Abstract

We demonstrate the creation of vortices in a macroscopically occupied polariton state formed in a semiconductor microcavity. A weak external laser beam carrying orbital angular momentum (OAM) is used to imprint a vortex on the condensate arising from the polariton optical parametric oscillator (OPO). The vortex core radius is found to decrease with increasing pump power, and is determined by polariton-polariton interactions. As a result of OAM conservation in the parametric scattering process, the excitation consists of a vortex in the signal and a corresponding anti-vortex in the idler of the OPO. Abstract

Polariton condensates are investigated in periodical potentials created by surface acoustic waves using both resonant and non-resonant optical excitation. Under resonant pumping condensates are formed due to polariton parametric scattering from the pump. In this case the single particle dispersion in the presence of the condensate shows a strong reduction of the energy gap arising from the acoustic modulation, indicating efficient screening of the surface acoustic wave potential by spatial modulation of the polariton density. The observed results are in good agreement with a model based on generalised Gross-Pitaveskii equations with account taken of the spatial dependence of the exciton energy landscape. In the case of incoherent, non-resonant pumping coexisting non-equilibrium condensates with s- and p- type wavefunctions are observed, which have different energies, symmetry and spatial coherence. The energy splitting between these condensate states is also reduced with respect to the gap of the one particle spectrum below threshold, but the screening effect is less pronounced than in the case of resonantly pumped system due to weaker modulation of the pump state.

Abstract

Background: Data regarding outcome of Coronavirus disease 2019 (COVID-19) in vaccinated patients with autoimmune hepatitis (AIH) are lacking. We evaluated the outcome of COVID-19 in AIH patients who received at least one dose of Pfizer- BioNTech (BNT162b2), Moderna (mRNA-1273) or AstraZeneca (ChAdOx1-S) vaccine. Patients and methods: We performed a retrospective study on AIH patients with COVID-19. The outcomes of AIH patients who had acute respiratory syndrome coronavirus 2 (SARS-CoV-2) breakthrough infection after at least one dose of COVID-19 vaccine were compared to unvaccinated patients with AIH. COVID-19 outcome was classified according to clinical state during the disease course as: (i) no hospitalization, (ii) hospitalization without oxygen supplementation, (iii) hospitalization with oxygen supplementation by nasal cannula or mask, (iv) intensive care unit (ICU) admission with non-invasive mechanical ventilation, (v) ICU admission with invasive mechanical ventilation or (vi) death, and data was analyzed using ordinal logistic regression. Results: We included 413 (258 unvaccinated and 155 vaccinated) patients (81%, female) with a median age of 52 (range: 17–85) years at COVID-19 diagnosis. The rates of hospitalization were (36.4% vs. 14.2%), need for any supplemental oxygen (29.5% vs. 9%) and mortality (7% vs. 0.6%) in unvaccinated and vaccinated AIH patients with COVID-19. Having received at least one dose of SARS-CoV-2 vaccine was associated with a significantly lower risk of worse COVID-19 severity, after adjusting for age, sex, comorbidities and presence of cirrhosis (adjusted odds ratio [aOR] 0.18, 95% confidence interval [CI], 0.10–0.31). Overall, vaccination against SARS-CoV-2 was associated with a significantly lower risk of mortality from COVID-19 (aOR 0.20, 95% CI 0.11–0.35). Conclusions: SARS-CoV-2 vaccination significantly reduced the risk of COVID-19 severity and mortality in patients with AIH.

Abstract

The principal limitations of the terms NAFLD and NASH are the reliance on exclusionary confounder terms and the use of potentially stigmatising language. This study set out to determine if content experts and patient advocates were in favour of a change in nomenclature and/or definition. A modified Delphi process was led by three large pan-national liver associations. The consensus was defined a priori as a supermajority (67%) vote. An independent committee of experts external to the nomenclature process made the final recommendation on the acronym and its diagnostic criteria. A total of 236 panellists from 56 countries participated in 4 online surveys and 2 hybrid meetings. Response rates across the 4 survey rounds were 87%, 83%, 83%, and 78%, respectively. Seventy-four percent of respondents felt that the current nomenclature was sufficiently flawed to consider a name change. The terms “nonalcoholic” and “fatty” were felt to be stigmatising by 61% and 66% of respondents, respectively. Steatotic liver disease was chosen as an overarching term to encompass the various aetiologies of steatosis. The term steatohepatitis was felt to be an important pathophysiological concept that should be retained. The name chosen to replace NAFLD was metabolic dysfunction–associated steatotic liver disease (MASLD). There was consensus to change the definition to include the presence of at least 1 of 5 cardiometabolic risk factors. Those with no metabolic parameters and no known cause were deemed to have cryptogenic steatotic liver disease. A new category, outside pure metabolic dysfunction–associated steatotic liver disease, termed metabolic and alcohol related/associated liver disease (MetALD), was selected to describe those with metabolic dysfunction–associated steatotic liver disease, who consume greater amounts of alcohol per week (140–350 g/wk and 210–420 g/wk for females and males, respectively). Abstract

