Abstract

Automatic delineation of the walls of retinal arteries in adaptive optics images.Pathological and healthy subjects are considered in the same paper.The impact of the tracking parameters is studied.The robustness of the axial reflections detection is evaluated.The accuracy and the convergence speed of the tracking and the active contour model are analyzed. Adaptive optics imaging of the retina has recently proven its capability to image micrometric structures such as blood vessels, involved in common ocular diseases. In this paper, we propose an approach for automatically segmenting the walls of retinal arteries in the images acquired with this technology. The walls are modeled as four curves approximately parallel to a previously detected reference line located near the vessel center (axial reflection). These curves are first initialized using a tracking procedure and then more accurately positioned using an active contour model embedding a parallelism constraint. We consider both healthy and pathological subjects in the same framework and show that the proposed method applies in all cases. Extensive experiments are also proposed, by analyzing the robustness of the axial reflections detection, the influence of the tracking parameters as well as the performance of the tracking and the active contour model. Noticeably, the results show a good robustness for detecting axial reflections and a moderate influence of the tracking parameters. Compared to a naive initialization, the active contour model coupled with the tracking also offers faster convergence and better accuracy while keeping an overall error smaller or very near the inter-physicians error.

Abstract

Importance Arteriovenous nickings (AVNs) in the retina are the cause of retinal vein occlusions and are also surrogates of cerebrovascular aging. The prevalent mechanistic model of AVNs stating that arteries crush veins remains somewhat unchallenged despite the lack of evidence other than fundus photographs. Here, we observed that venous nicking may be observed in the absence of physical contact with an arteriole. Observations This observational study, conducted from January 2013 to September 2014, included 7 patients showing remodeling of a venous segment close to a retinal arteriole without arteriovenous overlap were imaged by adaptive optics imaging. Affected venous segments showed a variable association of nicking, narrowing, deviation, and opacification. Venous segments were deviated toward the arterioles in 6 of the 7 cases. The degree of venous narrowing ranged from 40% to 77%, while at these sites, the width of the intervascular space ranged from 16 µm to 42 µm. Similar features were identified in typical AVNs. Conclusions and Relevance Arteriovenous nickings do not necessarily involve an arteriovenous compression. Instead, the topology of venous changes suggests a retractile process originating in the intervascular space. These findings have important implications for the understanding of retinal vein occlusions and of cerebrovascular aging.

International audience; Adaptive optics (AO) fundus imaging is an optoelectronic technique allowing an improvement of an order of magnitude of lateral resolution of retinal images. Currently, its main applications in ophthalmology span from photoreceptor to retinal pigment epithelial cells and vessels, each of them being affected by specific diseases. Technological and image processing improvements are expanding the scope of its medical applications. Here we will review some of the current and envisioned applications of AO in clinical practice.

Abstract

International audience; In this paper, we propose two important improvements of an existing approach for automatically segmenting the walls of retinal arteries of healthy / pathological subjects in adaptive optics images. We illustrate the limits of the previous approach and propose to (i) modify the pre-segmentation step, and (ii) embed additional information through coupling energy terms in the parallel active contour model. The interest of these new elements as well as the pre-segmentation step is then evaluated against manual segmentations. They improve the robustness against low contrasted walls and morphological deformations that occur along vessels in case of pathologies. Noticeably, this strategy permits to obtain a mean error of 13.4% compared to an inter-physicians error of 17%, for the wall thickness which is the most sensitive measure used. Additionally, this mean error is in the same range than for healthy subjects.

Abstract

International audience; Spectral-Domain Optical Coherence Tomography (SD-OCT) is a widely used method to observe retinal layers and follow pathological events in human. Recently, this technique has been adapted for animal imaging. This non-invasive technology brings a cross-sectional visualization of the retina, which permits to observe precisely each layer. There is a clear expansion of the use of this imaging modality in rodents, thus, a precise characterization of the different outer retinal layers observed by SD-OCT is now necessary to make the most of this technology. The identification of the inner strata until the outer nuclear layer has already been clearly established, while the attribution of the layers observed by SD-OCT to the structures corresponding to photoreceptors segments and retinal pigment epithelium is much more questionable. To progress in the understanding of experimental SD-OCT imaging, we developed a method for averaging SD-OCT data to generate a mean image allowing to better delineate layers in the retina of pigmented and albino strains of mice and rats. It allowed us to locate precisely the interface between photoreceptors and retinal pigment epithelium and to identify unambiguously four layers corresponding to the inner and outer parts of photoreceptors segments. We show that the thickness of the various layers can be measured as accurately in vivo on SD-OCT images, than post-mortem by a morphometric analysis of histological sections. We applied SD-OCT to different models and demonstrated that it allows analysis of focal or diffuse retinal pathological processes such as mutation-dependant damages or light-driven modification of photoreceptors. Moreover, we report a new method of combined use of SD-OCT and integration to quantify laser-induced choroidal neovascularization. In conclusion, we clearly demonstrated that SD-OCT represents a valuable tool for imaging the rodent retina that is at least as accurate as histology, non-invasive and allows longitudinal follow-up of the same animal.

Abstract

International audience; In this paper, we present a method for automatically segmenting the walls of retinal arteries in adaptive optics images. To our best knowledge, this is the first method addressing this problem in such images. To achieve this goal, we propose to model these walls as four curves approximately parallel to a common reference line located near to the center of vessels. Once this line detected, the curves are simultaneously positioned as near as possible to the borders of walls using an original tracking procedure to cope with deformations along vessels. Then, their positioning is refined using a deformable model embedding a parallelism constraint. Such an approach enables us to control the distance of the curves to their reference line and improve the robustness to image noise. This model was evaluated on healthy subjects by comparing the results against segmentations from physicians. Noticeably, the error introduced by this model is smaller or very near to the inter-physicians error.
