Abstract

To verify whether it is possible, in people with diabetes mellitus (DM) considered at very high cardiovascular (CV) risk, stratify this risk better and identify significant modifiable risk factor (including lifestyle habits) to help patients and clinicians improve CV prevention. People with DM and microvascular diseases or one or more CV risk factors (hypertension, hyperlipidemia, smoking, poor dietary habits, overweight, physical inactivity) included in the Risk and Prevention study were selected. We considered the combined endpoint of non-fatal acute myocardial infarction and stroke and CV death. A multivariate Cox proportional analysis was carried out to identify relevant predictors. We also used the RECPAM method to identify subgroups of patients at higher risk. In our study, the rate of major CV events was lower than expected (5 % in 5 years). Predictors of CV events were age, male, sex, heart failure, previous atherosclerotic disease, atrial fibrillation, insulin treatment, high HbA1c, heart rate and other CV diseases while being physically active was protective. RECPAM analysis indicated that history of atherosclerotic diseases and a low BMI defined worse prognosis (HR 4.51 95 % CI 3.04–6.69). Among subjects with no previous atherosclerotic disease, men with HbA1c more than 8 % were at higher CV risk (HR 2.77; 95 % CI 1.86–4.14) with respect to women. In this population, the rate of major CV events was lower than expected. This prediction model could help clinicians identify people with DM at higher CV risk and support them in achieving goals of physical activity and HbA1c.

Abstract

Background Trials have shown a beneficial effect of n-3 polyunsaturated fatty acids in patients with a previous myocardial infarction or heart failure. We evaluated the potential benefit of such therapy in patients with multiple cardiovascular risk factors or atherosclerotic vascular disease who had not had a myocardial infarction. Methods In this double-blind, placebo-controlled clinical trial, we enrolled a cohort of patients who were followed by a network of 860 general practitioners in Italy. Eligible patients were men and women with multiple cardiovascular risk factors or atherosclerotic vascular disease but not myocardial infarction. Patients were randomly assigned to n-3 fatty acids (1 g daily) or placebo (olive oil). The initially specified primary end point was the cumulative rate of death, nonfatal myocardial infarction, and nonfatal stroke. At 1 year, after the event rate was found to be lower than anticipated, the primary end point was revised as time to death from cardiovascular causes or admission to the hospital for cardiovascular causes. Results Of the 12,513 patients enrolled, 6244 were randomly assigned to n-3 fatty acids and 6269 to placebo. With a median of 5 years of follow-up, the primary end point occurred in 1478 of 12,505 patients included in the analysis (11.8%), of whom 733 of 6239 (11.7%) had received n-3 fatty acids and 745 of 6266 (11.9%) had received placebo (adjusted hazard ratio with n-3 fatty acids, 0.97; 95% confidence interval, 0.88 to 1.08; P=0.58). The same null results were observed for all the secondary end points. Conclusions In a large general-practice cohort of patients with multiple cardiovascular risk factors, daily treatment with n-3 fatty acids did not reduce cardiovascular mortality and morbidity. Abstract

The identification of subjects at high risk for sudden cardiac death is a key factor for an adequate preventive strategy. Relevant animal models may provide both a better understanding of the pathophysiologic mechanism involved and suggestions for a more precise risk stratification. Vagal hyperactivity is generally beneficial in the setting of acute myocardial ischemia. In a conscious animal model for sudden cardiac death, electric vagal stimulation markedly reduces the incidence of ventricular fibrillation (VF) in a high risk subgroup, whereas muscarinic blockade increases malignant arrhythmias in low risk animals. Among 192 dogs, those that develop VF during an episode of acute myocardial ischemia one month after myocardial infarction, have a significantly lower baroreceptor reflex sensitivity, compared to the survivors (9.1 +/- 6.0 vs 17.7 +/- 6.5 msec/mmHg, p0.0001). Furthermore, the analysis of vagal reflexes may identify high risk animals also before myocardial infarction. Therefore, the experimental studies indicate that the analysis of baroreceptor reflexes may be a powerful tool in the stratification of risk for sudden cardiac death.

Abstract

The effects of renal ischemia and backflow of non-diuretic urine into the renal pelvis on renal efferent sympathetic postganglionic nerve activity, femoral arterial pressure and heart rate were studied to verify whether stimulation of renal chemoreceptors elicits autonomic reflexes. In rats with intact spinal cord or spinal cord sectioned at the T6 level a brief activation of renal chemoreceptors produced excitatory ipsilateral and contralateral renorenal reflexes, whereas it only slightly and insignificantly altered arterial pressure and heart rate. These results indicate that stimulation of renal chemoreceptors elicits renorenal excitatory reflexes which might be integrated both at spinal and supraspinal levels.

