This article attempts to offer a detailed profile of the labor force in early nineteenth-century Istanbul. The primary source upon which this study is based is the register of an original Ottoman survey, conducted in the Istanbul districts of the Bosphorus and the Golden Horn, where some 2,000 shops along with their work force of 6,000 people as well as 1,500 peddlers (working as, among other things, boatmen, fishermen, and water carriers) were listed in a comprehensive fashion. Through an examination of the register, this article seeks to illuminate the general characteristics of employment and shop sizes, the ethno-religious profile of the labor force, occupational patterns in connection with religious allegiances and migration networks, and the degree to which the military corps were involved in commercial activities. It also questions the supposed existence of an “ethnic division of labor” in labor market and commercial activities, and demonstrates the centrality of regional allegiances in occupational specializations.

Neuropathic pain is common. In this study, our goal was to investigate the diagnostic yield of Lanss and DN4 scores which have not been used in Fabry Disease until now, to our knowledge. We also kept track of these tests in the followup. The pain scores before and after each and every two months of ERT (the patients received ERT every two weeks) regressed during ten months of therapy and this regression was statistically significant (p=0.001).

Chromosome Abnormalities and Hemodialysis in Uremic Patients.Abstract: Phytohemagglutinin (PHA) stimulated peripheral blood lymphocytes form 44 uremic patients and 24 control subjects were studied cytogenetically and the effect of hemodialysis was discussed. The effect of hemodialysis on chromosomal abnormalities was not detected.Üremili hastalarda hücresel seviyede meydana gelebilecek düzensizlikleri ve bunlara diyalizin etkisini araştırmak amacı ile, 44 üremili hasta ve 24 sağlıklı kişinin fitohemaglutinin (PHA) ile uyarılmış periferik kan lenfosi-telerînde kromozom analizi yapıldı. Hemodiyalizin etkisi araştırıldı. Hemodiyalizin kromozom bozukluklarına etkisi gözlenmedi.

SUMMARYCOMPUTED TOMOGRAPHY OF THE PRIMARY PULMONARY MALIGNANCIES AMD SOLITARY PULMONARY MODULESIn this study computed tomographic (CT) findings of the 83 patients with solitary pulmonary nodule and who have been thought primary pulmonary malignancy were evaluated. CT-guided transthoracic oeedle aspiration biopsy (TNAB) were also perfonned in 23 patients of this group. The valúes of CT and TNAB in primary pulmonary malignancy and solitary pulmonary nodules nave been discussed.ÖZETÇalışmamızda toraks bilgisayarlı tomografi tetkikleri sıra¬sında primer pulmoner malignensi olduğu düşünülen veya soliter pulmoner nödülü olan 83 hasta incelendi. Bu grupta yer alan 23 hastaya bilgisayarlı tomografi yol göstericiliğinde transtorasik iğne aspirasyon biopsisi uygulandı. Secondary Hyperparathyroidism and Depression in Renal FailureDepression is the most commonly described psychological disorder seen in chronic renal failure (CRF) patients. Apart from different psychological theories about the pathogenesis of depression in CRF, the literature also refers to organic factors, because CRF is accompanied by various pathobiocemical changes. Parathyroid hormon is one of the most important uremic toxin in CRF. But there are few and conflicting results about the secondary hyperparathyroidism (SHP) and depression in CRF. This study was aimed at studing the relation between SHPT and depression in CRF.In this study 31 CRF patients (18 male and 13 female) were studied. The "Hamilton Depression Scale" and the Mini Mental Test were performed. On the same day serum values of BUN, Creatinine, electrolytes and PTH were measured. Serum PTH levels were higher in the patients with depressive CRF than non-depressive CRF (p0.05).As a result of our findings it has been concluded that high levels of PTH can play an important role in the pathogenesis of depression in CRF. We purpose keeping SHPT under control rather than giving nonspecific antidepressants in the treatment of this type of depression. Further controlled studies are indicated to enlighten the psychopathology of depression seen in CRF.Depresyon, kronik böbrek yetmezliği (KBY) olan hastalarda tanımlanan en yaygın psikiyatrik bozukluktur. Önemli bir üremik toksin olan paratiroid hormon (PTH) ile depresyon arasındaki ilişkiyi gösterir çok az ve çelişkili yayınlar vardır. Çalışmamızda KBY olan hastalarda depresyon ile sekonder hiperparatiroidi arasındaki ilişkiyi araştırmayı amaçladık.Çalışmada, yaş ortalaması 49.12+1.1 yıl olan 18 erkek ve 13 kadın, toplam 31 KBY'li hasta çalışmaya alındı. Hastalara Hamilton Depresyon Skalası (HAM) ve minimental test uygulandı. Depresyon tesbit edilen KBY'li hastalarda PTH düzeyleri, depresyonu olmayanlara kıyasla daha yüksek bulundu (p0.05).Çalışmamızda sekonder hiperparatiroidizmin depresyonun patogenezinde önemli bir rol oynayabileceği sonucuna varıldı. KBY'de görülen depresyon tedavisinde spesifik olmayan antidepresan ilaçlar yerine, sekonder hiperparatiroidinin kontrol altında tutulması önerilebilir. İleride daha geniş kontrollü çalışmalarla konunun fizyopatolojik olarak aydınlatılması yararlı olabilecektir.

