2. The effect of migration and time spent abroad on migrants’ health: A home/host country perspective

Abstract

BACKGROUND It is widely recognized that migrants are generally healthy upon arrival, but for several reasons, a longer length of stay abroad can have detrimental effects on health. Empirical evidence suggests the use of different comparison groups (natives in the destination country; co-nationals in the origin country) to analyse migrants’ health, depending on research aims and data availability. OBJECTIVE Using data from two nationally representative surveys, the Italian survey Social Condition and Integration of Foreign Citizens (2011–2012) and the Albanian Living Standard Measurement Survey (2012), this study aims to (1) analyse health differences between migrants abroad and non-migrants in their origin countries, focusing on the Albania–Italy migration corridor; and (2) assess health differences among Albanian migrants living in Italy according to their length of stay. METHODS We apply propensity score matching analysis to compare health outcomes between the two groups and use logistic regression models to investigate the effect of the length of stay in Italy on migrants’ health. RESULTS Our findings show that migrants exhibit poorer health compared to their co-nationals in the origin country and, in line with previous studies, that longer residence in Italy is associated with health deterioration. CONTRIBUTION This study is the first in Italy to contribute to a more comprehensive understanding of the relationship between migration and health. It does so by adopting a comparative home/host country perspective.

Abstract

Despite the growing interest in, and empirical research on, migrants’ health and well-being, few studies on these topics concern older migrants. Previous studies show that in addition to classical determinants, other migration-specific stressful factors negatively affect older migrants’ subjective well-being in the destination countries. In general, it is acknowledged that migrants at arrival have an initial advantage in terms of health compared to the native-born population, but migrants’ initial advantage is lost over time. The results concerning older migrants are mixed and strongly depend on the context, the migrant groups studied, and the health outcome considered in the analysis. Previous results show that migrants’ health at older ages depends not only on the number of years since migration but also on age upon arrival. Moreover, the empirical evidence show that older migrants have a higher life expectancy at age 65, lower rate of chronic disease but worse functioning and higher rates of disability. Previous studies have some limitations providing a fragmentation of country-specific studies, focusing on one or a few migrant groups in one destination country or analysing only one health dimension and limiting the analysis to cross-sectional data.

9. Labour market profiles of Albanian migrants in Italy: Evidence from Lombardy 2001–2015

Abstract

Albanian migration to Italy represents one of the most intense and iconic East-West migration flows of recent decades and a paradigmatic case of intra-Mediterranean migration. This paper uses a unique data set collected by the Lombardy region’s Observatory of Integration and Multiethnicity in 15 annual cross-sectional surveys over the period 2001–2015. Analysis of this data set enables us to address three questions: first, what are the main factors influencing the labour market performance of Albanian migrants in Lombardy compared with other migrants? Second, what were the differential effects of the post-2008 economic crisis on migrants’ employment stability? Third, were there any significant gender differences in labour market profiles and outcomes? Our main finding is that Albanian migrants have a lower probability of (long-term)unemployment and irregular employment and higher probability of employment compared with other migrants, an effect that is partially attributable to the different structural composition of the Albanian migrant group.

Abstract

Background: Direct-acting antivirals are highly effective for the treatment of hepatitis C virus (HCV) infection, regardless race/ethnicity. We aimed to evaluate demographic, virological and clinical data of HCV-infected migrants vs. natives consecutively enrolled in the PITER cohort. Methods: Migrants were defined by country of birth and nationality that was different from Italy. Mann-Whitney U test, Chi-squared test and multiple logistic regression were used. Results: Of 10,669 enrolled patients, 301 (2.8%) were migrants: median age 47 vs. 62 years, (p < 0.001), females 56.5% vs. 45.3%, (p < 0.001), HBsAg positivity 3.8% vs. 1.4%, (p < 0.05). Genotype 1b was prevalent in both groups, whereas genotype 4 was more prevalent in migrants (p < 0.05). Liver disease severity and sustained virologic response (SVR) were similar. A higher prevalence of comorbidities was reported for natives compared to migrants (p < 0.05). Liver disease progression cofactors (HBsAg, HIV coinfection, alcohol abuse, potential metabolic syndrome) were present in 39.1% and 47.1% (p > 0.05) of migrants and natives who eradicated HCV, respectively. Conclusion: Compared to natives, HCV-infected migrants in care have different demographics, HCV genotypes, viral coinfections and comorbidities and similar disease severity, SVR and cofactors for disease progression after HCV eradication. A periodic clinical assessment after HCV eradication in Italians and migrants with cofactors for disease progression is warranted.

