Abstract

GAW, acronym for Gamma Air Watch, is a Research and Development experiment in the TeV range, whose main goal is to explore the feasibility of large field of view Imaging Atmospheric Cherenkov Telescopes. GAW is an array of three relatively small telescopes (2.13 m diameter) which differs from the existing and presently planned projects in two main features: the adoption of a refractive optics system as light collector and the use of single photoelectron counting as detector working mode. The optics system allows to achieve a large field of view (24x24 squared degrees) suitable for surveys of large sky regions. The single photoelectron counting mode in comparison with the charge integration mode improves the sensitivity by permitting also the reconstruction of events with a small number of collected Cherenkov photons. GAW, which is a collaboration effort of Research Institutes in Italy, Portugal and Spain, will be erected in the Calar Alto Observatory (Sierra de Los Filabres - Andalucia, Spain), at 2150 m a.s.l.). The first telescope will be settled within Autumn 2007. Abstract

GAW, acronym for Gamma Air Watch, is a path-finder experiment to test the feasibility of a new generation of imaging atmospheric Cherenkov telescopes that join high flux sensitivity with large field of view capability. GAW is conceived as an array of three identical imaging telescopes disposed at the vertexes of an equilateral triangle, about 80 m side. Two main features characterize GAW with respect to all the existing and presently planned ground-based Cherenkov telescopes. The first difference concerns the optics system: GAW uses a Fresnel refractive lens (2.13 m diameter) as light collector instead of classical reflective mirror. The second main difference is the detection working mode used: the detector at the focal surface operates in single photoelectron counting mode instead of the usual charge integration one. The GAW array is planned to be located at the Calar Alto Observatory site, Spain, 2150 m a.s.l. During its first phase, only 6x6 degrees of the focal plane detector will be implemented; moving it along the field of view, the sensitivity of the telescopes will be tested observing the Crab Nebula with on-axis and off-axis pointing up to 20 deg and with energy threshold of 300 GeV and energy peak of 700 GeV. As path-finder, GAW will also monitor the Very High Energy activity of some flaring Blazars as well as will follow-up GLAST detections at high energies. In a second phase, the focal plane will be enlarged to cover a field of view of 24 deg (full); pointing along different North-South directions, GAW would reach a survey of 360x60 degrees region of the sky. Abstract

Background Neonatal units’ volume of activity, and other quantitative and qualitative variables, such as staffing, workload, work environment, care organization and geographical location, may influence the outcome of high risk newborns. Data about the distribution of these variables and their relationships among Italian neonatal units are lacking. Methods Between March 2010-April 2011, 63 neonatal intensive care units adhering to the Italian Neonatal Network participated in the SONAR Nurse study. Their main features and work environment were investigated by questionnaires compiled by the chief and by physicians and nurses of each unit. Twelve cross-sectional monthly-repeated surveys on different shifts were performed, collecting data on number of nurses on duty and number and acuity of hospitalized infants. Results Six hundred forty five physicians and 1601 nurses compiled the questionnaires. In the cross-sectional surveys 702 reports were collected, with 11082 infant and 3226 nurse data points. A high variability was found for units’ size (4–50 total beds), daily number of patients (median 14.5, range 3.4-48.7), number of nurses per shift (median 4.2, range 0.7-10.8) and number of team meetings per month. Northern regions performed better than Central and Southern regions for frequency of training meetings, qualitative assessment of performance, motivation within the unit and nursing work environment; mean physicians’ and nurses’ age increased moving from North to South. After stratification by terciles of the mean daily number of patients, the median number of nurses per shift increased at increasing volume of activity, while the opposite was found for the nurse-to-patient ratio adjusted by patients’ acuity. On average, in units belonging to the lower tercile there was 1 nurse every 2.5 patients, while in those belonging to the higher tercile the ratio was 1 nurse every 5 patients. Conclusions In Italy, there is a high variability in organizational characteristics and work environment among neonatal units and an uneven distribution of human resources in relation to volume of activity, suggesting that the larger the unit the greater the workload for each nurse. Urgent modifications in planning and organization of services are needed in order to pursue more efficient, homogeneous and integrated regionalized neonatal care systems.

