Subjective cognitive decline (SCD), which precedes Mild Cognitive Impairment and dementia, may be affected by purpose in life (PiL) and loneliness in older adults. We investigated associations among PiL, loneliness, and SCD in US Latino ( n = 126), Black ( n = 74), Asian ( n = 33), and White ( n = 637) adults. Higher PiL predicted lower SCD in all groups (p-values < .012), except Black participants. Lower loneliness predicted lower SCD in Latino and White groups (p-values < .05), and PiL moderated this association in White adults. PiL and loneliness may play important roles in cognitive decline. Differential predictors of SCD suggest differential targets for preventing cognitive decline and dementia across ethnoracial groups.

Resumen Introduccion y objetivos La actividad fisica tiene beneficios conocidos para numerosas enfermedades cronicas, incluyendo las demencias y las enfermedades relacionadas. Sin embargo, la evidencia disponible relacionada a la asociacion con el desempeno cognitivo o la tasa de declinacion cognitiva o conversion a demencia es contradictoria. El objetivo del presente trabajo fue realizar una revision sistematica del papel de la actividad fisica en la prevencion del deterioro cognitivo o demencia en adultos mayores. Metodos Se realizo una busqueda en las bases de datos Medline, PsycINFO y SciELO desde enero del ano 2000 hasta diciembre del 2018. La busqueda se focalizo en estudios que incluyeran datos de la tematica antes planteada en adultos mayores de 50 anos o mas cognitivamente normales, con deterioro cognitivo leve o demencia. Resultados Cuarenta articulos cumplieron con los criterios de inclusion. De acuerdo con su analisis, se puede inferir que existe evidencia cientifica de resultados positivos con respecto a la actividad fisica en el funcionamiento cognitivo, especialmente en cognicion global, memoria, atencion y funciones ejecutivas. La mayoria de los estudios incluidos demostraron una mejoria en las medidas cognitivas evaluadas. Sin embargo, se observo una gran heterogeneidad en el diseno de los estudios. Conclusiones En general, los hallazgos de nuestra revision sugieren que el ejercicio fisico puede proporcionar una estrategia ampliamente disponible para mejorar el funcionamiento cognitivo, especialmente de las funciones ejecutivas y memoria, y retrasar la aparicion de la demencia. La heterogeneidad de los estudios publicados hasta el momento no permite definir una recomendacion formal definitiva acerca de cuales serian las mejores propuestas en la practica clinica habitual. Sin embargo, un abordaje personalizado para cada paciente, teniendo en cuenta sus antecedentes clinicos y fisicos, como asi tambien sus preferencias y posibilidades, conjuntamente a una actividad fisica regular y sostenida, podria ser una recomendacion altamente aceptable hasta contar con nuevas y mejores evidencias.

Since 1 January 2020, the Central Research Ethics Committee of the Health Ministry implemented PRIISA.BA, an in-house developed electronic system for online submission of health research applications to the 63 public and private research ethics committees (RECs) of Buenos Aires City, Argentina. This study though to compare the times to first review and the time to approval among applications submitted prior to PRIISA.BA and thereafter, across public RECs. All public RECs of the city were invited to participate. Overall, 453 applications from 10 RECs (242 pre- and 211 post-PRIISA.BA) were available for the analyses. There was a decrease in the time to first review and an increase in the time to approval after PRIISA.BA implementation. The increase in time to approval was transient and limited to the first three months. The results were consistent with analyses limited to non-COVID applications. Our results show an increase in the times to approval after the implementation of an electronic system for online submission of health research applications that, although transient, was significant. These data could be relevant to other RECs implementing this technology since it emphasizes the need of monitoring potential unnecessary delays in reviews during the critical initial period.

