Abstract

It is still debated whether prophylactic doses of low-molecular- weight heparin (LMWH) are always effective in preventing Venous Thromboembolism (VTE) and mortality in COVID-19. Furthermore, there is paucity of data for those patients not requiring ventilation. We explored mortality and the safety/efficacy profile of LMWH in a cohort of Italian patients with COVID-19 who did not undergo ventilation. From the initial cohort of 422 patients, 264 were enrolled. Most (n = 156, 87.7%) received standard LMWH prophylaxis during hospitalization, with no significant difference between medical wards and Intensive Care Unit (ICU). Major or not major but clinically relevant hemorrhages were recorded in 13 (4.9%) patients: twelve in those taking prophylactic LMWH and one in a patient taking oral anticoagulants (p: n.s.). Thirty-nine patients (14.8%) with median age 75 years. were transfused. Hemoglobin (Hb) at admission was significantly lower in transfused patients and Hb at admission inversely correlated with the number of red blood cells units transfused (p < 0.001). In-hospital mortality occurred in 76 (28.8%) patients, 46 (24.3%) of whom admitted to medical wards. Furthermore, Hb levels at admittance were significantly lower in fatalities (g/dl 12.3; IQR 2.4 vs. 13.3; IQR 2.8; Mann–Whitney U-test; p = 0.001). After the exclusion of patients treated by LMWH intermediate or therapeutic doses (n = 32), the logistic regression showed that prophylaxis significantly and independently reduced mortality (OR 0.31, 95% CI 0.13–0.85). Present data show that COVID-19 patients who do not require ventilation benefit from prophylactic doses of LMWH.

Abstract

D-dimer assay is used to stratify patients with unprovoked venous thromboembolism (VTE) for the risk of recurrence. However, this approach was never evaluated since direct oral anticoagulants are available. With this multicenter, prospective cohort study, we aimed to assess the value of an algorithm incorporating serial D-dimer testing and administration of reduced-dose apixaban (2.5 mg twice daily) only to patients with a positive test. A total of 732 outpatients aged 18 to 74 years, anticoagulated for ≥12 months after a first unprovoked VTE, were included. Patients underwent D-dimer testing with commercial assays and preestablished cutoffs. If the baseline D-dimer during anticoagulation was negative, anticoagulation was stopped and testing repeated after 15, 30, and 60 days. Patients with serially negative results (286 [39.1%]) were left without anticoagulation. At the first positive result, the remaining 446 patients (60.9%) were given apixaban for 18 months. All patients underwent follow-up planned for 18 months. The study was interrupted after a planned interim analysis for the high rate of primary outcomes (7.3%; 95% confidence interval [CI], 4.5-11.2), including symptomatic proximal deep vein thrombosis (DVT) or pulmonary embolism (PE) recurrence, death for VTE, and major bleeding occurring in patients off anticoagulation vs that in those receiving apixaban (1.1%; 95% CI, 0.4-2.6; adjusted hazard ratio [HR], 8.2; 95% CI, 3.2-25.3). In conclusion, in patients anticoagulated for ≥1 year after a first unprovoked VTE, the decision to further extend anticoagulation should not be based on D-dimer testing. The results confirmed the high efficacy and safety of reduced-dose apixaban against recurrences. This trial was registered at www.clinicaltrials.gov as #NCT03678506.

Abstract

Background. The International Standardization Organization operates the world's most widely recognized quality management system standard, the ISO 9001:2015. In the healthcare sector, the adoption of this standard within an organization helps to improve the overall performance and provides a foundation for development and continuous progress. Our study aims to describe the implementation process of a quality management system according to the ISO 9001:2015 standards in an Angiology Unit of an Italian University hospital. Methods. The project was structured in 5 operational phases, which were carried out during a time frame of 14 months (March 2018-May 2019) and entailed several improvement actions associated with quality and safety outputs such as clinical management, clinical practice, safety, and patient-centeredness. Results. Implementation of the quality management system led to the improvement of many aspects of the processes performed in the Angiology Unit, both in the outpatient and day hospital setting. Overall, the project positively impacted on systems for patient safety, particularly in communication and data transmission, and clinical leadership. Conclusions. The implementation of the ISO 9001 certification is a process that apparently may seem expensive in terms of resources used, commitment, work, comparison, but it leads to substantial and always progressive improvements in the offer of Services to the user, safety both for the users and for the healthcare personnel involved, in addition to the care processes that translate into significant benefits in terms of quality of care for patients, as well as management savings for the organization. Hemoglobin (Hb) at admission was significantly lower in transfused patients and Hb at admission inversely correlated with the number of red blood cells units transfused (p 0.001). In-hospital mortality occurred in 76 (28.8%) patients, 46 (24.3%) of whom admitted to medical wards. Furthermore, Hb levels at admittance were significantly lower in fatalities (g/dl 12.3; IQR 2.4 vs. 13.3; IQR 2.8; Mann-Whitney U-test; p = 0.001). After the exclusion of patients treated by LMWH intermediate or therapeutic doses (n = 32), the logistic regression showed that prophylaxis significantly and independently reduced mortality (OR 0.31, 95% CI 0.13-0.85). Present data show that COVID-19 patients who do not require ventilation benefit from prophylactic doses of LMWH.

