Cervical cancer is more common in Sub-Saharan Africa (SSA) compared to developed countries, with persistent genital high-risk HPV (HR-HPV) infection identified as the cause. However, other factors contributing to this gap remain unclear. This review explores the potential role of cervicovaginal microbiota (CVM) in genital HPV infection and cervical cancer development among women in SSA. Many women of African descent, including those from SSA, lack lactobacilli dominance in their CVM, which is considered a biomarker of cervicovaginal health. Published literature has associated Lactobacillus-dominated CVM with a lower risk of HPV infection and cervical cancer. However, findings on the relationship between specific bacterial abundance and cervical disease severity are inconsistent and inconclusive due to differences in study design, study population, sampling, and potential confounders. Thus, there is a need to form consensus to advance research on CVM and HPV-associated cervical disease. Despite the exact mechanisms by which CVM influence HR-HPV persistence and cervical carcinogenesis being unknown, the backbone of the mechanisms appears to be mediated in part by the following: cervicovaginal dysbiosis, elevated vaginal pH, high ratio of L-to-D-lactic acid, cohesive biofilm formation, chronic inflammation, and immune dysregulation. Consequently, these promote cellular proliferation, genetic instability, and evasion of immune surveillance. This review calls for larger, prospective studies to unravel causal links, identify protective features, and integrate CVM interventions into HPV and cervical cancer prevention strategies in SSA.

Objective. The objective of this study was to identify a unique serum profile of circulating miRNAs and inflammatory markers in patients with degenerative cervical myelopathy (DCM) compared with healthy controls (HC). Summary of Background Data. Currently, DCM is diagnosed with a combination of history, physical examination, and close correlation to advanced imaging. To date, no serum marker has been identified to be diagnostic of this condition. Methods. Whole venous blood was collected from patients with DCM as well as healthy age-matched and gender-matched controls. miRNA was extracted from venous blood, and a screening analysis was initially conducted to identify miRNA dysregulation in DCM patients. RT-qPCR was used to analyze the expression of 2 specific miRNAs based on screening analysis and literature review. Bioinformatics analysis was used to identify gene networks and potential targets of the miRNA. In addition, the serum inflammatory profile of DCM and HC groups was differentiated using a pro-inflammatory panel. Results. Thirty-six patients were enrolled in the DCM group (36.1% male, 61.5 ±9.5 y), while 35 patients were enrolled in the HC group (31.4% male, 57.5± 8.9 y). Of the 15 total miRNAs differentially expressed between DCM and HC groups, two were selected for further analysis: miR-223-3p (upregulated) and miR-451a (downregulated). Functional gene network analysis revealed the highest-ranking gene network was involved in neurological disease, while the most overexpressed miRNA in this network (miR-233-3p) was noted to have over 100 targets, including CDKN1B and the insulin receptor. Serum cytokine analysis showed significant upregulation of several pro-in- flammatory cytokines in the DCM cohort compared with the HC group. Conclusion. DCM patients demonstrated a set of unique circulating miRNAs in addition to a different serum inflammatory profile compared with HC. Introduction: Cervical disease control might be challenging in advanced thyroid cancer (DTC). Indications for cervical external beam radiation therapy (EBRT) are controversial. Purpose: To identify clinical and molecular factors associated with control of cervical disease with EBRT. Methods: Retrospective evaluation and molecular analysis of the primary tumor DTC patients who underwent cervical EBRT between 1995 and 2022 was performed. Results: Eighty adults, median age of 61 years, were included. T4 disease was present in 43.7%, lymph node involvement in 42.5%, and distant metastasis in 47.5%. Those with cervical progression were older (62.5 vs. 57.3, p = 0.04) with more nodes affected (12.1 vs. 2.8, p = 0.04) and had EBRT performed later following surgery (76.6 vs. 64 months, p = 0.05). EBRT associated with multikinase inhibitors showed longer overall survival than EBRT alone (64.3 vs. 37.9, p = 0.018) and better local disease control. Performing EBRT before radioiodine (RAI) was associated with longer cervical progression‐free survival (CPFS) than was RAI before (67.5 vs. 34.5, p < 