4. Sleep disturbances partially mediate the association between problematic internet use and somatic symptomatology in adolescence

The Internet is widely used among adolescents for different purposes, including social connections, entertainment and schoolwork. Nevertheless, research has demonstrated the negative impact of problematic internet use (PIU) on psychophysical well-being in adolescence. This study aims to investigate internet use and its association with somatic symptoms, exploring the specific role of sleep disturbances that affect overall sleep quality. A mediation model was used to demonstrate the link between PIU, somatic symptoms and sleep disturbances in a sample of 412 adolescents (52% of males) aged 10-17 (M = 14.12, SD = 1.46). We also investigated whether somatic symptoms were differently experienced and reported by participants at risk of developing internet addiction (n = 58) from those not-at risk (n = 354). Sleep disturbances partially mediated the relationship between PIU and somatic symptoms. Furthermore, PIU seems to lead to poorer sleep quality which may in turn result in physical problems with an increase in somatic symptoms. Findings of this study showed that adolescents at risk of developing internet addiction suffered from a specific somatic symptomatology respect to those not-at risk. Finally, PIU has a positive direct effect on somatic symptoms. These results highlight the role of sleep disturbances with implications for overall sleep quality in the association between PIU and somatic symptoms. When designing intervention programs, the associations between PIU, sleep quality and somatic symptoms in adolescence and the negative repercussions of PIU on psychophysical health should be kept in mind. 5. A survey of extended radio jets with Chandra and HST

We present the results from an X-ray and optical survey of a sample of 17 radio jets in AGN performed with Chandra and HST. The sample was selected from the radio and is unbiased toward detection at shorter wavelengths, but preferentially it includes beamed sources. We find that X-ray emission is common on kpc-scales, with over half radio jets exhibiting at least one X-ray knot on the Chandra images. The distributions of the radio-to-X-ray and radio-to-optical spectral indices for the detected jets are similar to the limits for the non-detections,suggesting all bright radio jets have X-ray counterparts which will be visible in longer observations. Comparing the radio and X-ray morphologies shows that the majority of the X-ray jets have structures that closely map the radio. Analysis of the SED of the jet knots suggest the knots in which the X-ray and radio morphologies track each other produce X-rays by IC scattering of the Cosmic Microwave Background. The remaining knots produce X-rays by the synchrotron process. Spectral changes are detected along the jets, with the ratio of the X-ray-to-radio and optical-to-radio flux densities decreasing from the inner to the outer regions. This suggests the presence of an additional contribution to the X-ray flux in the jet's inner part, either from synchrotron or IC of the stellar light. Alternatively, in a pure IC/CMB scenario, the plasma decelerates as it flows from the inner to the outer regions. Finally, the X-ray spectral indices for the brightest knots are flat, indicating that the bulk of the luminosity of the jets is emitted at GeV energies, and raising the interesting possibility of future detections with GLAST., Comment: 26 pages, 6 ps figures, 6 jpeg figures (1 replaced); accepted for publication in ApJ

6. Prevalence and Clinical Correlates of Deliberate Self-Harm among a Community Sample of Italian Adolescents

- Abstract
The aims of this study were to investigate the rates of deliberate self-harm (DSH) behavior among an Italian adolescent sample, as well as to explore its clinical correlates. On a sample of 234 adolescents in Italian secondary schools (Mean age = 16.47; SD = 1.7) were assessed the DSH as well as externalizing symptoms (including both conduct disorder [CD] and oppositional defiant disorder [ODD] symptoms), borderline personality disorder [BPD] symptoms, dissociative symptoms, and the incidence of life-stressors. Consistent with past research on DSH in youth, 42% of the adolescents in this sample engaged in DSH. Results indicate a positive association between DSH and all psychopathological correlates, including BPD, dissociative, and ODD and CD symptoms. Further, findings revealed an association between DSH and specific life-stressors (i.e., psychological and sexual abuse, natural disasters and serious accidents, the loss of someone important, and the witnessing of family violence or a serious accident). 7. Clinical symptoms in pre-COVID 19 pandemic versus COVID 19 pandemic samples of Italian university students

- Abstract
BACKGROUND Recent studies on the effect of the COVID-19 pandemic have revealed that university students are in a more vulnerable position in terms of their mental health. METHODS: The aim of this study is to examine differences in clinical symptoms and psychological distress comparing two groups of university students seeking psychological intervention at a University Psychological Counselling Center before the COVID-19 pandemic period and during the pandemic. A sample of 187 students was distributed into two groups: Pre-COVID-19 pandemic (n = 115) and COVID-19 Pandemic (n = 72). The Personality Inventory-Brief Form for DSM-5 was used to measure personality traits; the Symptom Checklist-90-Revised was used to assess clinical symptoms and the Outcome Questionnaire-45 explored important psychological distress. RESULTS Findings highlighted higher levels of clinical symptoms in COVID-19 pandemic students than in pre-pandemic ones. Conversely, the two groups did not differ in psychological distress. Data seems to confirm that the pandemic period had a negative impact on the mental health of university students seeking psychological help. CONCLUSION Findings indicate the need to monitor the clinical symptomatology of university students to prevent long-term psychopathology influencing academic functioning.

