1. Effects of PM2.5 Exposure on the ACE/ACE2 Pathway: Possible Implication in COVID-19 Pandemic

Abstract

Particulate matter (PM) is a harmful component of urban air pollution and PM2.5, in particular, can settle in the deep airways. The RAS system plays a crucial role in the pathogenesis of pollution-induced inflammatory diseases: the ACE/AngII/AT1 axis activates a pro-inflammatory pathway counteracted by the ACE2/Ang(1-7)/MAS axis, which in turn triggers an anti-inflammatory and protective pathway. However, ACE2 acts also as a receptor through which SARS-CoV-2 penetrates host cells to replicate. COX-2, HO-1, and iNOS are other crucial proteins involved in ultrafine particles (UFP)-induced inflammation and oxidative stress, but closely related to the course of the COVID-19 disease. BALB/c male mice were subjected to PM2.5 sub-acute exposure to study its effects on ACE2 and ACE, COX-2, HO-1 and iNOS proteins levels, in the main organs concerned with the pathogenesis of COVID-19. The results obtained show that sub-acute exposure to PM2.5 induces organ-specific modifications which might predispose to greater susceptibility to severe symptomatology in the case of SARS-CoV-2 infection. The novelty of this work consists in using a molecular study, carried out in the lung but also in the main organs involved in the disease, to analyze the close relationship between exposure to pollution and the pathogenesis of COVID-19.

2. Health Status and Nutritional Habits in Maldives Pediatric Population: A Cross-Sectional Study

Abstract

Chronic noncommunicable diseases (NCDs) have become the major cause of morbidity and mortality in the Maldives, triggered by the nutrition transition to a "Western diet" that dramatically increases the prevalence of excess weight and hypertension. Our study aimed to evaluate dietary habits, blood pressure (BP) and body mass index in Maghoodoo Public School's students. A sample of 145 students (72 males and 73 females, age 9.37 +/- 2.97 years) was enrolled. Factors causing excess weight were investigated through descriptive statistics. The relationship between blood pressure percentiles and possible influencing factors was investigated by a linear regression model.. Excess weight was present in 15.07% and 12.5% females and males, respectively. 15.18% of the subjects had elevated BP, with a significant difference according to gender detected only in the PAS z-score. Eating habits were investigated through a parent-filled questionnaire; 70.15% of the students consumed less than two portions of fruit per day, with a significant difference between gender (84.06% and 55.38% for boys and girls, respectively, p < 0.0001) and 71.64% ate less than two servings of vegetables per day. An alarming finding emerged for sweet snacks (30.6% of the students consumed 2-3 servings per day) and sugary drinks (2-3 servings per day for 32.84% of students) consumption. Our findings suggest that excess weight and hypertension in this population could be due to energy-rich, packaged-foods consumption. A nutrition education approach might thus help to reduce cardiovascular risk.

Abstract

Age-related injuries are often connected to alterations in redox homeostasis. The imbalance between free radical oxygen species and endogenous antioxidants defenses could be associated with a growing risk of transient ischemic attack and stroke. In this context, a daily supply of dietary antioxidants could counteract oxidative stress occurring during ischemia/reperfusion injury (I/R), preventing brain damage. Here we investigated the potential antioxidant properties of coffee-derived circulating metabolites and a coffee pulp phytoextract, testing their efficacy as ROS scavengers in an in vitro model of ischemia. Indeed, the coffee fruit is an important source of phenolic compounds, such as chlorogenic acids, present both in the brewed seed and in the discarded pulp. Therefore, rat brain endothelial cells, subjected to oxygen and glucose deprivation (OGD) and recovery (ogR) to mimic reperfusion, were pretreated or not with coffee by-products. The results indicate that, under OGD/ogR, the ROS accumulation was reduced by coffee by-product. Additionally, the coffee extract activated the Nrf2 antioxidant pathway via Erk and Akt kinases phosphorylation, as shown by increased Nrf2 and HO-1 protein levels. The data indicate that the daily intake of coffee by-products as a dietary food supplement represents a potential nutritional strategy to counteract aging.

Although toxicity at cellular level has been investigated, we are still far from using MSNPs in the clinic, because the mechanisms involved in the cellular responses activated by MSNPs have not yet been elucidated.Materials and methods: This study used an in vitro multiparametric approach to clarify relationships among size, dose, and time of exposure of MSNPs (0.05–1 mg/mL dose range), and cellular responses by analyzing the morphology, viability, and functionality of human vascular endothelial cells and neurons.Results: The results showed that 24 hours of exposure of endothelial cells to 250 nm MSNPs exerted higher toxicity in terms of mitochondrial activity and membrane integrity than 30 nm MSN at the same dose. 7. The impact of a nutritional intervention program on eating behaviors in Italian athletes

Abstract

A balanced diet is a fundamental component of athletes’ health, training, and performance. The majority of athletes choose adequate quantities of macronutrients but, at the same time, do not respect World Health Organization dietary guidelines, eating a lot of discretionary food and not drinking enough water. Athletes need more nutritional education to improve the quality of their food choice. By modifying their eating habits, they could also enhance their performance. Our study aimed to evaluate the impact of nutritional intervention on eating habits in a group of Northern Italian athletes. A sample of 87 athletes (41 males and 46 females) aged 16.5 ± 2.9 was enrolled. We organized meetings and detected eating habits (before and after the meetings) using a food frequencies questionnaire. We found that nutritional intervention positively affected participants consumption of vegetables (p < 0.05), nuts (p < 0.001), legumes (p < 0.001), and fish (p < 0.05). Other aspects of the athletes’ eating habits were not significantly improved. Some gender differences were found; males increased their consumption of vegetables (p < 0.05) and nuts (p < 0.001), while females increased their intake of legumes (p < 0.001). Our finding suggested that nutritional intervention could promote healthy eating habits among athletes. If sports nutrition experts, coaches, personal trainers, sports medicine experts, and athletes cooperated, they could guarantee athletes’ health status.