Cognitive decline due to neurodegenerative diseases is a prevalent worldwide problem. Both pharmacological and non-pharmacological treatments to improve, delay or stop disease progression are of vital importance. Cognitive stimulation is frequently used in clinical practice; however, there are few studies that demonstrate its efficacy. Aim: To evaluate the efficacy of cognitive stimulation in patients with mild cognitive impairment (CDR = 0.5) and dementia (CDR = 1). Methods: A retrospective cohort study was performed. Patients with cognitive impairment receiving weekly cognitive stimulation (16 or 24 sessions) were evaluated with a complete neuropsychological battery before and after the stimulation program. Each stimulation session was carried out by a trained neuropsychologist. Results: Forty two patients receiving cognitive stimulation were evaluated over a period of 12.53 months (SD 5.5). Patients were grouped as 11 amnesic mild cognitive impairment (aMCI), 23 multi domain mild cognitive impairment (mMCI) and 8 Mild Alzheimer’s Dementia (CDR 1). None of the groups improved their cognitive functions after the cognitive stimulation program. MCI group was also divided according to their global intelligence quotient (IQ) into two groups: low (IQ < 98.5) and high (IQ > 98.5). Each group was compared before and after the stimulation program and no significant difference was found (p ≥ 0.05). Moreover, MCI group was also analyzed according to the duration of the stimulation program: less than 9, between 9 and 13 and more than 13 months. Different duration groups were compared before and after the cognitive stimulation program and no significant differences were found. Depression, anxiety and subjective memory symptoms were also analysed and neither improvement nor worsening could be demonstrated. Conclusions: Patients remained stable, both in cognitive and behavioural domains, for more than 18 months. However, no significant cognitive or behavioural improvement can be reported in these patients after the stimulation program (duration time: 12.53 months SD 5.5). Fil: Filipin, Federerico. Frontotemporal dementia (FTD) is the main differential diagnosis with early stages of Alzheimer’s disease (AD). Usually, differential diagnosis between a first depressive episode and the beginning of an early degenerative dementia with mood disorders, either AD or FTD, can be difficult. Objective: To evaluate the clinical characteristics of patients with senile and presenile onset dementia, to compare their neuropsychiatric and neuropsychological profiles according to onset age and to provide clinical approach. Methods: A two year prospective-retrospective study was conducted. All patients were evaluated with a complete neuropsychiatric and neuropsychological battery, laboratory tests and neuroimaging. Healthy control subjects were also studied. Results: Included 366 subjects were divided into over or under 65 years old, and then matched for educational level. AD was the most common cause of dementia in subjects over 65 years of age, followed by depression and FTD. Subjects younger than 65 years old, showed higher prevalence of depression followed by FTD, AD, and finally primary progressive aphasia (PPA). At younger ages, the highest severity of cognitive impairment, behavioral disorder and major depression were observed. Conclusion: Onset age of cognitive and/or behavioral impairment may be one of the variables influencing the clinical heterogeneity of dementias. Many of the young-onset dementias may be potentially reversible so, its early identification and pathophysiology understand, increase pharmacological intervention opportunities of halting the cascade of events that lead inexorably to dementia. In the new era of biomarkers, their help in identifying each clinical phenotype could encourage their best use in clinical practice and help selecting more accurate pharmacological treatment. Resumen Introduccion y objetivo La enfermedad de Alzheimer es una enfermedad neurodegenerativa progresiva y representa la causa mas comun de demencias, pues supone el 60% de todos los casos de demencia. Durante la ultima Conferencia de la Asociacion de Alzheimer Internacional 2011, se presentaron los resultados de una encuesta internacional realizada en 5 paises (EE. UU., Francia, Alemania, Polonia y Espana). El objetivo principal de nuestro trabajo fue aplicar una version simplificada y adaptada de la encuesta original con el fin de evaluar la percepcion y el grado de conocimiento publico acerca de la enfermedad de Alzheimer y el punto de vista con respecto al valor de hacer un diagnostico de la enfermedad. Sujetos y metodos En agosto de 2011 se aplico la encuesta a 360 sujetos en las salas de espera de FLENI y del Hospital Zubizarreta de Buenos Aires. La edad promedio fue de 50,33 ± 16,6 anos. El 68,9% (248) fueron mujeres. El 60% tuvieron menos de 12 anos de educacion. La enfermedad de Alzheimer fue la tercera causa (15%) que genera mayor temor despues del cancer (40%) y el accidente cerebrovascular (23%). Mas del 85% identificaron los sintomas comunes de la enfermedad. El 40% la considero una enfermedad fatal. El 90% quieren conocer el diagnostico, en caso de tener la enfermedad. El 25% cree que existe tratamiento efectivo para tratar la enfermedad. El 50% cree que hay una prueba diagnostica definitiva. Conclusiones La encuesta de opinion publica demuestra el temor y preocupacion con relacion a la enfermedad; la necesidad de brindar un diagnostico y de conseguir una educacion publica mejor.