Abstract

Ageing and migration are two of Europe’s fundamental demographic processes, challenging a wide range of social policies. Their intersection generates a wide taxonomy of types of individuals, from the most well-off and active ones to those more deprived and marginalised and for whom being old and having a migration background represents a double jeopardy. Yet in discussions of the ‘demographic time bomb’ of ageing, the special circumstances of ageing migrants are rarely recognised. Thus far, in Europe, very little is known about ageing populations with a migratory background, and this is even more true for Italy, that turned into an immigration country only in the late 1980s and where older migrants are a neglected, although growing, group in the Italian population. In this paper, we unpack ageing migrants’ wellbeing by focusing on life satisfaction, as a broader indicator of individual’s wellbeing; in particular, we analyse the effect of migration experiences on migrants’ perception of the quality of life in old age. We use data from the Social Condition and Integration of Foreign Citizens survey (2011–2012) and build several indicators related to different structural and socio-cultural dimensions of integration to analyse different patterns of association with life satisfaction among different groups of ageing migrants in Italy. Our study has important implications form both theoretical and policy perspective as it contributes to the advancement of original knowledge on the factors that shape wellbeing in later life in a context of migration.

Abstract

The aim of this study was to assess the impact of glucose control, diabetes-related complications and cardiometabolic risk factors on the risk of diabetic foot ulcers (DFUs) and DFU complications in Albanian adult inpatients with T2D.We conducted a retrospective case-control study on 482 Albanian adult inpatients with T2D. DFU was defined as a full-thickness skin lesion requiring ≥14 days for healing and was classified at the time of hospital admission. Demographic and biochemical parameters of the study participants, the presence of comorbidities and diabetes-related complications at the time of hospital admission were evaluated through a retrospective chart review.Mean age of study participants was 54.8±10.7 years. Participants (284 males and 198 females) were divided into two groups: DFU (cases; n=104) and non-DFU (controls; n=378). Multivariate analysis (performed by a logistic regression model) revealed that the most relevant independent variables associated with DFU were BMI [OR=0.62; p=0.007], HDL-cholesterol [OR=0.00; p0.0001], triglycerides [OR=7.48; p=0.0004], cigarette smoking [OR=26.46; p=0.005], duration of diabetes [OR=1.53; p0.0001], fasting plasma glucose (FPG) [OR=1.06; p0.0001], systolic blood pressure (SBP) [OR=1.13; p=0.0004] and insulin therapy alone [OR=0.11; p=0.02]. ROC curve analysis showed that FPG (AUC=0.83), glycated hemoglobin (HbA1c) (AUC=0.75), triglycerides (AUC=0.78) and HDL-cholesterol (AUC=0.82) were the most reliable biomarkers able to detect DFU. In the DFU group, the most relevant independent variables associated with previous minor lower-extremity amputations (LEAs) were represented by HbA1c [OR=1.47; p=0.03], age55 years [OR=0.12; p=0.05] and female sex [OR=4.18; p=0.03]; whereas the most relevant independent variables associated with diabetic peripheral neuropathy (DPN) were HbA1c [OR=1.70; p=0.006], SBP [OR=1.08; p=0.05], BMI [OR=1.20; p=0.03] and lack of cigarette smoking [OR=0.07; p=0.01]. Correlation analysis (performed through the nonparametric Spearman's rank correlation test or through the parametric Pearson test, as appropriate) revealed a significant positive relationship between HbA1c and FPG (r=0.58; p0.0001), ulcer surface area (r=0.50; p0.0001), ulcer grade (r=0.23; p=0.02), minor LEAs (r=0.20; p=0.04), DPN (r=0.41; p0.0001), and metformin therapy alone (r=0.72; p0.0001). There was a significant inverse correlation between HbA1c and insulin therapy alone (r=-0.31; p=0.01) and combined metformin and insulin therapy (r=-0.60; p0.0001). Both DFU and non-DFU groups exhibited suboptimal mean LDL-cholesterol levels (100 mg/dl) and mean HbA1c values7.5%. Moreover, in DFU group HbA1c values were markedly elevated (≥10%) particularly in patients with a grade 3 ulcer and an ulcer surface area ≥4 cm2, as well as in patients with history of minor LEAs and in patients affected by DPN.The present study suggested that longer duration of diabetes, cigarette smoking, lower HDL-cholesterol levels, poor glucose control, and elevated triglyceride and SBP values may all represent major risk factors for the development of DFU in Albanian patients with T2D. Thus, community interventions and health policies aimed to improve the management of diabetes and related cardiometabolic risk factors should be urgently implemented in Albania, in order to prevent DFUs and other diabetes complications in patients with T2D.

