- Abstract
The morphology of bird wings is subject to a variety of selective pressures, including migration, predation, habitat structure and sexual selection. Variation in wing morphology also occurs at the intraspecific and intrapopulation level, and can be related to sex, age, migration strategy and environmental factors. The relationship between environment and intraspecific variation in wing morphology is still poorly understood. In this work, we studied the relationship between wing morphology and breeding environment in a high-elevation specialist bird, the water pipit Anthus spinoletta. We calculated wing isometric size, pointedness and convexity of 84 birds mist-netted at breeding sites in year 2021 in the European Alps. We then searched for associations between these traits and potentially relevant breeding site characteristics (vegetation structure, elevation, latitude). For all wing traits, sex and one or more environmental factors best explained the variation, with environmental factors explaining between 3 and 8% of the variation. Wing size was negatively related to tree cover and wing convexity was negatively related to bush cover. Elevation contributed to explain variation in wing pointedness, but the direction of its effect was unclear. The negative relationship between wing size and tree cover could be due to intraspecific competition, i.e. to the relegation of smaller winged low-quality individuals in marginal grassland areas. Higher wing convexity could improve predator escape ability in areas with scarce protecting vegetation, with possible effects on habitat choice. These findings represent one of the few demonstrated cases of wing morphology–environment relationships at the intraspecific level.

We report the case of an unexpected massive haemolysis occurred early after mitral valve repair. Potential underlying causes have been investigated, with review of the literature. After mitral valve replacement, haemolysis disappeared and the patient recovered, returning to normal life after 12 months.

Background: Venous thromboembolism (VTE) is a major health problem, with over one million events every year in Europe. However, there is a paucity of data on the current management in real life, including factors influencing treatment pathways, patient satisfaction, quality of life (QoL), and utilization of health care resources and the corresponding costs. The PREFER in VTE registry has been designed to address this and to understand medical care and needs as well as potential gaps for improvement. Methods/design: The PREFER in VTE registry was a prospective, observational, multicenter study conducted in seven European countries including Austria, France Germany, Italy, Spain, Switzerland, and the UK to assess the characteristics and the management of patients with VTE, the use of health care resources, and to provide data to estimate the costs for 12 months treatment following a first-time and/or recurrent VTE diagnosed in hospitals or specialized or primary care centers. In addition, existing anticoagulant treatment patterns, patient pathways, clinical outcomes, treatment satisfaction, and health related QoL were documented. The centers were chosen to reflect the care environment in which patients with VTE are managed in each of the participating countries. Patients were eligible to be enrolled into the registry if they were at least 18 years old, had a symptomatic, objectively confirmed first time or recurrent acute VTE defined as either distal or proximal deep vein thrombosis, pulmonary embolism or both. After the baseline visit at the time of the acute VTE event, further follow-up documentations occurred at 1, 3, 6 and 12 months. Follow-up data was collected by either routinely scheduled visits or by telephone calls. Results: Overall, 381 centers participated, which enrolled 3,545 patients during an observational period of 1 year. Conclusion: The PREFER in VTE registry will provide valuable insights into the characteristics of patients with VTE and their acute and mid-term management, as well as into drug utilization and the use of health care resources in acute first-time and/or recurrent VTE across Europe in clinical practice. We assessed the degree of genetic differentiation between insular and continental birds, their demographic history and genetic diversity. We compared the results, taking into account the differences in migration strategy and morphology of the two warblers. We found slightly lower genetic diversity in the insular populations than in the continental birds, possibly because of the lower population size or reduced immigration. The genetic differentiation between island and mainland birds was low, but higher in Cetti¿s Warbler than in the Great Reed Warbler, suggesting differences in the species¿ capability of crossing the sea. We found evidence for a past bottleneck in both the insular and continental populations of Cetti¿s Warbler, while for the Great Reed Warbler we found no signs of past population reductions. A high dispersal capability of the Great Reed Warbler may have allowed high gene flow, which may explain the observed interspecific differences in the demographic history of the Great Reed Warbler and Cetti¿s Warbler., We acknowledge M. - Abstract
Background: Venous thromboembolism (VTE) is a major health problem, with over one million events every year in Europe. However, there is a paucity of data on the current management in real life, including factors influencing treatment pathways, patient satisfaction, quality of life (QoL), and utilization of health care resources and the corresponding costs. The PREFER in VTE registry has been designed to address this and to understand medical care and needs as well as potential gaps for improvement. Methods/design: The PREFER in VTE registry was a prospective, observational, multicenter study conducted in seven European countries including Austria, France Germany, Italy, Spain, Switzerland, and the UK to assess the characteristics and the management of patients with VTE, the use of health care resources, and to provide data to estimate the costs for 12 months treatment following a first-time and/or recurrent VTE diagnosed in hospitals or specialized or primary care centers. In addition, existing anticoagulant treatment patterns, patient pathways, clinical outcomes, treatment satisfaction, and health related QoL were documented. The centers were chosen to reflect the care environment in which patients with VTE are managed in each of the participating countries. Patients were eligible to be enrolled into the registry if they were at least 18 years old, had a symptomatic, objectively confirmed first time or recurrent acute VTE defined as either distal or proximal deep vein thrombosis, pulmonary embolism or both. After the baseline visit at the time of the acute VTE event, further follow-up documentations occurred at 1, 3, 6 and 12 months. Follow-up data was collected by either routinely scheduled visits or by telephone calls. Results: Overall, 381 centers participated, which enrolled 3,545 patients during an observational period of 1 year. We aimed to document the epidemiology, management and outcomes of VTE using data from a large, observational database. PREFER in VTE was an international, non-interventional disease registry conducted between January 2013 and July 2015 in primary and secondary care across seven European countries. Consecutive patients with acute VTE were documented and followed up over 12 months. PREFER in VTE included 3,455 patients with a mean age of 60.8 ± 17.0 years. Overall, 53.0% were male. The majority of patients were assessed in the hospital setting as inpatients or outpatients (78.5%). The diagnosis was deep-vein thrombosis (DVT) in 59.5% and pulmonary embolism (PE) in 40.5%. The most common comorbidities were the various types of cardiovascular disease (excluding hypertension; 45.5%), hypertension (42.3%) and dyslipidaemia (21.1%). Following the index VTE, a large proportion of patients received initial therapy with heparin (73.2%), almost half received a vitamin K antagonist (48.7%) and nearly a quarter received a DOAC (24.5%). Almost a quarter of all presentations were for recurrent VTE, with >80% of previous episodes having occurred more than 12 months prior to baseline. In conclusion, PREFER in VTE has provided contemporary insights into VTE patients and their real-world management, including their baseline characteristics, risk factors, disease history, symptoms and signs, initial therapy and outcomes.