Cognitive reserve is the ability to optimize performance through differential recruitment of brain networks, which may reflect the use of alternative cognitive strategies.To identify factors related to cognitive reserve associated with progression from mild cognitive impairment (MCI) to degenerative dementia.A cohort of 239 subjects with MCI (age: 72.2±8.1 years, 58% women, education: 12 years) was assessed and followed for five years (2001 to 2006).In the first year, 13.7% of MCI converted to dementia and 34.7% converted within three years (78.3% converted to Alzheimer's dementia). Risk factors for those who converted were education less than 12 years, MMSE score less than 27, Boston naming test score less than 51, IQ (Intelligence Quotient) less than 111, age over 75 years, lack of occupation at retirement, and presence of intrusions in memory recall (all account for 56% of the variability of conversion).MCI patients are a population at high risk for dementia. The study of risk factors (e.g. IQ, education and occupation), particularly those related to cognitive reserve, can contribute important evidence to guide the decision-making process in routine clinical activity and public health policy.Reserva cognitiva é a habilidade em otimizar o desempenho através do recrutamento de redes neurais, que talvez reflitam o uso de estratégias cognitivas alternativas.Identificar fatores relacionados à reserva cognitiva associados à progressão do comprometimento cognitivo leve (CCL) para demência degenerativa.Uma coorte de 239 indivíduos com CCL (idade: 72.2±8.1anos, 58% mulheres, educação: 12 anos) foram avaliados e seguidos por cinco anos (2001-2006).No primeiro ano 13.7% dos CCL converteram para demência e 34.7% em três anos (78.3% converteram para doença de Alzheimer). Os fatores de risco para aqueles que converteram foram: educação menor do que 12 anos, MMSE menor do que 27, teste de Nomeação de Boston menor do que 51, QI (Quociente de Inteligência) menor do que 111, idade superior a 75 anos, falta de ocupação na aposentadoria, e presença de intrusões na memória de evocação (todos contando para 56% da variabilidade de conversão).Pacientes com CCL são uma população de risco para demência. O estudo dos fatores de risco (como QI, educação e ocupação), principalmente, aqueles relacionados à reserva cognitiva podem contribuir para uma evidência importante para o processo de decisões na atividade clínica e na saúde pública.