Abstract

Leguminosae is a family with high value of use for food, medicine, forage, ornamental and restoration ecology purposes. One obstacle for the use and management of many legume species is the presence of seeds with physical dormancy. Here, we evaluated the presence of physical dormancy in nine native species of Argentina and identified possible methods for breaking dormancy. Caesalpinia gilliesii, Geoffroea decorticans, and Prosopis alpataco have seeds with no physical dormancy, whereas Crotalaria incana, C. pumila, C. stipularia, Desmanthus virgatus, Galactia texana, and Senna aphylla have seeds with physical dormancy. The most effective methods for breaking physical dormancy were mechanical and wet heat (100°C) scarification for Crotalaria spp.; mechanical, acid (20 and 30 min) and wet heat scarification (80 and 100°C) for D. virgatus; mechanical scarification for G. texana and mechanical and all acid scarification treatments for S. aphylla. These results contribute to the knowledge of the germination biology for these species, and are of particular interest for their propagation in glasshouse and for restoration and conservation programs. Leguminosae es una familia con un alto valor de uso como alimento, forraje, medicina y para fines ornamentales y de restauración ecológica. Uno de los principales problemas para el uso y manejo de muchas de estas especies es la presencia de semillas con dormición física. En el presente trabajo se evaluó la presencia de dormición física en nueve especies nativas de Argentina y los posibles métodos que permitan la salida de este estado. Las semillas de Caesalpinia gilliesii, Geoffroea decorticans y Prosopis alpataco presentaron semillas sin dormición, mientras que las semillas de Crotalaria incana, C. pumila, C. stipularia, Desmanthus virgatus, Galactia texana y Senna aphylla presentaron dormición física. Los métodos más eficientes para romper la dormición física de las semillas de las especies del género Crotalaria fueron la escarificación mecánica y el calor húmedo (100°C); la escarificación mecánica, la química (ácido sulfúrico: 20 y 30 minutos) y la escarificación con calor húmedo (80 y 100°C) para D. virgatus; la escarificación mecánica para G. texana; y la escarificación mecánica y la química (ácido sulfúrico: 10, 20 y 30 minutos) para S. aphylla. Estos resultados contribuyen al conocimiento de la biología de las semillas de estas especies y a su utilización en programas de propagación de plantas en invernaderos, restauración ecológica y conservación. Abstract

International audience; The major element composition of the insoluble, windborne long-range dust archived in the European Project for Ice Coring in Antarctica Dome C ice core has been determined by Particle Induced X-ray Emission analyses. The geochemistry of dust from the last glacial maximum (LGM) and from the Holocene is discussed in terms of past environmental changes, throughout the last climatic cycle. Antarctic dust from glacial and interglacial climate clearly reveals different geochemical compositions. The weathered crustal-like signature of LGM dust is characterized by a low compositional variability, suggesting a dominant source under the glacial regime. The close correspondence between the major element composition of Antarctic glacial dust and the composition of southern South American sediments supports the hypothesis of a dominant role of this area as major dust supplier during cold conditions. Conversely, the major element composition of Holocene dust displays high variability and high Al content on average. This implies that an additional source could also play some role. Comparison with size-selected sediments suggests that a contribution from Australia is likely during warm times, when a reduced glacial erosion decreases the primary dust production and a more intense hydrological cycle and larger vegetation cover inactivates dust mobility in a large part of southern South America, weakening its contribution as a massive dust supplier to Antarctica.

Abstract

Il presente progetto si proponeva come obiettivi principali la definizione di linee guida per l’analisi sperimentale dei metalli definiti nella Direttiva 107/2004 (recepita con D.Lgs 152/2007) ‐ arsenico, cadmio, nichel ‐ integrati dal piombo (normato dal DM 60/2002) e la costituzione di un database informativo/composizionale del PM10 in Emilia Romagna con riferimento ai metalli sopra citati e alla composizione ionica di una stazione costiera (Rimini). Le attività intraprese hanno consentito di mettere a punto la metodica di campionamento ed analisi dei metalli in accordo alle specifiche dettate dalla normativa e di acquisire una serie storica della durata di un intero anno (1/4/2008 – 31/3/2009) relativa alle zone rappresentative delle tre aree meteoclimatiche caratteristiche della regione: costa (Rimini), centro (Ferrara), ovest (Parma), oltre all’area metropolitana di Bologna. All’interno di ciascuna zona è stata identificata una postazione di misura del tipo “fondo urbano” ad eccezione di Ferrara, dove, per differenziare, è stata scelta una stazione da traffico. A Rimini è stata effettuata anche la misura di un ampio spettro di specie ioniche in un periodo invernale e in uno primaverile. Gli esiti delle prove di confronto effettuate sulle tre tecniche analitiche ICP‐MS, XRF e PIXE hanno indicato che la tecnica più sensibile risulta essere, come atteso, l’ICP‐MS (che è anche la tecnica indicata nella metodica prevista dalla normativa) che ha consentito di rilevare agevolmente le basse concentrazioni trovate in regione. Anche l’XRF, nella particolare configurazione commerciale utilizzata dai laboratori che hanno collaborato a questa ricerca, ha dato valori di Pb e Ni molto ben correlati alla tecnica di riferimento. La PIXE presenta invece limiti di rilevabilità che sono alti rispetto alle concentrazioni medie rilevate in regione. Dall’analisi dei dati è emerso che in tutte le quattro zone tutti i metalli hanno fatto registrare medie annuali inferiori alla Soglia di Valutazione Inferiore (SVI) prevista dalla normativa, sia per le stazioni di fondo che per la stazione da traffico. Pertanto si registra una situazione media regionale in cui le misure continuative non sono strettamente necessarie, ma è sufficiente l’utilizzo di tecniche di modellizzazione o di stima obiettiva.