8. Inner speech recognition through electroencephalographic signals

Abstract

This work focuses on inner speech recognition starting from electroencephalographic (EEG) signals. Inner speech recognition is defined as the internalised process in which the person thinks in pure meanings, generally associated with an auditory imagery of own inner “voice”. The decoding of the EEG into text should be understood as the classification of a limited number of words (commands) or the presence of phonemes (units of sound that make up words). Speech-related brain computer interfaces provide effective vocal communication strategies for controlling devices through speech commands interpreted from brain signals, improving the quality of life of people who have lost the capability to speak, by restoring communication with their environment. Two public inner speech datasets are analysed. Using this data, some classification models are studied and implemented starting from basic methods such as Support Vector Machines, to ensemble methods such as the eXtreme Gradient Boosting classifier up to the use of neural networks such as Long Short Term Memory (LSTM) and Bidirectional Long Short Term Memory (BiLSTM). With the LSTM and BiLSTM models, generally not used in the literature of inner speech recognition, results in line with or superior to those present in the state-of-the-art are obtained.

In this study, we focused on biocompatibility issues, evaluating production of nitric oxide by cultured human umbilical vein endothelial cells and macrophages, used as models of cells which would be exposed to nanoparticles after systemic administration. Methods: The nanoparticles tested were liposomes and solid lipid nanoparticles carrying phosphatidic acid or cardiolipin, and PEGylated poly(alkyl cyanoacrylate) nanoparticles (PEG-PACA). We measured nitric oxide production using the Griess method as well as phosphorylation of endothelial nitric oxide synthase and intracellular free calcium, which are biochemically related to nitric oxide production. MTT viability tests and caspase-3 detection were also undertaken. Results: Exposure to liposomes did not affect the viability of endothelial cells at any concentration tested. Increased production of nitric oxide was detected only with liposomes carrying phosphatidic acid or cardiolipin at the highest concentration (120 µg/mL), together with increased synthase phosphorylation and intracellular calcium levels. Macrophages exposed to liposomes showed a slightly dose-dependent decrease in viability, with no increase in production of nitric oxide. Exposure to solid lipid nanoparticles carrying phosphatidic acid decreased viability in both cell lines, starting at the lowest dose (10 µg/mL), with increased production of nitric oxide detected only at the highest dose (1500 µg/mL). Exposure to PEG-PACA affected cell viability and production of nitric oxide in both cell lines, but only at the highest concentration (640 µg/mL). Conclusion: Liposomal and PEG-PACA nanoparticles have a limited effect on vascular homeostasis and inflammatory response, rendering them potentially suitable for treatment of Alzheimer’s disease. Moreover, they highlight the importance of testing such nanoparticles for production of nitric oxide in vitro in order to identify a therapeutic dose range suitable for use in vivo. Abstract

The contributing role of environmental factors to the development of neurodegenerative diseases has become increasingly evident. Here, we report that exposure of C6 glioma cells to diesel exhaust particles (DEPs), a major constituent of urban air pollution, causes intracellular reactive oxygen species (ROS) production. In this scenario, we suggest employing the possible protective role that coffee phenolic metabolites may have. Coffee is a commonly consumed hot beverage and a major contributor to the dietary intake of (poly) phenols. Taking into account physiological concentrations, we analysed the effects of two different coffee phenolic metabolites mixes consisting of compounds derived from bacterial metabolization reactions or phase II conjugations, as well as caffeic acid. The results showed that these mixes were able to counteract DEP-induced oxidative stress. The cellular components mediating the downregulation of ROS included extracellular signal-regulated kinase 1/2 (ERK1/2), nuclear factor erythroid 2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), and uncoupling protein 2 (UCP2). Contrary to coffee phenolic metabolites, the treatment with N-acetylcysteine (NAC), a known antioxidant, was found to be ineffective in preventing the DEP exposure oxidant effect. These results revealed that coffee phenolic metabolites could be promising candidates to protect against some adverse health effects of daily exposure to air pollution.

