International audience; IntroductionLes déterminants génétiques des troubles du comportement ont été proposés à partir d’études familiales. Aucune étude n’a examiné le lien apolipoprotéine E (APOE) et troubles du comportement en Afrique centrale.ObjectifsL’objectif de notre étude était d’évaluer l’association entre l’allèle ɛ4 de l’APOE et les troubles du comportement chez les personnes âgées en République centrafricaine et en République du Congo.MéthodesUne étude transversale, multicentrique, en population générale a été menée de novembre 2011 à décembre 2012 en zones rurales et urbaines en République centrafricaine et en République du Congo. Les troubles du comportement ont été évalués à l’aide de l’inventaire neuropsychiatrique. Un échantillon de sang a été prélevé chez tous les participants consentants. Notre étude a inclus les participants ayant donné leur consentement pour les prélèvements génétiques, et ayant bénéficié de l’inventaire neuropsychiatrique.RésultatsLa population étudiée était composée de 322 personnes âgées. L’âge médian était de 75,5 ans [65–99]. Il existait une prédominance féminine (81,1 %). L’allèle ɛ4 de l’APOE était présent chez 135 (41,9 %) participants. La prévalence des troubles du comportement tendait à diminuer de manière non significative avec le nombre d’allèles ɛ4 : 61,4 % des sujets présentant les troubles du comportement n’avaient aucun allèle ɛ4, 34,4 % avaient un allèle ɛ4 et 4,2 % avaient 2 allèles ɛ4 (p = 0,27).DiscussionLa présente étude ne confirme pas le lien entre l’allèle ɛ4 de l’APOE et la présence des troubles du comportement chez les sujets âgés. Notre étude concorde avec les précédentes études rejetant le rôle de l’APOE dans la survenue des troubles du comportement.ConclusionIl s’agit de la première étude visant à caractériser l’association entre l’APOE et les troubles du comportement en Afrique centrale. Des analyses complémentaires semblent nécessaires pour mieux comprendre cette association.

International audience; IntroductionIl existe peu d’études sur les facteurs associés aux troubles du comportement chez les sujets âgés en Afrique.ObjectifsL’objectif de notre étude était de décrire les facteurs associés aux troubles du comportement chez les sujets âgés dépistés avec un trouble cognitif en République centrafricaine et au Congo.MéthodesUne étude transversale multicentrique en population générale a été menée de novembre 2011 à décembre 2012 en zones rurales et urbaines de la République centrafricaine et en République du Congo. Les enquêtes étaient en 2 phases : dépistage des troubles cognitifs et confirmation du diagnostic de démence. Notre étude a inclu les participants à la deuxième phase ayant bénéficié de l’inventaire neuropsychiatrique. Un modèle de régression logistique a été utilisé pour rechercher les facteurs associés.RésultatsUn échantillon de 532 personnes âgées a été inclus. Le site, l’audition normale et les liens amicaux dans la communauté ont été identifiés comme des facteurs potentiellement protecteurs des troubles du comportement. Le sexe féminin, le handicap physique, la personnalité dépendante et le statut cognitif notamment la démence (OR = 7,79 IR : 3,46–17,57, p < 0,0001) ont été identifiés comme étant associés à la présence des troubles du comportement chez les sujets âgés.DiscussionLa présente étude est la première étude du genre en Afrique centrale. La forte association de la démence avec la présence des troubles du comportement confirme les données de la littérature. Les liens d’amitié sont des facteurs de socialisation qui pourraient être protecteurs vis-à-vis de la démence et donc des troubles du comportement.ConclusionLes troubles du comportement étaient fortement associés aux démences dans cette population. Toutefois, des études longitudinales sont nécessaires pour démontrer formellement le lien entre les différentes variables identifiées.

Abstract

Several studies in Western countries have shown an association between cognitive disorders and low BMI or weight loss in elderly people. However, few data are available in Africa. We analysed the association between cognitive disorders and undernutrition among elderly people in Central Africa. A cross-sectional, multicentre, population-based study using a two-phase design was carried out in subjects aged 65 years and above in the Central African Republic (CAR) and the Republic of Congo (ROC). All subjects were interviewed using the Community Screening Interview for Dementia, and those with low performance were clinically assessed by a neurologist and underwent further psychometrical tests. Diagnostic and Statistical Manual-IV and Petersen's criteria were required for the diagnoses of dementia and mild cognitive impairment (MCI), respectively. Abstract

International audience; A multicenter population-based study was carried out in Central African Republic (CAR) and Republic of Congobetween 2011 and 2012 including both urban and rural sites in each country. Participants aged ≥65 years oldwere interviewed and participants in Congo were followed-up annually between 2012 and 2015 (EPIDEMCA-FUstudy). Qualitative interviews using an ethno-anthropological approach were also performed with peopleidentified with dementia and their relatives in order to investigate social representations, beliefs and stigma.ResultsOverall, 2,002 elderly were interviewed in both countries, including 473 in Nola (rural CAR), 500 in Bangui (urbanCAR), 529 in Gamboma (rural Congo) and 500 in Brazzaville (urban Congo). The crude prevalence of DSM-IVdementia was estimated at 8.5% (CI95%[6.1-11.3] in Nola, at 6.4% (CI95%[4.4-8.9]) in Bangui, 5.7% (CI95%[3.9-8.0] in Gamboma and at 6.6% (CI95%[4.6-9.1]) in Brazzaville. The crude incidence was estimated at 15.8(95% CI 10.2 – 23.3) per 1000 PY. Factors associated with either prevalent or incident dementia in thesestudies, as well as the results of the qualitative interviews, will be detailed during the presentation.ConclusionThe need for targeted social and health policies for older people, as well as strategies for dementia care, isevident and growing in sub-Saharan Africa (SSA).

