To investigate whether a combination of dry-needling treatments and upper extremity neuromuscular reeducation exercises can significantly improve neck pain and/or headache-related disability, joint position error, cervical range of motion, and pain pressure threshold in individuals suffering from cervicogenic headache (CGH). This study is a quasi-experimental design. Seven participants met the inclusion criteria and received dry needling to three muscles that reproduced the participant's headache symptoms and completed the Neck Disability Index (NDI), Headache Disability Inventory (HDI), Visual Analog Scale (VAS), and Numeric Pain Rating Scale (NPRS). Participants performed an exercise regimen designed to address strength and mobility of cervical and scapulothoracic musculature. There were significant improvements seen in cervical range of motion and neck pain-related disability (NDI) during the 4-week treatment period. Recognizing these challenges, this study aimed to investigate Cervical Health Parameters in Car Drivers.Methods: The sample consisted of 160 car drivers between 25 and 45 years. This subject was then divided into two groups based on neck pain. Participants met the required criteria, such as being between 25– 45 years of age, maintaining a BMI of 18– 24, and driving for at least 2 hours each day for at least 3– 5 years. To evaluate the results, we employed a clinometer and compass app on a smartphone to measure the Cervical Range of Motion (CROM). We used Surgimap software to estimate the Craniovertebral Angle (CVA), and a (Cervical range of motion) CROM device was used for proprioception assessment.Results: The result shows the participants in neck pain group displayed lower Cervical Range of Motion (CROM) values than without neck Pain Group. Similarly, the Craniovertebral Angle (CVA) was smaller in the neck Pain Group (mean difference of − 6.3°), indicating a more forward head posture. Neck pain resulted in a mean difference of − 4.5° in proprioception accuracy. This indicates that neck pain affects CROM, CVA, and proprioception in car drivers.Conclusion: Car driving significantly impacts cervical parameters in individuals with neck pain, reducing cervical range of motion, altered craniovertebral angle, and diminished proprioceptive accuracy. These findings emphasize the need for ergonomic interventions and proprioceptive training tailored for drivers. The purpose of the present cross-sectional study was to examine the relationship between heart rate variability (HRV) and the range of cervical motion, disability, pain intensity, pain catastrophizing, and quality of life in patients with chronic, non-specific neck pain. Thirty-five patients, aged 20–48 years, with chronic non-specific neck pain, completed validated questionnaires regarding neck pain intensity, pain-associated disability, catastrophic thoughts, and quality of life. The range of cervical motion was assessed using a digital goniometer. HRV indices were recorded in three positions (supine, sitting, and standing) through a smartphone application. Several significant correlations were observed between HRV indices and neck pain disability, the helplessness factor of catastrophizing, neck rotation, and quality of life. These correlations were only observed in the standing position. Pain catastrophizing was positively correlated with disability and pain intensity during active neck movement (Pearson r = 0.544, p < 0.01; Pearson r = 0.605, p < 0.01, respectively). Quality of life was negatively correlated with pain intensity during active movement (Pearson r = −0.347, p < 0.05). HRV indices were correlated with the psychological and physical domains of neck pain. These cardiac indices have been related to neck pain variables in some previous studies. Background and Objectives: The aim of this study were to compare the cervical ROM data obtained from accelerometer exams between patients suffering from TMDs (study group) and healthy patients (control group). Material and Methods: A sample of 43 young adult subjects (23 control subjects and 20 TMD patients) were included in this study and analyzed with the accelerometer (Baiobit™, BTS, Garbagnate Milanese, Milan, Italy) to assess cervical ROM using a standardized protocol, including the following occlusal conditions: mandibular rest position, clenching, clenching with cotton rolls, maximal intercuspation, and mandibular position with cotton rolls. The cervical ROM was measured in degrees and expressed as the mean and standard deviation. Results: TMD patients showed a reduced cervical extension compared to control subjects in all the conditions, with statistically significant relevance. Regarding the other movements, the differences were not statistically or clinically significant. Conclusions: Based on the results of the present study, it can be observed that temporomandibular disorders are associated with a decreased cervical extension range, while the remaining components of ROM do not seem to be significantly associated. The incidence of cervical spondylotic radiculopathy is relatively high among cervical spine diseases. Surgical options for its treatment include anterior cervical discectomy and fusion, disc arthroplasty, and posterior cervical foraminotomy. Posterior cervical foraminotomy, which can avoid problems related to fusion and surgical instruments, as well as the complications associated with anterior approaches, has become a popular alternative to anterior cervical surgery and is now considered a simple and effective surgical method for treating cervical spondylotic radiculopathy. With the continuous development of minimally invasive concepts and the innovation of various auxiliary techniques in spinal surgery, the technique is being continuously improved. It has broad prospects for application in the treatment of cervical spondylotic radiculopathy.