13. An Interdisciplinary Oral Health Program for Children in Kindergartens of Northern Italy

Abstract

Background: The aim of the study was to investigate if a parents’ training program on nutritional and oral health behaviours held by health professionals can influence children habits in a sample of kindergartens in Northern Italy. Methods: The study designed was a longitudinal study. The study population were children aged 6–36 months attending four kindergartens. Parents were invited to participate filling out a self-administered questionnaire, and after having returned the informed consent form they were asked to participate to a training meeting. Parents had to fill out a questionnaire at baseline and after 3 months from the training meeting. The questionnaire included information on socio-demographics about parents, oral hygiene habits of parents and child, and eating habits of child. Wald test was used to analyse data collected. Results: After the training program, almost all children were able to use a toothbrush suitable for kids (from 91% at the baseline to 99% after the 3-months). The analysis shows that the given training significantly increased the number of children who use toothpaste from 86% at baseline (95%CI: 85%-88%) to 96% (95%CI: 94%-98%). The use of fluoride toothpaste significantly increased after the training intervention as the baseline measure of 59% proportion increased to 80%. The intervention study showed a positive impact on the number of pupils who wash their tooth more than twice per week and on the timing of oral hygiene as both night and morning time proportion increased after the 3-month. Regarding the feeding habits the given consultation resulted in a statistically significant increase regarding the importance of morning snack from 94% at baseline (95%CI: 92%-96%) to 97% after 3-month (95%CI: 94%-99%). Another promising effect of the education is the decrease from 47% at baseline (95%CI: 46%-49%) to 42% after the study (95%CI:41%-43%) of the bad habit of having a snack after dinner. Abstract

The strong spread of COVID-19 and the significant number of deaths associated with it could be related to improper lifestyles, which lead to a low-grade inflammation (LGI) that not only increases the risk of chronic diseases, but also the risk of facing complications relating to infections and a greater susceptibility to infections themselves. Recently, scientific research has widely demonstrated that the microbiota plays a fundamental role in modulating metabolic responses in the immune system. There is, in fact, a two-way interaction between lifestyle, infection, and immunity. The immune response is compromised if nutrition is unbalanced or insufficient, because diet affects the intestinal flora predisposing people to infections and, at the same time, the nutritional state can be aggravated by the immune response itself to the infection. We evaluate the link between balanced diet, the efficiency of the immune system, and microbiota with the aim of providing some practical advice for individuals, with special attention to the elderly. A correct lifestyle that follows the Mediterranean model, which is especially rich in plant-based foods along with the use of extra-virgin olive oil, are the basis of preventing LGI and other chronic pathologies, directly influencing the intestinal microbiota and consequently the immune response.

15. Eating Out of Home and Risk for Obesity: An Overview

Abstract

Despite Mediterranean dietary pattern has become a healthy eating standard, even Mediterranean countries such as Italy, show a progressive disaffection from this diet in parallel to the increasing trend in prevalence of body overweight and obesity and correlate pathologies. The causes that led to the explosion of the body overweight and obesity phenomenon and the differences in the prevalence of this condition among various social groups, are cultural and economic as well as biological and epidemiological. In developed countries out-of-home eating, defined as the consumption of foods and beverages out of home, has received increasing attention among cultural and behavioral factors linked to obesity. Indeed, with the rapid development of economy, diet habits have undergone great changes. An increasingly number of people, especially in urban settings, is eating out of home not only for working people during their lunch break, but also at dinnertime. Studies showed that a very high dietary energy content of both full service and fast-food restaurant meals is a widespread phenomenon and that there is a positive relationship between the consumption of food away from home and weight gain. This has garnered considerable attention from public health experts in formulating national policies and programs designed to curb the growing trends in obesity. USA, Canada and Australia, proposed calorie-labeling policy that required chain restaurants to post the calorie content of items on menus. In Europe, there is not such a policy yet. Unfortunately, many studies show that menu restaurant calories and nutritionallabeling mandatory policies did not have achieved the set goals in guiding consumers throughout restaurant menu to choose healthier dishes in terms of less calories and nutrients composition. Probably, educational and information campaigns are factors to put beside calorie and nutrient labeling.

16. The feasibility of a diet which enhances inositol availability

Abstract

Summary. Background: Supplementation of myo-inositol has proved effective in different pathological conditions associated with insulin-resistance, including polycystic ovary syndrome, diabetes, gestational diabetes,and metabolic syndrome. At the same time, dietary habits in developed countries tend to reduce inositol intake,due to reduced consumption of whole grain foods, legumes, and nuts, rich in phytic acid, the main source of inositol. Aim: The review aims at providing a collection of foods with high phytic acid content to be used for diets that can make available as much inositol as is obtained from nutritional supplements commonly present in the market. Methods: An extensive review concerning the phytic acid content of foods was obtained from literature; furthermore, we designed an exemplifying phytic acid rich diet in by means of a specific nutritional software. Results: Foods with high phytic acid content are: cereals (0.04-3.3% on the dry matter), legumes (0.2-2.4%), oil seeds (0.4-5.7%), and nuts (0.2-9.4%). A moderate amount of phytic acid has been found in root vegetables, tubers and fruits, while low levels are found in the leafy green vegetables. Using these data, we developed an example of weekly diet which provides a mean phytic acid content of 5 g/1660Kcal. Conclusions: This study shows that it is possible to increase phytic acid intake, and consequently inositol availability, by means of an appropriate diet as a complementary treatment to dietary supplements. In people who regularly consume fruits and vegetables, the gut microbiota efficiently degrades phytic acid to myo-inositol phosphate products, therefore this diet could be proposed to patients with increased inositol needs, such as those suffering from polycystic ovary syndrome and in insulin resistance.