0.01). EBRT ≥2 years after surgery was associated with worse CPFS (4.9 vs. 34, p = 0.04). The most common molecular alterations were ERBB2, BRAF, FAT1, RET and ROS1 and TERT mutation was predictive of worse disease control after EBRT (p = 0.04). Conclusion: Younger patients, with fewer affected nodes and treated earlier after surgery had better cervical disease control. Combination of EBRT with MKI improved OS. TERT mutation might indicate worse responders to EBRT; however, further studies are necessary to clarify the role of molecular testing in selecting candidates for cervical EBRT. Clinical symptoms of cervical radiculopathy include radiating pain and paresthesia along the distribution of a nerve root, often associated with sensory loss and motor dysfunction. Radiculopathy often affects the cervical nerve root segments 4 to 8, resulting in well-recognized clinical syndromes. Patient were divided into two groups as surgeon choice, first group includes 15 patients who had been managed with anterior cervical plate with titanium cage. The second group included 15 patients who had been managed with locking standalone cage. The anterior cervical approach was used. The cartilaginous end plates of the upper and lower end plates were removed after inter body distraction under microscopic view. The removal of the posterior osteophytes was associated with the incision of the posterior longitudinal ligament. The test results demonstrated the mean Visual analogue scores at different time intervals for both the implant groups i.e. locking standalone cage and anterior cervical plate with disc cage groups, the mean VAS scores at pre-op period was 8.12 ± 0.99 for the LSC group and 7.53±0.74 for the APC group. In the post-operative period it was 4.47 ± 1.18 in the LSC group and 4.07±0.26 in the APC group. At 3 months post op, the mean VAS score was 3.47 ± 1.19 in the LSC group and 4.0±0.38 in the APC group, at 6 Months was 3.20±1.21 in the LSC group and 3.47±0.52 in the APC group and at 1 year post op period, it was 2.87 ± 1.06 in the LSC group and 2.87±0.52 in the APC group. Summary: Background: Hyperthermia is a well-accepted cancer therapy. Microwaves provide a very precise, targeted means of hyperthermia and are currently used to treat plantar warts caused by cutaneous-infective human papillomaviruses (HPVs). Other HPV genotypes infecting the anogenital mucosa cause genital warts or preneoplastic lesions or cervical cancer. Effective, non-ablative therapies for these morbid HPV-associated lesions are lacking. Methods: The molecular consequences of microwave treatment were investigated in in vitro cultured three-dimensional HPV-positive cervical tumour tissues, and tissues formed from HPV-infected normal immortalised keratinocytes. Microwave energy delivery to tissues was quantified. Quantitative reverse transcriptase PCR was used to quantify mRNA expression. Immunohistochemistry and fluorescence immunostaining was used to assess protein expression. Findings: Microwave energy deposition induced sustained, localised cell death at the treatment site. There was a downregulation in levels of HPV oncoproteins E6 and E7 alongside a reduction in cellular growth/proliferation and induction of apoptosis/autophagy. HSP70 expression confirmed hyperthermia, concomitant with induction of translational stress. Interpretation: The data suggest that microwave treatment inhibits tumour cell proliferation and allows the natural apoptosis of HPV-infected cells to resume. Precision microwave delivery presents a potential new treatment for treating HPV-positive anogenital precancerous lesions and cancers. Human papillomavirus (HPV) is the most common sexually transmitted virus in the world. About 70% of cervical cancers are caused by the most oncogenic HPV genotypes of 16 and 18. Since available prophylactic vaccines do not induce immunity in those with established HPV infections, the development of therapeutic HPV vaccines using E6 and E7 oncogenes, or both as the target antigens remains essential. Also, knocking out the E6 and E7 oncogenes in host genome by genome-editing CRISPR/Cas system can result in tumor growth suppression. These methods have shown promising results in both preclinical and clinical trials and can be used for controlling the progression of HPV-related cervical diseases. This comprehensive review will detail the current treatment of HPV-related cervical precancerous and cancerous diseases. We also reviewed the future direction of treatment including different kinds of therapeutic methods and vaccines, genome-editing CRISPR/Cas system being studied in clinical trials. Although the progress in the development