SummaryBackground Small intestinal bacterial overgrowth (SIBO) is a heterogeneous syndrome, characterised by an increased number and/or abnormal type of bacteria in the small bowel. Over the past decades, rifaximin has gained popularity for this indication despite its use is not evidence based. Aim To perform a systematic review and meta-analysis to summarise evidence about the efficacy and safety of rifaximin to eradicate SIBO in adult patients. Methods MEDLINE, EMBASE, CCRCT, Scopus and Web of Science were searched from inception to March 16, 2015 for RCTs and observational studies. Furthermore, abstract books of major European, American and Asian gastroenterological meetings were also examined. Results Thirty-two studies involving 1331 patients were included. The overall eradication rate according to intention-to-treat analysis was 70.8% (95% CI: 61.4–78.2; I2 = 89.4%) and to per protocol analysis 72.9% (95% CI: 65.5–79.8; I2 = 87.5%). Meta-regression identified three covariates (drug dose, study design and co-therapy) independently associated with an increased eradication rate. The overall rate of adverse events was 4.6% (95% CI: 2.3–7.5; I2 = 63.6%). In the subset of studies (n= 10) allowing the analysis, improvement or resolution of symptoms in patients with eradicated SIBO was found to be 67.7% (95% CI: 44.7–86.9; I2 = 91.3%). Conclusions Rifaximin treatment seems to be effective and safe for the treatment of SIBO. However, the quality of the available studies is generally poor. Well-designed RCTs are needed to substantiate these findings and to establish the optimal regimen.

Abstract

Patients admitted with acute variceal bleeding (AVB) and Child-Pugh C score (CP-C) or Child-Pugh B plus active bleeding at endoscopy (CP-B+AB) are at high risk for treatment failure, rebleeding, and mortality. A preemptive transjugular intrahepatic portosystemic shunt (p-TIPS) has been shown to improve survival in these patients, but its use in clinical practice has been challenged and not routinely incorporated. The present study aimed to further validate the role of preemptive TIPS in a large number of high-risk patients. This multicenter, international, observational study included 671 patients from 34 centers admitted for AVB and high risk of treatment failure. Patients were managed according to current guidelines, and use of drugs and endoscopic therapy (D+E) or p-TIPS was based on individual center policy. p-TIPS in the setting of AVB is associated with a lower mortality in CP-C patients compared with D+E (1 year mortality 22% vs. 47% in D+E group; P = 0.002). Mortality rate in CP-B+AB patients was low, and p-TIPS did not improve it. In CP-C and CP-B+AB patients, p-TIPS reduced treatment failure and rebleeding (1-year cumulative incidence function probability of remaining free of the composite endpoint: 92% vs. 74% in the D+E group; P = 0.017) and development of de novo or worsening of previous ascites without increasing rates of hepatic encephalopathy. Conclusion: p-TIPS must be the treatment of choice in CP-C patients with AVB. Abstract

A pervading anxiety marks our society. Men and women find themselves disturbed by permanent and rapid changes. The result can be disenchantment, abulia, or unrestraint. Often the result is an overinsistence on rights, with no acknowledgement of the duties that accompany them. Some people opt for personal happiness, placing a high value on pleasure in order to “be themselves”. Others choose to escape, to be self-complacent, to alienate themselves from the world. Indeed, some consider themselves incapable of finding integration with the world.A look back at history, though, shows that this crisis is not new and that philosophers, scientists, and educators have already formulated ideas and proposals on how to face such times. Their thoughts form a common body of ideas that act as a key bone or an axic which is a global, total, holistic paradigm. What they have said in response to their circumstance is a broad, integrated and unifying answer for our own age. Education must pick up these ideas, taking advantage of the current educational reform. &nbsp, La modernidad muestra signos de una sociedad “ansiógena”; al ser humano le resulta difícil adherirse al cambio, a un progreso permanente y vertiginoso, cuya rapidez lo perturba y desestabiliza. Emerge el desencanto, la abulia o el desenfreno; la exaltación de los derechos y, a la vez, una ingenua justificación que soslaya deberes inseparables a éstos. Opciones de vida que trasuntan, por una parte, una búsqueda de felicidad individual, con una valoración del placer, en un intento de ser más que nada “él mismo”, o bien el escape, la autocomplacencia, la enajenación, verse a sí mismo como ajeno al mundo, incapaz de integrarse a él, desagregado.Una mirada retrospectiva de la humanidad permite constatar, por un lado, que esta situación de crisis no es nueva; pero, por otro, rastrear y rescatar los planteamientos, ideas y propuestas de filósofos, científicos y educadores frente a ello. Se observa, en este proceso, un conjunto de ideas vertebradoras, axiales y fundantes, que convergen en un paradigma global, totalizante, holístico. Una respuesta amplia, integral y unificadora, para fenómenos de iguales características. La educación debe atender y acoger estas ideas, capitalizando la oportunidad que le ofrece la reforma educacional en marcha.

