Abstract: OBJECTIVES: To evaluate the association between the pre-extubation sum of eye and motor components of the Glasgow Coma Score (GCS-EM) and odds of extubation failure in patients with acute brain injury being liberated from mechanical ventilation. DESIGN: Secondary analysis of a prospective, multicenter observational study (ClinicalTrials.gov identifier NCT03400904). SETTING: Sixty-three hospital sites worldwide, with patient recruitment from January 2018 to November 2020. PATIENTS: One thousand one hundred fifty-two critically ill patients with acute brain injury, with a median age of 54 years, of whom 783 (68.0%) were male, 559 (48.5%) had traumatic brain injury, and 905 (78.6%) had a GCS-EM greater than 8 before extubation (scores range from 2 to 10). INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: GCS-EM was computed in intubated patients on the day of extubation. The main outcome was extubation failure, defined as unplanned reintubation within 5 days of extubation. Analyses used multilevel logistic regression with adjustment for patient characteristics and a random intercept for hospital site. In the primary analysis, GCS-EM was not associated with extubation failure (odds ratio, 1.07 per additional point; 95% CI, 0.87-1.31). Findings were consistent in sensitivity analyses that: 1) used different adjustment covariates, 2) included a verbal estimate to derive an overall GCS, 3) accounted for missing data, 4) considered a 2-day time interval to define extubation failure, 5) accounted for competing risks, and 6) used a propensity score-based model. There was no association between GCS-EM and extubation outcome in subgroups defined by brain injury diagnosis or age. CONCLUSIONS: In this large, contemporary, multicenter cohort of patients with acute brain injury, we found no association between the GCS-EM and odds of extubation failure. Abstract: Purpose: The use of arterial partial pressure of carbon dioxide (PaCO2) as a target intervention to manage elevated intracranial pressure (ICP) and its effect on clinical outcomes remain unclear. We aimed to describe targets for PaCO2 in acute brain injured (ABI) patients and assess the occurrence of abnormal PaCO2 values during the first week in the intensive care unit (ICU). The secondary aim was to assess the association of PaCO2 with in-hospital mortality. Methods: We carried out a secondary analysis of a multicenter prospective observational study involving adult invasively ventilated patients with traumatic brain injury (TBI), subarachnoid hemorrhage (SAH), intracranial hemorrhage (ICH), or ischemic stroke (IS). PaCO2 was collected on day 1, 3, and 7 from ICU admission. Normocapnia was defined as PaCO2 > 35 and to 45 mmHg; mild hypocapnia as 32–35 mmHg; severe hypocapnia as 26–31 mmHg, forced hypocapnia as < 26 mmHg, and hypercapnia as > 45 mmHg. Results: 1476 patients (65.9% male, mean age 52 ± 18 years) were included. On ICU admission, 804 (54.5%) patients were normocapnic (incidence 1.37 episodes per person/day during ICU stay), and 125 (8.5%) and 334 (22.6%) were mild or severe hypocapnic (0.52 and 0.25 episodes/day). Forced hypocapnia and hypercapnia were used in 40 (2.7%) and 173 (11.7%) patients. PaCO2 had a U-shape relationship with in-hospital mortality with only severe hypocapnia and hypercapnia being associated with increased probability of in-hospital mortality (omnibus p value = 0.0009). Important differences were observed across different subgroups of ABI patients. Conclusions: Normocapnia and mild hypocapnia are common in ABI patients and do not affect patients' outcome. Extreme derangements of PaCO2 values were significantly associated with increased in-hospital mortality.

Abstract: Background: There is insufficient evidence to guide ventilatory targets in acute brain injury (ABI). Recent studies have shown associations between mechanical power (MP) and mortality in critical care populations. We aimed to describe MP in ventilated patients with ABI, and evaluate associations between MP and clinical outcomes. Methods: In this preplanned, secondary analysis of a prospective, multi-center, observational cohort study (ENIO, NCT03400904), we included adult patients with ABI (Glasgow Coma Scale ≤ 12 before intubation) who required mechanical ventilation (MV) ≥ 24 h. Using multivariable log binomial regressions, we separately assessed associations between MP on hospital day (HD)1, HD3, HD7 and clinical outcomes: hospital mortality, need for reintubation, tracheostomy placement, and development of acute respiratory distress syndrome (ARDS). Results: We included 1217 patients (mean age 51.2 years [SD 18.1], 66% male, mean body mass index [BMI] 26.3 [SD 5.18]) hospitalized at 62 intensive care units in 18 countries. Hospital mortality was 11% (n = 139), 44% (n = 536) were extubated by HD7 of which 20% (107/536) required reintubation, 28% (n = 340) underwent tracheostomy placement, and 9% (n = 114) developed ARDS. The median MP on HD1, HD3, and HD7 was 11.9 J/min [IQR 9.2–15.1], 13 J/min [IQR 10–17], and 14 J/min [IQR 11–20], respectively. MP was overall higher in patients with ARDS, especially those with higher ARDS severity. After controlling for same-day pressure of arterial oxygen/fraction of inspired oxygen (P/F ratio), BMI, and neurological severity, MP at HD1, HD3, and HD7 was independently associated with hospital mortality, reintubation and tracheostomy placement. The adjusted relative risk (aRR) was greater at higher MP, and strongest for: mortality on HD1 (compared to the HD1 median MP 11.9 J/min, aRR at 17 J/min was 1.22, 95% CI 1.14–1.30) and HD3 (1.38, 95% CI 1.23–1.53), reintubation on HD1 (1.64; 95% CI 1.57–1.72), and tracheostomy on HD7 (1