Abstract

SummaryIn animals, defective brown adipogenesis leads to bone loss. Whether brown adipose tissue (BAT) mass relates to bone mineral density (BMD) in humans is unclear. We determined the relationship between BAT mass and BMD by cold-stimulated positron-emission tomography (PET) and dual-energy X-ray absorptiometry (DXA) in healthy volunteers. Higher BAT mass was associated with higher BMD in healthy women, but not in men, independent of age and body composition.IntroductionContrary to the traditional belief that BAT is present only in infants, recent studies revealed significant depots of BAT present in adult humans. In animals, defective brown adipogenesis leads to bone loss. While white adipose tissue mass is a known determinant of BMD in humans, the relationship between BAT and BMD in humans is unclear. We thus examined the relationship between BAT and BMD in healthy adults.MethodsBAT volume (ml) and activity (standard uptake value) were determined by 18F-fluorodeoxyglucose PET after overnight mild cold exposure at 19 °C, and BMD was determined by DXA.ResultsAmong 24 healthy adults (age 28 ± 1 years, F = 10), BAT volumes were 82.4 ± 99.5 ml in women and 49.7 ± 54.5 ml in men. Abstract

OBJECTIVE: The selenoenzyme type 2 iodothyronine 5' deiodinase (DII) catalyzes the conversion of thyroxine into its active form tri-iodothyronine (T3), modulating thyroid hormone homeostasis in a local, tissue-specific manner. The amphibian, rodent and human cDNAs encoding this enzyme have been recently cloned and expressed. At present, little information regarding the genomic structure of mammalian DII is available. DESIGN AND METHODS: The complete structure, including intron-exon junctions, of the human DII (hDII) gene was obtained by long PCR and rapid amplification of cDNA ends (RACE). Chromosomal assignment of the hDII gene was performed by fluorescence in situ hybridization using a highly specific probe. RESULTS AND CONCLUSIONS: Our data demonstrated that hDII is a single copy gene located on chromosome 14, position 14q24.3. The gene spans over 15 kb, and the 7 kb transcript is encoded by three exons of 149 bp, 273 bp and 6.6 kb separated respectively by two 274 bp and 7.4 kb introns. A restriction map of the hDII gene is also reported. These data will help in further studies of the role of DII in the maintenance of peripheral thyroid hormone homeostasis.

Abstract

Weight loss intervention is the principal nonpharmacological method for prevention and treatment of type 2 diabetes. However, little is known whether it influences insulin sensitivity directly or via its anti-inflammatory effect. The aim of this study was to assess the independent role of changes in inflammation status and weight loss on insulin sensitivity in this population. Overweight and obese nondiabetic participants without co-morbidities underwent a one-year weight loss intervention focused on caloric restriction and behavioral support. Markers of inflammation, body composition, anthropometric para meters, and insulin sensitivity were recorded at baseline, 6, and 12 months. Insulin sensitivity was assessed with frequently sampled intravenous glucose tolerance test and Minimal Model. Twenty-eight participants (F: 15, M: 13, age 39 ± 5 years, BMI 33.2 ± 4.6 kg/m2 ) completed the study, achieving 9.4 ± 6.9 % weight loss, which was predominantly fat mass (7.7 ± 5.6 kg, p < 0.0001). Dietary intervention resulted in significant decrease in leptin, leptin-to-adiponectin ratio, hs-CRP, and IL-6 (all p < 0.02), and improvement in HOMA-IR and Insulin Sensitivity Index (SI) (both p < 0.001). In response to weight loss IL-1α, IL-2, leptin, and resistin were significantly associated with insulin, sensitivity, whereas sICAM-1 had only marginal additive effect. Moderate weight loss in otherwise healthy overweight and obese individuals resulted in an improvement in insulin sensitivity and in the overall inflammation state; the latter played only a minimal independent role in modulating insulin sensitivity. Abstract

Aim of the present study was to assess the increase of peripheral white blood cell (WBC) count and its clinical and prognostic significance in patients with cerebral ischemic infarction (CI). Thirty-six subjects (17 males, 19 females; mean age 73 +/- 10 years) with CI confirmed at CT scans were studied. A significant increase of WBC, not related to complicating infections, was observed in the early phase of CI. There was a significant linear correlation between the level of serum creatine phosphokinase and the number of circulating WBC which were higher in patients who died 6-30 days after the occurrence of CI. These data suggest that the increase in circulating WBC is dependent on the extent of the cerebral damage and should be considered as unfavourable prognostic sign.