This study aims to assess the connection between cervical ROM, neck proprioception, CVA, and QOL in bike drivers with neck pain compared to those without neck pain so that the targeted interventions can be developed to enhance their well-being.Methods: A cross-sectional study involving 100 bike drivers aged 20– 50 years was conducted, split into two groups: those with neck pain (n=50) and those without neck pain (n=50). Cervical ROM was measured using a smartphone, neck proprioception was assessed through a head repositioning test, and CVA was determined using lateral-view photographs with a plumb line. The Short Form-36 (SF-36) questionnaire was employed to evaluate QOL. Data analysis was conducted using independent t-tests and Pearson’s correlation coefficient.Results: Bike drivers with neck pain exhibited significantly reduced cervical ROM (p-value=< 0.001), impaired neck proprioception (p-value=< 0.001), and decreased CVA (p-value=< 0.001) compared to their counterparts without neck pain. A strong negative correlation was found between neck pain and QOL, with lower scores in all eight domains of the SF-36. Cervical ROM, neck proprioception, and CVA showed moderate correlations with various QOL domains (p-value=< 0.05).Conclusion: Neck pain in bike drivers is linked to decrease cervical ROM, compromised neck proprioception, and reduced CVA. These factors correlate with a lower quality of life, both physical and mental domains. Purpose: Return to work (RTW) is important for survivors of head and neck cancer (HNC). The purposes of the study were to investigate the RTW ratio among HNC survivors and identify factors significantly affecting RTW in this population. Methods: A cross-sectional study with consecutive sampling was conducted in a medical center in Taiwan, with 111 patients with HNC who had completed major treatments within 5 years and were employed before their cancer diagnosis enrolled as participants. Cervical range of motion (CROM) functionality, handgrip and hip flexor strength, maximal mouth opening (MMO), selected symptoms, depression, and disease/treatment-related factors were assessed. All of the factors were analyzed using t-test, chi-square test, and multiple logistic regression. Results: Less than half (44.1%, n = 49) of the participants had returned to work. The t-test/chi-square test results showed the RTW group to be younger in age and better educated; have better handgrip/hip flexor strength, MMO, and CROM; have less speech difficulty and pain; and have less-advanced cancer than the non-RTW group. Further analysis of the above significant variables by logistic regression revealed early cancer stage, dominant handgrip strength, and less speech difficulty were the robust factors related to RTW. Conclusions: The RTW ratio is low in HNC survivors. RTW in HNC survivors is a multifactorial and complicated issue and needs to be further examined. Implications for Cancer Survivors: Assessing the factors related to RTW systematically and developing comprehensive interventions and rehabilitation programs to reduce related dysfunctions are necessary to enhance RTW ability in HNC survivors. Introduction/Aims: Some patients with Hirayama disease (HD) may have generalized joint hypermobility (GJH), which may excessively increase cervical range of motion (ROM) and then worsen the HD. The purpose of this study was to identify the frequency of GJH in HD patients and to analyze the effect of GJH on cervical ROM and the severity of HD. Methods: The Beighton scoring system (≥4) was used to diagnose GJH in 84 HD patients. All patients underwent assessments of cervical‐flexion/extension ROM; motor unit number estimation in bilateral abductor pollicis brevis (APB) muscles; handgrip strength; and the disabilities of the arm, shoulder, and hand assessments. Results: Concomitant GJH was identified in 20 (23.8%) HD patients. The HD patients with GJH exhibited greater cervical‐flexion (P <.001) and cervical‐extension (P =.033) ROM than those without GJH. Both greater single motor unit potential amplitudes (symptomatic side: P =.005; less‐symptomatic side: P =.011) and lower motor unit numbers (symptomatic side: P =.008; less‐symptomatic side: P =.013) in bilateral APB, along with lower compound muscle action potential amplitudes on the symptomatic‐side APB (P =.039), were observed in patients with GJH than those without GJH. There was a mild negative correlation between motor unit number and cervical‐flexion ROM in HD patients (symptomatic side: r = −0.239, P =.028; less‐symptomatic side: r = −0.242, P =.027). Discussion: The frequency of GJH in HD patients may be higher than in the general population. Importantly, GJH may exacerbate excessive cervical‐flexion ROM, thereby worsening motor unit loss in HD patients. A cautious approach should be taken when treating HD due to possible comorbid GJH. IntroductionMechanical neck pain has become prevalent among computer professionals possibly because of prolonged computer use. This study aimed to investigate the relationship between neck pain intensity, anthropometric metrics, cervical range of motion, and related disabilities using advanced machine learning techniques.MethodThis study involved 75 computer professionals, comprising 27 men and 48 women, aged between 25 and 44 years, all of whom reported neck pain following extended computer sessions. The study utilized various tools, including the visual analog scale (VAS) for pain measurement, anthropometric tools for body metrics, a Universal Goniometer for cervical ROM, and the Neck Disability Index (NDI). For data analysis, the study employed SPSS (v16.0) for basic statistics and a suite of machine-learning algorithms to discern feature importance. The capability of the kNN algorithm is evaluated using its confusion matrix.ResultsThe “NDI Score (%)” consistently emerged as the most significant feature across various algorithms, while metrics like age and computer usage hours varied in their rankings. Anthropometric results, such as BMI and body circumference, did not maintain consistent ranks across algorithms. The confusion matrix notably demonstrated its classification process for different VAS scores (mild, moderate, and severe). The findings indicated that 56% of the pain intensity, as measured by the VAS, could be accurately predicted by the dataset.DiscussionMachine learning clarifies the system dynamics of neck pain among computer professionals and highlights the need for different algorithms to gain a comprehensive understanding. Such insights pave the way for creating tailored ergonomic solutions and health campaigns for this population.

Study Design.: A prospective multicenter study. Objective.: The objective of this study is to compare the surgical outcomes of anterior and posterior fusion surgeries in patients with K-line (−) cervical ossification of the posterior longitudinal ligament (OPLL). Summary of Background Data.: Although laminoplasty is effective for patients with K-line (+) OPLL, fusion surgery is recommended for those with K-line (−) OPLL. However, whether the anterior or posterior approach is preferable for this pathology has not been effectively determined. Materials and Methods.: A total of 478 patients with myelopathy due to cervical OPLL from 28 institutions were prospectively registered from 2014 to 2017 and followed up for two years. Of the 478 patients, 45 and 46 with K-line (−) underwent anterior and posterior fusion surgeries, respectively. After adjusting for confounders in baseline characteristics using a propensity score-matched analysis, 54 patients in both the anterior and posterior groups (27 patients each) were evaluated. Clinical outcomes were assessed using the cervical Japanese Orthopaedic Association and the Japanese Orthopaedic Association Cervical Myelopathy Evaluation Questionnaire. Results.: Both approaches showed comparable neurological and functional recovery. The cervical range of motion was significantly restricted in the posterior group because of the large number of fused vertebrae compared with the anterior group. The incidence of surgical complications was comparable between the cohorts, but the posterior group demonstrated a higher frequency of segmental motor paralysis, whereas the anterior group more frequently reported postoperative dysphagia. Conclusions.: Clinical improvement was comparable between anterior and posterior fusion surgeries for patients with K-line (−) OPLL. Introduction: Prakriti of an individual is formed by the dominance of Dosha at the time of fertilization and it indicates the phenotypic and genotypic constitution. Most of the characters described for the assessment of Prakriti are subjective and the objective characters described are also not well measurably quantified, leading to gross chances of variability in the assessment of Prakriti. To minimize this variability in assessment, measurable variables need to be developed for the assessment of Dosha and Prakriti of individuals. Estimating the range of motions (ROMs) of the joints can be a good measurable variable for the assessment of the dominant Dosha. In the present study, cervical ROMs (CROMs) have been estimated in individuals of different Prakriti. Methods: The study's design was a cross-sectional study. A total of 150 apparently healthy individuals of either sex of age 21–40 years were selected through simple random sampling from the population residing in Delhi. Prakriti of the study participants was assessed through the questionnaire, and the different CROMs were measured by the goniometer. The association of CROM in different groups of Prakriti was statistically analyzed by one-way analysis of variance. Result: Individuals of Pitta Kaphaj and Pitta Vataj Prakriti have a significantly higher value of the ROM in cervical flexion (P < 0.001) and the individuals of Pitta Kaphaj Prakriti have also a significantly higher value of the ROM in cervical right lateral flexion (P = 0.016) and cervical left lateral flexion (P = 0.045). Conclusion: There are differences observed in the goniometric measurement of CROM among different Prakriti. Individuals of Pitta-dominant Prakriti have more CROMs in comparison to Vata-dominant and Kapha-dominant Prakriti individuals.