Abstract: Background: In acute brain injury (ABI), the effects of hypoxemia as a potential cause of secondary brain damage and poor outcome are well documented, whereas the impact of hyperoxemia is unclear. The primary aim of this study was to assess the episodes of hypoxemia and hyperoxemia in patients with ABI during the intensive care unit (ICU) stay and to determine their association with in-hospital mortality. The secondary aim was to identify the optimal thresholds of arterial partial pressure of oxygen (PaO2) predicting in-hospital mortality. Methods: We conducted a secondary analysis of a prospective multicenter observational cohort study. Adult patients with ABI (traumatic brain injury, subarachnoid aneurysmal hemorrhage, intracranial hemorrhage, ischemic stroke) with available data on PaO2 during the ICU stay were included. Hypoxemia was defined as PaO2 < 80 mm Hg, normoxemia was defined as PaO2 between 80 and 120 mm Hg, mild/moderate hyperoxemia was defined as PaO2 between 121 and 299 mm Hg, and severe hyperoxemia was defined as PaO2 levels ≥ 300 mm Hg. Results: A total of 1,407 patients were included in this study. The mean age was 52 (±18) years, and 929 (66%) were male. Over the ICU stay, the fractions of patients in the study cohort who had at least one episode of hypoxemia, mild/moderate hyperoxemia, and severe hyperoxemia were 31.3%, 53.0%, and 1.7%, respectively. PaO2 values below 92 mm Hg and above 156 mm Hg were associated with an increased probability of in-hospital mortality. Differences were observed among subgroups of patients with ABI, with consistent effects only seen in patients without traumatic brain injury. Conclusions: In patients with ABI, hypoxemia and mild/moderate hyperoxemia were relatively frequent. Hypoxemia and hyperoxemia during ICU stay may influence in-hospital mortality. However, the small number of oxygen values collected represents a major limitation of the study.

Abstract: The goal of sustainable coffee production requires multiple functions from agroforestry systems. Many are difficult to quantify and data are lacking, hampering the choice of shade tree species and agronomic management. Process-based modelling may help quantify ecosystem services and disservices. We introduce and apply coffee agroforestry model CAF2021 (https://doi.org/10.5281/zenodo.5725277). The model allows for complex systems with up to three shade tree species. It simulates coffee yield, timber and fruit production by shade trees, soil loss in erosion, C-sequestration, N-fixation, -emission and -leaching. To calibrate the model, we used multivariate data from 32 different treatments applied in two long-term coffee agroforestry experiments in Costa Rica and Nicaragua. Without any further calibration, the model was then applied to agroforestry systems on 89 farms in Costa Rica and 79 in Guatemala where yields had been reported previously in farmer interviews. Despite wide variation in environmental and agronomic conditions, the model explained 36% of yield variation in Costa Rica but only 15% in Guatemala. Model analysis quantified trade-offs between yield and other ecosystem services as a function of fertilisation and shading.