SUMMARYSERUM LACTIC DEHYDROGENAZE, PROGESTERONE AND TESTESTERON LEVELS IN ACUTE MYOCARDIAL INFARCTIONIn order to study the predisposing effects of sex hormones in acute myocardial infarction, plasma lactic dehydrogenaze (LDH), progesteron and testesteron levels were studied in 26 male patients who had acute myocardial infarction and 15 age-matched controls.Mean serum LDH level was significantly elevated in patient group (p < 0.05). Serum Progesterone level was significantly higher in the first day of myocardial infarction than the second and third day of myocardial infarction and the control group (p < 0.05). There was no significant difference in the serum progesterone level in the second and third day of myocardial infarction and the control group (p > 0.05). There was no significant difference in testesteron levels between two groups (p >0.05).Serum LDH Levels were significantly and positively correlated with serum progesteron levels in the first day of myocardial infarction (r=0.43, p 0.05). Serum testesteron düzeyleri bakımından hasta ve kontrol grubu arasında önemli farklılık bulunamadı (p>0.05).Akut miyokard infarktüsünde serum LDH düzeyi ile progesteron arasında önemli pozitif bir korelasyon varken (r=0.43). Serum LDH ile serum testesteron düzeyleri arasında önemli bir korelas¬yon yoktu (r=0.35).

SUMMARYLARGE SUBSTERNAL GOITER WITH HYPERTHYROIDISM, TRACHEAL COMPRESSION, VENA CAVA SUPERIOR AND HORNER'S SYNDROMESSubsternal goiter is an important cause of mediastinal masses. In this article, we presented a patient with hyperthyroidism, tracheal compression, vena cava superior syndrome and Horner's syndrome due to large substernal goiter and we reviewed substernal goiters. US and CT EVALUATION OF RENAL MULTICYSTIC TRANSFORMATION M PATIENTS UNDER HEMODIALYSISt/ 31 patients on maintenance hemodialysis therapy are examined with renal ultrasonograp¬hy (US) and computed tomography (CT). Patients having renal cysts prior to hemodialysis or polycystic renal disease are excluded from the study. Renal cysts are detected in 16 (54.8%). patients by US and in 18 patients (58.1%) by CT. It is concluded that there is no significant difference between the two methods in evaluating the renal multicystic trans-formation. The incidence of renal multicystic transformation is parallel to duration of he-modialysis, but irrelevant to age.HEMODİYALİZ HASTALARINDA GELİŞEN RENAL MULTİKİSTİK TRANSFORMASYONUN US ve BT İLE DEĞERLENDİRİLMESİ%/ Hemodiyaliz (HD) ünitesinde sürekli tedavi gören olgulardan 31'ine renal ultrasonografï (US) ve bilgisayarlı tomografi (BT) tetkikleri uygulandı. Diyalize başlamadan böbreklerinde kist saptananlar ile polikistik böbrek hastalan çalışmaya dahil edilmedi. Olgulardan 17'sinde (%54.8) US ile, 18'inde (%58.1) BT ile böbreklerde multikistik transformasyon saptandı. Renal multikistik transformasyonun değerlendirilmesinde, US ve BT yöntemleri arasında anlamlı bir farklılığın olmadığı görüldü. Renal multikistik transformasyon gelişmesi hasta yaşıyla ilişkisiz olup hemodiyaliz süresiyle anlamlı bir paralellik göster¬mektedir.