Seasonal variations of endocrine function have been extensively studied both in rodents and in man. However, only few studies have attempted to concomitantly evaluate and compare circadian and circannual components by inferential rhythmometric procedures. We have examined [9] circadian profiles (single cosinor: P less than 0.01) of plasma cortisol obtained from samples drawn at 4-h intervals in healthy adult subjects of both sexes (71 males, 120 females, aged 20-54 years) along a 7-year span, from 1974 to 1980. All subjects were routine synchronized. Circulating cortisol was measured by a competitive protein-binding technique and expressed as microgram/dl; intra-assay coefficient of variation was always less than 5%. Both conventional and rhythmometric analysis were used to evaluate seasonal differences of mesor, amplitude and acrophase. The population-mean cosinor analyses revealed statistically significant differences as a function of the time of the year in both the circadian amplitude and acrophase. Examining data year by year revealed in 5 out of 7 annual cycles an apparent tendency to a delayed acrophase and to a reduced amplitude with advancing months towards winter. No significant changes in mesor values were found. Further fitting to a 365.25-day cosine curve of circadian parameters available for different months of an 'average' year yielded significant circannual rhythmicity for acrophase and amplitude; crest-value resulted in spring and autumn, respectively. Our data extend previous observations and are consistent with the existence of circannual variations in plasma cortisol levels in the human. In Italian people, cortisol secretion is conceivably higher in cold months; seasonal changes, on the other hand, appear to be of minor entity of at least as inferred by transverse sampling on a relatively large population. Effects of lighting regimen and of other climatological variables upon the adrenal cycle have to be considered.