Abstract: The aim of this paper is to understand Social Representations of educational quality, in relation to improvement factors, in actors' discourses which appear in written Mass Media within the context of the students' conflict in Chile 2011 from theoretic-methodological analysis of Grounded Theory. Atlas ti v.6.4 was used to make open codification of 29 pieces of news from Chilean written Media, from where 79 codes were created. Through Axial codification, which is an iterative process, 16 categories were built upon the basis of these codes, constituting a preliminary explanatory model about the phenomenon emerged. This phenomenon is defined as the ideas of quality linked to the notions and the aims of education as to improvement strategies. This model is based upon the causal conditions that describe the quality of education as insufficient, and it is related to three key categories: quality guarantees, financing, and assessment. &nbsp, Este trabajo tiene por objetivo comprender las Representaciones Sociales (RS) de calidad educativa en relación con sus factores de mejoramiento, que aparecen en los discursos de los actores en medios escritos en el contexto del conflicto estudiantil en Chile 2011, a partir del análisis teórico-metodológico de la teoría fundamentada. Se utiliza software Atlas ti v.6.4 para realizar codificación abierta sobre un cuerpo de 29 noticias de medios escritos chilenos, en donde emergen 79 códigos, los cuales sirven de fundamento para la codificación axial. Este proceso iterativo constituyó 16 categorías, del cual surge un modelo explicativo preliminar sobre el fenómeno -entendido como las ideas de calidad, asociadas a nociones y fines de la educación y a estrategias de mejoramiento. Este modelo tiene como base las condiciones causales que definen la calidad de la educación como deficitaria y se relaciona con 3 categorías nucleares: Garantías de calidad, Financiamiento y Evaluación. &nbsp, Objetiva-se, a partir de análise teorico-metodológica da teoria fundamentada, compreender as Representares Sociais (RS) de qualidade educativa em relacáo a seus fatores de melhoramento, que aparecem nos discursos dos atores da imprensa escrita no contexto do conflito estudantil no Chile, em 2011 no Chile. Abstract: The PREDICTS project—Projecting Responses of Ecological Diversity In Changing Terrestrial Systems (www.predicts.org.uk)—has collated from published studies a large, reasonably representative database of comparable samples of biodiversity from multiple sites that differ in the nature or intensity of human impacts relating to land use. We have used this evidence base to develop global and regional statistical models of how local biodiversity responds to these measures. We describe and make freely available this 2016 release of the database, containing more than 3.2 million records sampled at over 26,000 locations and representing over 47,000 species. We outline how the database can help in answering a range of questions in ecology and conservation biology. To our knowledge, this is the largest and most geographically and taxonomically representative database of spatial comparisons of biodiversity that has been collated to date; it will be useful to researchers and international efforts wishing to model and understand the global status of biodiversity.

Abstract: Este trabajo tiene por objetivo comprender las Representaciones Sociales (RS) de calidad educativa en relación con sus factores de mejoramiento, que aparecen en los discursos de los actores en medios escritos en el contexto del conflicto estudiantil en Chile 2011, a partir del análisis teórico-metodológico de la teoría fundamentada. Se utiliza software Atlas ti v.6.4 para realizar codificación abierta sobre un cuerpo de 29 noticias de medios escritos chilenos, en donde emergen 79 códigos, los cuales sirven de fundamento para la codificación axial. Este proceso iterativo constituyó 16 categorías, del cual surge un modelo explicativo preliminar sobre el fenómeno -entendido como las ideas de calidad, asociadas a nociones y fines de la educación y a estrategias de mejoramiento. Este modelo tiene como base las condiciones causales que definen la calidad de la educación como deficitaria y se relaciona con 3 categorías nucleares: Garantías de calidad, Financiamiento y Evaluación.

Abstract: Background: Monitoring urine protein:creatinine ratios (UPC) in dogs with protein‐losing nephropathy (PLN) is challenging because of day‐to‐day variation in UPC results. Hypothesis/Objectives: Determine whether single, averaged, or pooled samples from PLN dogs receiving medical treatment yield comparable UPCs, regardless of degree of proteinuria. Animals: Twenty‐five client‐owned PLN dogs receiving medical treatment. Methods: UPC ratios were prospectively measured in each dog utilizing 3 methods: single in‐hospital sample (day 3), average sample (days 1–3), and pooled sample (equal pooling of urine from days 1–3). Bland‐Altman analysis was performed to evaluate agreement between methods for all dogs, as well as in subgroups of dogs (UPC ≤4 or UPC >4). Results: For all dogs, Bland‐Altman log‐transformed 95% limits of agreement were −0.07–0.18 (single versus pooled UPC), −0.06–0.16 (single versus average UPC), and −0.06–0.04 (pooled versus average UPC). For dogs with UPC ≤4, Bland‐Altman 95% limits of agreement were −0.42–0.82 (single versus pooled UPC), −0.38–0.76 (single versus average UPC), and −0.27–0.25 (pooled versus average UPC). For dogs with UPC >4, Bland‐Altman 95% limits of agreement were −0.17–2.4 (single versus pooled UPC), −0.40–2.2 (single versus average UPC), and −0.85–0.43 (pooled versus average UPC). Conclusions and Clinical Importance: UPC ratios from all methods were comparable in PLN dogs receiving medical treatment. In PLN dogs with UPC >4, more variability between methods exists likely because of higher in‐hospital results, but whether this finding is clinically relevant is unknown. [ABSTRACT FROM AUTHOR]