of therapeutic HPV vaccine has been slow, encouraging results from recent trials showed vaccine-induced regression in high-grade CIN lesions. CRISPR/Cas genome-editing system is also a promising strategy for HPV cancer therapy. However, its safety and specificity need to be optimized before it is used in clinical setting. Abstract Background An infection with high-risk human papillomavirus (HPV) is the obligatory aetiological factor for the development of cervical cancer. In Switzerland, the prevention strategy for cervical cancer is based on primary prevention via HPV vaccination and secondary prevention with an opportunistic screening programme for precancerous lesions. Vaccination is recommended to 11–26 years old male and female persons. The objective of the study was to assess the epidemiological impact on cervical cancer of switching from the currently implemented programme with the 4-valent vaccine to the 9-valent vaccine, in an 11–26 years old gender-neutral vaccination programme in Switzerland. Methods A previously validated dynamic transmission model of HPV infections was adapted and calibrated to the Swiss setting assuming an 80% coverage rate in HPV-vaccination and lifelong vaccine type-specific protection. A gender-neutral vaccination programme (males and females) for 11–26 years old with a 9-valent HPV vaccine was compared with the current 11–26 years old gender-neutral 4-valent vaccination programme. Sensitivity analyses were conducted in order to test the impact of lower vaccination coverage rates and a shorter duration of protection on the model outcomes. Results In Switzerland, a 9-valent gender-neutral vaccination programme would result in an additional prevention of 2979 cervical cancer cases, 13,862 CIN3 and 15,000 CIN2 cases, compared with the 4-valent gender-neutral vaccination programme over 100 years. These additional disease cases avoided would correspond to a 24, 36 and 48% cumulative incidence decrease in cervical cancer, CIN3 and CIN2 cases, respectively. It would also prevent additional 741 cervical cancer-related deaths over 100 years. A substantial additional reduction in cervical cancer and precancerous lesions burden is still observed when varying the vaccination coverage rate from 30 to 60% or reducing the duration of protection from lifelong to 20 years. Conclusions The switch to the 9-valent vaccine in Switzerland to prevent cervical diseases showed an important contribution in terms of public health impact compared with the 4-valent vaccine in an 11–26 years old gender-neutral population, even with very conservative assumptions such as low coverage rates or low duration of protection and limiting analysis to only cervical disease.

The investigation of differentially expressed genes (DEGs) and their interactome could provide valuable insights for the development of markers to optimize cervical intraepithelial neoplasia (CIN) screening and treatment. This study investigated patients with cervical disease to identify gene markers whose dysregulated expression and protein interaction interface were linked with CIN and cervical cancer (CC). Literature search of microarray datasets containing cervical epithelial samples was conducted in Gene Expression Omnibus and Pubmed/Medline from inception until March 2021. Retrieved DEGs were used to construct two protein-protein interaction (PPI) networks. Module DEGs that overlapped between CIN and CC samples, were ranked based on 11 topological algorithms. The highest-ranked hub gene was retrieved and its correlation with prognosis, tissue expression and tumor purity in patients with CC, was evaluated. Screening of the literature yielded 9 microarray datasets (GSE7803, GSE27678, GSE63514, GSE6791, GSE9750, GSE29570, GSE39001, GSE63678, GSE67522). Two PPI networks from CIN and CC samples were constructed and consisted of 1704 and 3748 DEGs along 21393 and 79828 interactions, respectively. Two gene clusters were retrieved in the CIN network and three in the CC network. Multi-algorithmic topological analysis revealed PCNA as the highest ranked hub gene between the two networks, both in terms of expression and interactions. Further analysis revealed that while PCNA was overexpressed in CC tissues, it was correlated with favorable prognosis (log-rank P=0.022, HR=0.58) and tumor purity (P=9.86 × 10-4, partial rho=0.197) in CC patients. This study identified that cervical PCNA exhibited multi-algorithmic topological significance among DEGs from CIN and CC samples. Overall, PCNA may serve as a potential gene marker of CIN progression. Experimental validation is necessary to examine its value in patients with cervical disease.