Both patient groups were compared with normal subjects.Materials and methods: We recruited 76 patients with depressive symptoms (37 late onset and 39 early onset) and 17 normal controls matched by age and educational level. All subjects were assessed using a semistructured neuropsychiatric interview and an extensive neuropsychological battery. Vascular and sociological risk factors were also evaluated.Results: We found a significant variation in performance between depressive patients and normal controls in most cognitive functions, especially memory (P < 0.0001), semantic fluency (P < 0.0001), verbal fluency, and digit-symbol (P < 0.0001). Late-onset depression patients scored lower and exhibited more severe impairment in memory domains than early-onset depression patients (P < 0.05). Cholesterol levels and marital status were significantly (P < 0.05) different between the depressive groups. Both depressed groups (early- and lateonset) were more inactive than controls (P < 0.05; odds ratio: 6.02).Conclusion: Geriatric depression may be a manifestation of brain degeneration, and the initial symptom of a dementia. Mild cognitive impairment (MCI) is a transitional state between normal aging and dementia, at least for some patients. Behavioral symptoms in MCI are associated with a higher risk of dementia, but their association with dementia risk in patients without MCI is unknown. Mild behavioral impairment (MBI) refers to a late-life syndrome with prominent psychiatric and related behavioral symptoms in the absence of prominent cognitive symptoms that may also be a dementia prodrome. This study sought to compare MCI and MBI patients and to estimate the risk of dementia development in these 2 groups.Between January 2001 and January 2006, a consecutive series of 358 elderly (or= 65 years old) patients (239 with MCI and 119 with MBI) presenting to an outpatient general hospital specialty clinic were followed for up to 5 years until conversion to dementia or censoring.Thirty-four percent of MCI patients and over 70% of patients with MBI developed dementia (log-rank p = .011). MBI patients without cognitive symptoms were more likely to develop dementia (log-rank p.001). MBI patients were more likely to develop frontotemporal dementia (FTD) than dementia of the Alzheimer's type (DAT).MBI appears to be a transitional state between normal aging and dementia. MBI (specifically in those without cognitive symptoms) may confer a higher risk for dementia than MCI, and it is very likely an FTD prodrome in many cases. These findings have implications for the early detection, prevention, and treatment of patients with dementia in late life, by focusing the attention of researchers on the emergence of new behavioral symptoms.

El deterioro cognitivo leve es una entidad caracterizada por el compromiso predominante de la memoria en ausencia de trastorno funcional. Ha sido asociado a un período predemencial de la enfermedad de Alzheimer. Sin embargo, se considera que otras áreas cognitivas pueden verse afectadas, pudiendo implicar también otras etiologías. Así, algunos profesionales lo consideran un término etiológico, dirigido hacia la enfermedad de Alzheimer y otros un complejo sindromático, incluyendo varias enfermedades. Otro factor agregado que generó confusión ha sido el porcentaje de conversión a demencia. El objetivo de nuestro trabajo fue evaluar la opinión médica sobre el deterioro cognitivo leve y los problemas que genera en la práctica diaria. Se adaptó el cuestionario sobre deterioro cognitivo leve de Dubois y col, 2003; 24 médicos expertos en demencia de Argentina y Brasil (16 neurólogos y 8 psiquiatras) y 30 médicos generalistas aceptaron responder al mismo. Los resultados muestran que el 92% de los médicos expertos piensa que es una entidad heterogénea, no la limitan a un estadio predemencia de la enfermedad de Alzheimer; y el 63% refieren que puede empeorar. El 83% de los expertos inician tratamiento específico utilizando preferentemente anticolinesterásicos, memantine y vitamina E. La vasta mayoría de médicos considera al deterioro cognitivo leve como una entidad sindromática, que no está limitada a la enfermedad de Alzheimer; pero sin embargo, inicia tratamiento con drogas dirigidas hacia ella. La ambigüedad existente hace necesario estandarizar definiciones y reconceptualizar la enfermedad de Alzheimer en su estadio pre-demencia.Mild cognitive impairment (MCI) refers to persons who are slightly cognitively impaired for age but do not meet the criteria for dementia. MCI has been related to a pre-dementia stage of Alzheimer&'s disease (AD). However, other possible diagnoses such as cerebro-vascular disease, frontotemporal dementia or normal aging have been considered. Diagnosis, etiology and conversion to dementia are a source of ambiguity in MCI. The aim was to evaluate the opinion of experts on dementia and of general practitioners concerning MCI. A total of 24 experts from Argentina and Brazil (16 neurologists and 8 psychiatrists) and 30 general practitioners agreed to reply to a questionnaire on MCI (adapted from Dubois inventory, 2003). Of these, 92% of experts considered MCI as an ambiguous entity, not necessarily as a "pre-dementia" stage; 63% confirmed a tendency to worsen over the time and 83% of experts decided to initiate treatment using cholinesterase inhibitors, memantine and vitamin E. The opinion on MCI was that a priori it is not only an Alzheimer disease pre-dementia stage, but most of them consider the treatment against AD. MCI is a heterogeneous entity that should be classified as an open category and making it necessary to standardize definitions and design diagnosis guides to better understand Alzheimer disease pre-dementia stage.