Abstract

Background and aims: In patients with hepatocellular carcinoma (HCC), macrovascular invasion (MaVI) limits treatment options and decreases survival. Detailed data on the relationship between MaVI extension and patients' characteristics, and its impact on patients' outcome are limited. We evaluated the prevalence and extension of MaVI in a large cohort of consecutive HCC patients, analysing its association with liver disease and tumour characteristics, as well as with treatments performed and patients' survival. Methods: We analysed data of 4774 patients diagnosed with HCC recorded in the Italian Liver Cancer (ITA.LI.CA) database (2008-2018). Recursive partition analysis (RPA) was performed to evaluate interactions between MaVI, clinical variables and treatment, exploring the inter-relationship determining overall survival. Results: MaVI prevalence was 11.1%, and median survival of these patients was 6.0months (95% CI, 5.1-7.1). MaVI was associated with younger age at diagnosis, presence of symptoms, worse Performance Status (PS) and liver function, high alphafetoprotein levels and large HCCs. MaVI extension was associated with worse PS, ascites and greater impairment in liver function. RPA identified patients' categories with different treatment indications and survival, ranging from 2.4months in those with PS>1 and ascites, regardless of MaVI extension (receiving best supportive care in 90.3% of cases), to 14.1months in patients with PS 0-1, no ascites and Vp1-Vp2 MaVI (treated with surgery in 19.1% of cases). Conclusions: MaVI presence and extension, together with PS and ascites, significantly affect patients' survival and treatment selection. The decision tree based on these parameters may help assess patients' prognosis and inform therapeutic decisions.

Abstract

Objective: We estimated the cost consequence of Italian National Health System (NHS) investment in direct-acting antiviral (DAA) therapy according to hepatitis C virus (HCV) treatment access policies in Italy. Methods: A multistate, 20-year time horizon Markov model of HCV liver disease progression was developed. Fibrosis stage, age and genotype distributions were derived from the Italian Platform for the Study of Viral Hepatitis Therapies (PITER) cohort. The treatment efficacy, disease progression probabilities and direct costs in each health state were obtained from the literature. The break-even point in time (BPT) was defined as the period of time required for the cumulative costs saved to recover the Italian NHS investment in DAA treatment. Three different PITER enrolment periods, which covered the full DAA access evolution in Italy, were considered. Results: The disease stages of 2657 patients who consecutively underwent DAA therapy from January 2015 to December 2017 at 30 PITER clinical centres were standardized for 1000 patients. The investment in DAAs was considered to equal €25 million, €15 million, and €9 million in 2015, 2016, and 2017, respectively. For patients treated in 2015, the BPT was not achieved, because of the disease severity of the treated patients and high DAA prices. For 2016 and 2017, the estimated BPTs were 6.6 and 6.2 years, respectively. The total cost savings after 20 years were €50.13 and €55.50 million for 1000 patients treated in 2016 and 2017, respectively. Conclusions: This study may be a useful tool for public decision makers to understand how HCV clinical and epidemiological profiles influence the economic burden of HCV.