The purpose of this study was to investigate the incidence of cervical disease in women of different age groups in the Republic of Sakha (Yakutia). Materials and Methods: The cytological material of the cervix of 7,600 women aged between 18 and 88 years was analyzed in the laboratory of pathomorphology, histology and cytology. The material of the cytological study consisted of smears of cervical mucosa and the cervical canal, stained according to the method of Romanovsky-Giemsa. The study was conducted with subjects grouped according to age: Group 1 (18-29), Group 2 (30-44), Group 3 (45-59), and Group 4 (60-74). Results: According to the results of cytological analysis, inflammatory diseases of the cervix uteri were diagnosed in 4,629/61% cases. Among age groups, the highest rate of inflammatory diseases of the cervix uteri was registered in Group 1 and Group 2. Benign cervical lesions were found in 563/7.4% cases with the highest incidence in Groups 1 and 2. The most frequently diagnosed pathology was squamous cell metaplasia with maximum frequency in Group 2 and Group 1. Cervical intraepithelial neoplasia (or dysplasia) (CIN) was detected in 359/4.7% cases. CIN I, CIN II and CIN III were registered in 220/61.3%, 84/24.5%, and 38/10.6% women, respectively. At the same time, the maximum frequency of dysplasia was noted in Group 1 and Group 2. Thus, results obtained indicate a high incidence of cervical disease in women of reproductive age.

The human papillomavirus (HPV) replication cycle is tightly linked to epithelial cell differentiation. To examine HPV-associated changes in the keratinocyte transcriptome, RNAs isolated from undifferentiated and differentiated cell populations of normal, spontaneously immortalized keratinocytes (NIKS) and NIKS stably transfected with HPV16 episomal genomes (NIKS16) were compared using nextgeneration sequencing (RNA-Seq). HPV16 infection altered expression of 2,862 cellular genes. Next, to elucidate the role of keratinocyte gene expression in late events during the viral life cycle, RNA-Seq was carried out on triplicate differentiated populations of NIKS (uninfected) and NIKS16 (infected). Of the top 966 genes altered (>log2 = 1.8, 3.5-fold change), 670 genes were downregulated and 296 genes were upregulated. HPV downregulated many genes involved in epithelial barrier function, which involves structural resistance to the environment and immunity to infectious agents. For example, HPV infection repressed expression of the differentiated keratinocyte-specific pattern recognition receptor TLR7, the Langerhans cell chemoattractant CCL20, and proinflammatory cytokines interleukin 1α (IL-1α) and IL-1β. However, the type I interferon regulator IRF1, kappa interferon (IFN-κ), and viral restriction factors (IFIT1, -2, -3, and -5, OASL, CD74, and RTP4) were upregulated. HPV infection abrogated gene expression associated with the physical epithelial barrier, including keratinocyte cytoskeleton, intercellular junctions, and cell adhesion. Quantitative PCR (qRT-PCR) and Western blotting confirmed changes in expression of seven of the most significantly altered mRNAs. Expression of three genes showed statistically significant changes during cervical disease progression in clinical samples. Taken together, the data indicate that HPV infection manipulates the differentiating keratinocyte transcriptome to create an environment conducive to productive viral replication and egress. IMPORTANCE HPV genome amplification and capsid formation take place in differentiated keratinocytes. The viral life cycle is intimately associated with host cell differentiation. Deep sequencing (RNA-Seq) of RNA from undifferentiated and differentiated uninfected and HPV16-positive keratinocytes showed that almost 3,000 genes were differentially expressed in keratinocytes due to HPV16 infection. Strikingly, the epithelial barrier function of differentiated keratinocytes, comprising keratinocyte immune function and cellular structure, was found to be disrupted. These data provide new insights into the virus-host interaction that is crucial for the production of infectious virus and reveal that HPV infection remodels keratinocytes for completion of the virus replication cycle. Objective To observe the effect of 5-aminolevulinic acid photodynamic therapy (ALA-PDT) in the treatment of high-risk human papillomavirus (HR-HPV) infection, and summarize related nursing interventions during the treatment. Methods Totally 25 patients with HR-HPV positive for more than 12 months were included in this study. The 5-aminolevulinic acid was used according to the anatomical features of cervix uteri. The comprehensive nursing was carried out during the period of ALA-PDT. The rate of negative conversion of HPV and adverse reaction were observed at 4 weeks after treatment. Results All 25 patients completed the trail of ALA-PDT. The cumulative negative conversion rate was 84. 00% (21/25). Conclusion The ALA-PDT is a safe and effective treatment for HR-HPV infection. It is also required to carry out comprehensive nursing interventions and humanistic care during the treatment, in order to relieve the negative emotion of patients. [ABSTRACT FROM AUTHOR]