Musculoskeletal System Signs In Chronic Haemodialysis Patients In order to determine the prevalence and distribution of locomotor system signs and symptoms in haemodialysis patients we conducted a survey in haemodialysis centre at Ondokuzmayıs University. Thirthy patients, dialysed over a long period have been exa¬mined clinically and radiologically to evaluate their musculoskeletal problems. There were 17 women (%56.6) and 13 men (%43.3). The mean age of the patients was 34+2.5 (range 15-60). The cause of the renal failure was chronic glomerulonephritis (36.6%), renal anomaly (30%), renal tuberculosis (6.6%), nephrolithiasis (6.6%), chronic pyelo¬nephritis (3.3%), amyloidosis (3.3%) or was unknown (13.3%). The average duration of dialysis at the beginning of the study was 60.7+7.6 months (range 6-120). All of the patients were dialysed with cuprophane membranes. At the end of the study we found that 24 patients (80%) had muscle cramps and 14 patients (46.6%) had arthralgia. Ra¬diological abnormalities were found in 14 patients (46.6%). The 14 patients with arti¬cular involvement were compared with the 16 dialysis patients free of these clinical abnormalities. The affected patients were significantly older at the start of the dialysis than the unaffected patients. Pulmonary AmyloidosisAmyloidosis results from the deposition of insoluble, fibrous amyloid proteins, nearly always in the extracelluler species of organs and tissues. Amyloid deposition may occur in association with inflammatory, hereditary or neoplastic conditions. It may develop as part of a disorder of Immunglobulins. Virchow first described amyloid involving the lungs in 1857. Pulmonary amyloidosis may be localized to be respiratory tract or may be part of a widespread process involving many organs. Localized pulmonary amyloidosis is a rare disorder that appears in four forms; tracheobronchial, nodular parenchymal, diffuse interstitial and senile pulmonary amyloidosis.Amiloid birçok doku ve organda ekstraselüler biriken, fibriller yapıda, anormal bir proteindir. Amiloid birikimi inflamatuvar, kalıtsal veya neoplastik hastalıklarla ilişkili olabilir. Immunglobulin bozukluklarının bir parçası olarak da gelişebilir. Hepatorenal syndromes/ Hepatorenal syndrome (HRS) is a progressive, functional renal failure seen in patients witli severe liver diseases as advanced cirrhosis and fulminant hepatitis. The pathogenesis of HRS is not cleariy understood, reduced effective plasma volume and intense renal corti¬cal vasoconstriction seem to have important roles. The diagnosis of HRS is confirmed by elimination of the other cautions of renal failure. The most common conditions to be con¬sidered in the differential diagnosis are prerenal azotemia and acute tubular necrosis. The prognosis of HRS is poor. The management of HRS is to identify and correct the precipita¬ting factors leading to HRS, and it is important to avoid potential hepatotoxic and nephro¬toxic drugs. The reversible cautions of liver diseases should be treated, and hypovolemia should be corrected. Dialysis may benefit patients with fluid overload and electrolyte im¬balance or awaiting liver transplantation. Ño one of the drugs were used in the treatment of HRS has benefit continuously. Liver transplantation is the only curative therapy avai-lable at present.Hepatorenal sendrom (HRS); şiddetli karaciğer sirozu ve fulminant hepatit gibi ciddi kara¬ciğer hastalıklarında ortaya çıkan ilerleyici, fonksiyonel bir böbrek yetmezliğidir. HRS tanısı diğer böbrek yetmezliği yapan hastalıklar ekarte edilerek konur. Ayırıcı tanıda en sık dikkat edilmesi gereken durumlar; prerenal azotemi ve akut tubuler nekrozdur. HRS'un prognozu kötüdür. Tedavide HRS'a yol açan presipite edici faktörleri tanımak ve düzeltmek, he-patotoksik ve nefrotoksik ilaçlardan kaçınmak önemlidir. Karaciğer hastalığının reverzibl nedenleri tedavi edilmeli, hipovolemi düzeltilmelidir. Diyaliz, aşın sıvı yüklenmesi veya elektrolit imbalansı olan ya da karaciğer transplantasyonu için bekleyen hastalarda yararlı olabilir. HRS tedavisinde kullanılmış olan farmakolojik ajanların hiçbiri ile sürekli yarar sağlanmamıştır. Bugün için karaciğer transplantasyonu mevcut tek küratif tedavi yöntemidir.