The head-mounted display (HMD) for virtual reality (VR) systems is becoming more popular due to technological advancements, providing users with a more immersive experience. Since an HMD is a wearable device, head movement may be restricted, compared to situations in which an HMD is not worn, and perceived discomfort during head movement may increase. Thus, this study recruited 30 participants and investigated the effects on perceived comfort from wearing an HMD, considering the cervical range of motion (CROM) in four head movement directions (flexion, extension, lateral bending, and rotation). We propose angle boundaries for joint isocomfort in order to evaluate the appropriateness of working postures in an HMD-based VR environment. Although the maximum CROM for head movement in all directions was not significantly limited by wearing the HMD, the main effects of wearing HMD on perceived discomfort were observed from extension and lateral bending. Based on the perceived CROM comfort score, appropriate ranges for each head movement direction are proposed. Introduction: Text neck syndrome is an overuse condition or repetitive stress injury in which the cervical spine is flexed forward or down for extensive stretch while gazing at the smartphone or other electronic device. Individual with this Syndrome usually present with neck pain, restricted cervical range of motion, reduced muscle strength and endurance, altered cervical joint position error along with postural alterations like forward head posture and rounded shoulders as well. Methods: We performed a systematic review on the Text Neck Syndrome and discussed the evidence-based studies. For the purpose of the review, an electronic search for relevant articles using PUBMED, MEDLINE, Pedro, Research Gate, Google Scholar and CINHAL databases up to March 2022 was done wherein MeSH search terms and free words were used. In addition to the electronic search, articles were searched manually for relevant studies. Articles were selected based on authors expertise, self- knowledge and reflective practice. Results:The current study included 17 articles that fulfilled the inclusion criteria suggesting that Text Neck Syndrome is an upcoming concern and requires attention .The above reviewed studies provided evidence that physiotherapy as a mode of treatment is definitely effective in treating the Text Neck Syndrome. Conclusion: This review spoke about the text neck syndrome and its rising consequences and also about the treatment approaches available in treating and minimizing the symptoms of this syndrome. Further research is required over what more preventive strategies can be adopted in case of the Text neck syndrome.

The stretching intervention of the current study was applied from several principles and the latest updated method of slowed and synchronized patterns between self-stretching with mindful breathing (MB) and eyes closed period before and after the self-stretching was performed at least four times repeatedly within 150 s per each pose. To investigate effects of self-stretching in five poses with and without MB on pain and cervical range of motion (CROM). The study was a randomized clinical trial with a blinded assessor. The participants were 30 females per group, aged 30–59 years with myofascial pain syndrome (MPS). The participants were evaluated via the pressure pain threshold (PPT) at the upper trapezius muscles and the CROM. Both groups showed that the PPT at upper trapezius muscles significantly increased after performing the stretching (p < 0.001). The ROM in the MB group significantly increased in extension, left and right lateral flexion, and left rotation (p < 0.05). Self-stretching with mindful breathing was designed to allow for effects within the shortest time in patients with myofascial pain syndrome. Since this treatment is a non-pharmacological intervention and was considered as part of active self-care, we suggest that this therapy could be used as alternative therapy for patients with MPS. The cervical range of motion (CROM) is a tool used to assess the neck's state across three movement axes: flexo-extension, rotation, and lateral flexion. People with neck pain often have a reduced CROM, and they feel pain at the end-range and/or accompany neck movements with compensatory trunk movements. Virtual reality (VR) setups can track the movement of the head and other body parts in order to create the sensation of immersion in the virtual environment. Using this tracking position information, a CROM assessment can be performed using a VR setup that may be carried out autonomously from the user's home. The objectives of this study were to develop a VR experience that could be used to perform a CROM assessment, and to evaluate the intra-rater and inter-rater reliability of the CROM measures guided by this VR experience. To the best of our knowledge, a study of this type has not been carried out before. Materials & Methods. A total of 30 asymptomatic adults were assessed using a VR device (HTC Vive Pro EyeTM). Two raters provided support with the VR setup, and the participants were guided by the VR experience as they performed the movements. Each rater tested each subject twice, in random order. In addition to a head-mounted display (HMD), a tracker located on the subject's back was used to measure trunk compensatory movements. The CROM was estimated using only the HMD position and this measurement was corrected using the tracker data. The mean and standard deviation were calculated to characterize the CROM. To evaluate the reliability, the interclass correlation coefficients (ICC) were calculated for intra-rater and inter-rater analysis. The standard error of measurement and minimum detectable change were also calculated. The usability of the VR system was measured using the Spanish version of the System Usability Scale. Results. The mean CROM values in each axis of movement were compatible with those described in the literature. ICC values ranged between 0.86 and 0.96 in the intra-rater analysis and between 0.83 and 0.97 in the inter-rater analysis; these values were between good and excellent. When applying the correction of the trunk movements, both the intra-rater and inter-rater ICC values slightly worsened