To investigate whether early postoperative changes in blood lactate concentration indicate the functional recovery of the newly grafted liver, changes in oxygen supply, oxygen consumption, acid-base equilibrium, and blood lactate concentrations were prospectively studied in a group of 53 postnecrotic cirrhotic patients during the various phases of orthotopic liver transplantation (preanhepatic, anhepatic, neohepatic) and for the first 48 h following reperfusion. The patients were divided into two groups according to the quality of the early graft function, as indicated by alanine aminotransferase, bile flow, and prothrombin activity: group A (49 patients), good immediate graft function and group B (4 patients), immediate graft non-function. Lactate levels rose in the same manner during the preanhepatic and anhepatic stages and peaked after revascularization of the graft. Following reperfusion, however, distinctly different blood lactate profiles were recorded in the two groups of patients. A fall in lactate concentration was recorded in group A patients, whereas a continuous rise occurred in group B patients: the difference becoming significant by the end of surgery (P<-0.05). During the first 48 h following revascularization of the graft, opposite trends in lactate concentration, bile flow, alanine aminotransferase, and prothrombin activity were evident in the two groups of patients: 24 h after reperfusion, lactate levels were below 2 mmol/1 in 47 of 49 patients from group A, while they plateaued above 4 mmol/1 in all patients from group B. Group A patients had lower alanine aminotransferase levels ( P<-0.001), higher prothrombin activity, ( P<-0.01), and greater bile flow (P<-0.02). If validated in larger series, the blood lactate profile, probably more than the absolute level, appears to be a useful indicator of the early recovery of liver metabolic capacities in the immediate postoperative period of orthotopic liver transplantation. A model with two compartments (glucose and insulin) by and a negative feedback has been used to fit the values of blood glucose obtained experimentally by infusing intravenously glucose (0.5 gm. min−1) or insulin (0.055 U. min−1) for about 250 minutes. Glucose was infused to normal ambulatory or hospitalized subjects and to diabetic patients; insulin was infused to normal subjects. The model employed fits the data of all the groups. The SAAM program and a digital computer (IBM 7094) were used. The model cannot be solved completely due to the number of degrees of freedom of the system but values related to the transfer constants of the model can be computed, as the damping constant a, the natural frequency w, the natural period T and the free frequency b of the system, which shows damped oscillations in normal subjects; in diabetic patients the system passes through states of overdamping until no oscillations can be seen in the more severe cases. The effect of ten days of treatment with prednisone (25 mg. m per day, per os) has been evaluated in a group of normal subjects. Discriminant analysis between normal and diabetic subjects and between glucose and insulin infusion has been carried out by using a BMD program and a digital computer.

Nelson & Samuels(1952), Bondy & Altrock(1953), Morris & Williams(1953), Kassenaar et al.(1954) and Silber & Porter(1954) elaborated methods for the determination of the free 17-hydroxycorticosteroids (17-OH-CS) of the plasma. The separate determination both of the free and the glucuronidate fractions was first achieved by Bongiovanni(1954) submitting the plasma after extraction of the free 17-OH-CS to a dialytic procedure: a similar procedure was recently followed by Brown et al.(1957). These methods employ, as does also the method of Weichselbaum & Margraf(1955), the enzymatic hydrolysis with —-glucuronidase for the assay of the glucuronidate fraction. 44. Performance of the Mammoth Balloon Catheter in Patients with Severe Aortic Valve Stenosis Undergoing Percutaneous Treatment.

Background : Balloon aortic valvuloplasty (BAV) is currently used as pre-treatment for patients undergoing trans-catheter aortic valve replacement (TAVR) as well as a stand-alone option for subjects with significant contraindications to TAVR. Mammoth is a newly available non-compliant balloon catheter (BC) included in the balloon-expandable Myval THV system (Meril Life Sciences Pvt. Ltd., India). As limited data on the performance of this BC are available, we here report the results following its use for BAV as pre-dilatation during TAVR or as a stand-alone procedure. Methods : A retrospective, single-center cohort analysis was performed on patients with severe aortic valve stenosis (AS) treated with the Mammoth BC at IRCCS Ospedale Galeazzi Sant'Ambrogio, Milan, Italy. The primary endpoint was technical success defined as successful Mammoth BC advancement across the AS followed by its full and homogeneous inflation without major complications such as aortic root/left ventricular outflow tract injury and/or stroke. Results : A total of 121 patients were treated by BAV with Mammoth BC during the study period. Among these, 105 patients underwent BAV pre-dilatation before TAVR while 16 patients underwent a stand-alone BAV procedure. Mammoth BC was delivered and successfully inflated at the target site in all of the 121 cases without BC-related complications (100% technical success). However, in the BAV "stand-alone group", three patients required two different balloon sizes while in nine patients multiple rounds (two to three) of balloon inflation were needed to significantly lower the transvalvular gradient. No cases of aortic root injury or massive aortic regurgitation due to Mammoth BC-related aortic leaflet injury were reported while one major stroke occurred late after TAVR. No intra-procedural deaths occurred nor bleeding (BARC 3-4) or major vascular complication. Conclusions : Mammoth BC use in patients with severe AS proved safe and effective, either before TAVR or as a stand-alone procedure, expanding the range of available tools for structural operators.