Evaluation of serum thyroid hormones and. Cortisol levels in patients with type 11 diabetes mellutus.v? Changes in thyroid hormone levels in many euthyroid patients with nonthyroidal diseases have been reported. This study is planned to show the effects of clinical picture on cortisol and thyroid hormones in patients with type II diabetes mellitus. 13 patients with type II dia¬betes mellitus without complications, 31 with chronic complications, 22 with acute complica¬tions and II control subjects are studied and results are compared. Serum T4. fT4 and TSH levels did not differ satistically in all 4 groups, fT3 levels were significantly lower in all diabet¬ic groups than the control group, the lowest levels in the acute complication group, T3 levels were significantly lower in patients with acute complications than the other 3 groups. There was a negative correlation between blood sugar levels and T3, fT3 and fT4 levels and a positive correlation between. cortisol and blood sugar levels. A negative correlation existed between cortisol and T3 levels. Tiroid dışı akut veya kronik hastalığı olan bazı hastalarda serum tiroid hormonlarında değişiklikler olduğu gösterilmiştir. Diabetli hastalarda klinik tablonun bu hormonlar ve korti¬zol üzerine etkisi olup olmadığını göstermek amacıyla bu araştırma planlandı. Hiç komplikasyonu olmayan 13, kronik komplikasyonu olan 31, akut komplikasyonlu 22 tip II -diabetik has¬ta ile 11 kişilik kontrol grubunun değerleri karşılaştırıldı. Serum T4, serbest T4 ve TSH düzeyleri arasında 4 grupta da fark yoktu. Serbest T3 düzeyleri bütün diabetik gruplarda kontroldan düşük bulundu, en düşük akut komplikasyonlu grupta idi. Akut komplikasyonlularda serum T3 düzeyi diğer 3 gruptan önemli derecede düşüktü. Kan şekeri ile T3, serbest T3 ve serbest T4 arasında negatif korrelasyon, kortizol ve kan şekeri arasında pozitif korrelasyon bulundu. Kortizol ve T3 arasında bir negatif korrelasyon vardı.

Amyloidosis and The Gastrointestinal TractSystemic amyloidosis is not a single disease but results from a variety different diseases. Involvement of the gastrointestinal tract is commonly reported in systemic amyloidosis. Autopsy studies show amyloid deposition in the gastrointestinal tract and liver, %70 to %100. It may cause abdominal pain, vomiting, anorexia, diarrhea, constipation and gastrointestinal bleeding. Systemic amyloidosis affects every part of the gastrointestinal tract from the mouth to the anus.Sistemik amiloidoz tüm sistemleri etkileyebilir. Sistemik amiloidozda gastrointestinal tutulum sıktır. Otopsi çalışmalarında %70 ile %100 arasında gastrointestinal tutulum gösterilmiştir. Amiloid gastrointestinal sistemin tüm bölümlerini, etkileyebilir. Abdominal ağrı, bulantı, kusma, diyare. kilo kaybı, anoreksi, konstipasyon, üst ve alt gastrointestinal kanamaya neden olabilir. Motilite bozukluğu sıktır.