Abstract

OBJECTIVE:\ud We estimated the cost consequence of Italian National Health System (NHS) investment in direct-acting antiviral (DAA) therapy according to hepatitis C virus (HCV) treatment access policies in Italy.\ud \ud METHODS:\ud A multistate, 20-year time horizon Markov model of HCV liver disease progression was developed. Fibrosis stage, age and genotype distributions were derived from the Italian Platform for the Study of Viral Hepatitis Therapies (PITER) cohort. The treatment efficacy, disease progression probabilities and direct costs in each health state were obtained from the literature. The break-even point in time (BPT) was defined as the period of time required for the cumulative costs saved to recover the Italian NHS investment in DAA treatment. Three different PITER enrolment periods, which covered the full DAA access evolution in Italy, were considered.\ud \ud RESULTS:\ud The disease stages of 2657 patients who consecutively underwent DAA therapy from January 2015 to December 2017 at 30 PITER clinical centres were standardized for 1000 patients. The investment in DAAs was considered to equal €25 million, €15 million, and €9 million in 2015, 2016, and 2017, respectively. For patients treated in 2015, the BPT was not achieved, because of the disease severity of the treated patients and high DAA prices. For 2016 and 2017, the estimated BPTs were 6.6 and 6.2 years, respectively. The total cost savings after 20 years were €50.13 and €55.50 million for 1000 patients treated in 2016 and 2017, respectively.\ud \ud CONCLUSIONS:\ud This study may be a useful tool for public decision makers to understand how HCV clinical and epidemiological profiles influence the economic burden of HCV.

Abstract

Objectives The prevalence of sickle cell disease (SCD) in Spain is markedly inferior compared with other European and Mediterranean countries. However, the diagnosis of new patients with SCD is expected to increase. In this multicenter retrospective study, we analyze the hematopoietic stem cell transplantation (HSCT) results obtained in Spain. Methods Forty-five patients who underwent a matched sibling donor (MSD) HSCT between 1999 and 2018 were included. Primary endpoint was event-free survival (EFS), and secondary endpoints included acute and chronic graft-versus-host disease (GvHD) and overall survival (OS). Results Bone marrow was the most frequent stem cell source (93.3%). Most patients received a conditioning regimen based on busulfan and cyclophosphamide (69%). Cumulative incidence of grade III-IV acute GvHD and chronic GvHD was 6.8% (95% CI: 2.3%-20.1%) and 5.4% (95% CI: 1.38%-19.9%), respectively. EFS and overall survival (OS) at 3 years post-HSCT were 89.4% (95% CI: 73.9%-95.9%) and 92.1% (95% CI: 77.2%-97.4%), respectively. All patients aged

33. International retirement and later-life migrants in the Marche region, Italy: materialities of landscape, ‘home’, lifestyle and consumption

Abstract

Within the general framework of ‘lifestyle migration’, the paper explores three materialities associated with the arrival and settlement of British, German and Dutch later-life migrants in the Italian region of Marche, a relatively new ‘frontier’ region for international retirement migration. The first is about the aesthetics of landscape and the scenic and emotional qualities of the physical and social environment. The second concerns ‘home’, where we examine house types, property location and home-making practices in terms of ‘authenticity’, material objects and the cultivation of land for productive purposes. The paper's third thematic focus is on consumption patterns. Most of the 69 participants interviewed for this study hanker after what they perceive as a simpler, more genuine way of life, in tune with the surrounding mixed-farming agricultural environment and distinct from other regions where tourism has taken hold. Many grow their own produce, including some who have small vineyards and olive groves. They enjoy shopping in local markets, eating out in inexpensive local hostelries, visiting museums and cultural festivals, and exploring the many pretty villages and historic towns of the region. The participants embody later-life migration as ‘active ageing’, but those who are older and/or frailer must consider, often reluctantly, the reality of a less-active and more isolated life in the Italian countryside.

Abstract

Background and aims: In patients with hepatocellular carcinoma (HCC), macrovascular invasion (MaVI) limits treatment options and decreases survival. Detailed data on the relationship between MaVI extension and patients' characteristics, and its impact on patients' outcome are limited. We evaluated the prevalence and extension of MaVI in a large cohort of consecutive HCC patients, analysing its association with liver disease and tumour characteristics, as well as with treatments performed and patients' survival. Methods: We analysed data of 4774 patients diagnosed with HCC recorded in the Italian Liver Cancer (ITA.LI.CA) database (2008-2018). Recursive partition analysis (RPA) was performed to evaluate interactions between MaVI, clinical variables and treatment, exploring the inter-relationship determining overall survival. Results: MaVI prevalence was 11.1%, and median survival of these patients was 6.0 months (95% CI, 5.1-7.1). MaVI was associated with younger age at diagnosis, presence of symptoms, worse Performance Status (PS) and liver function, high alphafetoprotein levels and large HCCs. MaVI extension was associated with worse PS, ascites and greater impairment in liver function. RPA identified patients' categories with different treatment indications and survival, ranging from 2.4 months in those with PS > 1 and ascites, regardless of MaVI extension (receiving best supportive care in 90.3% of cases), to 14.1 months in patients with PS 0-1, no ascites and Vp1-Vp2 MaVI (treated with surgery in 19.1% of cases). Conclusions: MaVI presence and extension, together with PS and ascites, significantly affect patients' survival and treatment selection. The decision tree based on these parameters may help assess patients' prognosis and inform therapeutic decisions.