There is controversy about the differences between conventional and laparoscopic approach. The aim of this study is to compare satisfaction of patients initially operated of inguinal conventional hernioplasty (Lichtenstein procedure) and then of a laparoscopic contralateral hernioplasty (TAPP procedure) with a follow-up of 10 years. There were excluded those patients older than 85 years old to avoid biases of comparison and those who presented surgery complications, inguinal-escrotal hernias or hernia recurrence. We compared degree of satisfaction in both procedures using a 5 steps scale: 1. Much better, 2. Better, 3. Equal,4. Worse and 5. Much worse. Of the total of 62 registered patients, 32 were excluded. Of 30 evaluated patients, 26 presented a major degree of satisfaction as the laparoscopic technique (20 much better, 6 Better),whereas the 4 remaining ones demonstrated equal satisfaction with both techniques.

Non-familial small bowel carcinomas are relatively rare and have a poor prognosis. Two small bowel carcinoma subsets may arise in distinct immune-inflammatory diseases (celiac disease and Crohn's disease) and have been recently suggested to differ in prognosis, celiac disease-associated carcinoma cases showing a better outcome, possibly due to their higher DNA microsatellite instability and tumor-infiltrating T lymphocytes. In this study, we investigated the histological structure (glandular vs diffuse/poorly cohesive, mixed or solid), cell phenotype (intestinal vs gastric/pancreatobiliary duct type) and Wnt signaling activation (beta-catenin and/or SOX-9 nuclear expression) in a series of 26 celiac disease-associated small bowel carcinoma, 25 Crohn's disease-associated small bowel carcinoma and 25 sporadic small bowel carcinoma cases, searching for new prognostic parameters. In addition, non-tumor mucosa of celiac and Crohn's disease patients was investigated for epithelial precursor changes (hyperplastic, metaplastic or dysplastic) to help clarify carcinoma histogenesis. When compared with non-glandular structure and non-intestinal phenotype, both glandular structure and intestinal phenotype were associated with a more favorable outcome at univariable or stage-and microsatellite instability/tumor-infiltrating lymphocyte-inclusive multivariable analysis. The prognostic power of histological structure was independent of the clinical groups while the non-intestinal phenotype, associated with poor outcome, was dominant among Crohn's disease-associated carcinoma. Both nuclear beta-catenin and SOX-9 were preferably expressed among celiac disease-associated carcinomas; however, they were devoid, per se, of prognostic value. We obtained findings supporting an origin of celiac disease-associated carcinoma in SOX-9-positive immature hyperplastic crypts, partly through flat beta-catenin-positive dysplasia, and of Crohn's disease-associated carcinoma in a metaplastic (gastric and/or pancreatobiliary-type) mucosa, often through dysplastic polypoid growths of metaplastic phenotype. In conclusion, despite their common origin in a chronically inflamed mucosa, celiac disease-associated and Crohn's disease-associated small bowel carcinomas differ substantially in histological structure, phenotype, microsatellite instability/tumor-infiltrating lymphocyte status, Wnt pathway activation, mucosal precursor lesions and prognosis.

Background: A growing body of research claims that sexual minority individuals have nearly twice the suicidal ideation rate of heterosexual individuals. Method: The main objective of the current study was to test a model wherein internalized sexual stigma (ISS) mediates the association between some gay-related stressors and suicidal ideation. The present cross-sectional survey involved two samples of lesbian and gay young adults from Spain (N = 209) and Italy (N = 345). The total sample included 316 gay men and 228 lesbian women. Path analysis was used to test the direct and indirect (mediated) effects of the variables from different domains on repulsion by life. Results: In both the Spanish and Italian samples, past victimization experiences and ISS had a significant direct effect on repulsion by life. In both samples, ISS acted as a significant mediator in the effect of the concealment of one’s sexual orientation and the religious involvement of repulsion by life. Conclusion: The current study indicates that ISS is a potential moderator of the effect of concealing one’s sexual orientation, religious involvement, and past experiences of victimization on suicidal attitudes.