except in the case of the lateral flexion movement, where they slightly improved. The usability score of the CROM assessment/VR system was 86 points, which is an excellent usability score. Conclusion. The reliability of the measurements and the usability of the system indicate that a VR setup can be used to assess CROM. The reliability of the VR setup can be affected by slippage of the HMD or tracker. Both slippage errors are additive, i.e., only when the sum of these two errors is less than the compensatory movement do the measurements improve when considering the tracker data. To analyze the cervical range of motion (CROM) and clinical parameters in patients affected by myogenous temporomandibular disorders (TMD), cervicogenic dizziness (CGD), both TMD and CGD (TMD/CGD), and a group of healthy subjects (HS). CROM degrees, Dizziness Handicap Inventory (DHI), Tampa Scale for Kinesiophobia (TSK-17), Hospital Anxiety and Depression Scale (HADS), and Jaw Functional Limitation Scale 20 (JFLS-20) scores were compared between 46 TMD patients, 49 CGD subjects, 43 TMD/CGD patients, and 98 HS. TMD/CGD and CGD patients demonstrated significantly lower CROM degrees and higher DHI, TSK-17, and HADS values when compared to TMD patients. TMD/CGD and TMD patients demonstrated higher JFLS-20 values when compared to CGD and HS. Significant negative correlations were found in TMD/CGD and TMD patients between JFLS-20 and CROM in flexion and extension. Introduction:  Primary preventative medicine lacks a consensus on effective concussion prevention strategies for collegiate athletes. Cervical strength has been identified as a potential factor in concussion risk reduction. This study evaluates the impact of a commercially available, portable cervical muscle stretching and strengthening device, NeckX®, on cervical strength, range of motion (ROM), and concussion incidence in collegiate athletes participating in high-concussion-risk sports., Methods:  A single-arm prospective cohort study was conducted with 162 collegiate athletes from various sports. Participants underwent a 12-week neck exercise protocol using the NeckX® device. Clinical data, including neck strength and ROM, were collected at weeks 0, 6, and 12. Concussion incidence was self-reported by participants and cross-referenced with records from the athletic department. Data were analyzed for significant neck strength and ROM changes throughout the 12-week study. A two-way analysis of variance multiple comparisons with the Tukey-Kramer significant difference test was utilized, using the Holm-Sidak method, with an alpha of 0.05., Results:  All athletic teams experienced a significant increase in cervical strength during the 12-week intervention (α = 0.05, p < 0.05). Increases in cervical flexion and extension force were most consistent between teams. Cervical ROM increased significantly in male and female soccer players (α = 0.05, p < 0.05). The overall incidence of head and neck injuries, including concussions, was reduced to 6.60% during the study period, the lowest recorded value in the university's athletic department history., Conclusion:  The use of the NeckX® device for 12 weeks was effective in enhancing pericervical muscle strength and ROM while reducing concussion incidence in collegiate athletes participating in high-concussion-risk sports. Interestingly, the positive outcomes were consistent for both males and females, indicating the universal advantages of neck training among collegiate athletes. Objective: To investigate whether a combination of dry-needling treatments and upper extremity neuromuscular reeducation exercises can significantly improve neck pain and/or headache-related disability, joint position error, cervical range of motion, and pain pressure threshold in individuals suffering from cervicogenic headache (CGH)., Methods: This study is a quasi-experimental design. Seven participants met the inclusion criteria and received dry needling to three muscles that reproduced the participant's headache symptoms and completed the Neck Disability Index (NDI), Headache Disability Inventory (HDI), Visual Analog Scale (VAS), and Numeric Pain Rating Scale (NPRS). Participants performed an exercise regimen designed to address strength and mobility of cervical and scapulothoracic musculature., Results: There were significant improvements seen in cervical range of motion and neck pain-related disability (NDI) during the 4-week treatment period., Conclusion: Dry needling and neuromuscular re-education (NMR) exercises could be effective components of treatment for individuals suffering from CGH to reduce disability and pain.

Background: Cervical spine allow maximum and necessary movements for the functioning of head and sensory organs. Any issue with the flexibility and joint movement can be determined by assessing the cervical range of motion. Cervical range of motion (CROM) assessment is commonly used in clinical practice. The current study aims to find the effects of wearing a hijab on the cervical range of motion compared to the normal cervical range of motions. Methodology: A cross-sectional study was conducted among hijab-wearing female students from Bahria University Medical and Dental College, from November 2019 to January 2020. The study subjects were recruited using a CROM device. Hijab-wearing female students between 18–23 years of age who wear hijab for at least 3 months, with the duration of wearing hijab for at least or at most 6 hours/day were included in the study. Results: A total of 384 students were recruited. Mean Cervical flexion for wearing hijab ≤ 6 hours was 61.06 ± 17.19, and > 6 hours was 55.28 ± 16.09 (p 6 hours was 37.96 ± 10.94 (p=0.008). Mean Lateral flexion for wearing hijab ≤ 6 hours was 42.66 ± 10.32, and > 6 hours was 38.96 ± 11.01 (p=0.002). While mean right rotation for wearing hijab ≤ 6 hours was 59.50 ± 14.27, and 6 hours was 63.47 ± 14.13 (p=0.010). Moreover, the mean Left rotation for wearing hijab ≤ 6 hours was 64.66 ± 17.86, and >6 hours was 70.58 ± 14.34 (p=0.001). Conclusion: The routine wearing of the hijab affects cervical mobility. Moreover, it is concluded that wearing a hijab for > 6 hours greatly affects the range of the head for movement, by decreasing the cervical range of motion.