Given the potential use of biomarkers in the diagnosis of Alzheimer's disease (AD) in early stages, new ethical and communication dilemmas appear in everyday clinical practice. The aim of this study was to know the opinion of health professionals (HP) and general public (GP) on the implementation of early diagnostic techniques in AD and the use of biomarkers for this purpose. A survey with multiple choice answers was elaborated in two versions: one for HP and the other for GP. Respondents were invited to participate through a system of mass mailing e-mail; e-mail addresses were collected from CEMIC database. A total of 1503 answers were analyzed: 807 HP and 696 GP. Most respondents, 84.7%, preferred the option of early diagnosis of AD even knowing the lack of curative treatment. Forty five percent of GP and 26.8% of HP replied that there is no ethical dilemma in the use of biomarkers and that no communication or ethical dilemma is generated to physicians when informing the diagnosis of the disease. The HP group showed more divergence in the views than the GP group. These results may indicate a change in the physician-patient relationship, showing the GP group with an active and supportive position towards the use of biomarkers for early diagnosis of AD.

Population aging has increased age-related diseases such as dementia, Alzheimer's disease (AD) being the most common in older adults (50%-60%). It is one of the most feared conditions for its irreversible and incurable, by its chronicity and it consists of a long process of depersonalization. The clinical diagnosis is mainly based on DSM-IV and NINCDS-ADRDA. Definitive diagnosis is post-mortem, as it requires histopathological confirmation. However, there have been new diagnostic criteria based EA biomarkers, which can be done in life, anticipating the course of several years before dementia. Thus, physicians are increasingly exposed to AD patients in early stages, having to face the dilemma of communicating the diagnosis to a patient with cognitive failures and disease awareness, with the implications that generates about him and his family. The objectives of this paper will analyze the ethical problem of communication of clinical diagnosis of AD in prodromal stages (pre-dementia) in our country, based on the analysis of ethical principles (autonomy and non maleficence) involved in decision making.

Os estados iniciais da demência tipo Alzheimer (DTA) caracterizam-se classicamente por deterioração da memória enquanto que as mudanças de conduta e de personalidade aparecem nas etapas iniciais da demência frontotemporal (DFT). Entretanto, na prática clínica, o diagnóstico diferencial é difícil. O objetivo do presente trabalho foi estudar o rendimento da memória de pacientes com DTA (n= 20) e com DFT (n= 20) comparando-o com um grupo de controles (n = 20). Os pacientes, emparelhados por idade e escolaridade, foram avaliados com uma bateria neuropsicológica exaustiva. Para a avaliação da memória, examinou-se a "queixa subjetiva" de perda de memória (memória subjetiva), a aprendizagem de uma lista de palavras (memória episódica) e o desempenho no teste de denominação de Boston (memória semântica). As pontuações de ambos os grupos de pacientes, na grande maioria das provas, foram significativamente inferiores às dos controles. Os pacientes com DTA mostraram uma deterioração global da memória episódica (tipo amnésia) e semântica com um alto nível de queixa subjetiva. Os sujeitos com DFT, por outro lado, apresentaram um déficit de memória importante na recuperação da informação, mas com melhores capacidades de registro da informação, apesar das dificuldades de reconhecimento do seu distúrbio. While in Dementia of Alzheimer's Type (DAT) the initial states are characterized by the deterioration of memory, in frontotemportal dementia (FTD) the early symptoms are basically changes in behavior. However, in daily clinical practice, the establishment of a differential diagnosis is very difficult to be found. The objective of the present work is to compare the memory efficiency of patients with DTA (n= 20) and FTD (n= 20) to a control group. The patients, matched for age and educational level, were evaluated with basis on a number of exhausting neuropsychology tests. In order to examine memory, we used the learning, free recall and recognition of a list of words (episodic memory), the analysis of the performance in the Boston Naming Test (semantic memory) and the subjects memory complaints (subjective memory). The scores obtained on the most part of the tests by the two groups (DAT and FTD) were significantly worse than those of the control group. DAT patients, on the other hand, showed an important memory deficit on free recall but less encoding capacities, despite the difficulties in acknowledging their disturbance. Memory profile of FTD patients was characterized by forgetfulness while DAT profile was an amnesic one.