Objective To understand the infection rate of human papillomavirus（HPV） and the distribution of HPV genotype in female cervix in Zhaotong city. Methods 5 430 female patients in the outpatient section of gynaecology of our hospital were given HPV genotyping test with the method of Flowcytometry Fluorescence hybridization. At the same time， 92 patients were given cervical biopsy pathological examination. Result 705 out of 5 340 cases were HPV positive， and the infection rate was 12.98%; 554 cases （78.58%） were infected with a single subtype，151 cases （21.42%） infected with mixed subtype， among which 106 cases（15.04%） were double infection，38cases（5.39%） triple infection， 5 cases （0.71%） fourfold and 2 cases （0.28%） fivefold infection. In the cases of single positive infection， high-risk subtypes infection accounts for 60.85％ （429/705） and low risk subtype infection accounts for 17.73％（125/705） . HPV 52 infection patient take up most in low grade cervical intraepithelial neoplasia group， while HPV16 infection patients take up most in the high grade cervical intraepithelial neoplasia group. Conclusion The positive rate of HPV virus infection in Zhaotong city was 12.98%， and the most of genotyoe was HPV52， followed by HPV16， 81， 53， 58. HPV16 infection has an important role in the pathogenesis of cervical cancer. Objective: To explore the clinical application of cervical multi-point biopsy under colposeopy compared with routine cervical biopsy in the diagnosis of cervical disease and influencing factors.Methods:Selected 86 cases of patients with cervical disease admitted in department of gynaecology of our hospital from May 2015 to March 2016,and were divided into control group and research group according to the random number table method,with 43 cases in each group.The patients in control group were treated with routine cervical biopsy,while the patients in research group were treated by cervical multi-point biopsy under colposcopy.Compared the relevance ratio of two kinds of methods and pathologic examination results,andcompared the omission diagnostic rate of two methods.In addition,explored the related factors of missed diagnosis of cervical multi-point biopsy under colposcopy.Results:Compared with pathologic examination results,except cervical disease and cervical cancer,the detection rate and total detection rate of other types of cervical disease in research group were significantly higher than those of control group (P＜0.05);In addition to the cervical cancer,the missed diagnosis rate ofcervical disease ofresearch group significantly decreased compared with the control group (P＜0.05);Aged over 50,high-level CIN types,the decreased of number of specimens and cervical small lesions were likely to rise the missed diagnosis rate of cervical multi-point biopsy under colposcopy. Conclusion: Compared with the routine cervical biopsy,cervical multi-point biopsy under colposcopy in the diagnosis of cervical disease has high diagnostic rate and low missed diagnosis rate. We can appropriately increase number of biopsy samples to reduce the missed diagnosis so that cervical disease is treated by early detection and effective treatment.It is worth popularization and application in clinic. Objective: To study the results of FRD in the screening of cervical disease, to provide basis for clinical diagnosis and treatment. Methods: 306 cases of cervical disease screening in our hospital from May 2015 to May 2016 were selected, FRD and liquid based cytology test were performed for all patients, two kinds of detection have a positive then do the vagina by pathological examination, pathological examination as the standard, diagnostic value of two kinds of detection methods were compared. Results: The sensitivity of the two detection methods in the screening of cervical precancerous lesions and cervical cancer were 87.6% and 84%, specificity was 69.0% and 63.3%, the diagnostic accordance rate was 79.7% and 75.8%, positive predictive valu was 79.2% and 78.4%, negative predictive value was 81.7% and 71.4%, Kappa values were 0.56 and 0.48, the differences were not statistically significant (P>0.05). Conclusion: FRD and liquid based cytology test for screening cervical precancerous lesion and cervical cancer has similar effect, but it is easy to operate, and it is worthy of clinical application. Objective: We report two patients with benign giant cervical polyps. Case Report: A 47-year-old multiparous woman complained of a protruding mass from her vagina for 6 months. On pelvic examination, there was a large cervical polyp measuring 7 × 2.5 cm arising from the endocervix. The polyp was excised by electrocauterization at the base. The pathological report revealed no evidence of malignancy in the polyp. A 45-year-old nulligravid woman suffered from menorrhagia for 2 months and a mass protruding from the vagina was noted for 10 days. On pelvic examination, there were two cervical polyps. One was 5 × 2 cm and arose from the posterior lip of the cervix and the other was 1 × 1 cm and arose from the right lateral lip. The polyps were excised by electrocauterization. The pathological report revealed benign cervical polyps. Conclusion: Giant cervical polyp is uncommon and malignancy has not been reported in the 11 cases in the literature. Excision was adequate for these lesions.