Introduction L’association des troubles du comportement avec les troubles cognitifs est frequente. Peu d’etudes sont disponibles sur les troubles du comportement chez les sujets âges en Afrique. Objectifs L’objectif de notre etude etait de decrire les troubles du comportement chez les sujets âges depistes avec un trouble cognitif en Republique Centrafricaine et au Congo. Patients et methodes Une etude transversale multicentrique en population generale a ete menee de novembre 2011 a decembre 2012 en zones rurales et urbaines de la Republique Centrafricaine (Nola et Bangui) et en Republique du Congo (Gamboma et Brazzaville). Les enquetes etaient en 2 phases : depistage des troubles cognitifs (CSI-D) et confirmation du diagnostic de demence (criteres DSM-IV). Notre etude a inclus les participants a la deuxieme phase ayant beneficie de l’inventaire neuropsychiatrique (NPI). Resultats Un echantillon de 532 personnes âgees a ete inclus. La prevalence globale des troubles du comportement etait de 63,7 % et variait de 34,2 % a Gamboma a 91,9 % a Bangui. Cette prevalence etait de 48,7 % chez les sujets normaux, 73,4 % chez les sujets avec Mild Cognitive Impairment et 89,9 % chez les sujets dements. Les plus frequents troubles du comportement etaient la depression (45,3 %), l’anxiete (28,1 %) et l’irritabilite (23,7 %). Discussion La presente etude est la premiere etude du genre en Afrique centrale. Nos donnees sont comparables a celles issues des precedentes etudes realisees en population generale en Afrique subsaharienne au Nigeria et en Tanzanie. Nos donnees sont egalement proches de celles issues des etudes realisees chez les sujets dements dans les pays developpes. Conclusion Nos donnees permettent une amelioration de la connaissance des troubles du comportement en Afrique. Toutefois, il s’avere utile de realiser des etudes complementaires pour mieux les caracteriser.

International audience; Hypertension represents a major global health burden. While older individuals of African descent are at higher risk of hypertension in western countries, epidemiologic data on hypertension in older native Africans are scarce. We assessed the prevalence and the level of awareness and control of hypertension among older adults in Central Africa. A total of 1990 individuals aged 65 years and older from the Republic of Congo and the Central African Republic participated into a cross-sectional population-based survey. Hypertension was defined by self-reporting of ongoing treatment and/or systolic blood pressure/diastolic blood pressure at rest being ≥140 and/or 90 mm Hg. Abstract

Background: Stressful life events (SLEs) are considered potential risk factors for cognitive disorders. Our objective was to investigate the association between SLEs and cognitive disorders among the elderly people in Central Africa. Method: A population-based study was conducted in the Central African Republic (CAR) and the Republic of Congo (ROC). Participants aged ≥65 were interviewed using the Community Screening Interview for Dementia. Those who performed poorly were clinically assessed by neurologists. DSM-IV and Petersen criteria were required for a diagnosis of dementia or mild cognitive impairment (MCI), respectively. SLEs were assessed through 18 questions about events that occurred during childhood, adulthood and late-life. Sociodemographic, vascular and psychological factors were also documented. Multivariate multinomial logistic regression models were used to estimate the associations. Results: MCI was positively associated with: the total number of SLEs (OR = 1.1, 95% CI: 1.0-1.2), the number of SLEs from the age of 65 (OR = 1.2, 95% CI: 1.0-1.3), the number of SLEs before the age of 16 among non-depressive participants (OR = 1.6, 95% CI: 1.2-2.2) and with a serious illness in a child experienced when the participant was aged 65 or more (OR = 2.8, 95% CI: 1.6-4.6). No association with dementia was observed. Conclusion: SLEs were positively associated with MCI but not dementia. More comprehensive studies are needed to further investigate this relationship.

Abstract

Background:The worldwide population is ageing and the proportion of elderly aged 60 and over is expected to dramatically rise in Low and Middle Income Countries (LMIC). The epidemic of dementia will not spare those countries, where the largest increases in numbers of people affected are estimated. Besides, dementia is still understudied in sub-Saharan Africa (SSA) compared to other regions. This paper describes the protocol for the 'Epidemiology of Dementia in Central Africa' population-based study, which aims at estimating the prevalence of dementia in two countries of Central Africa and investigating possible risk factors.Methods/Design:A multicenter population-based study was carried out in Central African Republic and Republic of Congo between 2011 and 2012 including both urban and rural sites in each country. Around 2000 participants aged ≥65 years old were interviewed in total using the Community Screening Interview for Dementia (CSI-D), the GMS-AGECAT and the CERAD's 10-word list. Elderly with low performance to the cognitive part of the CSI-D (COGSCORE ≤ 24.5) were then clinically assessed by neurologists and underwent further psychometrical tests. DSM-IV and NINCDS-ADRDA criteria were required for dementia and Alzheimer's disease (AD) diagnoses respectively. The algorithmic 10/66 dementia diagnosis was also determined. Petersen's criteria were required for the diagnosis of Mild Cognitive Impairment. Sociodemographic, and environmental factors including vascular, nutritional, biological, psychosocial and lifestyle factors were collected in each setting in order to investigate factors associated with dementia. Blood sampling was realized to investigate genetic variations that could modify the risk of dementia.Discussion:For now, no large epidemiological study has been undertaken to compare the prevalence of dementia in both rural and urban areas within SSA countries. This programme will provide further evidence regarding the prevalence of dementia in SSA, and also the possible rural/urban disparities existing with associated factors. Furthermore, the genetics of AD in those populations will be addressed.