Abstract

Background and aims: Epidemiology of hepatocellular carcinoma (HCC) is changing in most areas of the world. This study aimed at updating the changing scenario of aetiology, clinical presentation, management and prognosis of HCC in Italy during the last 15 years. Methods: Retrospective analysis of the Italian Liver Cancer (ITA.LI.CA) database included 6034 HCC patients managed in 23 centres from 2004 to 2018. Patients were divided into three groups according to the date of cancer diagnosis (2004-2008, 2009-2013 and 2014-2018). Results: The main results were: (i) a progressive patient ageing; (ii) a progressive increase of non-viral cases and, particularly, of ‘metabolic’ and ‘metabolic + alcohol’ HCCs; (iii) a slightly decline of cases diagnosed under surveillance, but with an incremental use of the semiannual schedule; (iv) a favourable cancer stage migration; (v) an increased use of radiofrequency ablation to the detriment of percutaneous ethanol injection; (vi) improved outcomes of ablative and transarterial treatments; (vii) an improved overall survival (adjusted for the lead time in surveyed patients) in the last calendar period, particularly in viral patients; (viii) a large gap between the number of potential candidates (according to oncologic criteria and age) to liver transplant and that of transplanted patients. Conclusions: During the last 15 years several aspects of HCC scenario have changed, as well as its management. The improvement in patient survival observed in the last period was likely because of a larger use of thermal ablation with respect to the less effective alcohol injection and to an improved management of intermediate stage patients.

Abstract

Background: Direct-acting antivirals are highly effective for the treatment of hepatitis C virus (HCV) infection, regardless race/ethnicity. We aimed to evaluate demographic, virological and clinical data of HCV-infected migrants vs. natives consecutively enrolled in the PITER cohort. Methods: Migrants were defined by country of birth and nationality that was different from Italy. Mann-Whitney U test, Chi-squared test and multiple logistic regression were used. Results: Of 10,669 enrolled patients, 301 (2.8%) were migrants: median age 47 vs. 62 years, (p < 0.001), females 56.5% vs. 45.3%, (p < 0.001), HBsAg positivity 3.8% vs. 1.4%, (p < 0.05). Genotype 1b was prevalent in both groups, whereas genotype 4 was more prevalent in migrants (p < 0.05). Liver disease severity and sustained virologic response (SVR) were similar. A higher prevalence of comorbidities was reported for natives compared to migrants (p < 0.05). Liver disease progression cofactors (HBsAg, HIV coinfection, alcohol abuse, potential metabolic syndrome) were present in 39.1% and 47.1% (p > 0.05) of migrants and natives who eradicated HCV, respectively. Conclusion: Compared to natives, HCV-infected migrants in care have different demographics, HCV genotypes, viral coinfections and comorbidities and similar disease severity, SVR and cofactors for disease progression after HCV eradication. A periodic clinical assessment after HCV eradication in Italians and migrants with cofactors for disease progression is warranted.

37. Loneliness Among Migrants in Italy: Risks and Protectors

Abstract

In this study we focused on migrants’ loneliness, in order to unpack risks for, and protective factors against, loneliness among migrants in Italy. Our data come from the ‘Social Condition and Integration among Foreign Citizens’ survey conducted by ISTAT during 2011–2012 on a sample of 25,000 individuals living in a household with at least one foreign-born member. Our results show that economic resources and employment protect from loneliness feelings, whereas education does not. Family and social embeddedness and satisfaction with life are protective factors, whereas discrimination, language barriers, deprived neighbourhoods and poor health are associated with a higher risk of loneliness. Gender is a key lens to consider when analysing loneliness especially in relation to fragile populations like those with a migratory background.