Mild cognitive impairment (MCI) has 3 clinical subtypes: amnestic (aMCI), multiple domains (mdMCI) and non-amnestic single domain (na-SD-MCI) whose evolutive possibility to dementia has not been profoundly studied.This paper aims to determine the conversion to dementia of the different subtypes of MCI and determine risk factors associated to conversion to dementia.A total of 127 patients diagnosed with MCI (age=70.21; SD=13.17) were evaluated with a neuropsychological and neuropsychiatric battery. They were classified into 3 groups: amnestic MCI (n=20), multiple-domain MCI (n=98), non-amnestic MCI (n=9). Seventeen normal subjects (age=74.59; SD=10.63) were included.Of those included, 27.1% developed Alzheimer's type dementia [average time for conversion to Alzheimer's dementia (AD) 11.12 months (SD=0.183)]. None of the controls developed dementia. Thirty-five percent (n=7) of amnestic MCI converted to AD: 20% (n=4) at 6 months and 15% (n=3) at 12 months); 11.1% (n=1) of the non-amnestic single domain MCI converted to AD at 6 months. It was found that 31.6% (n=31) of multiple domain MCI rotated to AD: 15.3% (n=15) at 6 months and 16.3% (n=16) at 12 months. Age (p0.05, β=1.03) increased the likelihood of rotation to AD. Multi-domain MCI subtype was the most frequent. However, the conversion to dementia in amnestic subtype was the highest, age and retirement being the variables that increased the likelihood of conversion to Dementia.

Neuropsychiatric symptoms (NPS) are core features of Alzheimer's disease and related dementias. On one hand, behavioral symptoms in patients with mild cognitive impairment (MCI) can indicate an increased risk of progressing to dementia. On the other hand, mild behavioral impairment (MBI) in patients who usually have normal cognition indicates an increased risk of developing dementia. Whatever the cause, all dementias carry a high rate of NPI. These symptoms can be observed at any stage of the disease, may fluctuate over its course, are a leading cause of stress and overload for caregivers, and increase rates of hospitalization and early institutionalization for patients with dementia. The clinician should be able to promptly recognize NPI through the use of instruments capable of measuring their frequency and severity to support diagnosis, and to help monitor the treatment of behavioral symptoms. Background: Mild cognitive impairment (MCI) is a transitional state between normal aging and dementia. Identifying this condition would allow early interventions that may reduce the rate of progression to Alzheimer's disease (AD). We examined the efficacy of a six-month cognitive intervention program (CIP) in patients with MCI and to assess patients’ condition at one-year follow-up.Methods: Forty-six MCI participants assessed with neuropsychological, neurological, neuropsychiatry, and functional procedures were included in this study and followed up during a year. The sample was randomized into two subgroups: 24 participants (the “trained group”) underwent the CIP during six months while 22 (control group) received no treatment. Sixteen participants dropped out of the study. The intervention focused on teaching cognitive strategies, cognitive training, and use of external aids, in sessions of two hours, twice per week for six months. Cognitive and functional measures were used as primary outcome and all were followed up at one year.Results: The intervention effect (mean change from baseline) was significant (p < 0.05) on the Mini-Mental State Examination (1.74), the Clinical Dementia Rating Scale (0.14), the Boston Naming Test (2.92), block design (−13.66), matrix reasoning (−3.07), and semantic fluency (−3.071) tasks. Four patients (one trained and three controls) progressed to dementia after one year of follow-up.Conclusions: These results suggest that persons with MCI can improve their performance on cognitive and functional measures when provided with early cognitive training and it could persist in a long-term follow-up.