Abstract

The attention on neurodegenerative diseases is growing due to the increase of the life expectancy and the consequent increase of average age. A contributory cause of neurodegeneration is the chronical exposure to ambient air pollution that can affect the SNC inducing neuroinflammation and oxidative stress. In this scenario, nutrition, through the assumption of essential micronutrients, might modify the oxidative impacts of air pollution on health. Recently, it has been shown that the use of phenolic acids such as caffeic, chlorogenic and ferulic, can modulate oxidative stress. Once introduced into the body through the diet, these substances undergo a process of metabolization by the microbiota and liver leading to different metabolites generation. To investigate the possible antioxidant action of these molecules, C6 glioma cells were pretreated with coffee metabolites and subsequently treated with Diesel Exaust Particles (DEP). ROS levels and cell viability were assessed. Preliminary results showed that cell vitality reduction and significant ROS level increase obtained after DEP treatment, are decreased after coffee metabolites administration, bringing back vitality and ROS levels comparable to those of the controls. Therefore, this data suggest an antioxidant effect that might counteract the oxidative stress induced by DEP. The results obtained may potentially be applicable in the future for the production of functionalized foods containing coffee metabolites, in order to mitigate the harmful effects of atmospheric pollution

Abstract

Background: Continuous subcutaneous insulin infusion (CSII) is increasing worldwide, mostly because of improved technology. The aim of this study was to evaluate the current status of CSII in Italy. Materials and Methods: Physicians from 272 diabetes centers received a questionnaire investigating clinical features, pump technology, and management of patients on CSII. Results: Two hundred seventeen centers (79.8%) joined the study and, by the end of April 2013, gave information about 10,152 patients treated with CSII: 98.2% with type 1 diabetes mellitus, 81.4% adults, 57% female, and 61% with a conventional pump versus 39% with a sensor-augmented pump. CSII advanced functions were used by 68% of patients, and glucose sensors were used 12 days per month on average. Fifty-eight percent of diabetes centers had more than 20 patients on CSII, but there were differences among centers and among regions. The main indication for CSII was poor glucose control. Dropout was mainly due to pump wearability or nonoptimal glycemic control. Twenty-four hour assistance was guaranteed in 81% of centers. A full diabetes team (physician+ nurse + dietician + psychologist) was available in 23% of adult-care diabetes centers and in 53%of pediatric diabetes units. Conclusions: CSII keeps increasing in Italy. More work is needed to ensure uniform treatment strategies throughout the country and to improve pump use.

Abstract

The global prevalence of childhood obesity is expected to reach 60 million by 2020. Childhood obesity is now an accepted as a public health problem because of the serious impact on children's health and health care costs. It is shown that obese children are more likely to stay obese into adulthood and to develop cardiovascular diseases at a younger age. In addition to complications related to physical state, obese children can experiment psychosocial consequences such as discrimination, low self-esteem and loneliness. Given the difficulties involved in weight loss and the costly treatment of obesity, initiating obesity prevention at an early age has reached general consensus. Excess weight is influenced by complex interactions among genetic, biological, and environmental factors. Although genetic and biological factors are indeed important, they cannot fully explain this current global trends, given that factors are also underlying causes of such phenomenon. Therefore, inadequate food intake (quality and quantity of food) and sedentary (low o none physical activity) should be the primary focus to work to prevent or mitigate childhood obesity. Given the significance of this early on an individual’s life course, schools are a primary setting for health promotion as they represent a micro-environments in which children spend a substantial part of their time and have a key role in influencing their daily choices. For this purpose, important targets are increaseing nutrition knowledge, improveing dietary habits and physical education. Moreover, schools can provide an ideal setting for nutrition interventions as they serve as a focal point to engage families, educators, administrators and community members to implement effective and sustainable aid for children and adolescents. Abstract

The aim of this study was to investigate the relationship between endothelin-1, nitric oxide, insulin resistance, and blood pressure in young subjects with a high prevalence of excess weight and/or elevated blood pressure. In a cohort of 238 children (mean age = 11.1 years), height, weight, waist circumference, and blood pressure were assessed. Body mass index, waist-to-height ratio, and blood pressure percentiles were calculated, and the children were classified as having excess weight and elevated blood pressure according to the International Obesity Task Force and the US blood pressure nomograms specific for gender, age and height, respectively. Endothelin-1 and nitric oxide production were assessed, and the homeostatic model assessment index was calculated. Forty-three percent of children were male, 71% had excess weight, and 37% had systolic and/or diastolic values above the ninetieth percentile. Plasma endothelin-1 and nitric oxide production were independently correlated (p < 0.05). In multivariate analyses, the HOMA index was associated with systolic and diastolic blood pressure (p = 0.01), and nitric oxide was independently related to diastolic blood pressure (p = 0.04), even after adjustment for measures of body composition. By using the waist-to-height ratio instead of BMI in the statistical model, the association between the homeostatic model assessment index and blood pressure was attenuated, while the results remained similar for nitric oxide. No correlation was found between endothelin-1 and blood pressure. In our study population, the correlation between nitric oxide and blood pressure and the lack of a relationship between endothelin-1 and blood pressure could be explained by an increase in the vasodilator effect of local and systemic nitric oxide, which counteracts the possible hypertensive effect of endothelin-1.