Introduction: Posterior C1–C2 pedicle screw fixation is a reliable technique used in treatment of type II odontoid fracture. However, the loss of cervical range of rotation motion (RORM) was inevitable. There were few studies focusing on the influence of short-term C1–C2 fixation with nonfusion technique to preserve cervical function in patients younger than 60 years. The purpose of this study was to compare cervical RORM which was measured by an improved goniometer, and the clinical outcomes between short-term and long-term C1–C2 fixation techniques in the treatment of Grauer type 2B and 2C odontoid fracture. Materials and methods: This study represents a retrospective analysis, including patients who underwent primary C1–C2 fixation surgery. These patients were divided into short-term and long-term groups based on whether underwent a fixation removal operation. The clinical results were collected and compared between the two groups. Independent T test and Chi-square analyses were used to identify significant differences between the two groups and dependent T test was used within each group. Statistical significance was set at p <.05. Results: There were no severe postoperative complications, and all 60 patients achieved spinal stabilization after primary surgery. The mean rotation angle in the short-term group at last follow-up time was 138.39 ± 21.06°, which was better than 83.59 ± 13.06° in the long-term group (p <.05). The same statistical difference was observed in flexion–extension angle, which was 71.11 ± 18.73° in short-term group and 53.34 ± 18.23° in long-term group. The mean NDI score in short-term group at last follow-up time was 1.23 ± 0.86 and better than 8.24 ± 3.17 in long-term group. However, the VAS score in short-term group was 1.82 ± 0.54 which was worse compared to 0.64 ± 0.29 in long-term group. Conclusions: The results demonstrated that primary C1–C2 fixation with nonfusion technique could support satisfactory clinical effects. In addition, the removal of instruments after bony fusion could improve the function of cervical movement significantly in patients under 60 years. [ABSTRACT FROM AUTHOR]

Purpose: To explore possible associations between cervical spine mobility, measured by cervical range of motion (CROM) and a possible earlier onset of recurrent benign paroxysmal positional vertigo (BPPV), as well as an increased failure rate of canalith repositioning procedures. Methods: Medical records of 749 patients (247, 253 and 249 patients with a CROM ≤ 45°, between 45.1° and 55° and > 55.1°, respectively) with a first-time diagnosis of non-traumatic BPPV were included in this retrospective study. Age, gender, canal involvement and CROM values were treated as prospective prognostic factors for time of BPPV recurrence onset (RO) and number of manoeuvres needed to achieve resolution (resolution rate, RR). A multiple regression analysis was performed. Results: A significant increase in the incidence of recurrent BPPV was found in patients with reduced CROM (139 [56.27%;], 102 [40.31%] and 87 [34.93%], respectively, belonging to ≤ 45°, 45.1°–55° and > 55.1° subgroups; X2 = 9.42, p = 0.008). A strong association between age, CROM and recurrent BPPV RO and RR was demonstrated, respectively (multiple correlation coefficients = 0.492678 and 0.593493, respectively, p value < 10−4). Canal involvement was in line with the previous experiences. Conclusion: The results from this retrospective analysis unveiled the previously unexplored relation between reduction in cervical spine mobility and BPPV recurrence and treatment failure. The data from this study do not indicate the mechanisms by which this comorbidity might directly cause recurrent BPPV. However, they may suggest CROM to be evaluated, in association with other known risk factors for increased susceptibility to BPPV recurrence. [ABSTRACT FROM AUTHOR]

Background/Objective: The intent of the current study was to compare the immediate effects of positional release therapy (PRT) and manual trigger point release (MTpR) on neck range of motion and pain in upper trapezitis. Materials and Methods: Sixty participants with upper trapezius myofascial trigger points (MTrPs)' participated in this study. Subjects were randomly classified into two groups (30 in each group): the subjects in Group A received PRT in a shortened position while those in Group B received MTpR in the neutral position for the upper trapezius muscle. They received four therapy sessions every day for four days. The pain intensity and range of motion were measured using the Numerical Pain Rating Scale (NPRS) and cervical range of motion (CROM), respectively, before treatment sessions and repeated immediately after the first and fourth treatment sessions in each group till the last day of their interventions. Results: Paired and unpaired t-Test was used for the data analysis. Pre- and postinterventional effects measured on each day and between groups, revealed that CROM and NPRS values were significantly improved in (MTpR) group (CROM, and NPRS P < 0.05). Conclusion: Both groups (PRT and MTpR) showed an increase in range of motion on CROM and a decrease in pain intensity on the NPRS during four sessions of therapy, but MTpR showed to be more effectual in these participants.

Introduction: The purpose of the present study is to compare the effect of the physiotherapy to onabolulinumtoxin-A, and their combination, in relation to cervical and headache parameters in patients with chronic migraine. Methods: This is an observational cohort study conducted by a headache center and a physiotherapy degree course on 30 patients with chronic migraine. The patients were distributed in three groups of treatments for three months: onabolulinumtoxin-A only, physiotherapy only, and onabolulinumtoxin-A plus physiotherapy. The patients were evaluated, before and after each treatment, using the following: the postural assessment software SAPO for the forward head posture; the CROM goniometer for the cervical range of motion; the Migraine Disability Assessment Score for headache parameters. Results: After 3 months of each treatment, the scores obtained for the headache-related disability and the frequency of migraine decreased significantly for all groups, but the pain intensity scores changed significantly only in the onabolulinumtoxin-A (p = 0.01) and in the onabolulinumtoxin-A plus physiotherapy groups (p = 0.007). On the other hand, the forward head posture was reduced significantly in the physiotherapy (p = 0.002) and in the onabolulinumtoxin-A plus physiotherapy groups (p = 0.003). The cervical range of motion increased significantly in certain directions in the physiotherapy group and in the onabolulinumtoxin-A plus physiotherapy groups. Conclusions: The physiotherapy improved the cervical parameters. The onabolulinumtoxin-A decreased pain intensity. As a consequence, it can be said that the combined treatment was more useful than a mono-therapy alone. From our results, it can be concluded that onabolulinumtoxin-A plus physiotherapy could be a good option in the management of chronic migraine. [ABSTRACT FROM AUTHOR]