SUMMARYCOMPUTED TOMOGRAPHY OF THE MEDİASTİNAL MASSES:53 CASESIn this study, CT findings of 53 patients with mediastinal masses diagnosed between 1986 and 1988 were evaluated. Of these patients, 22 cases had lymphadenopathy, 8 cases had retrosternal goiter, 7 cases had esophagus tumor, 4 cases had thymoma, 3 cases had thymus hyperplasia, 4 cases had epypericardial fat pad, 1 case had pericardial cyst 3 cases had aortic aneurysms and 1 case had rhabdomyosarcoma.ÖZETBu çalışmada, 3986 ile 1988 tarihleri arasında mediastinal kitle ön tanısı alan 53 hastanın BT bulguları incelendi. Bunlardan 22 sinde lenfadenopati, 8 inde retrosternal guatr, 7'sinde özefagus tümörü, 4'ünde timoma, 3'ünde timus hiperplazisi, 4'ünde epiperikardial yağ yastıkçığı, 1'inde perikardial kist, 3'ünde aort anevrizması ve 1 inde rabdomyosarkom izlendi. Uremic EncephalopathyUremic encephalopathy (UE) is a clinical syndrome that was frequenfly seen in patients with end stage renal disease. Pathophysiology is multifactorial and partially characterised. Patients may have symptoms ranging from mild sensorial clouding to coma. Because of those, clinical symptoms like disorder of consantration, decrease in attention span, unwillingness and debility which may be seen on early stage couldn't realised by the patient's family or the clinician or was thought to be the clinical features of the primary disease. With renal transplantation and advanced dialysis techniques UE becomes less frequent but also different clinical features present as a problem. For these reasons, good cooperation between nephrologists and neurologists who care for these patients are important. This article consantrates on recent developments in pathophisyology, clinics, diagnosis and differential diagnosis of UE.Üremik ensefalopati (ÜE) son dönem renal yetmezlikli hastalarda sık rastlanan bir klinik sendromdur. Patofizyolojisi multifaktöriyeldir ve tam açıklık kazanmamıştır. Hastalarda hafif mental değişiklerden komaya kadar varan klinik özellikler gözlenebilir. Bu nedenle ÜE'de hastada erken dönemde görülen konsantrasyon bozukluğu, dikkat eksikliği, isteksizlik ve halsizlik gibi belirtiler hasta yalanlan tarafından ya da takip eden hekim tarafından farkedilmeyebilir ya da primer hastalığın bir sonucu olarak düşünülebilir. Renal transplantasyon ve gelişen diyaliz teknikleri sonucu ÜE görülme sıklığı azalmaktayken, tedavi komplikasyon-lan olarak farklı klinik tablolar sorun olarak karşımıza çıkmaktadır. Bu nedenle renal yetmezlikli bir hastanın takibini üstlenen nefrologlar ve nörologlar arasında sıkı bir işbirliği olmalıdır. Bu bakımdan; bu makalede, ÜE patofizyolojisindeki son gelişmeler, kliniği, tanı ve ayırıcı tanı yöntemleri gözden geçirilmiştir.