Abstract

The attention on neurodegenerative diseases is growing due to the increase of the life expectancy and the consequent increase of average age. A contributory cause of neurodegeneration is the chronical exposure to ambient air pollution that can affect the SNC inducing neuroinflammation and oxidative stress. In this scenario, nutrition, through the assumption of essential micronutrients, might modify the oxidative impacts of air pollution on health. Recently, it has been shown that the use of phenolic acids such as caffeic, chlorogenic and ferulic, can modulate oxidative stress. Once introduced into the body through the diet, these substances undergo a process of metabolization by the microbiota and liver leading to different metabolites generation. To investigate the possible antioxidant action of these molecules, C6 glioma cells were pretreated with coffee metabolites and subsequently treated with Diesel Exaust Particles (DEP). ROS levels and cell viability were assessed. Preliminary results showed that cell vitality reduction and significant ROS level increase obtained after DEP treatment, are decreased after coffee metabolites administration, bringing back vitality and ROS levels comparable to those of the controls. Therefore, this data suggest an antioxidant effect that might counteract the oxidative stress induced by DEP. The results obtained may potentially be applicable in the future for the production of functionalized foods containing coffee metabolites, in order to mitigate the harmful effects of atmospheric pollution

Abstract

Over the past 60 years there has been a dramatic increase in the prevalence of overweight in children and adolescents, ranging from 4% in 1975 to 18% in 2016. Recent estimates indicate that overweight or obese children and adolescents are more than 340 million. Obesity is often associated with hypertension, which is an important cardiovascular risk factor. Recent studies show that the presence of hypertension is a frequent finding in the pediatric age. Hypertensive children easily become hypertensive adults. This phenomenon contributes to increasing cardiovascular risk in adulthood. Primary hypertension is a growing problem especially in children and adolescents of western countries, largely because of its association with the ongoing obesity epidemic. Recently, it has been hypothesized that a dietary link between obesity and elevated blood pressure (BP) values could be simple carbohydrate consumption, particularly fructose, both in adults and in children. Excessive intake of fructose leads to increased serum uric acid (SUA) and high SUA values are independently associated with the presence of hypertension and weaken the efficacy of lifestyle modifications in children. The present review intends to provide an update of existing data regarding the relationship between BP, simple carbohydrates (particularly fructose), and uric acid in pediatric age. In addition, we analyze the national policies that have been carried out over the last few years, in order to identify the best practices to limit the socio-economic impact of the effects of excessive sugar consumption in children

Abstract

Continuous subcutaneous insulin infusion (CSII) is increasingly being used worldwide, mostly thanks to technical improvements. This study examined the current status of CSII in Italy. Physicians in charge of 272 diabetes centers caring for patients using CSII were sent a questionnaire investigating clinical features, pump technology and management of these patients; a large proportion (217 centers, 79.8%) joined the study. By end-April 2013, data had been collected on 10152 patients treated with CSII; 98.2% had type 1 diabetes, 82.4% were adults, 57% female. Only just over half the centers (59%) managed more than 20 CSII patients each. The distribution of patients varied widely both among and within different regions. The main indication for CSII was the de- sire to improve glycemic control. Dropouts (8.65%) were mainly due to difficulties with pump wearability or non-optimal glycemic control. Among CSII patients 61% used a traditional pump, 39% a sensor augmented pump. Only 68% used the CSII advanced functions and glucose sensors were used twelve days per month on average. Round-the-clock assistance was guaranteed in 81% of centers; a full diabetes team followed patients in only 40% of adult-care centers and 50% of pediatric units. CSII is increasingly used in Italy, by adults and pediatric patients. However, further work is needed to unify treatment strategies throughout the country and to encourage optimal pump use and applications.

Abstract

Background: Continuous subcutaneous insulin infusion (CSII) is increasing worldwide, mostly because of improved technology. The aim of this study was to evaluate the current status of CSII in Italy. Materials and Methods: Physicians from 272 diabetes centers received a questionnaire investigating clinical features, pump technology, and management of patients on CSII. Results: Two hundred seventeen centers (79.8%) joined the study and, by the end of April 2013, gave information about 10,152 patients treated with CSII: 98.2% with type 1 diabetes mellitus, 81.4% adults, 57% female, and 61% with a conventional pump versus 39% with a sensor-augmented pump. CSII advanced functions were used by 68% of patients, and glucose sensors were used 12 days per month on average. Fifty-eight percent of diabetes centers had more than 20 patients on CSII, but there were differences among centers and among regions. The main indication for CSII was poor glucose control. Dropout was mainly due to pump wearability or nonoptimal glycemic control. Twenty-four hour assistance was guaranteed in 81% of centers. A full diabetes team (physician+ nurse + dietician + psychologist) was available in 23% of adult-care diabetes centers and in 53%of pediatric diabetes units. Conclusions: CSII keeps increasing in Italy. More work is needed to ensure uniform treatment strategies throughout the country and to improve pump use.