Cervical spondylosis has gradually become a high-incidence disease in today’s society. The cervical range of motion (CROM) is widely used as the evaluation criterion of cervical status, whereas the existing methods of CROM measuring are not humanized enough. Consequently, the purpose of this study was to develop a novel photogrammetric method to assess three-dimensional CROM. Three smartphone cameras were controlled to simultaneously capture three-direction photographs of the subject wearing the special designed device with three mark lines. The obtained photographs were uploaded to a PC and the mark lines in each photograph were extracted by utilizing both the Radon transform and the Hough transform. By calculating and combining the tilt angles of three mark lines, the CROM of the subject was indirectly determined. The performance of our method was compared with the goniometer-based method: the inter-instrument reliability was excellent for all six cervical movements with intraclass correlation coefficients $>$ 0.99; the degree of agreement between the two methods was high with Pearson’s coefficients $>$ 0.98; and the Bland-Altman plots also revealed the validity of our method. Moreover, the concept of a cervical motion curve was put forward to describe the movement track of the neck in order to reflect the cervical health status. The proposed approach is feasible, automatic and convenient for the measurement of CROM and the generated cervical motion curve can intuitively exhibit the trajectory of the neck. This technique that can easily acquire the biomedical information of the cervical spine has tremendous potential in the diagnosis, healthcare and wellness management of the neck. • Multilevel fusion leads to an important restriction for all directions of movement. • Objective CROM-measurements correlate with the patient-reported general restriction. • Measurements for flexion-extension and total rotation correlate with the PCS of SF-8. For patients with multilevel degenerative cervical myelopathy (DCM), laminectomy and fusion is an established technique. A concomitant effect of multilevel fusion is a restriction of cervical spine mobility. This retrospective study on DCM-patients with at least 4 laminectomy and fusion levels, compares data between objective and subjective restriction of the postoperative cervical spine mobility. The patient-reported restriction of cervical spine mobility was acquired by a five-step score. Measurements of cervical range of motion were performed using the CROM device and were correlated with the subjective scores. Fusion was performed over 6 levels in most of the 36 patients. For the subjective cervical spine mobility, 52.8% reported none to medium, 38.9% severe and 8.3% complete restriction. Mean objective cervical range of motion was 45.0° for flexion-extension, 26.3° for total lateral flexion and 51.4° for total rotation and therefore evidently reduced compared to non-operated patient cohorts in literature. There was a significant medium, negative correlation between the objective measurements and the patient-reported general restriction of cervical spine mobility, and with the physical component summary of SF-8. The significant objective reduction of cervical range of motion after laminectomy and multilevel fusion correlates with the patient-reported assessment for general restriction. Positional plagiocephaly (PP) is a general term describing cranial distortion from pre- or postnatal forces on the infant head. Abnormal intrauterine forces, multiple births, primiparous mothers, obstetric interventions, prematurity, male sex, excessive time lying in the supine position, and mobility restrictions of the cervical spine have been considered as the main predisposing factors. The objective was to investigate the association between the severity of PP and the active cervical rotation and to analyze the influence of predisposing factors in babies with PP. An analytical cross-sectional study was performed on 74 babies with moderate PP. Clinical and demographic data, cranial vault asymmetry, and active cervical rotation range of motion (ROM) were measured. Associations were analyzed with generalized linear models. The mean age was 16.8 ± 5.0 weeks, and 56.8% were male. A restriction in the ROM of active cervical rotation, especially to the left side, was observed. Our models showed that cranial asymmetry was related with left active cervical rotation ROM (p = 0.034) and with being transported in a pushchair (p < 0.001). Conclusions: An increased severity of PP was related with being transported in a baby pushchair and with a reduced active cervical rotation ROM toward the most restricted side. Purpose: The purposes of the study were (1) to assess the physical activity (PA) status, muscle strength (MS), and flexibility of survivors of head and neck cancer (HNC) and compare these findings with normative data from national labor fitness measures; (2) to examine the differences among PA subgroups, as categorized using Godin's scores; and (3) to examine the association between stretching exercises and cervical range of motion (CROM). Methods: A cross-sectional study with consecutive sampling was used to recruit HNC survivors from a medical center in Northern Taiwan who had completed either radiation therapy (RT) or multimodality treatments including RT within the current 5 years. The level of PA, daily function, fatigue, quality of life (QOL), MS (handgrip and hip flexor), BMI, and flexibility (CROM and fingertip-to-floor tests) of the participants were assessed. Results: A total of 108 participants completed the assessments from 135 eligible patients (80% response rate). Although 60.2% reported engaging in PA, only 16.7% met WHO guidelines. Compared to subjects in the normative data, the survivors of HNC in this study had poorer handgrip strength, BMI, and CROM, but better forward flexion. The participants who were consistent with WHO PA guidelines reported less fatigue, better right hip flexor MS, and better QOL than those who did not engage in any PA. Conclusion: Lack of sufficient PA and generally poorer fitness were found in study subjects. Objective: To evaluate how self-report and posturographic measures could be affected in patients with cervicogenic dizziness undergoing sustained natural apophyseal glides. Design: Randomised controlled single-blind study. Setting: Tertiary rehabilitation centre. Subjects: Patients affected by cervicogenic dizziness, diagnosed by applying accepted criteria. Forty-one patients (19 male, 22 female: mean age 44.3 ± 14.8 years) receiving treatment, and 39 patients (18 male, 21 female: mean age 43.8 ± 13.9 years) receiving placebo were included in the study. Interventions: The treatment group underwent sustained natural apophyseal glides, while the placebo was constituted by a detuned laser. Both groups received their interventions six times over 4 weeks. Main measures: Outcomes were tested by means of self-report measures such as perceived dizziness, neck disability, anxiety and depression. Also, cervical range of motion and posturography testing with power spectra frequency were analysed. Results: When compared to placebo, treated patients demonstrated a significant decrease in perceived dizziness (post-treatment total Dizziness Handicap Inventory score 20.5 ± 5.3 as compared to 26.2 ± 6 baseline), neck disability and pain (Neck Disability Index and Neck Pain Index post-treatment scores 12.5 ± 4.3 and 45.6 ± 15.1, respectively, as compared to baseline scores of 15.1 ± 4.8 and 62.5 ± 14.3), as well as significant improvement in cervical range of motion and some posturographic parameters. Conclusion: Sustained natural apophyseal glides may represent a useful intervention in reaching short-term beneficial effects in patients with cervicogenic dizziness, with respect to self-perceived symptoms, proprioceptive integration and cervical range of motion improvement. Cervical range of