Antifosfolipid sendromu (AFS), yükselmiş antifosfolipid antikor titrelerinin eşlik ettiği arteriyel ya da venöz tromboz ve tekrarlayan fetal kayıp kombinasyonu ile karakterize sis-temik otoimmün bir bozukluktur. İki formu tanımlanmıştır: başka hiç bir hastalığın eşlik etmediği "primer sendrom" ve çoğunlukla SLE ile birlikte olan sekonder sendrom. 1998 yılında Sappora'da Uluslararası Uzmanlar Görüş panelinde AFS tanı kriterleri belirlenmiştir. Hasta plazmalarında antifosfolipid antikor varlığını göstermede iki ölçümden yararlanılır; ELİSA ile gösterilen antikardyolipin antikorları ve koagülasyon testleri ile taranan lupus antikoagülanı. Tedavi karar 4 ana grupta verilir: profilaksi, önleme, akut trombozun tedavisi ve antifosfolipid antikor ile birlikte olan gebeliğin tedavisi.Primary Anti-Phospholipid SyndromeAntiphospholipid syndrome (APS) is a systemic autoimmune disorder characterized by a combination of arterial or venous thrombosis and recurrent fetal loss, accompanied by elevated titers of antiphospholipid antibodies (aPL). Two forms have bee described: the "primary syndrome" where there is no evidence of an underlying disease, and the "secondary" syndrome, mainly in the setting of systemic lupus eryhematosus. In 1998; an international panel of experts met in Sapporo to establish the classification criteria for definite antiphos-pholipid syndrome. Two assays are available to demonstrate the presence of antiphospho-lipid antibodies in plasma of patients: ELİSA for the detection of anticardiolipin antibodies and coagulation assays for the detection of the lupus anticoagulants. Treatment decisions fall into four main areas. Prophylaxis, prevention, treatment of acute thrombosis, and management of pregnancy in association with antiphospholipid antibodies.

Rupture of extensor pollicis longus tendon (EPL) is rare. It can be caused by non-traumatic conditions as well as acute traumatic disorders. EPL tendon rupture has been reported as a sequela of chronic tenosynovitis secondary to rheumatoid arthritis and as a late complication of distal radius fracture. Here we present a case with acute spontaneous EPL tendon rupture with no obvious underlying etiological factor. Tendon rupture was repaired by transfer of extensor indicis proprius tendon. Snapping scapula syndrome is an infrequently described clinical condition characterized bt audible and/or palpable crepitation of scapulothoracic articulation. Pain may or may not accompany the movements of the affected scapula. Among etiological factors skeletal and soft tissue abnormalities are accused. In approximately 30% of cases no underlying abnormality can be demonstrated. In this article we presented two cases with SSS and reviewed the etiological factors and treatment options of this rare clinical entity.

Mit der Zunahme der Bedeutung von Kooperationen, Netzwerken, Allianzen und Partnerschaften steigt auch das Interesse an einem effizienten Management solcher Organisationsformen. Noch sind die Instrumente und Verfahren ebenso unzureichend entwickelt und ausdifferenziert wie ein umfassendes Problembewusstsein vieler Kooperationspartner. Doch gearbeitet wird an vielen Stellen, so dass davon ausgegangen werden kann, dass Fortschritte nicht ausbleiben werden. Nicht nur die Praxis ist an der Entwicklung von Konzepten und Instrumenten des Managements von Netzwerken interessiert, sondern auch die wissenschaftliche Erforschung von Kooperationen schreitet fort. Auch hier sind viele Facetten noch unterbelichtet, dennoch mehren und schärfen sich die Erkenntnisse. Dabei kann nicht überraschen, dass immer noch Fragen der Definition, der Abgrenzung und der paradigmatischen Einordnung kontrovers erörtert werden. Zu wenig konturiert ist die Theorie der Netzwerke bislang, als dass diesbezüglich alle Fragen geklärt wären. Dies korrespondiert mit der facettenreichen Ausprägung der kooperativen Ökonomie. Mit solchen grundlegenden Fragen sowie der Einordnung und Behandlung des Kooperationsmanagements in der Literatur setzt sich dieses IfG-Arbeitspapier von Cengiz Iristay auseinander. Es ist in den Forschungscluster Unternehmenskooperationen einzuordnen.