Abstract

We studied the hemodynamic mechanism responsible for the antihypertensive effect of indapamide in eight patients with mild essential hypertension. Systemic and pulmonary hemodynamics were measured using direct techniques (right heart catheterization and thermodilution method), before and 7-10 days after oral treatment with indapamide (2.5 mg/day). Indapamide reduced mean arterial blood pressure from 120 +/- 1.6 (mean +/- SE) to 101 +/- 1.4 mm Hg (p less than 0.01), and mean pulmonary artery pressure from 21 +/- 0.59 to 17 +/- 1.05 mm Hg (p less than 0.01). Total peripheral vascular resistance (TPR) and pulmonary vascular resistance were reduced from 36 +/- 0.85 to 29 +/- 0.72 U/m2 (p less than 0.01) and from 4.3 +/- 0.17 to 3.8 +/- 0.18 U/m2 (p less than 0.01), respectively. Indapamide did not change cardiac index (CI) (3,311 +/- 61.6 vs. 3,325 +/- 72.1 ml/min/m2), heart rate (HR) (75 +/- 1.7 vs. 75 +/- 9 beats/min), mean rate of left ventricular ejection index 140 +/- 2.04 vs. 139 +/- 1.99 ml/s/m2, and stroke index (44 +/- 5.6 vs. 43 +/- 5.8 ml/m2). Mean pulmonary wedge pressure decreased from 7 +/- 0.6 to 5 +/- 0.5 mm Hg (p less than 0.05). Body weight, 24-h urinary volume, and hematocrit were unchanged after treatment. We conclude that the hemodynamic mechanism responsible for the antihypertensive action of indapamide is a reduction in TPR without changes in CI and HR.

Experiments were carried out to investigate whether the activation of renal chemoceptive receptors by natural stimuli might induce reflex alterations of efferent postganglionic activity to the ipsilateral kidney and preganglionic activity to the ipsilateral adrenal. 2. In anaesthetized rats with intact nervous system back-flow of urine and occlusion of the renal artery were accompanied by increments in efferent sympathetic activity both to the kidney and adrenal without concomitant changes in heart rate and blood pressure. 3. Greater excitatory responses in nerve activity to the same test stimuli were observed in rats with the spinal cord cut at C 1. 4. These results indicate that the natural activation of renal chemoceptive receptors might induce reno-renal and reno-adrenal excitatory reflexes which are likely to be integrated at spinal and supraspinal levels.

Arterial blood pressure, heart rate, efferent postganglionic sympathetic activity and afferent nerve activity from the right renal nerves were recorded in anaesthetized, spontaneously breathing, non diuretic rats, either with an intact spinal cord or with a spinal cord transected at the T6 level, before, during and after intravenous injections of 0.5--1.0 mg/kg of teprotide or captopril. 3. After injection of drugs, efferent sympathetic nerve activity markedly increased and reached its peak value 4 min later, both in rats with an intact spinal cord (101 +/- 21% mean +/- SE, above the control discharge) and with a spinal cord transected at the T6 level (166 +/- 41%, above the control). 4. Afferent activity from the renal nerve, on the other hand, did not consistently change during converting enzyme blockade. 5. The results indicate that the efferent sympathetic excitation cannot be due either to a baroreceptor or to a renorenal reflex. This excitation might be responsible, at least in part, for the increase in renin secretion which follows the blockade of angiotensin-converting enzyme.

Afferent activity from the renal nerve, on the other hand, did not consistently change during converting enzyme blockade. 5. The results indicate that the efferent sympathetic excitation cannot be due either to a baroreceptor or to a renorenal reflex. This excitation might be responsible, at least in part, for the increase in renin secretion which follows the blockade of angiotensin-converting enzyme.

Abstract

The effects of renal ischemia and backflow of non-diuretic urine into the renal pelvis on renal efferent sympathetic postganglionic nerve activity, femoral arterial pressure and heart rate were studied to verify whether stimulation of renal chemoreceptors elicits autonomic reflexes. In rats with intact spinal cord or spinal cord sectioned at the T6 level a brief activation of renal chemoreceptors produced excitatory ipsilateral and contralateral renorenal reflexes, whereas it only slightly and insignificantly altered arterial pressure and heart rate. These results indicate that stimulation of renal chemoreceptors elicits renorenal excitatory reflexes which might be integrated both at spinal and supraspinal levels.

