Abstract

Introduction and Objectives. Urothelial carcinomas can arise throughout the entire urinary tract, the urinary bladder being the predominant point of origin. Synchronous multifocal non-muscle invasive urothelial carcinomas of the bladder (UCB) are not a rare finding. Although rare, the incidence of concomitant bladder and upper urothelial tract tumors (UTUC) is 8-17% and mostly found in recurrences after initial treatment. The aim of this article is to share our individual experience regarding the case management of a patient with synchronous bilateral UTUC and UCB, at presentation, for a better understanding and optimization of future patient care. Materials and Methods. A late November 2022 evening, Mr. V.J.M, an 81-year-old male patient, is brought into our emergency room at the request of his general practitioner for suspicion of acute ischemic stroke. A kidney and bladder ultrasound (US) scan was performed, finding bilateral hydronephrosis and the presence of a tumor mass measuring almost 3 centimeters on the right lateral wall of the bladder. A non-contrast CT scan confirmed the bilateral hydronephrosis as well as the presence of an endo-ureteral tissue density mass at L4 level on the right side and another an endo-ureteral tissue density mass in regard to the iliac vessels on the left side, highly in favor of urothelial lesions Results. An urgent urine drainage was granted, the patient undergoing surgery for a bilateral retrograde stent insertion. A second intervention consisting in a transurethral resection of the bladder and bilateral rigid and flexible diagnostic ureteroscopy (URS) was performed. The histopathological examination reported a non-muscle invasive low-grade papillary urothelial carcinoma (NMIBC) for all the lesions of the upper tractus as well as the bladder lesion. The patient then underwent surgery allowing the laser vaporization of all urothelial masses and bilateral double J stents were left in place as to ensure good kidney drainage. Conclusions. Although the incidence of concomitant bladder and upper urothelial tract tumors is very rare, it is something that we may stumble upon in everyday clinical practice. The management of multifocal and synchronous locations is difficult, requiring a multidisciplinary approach with a meticulous preoperative and postoperative planning as to offer the most adapted treatment plan, where minimal invasive treatment might be offered in highly selected cases as this one. If radical invasive treatment cannot be proposed, a more rigorous follow-up may be useful as not to compromise the oncological results. [ABSTRACT FROM AUTHOR]

Abstract

The validation of the correct functioning of star trackers is a crucial step for the success of any space mission. In recent years, there has been an increasing interest for the development of miniaturized, light-weighted test systems to be installed directly on the star tracker, opening the way to the test of completely assembled, on-board attitude sensors in pre-launch environment. Here we present the main technical characteristics of a new prototype, the MINISTAR, recently developed by a consortium of Italian enterprises and the Applied Physics Institute of the National Research Council. Besides its main technical features, here we present the results of a set of tests for its performance evaluation, with particular reference to its radiometric and geometric calibration in the laboratory. The MINISTAR can generate synthetic images of dynamic star fields for the simultaneous test of multiple star trackers (up to three head devices). The generation of dynamic star fields in a realistic scenario also includes large objects, such as the Sun or the Moon, and disturbances (e.g. cosmic rays and stray light effects). Thanks to its reduced dimensions and weight, the MINISTAR is suitable for the installation on the star tracker's baffle. Special attention has also been paid to the use of materials and technologies that could be compliant for vacuum operation in the future. Thanks to the design and construction of an interchangeable mechanical interface, the MINISTAR is compatible with the majority of star trackers available on the market.

Abstract

We analysed the first 84 coronavirus disease (COVID-19) patients hospitalised in an infectious and tropical disease unit in Florence, Italy, over 30 days after the start of the COVID-19 outbreak in Italy. A 12% reduction in the rate of intensive care unit transfer was observed after the implementation of intensity care measures in the regular ward such as increasing the nurse/patient ratio, presence of critical care physicians and using high flow nasal cannulae oxygenation.

Abstract

Objective The objective of this study was to report the methodology and 1-year experience of a regional service model of teleconsultation for planning and treatment of complex thoracoabdominal aortic disease (TAAD). Methods Complex TAADs without a feasible conventional surgical repair were prospectively evaluated by vascular surgeons of the same public health service (National Health System) located in a huge area of 22,994 km 2 with 3.7 million inhabitants and 11 tertiary hospitals. Surgeons evaluated computed tomography scans and clinical details that were placed on a web platform (Google Drive; Google, Mountain View, Calif) and shared by all surgeons. Patients gave informed consent for the teleconsultation. The surgeon who submits a case discusses in detail his or her case and proposes a possible therapeutic strategy. The other surgeons suggest other solutions and options in terms of grafts, techniques, or access to be used. Computed tomography angiography, angiography, and clinical outcomes of cases are then presented at the following telemeetings, and a final agreement of the operative strategy is evaluated. Teleconsultation is performed using a web conference service (WebConference.com; Avaya Inc, Basking Ridge, NJ) every month. An inter-rater agreement statistic was calculated, and the κ value was interpreted according to Altman's criteria for computed tomography angiography measurements. Results The rate of participation was constant (mean number of surgeons, 11; range, 9-15). Twenty-four complex TAAD cases were discussed for planning and operation during the study period. The interobserver reliability recorded was moderate (κ = 0.41-0.60) to good (κ = 0.61-0.80) for measurements of proximal and distal sealing and very good (κ = 0.81-1) for detection of any target vessel angulation >60 degrees, significant calcification (circumferential), and thrombus presence (>50%). The concordance for planning and therapeutic strategy among all participants was complete in 16 cases. In one case, the consultation was decisive for creating an innovative therapeutic strategy; in the remaining seven cases, the strategy proposed by the patient's surgeon was changed completely after the discussion. Technical success was the same (100%) if concordance in planning was present initially or not. Overall 6-month mortality was 4%, 0% for those patients with initial concordance in planning vs 12% for those without initial concordance ( P = .33). Surgery was always performed in a tertiary hospital by local surgeons, and in two cases (8%) external surgeons joined the local surgical team. Conclusions Such a regional service of teleconsultation may be of value in standardizing the treatment and derived costs of complex TAADs in a huge region under the same health provider. The shared decision-making strategy may be of medical-legal value as well.

Abstract

Abstract The half-linear difference equations with the deviating argument , are considered. We study the role of the deviating argument , especially as regards the growth of the nonoscillatory solutions and the oscillation. Moreover, the problem of the existence of the intermediate solutions is completely resolved for the classical half-linear equation ( = 1). Some analogies or discrepancies on the growth of the nonoscillatory solutions for the delayed and advanced equations are presented; and the coexistence with different types of nonoscillatory solutions is studied.