The symptomatic predementia phase of Alzheimer's disease (AD), known as mild cognitive impairment (MCI) is a clinical and neuropsychological condition which defines the transitional state between normal aging and dementia, and is used as a clinical description of people at risk of developing AD. A review of the diagnostic criteria of MCI due to Alzheimer's disease was recently published by the Alzheimer's Association and the National Institute on Aging of the U.S. in order to ensure early diagnosis of the disease, useful for both clinical practice and clinical trials. The objectives of this paper are to review and analyze the revised diagnostic criteria for MCI due to Alzheimer's disease recently proposed, to compare with criteria for MCI available and to establish current strengths and limitations of the new proposal in clinical practice. The new diagnostic criteria for MCI due to AD have a radical importance since they are potentially applicable in the clinical or research protocols and in all clinical settings where such markers are available. They provide a useful, consistent and valuable tool to homogenize the subgroup of patients with MCI who already has AD in a predementia phase with inexorable progression to dementia by AD over the years.

Background: The economic cost of dementia is high and can be predicted by cognitive and neuropsychiatric profiles. The differential costs of the various subtypes of dementia are unknown in Argentina, and this study therefore aimed to compare these costs.Methods: Patients with a diagnosis of dementia of Alzheimer-type (DAT), frontotemporal dementia (FTD) and vascular dementia (VaD), and their primary caregivers, were evaluated between 2002 and 2008.Results: 104 patients with dementia (DAT = 44, FTD = 34, VaD = 26) were screened and matched by age and educational level with 29 healthy subjects. Demographic variables showed no significant differences among dementia patients. The annual direct costs were US$4625 for DAT, US$4924 for FTD, and US$5112 for VaD (p > 0.05 between groups). In the post hoc analysis VaD showed higher hospitalization costs than DAT (p < 0.001). VaD exhibited lower medication costs than FTD (p < 0.001). DAT exhibited higher anti-dementia drug costs; FTD had higher psychotropic costs. In the multivariate analysis, depression, activities of daily living, and caregiver burden were correlated with direct costs (r2 = 0.76).Conclusions: The different dementia types have different costs. Overall, costs increased with the presence of behavioral symptoms, depression and functional impairment of activities of daily living.

Background: Being a caregiver of a patient with Alzheimer’s disease is associated with impaired health status and declines in health-related quality of life (HRQoL). This paper evaluates the reliability and validity of the Argentinean version of the Medical Outcomes Study Short-Form Health Survey (SF-36) among caregivers of patients with Alzheimer’s disease. Methods: Forty-eight caregivers of Alzheimer’s disease patients completed the SF-36, the Zarit Burden Interview (ZBI) and the Neuropsychiatric Inventory (NPI). Patients were evaluated for dementia severity using the Clinical Dementia Rating (CDR) and for cognitive status using the Mini Mental State Examination (MMSE). Results: The SF-36 scales demonstrated adequate-to-strong internal consistency (Cronbach’s alpha range: 0.72 to 0.92). Correlations between the SF-36 scales and the ZBI were moderate to strong (range: –0.19 to –0.79, all p < 0.01 expect for physical function). Significant correlations between the SF-36 scales and the CDR, MMSE and NPI were lower (range: –0.30 to –0.40, p < 0.001) and strongest in mental health-related scales of the SF-36. The SF-36 demonstrated good factorial validity. Conclusions: The Argentinean translation of the SF-36 is reliable and valid for use to measure the HRQoL of caregivers of patients with Alzheimer’s disease.