Abstract

Resume Les hallucinations ont une origine complexe chez les personnes âgees et parfois les causes n’en sont pas claires. La depression severe, les effets secondaires des medicaments, les symptomes comportementaux et psychologiques de demence, la maladie a corps de Lewy et la psychose peuvent etre responsables et se chevaucher. Le DAT-scan a ete utilise dans cette etude pour obtenir un diagnostic medical precis dans 78 cas de personnes de plus de 65 ans presentant des hallucinations visuelles. Le protocole a ete approuve par le Comite d’ethique regional. Soixante-dix-huit personnes, avec des hallucinations visuelles, ont ete recrutees apres leur acception formelle pour joindre l’etude. Une evaluation medicale, incluant la recherche de depression, les effets secondaires des medicaments, un syndrome parkinsonien et des antecedents de chute, a ete effectuee. Le DAT-scan etait realise dans le departement de medecine nucleaire avec mesure semi-quantitative de la synthese de dopamine. Dix-neuf patients, avec maladie de Parkinson mais sans troubles cognitifs ou hallucinations, ont beneficie d’un DAT-scan pour evaluer un tremblement et leurs resultats ont ete compares avec ceux de patients ayant une maladie a corps de Lewy. Trente-trois patients, avec une maladie a corps de Lewy, avaient une synthese de dopamine reduite dans le striatum. Le DAT-scan ne permettait pas de faire la difference entre la maladie a corps de Lewy et la maladie de Parkinson. Le DAT-scan, dans ces deux cas, montrait une activite dopaminergique des noyaux gris moindre que chez les patients deprimes, qu’ils aient ou non des troubles cognitifs, ou dans la psychose, meme en presence d’un syndrome parkinsonien secondaire a un traitement antipsychotique. En conclusion, dans certains cas difficiles de personnes ayant des hallucinations, le DAT-scan peut etre utile pour faire le diagnostic entre maladie a corps de Lewy et d’autres pathologies psychiatriques. Le seuil de trois pour le ratio striatum/cervelet est celui qui permet la meilleure discrimination entre presence et absence de denervation dopaminergique.

Abstract

International audience; Therapeutic patient education (TPE) is the process by which health professionals impart information to help patients self-manage their chronic disease: it is an essential part of treatment of long-term diseases and conditions. Memory loss and other cognitive disorders are usually considered as obstacles to TPE for patients with Alzheimer's disease or related disorders (ADRD). Over 100 patients with different forms of ADRD and caregivers have benefited from TPE programs since 2011 at the Limoges University Clinical and Research Memory Center. Participants report better understanding of the disease and improved relationships. TPE may prevent anxiety and depression in patient and in caregivers, and reduce burden of caregivers. General guidelines and perspectives for TPE in ADRD are outlined.

Abstract

Background Assessment of cognitive disorders in schizophrenia is becoming a part of clinical and research practice by using batteries that differ widely in their content. The Brief Assessment of Cognition in Schizophrenia (BACS) was developed to cover the main cognitive deficits of schizophrenia. The objective of this study was to assess concurrent validity of the Arabic version of the BACS with a standard neurocognitive battery of tests in Lebanese patients with schizophrenia and healthy controls. Methods A sample of 120 stable inpatients diagnosed with schizophrenia and 60 healthy controls received the Arabic version of the BACS in a first session, and a standard battery in a second session. Results Mean duration of completion for the BACS was 31.2 ± 5.4 min in patients with schizophrenia. All tests demonstrated significant differences between controls and patients (p r = .73, p r = .78, p p Conclusion Results showed that the Arabic version of the BACS demonstrated high ability to discriminate patients with schizophrenia from healthy controls and it is a useful tool for assessing cognition in patients with schizophrenia and could be used in clinical practice in Lebanon.

Abstract

International audience; Objectives: Older people are vulnerable to mental and cognitive health issues. Studies reported that depressive or anxious older people are more likely to present or to develop dementia over time. The relations between these psychiatric disorders and dementia are poorly explored in sub-Saharan Africa. Therefore, we investigated the association between depression, anxiety and dementia among older people living in two Central Africa countries.Methods: A multicenter cross-sectional study was carried out between 2011 and 2012 in Republic of Congo (ROC) and in Central African Republic among people aged ≥65 years. An annual follow-up was performed only in ROC until 2015. Data were collected using a standardized questionnaire and participants underwent a short physical examination. Depression and anxiety symptoms were collected at baseline and were ascertained using a community version of the Geriatric Mental State (GMS-B3). Probable cases were defined as having a GMS-AGECAT score ≥3. Dementia diagnosis was performed by a neurologist according to DSM-IV criteria. Cross-sectional and longitudinal associations between depression, anxiety and dementia were investigated using logistic and competing-risk regression models respectively.Results: Sample for cross-sectional analyses was 1773 participants (median age: 72 [interquartile range: 67-77] years) and 1053 (59.4%) were female. In total, 650 (36.7%), 133 (7.5%) participants presented respectively depression and anxiety symptoms, and 135 (7.6%) participants were diagnosed with dementia. After accounting for several covariates, depression significantly increased the odds of dementia (adjusted Odds Ratio= 1.62; 95%CI: 1.05-2.50), while anxiety was not associated with dementia (aOR= 0.81; 95%CI: 0.40-1.65). The effects of both psychiatric disorders on dementia incidence within the Congolese participants will be presented in detail.Conclusions: Many Central African older people experience depression more than anxiety and are more likely to present dementia. Promoting and strengthening social support around depressive older people could help to delay or manage dementia in this population.