43. Clinical and epidemiological characterization of psoriasis and psoriatic arthritis in a multidisciplinary assessment model

Abstract

Background: Presentation and clinical course in Psoriasis are heterogeneous. A multidisciplinary assessment model was designed, under a procedure manual for five specialties related to disease manifestations: dermatology, rheumatology, internal medicine, nutrition and psychiatry, to evaluate at the same time and place, patients with psoriasis, to determine the frequency of psoriatic arthritis and other comorbidities in patients with psoriasis, as well as performing a clinical and epidemiological characterization of psoriasis and psoriatic arthritis in patients seeking medical consultation in a multidisciplinary assessment model in Guadalajara Jalisco, Mexico. Methods and Findings: A prospective study that included patients that attended a multidisciplinary psoriasis clinic from August 2016 to February 2018. Patient triage and evaluation of psoriasis or psoriatic arthritis was performed either by a rheumatologist or a dermatologist at an appointment prior to enrollment. If a clinical, radiological or histological criterion was met, patient was scheduled to an appointment to the multidisciplinary clinic. Fifty three patients were included. Median age was 42 years old and 62.3% were female. Family history of psoriasis was found in 26.4% of population and 19.4% had spondyloarthropathy family history. Most common comorbidities found were overweight and obesity 66%, metabolic syndrome in 35.8%, diabetes mellitus in 15.1%, dyslipidemia in 22.6%, hypertension in 13.1% and fatty liver in 3.8%. Affective assessment was performed and 49.1% met criteria for anxiety. Psoriasis was found in 96.2% and 56.6% in nails. PASI was moderate to severe in 49.1%. Psoriatic arthritis in 58.5% of patients that met CASPAR criteria. Axial disease was present in 51.6%, peripheral joint disease in 71% and in 13.2% both. Enthesitis was present in 54.8% and 16.1% had dactylitis. DAS28 was performed and showed 45.2% with severe activity. Total number of subjects with peripheral disease did not met criteria for remission measured by DAS28 or MDA. Conclusions: We found that nearly half of the patients have moderate to severe form of psoriasis. The number of patients with psoriatic arthritis was higher than expected with both axial and peripheral manifestations. We have found a high proportion of patients with comorbidities such as metabolic syndrome, diabetes mellitus, hypertension, dyslipidemia and obesity. We observed a high frequency of psychiatric illness like depression and anxiety. The limitations of our study are the small sample to highlight if comorbidities had an impact on the severity of skin and joint disease. This multidisciplinary model has identified psoriasis as a disease with an unpredictable course, which requires several evaluations by a multidisciplinary model, with a group of experts beyond Dermatologist and Rheumatologist.

Abstract

Psoriatic Arthritis (PsA) is considered part of the spondyloarthritis group, which is present in up to 42% of individuals with psoriasis and up to 15% of patients with psoriasis may have undiagnosed PsA. Psoriatic arthritis Mutilans is a rare but very aggressive type of advanced joint disease in patients with psoriasis. This report describes the case of a 67 years old patient with family history of psoriasis and undiagnosed psoriatic arthritis with nail lesions for more than 16 years, arthritis for 15 years and severe disease during the last 3 years with disability and bedridden. She had serious joint complications of the disease, as well as severe malnutrition. A missdiagnosis of rheumatoid arthritis was given during 15 years, HLAB27 was positive and radiography of hands and feet showed classic findings of psoriatic arthritis mutilans. We present this case to show the severity of the disease with rapid progression and multiple additional joint complications.