45. Veno-arterial extracorporeal membrane oxygenation supported transcatheter aortic valve implantation in a high-risk COVID-19 patient: a comprehensive case report.

Background: The sudden onset of heart failure in high-risk transcatheter aortic valve implantation (TAVI) candidates poses significant challenges, necessitating meticulous planning and consideration of mechanical circulatory support options. Nevertheless, existing data on the efficacy and safety of mechanical circulatory support in this context are limited, along with criteria for patient selection., Case Summary: An 87-year-old patient, with severe low-flow low-gradient aortic stenosis, presented with acute heart failure and concurrent COVID-19 pneumonia. Despite initial conservative management, her clinical condition deteriorated, requiring inotropic support. The decision was made to perform a rescue TAVI procedure with veno-arterial extracorporeal membrane oxygenation (ECMO) support. The patient underwent successful TAVI while managing complications, including cardiac arrest, with haemodynamic support from veno-arterial ECMO. Post-procedure, the patient showed improved cardiac function and was discharged in stable condition., Discussion: This case underscores the significance of strategic patient selection, proactive haemodynamic management, and the judicious use of veno-arterial ECMO in high-risk TAVI, particularly in complex scenarios involving acute heart failure and respiratory insufficiency, exacerbated by COVID-19. 46. SURgical vs. PERcutaneous ACCESS in Transfemoral Transcatheter Aortic Valve Implantation (SU-PER-ACCESS Study).

Background: The transfemoral (TF) approach is the most common route in TAVI, but it is still associated with a risk of bleeding and vascular complications. The aim of this study was to compare the clinical outcomes between surgical cut-down (SC) and percutaneous (PC) approach. (2) Methods: Between January 2018 and June 2022, 774 patients underwent a transfemoral TAVI procedure. After propensity matching, 323 patients underwent TAVI in each group. (3) Results: In the matched population, 15 patients (4.6%) in the SC group vs. 34 patients in the PC group (11%) experienced minor vascular complications ( p = 0.02), while no difference for major vascular complication (1.5% vs. 1.9%) were reported. The rate of minor bleeding events was higher in the percutaneous group (11% vs. 3.1%, p <.001). The SC group experienced a higher rate of non-vascular-related access complications (minor 8% vs. 1.2%; major 2.2% vs. 1.2%; p < 0.001). (4) Conclusions: SC for TF-TAVI did not alter the mortality rate at 30 days and was associated with reduced minor vascular complication and bleeding. PC showed a lower rate of non-vascular-related access complications and a lower length of stay. The specific approach should be tailored to the patient's clinical characteristics.

47. Percutaneous Coronary Intervention before or after Transcatheter Aortic Valve Replacement: A Systematic Review and Meta-Analysis Involving 1531 Patients.

- Abstract
Background: The optimal timing to perform percutaneous coronary interventions (PCIs) in patients undergoing transcatheter aortic valve replacement (TAVR) is not well established. In this meta-analysis, we aimed to compare the outcomes of patients undergoing PCI before versus after TAVR. Methods: A comprehensive literature search was performed including Medline, Embase, and Cochrane electronic databases up to 5 April 2024 for studies that compared PCI before and after TAVR reporting at least one clinical outcome of interest (PROSPERO ID: CRD42023470417). The analyzed outcomes were mortality, stroke, and myocardial infarction (MI) at follow-up. Results: A total of 3 studies involving 1531 patients (pre-TAVR PCI n = 1240; post-TAVR PCI n = 291) were included in this meta-analysis following our inclusion criteria. Mortality was higher in the pre-TAVR PCI group (OR: 2.48; 95% CI: 1.19-5.20; p = 0.02). No differences were found between PCI before and after TAVR for the risk of stroke (OR: 3.58; 95% CI: 0.70-18.15; p = 0.12) and MI (OR: 0.66; 95% CI: 0.30-1.42; p = 0.29). Conclusions: This meta-analysis showed in patients with stable CAD undergoing TAVR that PCI after TAVR is associated with lower mortality compared with PCI before TAVR.