Abstract

Introduction: The prevalence of neurocognitive disorders (NCDs) increases with age and is associated with cognitive impairment. Older patients with NCD admitted to the emergency department (ED) are readmitted after discharge to home more often than those without NCD. Comprehensive geriatric assessment (CGA) is effective for improving clinical outcomes in older patients; however, the usefulness of CGA for older patients with NCD admitted to the ED has not been investigated. The main objective of our study is to assess the effectiveness of a geriatric emergency medicine unit (GEMU) for elderly patients with NCD admitted to the ED. Methods: This historical cohort study included patients aged 75 years and older with NCD admitted to the ED of Limoges University Hospital in France over a 4-year period. We compared patients treated in our hospital’s GEMU, the MUPA unit (exposed group), and patients who received standard care by emergency physicians (control group). The primary end point was the incidence of 30-day readmissions. Results: The study included 801 patients admitted to the ED between January 1, 2015, and December 31, 2018 (400 in the exposed group). Of those, 72.5% were female, and the mean age was 87 ± 5 years. After adjusting for confounding factors, the 30-day readmission rate was significantly associated with the MUPA unit intervention. Conclusion: CGA in a GEMU improved health outcomes in elderly patients with NCD in the ED. We recommend that all EDs include a geriatric team, such as the MUPA unit, to treat all patients with NCD admitted to the ED.

International audience; Introduction: Depression and anxiety are two very common psychiatric disorders in late-life. They are markers of poor quality of life and are strongly associated with death among older people. Yet, few studies on these comorbidities have been conducted in the African population. This study aims to present the epidemiology of depression and anxiety among older people in Central Africa.Method: A cross-sectional population-based study was carried out in Republic of Congo (ROC) and Central African Republic (CAR) between 2011 - 2012 among older people aged ≥ 65 years (EPIDEMCA study). Data were collected using a standardized questionnaire and participants underwent a brief physical examination. Depression and anxiety symptoms were ascertained using a community version of the Geriatric Mental State (GMS-B3) and the Automated Geriatric Examination for Computer Assisted Taxonomy diagnostic system (AGECAT), probable cases were defined as having a GMS-AGECAT level of 3 or more. Logistic regression models were used to investigate the association between potential risk factors collected and each symptom.Results: Overall 2002 participants were included in the EPIDEMCA study (500 in Brazzaville and 529 in Gamboma in ROC, 500 in Bangui and 473 in Nola (473) in CAR). Median age of the participants was 72 years [interquartile range: 68 – 78 years] and females were mostly represented (61.8%). Prevalence was 38.1% (95% Confidence Interval: 35.9% - 40.2%) for depression, 7.7% (95% CI: 6.5% - 8.9%) for anxiety and 5.7% (95% CI: 4.6% - 6.7%) for the co-occurrence of both disorders. For all three outcomes, prevalence was significantly higher among females and in rural areas. Only depression increased with age. Abstract

Our aim is to describe changes in serum concentration for the pro-inflammatory factors TNF-α, IFN-γ, IL-1β, IL-8, IL-6, IL-10, IL-12 and MCP-1, for the satiety factor leptin and for factors associated with neuronal changes, neuron-specific enolase (NSE) and glial activation S100-beta protein (S100-β), and explore their association with abstinence in alcohol-dependent subjects after withdrawal., Serum sampling and clinical assessments from 115 alcohol-dependent subjects admitted to a psychiatric hospital for alcohol were repeated during the first 48 h of withdrawal (M0) and 1, 2, 4 and 6 months (M1, M2, M4 and M6) thereafter. Serum factors were determined with Luminex technology or by ELISA., The levels of TNF-α, IL-1β, IL-8, IL-6, IL-12, MCP-1, and leptin decreased after withdrawal and remained low until M6, regardless of alcohol consumption. IFN-γ levels remained constant and IL-10 levels changed only slightly. NSE levels were not modified, whereas serum S100-β concentration increased significantly on M1 and then plateaued, regardless of abstinence status at 6 months., Alcohol-dependent subjects present an inflammatory condition that is not dependent on alcohol consumption. An understanding of the changes in concentration of the various proteins considered here would provide insight into the physiology of withdrawal or dependence.

Abstract

International audience; Our study aimed at estimating the prevalence of neuropsychiatric symptoms and investigating associated factors among older adults living in two countries in Central Africa (Central African Republic [CAR] and Republic of Congo [ROC]).METHODS:The EPIDEMCA multicentre population-based study was carried out in rural and urban areas of CAR and ROC between 2011 and 2012 among people aged 65 and over. After cognitive screening using the Community Screening Interview for Dementia, participants with low performances underwent neurological examination including the brief version of the Neuropsychiatric Inventory Questionnaire (NPI-Q). Multivariate logistic regression analyses were performed to identify factors independently associated with neuropsychiatric symptoms in this population.RESULTS:NPI-Q data were available for 532 participants. Overall, 333 elderly people (63.7%) reported at least one neuropsychiatric symptom. The prevalence of neuropsychiatric symptoms was 89.9% (95% CI, 84.6-95.1) in participants with dementia, 73.4% (95% CI, 65.1-81.7) in participants with mild cognitive impairment (MCI), and 48.7% (95% CI, 42.9-54.6) in participants with no MCI nor dementia after neurological examination (P < 0.0001). The most common symptoms were depression, anxiety, and irritability. Participants living in Gamboma, with normal hearing and with friends in the community, were less likely to present neuropsychiatric symptoms. Physical disability, difficulties in eating, female sex, and dementia were significantly associated with neuropsychiatric symptoms.CONCLUSION:Neuropsychiatric symptoms are common among older people with neurocognitive disorders in CAR and ROC. Our results confirm those from previous studies in Nigeria and Tanzania. Nevertheless, knowledge of these symptoms remains limited in sub-Saharan Africa, hampering their appropriate management.