Abstract

BACKGROUND AND OBJECTIVE: Patients with thalassaemia major (TM) and sickle cell disease (SCD) in Spain have been counted since the creation of the Spanish registry of haemoglobinopathies (REHem). The objective of this paper is to update the published data after the increase in cases due to the inclusion of adults and introduction of new-born screening in almost the whole country. MATERIAL AND METHODS: An observational, descriptive, multicentre and ambispective study that included patients with haemoglobinopathies registered in the REHem, started in January 2014 and followed up annually. The data presented correspond until December 31, 2017. RESULTS: Nine hundred and fifty-nine patients were collected. There were 75 cases of thalassaemia (62 TM), 826 of ECF and 58 of other types of haemoglobinopathies. The main diagnostic reason in the TM cohort was anaemia symptoms (70.6%), with a mean age at diagnosis of .7 years; in the SCD cohort it was neonatal screening (33.1%), with a mean age at diagnosis of 2.7 years; 26 patients with TM (41.9%) and 30 with SCD (3.6%) underwent a transplant. There were 2 deaths (3.2%) with TM and 19 (2.3%) with SCD. Overall survival was 96.7% in the TM and 97.5% in the SCD cases at 15 years. CONCLUSIONS: Since the previous publication and after the diffusion of new-born screening, the most frequent diagnostic method, to the majority of autonomous regions, and the inclusion of adult patients to the registry, the REHem has increased by more than 240 cases, reaching a total of 959 records.

48. The Marche: Italy's new frontier for international retirement migration

Abstract

Within the linked fields of population, migration, and ageing, international retirement migration (IRM) has emerged as a strong focus for empirical research. Thus far, the classic studies have been on European North–South IRM, with a specific concentration on iconic regions such as the Costa del Sol, southern France, and Tuscany. But the geography of IRM is constantly changing, as prior destination regions become “saturated” and perhaps too expensive, so that new “frontiers” are opened up. One such frontier is the Italian region of Marche, now seen as a cheaper and more “authentic” region than Tuscany. This paper uses interview narratives from 69 older age British, Dutch, and German people who have relocated, as either permanent settlers or seasonal residents, in the Marche, in order to answer two questions. The questions relate first to the main drivers of IRM at the decision‐making level and, second, to the experiences of living there, including advantages and disadvantages. We find the participants for the most part engaged in active ageing, growing their own produce, and joining in local community life and are appreciative of the beauty of the hilly landscape. However, “full” integration is hampered by language and cultural barriers, whereas the bureaucracy is their main complaint.

49. Ageing in a foreign country: determinants of self-rated health among older migrants in Italy

Abstract

Within a framework of increasing ethnic diversity of developed countries' population, a growing body of migration literature has focused on migrants’ health. Nonetheless, although ageing is a major demographic trend of western societies involving both natives and migrants, older migrants remain a relatively under-researched group so far. In Italy, despite the increasing incidence of migration on total population, its fast ageing process, and the acknowledged difficulties that migrants face in becoming full members of the Italian society, thus far, very little is known about the migrant population approaching old age. Our study focuses on the main protective and risk factors of health in later life using a unique dataset from the ‘Social condition and integration of foreign citizens’ (SCIF)’ survey conducted by ISTAT between 2011 and 2012. Our findings show that a number of variables related to both the migration process and living conditions in Italy are important determinants of self-rated health, stressing the necessity to design and implement tailored policy responses and services that address migrants’ vulnerabilities in later life.

Abstract

Capsicum products, such as chilli powder, paprika, sauces, and spice mixes, are consumed worldwide and become very important export products for Asian and Latin-American countries. Increased commercialisation of Capsicum products has triggered an alert, mainly due to the food safety and quality issues of these products, which have been introduced into the international market. Capsicum and derivative-products are substrates with unusual nutritional characteristics for microbial growth. Despite this, the presence of spoilage fungi and the co-occurrence of mycotoxins in the pepper production chain have been commonly detected. The production of pepper and pepper-based products which may be contaminated with mycotoxins represents a serious risk to consumer health, and also impacts negatively on this agribusiness sphere. In this context, the key aim of this work was to review the critical control points, with a focus on mycotoxin contamination, during the production, storage and distribution of Capsicum products from a safety perspective; outlining the important role of ecophysiological factors in stimulating or inhibiting mycotoxin biosynthesis in these food commodities. Moreover, the human health risks caused by the ingestion of peppers contaminated with mycotoxins are discussed.