Abstract: Biodiversity continues to decline in the face of increasing anthropogenic pressures such as habitat destruction, exploitation, pollution and introduction of alien species. Existing global databases of species’ threat status or population time series are dominated by charismatic species. The collation of datasets with broad taxonomic and biogeographic extents, and that support computation of a range of biodiversity indicators, is necessary to enable better understanding of historical declines and to project – and avert – future declines. We describe and assess a new database of more than 1.6 million samples from 78 countries representing over 28,000 species, collated from existing spatial comparisons of local-scale biodiversity exposed to different intensities and types of anthropogenic pressures, from terrestrial sites around the world. The database contains measurements taken in 208 (of 814) ecoregions, 13 (of 14) biomes, 25 (of 35) biodiversity hotspots and 16 (of 17) megadiverse countries. The database contains more than 1% of the total number of all species described, and more than 1% of the described species within many taxonomic groups – including flowering plants, gymnosperms, birds, mammals, reptiles, amphibians, beetles, lepidopterans and hymenopterans. The dataset, which is still being added to, is therefore already considerably larger and more representative than those used by previous quantitative models of biodiversity trends and responses. The database is being assembled as part of the PREDICTS project (Projecting Responses of Ecological Diversity In Changing Terrestrial Systems – www.predicts.org.uk). We make site-level summary data available alongside this article. The full database will be publicly available in 2015.

Abstract: Biodiversity continues to decline in the face of increasing anthropogenic pressures such as habitat destruction, exploitation, pollution and introduction of alien species. Existing global databases of species’ threat status or population time series are dominated by charismatic species. The collation of datasets with broad taxonomic and biogeographic extents, and that support computation ofa range of biodiversity indicators, is necessary to enable better understanding of historical declines and to project – and avert – future declines. We describe and assess a new database of more than 1.6 million samples from 78 countries representing over 28,000 species, collated from existing spatial comparisons of local-scale biodiversity exposed to different intensities and types of anthropogenic pressures, from terrestrial sites around the world. The database containsmeasurements taken in 208 (of 814) ecoregions, 13 (of 14) biomes, 25 (of 35) biodiversity hotspots and 16 (of 17) megadiverse countries. The database contains more than 1% of the total number of all species described, and more than 1% of the described species within many taxonomic groups – including flowering plants, gymnosperms, birds, mammals, reptiles, amphibians, beetles, lepidopterans and hymenopterans. The dataset, which is still being added to, is therefore already considerably larger and more representative than those used by previous quantitative models of biodiversity trends and responses. The database is being assembled as part of the PREDICTS project (Projecting Responses of Ecological Diversity In Changing Terrestrial Systems – www.predicts.org.uk).

Abstract: Background: With the aim of contributing to he Healthy Goal 2010 of reducing significantly the prevalence of childhood obestiy we developed and implemented during 2003 and 2004, a school-based obesity prevention intervention which included nutrition education and the promotion of physical activity. Aim To report the results of the intervention. Material and methods: The sample included 1760 children (1st to 7th grade) from 3 elementary public schools in Casablanca (experimental group) and 671 from a similar school located in Quillota, a neighboring city (control). Primary outcomes were body mass index (BMI) Zscore, the mile and shuttle-run tests and obesity prevalence. We also compared changes in waist circumference and triceps skinfold between both groups. Effectiveness of the intervention was assessed by analyzing separately the group *age* time interaction for the first 3 outcomes (follow-up-baseline), using a mixed model of covariance and by comparing variations in obesity prevalence between both groups. Abstract: Se diseñó, implementó durante tres años (de 2002 a 2004) y evaluó una intervención en alimentación/nutrición y actividad física en escolares de educación básica de Casablanca. El objetivo general fue determinar la evolución en la prevalencia de obesidad en los mismos niños durante los tres años del estudio. La muestra incluyó 1103 escolares que en el 2002 estaban entre 1º a 6º básico en las tres escuelas municipales de la ciudad y que contaban con mediciones durante el período completo. Los componentes más importantes de la intervención incluyeron la aplicación de programa educativo FAO/INTA, charlas a los padres, aumento de las clases de educación física y un programa motivacional de actividad física. Los resultados muestran que la prevalencia de obesidad fue muy alta al inicio y que disminuyó de 21,6% a 12,2% en hombres y de 19,4% a 8,7% en mujeres. Este mismo análisis según categoría de edad al inicio y sexo, mostró una disminución a un tercio de la prevalencia original en mujeres que tuvieron entre 8 y 10 años y hombres entre 10 y 12 años al comienzo del estudio. Abstract: Background: The Chilean Ministry of Health has developed a health promotion program called «Vida Chile», whose aim is to target risk factors for chronic diseases. To achieve this, school-based initiatives in nutrition and physical activity have been implemented. The evaluation of these initiatives includes a baseline assessment of these two variables. Aim: Baseline assessment of the nutritional status and aerobic capacity of elementary school children (1st to 8th grade). Material and methods: Children from seven public schools were studied. Weight, height, triceps skinfold thickness and waist circumference were measured. To test the aerobic capacity, the 20-m Shuttle Run Test (Navette) was utilized. Results: The assessment included 4,271 children, mean age was 10.2 years. The percentile of body mass index, as compared to the CDC 2000 Reference was 67.8±26.7 and 68±26.7 for boys and girls, respectively. The prevalence of obesity among children from 6 to 8 years, was 20.3 and 17.7% for boys and girls, respectively. Approximately 30% of males and 15% of females with normal weight had a good aerobic resistance, compared to 15 and 4% of their overweight counterparts. Aerobic capacity decreased with increasing age. Conclusions: There is a high prevalence of obesity and a deficient level of aerobic capacity among children attending public schools in Chile (Rev Med Chile 2004; 132: 1395-1402).