International audience; OBJECTIVE:We aimed at estimating the seroprevalence of Toxoplasma gondii infection in older adults living in Central Africa and investigating its association with dementia using data from the Epidemiology of Dementia in Central Africa (EPIDEMCA) programme.METHODS:A cross-sectional multicentre population-based study was carried out among participants aged 73 (±7) years on average, living in rural and urban areas of the Central African Republic and the Republic of Congo between November 2011 and December 2012. Blood samples were collected from each consenting participant. The detection of anti-T.gondii immunoglobulin G antibodies was performed in 2014 in France using a commercially available ELISA kit. Participants were interviewed using a standardized questionnaire including sociodemographic characteristics. DSM-IV criteria were required for a diagnosis of dementia. Multivariate binary logistic regression models were used to estimate the association between toxoplasmosis infection and dementia.RESULTS:Among 1,662 participants, the seroprevalence of toxoplasmosis was 63.0% (95% confidence interval (CI): 60.7-65.3) overall, 66.6% (95%CI: 63.4-69.8) in Central African Republic and 59.4% (95%CI: 56.1-62.7) in the Republic of Congo. In multivariate analyses, toxoplasmosis status was significantly associated with increasing age (p=0.006), Republic of Congo (p=0.002), urban area (p=0.001) and previous occupation (p=0.002). No associations between dementia and toxoplasmosis status or anti- T.gondii IgG titres were found.CONCLUSION:T.gondii infection was not associated with dementia among older adults in Central Africa. Our findings are consistent with previous studies and add to the knowledge on the relationship between T.gondii infection and neurological disorders. This article is protected by copyright. Abstract

International audience; INTRODUCTION:Diagnostic relevance of plasma amyloid β (Aβ) for Alzheimer's disease (AD) process yields conflicting results. The objective of the study was to assess plasma levels of Aβ42 and Aβ40 in amnestic mild cognitive impairment (MCI), nonamnestic MCI, and AD patients and to investigate relationships between peripheral and central biomarkers.METHODS:One thousand forty participants (417 amnestic MCI, 122 nonamnestic MCI, and 501 AD) from the Biomarker of AmyLoïd pepTide and AlZheimer's diseAse Risk multicenter prospective study with cognition, plasma, cerebrospinal fluid (CSF), and magnetic resonance imaging assessments were included.RESULTS:Plasma Aβ1-42 and Aβ1-40 were lower in AD (36.9 [11.7] and 263 [80] pg/mL) than in amnestic MCI (38.2 [11.9] and 269 [68] pg/mL) than in nonamnestic MCI (39.7 [10.5] and 272 [52] pg/mL), respectively (P = .01 for overall difference between groups for Aβ1-42 and P = .04 for Aβ1-40). Globally, plasma Aβ1-42 correlated with age, Mini-Mental State Examination, and APOE ε4 allele. Plasma Aβ1-42 correlated with all CSF biomarkers in MCI but only with CSF Aβ42 in AD.DISCUSSION:Plasma Aβ was associated with cognitive status and CSF biomarkers, suggesting the interest of plasma amyloid biomarkers for diagnosis purpose.Copyright © 2018 The Authors. Abstract

Introduction Alcohol use is responsible for a high level of mortality and morbidity throughout the world. The WHO global strategy recommends that the detrimental effects of alcohol use be reduced. Aims The objective of this paper was to investigate, using data from the 2010 Togo STEPS survey, alcohol use and other health-related factors in the general population of Togo. Methods This epidemiological investigation using the STEPwise approach was undertaken from December 1st, 2010, to January 23rd, 2011, throughout the five regions of Togo. Togo is a low-income country (World Bank) located in West Africa. The study involved 4800 people aged 15 to 64 who were representative of the population of Togo and who were selected using the one-stage cluster sampling method. Results The sample was young and predominantly male. Approximately one-third of the respondents were alcohol abstainers, with the majority of these being women. Approximately the same proportion of current drinkers (daily consumption) by gender was observed. The reported daily average consumption of alcohol was 13 g of pure alcohol for men and 9 g for women. The mean number of heavy drinking days over the previous 30 days was higher for men (3 days), and this included 37.5% of the men who drink. Conclusion We suggest a comparative analysis of the prevalence of harmful alcohol use in Togo and the WHO African region., Highlights • Men consumed an average of 13 g of pure alcohol daily. • Women consumed an average of 9 g of pure alcohol daily. • A comparative analysis of the prevalence of harmful alcohol use in Togo vs the African region is recommended.

Abstract

International audience; Objective: The aim of our study was to estimate the prevalence of behavioral symptoms and associated factors in the elderly in the Central African Republic and the Republic of Congo.Methods: An analysis of baseline data was performed from the EPIDEMCA (Epidemiology of Dementia in Central Africa) multicenter population-based study carried out in rural and urban areas of the Central African Republic and the Republic of Congo between 2011 and 2012 in elderly aged 65 and over. Cognitive disorders were assessed in two phases. The first phase was cognitive screening using the Community Screening Interview in Dementia. Following cognitive screening, in the second phase, participants suspected of dementia underwent clinical assessment based on the abridged version of the neuropsychiatric inventory (NPI) which assesses 12 behavioral disorders in the elderly over the previous 30 days. Results: NPI data were available for 532 out of participants examined in the second phase of EPIDEMCA. Among the 532 elderly included in the analytic sample, 284 were cognitively normal, 130 were demented, 113 had Mild Cognitive Impairment (MCI) and 5 remained without diagnosis. The median age was 75 and most were female. At least one behavioral disorder was reported by 333 elderly (63.7%) with 16.6% reporting 1 or 2 behavioral disorders and 47.1% reporting 3 or more symptoms. In participants with no cognitive impairment, 48.7% reported at least one symptom. The prevalence of behavioral disorders was 73.4% in people with MCI and 89.9% in people with dementia. In our sample, the most frequent symptoms were depression/dysphoria (45.3%), anxiety (28.1%) and irritability (23.7%).Discussion: Behavioral disorders are common in this population. Our data are comparable to those reported in previous population-based studies in Nigeria and Tanzania. It would be important to carry out additional studies to better characterize behavioral disorders in others countries in Africa.