motion (CROM) is one of the first things evaluated in cervical disorders. DyCare-Lynx is an inertial measurement unit device that was recently designed to measure CROM. Therefore, the objectives of the present study were to test the reliability and validity of the DyCare-Lynx device for active CROM. This study included 36 healthy individuals for the reliability study and 31 individuals for the validity study. Test–retest reliability was examined in three different days, by the same examiner with a 4 ± 1-day interval between them in all cervical movements in random order. For validity, the CROM was tested with the Zebris Motion Analysis system and DyCare-Lynx simultaneously. The interclass correlation coefficient (ICC) of the DyCare-Lynx ranged from 0.54 to 0.90. The standard error of measurement (SEM) ranged from 2.12°-7.65°. The smallest detectable change (SDD) ranged from 11.25% to 29.75%. The Pearson's r correlation of DyCare-Lynx with Zebris ranged from 0.655 to 0.957. DyCare-Lynx showed moderate to excellent reliability and moderate-to-high validity. Moreover, SEM was low with acceptable SDD values for all movements. Overall, it can be suggested that DyCare-Lynx is a reliable and valid tool to evaluate active CROM. [ABSTRACT FROM AUTHOR]

Study Design: Prospective, observational study.Objective: The aim of this study was to collect objective and especially subjective data on changes in cervical spine mobility after single-level anterior cervical decompression and fusion (ACDF) and to investigate the impact on quality of life and activities of daily living (ADLs).Summary Of Background Data: Although there are several studies dealing with the objective change in mobility after single-level ACDF, there are few data on how spondylodesis of a motion segment affects subjective restriction of cervical spine mobility.Methods: Patients undergoing first-time, single-level ACDF for a symptomatic spondylotic process were eligible. Data were collected before surgery, at 3-month, and 1-year follow-up. Patients were assessed via clinical scores (pain intensity, Short-Form 8 [SF-8], among others) and asked for impairment in ADLs due to restriction of cervical spine mobility. The subjective restriction was acquired by a five-step patient-reported score. The range of motion was measured by the CROM device.Results: Data of 97 patients could be evaluated. For pain scores and SF-8 there were significant improvements 3 months and 1 year after surgery (P < 0.001). The impairment for most ADLs improved 3 months after surgery and further after 1 year. The subjective restriction showed a significant improvement in general and for all single directions 1 year after surgery. In the objective measurements, a significantly higher total rotation could be found 1 year after surgery compared to preoperatively (101.6° ± 21.2 vs. 93.9° ± 23.4; P = 0.002). There were no significant differences in total flexion-extension and lateral flexion. Increasing age was a significant predictor for objective and subjective restriction.Conclusion: The concern of many patients of being severely restricted in their cervical spine mobility after single-level ACDF can be denied. Objectively, the rotation even showed a significant improvement. Regarding the subjective restriction, which is more important for the patients, we found a significant improvement in general and for all directions of movement after surgery.Level of Evidence: 3. Background/Objective: The intent of the current study was to compare the immediate effects of positional release therapy (PRT) and manual trigger point release (MTpR) on neck range of motion and pain in upper trapezitis. Materials and Methods: Sixty participants with upper trapezius myofascial trigger points (MTrPs)' participated in this study. Subjects were randomly classified into two groups (30 in each group): the subjects in Group A received PRT in a shortened position while those in Group B received MTpR in the neutral position for the upper trapezius muscle. They received four therapy sessions every day for four days. The pain intensity and range of motion were measured using the Numerical Pain Rating Scale (NPRS) and cervical range of motion (CROM), respectively, before treatment sessions and repeated immediately after the first and fourth treatment sessions in each group till the last day of their interventions. Results: Paired and unpaired t-Test was used for the data analysis. Pre- and postinterventional effects measured on each day and between groups, revealed that CROM and NPRS values were significantly improved in (MTpR) group (CROM, and NPRS P < 0.05). Conclusion: Both groups (PRT and MTpR) showed an increase in range of motion on CROM and a decrease in pain intensity on the NPRS during four sessions of therapy, but MTpR showed to be more effectual in these participants. Seventeen MS patients were enrolled in the study and randomly assigned to an experimental group (EG) in which the Taopatch® nanotechnological device was applied or to a control group (CG). All the participants carried out a cervical range of motion (1) assessment and the hand grip test at baseline (T0) and after TP (T1), also investigating the lactate levels to figure out if there could be a correlation with the possible changes in the investigated parameters. The results showed no significant differences in both groups for ROM. As regards the hand grip test, EG showed a statistically significant improvement on strength for both hands, dominant (p = 0.01) and non-dominant (p = 0.04), while the CG showed an improvement only for the non-dominant hand (p = 0.001). No correlation was found between baseline lactate level and cervical ROM change. We can definitely conclude that exercise and Taopatch® can help to improve and maintain hand strength in MS subjects and also can prevent sedentary lifestyle during the COVID-19 pandemic time. To evaluate posturography measurements, and their association with other clinical tests used for cervicogenic dizziness diagnosis, in a cohort of subjects suffering from cervicogenic dizziness, compared with healthy subjects. Ninety-three cervicogenic dizziness patients and 98 age- and gender-matched healthy subjects underwent video-Head impulse test, posturography testing, evaluation of cervical spine movements by means of cervical range of motion goniometer and self-report and performance measures, including Dizziness Handicap Inventory, Neck Disability Index, Neck Pain Intensity, Tampa Scale for Kinesiophobia and Hospital Anxiety and Depression Scale. Cervicogenic dizziness patients demonstrated significant increases in classical posturography parameters (i.e., surface and length) and in power spectra values within middle and high-frequency interval depicting balance control alterations especially due to proprioceptive integration changes. Furthermore, decreases in degrees of cervical range of motion and increases of self-report and performance measures – highlighting significant complaints of subjective feeling of dizziness – were found in these patients when compared with healthy subjects. Multiple correlations were found between posturography testing and cervical range of motion and Dizziness Handicap Inventory as well as between different self-report and performance measures in cervicogenic dizziness patients. The implementation of posturography – including power spectra analysis - coupled with appropriate exclusion of other disorders, may represent a useful tool in improving cervicogenic dizziness assessment in terms of cost, time consumption