SUMMARY Objective To examine behavioral, cognitive and functional factors associated with psychosocial burden in caregivers of geriatric patients. Methods Primary caregivers assessed were included if the geriatric patient cared for had a cognitive impairment or dementia (degenerative, vascular or mixed) (Group 1) or depression and cerebrovascular disease (CVD) (Group 2). Caregivers completed the Zarit questionnaire, the Neuropsychiatric Inventory (NPI) and Instrumental Activities of Daily Living (IADL). Patients were evaluated for dementia severity using the Clinical Dementia Rating (CDR), Mini Mental State Examination (MMSE) and Beck Depression Inventory (BDI). Structural equation modelling (SEM) was used to assess measurement models and the factors associated with burden. Results Two hundred and fifty-eight caregiver‐patient pairs were included. The best model fit was obtained with a model with two constructs: function-cognition (CDR, MMSE, and IADL) and behavior (neuropsychiatric symptoms from the NPI). In Group 1, both function (B ¼0.32. T ¼2.79) and behavior (B ¼0.72, T ¼7.84) were significantly correlated with caregiver burden, although the strength of association was more than two times higher for behavior. In Group 2, behavior was related to caregiver burden (B ¼0.68, T ¼6) but not function-cognition (B ¼0.16, T ¼1.36). Conclusion These findings suggest that behavioral symptoms are an important factor associated with caregiver burden in patients with cognitive impairment, dementia, or depression, while functional and cognitive factors seem to also have an influence in patients with cognitive impairment. The mild cognitive impairment (MCI), has emerged as an identifiable condition and in many cases is an intermediate state preceding diagnosable Alzheimer disease (AD) characterized by acquired cognitive deficits, without significant decline in functional activities of daily living. The aim of this study was to determine both the presence and type of neuropsychiatric manifestations in MCI patients and to compare them with both those suffering from mild AD and normal controls.86 subjects were assessed, 27 were classified as having MCI, 39 as having presumable mild AD, and 20 normal controls matched by age and education. The Neuropsychiatric inventory (NPI-Q) was used to assess the neuropsychiatric manifestations.The most common symptoms in the MCI group were irritability (55%), dysphoria (44%), apathy (37%), and anxiety (37%). Statistically significant differences were observed between the MCI and control groups regarding the above mentioned symptoms (p0.05). However, the differences between the MCI and mild AD groups were not found to be statistically significant.MCI is associated with a high rate of neuropsychiatric symptoms (irritability, depression, anxiety and apathy). These symptoms have serious adverse consequences and should be considered in diagnosis criteria.

Mild cognitive impairment (MCI) refers to persons who are slightly cognitively impaired for age but do not meet the criteria for dementia. MCI has been related to a pre-dementia stage of Alzheimer's disease (AD). However, other possible diagnoses such as cerebro-vascular disease, frontotemporal dementia or normal aging have been considered. Diagnosis, etiology and conversion to dementia are a source of ambiguity in MCI. The aim was to evaluate the opinion of experts on dementia and of general practitioners concerning MCI. A total of 24 experts from Argentina and Brazil (16 neurologists and 8 psychiatrists) and 30 general practitioners agreed to reply to a questionnaire on MCI (adapted from Dubois inventory, 2003). Of these, 92% of experts considered MCI as an ambiguous entity, not necessarily as a "pre-dementia" stage; 63% confirmed a tendency to worsen over the time and 83% of experts decided to initiate treatment using cholinesterase inhibitors, memantine and vitamin E. The opinion on MCI was that a priori it is not only an Alzheimer disease pre-dementia stage, but most of them consider the treatment against AD. MCI is a heterogeneous entity that should be classified as an open category and making it necessary to standardize definitions and design diagnosis guides to better understand Alzheimer disease pre-dementia stage.

Depression is named as one of the most prevalent Mental Health problem, affecting almost 10 % of the population. According to Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) different subtypes are included. Dysthymic Disorder is characterized by a depressed mood for most of the day, for more days than not, as indicated either by subjective account or observation by others, for at least 2 years. The aim of this study was to investigate the neuropsychological profile of dysthymic disorder. Ninety three patients fulfilling criteria for dysthymia were examined on a variety of cognitive tasks, neurological exam and a semi-structured psychiatric interview. Forty one non-depressed individuals served as controls. Significant differences between groups were found in digit span; immediate and delayed recall of a story; and semantic and phonologic fluency. Logic memory was worse than serial learning task, and has a correlation with attention deficit and depressive features.This results indicate that the neuropsychological performance of patients with dysthymic disorder exhibit impairments in attention and memory. Attentional deficits appear to be the key of cognitive deficits in dysthymia.