Abstract: In this paper, the equilibrium and adsorption kinetics of Cr(VI), Cd(II) and Ni(II) onto Chitosan and Cu-chitosan was studied. Metal ion solutions at different initial concentration were put in contact with chitosan and Cu-chitosan and the mixture was agitated by 3 – 4 h. Afterwards, the samples were analyzed by atomic absorption spectroscopy. The experimental data of adsorption equilibrium were evaluated by application of Langmuir and Freundlich isotherms; while the kinetics experimental data were evaluated using kinetic models of pseudo-first order and pseudo-second order. The results showed that the metallic ions were effectively adsorbed by the chitosan (Cd and Ni) and Cuchitosan (Cr). Besides, it was found that the experimental data of adsorption equilibrium of Cr(VI) and Ni(II) fit the Langmuir model, while the equilibrium adsorption data of Cd(II) were better fitted by Freundlich model. The maximum adsorption capacity of chromium (29.7 mg/g Cu-chitosan), cadmium (102.0 mg/g chitosan) and nickel (83.31 mg/g chitosan) was determined using the Langmuir model. The results of adsorption kinetics of the metallic ions showed that the experimental data were better adjusted by pseudo-second order model. Therefore, the ratelimiting step is the adsorption reaction and not mass transfer processes., En este trabajo, se estudió el equilibrio y la cinética de adsorción de Cd(II), Ni(II) en quitosano y Cr(VI) en quitosano modificado con cobre. Para ello, soluciones de iones metálicos a diferentes concentraciones, fueron puestas en contacto con el adsorbente y se agitaron por un período de 3 – 4 h. Al final del proceso, las muestras fueron analizadas en un espectrómetro de absorción atómica. Los datos experimentales del equilibrio de adsorción de los iones metálicos fueron evaluados aplicando las isotermas de Langmuir y Freundlich; mientras los datos cinéticos fueron evaluados utilizando los modelos cinéticos de pseudo-primer orden y pseudo-segundo orden. Los resultados del proceso demostraron que los iones fueron eficazmente adsorbidos por el quitosano (Cd y Ni) y por el Cuquitosano (Cr). Además, se comprobó que los datos experimentales del equilibrio de adsorción de Cr(VI) y Ni(II) se ajustan al modelo de Langmuir; mientras que los datos experimentales del Cd(II) fueron mejor ajustados por el modelo de Freundlich. Mediante el uso de la isoterma de Langmuir se determinó la capacidad máxima de adsorción de cromo (29.7 mg/g Cu-quitosano), cadmio (102.0 mg/g quitosano) y níquel (83.31 mg/g quitosano). Los resultados de la cinética de adsorción de los iones metálicos mostraron que los datos experimentales fueron mejor ajustados por el modelo de pseudo-segundo orden; es decir, el paso limitante en la velocidad es la reacción de adsorción y no la transferencia de masa.

The system being used is fast and allows automatic generation of reports, communication with other systems, auditability, control, and integration with current technologies and processes. Grade control and reconciliation will move from manual to semiautomatic and automatic processes, maintaining the same flowchart of information, but reducing the times consumed in each stage of work. Information from blast holes is imported to the mining software and interpolation carried out to update of the block models for the short term. Extraction geometries are designed and a semiautomatic classification of the material for extraction is realised. Grade control reports are generated and information about resources for the areas of mine operation, topography and control of equipment are exported and an automatic form generated.
