was diagnosed.

Cardiovascular disease constitutes the leading cause of morbimortality worldwide. Non-ST-segment elevation acute coronary syndrome (NSTE-ACS) is a common cardiovascular condition, closely related to the ageing population and significantly affecting survival and quality of life. The management of NSTE-ACS requires specific diagnosis and therapeutic strategies, thus highlighting the importance of a personalized approach, including tailored antithrombotic therapies and regimens, combined with timely invasive management. Moreover, specific and frequent populations in clinical practice, such as the elderly and those with chronic kidney disease, pose unique challenges in the management of NSTE-ACS due to their increased risk of ischemic and hemorrhagic complications. In this scenario, comprehensive management strategies and multidisciplinary care are of great importance. Cardiac rehabilitation and optimal management of cardiovascular risk factors are essential elements of secondary prevention since they significantly improve prognosis. This review highlights the need for a personalized approach in the management of NSTE-ACS, especially in vulnerable populations, and emphasizes the importance of precise antithrombotic management together with tailored revascularization strategies, as well as the role of cardiac rehabilitation in NSTE-ACS patients.

Molecular cobaloxime-based heterogeneous systems have attracted great interest during the last decades in light-driven hydrogen production. Here, we present a novel cobaloxime-tethered periodic mesoporous organosilica (PMO) hybrid (Im-EtPMO-Co) prepared through the immobilization of a molecular cobaloxime complex on the imidazole groups present in ethylene-bridged PMO. The successful assembly of a molecular cobaloxime catalyst via cobalt-imidazole axial ligation has been evidenced by several techniques, such as 13C NMR, Raman spectroscopy, ICP-MS, and XPS. The catalytic performance of Im-EtPMO-Co catalyst was essayed on the hydrogen evolution reaction (HER) under visible light in presence of a photosensitizer (Eosin Y) and an electron donor (TEOA). It showed an excellent hydrogen production of 95 mmol hydrogen at 2.5 h, which corresponded to a TON of 138. These results reflect an improved photocatalytic activity with respect to its homogenous counterpart [Co(dmgH)2(Im)Cl] as well as a previous cobaloxime-PMO system with pyridine axial ligation to the cobaloxime complex.

Aim: To determine the prevalence of lesions in the oral mucosa in a Peruvian population. Materials and Methods: The sample consisted of 139 patients treated at the Moche Stomatology Clinic - Faculty of Stomatology of the National University of Trujillo, during the year 2019. A total of 139 excisional biopsies were performed and the diagnosis of the diseases or injuries was determined by histopathological studies. Results: The prevalence of benign lesions comprised 99.28% of all diagnoses, while only 0.72% were malignant lesions. Conclusions: Fibrous hyperplasia is the most prevalent lesion in the buccal mucosa and its most frequent location was the labial mucosa, followed by the dorsum of the tongue and the buccal mucosa.

Background & Aims: Model for End-Stage Liver Disease (MELD) score better predicts mortality in alcohol-associated hepatitis (AH) but could underestimate severity in women and malnourished patients. Using a global cohort, we assessed the ability of the MELD 3.0 score to predict short-term mortality in AH. Methods: This was a retrospective cohort study of patients admitted to hospital with AH from 2009 to 2019. The main outcome was all-cause 30-day mortality. We compared the AUC using DeLong's method and also performed a time-dependent AUC with competing risks analysis. Results: A total of 2,124 patients were included from 28 centres from 10 countries on three continents (median age 47.2 ± 11.2 years, 29.9% women, 71.3% with underlying cirrhosis). The median MELD 3.0 score at admission was 25 (20–33), with an estimated survival of 73.7% at 30 days. The MELD 3.0 score had a better performance in predicting 30-day mortality (AUC:0.761, 95%CI:0.732–0.791) compared with MELD sodium (MELD-Na; AUC: 0.744, 95% CI: 0.713–0.775; p = 0.042) and Maddrey’s discriminant function (mDF) (AUC: 0.724, 95% CI: 0.691–0.757; p = 0.013). However, MELD 3.0 did not perform better than traditional MELD (AUC: 0.753, 95% CI: 0.723–0.783; p = 0.300) and Age-Bilirubin-International Normalised Ratio-Creatinine (ABIC) (AUC:0.757, 95% CI: 0.727–0.788; p = 0.765). These results were consistent in competing-risk analysis, where MELD 3.0 (AUC: 0.757, 95% CI: 0.724–0.790) predicted better 30-day mortality compared with MELD-Na (AUC: 0.739, 95% CI: 0.708–0.770; p = 0.028) and mDF (AUC:0.717, 95% CI: 0.687–0.748; p = 0.042). The MELD 3.0 score was significantly better in predicting renal replacement therapy requirements during admission compared with the other scores (AUC: 0.844, 95% CI: 0.805–0.883). Conclusions: MELD 3.0 demonstrated better performance compared with MELD-Na and mDF in predicting 30-day and 90-day mortality, and was the best predictor of renal replacement therapy requirements during admission for AH. However, further prospective studies are needed to validate its extensive use in AH. Impact and implications: Severe AH has high short-term mortality. The establishment of treatments and liver transplantation depends on mortality prediction. We evaluated the performance of the new MELD 3.0 score to predict short-term mortality in AH in a large global cohort. MELD 3.0 performed better in predicting 30- and 90-day mortality compared with MELD-Na and mDF, but was similar to MELD and ABIC scores. MELD 3.0 was the best predictor of renal replacement therapy requirements. Thus, further prospective studies are needed to support the wide use of MELD 3.0 in AH.