The early differentiation of Alzheimer’s disease (AD) from frontotemporal dementia (FTD) may be difficult. The Tower of London (ToL), thought to assess executive functions such as planning and visuo-spatial working memory, could help in this purpose. Twentytwo Dementia Centers consecutively recruited patients with early FTD or AD. ToL performances of these groups were analyzed using both the conventional statistical approaches and the Artificial Neural Networks (ANNs) modelling. Ninety-four non aphasic FTD and 160 AD patients were recruited. ToL Accuracy Score (AS) significantly (p < 0.05) The use of hidden information contained in the different items of ToL and the non linear processing of the data through ANNs allows a high discrimination between FTD and AD in individual patients. However, the discriminant validity of AS checked by ROC curve analysis, yielded no significant results in terms of sensitivity and specificity (AUC 0.63). The performances of the 12 Success Subscores (SS) together with age, gender and schooling years were entered into advanced ANNs developed by Semeion Institute. The best ANNs were selected and submitted to ROC curves. The nonlinear model was able to discriminate FTD from AD with an average AUC for 7 independent trials of 0.82. The use of hidden information contained in the different items of ToL and the non linear processing of the data through ANNs allows a high discrimination between FTD and AD in individual patients.

Purpose: To apply and evaluate Bayesian inter-product quantitative methods for signaling an excess of adverse events to specific pharmaceutical products, taking into account sales data as well as other information accessible to a company's drug-monitoring system. Methods: The Bayesian confidence propagation neural network (BCPNN) and the gamma Poisson shrinkage (GPS) were applied to a selected sample of spontaneously reported adverse events following the administration of a Bracco contrast medium. Both the conventional approach and sales data were exploited to represent the patients' population drug exposure. Results: Available data allow the detection of potential safety issues of a drug in comparison to those expected in its pharmaceutical category. No difference in signal detection performance between the BCPNN and GPS methods was found. Instead, adjustment by sales data markedly affected the signals detected, with the desirable property of preserving the risk order, for any given adverse drug reaction, among different drugs. Conclusions: Without comprehensive data on the adverse events reported worldwide for all pharmaceutical products, signaling methods are appropriate to compare the safety of drugs sharing a similar clinical indication. Sales data show a relevant impact on the value of signals, improving the analysis of spontaneous reports collected by a company monitoring system.

- Abstract
Mitochondrial disorders are highly heterogeneous conditions characterized by defects of the mitochondrial respiratory chain. Pharmacological activation of mitochondrial biogenesis has been proposed as an effective means to correct the biochemical defects and ameliorate the clinical phenotype in these severely disabling, often fatal, disorders. Pathways related to mitochondrial biogenesis are targets of Sirtuin1, a NAD+-dependent protein deacetylase. As NAD+ boosts the activity of Sirtuin1 and other sirtuins, intracellular levels of NAD+ play a key role in the homeostatic control of mitochondrial function by the metabolic status of the cell. We show here that supplementation with nicotinamide riboside, a natural NAD+ precursor, or reduction of NAD+ consumption by inhibiting the poly(ADP-ribose) polymerases, leads to marked improvement of the respiratory chain defect and exercise intolerance of the Sco2 knockout/knockin mouse, a mitochondrial disease model characterized by impaired cytochrome c oxidase biogenesis. This strategy is potentially translatable into therapy of mitochondrial disorders in humans. © 2014 The Authors.

Aim: To determine the applicability and psychometric properties of the Italian version of the Disability Assessment of Dementia scale (DAD-I) in a community-residing population with early-stage Alzheimer’s disease (AD). Methods: The DAD-I was administered to the primary caregivers of 159 patients (mean age ± SD 77.1 ± 5.2) with mild AD (mean Mini Mental State Examination, MMSE, ± SD 23.1 ± 2.2). Results: The DAD-I showed excellent internal consistency reliability (Cronbach’s α = 0.92) and good construct validity. The DAD-I score was not significantly associated with gender, education and presumed duration of the illness, and had a low negative correlation with age. The DAD-I score correlated moderately with the traditional Instrumental Activities of Daily Living and Activities of Daily Living scales, respectively, with r = 0.53 and r = 0.54 (p < 0.0001). Relatively low, but statistically significant correlations (r ranging between 0.21 and 0.31) with the MMSE were also found. Conclusion: The DAD-I was found to be a reliable and valid instrument to assess functional disability in community-dwelling Italian subjects with early-stage AD.

Background: Executive dysfunction in Alzheimer’s disease (AD) has been recently recognized as an early and prominent clinical sign. The Tower of London (ToL), a task specifically devised to test executive functions of visuospatial planning and problem solving, has frequently been used in neuropsychological experiments, but rarely in the clinical ground. Methods: One hundred and sixty-one AD patients and 212 nondemented healthy controls were administered a simplified ToL version. Results: AD patients were significantly impaired (p < 0.0001) in all ToL scores and in the total execution time. The ‘accuracy’ score of ToL at a cut off of ≤29/36 yielded a sensitivity of 71.2% and a specificity of 76.4% (AUC 0.79) for the diagnosis of AD versus controls. Conclusions: Visuospatial planning and problem solving are significantly impaired in early dementia of the Alzheimer’s type. A successful sensitivity/specificity ratio, the independence of education and the simplicity of this version of ToL make it a useful executive functioning screening test for early AD.