Abstract

Introduction: Autism spectrum disorder (ASD) is a group of neurodevelopmental disorders characterized by abnormal development of cognitive, social, and communicative skills. Although ASD aetiology and pathophysiology are still unclear, various nutritional factors have been investigated as potential risk factors for ASD development, including omega-3 polyunsaturated fatty acids (PUFAs) and vitamin D deficiency. In fact, both omega-3 PUFAs and vitamin D are important for brain development and function. Case report: Herein, we report the case of a 23-year-old young adult male with autism who was referred to our Unit due to a 12-month history of cyclic episodes of restlessness, agitation, irritability, oppositional and self-injurious behaviours. Laboratory tests documented a markedly altered omega-6/omega-3 balance, along with a vitamin D deficiency, as assessed by serum levels of 25-hydroxyvitamin D. Omega-3 and vitamin D co-supplementation was therefore started, with remarkable improvements in ASD symptoms throughout a 24-month follow-up period. A brief review of the literature for interventional studies evaluating the efficacy of omega-3 or vitamin D supplementation for the treatment of ASD-related symptoms is also provided. Conclusion: To our knowledge, this is the first case reporting remarkable beneficial effects on ASD symptoms deriving from omega-3 and vitamin D combination therapy. This case report suggests omega-3 and vitamin D co-supplementation as a potential safe-effective therapeutic strategy to treat core symptoms of ASD. However, larger studies are needed to evaluate the real efficacy of such therapeutic approach in a broader sample of ASD patients.

Abstract

The identification of subjects at high risk for sudden cardiac death is a key factor for an adequate preventive strategy. Relevant animal models may provide both a better understanding of the pathophysiologic mechanism involved and suggestions for a more precise risk stratification. Vagal hyperactivity is generally beneficial in the setting of acute myocardial ischemia. In a conscious animal model for sudden cardiac death, electric vagal stimulation markedly reduces the incidence of ventricular fibrillation (VF) in a high risk subgroup, whereas muscarinic blockade increases malignant arrhythmias in low risk animals. Among 192 dogs, those that develop VF during an episode of acute myocardial ischemia one month after myocardial infarction, have a significantly lower baroreceptor reflex sensitivity, compared to the survivors (9.1 +/- 6.0 vs 17.7 +/- 6.5 msec/mmHg, p < 0.0001). Furthermore, the analysis of vagal reflexes may identify high risk animals also before myocardial infarction. Therefore, the experimental studies indicate that the analysis of baroreceptor reflexes may be a powerful tool in the stratification of risk for sudden cardiac death.

Abstract

We studied the hemodynamic mechanism responsible for the antihypertensive effect of indapamide in eight patients with mild essential hypertension. Systemic and pulmonary hemodynamics were measured using direct techniques (right heart catheterization and thermodilution method), before and 7-10 days after oral treatment with indapamide (2.5 mg/day). Indapamide reduced mean arterial blood pressure from 120 +/- 1.6 (mean +/- SE) to 101 +/- 1.4 mm Hg (p less than 0.01), and mean pulmonary artery pressure from 21 +/- 0.59 to 17 +/- 1.05 mm Hg (p less than 0.01). Total peripheral vascular resistance (TPR) and pulmonary vascular resistance were reduced from 36 +/- 0.85 to 29 +/- 0.72 U/m2 (p less than 0.01) and from 4.3 +/- 0.17 to 3.8 +/- 0.18 U/m2 (p less than 0.01), respectively. Indapamide did not change cardiac index (CI) (3,311 +/- 61.6 vs. 3,325 +/- 72.1 ml/min/m2), heart rate (HR) (75 +/- 1.7 vs. 75 +/- 9 beats/min), mean rate of left ventricular ejection index 140 +/- 2.04 vs. 139 +/- 1.99 ml/s/m2, and stroke index (44 +/- 5.6 vs. 43 +/- 5.8 ml/m2). Mean pulmonary wedge pressure decreased from 7 +/- 0.6 to 5 +/- 0.5 mm Hg (p less than 0.05). Body weight, 24-h urinary volume, and hematocrit were unchanged after treatment. We conclude that the hemodynamic mechanism responsible for the antihypertensive action of indapamide is a reduction in TPR without changes in CI and HR.