Abstract

Continuous subcutaneous insulin infusion (CSII) is increasingly being used worldwide, mostly thanks to technical improvements. This study examined the current status of CSII in Italy. Physicians in charge of 272 diabetes centers caring for patients using CSII were sent a questionnaire investigating clinical features, pump technology and management of these patients; a large proportion (217 centers, 79.8%) joined the study. By end-April 2013, data had been collected on 10152 patients treated with CSII; 98.2% had type 1 diabetes, 82.4% were adults, 57% female. Only just over half the centers (59%) managed more than 20 CSII patients each. The distribution of patients varied widely both among and within different regions. The main indication for CSII was the de- sire to improve glycemic control. Dropouts (8.65%) were mainly due to difficulties with pump wearability or non-optimal glycemic control. Among CSII patients 61% used a traditional pump, 39% a sensor augmented pump. Only 68% used the CSII advanced functions and glucose sensors were used twelve days per month on average. Round-the-clock assistance was guaranteed in 81% of centers; a full diabetes team followed patients in only 40% of adult-care centers and 50% of pediatric units. CSII is increasingly used in Italy, by adults and pediatric patients. However, further work is needed to unify treatment strategies throughout the country and to encourage optimal pump use and applications.

Abstract

Purpose: Mesoporous silica nanoparticles (MSNPs) are excellent candidates for biomedical applications and drug delivery to different human body areas, the brain included. Although toxicity at cellular level has been investigated, we are still far from using MSNPs in the clinic, because the mechanisms involved in the cellular responses activated by MSNPs have not yet been elucidated. Materials and methods: This study used an in vitro multiparametric approach to clarify relationships among size, dose, and time of exposure of MSNPs (0.05–1 mg/mL dose range), and cellular responses by analyzing the morphology, viability, and functionality of human vascular endothelial cells and neurons. Results: The results showed that 24 hours of exposure of endothelial cells to 250 nm MSNPs exerted higher toxicity in terms of mitochondrial activity and membrane integrity than 30 nm MSN at the same dose. This was due to induced cell autophagy (in particular mitophagy), probably consequent to MSNP cellular uptake (>20%). Interestingly, after 24 hours of treatment with 30 nm MSNPs, very low MSNP uptake (<1%) and an increase in nitric oxide production (30%, P<0.01) were measured. This suggests that MSNPs were able to affect endothelial functionality from outside the cells. These differences could be attributed to the different protein-corona composition of the MSNPs used, as suggested by sodium dodecyl sulfate polyacrylamide-gel electrophoresis analysis of the plasma proteins covering the MSNP surface. Moreover, doses of MSNPs up to 0.25 mg/mL perturbed network activity by increasing excitability, as detected by multielectrode-array technology, without affecting neuronal cell viability. Conclusion: These results suggest that MSNPs may be low-risk if prepared with a diameter <30 nm and if they reach human tissues at doses <0.25 mg/mL. Abstract

Surface functionalization with antitransferrin receptor (TfR) mAbs has been suggested as the strategy to enhance the transfer of nanoparticles (NPs) across the blood-brain barrier (BBB) and to carry nonpermeant drugs from the blood into the brain. However, the efficiency of BBB crossing is currently too poor to be used in vivo. In the present investigation, we compared 6 different murine mAbs specific for different epitopes of the human TfR to identify the best performing one for the functionalization of NPs. For this purpose, we compared the ability of mAbs to cross an in vitro BBB model made of human brain capillary endothelial cells (hCMEC/D3). Liposomes functionalized with the best performing mAb (MYBE/4C1) were uptaken, crossed the BBB in vitro, and facilitated the BBB in vitro passage of doxorubicin, an anticancer drug, 3.9 folds more than liposomes functionalized with a nonspecific IgG, as assessed by confocal microscopy, radiochemical techniques, and fluorescence, and did not modify the cell monolayer structural or functional properties. These results show that MYBE/4C1 antihuman TfR mAb is a powerful resource for the enhancement of BBB crossing of NPs and is therefore potentially useful in the treatment of neurologic diseases and disorders including brain carcinomas.

Abstract

Pancratium maritimum L. is an Amaryllidaceous species whose presence is severely endangered in its original range, the sandy coasts of the Mediterranean sea. A molecular analysis has been performed to evaluate the genetic distance among populations coming from different locations, in order to define the best repopulating strategy. The plant genome, analysed by AFLP markers, was found to be extremely homogeneous and conserved, evoking vegetative or autogamous reproductive habits. Seeds from two different locations showed a good germination capability in greenhouse tests, indicating the potential presence of an efficient sexual reproduction. The combination of molecular data and germination tests would support the hypothesis of an autogamous reproduction for this species. © Springer 2005.

Abstract

In the search of new drug delivery carriers for the brain, self-assembled nanoparticles (NP) were prepared from poly(N,N-dimethylacrylamide)-block-polystyrene polymer. NP displayed biocompatibility on cultured endothelial cells, macrophages and differentiated SH-SY5Y neuronal-like cells. The surface-functionalization of NP with a modified fragment of human Apolipoprotein E (mApoE) enhanced the uptake of NP by cultured human brain capillary endothelial cells, as assessed by confocal microscopy, and their permeability through a Transwell Blood Brain Barrier model made with the same cells, as assessed by fluorescence. Finally, mApoE-NP embedding doxorubicin displayed an enhanced release of drug at low pH, suggesting the potential use of these NP for the treatment of brain tumors. In the search of novel drug carriers for brain drug delivery, poly(N,N-dimethylacrylamide)-b-polystyrene nanoparticles (NP) have been functionalized with a modified fragment of Apolipoprotein E (mApoE). These NP show to be uptaken and to overcome a Blood-Brain Barrier (BBB) in vitro cellular model. The cytotoxicity of NP is also investigated on different cell lines, showing high biocompatibility.