Abstract

Background/Aims: Alzheimer disease (AD) is particularly devastating, with no cure, no means of prevention, and no proven way to slow progression. AD is associated with the worsening of cognitive function attributable to a variety of factors of which little is known. Our main objective was to determine factors associated with rapid cognitive decline (RCD) in older AD patients. Methods: We conducted a 12-month, prospective, multi-centre cohort study. Community-living individuals aged ≥65 years with mild-to-moderate AD were included. RCD was defined as the loss of ≥3 points/year in the Mini-Mental State Examination (MMSE) score. Potential individual-level predictors were collected at baseline. Results: A total of 521 individuals were included. The mean age was 80.8 ± 9.0 years and 66.0% were females. The average baseline MMSE score was 20.5 ± 4.5. The incidence of RCD was 40.9% (95% confidence interval [CI], 36.7–45.1). RCD was more common in patients with moderate (53.5%) than mild (22.3%) AD. The factors associated with RCD were: a parental history of dementia (odds ratio [OR], 2.32 [95% CI, 1.24–4.21], p = 0.011), psychotic symptoms (OR, 2.06 [95% CI, 1.22–3.48], p = 0.007), malnutrition (OR, 1.61 [95% CI, 1.06–2.63], p = 0.028), and the female gender (OR, 1.48 [95% CI, 1.03–2.15], p = 0.036). An MMSE score < 20 at treatment onset was also associated with RCD (p < 0.001). Conclusion: The factors associated with RCD were an MMSE score < 20 at treatment onset, female gender, psychotic symptoms, malnutrition, and a family history of dementia. These results may be directly relevant to patients, their families, and their physicians, enabling early anticipation of difficult clinical trajectories and poor functional outcomes.

Abstract

Summary Background No large trials have been done to investigate the efficacy of an intervention combining a specific compound and several lifestyle interventions compared with placebo for the prevention of cognitive decline. We tested the effect of omega 3 polyunsaturated fatty acid supplementation and a multidomain intervention (physical activity, cognitive training, and nutritional advice), alone or in combination, compared with placebo, on cognitive decline. Methods The Multidomain Alzheimer Preventive Trial was a 3-year, multicentre, randomised, placebo-controlled superiority trial with four parallel groups at 13 memory centres in France and Monaco. Participants were non-demented, aged 70 years or older, and community-dwelling, and had either relayed a spontaneous memory complaint to their physician, limitations in one instrumental activity of daily living, or slow gait speed. They were randomly assigned (1:1:1:1) to either the multidomain intervention (43 group sessions integrating cognitive training, physical activity, and nutrition, and three preventive consultations) plus omega 3 polyunsaturated fatty acids (ie, two capsules a day providing a total daily dose of 800 mg docosahexaenoic acid and 225 mg eicosapentaenoic acid), the multidomain intervention plus placebo, omega 3 polyunsaturated fatty acids alone, or placebo alone. A computer-generated randomisation procedure was used to stratify patients by centre. All participants and study staff were blinded to polyunsaturated fatty acid or placebo assignment, but were unblinded to the multidomain intervention component. Assessment of cognitive outcomes was done by independent neuropsychologists blinded to group assignment. The primary outcome was change from baseline to 36 months on a composite Z score combining four cognitive tests (free and total recall of the Free and Cued Selective Reminding test, ten Mini-Mental State Examination orientation items, Digit Symbol Substitution Test, and Category Naming Test) in the modified intention-to-treat population. The trial was registered with ClinicalTrials.gov (NCT00672685). Findings 1680 participants were enrolled and randomly allocated between May 30, 2008, and Feb 24, 2011. In the modified intention-to-treat population (n=1525), there were no significant differences in 3-year cognitive decline between any of the three intervention groups and the placebo group. Between-group differences compared with placebo were 0·093 (95% CI 0·001 to 0·184; adjusted p=0·142) for the combined intervention group, 0·079 (−0·012 to 0·170; 0·179) for the multidomain intervention plus placebo group, and 0·011 (−0·081 to 0·103; 0·812) for the omega 3 polyunsaturated fatty acids group. 146 (36%) participants in the multidomain plus polyunsaturated fatty acids group, 142 (34%) in the multidomain plus placebo group, 134 (33%) in the polyunsaturated fatty acids group, and 133 (32%) in the placebo group had at least one serious emerging adverse event. Four treatment-related deaths were recorded (two in the multidomain plus placebo group and two in the placebo group). The interventions did not raise any safety concerns and there were no differences between groups in serious or other adverse events. Interpretation The multidomain intervention and polyunsaturated fatty acids, either alone or in combination, had no significant effects on cognitive decline over 3 years in elderly people with memory complaints. An effective multidomain intervention strategy to prevent or delay cognitive impairment and the target population remain to be determined, particularly in real-world settings. Abstract