Objective: The aim of this study was to evaluate the complications of physical therapeutic modalities (PTMs) and patient satisfaction during and after physical therapy (PT) in our institution. Materials and methods: One hundred consecutive patients (64 female, 36 male), who underwent PT were enrolled in this study. A questionnaire involving demographic data, complications of PTMs and complaints of patients was applied to all patients. PTMs and patient numbers were as follows; hot pack for 86 patients, ultrasound for 69, interferential current for 56, diadynamic electrotherapy for 37, short wave diathermy for 26, whirlpool for 5 and cold pack for 2 patients. The patients' disorders were as follows respectively; 44 patients had lumbar or cervical spondylosis, 18 knee osteoarthritis, 15 soft tissue disease, 14 shoulder periarthritis, 9 cerebrovascular disease and 6 joint contractures. Results: The mean age was 51.14±13.42 years. Pain (n=4), tachycardia (n=2), bleeding (n=1) allergic reaction (n=1), hypertension (n=1) and bulla (n=1) were documented as PTMs complications. There was no statistically significant relationship between the occurrence of complications and patients' satisfaction (p>0.05). We did not find any significant relationship between the primary disorders and the complications, and PTMs and the complications (p>0.05). Conclusion: The PTMs are safe and did not raise serious complications when used by experienced physiotherapists. Patients were satisfied with their therapy as well.

Antiphospholipid syndrome (APS) is a systemic autoimmune disorder characterized by a combination of arterial or venous thrombosis and recurrent fetal loss, accompanied by elevated titers of antiphospholipid antibodies (aPL). Two forms have bee described: the "primary syndrome" where there is no evidence of an underlying disease, and the "secondary" syndrome, mainly in the setting of systemic lupus eryhematosus. In 1998; an international panel of experts met in Sapporo to establish the classification criteria for definite antiphospholipid syndrome. Two assays are available to demonstrate the presence of antiphospholipid antibodies in plasma of patients: ELISA for the detection of anticardiolipin antibodies and coagulation assays for the detection of the lupus anticoagulants. Treatment decisions fall into four main areas. Prophylaxis, prevention, treatment of acute thrombosis, and management of pregnancy in association with antiphospholipid antibodies.

The aim of this study is to determine the demographic and clinical features and compare them according to disability, anxiety and depression in patients with Failed Back Surgery Syndrome (FBSS). There were 35 patients (25 female, 10 male) in the study. To evaluate the social and demographic features of the patients, a semi structured form was used analysing Visual Analog Scala (VAS), Oswestry Disability Index (ODI), Hospital Anxiety and Depression Scala (HAD). Uremic encephalopathy (UE) is a clinical syndrome that was frequently seen in patients with end stage renal disease. Pathophysiology is multifactorial and partially characterised. Patients may have symptoms ranging from mild sensorial clouding to coma. Because of those, clinical symptoms like disorder of consantration, decrease in attention span, unwillingness and debility which may be seen on early stage couldn't realised by the patient's family or the clinician or was thought to be the clinical features of the primary disease. With renal transplantation and advanced dialysis techniques UE becomes less frequent but also different clinical features present as a problem. For these reasons, good cooperation between nephrologists and neurologists who care for these patients are important. This article consantrates on recent developments in pathophisyology, clinics, diagnosis and differential diagnosis of UE.

PubMed: 12090317 Patients with chronic renal failure (CRF) have a high incidence of malignant tumours. Uremia thus induces a remarkable suppression of immune status. In this study, we report that within the last 20 years, 188 (6.7%) various organ tumours were found in 2817 CRF patients that were hospitalised and treated. 69 (36.7%) of 188 patients with various organ tumours were on hemodialysis and the rest (63.3%) were CRF without hemodialysis. 49 (71%) of the 69 patients with hemodialysis were diagnosed with tumours in the first year of the hemodialysis therapy. In 110 (84%) of 119 CRF patients tumours were detected in less than 10 years after diagnosis of CRF. Localisation of the tumours were: 39 (19%) urologic malignancy, 30 (16%) parathyroid adenoma, 28 (15%) skin malignancy, 19 (10%) gynaecologic malignancy, 18 (9.5%) gastrointestinal tract (GIT) malignancy, 17 (9%) lung malignancy, 17 (9%) larynx malignancy, 13 (7%) thyroid malignancy, 5 (2.6%) multiple myeloma and 5 (2.6%) malignant lymphoma. No patients in the series had received a transplant kidney or were therapeutically immunosuppressed for other reasons and obstructive uropathy. CRF patients have a several times greater risk of developing malignant tumour than the general population.