38. Pin1 affects Tau phosphorylation in response to Abeta oligomers

Abstract

We show that in hippocampal cultured neurons, dephosphorylation of peptidyl-prolyl cis-trans isomerase Pin1 on Ser16 is occurring during the early stages of exposure to Abeta (1-42) oligomers. This occurrence, resulting in Pin1 activation, is paralleled by Tau(Thr231) dephosphorylation, probably due to Pin1-mediated Tau isomerisation. Indeed, in the presence of the specific Pin1 inhibitor juglone, Abeta-induced Tau(Thr231)dephosphorylation is prevented. The involvement of protein phosphatase 2A (PP2A) in dephosphorylation of isomerised Tau is shown by the co-treatment of neurons with Abeta (1-42) and okadaic acid, a PP2A inhibitor, leading to Tau(Thr231) hyperphosphorylation. We also report the modulation, via Pin1, of Ser199, Ser396, Ser400 and Ser404 phosphorylation state in response to Abeta treatment. Taken together, these data suggest for the first time that an early Pin1 response might be transiently evoked by Abeta 1-42 oligomers, preventing Tau hyperphosphorylation. This evidence highlights the role of Pin1 as Tau phosphorylation modulator during Alzheimer onset.

Abstract

Amyloid-beta (Abeta), a cytotoxic fragment of Amyloid Precursor Protein (APP), has been implicated in the etiopathogenesis of Alzheimer's disease (AD). Since several neurotrophins signalling pathways may be activated in response to toxic insults, we investigated whether a similar response is triggered also by Abeta. After Abeta (25-35) peptide administration to cultured rat hippocampal neurons, the nerve growth factor (NGF) and its receptor (TrkA) mRNA expression is up-regulated. Moreover, we observe an increased cellular TrkA expression (4.5 fold) and NGF release in the culture medium (5-fold). Concomitantly, TrkA, Akt and glycogen synthase kinase 3beta (Gsk3beta) phosphorylation significantly increase. Interestingly, when cells were treated with Abeta (25-35) in the presence of blocking antibody against NGF, only a partial TrkA activation (2-fold) was observed. These results have been confirmed by using pathophysiological Abeta (1-42) oligomers. Abstract

The influence of membrane lipid environment on the activity of GPI-anchored enzymes was investigated with human placental alkaline phosphatase reconstituted by a detergent-dialysis technique in liposomes composed of palmitoyloleoylphosphatidylcholine, alone or in mixture with lipids enriched along with the protein within lipid rafts: cholesterol, sphingomyelin, and GM1 ganglioside. The highest Vmax was recorded for a phosphatidylcholine/ 10% GM1 mixture (143 ± 5 nmol of substrate hydrolyzed per minute per microgram of protein), while the lowest for a phosphatidylcholine/30% cholesterol mixture and for raft-mimicking 1:1:1 phosphatidylcholine/ sphingolipid/cholesterol liposomes (M:M:M) (57 ± 3 and 52 ± 3, respectively). No significant differences in Km were detected. The protein segregation, assessed using the chemical cross-linker bis(sulfosuccinimidyl)suberate, increased with the protein:lipid ratio, within the 1:1200-1:4800 protein:lipid molar ratio range, but did not affect enzyme activity. The activity decreased when the order of the lipid bilayers was increased, higher for those containing cholesterol, as judged by steady-state fluorescence polarization of 1,6-diphenyl-1,3,5-hexatriene. Finally, the GPI-enzyme activity was affected by membrane curvature. This result was suggested by a strong inverse correlation (Pearson's correlation coefficient = 0.91; p < 0.0001) between activity and liposome diameter, measured by laser light scattering and ranging between 59 ± 6 nm for a phosphatidylcholine/ 10% GM1 mixture (displaying the highest activity) and 188 ± 25 nm for a phosphatidylcholine/30% cholesterol mixture and 185 ± 23 nm for raft-mimicking liposomes (displaying the lowest activities). The activity-membrane curvature relationship was further confirmed by comparing the activity of proteoliposomes having different sizes but identical lipid compositions. 42. Beta-amyloid (25-35) enhances lipid metabolism and protein ubiquitination in cultured neurons

Abstract

We investigated the effect of beta-amyloid (A beta) (25-35), a cytotoxic fragment of A beta pepticle, on lipid metabolism and protein ubiquitination in cultured rat hippocampal neurons. After treatment with A beta under conditions leading to apoptotis, as assessed by caspase activity assay, the total cell mass of lipids changed following a biphasic behavior, with an increase that reached a maximum after 16 hr of treatment, followed by a decrease. The increase at 16 hr was 15.3% in the case of phospholipids and 103.0% in the case of gangliosides and was due to enhanced biosynthesis as confirmed by increase of radioactivity incorporation (phospholipids +52.0%, gangliosides +193.1%) in cells fed with tritiated palmitic acid. No change with respect to cholesterol was observed. Strikingly, under these conditions, the ubiquitination state of cell proteins strongly increased. These effects were not observed with the (35-25) reverse sequence pepticle. Similarly to A beta, lactacystin treatment increased lipid synthesis and protein ubiquitination; only lactacystin, and not A beta, induced a strong decrease of proteasome chimotrypsin activity. These results suggest that A beta enhances protein ubiquitination, without inhibiting proteasomal activity, and lipid synthesis. These results may shed new light on the mechanisms of A beta toxicity. Abstract