48. Surgical Aortic Valve Replacement and Renal Dysfunction: From Acute Kidney Injury to Chronic Disease.

- Abstract
Background: Surgical aortic valve replacement (SAVR) is often complicated by acute kidney injury (AKI). Identifying patients at risk of AKI is important to start nephroprotective strategies or renal replacement therapy (RRT). This study investigated the incidence and risk factors of post-operative AKI in SAVR patients. Chronic kidney disease (CKD) developed in the post-cardiac-surgery follow-up period was also assessed. Methods : A total of 462 SAVR patients were retrospectively enrolled. The primary endpoint was the occurrence rate of AKI after surgery. Kidney recovery, during two planned outpatient clinic nephrological visits within 12 months after the surgery, was assessed. Results : A total of 76 patients experienced an AKI event. A Kaplan-Meier analysis revealed that subjects with CKD stage IV had a time to progression of 2.7 days, compared to patients with stages I-II, who were characterized by the slowest progression time, >11.2 days. A Cox regression indicated that CKD stages predicted a higher risk of AKI independently of other variables. During their ICU stay, 23 patients died, representing 5% of the population, most of them requiring RRT during their ICU stay. A severe CKD before the surgery was closely related to perioperative mortality. During the follow-up period, 21 patients with AKI worsened their CKD stage. Conclusions : AKI represents a common complication for SAVR patients in the early post-operative period, prolonging their ICU stay, with negative effects on survival, especially if RRT was required. Pre-operative CKD >3 stage is an independent risk factor for AKI in patients undergoing SAVR.

49. Left Ventricular Mass Index Predicts Renal Function Decline in Patients with Chronic Kidney Disease.

- Abstract
Background and Objectives : Several studies revealed a relation between abnormal cardiac remodeling and glomerular filtration rate (GFR) decline, but there are limited data regarding echocardiographic changes in chronic kidney disease (CKD). This study evaluated the abnormal cardiac structures characterizing patients with CKD, assessing the independent association between echocardiographic parameters and the risk of decline in renal function. Materials and Methods: In total, 160 patients with CKD were studied. All patients underwent an echocardiographic exam and 99mTc-DTPA renal scintigraphy to measure the GFR. After the baseline assessments, patients were followed prospectively for 12 months, or until the endpoint achievement, defined as a worsening in renal function (doubling of baseline serum creatinine, GFR decline ≥25%, the start of dialysis). Results: Patients with GFR values of 34.8 ± 15 mL/min, identifying stages III-IV of CKD, were associated with high levels of left ventricular mass index (LVMi) (101.9 ± 12.2 g/m
2 ), which was related to proteinuria, systolic blood pressure, and pulmonary artery systolic pressure in a multiple regression model. During the observational period, 26% of patients reached the endpoint. Regression analysis revealed LVMi as a predictor of change in renal function after adjusting for kidney and cardiac risk factors. Multiple Cox regression indicated that an increase in LVMi was associated with a 12% increased risk of kidney disease progression (HR: 1.12; 95% CI: 1.04-1.16; p = 0.001). Conclusions: In patients with CKD, high LVMi represents an independent predictor of the progressive decline of the renal function, until the start of renal replacement therapy. Echocardiography can help identify patients at high risk for renal disease worsening in patients with CKD independently of clinical cardiac involvement.- Published

50. Replacement of Ascending Aortic Aneurysm with Anomalous Origin of the Right Coronary Artery: Multidisciplinary Imaging for the Diagnosis.

This case report presents a rare scenario involving a congenital anomaly of the right coronary artery's (RCA) origin in association with an ascending aortic aneurysm. While both anomalies are individually recognized in the literature, their coexistence and potential interplay remain understudied. The aim of this report is to emphasize the challenges and implications associated with such a combination. A 78-year-old male patient with an enlarged ascending aortic aneurysm necessitating surgical intervention was found to have an anomalous origin of the RCA during preoperative coronary angiography, confirmed by computed tomography scan. Transesophageal echocardiography further elucidated the coronary abnormality. Intraoperatively, successful aortic replacement was performed, and careful repositioning of the anomalous right coronary ostium was achieved. This case raises important considerations regarding the potential complications arising from coronary anomalies and their impact on the surgical management of ascending aortic aneurysms. The rarity of this combination limits our understanding of their association, making a multidisciplinary approach crucial for optimal patient care. Further research and comprehensive evaluation of similar cases are necessary to better understand the relationship between coronary anomalies and ascending aortic aneurysms.