and correlation with other measurements. Uncertainties in diagnostic assessment of cervicogenic dizziness reflect doubts regarding the causes underpinning the symptoms of imbalance, unsteadiness, and disorientation. Posturography measurement is a low-cost, time-sparing technique and its implementations have proven to be useful in detecting significant alterations in body sway during perturbating situations. Implementation of power spectra analysis on posturography testing may represent a useful tool in improving cervicogenic dizziness assessment also in terms of rehabilitative treatment follow-up. Background and Objectives: The aim of this study was to assess the effects of physiotherapy with aerobic exercise together with temporomandibular joint range of motion exercises (supervised) and physiotherapy with aerobic exercise only (unsupervised), also to review the correlations between neck movements, pain, temporomandibular joint range of motion movements and quality of life in individuals with migraine. Methods: The flexion, extension and lateral flexion of the cervical spine were measured in degrees with a mechanical goniometer and pressure pain thresholds with algometer. Quality of life was assessed with the SF-36 questionnaire and temporomandibular joint range of motion with a centimeter. Results: The study showed statistically significant cervical flexion results in both groups (p < 0.05), masticatory muscle results and temporomandibular joint range of motion between the groups (p < 0.05). A correlation between left upper trapezius muscle pain and cervical lateral flexion was observed in the intervention group. Physical activity correlated with cervical extension, activity limitation due to physical ailments and general health. A correlation between temporomandibular joint and right-side masticatory muscles pain was found. A correlation between upper trapezius muscle pain and left- as well as right-side temporalis muscles were found in the control group. Strong correlations were found between pain and activity limitation due to physical ailments and emotional state. The temporomandibular joint range of motion strongly correlated with activity limitation due to physical ailments. Conclusions: Physiotherapy based on aerobic exercises together with temporomandibular joint exercises was more effective than physiotherapy based on aerobic exercise for decreasing pain, increasing pressure pain thresholds and cervical range of motion. Mineralisation of the longitudinal odontoid ligament has recently been identified in three horses undergoing computed tomographic (CT) examination, but published studies describing the clinical relevance of this finding are currently lacking. The objective of this retrospective, analytical, cross‐sectional study was to investigate the relationship of this image finding to primary presenting complaint, age, breed, use and sex of the patient, and determine any association to the clinical signs of head shaking, neck pain or restricted range of neck motion. Computed tomographic images of 96 horses undergoing examination of the head and cranial cervical spine, for a variety of clinical reasons, were assessed for the presence of mineralisation within the longitudinal odontoid ligament. Clinical records were reviewed; presenting problem, signalment, clinical signs and final diagnoses were recorded and potential associations of presenting primary problem, signalment and individual clinical signs with mineralisation in the longitudinal odontoid ligament investigated, using univariable and multivariable ordinal logistic regression analysis. Final multivariable analysis confirmed significant associations of increasing severity of mineralisation with increasing age (P =.002) and being female (P =.038). There was no association of mineralisation of the longitudinal odontoid ligament with the syndrome of idiopathic head shaking or other clinical signs investigated in this sample of horses. Background: This paper outlines a summary of examination technique to identify the range of movement of the cervical spine. Due to common difficulties in obtaining tools for cervical examination within the district, a standardised compilation of easy-to-replicate examination techniques are provided using different tools. Methods: Bedside instruments that can be used includes a measuring tape, compass, goniometer, inclinometer and cervical range of motion (CROM) instrument. Discussion: Cervical flexion-extension, lateral flexion and rotation will be assessed with bedside instruments. Introduction: Mechanical neck pain has become prevalent among computer professionals possibly because of prolonged computer use. This study aimed to investigate the relationship between neck pain intensity, anthropometric metrics, cervical range of motion, and related disabilities using advanced machine learning techniques., Method: This study involved 75 computer professionals, comprising 27 men and 48 women, aged between 25 and 44 years, all of whom reported neck pain following extended computer sessions. The study utilized various tools, including the visual analog scale (VAS) for pain measurement, anthropometric tools for body metrics, a Universal Goniometer for cervical ROM, and the Neck Disability Index (NDI). For data analysis, the study employed SPSS (v16.0) for basic statistics and a suite of machine-learning algorithms to discern feature importance. The capability of the kNN algorithm is evaluated using its confusion matrix., Results: The "NDI Score (%)" consistently emerged as the most significant feature across various algorithms, while metrics like age and computer usage hours varied in their rankings. Anthropometric results, such as BMI and body circumference, did not maintain consistent ranks across algorithms. The confusion matrix notably demonstrated its classification process for different VAS scores (mild, moderate, and severe). The findings indicated that 56% of the pain intensity, as measured by the VAS, could be accurately predicted by the dataset., Discussion: Machine learning clarifies the system dynamics of neck pain among computer professionals and highlights the need for different algorithms to gain a comprehensive understanding. Positional plagiocephaly (PP) is a general term describing cranial distortion from pre- or postnatal forces on the infant head. Abnormal intrauterine forces, multiple births, primiparous mothers, obstetric interventions, prematurity, male sex, excessive time lying in the supine position, and mobility restrictions of the cervical spine have been considered as the main predisposing factors. The objective was to investigate the association between the severity of PP and the active cervical rotation and to analyze the influence of predisposing factors in babies with PP. An analytical cross-sectional study was performed on 74 babies with moderate PP. Clinical and demographic data, cranial vault asymmetry, and active cervical rotation range of motion (ROM) were measured. Associations were analyzed with generalized linear models. The mean age was 16.8 ± 5.0 weeks, and 56.8% were male. A restriction in the ROM of active cervical rotation, especially to the left side, was observed. Our models showed that cranial asymmetry was related with left active cervical rotation ROM (p = 0.034) and with being transported in a pushchair (p < 0.001). Conclusions: An increased severity of PP was related with being transported in a baby pushchair and with a reduced active cervical rotation ROM toward the most restricted side.