Caveolins are the main structural proteins of glycolipid/cholesterol-rich plasmalemmal invaginations, termed caveolae. In addition, caveolin-1 isoform takes part in membrane remodelling as it binds and transports newly synthesized cholesterol from endoplasmic reticulum to the plasma membrane. Caveolin-1 is expressed in many cell types, including hippocampal neurons, where an abundant SNAP25-caveolin-1 complex is detected after induction of persistent synaptic potentiation. To ascertain whether caveolin-1 influences neuronal voltage-gated Ca2+ channel basal activity, we stably expressed caveolin-1 into transfected neuroblastoma x glioma NG108-15 hybrid cells [cav1(+) clone] that lack endogenous caveolins but express N-type Ca2+ channels upon cAMP-induced neuronal differentiation. Whole-cell patch-clamp recordings of cav1(+) cells demonstrated that N-type current density was reduced in size by approximately 70% without any significant change in the time course of activation and inactivation and voltage dependence. Moreover, the cav1(+) clone exhibited a significantly increased proportion of membrane cholesterol compared to wild-type NG108-15 cells. To gain insight into the mechanism underlying caveolin-1 lowering of N-current density, and more precisely to test whether this was indirectly caused by caveolin-1-induced enhancement of membrane cholesterol, we compared single N-type channel activities in cav1(+) clone and wild-type NG108-15 cells enriched with cholesterol after exposure to a methyl-beta-cyclodextrin-cholesterol complex. A lower Ca2+ channel activity was recorded from cell-attached patches of both cell types, thus supporting the view that the increased proportion of membrane cholesterol is ultimately responsible for the effect. This is due to a reduction in the probability of channel opening caused by a significant decrease of channel mean open time and by an increase of the frequency of null sweeps.

Abstract

Ethanol is a psychotropic drug, whose acute or chronic abuse may lead to physical dependency, cognitive dysfunctions, and brain damages. Many of the ethanol effects arise as a result of the alteration of membrane functions, which follows changes either in physicochemical properties or in the lipid composition or both of neural cells. In fact, in vitro studies have shown that ethanol affects membrane fluidity either directly, through incorporation in the bilayer, or indirectly, inducing changes in the overall unsaturated:saturated fatty acid ratio of membrane lipids. Indeed, ethanol induces more considerable changes of neural cell lipid composition; hippocampus, which plays a critical role in learning and memory, is especially sensitive. In fact, chronic exposure to ethanol in vivo and in vitro affects phospholipid and ganglioside pattern and, noteworthy, induces the formation of a unique phospholipid, phosphatidylethanol. Considering the role of phospholipids and gangliosides in neuronal development and in various signal transduction pathways, these changes may have significant implication in cognitive dysfunctions observed. Studies in vitro indicate that alcohol enhances ganglioside biosynthesis by increasing the sphingosine recycling, an hypothetic mechanism of cell defence during alcoholic stress. Chronic ethanol consumption also stimulates changes in lipid organization associated with lipid peroxidation, a critical event in many neurological diseases and disorders, in particular the fetal alcohol syndrome, caused by ethanol exposure during prenatal and developmental period. Abstract

Continuous subcutaneous insulin infusion (CSII) is increasingly being used worldwide, mostly thanks to technical improvements. This study examined the current status of CSII in Italy. Physicians in charge of 272 diabetes centers caring for patients using CSII were sent a questionnaire investigating clinical features, pump technology and management of these patients; a large proportion (217 centers, 79.8%) joined the study. By end-April 2013, data had been collected on 10152 patients treated with CSII; 98.2% had type 1 diabetes, 82.4% were adults, 57% female. Only just over half the centers (59%) managed more than 20 CSII patients each. The distribution of patients varied widely both among and within different regions. The main indication for CSII was the de- sire to improve glycemic control. Dropouts (8.65%) were mainly due to difficulties with pump wearability or non-optimal glycemic control. Among CSII patients 61% used a traditional pump, 39% a sensor augmented pump. Only 68% used the CSII advanced functions and glucose sensors were used twelve days per month on average. Round-the-clock assistance was guaranteed in 81% of centers; a full diabetes team followed patients in only 40% of adult-care centers and 50% of pediatric units. CSII is increasingly used in Italy, by adults and pediatric patients. However, further work is needed to unify treatment strategies throughout the country and to encourage optimal pump use and applications.

Abstract

Media conditioned with viable rat and calf spleen lymphocytes were seen to be able to inhibit 3H-thymidine uptake by rat thymocytes and the mitogenic response of human lymphocytes to PHA but not bone marrow cells proliferation. The inhibitor appeared to have a molecular weight greater than 50,000 daltons, as determined by Amicon Diaflo ultrafiltration. Media conditioned with rat granulocytes had a similar tissue specific but species non-specific inhibitory action. Also human leukemic cells appeared able to produce in vitro an inhibitor of cell proliferation when a certain degree of differentiation was present.