Blood brain-derived neurotrophic factor (BDNF) levels are influenced by both addiction and mood disorders, as well as somatic conditions, gender, and genetic polymorphisms, leading to widely varying results. Depressive symptoms and episodes are frequently observed in patients with alcohol use disorder, and vary widely over time, making it a challenge to determine which aspects are specifically involved in variations of serum BDNF levels in this population., We assessed 227 patients with alcohol dependence involved in a detoxification program, at baseline and after a follow-up of 6 months, for the Alcohol Use Disorders Identification Test score, the length of alcohol dependence, and the number of past detoxification programs. The Beck Depression Inventory and information on current tobacco and alcohol use, suicidal ideation, body mass index, age, gender, and psychotropic treatments were also collected. Serum BDNF (ELISA) and 2 genetic polymorphisms of the BDNF gene (Val33Met and rs962369) were analyzed., The presence of the Met allele, 2 markers of the history of alcohol dependence (gamma glutamyl transferase and the number of past treatments in detoxification programs), and the presence of a depressive episode (but not depressive score) were significantly associated with the 2 blood levels of BDNF at baseline and after 6 months. After controlling for baseline BDNF levels, the presence of the Met allele and an ongoing depressive episode were the only variables associated with changes in BNDF levels after 6 months., Low serum BDNF levels are associated with characteristics related to alcohol consumption and mood disorders, and variants of the BDNF gene in alcohol use disorder patients. The factors that most strongly influenced changes in serum BDNF levels following treatment in an alcohol detoxification program were variants of the BDNF gene and ongoing depression.

Abstract

International audience; IntroductionAccording to the World Health Organization, dementia poses a public health problem in sub-Saharan Africa because of the growing number of elderly. But the perception of this condition is poorly documented in low-income countries.ObjectivesTo Identify and analyze the perceptions of dementia in different population groups more or less concerned. MethodA Cross sectional study was conducted following a general population survey on dementia between October and December 2008 in Bangui and February-March 2009 in Brazzaville. Five groups of subjects were involved. The stigmatization of people with dementia was assessed using scale consisting of 15 questions. The Stat View 5.0 and SAS 9.1.3 software were used for data analysis.ResultsThe subjects of the groups interviewed were dominated by women with the exception of nursing staff. There was no equivalent to the biomedical concept of dementia in the local languages of the two countries in which the study was conducted. The problems of inability to work (73.3 to 82.7%) and reduction in family income (55.7 and 79.0%) were reported by subjects with mild cognitive impairment. The emotional impact was dominated by concerns about the progression of cognitive problems (52.5 to 76.8%), sadness and anxiety (50.8 to 79.4%).ConclusionIn urban zones in Central Africa, we find that perceptions of dementia include traditional and magical-religious aspects, even among health care workers who supposed best placed to provide tailored information.

Abstract

International audience; OBJECTIVES: Peripheral artery disease (PAD) is a common condition in Western countries, mostly in the elderly. Little is known about the epidemiology of PAD in Africa. We sought to determine the prevalence of this condition in the elderly in two community-dwelling cohorts in Central Africa. DESIGN: Prospective cross-sectional survey in general population over the age of 65 years in Bangui (Central African Republic) and Brazzaville (Congo). METHODS: We conducted a systematic door-to-door survey in two representative districts of each city. Demographic, clinical and biological data were collected. The ankle-brachial index (ABI) was used to detect PAD (ABI ≤ 0.90). RESULTS: Among the 976 participants, the prevalence of PAD was 15.0% in Bangui and 32.4% in Brazzaville, increasing with age. Adjusted to age, regular alcohol consumption was protective for women in Bangui (OR = 0.50, CI95%:0.25-0.98) and men in Brazzaville (OR = 0.43, CI95%:0.21-0.88). Hypertension was associated with PAD in women (OR = 4.14, CI95%:1.65-10.42 in Bangui and OR = 2.17, CI95%:1.16-4.06 in Brazzaville). Diabetes and smoking showed different associations according to gender and city. CONCLUSIONS: This first population study in Central Africa highlights the high prevalence of PAD in the older population, and emphasizes specificities regarding the risk factors, being different from data published in Western countries.

Abstract

Background: Dementia will concern more and more people in the developing countries, but the perception people have of dementia in these areas has not yet been studied. Method: During a general population survey (EDAC) carried out in Brazzaville (Republic of Congo), 27 elderly persons suspected of having dementia and 31 of their relatives, 90 cognitively impaired elderly persons and 92 of their relatives, as well as 33 hospital workers were interviewed according to the Explanatory Model Interview Catalogue. Results: Item prominence ratings indicate that the attention was mainly on the emotional and socio-economic consequences (scores >1.0 out of 5 points). Ageing and mental stress are the main perceived causes. Hospital workers are more aware of public stigma. Conclusion: The socio-cultural components of the dementia phenomenon have to be taken into account to enforce public health and social measures.

Abstract

Depression is often overlooked in elderly nursing home residents because symptoms may be masked or dismissed as an inevitable consequence of ageing. Current tools for the detection of depression in institutionalised older people are not always specific. Aims To construct and verify an instrument with which to detect depression in elderly nursing home residents (NH-SDI). Method Firstly for the construction, 328 elderly people were selected at random from the residents of 17 nursing homes in France, and examined by a single investigator. The examination included a psychiatric assessment, an evaluation of cognitive function using the MMSE, an evaluation of depressive state using four different instruments (mini-GDS, Goldberg, DMAS, CSDD), and assessment of any changes in behaviour in those suffering from dementia, using the NPI. A second stage was to confirm NH-SDI in 99 institutionalised subjects. Results Following the selection of items, we created a scale of 16 dichotomous items (NH-SDI). The internal consistency was satisfactory (α Cronbach = 0.85), as was its reliability with a sensitivity of 85.1% and a specificity of 86.5% for a cut-off score above 5. Conclusions The NH-SDI appears to be a useful instrument for the detection of depression in nursing homes and can easily be applied by healthcare staff as part of routine procedures. Abstract

International audience; The objective of this study was to assess the knowledge and attitudes of people living in two French counties. The data were collected from 1777 adults in the general population who were interviewed. Knowledge of and attitudes toward epilepsy in these French samples proved largely favorable: 90.6% identified epilepsy as different from a mental disorder or lunacy, and 79.2% correctly identified epilepsy as a brain disorder. The majority agreed with the statements that people with epilepsy should have the opportunity to get married and that children with epilepsy should be allowed to go to school with others. However, gaps in knowledge and erroneous beliefs prevailed, notably among men, old people, and individuals with low levels of education. This study yielded encouraging results. However, further studies are needed to confirm these findings and gain a better understanding of the contribution of French public knowledge to the causative factors generating stigma in France.