PubMed: 12090320 Leptospirosis is an infectious disease caused by pathogenic leptospires and is characterized by a broad spectrum of clinical manifestations, varying from inappearent infection to fulminant, fetal disease. Eighty-five to 90% of leptospirosis infections are self-limiting. However, 5-10% of infection by L.interrogans can cause renal tubular damage, microvascular injury, acute renal failure (ARF), and interstitial nephritis. We studied 36 patients with leptospirosis. Twenty-seven (65%) cases of 36 patients had ARF. Fourteen (51%) had nonoliguric ARF. In thirteen (48%) oliguria appeared on the third or fourth days of hospitalization. Serum BUN, creatinine, serum bilirubine, ALT, AST, potassium and thrombocytopenia levels were higher in oliguric than nonoliguric patients (p < 0.05). However, serum sodium, CPK levels were not different between oliguric and nonoliguric groups (p > 0.05). Thirteen patients (48%) needed in renal replacement therapy (RRT). 8 of them were treated by hemodialysis (HD) alone and 5 patients by HD in combination with hemoperfusion. Twenty-five patients (92%) recovered completely after 3-5 weeks. Two patients (7.4%) who had severe hepatorenal and hemorrhagic syndromes, died. We concluded that till now leptospirosis is actual problem for nephrologist in the developing countries because of very high percentage of renal disease, with good prognosis in patients without multiorgan failure and early treatment. Agricultural Research Foundation The diagnosis of ARF due to leptospirosis is supported by presence of high fever, intense muscle aching and jaundice. Renal involvement is frequent and hemodialysis is sometimes required. The pathogenesis of the tubular necrosis is unclear. Adequate therapy can prevent irreversible renal damage.

Amyloidosis results from the deposition of insoluble, fibrous amyloid proteins, nearly always in the extracelluler species of organs and tissues. Amyloid deposition may occur in association with inflammatory, hereditary or neoplastic conditions. It may develop as part of a disorder of immunoglobulins. Virchow first described amyloid involving the lungs in 1857. Pulmonary amyloidosis may be localized to be respiratory tract or may be part of a widespread process involving many organs. Localized pulmonary amyloidosis is a rare disorder that appears in four forms; tracheobronchial, nodular parenchymal, diffuse interstitial and senile pulmonary amyloidosis.

Systemic amyloidosis is not a single disease but results from a variety different diseases. Involvement of the gastrointestinal tract is commonly reported in systemic amyloidosis. Autopsy studies show amyloid deposition in the gastrointestinal tract and liver, %70 to %100. It may cause abdominal pain, vomiting, anorexia, diarrhea, constipation and gastrointestinal bleeding. Systemic amyloidosis affects every part of the gastrointestinal tract from the mouth to the anus.

Depression is the most commonly described psychological disorder seen in chronic renal failure (CRF) patinets. Apart from different psychological theories about the pathogenesis of depression in CRF, the literature also refers to organic factors, because CRF is accompanied by various pathobiocemical changes. Parathyroid hormon is one of the most important uremic toxin in CRF. But there are few and conflicting results about the secondary hyperparathyroidism (SHP) and depression in CRF. This study was aimed at studing the relation between SHPT and depression in CRF. In this study 31 CRF patients (18 male and 13 female) were studied. The 'Hamilton Depression Scale' and the Mini Mental Test were performed. On the same day serum values of BUN, Creatinine, electrolytes and PTH were measured. Serum PTH levels were higher in the patients with depressive CRF than non-depressive CRF (p < 0.001). But there was no relationship between depression and BUN, creatinine, electrolyte levels, sex and duration of the disease (p > 0.005). As a result of our findings it has been concluded that high levels of PTH can play an important role in the pathogenesis of depression in CRF. We purpose keeping SHPT under control rather than giving nonspecific antidepressants in the treatment of this type of depression. Further controlled studies are indicated to enlighten the psychopathology of depression seen in CRF.