HPV-16 varies geographically and is correlated with cervical cancer genesis and progression. This study aimed to determine the distribution of HPV-16 E6/E7 genetic variation in patients with invasive cervical cancer or precancer in Guizhou Province, China. A case-control study was designed, and the distribution of HPV-16 E6/E7 genetic variation was compared among women with cervical cancer, precancer, and sexually active without cervical lesion. HPV infection was detected through flow-through hybridization and gene chip techniques to determine the prevalence of HPV 16 E6/E7 genetic variation. Among 90 specimens (30 cervical cancer, 30 precancer, 30 controls), 81 were subjected to HPV-16 E6/ E7 gene sequencing. The rates of DNA sequence mutation and amino acid mutation were 76.5% (62/81) and 66.7% (54/81), respectively. Both E6 and E7 genes showed higher mutation rate than their prototypes. The prevalence of E6/E7 mutation significantly differed between the cervical cancer and the controls (P<0.05) and between the cervical precancer and the controls (P<0.05). Mutations were simultaneously detected at the E6-D32E (T96A) and E7-M28V (A82G)/L94P (T281C) sites of the amino acid sequence. The most common genetic variation was D32E/M28V/ L94P, which accounted for 35.8% of the cases (29/81). D32E/M28V/L94P mutation was higher in the cervical cancer and precancer compared with the prototype. HPV-16 E6/E7 genetic variations, such as D32E/M28V/L94P, are more prevalent in cervical cancer or precancer than those in the controls. Objective: To investigate the risk factors for female cervical disease in minority highland pastoral. Methods: 600 females in minority highland pastoral were selected from January 2013 to December 2013 at the region as the research objects. They were examined by bus smear, colposcopy examination and biopsy of cervical for disease screening and were given the investigation of those basic information, lifestyle behaviors and health knowledge. Results: The detection of cervical disease was 2.25%, which CIN I were 21 cases, CIN II were 9 cases, CIN III and cervical carcinoma in situ were 6 cases, and the detection rate of cervical cancer was 0.38%. Univariate Logistic regression analysis showed that the incidence of cervical cancer had significant correlation with gravidity,parity, cervicitis history, awareness of the disease, income, multiple sexual partners, and age at first intercourse(P<0.05). Multivariate analysis showed that whether cervicitis history, awareness of the disease, parity, multiple sexual partners, income, age at first intercourse were the independent risk factors of the cervical disease(P<0.05). Conclusions: The incidence of female cervical disease in minority highland pastoral is relatively high, and cervicitis history, awareness of the disease, parity, multiple sexual partners, the source of income,and age at first intercourse are the independent risk factors, suggesting that we should have intensify propaganda, popularization of science knowledge in cancer prevention, and actively give the management of prevention and health education. Abstract: Background: Few data exist regarding the effect of hormonal contraception (HC) on incidence and progression of cervical disease (e.g., cervical dysplasia, squamous intraepithelial lesions, cervical intraepithelial neoplasia) in HIV-infected African women. Study Design: We conducted an observational study of HIV-seropositive women in Johannesburg, South Africa. The effect of individual HC types on the incidence and progression of cervical disease was determined using Poisson regression to obtain adjusted incidence rate ratios. Results: We evaluated 594 HIV-infected women, with median follow-up time of 445 days; 75 of these women were receiving some form of HC (largely DMPA, NET-EN, or COCs) at baseline. Risks of incidence and progression of cervical disease were similar comparing women not receiving HCs to women receiving DMPA, NET-EN, or COCs both individually by HC-type and considering all HC together. Conclusions: There was no statistically significant effect of particular HC methods or of HC use in general on rates of incidence or progression of cervical disease in this study. These results should reassure us that use of HC is unlikely to substantially increase risks of cervical disease among HIV-positive women. [Copyright &y& Elsevier]