International audience; PURPOSE: Quality of life (QoL) in multiple system atrophy (MSA) is thought to be poorer than in Parkinson's disease (PD), primarily because of motor impairment, autonomic dysfunction and depression. The aim of the study was to investigate QoL in 10 patients with probable MSA (parkinsonian subtype) compared with 10 PD patients matched for motor disability on UPDRS III motor score. METHODS: All patients were ambulatory and non-demented. Mean durations of disease in MSA and PD patients were respectively 3.6 and 9.0 years. QoL was assessed using the SF-36 health-related questionnaire and a life satisfaction visual analogue scale. Patients were also evaluated for cognitive function (Mattis Dementia Rating Scale [Mattis DRS], Wisconsin Card Sorting Test [WCST], Stroop, Fluencies), depression (Beck Depression Inventory-II [BDI-II]), apathy (Modified Apathy Evaluation Scale) and were screened for non-motor symptoms (NMS Quest). RESULTS: The only difference in QoL between MSA and PD patients matched for motor disability was that the SF-36 vitality subscore was more impaired in MSA and negatively correlated with interference index on Stroop word colour testing. Depression and non-motor symptoms were associated with poorer QoL in both groups. Among MSA patients, cognitive impairment (Stroop interference index) and apathy also had a negative impact. CONCLUSION: There was no major difference in QoL between MSA and PD patients matched for motor disability with a disease duration about 5 years longer. The SF-36 vitality subscore was more impaired in MSA and associated with interference sensitivity.; INTRODUCTION: La qualité de vie (QDV) dans l’atrophie multisystématisée (AMS) est considérée plus altérée que celle de la maladie de Parkinson (MP). Le but de cette étude était de comparer la QDV chez dix patients atteints d’AMS probable (sous-type parkinsonien) et dix patients atteints de MP, appariés sur le handicap moteur. METHODE: Les patients inclus étaient ambulatoires et non déments. Les durées d’évolution dans le groupe AMS et MP étaient respectivement de 3,6 et 9,0 ans. Les patients étaient évalués sur la QDV, mesurée par le questionnaire SF-36 et une échelle visuelle analogique, l’état cognitif, l’apathie, la dépression, et les signes non moteurs. RESULTATS: La seule différence observée sur la QDV entre les deux groupes concernait la dimension vitalité de la SF-36, plus altérée chez les patients AMS et corrélée au score d’interférence du Stroop. La dépression et les signes non moteurs avaient un rôle délétère sur la QDV dans les deux groupes mais l’apathie et les difficultés au Stroop retentissaient uniquement sur la QDV des patients AMS. CONCLUSION: La QDV des patients AMS est comparable à celle des patients MP, appariés sur le handicap moteur, excepté sur la dimension vitalité plus altérée dans l’AMS et liée à la sensibilité à l’interférence.

Abstract

There is wide acknowledgement that apathy is an important behavioural syndrome in Alzheimer’s disease and in various neuropsychiatric disorders. In light of recent research and the renewed interest in the correlates and impacts of apathy, and in its treatments, it is important to develop criteria for apathy that will be widely accepted, have clear operational steps, and that will be easily applied in practice and research settings. Meeting these needs is the focus of the task force work reported here.The task force includes members of the Association Française de Psychiatrie Biologique, the European Psychiatric Association, the European Alzheimer’s Disease Consortium and experts from Europe, Australia and North America. An advanced draft was discussed at the consensus meeting (during the EPA conference in April 7th 2008) and a final agreement reached concerning operational definitions and hierarchy of the criteria.Apathy is defined as a disorder of motivation that persists over time and should meet the following requirements. Firstly, the core feature of apathy, diminished motivation, must be present for at least four weeks; secondly two of the three dimensions of apathy (reduced goal-directed behaviour, goal-directed cognitive activity, and emotions) must also be present; thirdly there should be identifiable functional impairments attributable to the apathy. Finally, exclusion criteria are specified to exclude symptoms and states that mimic apathy.

Abstract

International audience; IntroductionAccording to the World Health Organization, dementia poses a public health problem in sub-Saharan Africa because of the growing number of elderly. But the perception of this condition is poorly documented in low-income countries.ObjectivesTo Identify and analyze the perceptions of dementia in different population groups more or less concerned. MethodA Cross sectional study was conducted following a general population survey on dementia between October and December 2008 in Bangui and February-March 2009 in Brazzaville. Five groups of subjects were involved. The stigmatization of people with dementia was assessed using scale consisting of 15 questions. The Stat View 5.0 and SAS 9.1.3 software were used for data analysis.ResultsThe subjects of the groups interviewed were dominated by women with the exception of nursing staff. There was no equivalent to the biomedical concept of dementia in the local languages of the two countries in which the study was conducted. The problems of inability to work (73.3 to 82.7%) and reduction in family income (55.7 and 79.0%) were reported by subjects with mild cognitive impairment. The emotional impact was dominated by concerns about the progression of cognitive problems (52.5 to 76.8%), sadness and anxiety (50.8 to 79.4%).ConclusionIn urban zones in Central Africa, we find that perceptions of dementia include traditional and magical-religious aspects, even among health care workers who supposed best placed to provide tailored information.
